A scoping review of patient safety checklists in dentistry.

Not much is known about safety checklists use in dentistry. We aim to examine, assess, and provide a comprehensive understanding of the current knowledge concerning the use of checklists to improve patient safety in dentistry. We conducted a comprehensive literature search using Medline and Embase for studies that use or describe the development of dental patient safety checklists. All study designs were included for citations published through December 2023. Following abstract and full-text screening, 27 studies remained for data extraction and analysis. Oral surgery emerged as the main dental specialty where the utilization and evaluation of checklists were primarily concentrated (n = 15). The majority of the included studies described either checklist development, their implementation, or both (n = 18). Only 6 studies assessed the effectiveness of checklists in improving patient safety outcomes. Of the effectiveness studies, five found a decrease in incidents and dental adverse events, along with an improvement in patient care and safety, associated with the use of checklists. This scoping review highlights the limited current knowledge regarding checklist use and effectiveness in reducing adverse events across dental practices; it demonstrates the need on how to best guide the profession towards implementing this important evidence-based patient safety tool. Checklists are effective tools to improve patient safety and have become integral to medical practice. Dentistry can also benefit from these tools to improve patient safety.

Enhancing Musculoskeletal Injection Safety: Evaluating Checklists Generated by Artificial Intelligence and Revising the Preformed Checklist.

Background Musculoskeletal disorders are a significant global health issue, necessitating advanced management strategies such as intra-articular and extra-articular injections to alleviate pain, inflammation, and mobility challenges. As the adoption of these interventions by physicians grows, the importance of robust safety protocols becomes paramount. This study evaluates the effectiveness of conversational artificial intelligence (AI), particularly versions 3.5 and 4 of Chat Generative Pre-trained Transformer (ChatGPT), in creating patient safety checklists for managing musculoskeletal injections to enhance the preparation of safety documentation. Methodology A quantitative analysis was conducted to evaluate AI-generated safety checklists against a preformed checklist adapted from reputable medical sources. Adherence of the generated checklists to the preformed checklist was calculated and classified. The Wilcoxon signed-rank test was used to assess the performance differences between ChatGPT versions 3.5 and 4. Results ChatGPT-4 showed superior adherence to the preformed checklist compared to ChatGPT-3.5, with both versions classified as very good in safety protocol creation. Although no significant differences were present in the sign-in and sign-out parts of the checklists of both versions, ChatGPT-4 had significantly higher scores in the procedure planning part (p = 0.007), and its overall performance was also higher (p < 0.001). Subsequently, the preformed checklist was revised to incorporate new contributions from ChatGPT. Conclusions ChatGPT, especially version 4, proved effective in generating patient safety checklists for musculoskeletal injections, highlighting the potential of AI to streamline clinical practices. Further enhancements are necessary to fully meet the medical standards.

Integrating Patient Safety Discussions with First-Year Doctor of Pharmacy Students in a Skills Lab Course.

The patient safety problem has been well established for over 20 years in the United States (U.S.), and there is a recognized focus on ensuring that health professions' trainees receive explicit education in various patient safety principles and practices. While the literature provides examples of different approaches towards patient safety education for pharmacy students, there are few that focus on first-year pharmacy students. This educational observational study describes the implementation and evaluation of two 20 min patient safety learning activities integrated into a required pharmacy skills lab course. The first learning activity utilized a mock prescription and patient safety checklist that had students identify patient safety problems on the prescription, followed by a group discussion of implications for the patient. The second learning activity used images of common safety problems with a facilitated group discussion to have students identify systems-based solutions to those problems. Our study's findings revealed that students were able to identify basic patient safety problems and safety solutions, although some additional foundational information may be needed, particularly for students who may not have pharmacy work experience. Additional research is needed to continue building a literature base on patient safety education approaches, particularly for first-year pharmacy students.

The pre-anesthesia induction patient safety (PIPS) checklist: Testing dynamic versus static implementation strategies

BackgroundRetrospective studies and large-scale reviews suggest multiple properties of effective checklist implementation, such as active leadership, ongoing feedback, and local modification. We hypothesized that the use of these effective checklist implementation strategies would yield better results as compared to a more fixed implementation strategy of the Pre-anesthesia Induction Patient Safety (PIPS) checklist. MethodsWe conducted a prospective, pre-post, pragmatic cohort trial of a checklist implementation using a prospective difference-in-difference design observing simultaneous implementation of the same PIPS checklist in two community hospitals. At one hospital, we created a multidisciplinary committee of stakeholders to oversee implementation and provide active leadership, ongoing feedback, and local modification of the checklist (“Dynamic Implementation”). At the second hospital, a single clinical champion implemented the checklist with no local modification (“Static Implementation”). We examined checklist compliance and proximal outcomes three months before and nine months after implementation. We conducted surveys and interviews to examine attitudes and implementation barriers. ResultsWe observed 477 anesthesia inductions. The “Dynamic” cohort demonstrated significantly greater checklist compliance post-implementation compared to the “Static” cohort (B = 0.26, p < .001 for observable action and B = 0.24, p < .001 for verbal compliance). We were underpowered to detect changes in errors, or near-misses, but observers captured anecdotal checklist-related patient safety evidence, suggesting more research should be done in this area. There were no differences between cohorts on changes in team communication, or attitudes toward checklists. Participant interviews showed that two of the major criticisms of the PIPS checklist, ambiguity and redundancy, were more effectively resolved with the “Dynamic” implementation. ConclusionsDynamic implementation yielded significantly better compliance with the checklist than a more static approach. However, barriers such as time pressure and lack of team buy-in remain. Larger scale studies are needed to determine the full impact of the PIPS checklist on safety related outcomes such as near-misses.

The use of the World Health Organization Surgical Safety Checklist in operating theatres

Background: There is a global concern over intraoperative patient safety, as adverse events are on the rise. When the World Health Organization Surgical Safety Checklist (WHO SSC) is used correctly, it has the potential to prevent such events. Unfortunately, the intraoperative team in the designated hospital lacked the cooperation to successfully use the checklist.Aim: This study, therefore, aimed to explore and describe the factors that affect the use of the checklist in the operating theatres in a designated hospital.Methods: A qualitative research approach together with an implementation science strategy structured according to the Consolidated Framework for Implementation Research was used. Individual interviews with nine surgeons and focus group interviews with six operating theatre professional nurses provided sufficient data for inductive and deductive analysis.Results: A deeper understanding of the contextual and interventional factors that affect the use of the WHO SSC is provided by the findings. A high demand for surgery, the hierarchy in the surgical team, their uncertainty about hospital policies and reluctance to adjust to change contributed to the poor use of the checklist.Conclusion: A sustainable implementation process is crucial and should be embraced and promoted by the intraoperative team.Contribution: The article contributes a description of the factors that address the use of a checklist for intraoperative patient safety. It recommends that the factors that hinder the use of the checklist be timeously addressed.

Feasibility of implementing a surgical patient safety checklist: prospective cross-sectional evaluation

BackgroundThe World Health Organization’s Global Patient Safety Action Plan 2021–2030 call for attention to patient and family involvement to reduce preventable patient harm. Existing evidence indicates that patients’ involvement in their own safety has positive effects on reducing hospitalisation time and readmissions. One intervention reported in the literature is the use of checklists designed for patients’ completion. Studies on such checklists are small scale, but they are linked to reduction in length of hospital stay and readmissions. We have previously developed and validated a two-part surgical patient safety checklist (PASC). This study aims to investigate the feasibility of the PASC usage and implementation prior to its use in a large-scale clinical trial.MethodsThis is a prospective cross-sectional feasibility study, set up as part of the design of a larger stepped-wedge cluster randomised controlled trial (SW-CRCT). Descriptive statistics were used to investigate patient demographics, reasons for not completing the PASC and percentage of PASC item usage. Qualitative patient interviews were used to identify barriers and drivers for implementation. Interview was analysed through content analysis.ResultsOut of 428 recruited patients, 50.2% (215/428) used both parts of PASC. A total of 24.1% (103/428) of the patients did not use it at all due to surgical or COVID-19-related cancellations. A total of 19.9% (85/428) did not consent to participate, 5.1% (22/428) lost the checklist and 0.7% (3/428) of the patients died during the study. A total of 86.5% (186/215) patients used ≥ 80% of the checklist items. Barriers and drivers for PASC implementation were grouped into the following categories: Time frame for completing the checklist, patient safety checklist design, impetus to communicate with healthcare professionals and support throughout the surgical pathway.ConclusionsElective surgical patients were willing and able to use PASC. The study further revealed a set of barriers and drivers to the implementation. A large-scale definitive clinical-implementation hybrid trial is being launched to ascertain the clinical effectiveness and scalability of PASC in improving surgical patient safety.Trial registrationClinicaltrials.gov: NCT03105713. Placenta Accreta Spectrum Part I: anesthesia considerations based on an extended review of the literature.

"Placenta Accreta Spectrum" (PAS) describes abnormal placental adherence to the uterine wall without spontaneous separation at delivery. Though relatively rare, PAS presents a particular challenge to anesthesiologists, asit is associated with massive peripartum hemorrhage and high maternal morbidity and mortality. Standardized evidence-based PAS management strategies are currently evolving and emphasize: "PAS centers of excellence", multidisciplinary teams, novel diagnostics/pharmaceuticals (especially regarding hemostasis, hemostatic agents, point-of-care diagnostics), and novel operative/interventional approaches (expectant management, balloon occlusion, embolization). Though available data are heterogeneous, these developments affect anesthetic management and must be considered in planed anesthetic approaches. This two-part review provides a critical overview of the current evidence and offers structured evidence-based recommendations to help anesthesiologists improve outcomes for women with PAS. This first partdiscusses PAS management in centers of excellence, multidisciplinary care team, anesthetic approach and monitoring, surgical approaches, patient safety checklists, temperature management, interventional radiology, postoperative care and pain therapy. The diagnosis and treatment of hemostatic disturbances and preoperative prepartum anemia, blood loss, transfusion management and postpartum venous thromboembolism will be addressed in the second part of this series.

Development and validation of patients\u2019 surgical safety checklist

BackgroundPoor uptake and understanding of critical perioperative information represent a major safety risk for surgical patients. Implementing a patient-driven surgical safety checklist might enhance the way critical information is given and increase patient involvement in their own safety throughout the surgical pathway. The aim of this study was to develop and validate a Surgical Patient Safety Checklist (PASC) for use by surgical patients.MethodThis was a prospective study, involving patient representatives, multidisciplinary healthcare professionals and elective surgical patients to develop and validate PASC using consensus-building techniques in two Norwegian hospitals. A set of items intended for PASC were rated by patients and then submitted to Content Validation Index (CVI) analyses. Items of low CVI went through a Healthcare Failure Mode and Effect Analysis (HFMEA) Hazard Scoring process, as well as a consensus process before they were either kept or discarded. Reliability of patients’ PASC ratings was assessed using Intraclass Correlation Coefficient analysis. Lastly, the face validity of PASC was investigated through focus group interviews with postoperative patients.ResultsInitial development of PASC resulted in a checklist consisting of two parts, one before (32 items) and one after surgery (26 items). After achieving consensus on the PASC content, 215 surgical patients from six surgical wards rated the items for the CVI analysis on a 1-4 scale and mostly agreed on the content. Five items were removed from the checklist, and six items were redesigned to improve PASCs’ user-friendliness. The total Scale-level index/Average (S-CVI/Ave) before revision was 0.83 and 0.86 for pre- and post-operative PASC items, respectively. Following revision, these increased to 0.86 and 0.93, respectively. The PASC items reliability score was 0.97 (95% confidence interval 0.96 to 0.98). The qualitative assessment identified that patients who used PASC felt more in control of their situation; this was achieved when PASC was given to them at what they felt was the right time and healthcare professionals took part in its usage.ConclusionMultidisciplinary perioperative care staff and surgical patients agreed upon PASC content, the checklist ratings were reliable, and qualitative assessment suggested good face validity. PASC appears to be a usable and valid checklist for elective surgical patients across specialties.

Effect of implementing an anesthesia nurse checklist in a safety and teamwork climate: quasi-experimental study.

To evaluate the effect of implementing a Patient safety checklist: nursing in anesthetic procedure on the perception of safety climate and team climate of nurses and anesthesiologists from an operating room. Quasi-experimental study held in the operating room of a hospital in Brazil with a sample of nurses and anesthesiologists. The outcome was evaluated through the instruments "Safety Attitudes Questionnaire/Operating Room Version" and "Team Climate Inventory", applied before and after the implementation of a Patient safety checklist: nursing in anesthetic procedure by nurses. The mixed effects linear regression model was used to analyse the effect of the implementation. Altogether, 19 (30.2%) nurses and 44 (69.8%) anesthesiologists participated in the study, implementing the Patient safety checklist: nursing in anesthetic procedure in 282 anesthesias. The Safety Attitudes Questionnaire/Operating Room Version score changed from 62.5 to 69.2, with modification among anesthesiologists in the domain "Perception of management" (p = 0.02). Between both professionals, the Team Climate Inventory score increased after the intervention (p = 0.01). The implementation of the Patient safety checklist: nursing in anesthetic procedure changed the perception score of safety and teamwork climate, improving communication and collaborative work.

Safety elements and understanding of guidelines at discharge from Neonatal Intensive Care Units.

To compare safety elements and the understanding of guidelines at discharge from the Neonatal Intensive Care Unit between public and private hospitals. Cross-sectional study, developed in two Neonatal Intensive Care Units in the Midwest of Brazil. The Checklist for patient safety in preparing for discharge from the Neonatal Intensive Care Unit and the Assessment Scale of Patient Understanding about Discharge Guidelines were used for a sample (n=105) of caregivers of newborns. Descriptive and inferential statistical analysis was performed. The Checklist item "skin care and body hygiene" had greater conformity in the private sector (96.9%) than in the public sector (73.2%) (p-value<0.000) and the understanding of discharge guidelines had an overall mean of 62.9±8.3 points, with no significant difference between the locations. Compliance with safety elements and understanding of guidelines at discharge were equal among hospitals.

PATIENT SAFETY CHECKLIST IN NEUROANAESTHESIA: AN IMPACT ON PROTOCOL AND OUTCOME.

BACKGROUND: Surgical safety checklists is a common tool to prevent human fallacies and may help in avoiding complications .Major surgeries like neurosurgery need a thorough preparation and a specic checklist tailored to neurosurgery which may serve the purpose .So we intend to follow the neurosurgical safety checklist to elude anticipated complications. Objectives 1. To identify the adherence to various elements of the Modied WHO SSC for neurosurgery. 2. To analyse how a mandatory speciality-specic checklist practice can help in early identication of those interventions that are specic to neurosurgery, which can otherwise be overlooked. METHODOLOGY: Ÿ Study Design: Observational study Ÿ Duration of study: From May 2021 to August 2021. Ÿ Sampling Method: Convenience sampling Ÿ Sampling Procedure: Systematic Random Sampling. RESULT: This study was carried out in 77 patients undergoing various neurosurgery procedures and was found that none of the patients had diagnosis, procedure written on the name tag in pre-op period and ICP monitoring tools were not available. It was found that the anaesthesiologist as a safety list coordinator can implement the checklist ensuring minimal complications related to the surgery and also active involvement of all the members. CONCLUSION: The implementation of surgical safety checklist for Neurosurgery, by a designated checklist co-ordinator, can rectify anaesthetic and surgical facets promptly, without increasing the time required for the operation and also reduces the risk for post-op recovery.

Checklists in Femur Fractures: High Adherence After Implementation of Computer-based Pediatric Femur Guidelines.

Introduction:The American Academy of Orthopaedic Surgeons (AAOS) created an evidence-based clinical practice guideline for the care of pediatric diaphyseal femur fractures in 2010. Our institution implemented checklists based off these guidelines embedded in a standardized EMR order. The purpose of this study was to describe compliance with checklist completion and to assess safety improvement in a large urban pediatric hospital.Methods:Retrospective and prospective data were collected from 2 years before and 5 years after checklist implementation. This included the patient safety checklists from August 2011 through August 2016. Patients aged 0 to 18 years with a diaphyseal femur fracture were queried from the EMR and included in this study. Patient charts were reviewed for complications, including nerve injury, pressure sore, leg length discrepancy, loss of reduction, failure of fixation, nonunion, delayed union, and infection. Compliance rates were reported based on the AAOS clinical practice guidelines.Results:A total of 313 patients for the postchecklist period were reviewed in this study. Of 219 patients eligible for inclusion, 198 had checklists completed (group B). This group was compared with 100 patients with diaphyseal femur fractures from the period before implementation of the checklist (group A). We found no statistical difference in the number of patients with complications between groups (12% in both groups, P = 0.988). Postoperative checklists demonstrated that 89.9% of patients (178/198) received age-appropriate treatment consistent with the AAOS guideline recommendations after implementation of the checklist. Before the checklist implementation (group A), 94% (94/100) adhered to the guidelines.Conclusion:This study reveals high compliance rates with the AAOS evidence-based clinical practice guideline for the management of pediatric femur fractures. Implementing standardized checklists is possible by embedding them into the EMR. Implementation of checklists did not improve compliance or patient outcomes.

Adherence of the nursing team to patient safety actions in neonatal units.

to describe the nursing team's adherence to patient safety actions in neonatal units using a validated instrument. a cross-sectional study, carried out through direct observation of the nursing team and descriptive analysis of 182 records of the "Checklist for patient safety in nursing care during hospitalization in Neonatal Intensive Care Units" in a hospital in the municipality of Belo Horizonte. there was evidence of adherence greater than 90.0% in the units concerning the use of the identification wristband and guidance of the companions. It was identified 79.0% of absence on the checking of wristband identification and 59.0% of the absence of an evaluation of the crib wheels' locks. Three of the 21 items included in the checklist did not show non-conformities. partial adherence to patient safety actions was observed, especially regarding the target of patient identification and prevention of falls, which exposes newborns to preventable adverse events.

Intraoperative dislodgement of throat pack into esophagus and its management

Throat packs are commonly used in Intra-oral and Maxillofacial surgical procedures to prevent fluids, particulate matter as well as foreign bodies from entering into the aero-digestive tract. The pack is usually placed following endotracheal intubation which involves packing it around the Endotracheal tube (ETT) in the Oropharyngeal region as a reinforcement measure. The need for such a pack arises due to a variety of reasons such as, volumetric change in the airway resulting from a change in the tissue turgor and reducing pressure of the ETT cuff, which may result in the seepage of fluids into the airway, resulting in an emergency. If a throat pack has been inserted but not removed at the end of a procedure, then the obvious danger is that the retained pack may cause airway obstruction. We present an interesting case of a missing pharyngeal throat pack at the end of an operation. The insertion of the throat pack must be documented in the patient safety checklists well as prominently displayed to ensure proper removal at themed of surgery. If the pack is missing its location must be verified before extubating the patient as was in our case where the prudent and prompt action of the Anaesthetic and Surgical team prevented a major complication. This case report represents an interesting case of a missing pharyngeal throat pack at the end of an operation.

ASSESSMENT OF PATIENT SAFETY IN VACCINATION ROOMS

ABSTRACT Objective: identify adherence to patient safety recommendations in the vaccination room. Method: this is a cross-sectional study that analyzed 463 vaccination procedures in six vaccination rooms in Minas Gerais from June to July 2018. Data were obtained with the application of the Patient Safety Checklist for Vaccination Rooms through systematic observation of the vaccination procedure. Descriptive statistics were used for data analysis. Results: 463 vaccination procedures were observed, and the mean overall adherence score was 58.5%, min. 43.3% and max. 74.1%. The items of higher adherence were related to the records of vaccine data (name, date, and batch) on the vaccination card; vaccine application with dose, route of administration, location and correct materials; and records in an information system. The items of lower adherence were related to health guidance; investigation of adverse events following immunization and the health status of the vaccinated person; records of vaccine laboratory and vaccination center on the vaccination card; vaccine workforce handwashing; and proper vaccine preparation. Conclusion: the mean overall score found in this study was 58.5% for the adherence to recommendations for vaccination procedures. This result highlights the need for educational interventions that promote patient safety in the vaccination room and studies analyzing the factors that prevent such adherence.

Development of a Patient-Safety Checklist for Use During Fluoroscopic or Image-Guided, Minimally-Invasive Procedure in the Department of Radiology, Philippine General Hospital

Objective. This study aimed to develop a patient-safety checklist for use during fluoroscopic- or image-guided minimally-invasive procedures in the Department of Radiology, Philippine General Hospital.Methodology. A comparison of the Radiological Patient Safety System (RADPASS) and the Cardiovascular and Interventional Society of Europe (CIRSE) checklists was done. Together with the knowledge of the workflow and through observation of the different procedures in the department, a checklist was developed to suit the appropriate hospital setting. This developed checklist was tested in several procedures, and was evaluated and modified during trial testing.Result. A patient safety checklist for minimally-invasive and fluoroscopic procedures in the Department of Radiology, Philippine General Hospital was developed through the analysis of the workflow of the department, and observation during the data gathering and trial testing phases.Conclusion. This simple tool was developed to ensure that all the necessary details before a procedure have been addressed. It has been made as simple as possible, to make it user-friendly. The developed checklist is a step forward in promoting and ensuring the safety of patients undergoing fluoroscopic and minimally-invasive procedures in the Department of Radiology, PGH.

Quality Improvement Methodology to Optimize Safe Early Mobility in a Pediatric Intensive Care Unit.

EMP was implemented in our PICU in May 2017. The percentage of appropriate physical and occupational therapist consults were determined. We also evaluated the activity levels received by the patient and the levels for which they qualified based on their medical condition. Failure Modes and Effects Analysis (FMEA) was performed to identify potential complications related to the mobilization of critically ill children. We created 4 simulation scenarios based on FMEA prioritized results. After the implementation of EMP, appropriate physical and occupational therapist consults significantly increased (P < 0.0001). However, most patients still failed to receive the optimal level of activity recommended by protocol. This failure was partly due to concern for safety events during mobilization. FMEA identified vital sign changes [Risk Priority Number (RPN) 97.8], staff injury (RPN 64), and pain/anxiety (RPN 60.5) as potential safety events. We performed various in-situ simulation sessions based on these potential events. In post-simulation evaluations, 100% of participants agreed that the simulation experience would improve their performance in the actual clinical setting. Feedback from simulations led to the development of an EM patient safety checklist and clinical pathway. We describe a novel technique of using FMEA to develop scenarios that simulate potential adverse events to optimize safe EM in PICU. An EM checklist and pathway can guide in the implementation of safe EMP.

EVALUATION DE LA PERTE SANGUINE DURANT LA PÉRIODE PÉRIOPÉRATOIRE EN CHIRURGIE ORTHOPÉDIQUE ET TRAUMATOLOGIQUE ASSESSMENT OF BLOOD LOSS DURING THE PERIOPERATIVE PERIOD IN ORTHOPEDIC AND TRAUMA SURGERY

Introduction: The recognition of perioperative blood loss as a factor in patient mortality and morbidity led to the operating room patient safety checklist recommended by the World Health Organization. &#x0D; Methods: This was a prospective, descriptive study over a ten-month period from January to October 2019 including all the patients hospitalized in the Orthopedic Surgery and Traumatology department of the CHU Anosiala. All patients operated on for an orthopedic or trauma pathology were included in this study, patients having undergone percutaneous surgery were excluded and not included patients not operated on. &#x0D; Results: One hundred and twelve patients were collected. The average age of the patients was 32.9 years, there is a male predominance with a sex ratio of 2.5. Screw-retained plate osteosynthesis was the most frequent intervention (29.5%) in traumatology, in Orthopedics the installation of the total hip arthroplasty (THA) (6.3%) and sequestrectomy (11.6%) in osteo-articular infection. The mean blood loss for the whole procedure was 508 milliliters (ml) of which in orthopedics, the placement of the total knee arthroplasty (TKA) had the highest mean loss (1630 ml) followed by THA and open reduction neglected dislocations with a respective loss of and 1101 ml and 623.3 ml. In trauma, screw-retained plate osteosynthesis of the femur was the most hemorrhagic with an average blood loss of 820.3 ml and in osteoarticular infections, sequestrectomy with an average loss of 596.2 ml. &#x0D; Conclusion: The amount of blood loss in orthopedic and trauma surgery varies depending on the type of surgery performed. Arthroplasty, open reduction of a neglected dislocation, screw-retained femoral plate osteosynthesis and sequestrectomy were the most hemorrhagic type of procedure in the perioperative period

PurposeThis study aimed to evaluate the implementation of nursing actions in anesthesia guided by the Patient Safety Checklist: Nursing Anesthetic Procedure and the difficulties in performing patient care. DesignA cross-sectional study. MethodsNursing actions were evaluated in 282 anesthetic procedures of adult patients undergoing general anesthesia in three periods of anesthesia in a private hospital operating room in Brazil, from January to July 2018. The evaluated outcomes were the number of items performed by nurses during anesthesia periods and limitations for applying the Patient Safety Checklist: Nursing Anesthetic Procedure instrument. FindingsExecution of the nursing actions presented a mean percentage of 85.6, highlighting the anesthetic induction stage and differences between the periods before induction and reversal. The main items not performed in evaluating the nursing care in anesthesia were as follows: the anesthesia equipment test (84; 29.78%) in the preinduction period; the evaluation of adequate ventilation (132; 46.81%) in the induction period; and the tracheal aspiration assistance (62; 21.98%) during reversal. Receiving a call from another room with care interruption was highlighted among the nurses' justifications for not performing the activities and difficulty in continuing care because of shift change. ConclusionsThe execution of nursing care actions in anesthesia mainly occurred during the period related to induction, with limitations because of dimensioning of operating rooms and simultaneous procedures.
