Role of neoadjuvant chemotherapy in advanced carcinoma of the hypopharynx and larynx.

Background:To assess the response rate and impact of neoadjuvant chemotherapy (NACT) in advanced carcinoma of the hypopharynx and larynx.Materials and Methods:This is a retrospective case series of 80 patients with locally advanced laryngopharynx carcinoma who received NACT from April 2010 to October 2011 at our tertiary care center. The patients received NACT either for achieving resectability or for organ preservation.Results:Majority of the patients (60%) had T4 a disease. Grade 3 and 4 neutropenia was seen in 18%, febrile neutropenia in 4%, mucositis in 4%, diarrhea in 5%, and vomiting in 3% patients. Resectability could be achieved in 34%, and larynx was preserved in 51% patients at a mean follow-up of 13 months.Conclusions:NACT was safe with acceptable toxicity. Majority of the patients who achieved resectability had oropharyngeal involvement. NACT followed by concurrent chemoradiotherapy could provide a high rate of organ preservation.

Docetaxel and cisplatin combination chemotherapy in advanced carcinoma of the urothelium: A multicenter phase II study of the Hellenic Cooperative Oncology Group

Both docetaxel and cisplatin have moderate activity in patients with advanced urothelial cancer. We performed a multicenter phase II study in order to assess the efficacy and toxicity of the combination of these two agents in patients with advanced carcinoma of the urothelium. Sixty-six patients not amenable to curative surgery or irradiation were enrolled onto this cooperative group study and treated on an outpatient basis with docetaxel 75 mg/m2 followed by cisplatin 75 mg/m2, both administered intravenously. Granulocyte-colony stimulating factor was administered subcutaneously at a dose of 5 micrograms/kg daily from day 5 until resolution of neutropenia. The chemotherapy was administered every three weeks for a maximum of six courses in patients without evidence of progressive disease. Thirty-four of sixty-six patients (52%, 95% confidence interval 40%-64%) demonstrated objective responses, with eight achieving clinical complete responses and twenty-six partial responses. A multivariate logistic regression analysis indicated that the patients most likely to respond were those without lung metastasis and without weight loss before treatment. The median duration of response was 6.1 months and the median times to progression and survival for all patients were 5 and 8 months, respectively. Absence of anemia, of liver metastases and of weight loss correlated with longer survival. Grade > or = 3 toxicities included granulocytopenia in 33% of patients, anemia in 14%, diarrhea in 13% and emesis in 7% of patients. The combination of docetaxel and cisplatin appeared relatively well tolerated and moderately active in patients with advanced urothelial cancer. The patients most likely to benefit were those without weight loss and without lung or liver metastases.

DETRIMENTAL EFFECT OF NEOADJUVANT CHEMOTHERAPY IN PATIENTS WITH STAGE-IIIB CARCINOMA OF THE CERVIX - RESULTS OF A RANDOMIZED TRIAL

With the objective of testing the value of neoadjuvant chemotherapy in patients with stage III B carcinoma of the cervix, we began in 1984 a randomized, phase III trial comparing BOMP chemotherapy followed by pelvic radiotherapy versus pelvic radiotherapy alone. Patients were stratified by age, extension of parametrial involvement, and lymphangiographic findings. Despite a higher complete response rate, the overall 5-year survival was significantly inferior in the combined therapy group (39% vs 23%, P=0.02). Toxicity was more pronounced in the chemoradiation arm and 4 patients developed fatal pulmonary complications. Patterns of failure were similar in both groups. The use of primary chemotherapy in advanced carcinoma of the cervix was detrimental to the patients and its use outside a protocol setting is discouraged.

Further experience with radiation therapy and concomitant intravenous chemotherapy in advanced carcinoma of the lower female genital tract

Sixty-seven patients with advanced carcinoma of the lower female genital tract (cervix, vagina, and vulva) were treated with radiation and concomitant intravenous cisplatin and/or 5-fluorouracil. Fifty-seven patients (85%) responded completely clinically. Thirty-five (61%) complete responders recurred with a median time to recurrence of 6 months. Twenty-six of the thirtyfive patients who recurred had some component of local failure. The 22 complete responders who have not recurred have been followed a median of 13 months. Acute toxicity was minimal, with only 6 patients requiring interruption of therapy. Nine (13%) patients developed severe late complications and eight required surgery. The actuarial 5-year survival is 22%. This treatment regimen is disappointing in terms of both survival and local control.

Results of chemotherapy in advanced carcinoma of the fallopian tube

Forty-six patients with measurable disease received chemotherapy for advanced primary or recurrent carcinoma of the fallopian tube. The response rate was 9% with single-agent therapy, 29% with multiagent therapy without cisplatin, and 81% with cisplatin-containing combination therapy. Survival was significantly improved in Stage III/IV with the addition of cisplatin-based combination therapy.

The Value of Chemotherapy in Advanced Carcinoma of Head and Neck

From our study we like to draw the following conclusions.1. No significant difference could be detected between group A and group B of our study. In other words, combined chemotherapy does not lead to significantly better results than chemotherapy alone.2. 73% of all the patients with absolutely inoperable head and neck tumors had partial or complete tumor remission; for patients which tolerated 3 cycles of the chemotherapy the precentage of responders climbed up to even 80%.3. Tumor remission was always combined with a remarkable relief of the subjektive symptoms such as pain or dysphagia. This is particularly the case in the non-radiated patients of group B.4. The average survival time of 11 months may indicate that many patients of this study may finally not live longer, but they live definitely better.5. The therapy success in many of these hopeless cases encourages us to try a combination of our chemotherapy scheme with surgical procedures also in early stage of certain carcinoma to improve their chances of being cured from the tumor.

Phase I–II study of concurrent irradiation and multi-drug chemotherapy in advanced carcinoma of the breast: A pilot study by the Southeastern Cancer Study Group

A pilot study was begun by SECSG in October, 1976 for the treatment of patients with advanced inoperable carcinoma of the breast (stage T3–T4, N2–N3) without distant metastasis. Two courses of chemotherapy (5-FU, cytoxan, Adriamycin) are given on days 1 and 28, and after two weeks rest the radiation therapy begins. During the radiation therapy the Adriamycin is discontinued and the 5-FU and cytoxan are given at 60% of the initial dose. Two weeks after the completion of the radiotherapy, the three drugs are reinstated at the initial dose. Cycles are repeated every 28 days for a total of eight after radiotherapy (total one year therapy). The total maximum dose of Adriamycin allowable is 450 mm/m 2. Radiation therapy in doses of 6500 rad TD to the primary tumor, 6000 rad to the palpable lymph nodes and 4500 rad to electively treated lymph node bearing areas is administered using 4 MeV photons and for the internal mammary nodes 13 MeV electrons. The daily tumor dose is 180 rad, five weekly fractions. A total of 14 patients were entered to the study, five with a diagnosis of inflammatory carcinoma of the breast, five patients with advanced inoperable ductal carcinoma and four with isolated recurrences in the chest wall following a modified radical mastectomy. The primary tumor showed partial regression (50% to 75%) in 65% of the patients following the two courses of chemotherapy. All but three of the patients had complete regression of their tumors three months following the completion of the radiotherapy. Of the ten patients treated for primary carcinoma of the breast, five are alive without evidence of tumor; only two patients have developed chest wall recurrence, one of them in the posterior chest wall, behind the radiation portals.
