Survey on Patient Satisfaction with the Usability and Formulation of Budesonide Rectal Foam Following a Change in Head and Applicator Shape and Properties

Budesonide Rectal Foam (BF) was introduced in 2017 and changed in November 2022 upon request, addressing the challenges encountered with liquid rectal formulations indicated for ulcerative colitis (UC). This formulation is an important agent in the treatment of rectal to sigmoid colon lesions in moderate UC. As the characteristics of the formulation of the rectal formulation are thought to influence patient satisfaction, a survey was conducted on the formulation and patient satisfaction among patients who used BF before and after the change. The survey spanned from January 2023 to May 2023. As the primary endpoint, the same patients were evaluated on the Visual Analogue Scale (VAS) for patient satisfaction. Significant variations in formulation usability and patient satisfaction were observed in 20 eligible patients before and after the change (p<0.05). Patient satisfaction with the formulation was strongly correlated with formulation usability, ease of pushing the head, and ease of insertion (r>0.7). The change in packaging was thought to improve the usability of the formulation and patient satisfaction. The formulation's usability and ease of insertion had a clear influence on satisfaction with the rectal formulation.

Assessment of Minimally Invasive Surgical Techniques for Sagittal Craniosynostosis: A Multi-Center Time Series Study.

Minimally invasive strip craniectomy is a well-established treatment for sagittal craniosynostosis, however the temporality of change in head shape has not been assessed. In this study, we perform head shape analysis to compare time-series-based clinical outcomes between three different surgical techniques across three academic centers for the treatment of sagittal craniosynostosis. Retrospective, longitudinal 3D images were collected from patients who underwent surgery for the correction of sagittal craniosynostosis for up to 5 years post-operation. The surgical methods studied include spring-assisted craniectomy, narrow-strip craniectomy plus orthotic helmet therapy, and wide-strip craniectomy with biparietal and bitemporal barrel stave wedge osteotomies plus orthotic helmet therapy. Postoperative 3D images were binned into six age groups. Cranial index measurements were calculated on 3D images. 3D whole-head composite images were generated for each procedure to visually represent longitudinal outcomes. The median with interquartile range (IQR) CI measurements at 3-5 years postoperative follow up were 75.9 [73.1-78.6] for spring-assisted, 75.9 [75.4-78.5] for narrow-strip, and 79.4 [76.4-81.9] for wide-strip. The wide-strip CI was significanly different from the spring-assissted and narrow strip groups (p < 0.001). Concerning 3D analysis, spring-assisted patients showed normalization of frontal bossing and skull height compared to age-matched controls, while wide-strip patients showed greater correction of occipital bulleting. Narrow-strip patients had intermediate results between these outcomes. There was no statistically significant regression in longitudinal CI measurements across the three techniques aimed at treating sagittal craniosynostosis. Longitudinal comparison of 3D head shape outcome demonstrated satisfactory correction of scaphocephalic deformity across all three surgical groups.

Changes in Cephalic Index and Head circumference of Igbo Children and Adolescents in a Nigerian Population

Background: The cephalic index and head circumference are important anthropometric parameters that can provide insights into the physical characteristics and growth patterns of populations. There is dearth of data on Cephalic Index and Head circumference of Igbo Children and Adolescents. Method: This study examined 725 Igbo children and adolescents aged 5 to 19 years in Enugu, South-eastern Nigeria, using a cross-sectional approach. Results: The majority of participants exhibited mesocephalic head shape (43.2%), with brachycephalic and dolichocephalic shapes observed in 32.7% and 14.2% of the population, respectively. Mean head circumference increased with age in both sexes. Cephalic indices in boys under 12 and girls under 15 were below 80 but increased above these ages. Changes in head shape, potentially towards brachycephalization, were noted as children grew. Significant correlations were found between head circumference and cephalic index with age, height, weight, and body mass index, suggesting their relevance in forensic and anthropological investigations. Conclusion: Our results indicate diversity in head shapes and sizes, with mesocephalic heads being most common. As age increases, head circumference also tends to increase, and changes in head shape towards brachycephalization was observed. These findings underscore the importance of understanding anthropometric variations in different populations and suggest avenues for further research, particularly longitudinal studies, to explore the factors influencing these patterns.

Understanding the influence of surgical parameters on craniofacial surgery outcomes: a computational study.

Sagittal craniosynostosis (SC) is a congenital condition whereby the newborn skull develops abnormally owing to the premature ossification of the sagittal suture. Spring-assisted cranioplasty (SAC) is a minimally invasive surgical technique to treat SC, where metallic distractors are used to reshape the newborn's head. Although safe and effective, SAC outcomes remain uncertain owing to the limited understanding of skull-distractor interaction and the limited information provided by the analysis of single surgical cases. In this work, an SC population-averaged skull model was created and used to simulate spring insertion by means of the finite-element analysis using a previously developed modelling framework. Surgical parameters were varied to assess the effect of osteotomy and spring positioning, as well as distractor combinations, on the final skull dimensions. Simulation trends were compared with retrospective measurements from clinical imaging (X-ray and three-dimensional photogrammetry scans). It was found that the on-table post-implantation head shape change is more sensitive to spring stiffness than to the other surgical parameters. However, the overall end-of-treatment head shape is more sensitive to spring positioning and osteotomy size parameters. The results of this work suggest that SAC surgical planning should be performed in view of long-term results, rather than immediate on-table reshaping outcomes.

Analysis of ontogenetic changes in head shape and diet in a catfish with moderately enlarged jaw adductors (Clariallabes melas)

Jaw adductor hypertrophy, or the presence of enlarged jaw-closing muscles, has arisen several times independently in African clariid catfish. Previous work has demonstrated that species characterized by enlarged jaw adductors may have unusual ways of foraging such as terrestrial foraging and prey capture, and often include a large proportion of hard and terrestrial prey in their diet. However, relatively little is known about species of the genus Clariallabes with an intermediate degree of jaw adductor hypertrophy. In the present study we present data on head shape and diet for a range of sizes of specimens of a poorly known species of Clariallabes, C. melas. Our data show that growth patterns in this species and the previously studied &#x0D; C. longicauda are similar in some ways (e.g. positive allometry of the growth of the jaw muscles) but different in others (negative allometry in hyoid width). However, C. melas has a smaller head for its body size in all dimensions. Due to the large number of empty stomachs we encountered, dietary data remain preliminary, but suggest a varied diet including both hard and soft prey. Our data show that a large amount of variation in head shape may exist even among related species and that species with jaw adductor hypertrophy generally show positive allometry in the growth of the jaw adductors and associated structures. Whether jaw muscle hypertrophy is an adaptive trait in clariid catfish awaits further comparative analyses testing for the evolutionary association between jaw adductor hypertrophy and the inclusion of hard prey into the diet.

What can a morphometric study of unoperated children teach us about the natural history of metopic synostosis?

Outcomes of surgical repair of trigonocephaly are well reported in the literature, but there is a paucity of information on the natural history of unoperated children. The authors evaluated a group of unoperated children with metopic synostosis to describe the natural change in head shape over time. A database was screened for scans of children with unoperated trigonocephaly (2010-2021). Multisuture cases and those with a metopic ridge were excluded. Three-dimensional surface scans (3D stereophotogrammetry/CT) were used for morphological analysis. Nine previously published parameters were used: frontal angle (FA30°), anteroposterior (AP) volume ratio (APVR), AP area ratio (APAR), AP width ratios 1 and 2 (APWR1 and APWR2), and 4 AP diagonal ratios (30° right APDR [rAPDR30], 30° left APDR [lAPDR30], 60° right APDR [rAPDR60], and 60° left APDR [lAPDR60]). Ninety-seven scans were identified from a cohort of 316 patients with a single metopic suture, in which the male-to-female ratio was 2.7:1. Ages at the time of the scan ranged from 9 days to 11 years and were stratified into 4 groups: group 1, < 6 months; group 2, 6-12 months; group 3, 1-3 years; and group 4, > 3 years. Significant improvements were detected in 5 parameters (APVR, APAR, APWR1, rAPDR30, and lAPDR30) over time, whereas no significant differences were found in FA30, APWR2, rAPDR60, and lAPDR60 between age groups. Forehead shape (surface area and volume), as well as narrowing and anterolateral contour at the frontal points, differed significantly over time without surgery. However, forehead angulation, narrowing, and anterolateral contour at temporal points did not show significant differences. This knowledge will aid in surgical and parental decision-making.

Family Experiences with Diagnosis of Craniosynostosis: Thematic Analysis of Online Discussion Boards.

Apply thematic analysis of online discussion boards to characterize families' experiences and concerns regarding craniosynostosis diagnoses to aid physicians in tailoring care to families. Grounded theory-based qualitative analysis. Discussion boards related to craniosynostosis identified via Google and Yahoo. Posts about craniosynostosis between 2017-2022. Thematic analysis was performed using three rounds of coding. Post features including author type and use of technical language were examined. Overarching themes emerging from analysis of posts, with forums analyzed until sufficient thematic repetition was observed. 366 posts from 4 websites by 290 unique users were included. Parents of patients with craniosynostosis wrote 59% of posts while patients wrote 4%. Five selective codes were identified: 1) Building Community, 2) Diagnosis/Evaluation, 3) Treatment, 4) Outcomes, and 5) Emotional Concerns. Building Community was the most assigned code (85% of posts). 71% of parents' posts expressing emotional concerns expressed negative emotions, commonly regarding anxiety about diagnosis (71%), frustration about doctors' responses (21%), or negative reactions to online search results (17%). 88% of patients' posts expressed positive emotions, discussing positive long-term outcomes. Concerns that may guide physicians included anxiety about delayed diagnosis, difficulty distinguishing postpartum head shape changes from craniosynostosis, and difficulty finding a care team. Online discussion boards allow families of patients with craniosynostosis to share experiences and find community. Improving communication between surgeons, pediatricians, and families about timing of evaluation and revising online information about this condition may ameliorate some anxiety associated with this diagnosis.

Speeding up the integration of Ukraine into the European railway transportation is an important task in the current development of the Ukrainian railway transport. Currently, the most effective way to travel across borders between countries with different track gauges is the use of gauge-changeable wheelsets. Continuous traffic on the Ukrainian (1520 mm gauge) and European (1435 mm gauge) railways calls not only for gauge changing facilities, but also for the compatibility of the wheel-rail contact pair on both railways: R65 rails and a cant of 1/20 in Ukraine and UIC60 rails and a cant of 1/40 in Europe. At the Institute of Technical Mechanics of the National Academy of Sciences of Ukraine and the State Space Agency of Ukraine, a new wear-resistant wheel profile, ITM-73ER, was proposed. As predicted, its use in passenger cars will offer dynamic stability and a high dynamic performance throughout the range of operating speeds on the Ukrainian and European railways and acceptable indices of wheel – unworn rail interaction on both railways. In service, the shapes of the wheel and rail contact surfaces change due to wear, especially in curves. Because the Ukrainian and European railways mostly use wheel and rail profiles of their own, the use of the new wheel profile may impair the wheel–rail interaction process, enhance wheel flange wear, and shorten the wheel life. The goal of this work is to study the effect of the in-service rail head shape change in curves of the Ukrainian and European railways on the wheel–rail interaction indices of a passenger car with ITM-73ER profile wheels. The head shapes of outer rails of the Ukrainian and European railways’ circular curves were predicted for a side flange wear changing from 0 to 8 mm in 2 mm increments. The calculations were made for two circular curves of radius 300 m with UIC60 rails and a cant of 1/40 (Europe) and R65 rails and a cant of 1/20 (Ukraine). To speed up the prediction, it was assumed that the curves were traveled by four-axle fully loaded freight cars, which maximizes the rail wear. The freight car wheels were assumed to be unworn and machined to the S1002 profile (for the European railways) and to the standard profile specified by the Ukrainian State Standard GOST 10791:2016 (for the Ukrainian railways). Using the computed head shapes of R65 and UIC60 rails differing in wear degree, a study was conducted into their effect on the wheel–rail pair strain and stress field and the dynamic indices of car–track interaction for passenger cars with ITM73-ER profile wheels negotiating a circular curve of radius 300 m. It was shown that the use of the ITM-73ER wheel profile in passenger cars will offer improved indices of car–track interaction, for worn rails too, both on the Ukrainian railways and in the combined operation on the Ukrainian and European railways.

Geometric morphometrics and ecological niche modelling for delimitation of Triatoma pallidipennis (Hemiptera: Reduviidae: Triatominae) haplogroups

A recent phylogenetic analysis of Triatoma pallidipennis, an important Chagas disease vector in Mexico, based on molecular markers, revealed five monophyletic haplogroups with validity as cryptic species. Here, we compare T. pallidipennis haplogroups using head and pronotum features, environmental characteristics of their habitats, and ecological niche modeling. To analyze variation in shape, images of the head and pronotum of the specimens were obtained and analyzed using methods based on landmarks and semi-landmarks. Ecological niche models were obtained from occurrence data, as well as a set of bioclimatic variables that characterized the environmental niche of each analyzed haplogroup. Deformation grids for head showed a slight displacement towards posterior region of pre-ocular landmarks. Greatest change in head shape was observed with strong displacement towards anterior region of antenniferous tubercle. Procrustes ANOVA and pairwise comparisons showed differences in mean head shape in almost all haplogroups. However, pairwise comparisons of mean pronotum shape only showed differences among three haplogroups. Correct classification of all haplogroups was not possible using discriminant analysis. Important differences were found among the environmental niches of the analyzed haplogroups. Ecological niche models of each haplogroup did not predict the climatic suitability areas of the other haplogroups, revealing differences in environmental conditions. Significant differences were found between at least two haplogroups, demonstrating distinct environmental preferences among them. Our results show how the analysis of morphometric variation and the characterization of the environmental conditions that define the climatic niche can be used to improve the delimitation of T. pallidipennis haplogroups that constitute cryptic species.

Mosaic adaptive peak shifts underlie body shape diversification in pelagiarian fishes (Acanthomorpha: Percomorpha)

Abstract Extreme body elongation in fishes is a major evolutionary transformation that extends the boundaries of morphological diversity and alters aspects of function, behaviour and ecology. Prior studies have identified features of the cranial and axial skeleton that characterize elongate fishes, but a lack of detailed reconstructions of anatomical evolution has limited inferences about factors that underlie major shifts in body shape. In this study, we fitted multi-peak adaptive (Ornstein–Uhlenbeck) evolutionary models to species body shape and anatomical dimensions in Pelagiaria, a radiation of open-ocean fishes whose species span a continuum from deep bodied to highly elongate. We inferred an ancestral fusiform adaptive peak that is retained by several major pelagiarian lineages (e.g. Scombridae) and found robust support for multiple transitions to deep-bodied optima (in the families Stromateidae, Bramidae and Caristiidae) and elongate-bodied optima (within Trichiuroidei), including two instances of sequential shifts towards increasingly elongate optima that followed distinct paths of anatomical evolution. Within Trichiuridae, initial increases in head length and the number of vertebrae were followed by changes in head and vertebral shape. Within an elongate-bodied subclade of taxa traditionally identified as ‘gempylids’, changes in head and vertebral shape and in the number of precaudal vertebrae preceded an increase in the number of caudal vertebrae. Altogether, this mosaic of anatomical peak shifts suggests that body shape transformations were associated with differing selective demands and developmental changes.

Correction of trigonocephaly after endoscopic strip craniectomy with postoperative helmet orthosis therapy: a 3D stereophotogrammetric study.

Endoscopic strip craniectomy with postoperative helmet orthosis therapy (ESCH) has emerged as a less invasive alternative to fronto-orbital remodeling for correction of trigonocephaly. However, there is no standardized objective method for monitoring morphological changes following ESCH. Such a method should be reproducible and avoid the use of ionizing radiation and general anesthesia for diagnostic imaging. The authors analyzed a number of metrics measured using 3D stereophotogrammetry (3DSPG) following ESCH, an imaging alternative that is free of ionizing radiation and can be performed on awake children. 3DSPG images obtained at two time points (perisurgical and 1-year follow-up [FU]) of children with metopic synostosis who had undergone ESCH were analyzed and compared to 3DSPG images of age-matched control children without craniofacial anomalies. In total, 9 parameters were measured, the frontal angle and anteroposterior volume in addition to 7 novel parameters: anteroposterior area ratio, anteroposterior width ratios 1 and 2, and right and left anteroposterior diagonal ratios 30 and 60. Six eligible patients were identified in the operated group, and 15 children were in the control group. All 9 parameters differed significantly between perisurgical and age-matched controls, as well as from perisurgical to FU scans. Comparison of FU scans of metopic synostosis patients who underwent surgery to scans of age-matched controls without metopic synostosis revealed that all parameters were statistically identical, with the exception of the right anteroposterior diagonal ratio 30, which was not fully corrected in the treated patients. The left anterior part of the head showed the most change in surface area maps. In this pilot study, ESCH showed satisfactory results at 1 year, with improvements in all measured parameters compared to perisurgical results and normalization of 8 of 9 parameters compared to an age-matched control group. The results indicate that these parameters may be useful for craniofacial units for monitoring changes in head shape after ESCH for trigonocephaly and that 3DSPG, which avoids the use of anesthesia and ionizing radiation, is a satisfactory monitoring method.

P73. CRITICAL ANALYSIS OF SPRING ASSISTED CRANIOPLASTY OUTCOMES IN THE CONTEXT OF SURGICAL DECISION MAKING

PURPOSE: This purpose of this study is to examine the correlation between surgeon-selected parameters (number of springs, spring force and maximum excursion width, age at surgery) and outcomes of spring-assisted cranioplasty. METHODS: Patients with sagittal craniosynostosis who underwent spring-assisted cranioplasty for sagittal craniosynostosis from 2014-2021 were included. Clinical and radiographic data were collected. RESULTS: Sixty-two patients met inclusion criteria. Forty-five were male (73%) and 3 were syndromic (5%). The average age was 4.54 ± 0.83 months. The average number of springs implanted was 2.4 ± 0.5, the average spring force was 7.4 ± 0.4 N, and the average excursion was 72.3 ± 4.0 mm. The operative time was 1.4 ± 0.3 hours, the EBL was 37.7 ± 36.2 mL, and the length of stay was 1.3 ± 0.6 days. The average preoperative CI was 69.6 ± 5.5, the average postoperative CI was 75.0 ± 4.7, and the percent change in CI from pre to post-op was 7.7 ± 9.7%. There were no statistically significant associations between age at surgery and change in CI (p=0.51), spring force and change in CI (p=0.78), spring excursion and change in CI (p=0.54), or number of springs and change in CI (p=0.20). CONCLUSION: The change in CI from pre- to post-op was not affected by age at surgery, spring force or excursion, or number of springs. Despite the hypothesis that such parameters can be selected to increase or decrease the degree of head shape change, these modifiable factors do not seem to correlate with CI.

Determining the ideal osteotomy for stemless total shoulder replacement: a cadaveric study.

We sought to determine the angle of osteotomy that produces a circular humeral cut surface. A total of 49 cadaveric shoulders, from 25 cadavers, underwent sequential humeral head osteotomy from 180° (vertical, in line with the humeral diaphyseal shaft), in 10° increments, until the rotator cuff insertion was encountered. At each stage, the anteroposterior (AP) and superoinferior (SI) distances were recorded. The data were analyzed for normality and then assessed to determine the optimum cut angle. The AP/SI ratio is an indication of roundness. Plotting values of 1 - AP/SI (ie, error) vs. cut angle allowed us to plot the likelihood of producing a circular cut surface using a third-order curve that created the best fit to the data set (R2 = 0.99). The results from this study suggest that the optimum osteotomy angle that produces a circular cut surface is 23° from the vertical. The cohort data illustrated that at this angle, the average roundness error was 1% with a 95% confidence limit of <1%. There was no significant difference (P > .05) between sexes. The humeral head shape changes from oval to circular and then to an oval cut surface as the osteotomy angle increases from the vertical toward the horizontal. The range of angles within which the cut surface is circular, within a 10% error margin, is 18°-27° from the vertical, which is much less than the traditional osteotomy angle of 45°.

Correlation between head shape and volumetric changes following spring-assisted posterior vault expansion

The aim of the study was to investigate whether different head shapes show different volumetric changes following spring-assisted posterior vault expansion (SA-PVE) and to investigate the influence of surgical and morphological parameters on SA-PVE.Preoperative three-dimensional skull models from patients who underwent SA-PVE were extracted from computed tomography scans. Patient head shape was described using statistical shape modelling (SSM) and principal component analysis (PCA). Preoperative and postoperative intracranial volume (ICV) and cranial index (CI) were calculated. Surgical and morphological parameters included skull bone thickness, number of springs, duration of spring insertion and type of osteotomy.In the analysis, 31 patients were included. SA-PVE resulted in a significant ICV increase (284.1 ± 171.6 cm3, p < 0.001) and a significant CI decrease (−2.9 ± 4.3%, p < 0.001). The first principal component was significantly correlated with change in ICV (Spearman ρ = 0.68, p < 0.001). Change in ICV was significantly correlated with skull bone thickness (ρ = −0.60, p < 0.001) and age at time of surgery (ρ = −0.60, p < 0.001). No correlations were found between the change in ICV and number of springs, duration of spring insertion and type of osteotomy.SA-PVE is effective for increasing the ICV and resolving raised intracranial pressure. Younger, brachycephalic patients benefit more from surgery in terms of ICV increase. Skull bone thickness seems to be a crucial factor and should be assessed to achieve optimal ICV increase. In contrast, insertion of more than two springs, duration of spring insertion or performing a fully cut through osteotomy do not seem to impact the ICV increase. When interpreting ICV increases, normal calvarial growth should be taken into account.

Partial suturectomy for phenotypical craniosynostosis caused by incomplete fusion of cranial sutures: a novel surgical solution.

Patients presenting with head shape changes phenotypical for craniosynostosis may have incomplete fusion of the involved sutures. The surgical literature is lacking in appropriate management strategies for these patients. In this paper, the authors evaluate their experience with a novel treatment strategy: suturectomy of only the fused portion followed by helmeting therapy in patients with skull deformity secondary to incomplete suture synostosis. Patients with craniosynostosis with incomplete suture fusion requiring operative intervention between 2018 and 2020 were included for evaluation. Patients were selected for partial suturectomy if the patent portion of the suture had a normal appearance. All patients underwent craniectomy of the involved portion of the synostosed suture. Intraoperative ultrasound was used to reassess the degree of fusion at the time of surgery and incision planning. A 2- to 3-cm strip craniectomy was performed under direct visualization through a single minimal access incision. Postoperative helmeting was utilized for all patients. Demographic and perioperative data were collected, including laser scan data in the form of cranial index (CI) and cranial vault asymmetry (CVA), defined as the difference between two diagonal measurements, from the frontozygomaticus to the opposite eurion. Four males and 1 female with a mean age of 2.8 months (range 1.1-3.9 months) at presentation were included. All patients had incomplete sagittal synostosis (one patient also had an incomplete left lambdoid synostosis and another had an incomplete left coronal synostosis). The mean age at surgery was 3.5 months (range 2.0-4.7 months) without any major complications. All patients were compliant with postoperative helmeting. The average age at the last follow-up was 12.8 months (range 5.3-23.7 months) with a mean follow-up duration of 9.3 months (range 0.5-19.6 months). Final laser scan evaluations were available for 3 patients and showed an improvement of the CI from an average of 71.3 (range 70-73) to 84.3 (range 82-86). The CVA improved from an average of 9.67 mm (range 2-22 mm) to 1.67 mm (range 1-2 mm). Minimally invasive direct excision of the involved portion of fused cranial sutures followed by helmet therapy for phenotypical craniosynostosis is a safe and effective treatment strategy. This technique is suitable for very young patients and appears to offer similar outcomes to complete suturectomy. Further studies are required to see if this approach reduces the deformity severity for patients requiring vault remodeling later in life.

Morphological variations of the condylar head among children aged 4-15 years: A panoramic study.

Growth and development of dentocraniofacial complex occurs during various stage of development of dentition with TMJ as one of its centres of growth. The condition of temporomandibular joint can be visible from its condylar head form; therefore, it is crucial to recognize the normal morphology of condylar head during different stages of development of dentition. The aim of the study is to view the morphological appearance of condylar head during primary dentition, mixed dentition and permenant dentition. 400 panoramic images of 194 boys and 296 girls were collected and were divided into three groups bases on the stage of dentition. Shape of the condylar head were determined by tracing the print out of the panoramic image on an X ray viewer. The study showed that during primary dentition stage shape of the conylar head was dominated by round shape and as growth and development occurs the condylar head shape changes to convex. The study describes the normal morphology of mandibular condyles in a child population attending The dentition status as well as growth of craniofacial has a significant role in determining condylar morphology.

Endoscopic Versus Open Total Vault Reconstruction of Sagittal Craniosynostosis.

This is a retrospective comparative study involving 55 cases of sagittal craniosynostosis, 37 of which has open total cranial vault reconstruction and 18 had endoscopic suturectomy procedure. Preoperative and postoperative 3D photographs of both groups were analyzed and compared. The change in correction between preoperative and postoperative state was measured against a crowd-driven standard for acceptable head shape. Total cranial vault had higher percentage change between pre and postoperative cranial index than endoscopic suturectomy (14.7% versus 7.7%, P = 0.003). However, both techniques were able to achieve the minimum standard of 70% correction (TCV 107.5%, ES 100.4%, P = 0.02). Total cranial vault and endoscopic suturectomy are effective in correcting scaphocephaly among children with sagittal craniosynostosis. Additionally, both techniques are able to achieve a percentage correction that exceeds the 70% benchmark established by the lay public.

Identifying the Misshapen Head: Craniosynostosis and Related Disorders.

Pediatric care providers, pediatricians, pediatric subspecialty physicians, and other health care providers should be able to recognize children with abnormal head shapes that occur as a result of both synostotic and deformational processes. The purpose of this clinical report is to review the characteristic head shape changes, as well as secondary craniofacial characteristics, that occur in the setting of the various primary craniosynostoses and deformations. As an introduction, the physiology and genetics of skull growth as well as the pathophysiology underlying craniosynostosis are reviewed. This is followed by a description of each type of primary craniosynostosis (metopic, unicoronal, bicoronal, sagittal, lambdoid, and frontosphenoidal) and their resultant head shape changes, with an emphasis on differentiating conditions that require surgical correction from those (bathrocephaly, deformational plagiocephaly/brachycephaly, and neonatal intensive care unit-associated skill deformation, known as NICUcephaly) that do not. The report ends with a brief discussion of microcephaly as it relates to craniosynostosis as well as fontanelle closure. The intent is to improve pediatric care providers' recognition and timely referral for craniosynostosis and their differentiation of synostotic from deformational and other nonoperative head shape changes.

Using 3D‐digital photogrammetry to examine scaling of the body axis in burrowing skinks

Three-dimensional (3D) modeling techniques have been increasingly utilized across disciplines for the visualization and analysis of complex structures. We employ 3D-digital photogrammetry for understanding the scaling of the body axis of 12 species of scincid lizards in the genus Brachymeles. These skinks represent a diverse radiation which shows tremendous variation in body size and degree of axial elongation. Because of the complex nature of the body axis, 3D-methods are important for understanding how the body axis evolves. 3D-digital photogrammetry presents a flexible, inexpensive, and portable system for the reconstruction of biological forms. As body size increased among species, the cross-sectional area and circumference of the head and other portions of the body axis increased isometrically, which indicates that species of differing sizes possess proportionally similar head and body shapes. These results suggest that there are no substantial head and body shape changes with body size among the sampled species, but further comparative studies with larger sample sizes and functional studies of size and morphology effects on burrowing or above-ground locomotion are needed.

Growth trajectories of prenatal embryos of the deep-sea shark Chlamydoselachus anguineus (Chondrichthyes).

Chlamydoselachus anguineus, Garman 1884, commonly called the frilled shark, is a deep‐sea shark species occurring up to depths of 1300 m. It is assumed to represent an ancient morphotype of sharks (e.g., terminal mouth opening, more than five gill slits) and thus is often considered to represent plesiomorphic traits for sharks. Therefore, its early ontogenetic developmental traits are important for understanding the evolution of its particular phenotype. Here, we established six stages for prenatal embryos and used linear measurements and geometric morphometrics to analyse changes in shape and size as well as their timing during different embryonic stages. Our results show a change in head shape and a relocation of the mouth opening at a late stage of development. We also detected a negative allometric growth of the head and especially the eye compared to the rest of the body and a sexual dimorphism in total body length, which differs from the known data for adults. A multivariate analysis of covariance shows a significant interaction of shape related to the logarithm of centroid size and developmental stage. Geometric morphometrics results indicate that the head shape changes as a covariate of body size while not accounting for differences between sexes. The growth pattern of stages 32 and 33 indicates a shift in head shape, thus highlighting the moment in development when the jaws start to elongate anteriorly to finally achieve the adult condition of terminal mouth opening rather than retaining the early embryonic subterminal position as is typical for sharks. Thus, the antero‐terminal mouth opening of the frilled shark has to be considered a derived feature.
