Acupuncture Relieves Cervical Spondylosis Radiculopathy by Regulating Spinal Microglia Activation Through MAPK Signaling Pathway in Rats.

Local acupuncture has been found to have a good analgesic effect in rats with cervical spondylosis radiculopathy (CSR), but it lacks a regulatory effect on traditional Chinese medicine syndrome types of CSR. We proposed "Invigorating Qi and activating Blood" (IQAB) acupuncture, compared with Fenbid, and local electroacupuncture (LEA), to observe whether it has advantages in the protection of the CSR rat model and to elucidate its mechanism through the MAPK (mitogen-activated protein kinase) signaling pathway. Male Sprague-Dawley rats were randomly divided into six groups: control, sham, model, Fenbid, LEA, and IQAB. The CSR model was induced by inserting nylon sutures to compress the C4-T1 nerve root. The Fenbid group was treated with ibuprofen sustained-release capsules (15 mg/kg·d, ig). The LEA group received electroacupuncture at both C5 and C7 EX-B2 once a day. The IQAB group received acupuncture at both ST36 and BL17 based on the LEA group's intervention. Mechanical allodynia and gait, morphological changes in the spinal cord, IL-6 and TNF-α levels, MAPKs phosphorylation ratio, monocyte chemoattractant protein-1 (MCP-1) levels in the spinal cord, and the expression of p-p38 in the spinal cord and its colocalization with neurons and glial cell activation markers were detected. Mechanical allodynia, gait disorder, edema, reduced Nissl-positive cell numbers, and increased IL-6 and TNF-α levels in the spinal cord were observed in CSR rats. IQAB significantly alleviated these changes, and the effects were generally comparable to those of Fenbid. Meanwhile, the phosphorylation ratios of p38 and extracellular regulated protein kinase (ERK), co-expression of p-p38 with neuron/microglia, and MCP-1 levels in the spinal cord were markedly down-regulated by IQAB compared with those in CSR model rats. IQAB reduced p38-activation-related microglia activation and MCP-1 levels, thus alleviating pathological changes, inflammation levels in the local spinal cord, and pain behavior of CSR.

Study on the correlation between the thickness of superficial fascia at Dazhui(GV14) and cervical spondylosis

To observe the correlation between the thickness of superficial fascia at Dazhui (GV14) acupoint and cervical spondylosis, so as to explore the essence of its morphological and structural changes of acupoint sensitivity. A retrospective study was conducted. According to the diagnostic criteria of "Guidelines for Diagnosis, Treatment and Rehabilitation of Cervical Spondylosis" (2017), 344 cases of cervical spine magnetic resonance imaging (MRI) examination were included and divided into control group (73 cases) and observation group (271 cases). The control group was healthy population, and the observation group was patients with cervical spondylosis conforming to the diagnostic criteria, including cervical spondylosis of neck type, cervical spondylosis radiculopathy, cervical spondylotic myelopathy, cervical spondylosis of vertebral artery type, and sympathetic cervical spondylosis. According to MRI images of cervical spine, the structure of GV14 acupoint including skin, superficial fascia layer and aponeurosis ligament layer were measured. The acupoint depth and the superficial fascia thickness at GV14 in the observation group were (56.6±8.8) mm and (22.8±7.6) mm, the acupoint depth and the superficial fascia thickness at GV14 were (49.8±7.0) mm and (16.6±6.6)mm in the control group, which were significantly greater in the observation group than in the control group (P<0.01). The superficial fascia thickness at GV14 of cervical spondylotic mye-lopathy, cervical spondylosis of neck type and cervical spondylosis radiculopathy in the observation group was (23.8±8.1)mm, (23.0±7.3)mm and (22.6±6.5)mm, the acupoint depth of GV14 was (58.7±8.8)mm, (56.2±9.1)mm and (55.8±6.4)mm, which were significantly thicker than the superficial fascia thickness and the acupoint depth in the control group (P<0.01). In the observation group，the superficial fascia thickness of GV14 of cervical spondylosis myelopathy was significantly thicker than those of sympathetic cervical spondylosis (17.8±8.1) mm and cervical spondylosis of vertebral artery type (19.9±5.9) mm (P<0.01, P<0.05). In the observation group, the depth of GV14 of cervical spondylosis myelopathy was thicker than that of cervical spondylosis of neck type, cervical spondylosis radiculopathy, sympathetic cervical spondylosis and cervical spondylosis of vertebral artery type(P<0.05，P<0.01)； the depth of GV14 of sympathetic cervical spondylosis was thinner than that of cervical spondylosis of neck type and cervical spondylosis radiculopathy (P<0.01). The superficial fascia thickness at GV14 was correlated with cervical spondylosis, and it is also related to cervical spondylotic myelopathy, cervical spondylosis of neck type and cervical spondylosis radiculopathy. The morphological and structural changes of GV14 in the state of cervical spondylosis were mainly the thickness of the superficial fascia.

Application of Wearable Sensors in the Treatment of Cervical Spondylosis Radiculopathy with Acupuncture.

Research shows that cervical spondylosis radiculopathy (CSR) is the most common type of cervical spondylosis in clinic, and Chinese medicine treatment has obvious advantages, among which acupuncture therapy has received increasing attention. CSR has the characteristics of high incidence, long treatment time, and easy recurrence after treatment. In order to meet the different needs of different patients, this paper uses wearable sensors to collect patient dynamic data, extracts the action features of cervical spondylosis to design a scoring system, analyzes the input feature scores through a convolutional neural network (CNN) model, and then outputs personalized acupuncture treatment plan. The development status of wearable sensors at home and abroad is introduced, and the modules and functions of the wearable sensors are designed. The CNN network is used as the network model for classification and recognition. The experimental results show that the CNN model used in this paper has a high classification accuracy, achieving an accuracy of up to 97%, and can help produce an effective treatment plan. In order to determine whether the treatment plan output by the model is effective, each group of data is handed over to two cervical spondylosis experts for scoring, and then the final treatment plan is determined from 10 acupuncture plans. In our experiments, 9 out of 10 plans generated by the CNN model were the same as generated by the experts, which shows the effectiveness of the model.

Meta-analysis of the therapeutic effect of acupuncture and chiropractic on cervical spondylosis radiculopathy: A systematic review and meta-analysis protocol.

Background:The pathogenesis of cervical spondylotic is degenerative changes of the cervical intervertebral disc, or bone hyperplasia of the posterior and hook joints, and instability of the joints of the cervical vertebrae. It causes the nerve roots to be stimulated and oppressed. The clinical manifestations are the sensation, movement, and reflex disorder of the cervical spinal nerve roots that are stimulated and oppressed, especially the numbness and pain of the neck, shoulders, upper limbs, and fingers. In this systematic review, we aimed to evaluate the efficacy and safety of acupuncture and chiropractic in the treatment of cervical spondylotic.Methods and analysis:We will search for PubMed, Cochrane Library, AMED, Embase, WorldSciNet; Nature, Science online and China Journal Full-text Database (CNKI), China Biomedical Literature CD-ROM Database (CBM), and related randomized controlled trials included in the China Resources Database. The time is limited from the construction of the library to September 2019. We will use the criteria provided by Cochrane 5.1.0 for quality assessment and risk assessment of the included studies, and use the RevMan 5.3 and Stata 13.0 software for meta-analysis of the effectiveness, recurrence rate, and symptom scores of cervical spondylotic.Ethics and dissemination:This systematic review will evaluate the efficacy and safety of acupuncture and chiropractic for cervical spondylotic. Because all of the data used in this systematic review and meta-analysis have been published, this review does not require ethical approval. Furthermore, all data will be analyzed anonymously during the review process trial.

Assessment of the efficacy of acupuncture and chiropractic on treating Cervical spondylosis radiculopathy: A systematic review and meta-analysis.

Comparison of the Biomechanical Changes After Percutaneous Full-Endoscopic Anterior Cervical Discectomy versus Posterior Cervical Foraminotomy at C5-C6: A Finite Element-Based Study

Percutaneous full-endoscopic anterior cervical discectomy (PEACD) and posterior cervical foraminotomy (PCF) have been reported as effective treatments for the cervical spondylosis radiculopathy (CSR), but the biomechanical effects on the discs and facet joints of PEACD and PCF remain largely unclear. The purpose of this paper is to investigate and compare the biomechanical changes on cervical spine after PECAD and PCF procedures, thus providing evidences for surgeons to select a more appropriate approach. An intact cervical C5-C6 digital model was constructed and then modified to obtain the PCF and PEACD models using finite element method. All the models were subjected to a 73.6N preload accompanied by a 1.8 Nm moment during flexion, extension, axial rotation, lateral bending. The range of motion (ROM), intervertebral disc pressure (IDP), facet joint contact area, and contact pressure were calculated under different loading conditions. The ROM of the PCF model changed slightly (0.28%), whereas that of the PEACD model increased significantly (20.49%) compared with intact model. The trend of IDP changes in these 2 surgical models were similar to ROM in the corresponding motion state. The contact pressure on the facet joint of the PEACD model increased by 20.53%, 33.38%, and 17.46% during extension, lateral bending, and axial bending, respectively, compared with the intact model, and the PCF increased by 33.53% and 16.16% during extension and lateral bending, respectively, whereas it decreased 0.97% in axial rotation. The facet joint contact area of the PCF model increased by 85.71%, 1.54%, and 2.17% during extension, lateral bending, and axial rotation, respectively, and the area of the PEACD model increased by 157.14% and 36.96% during extension and axial rotation, whereas it decreased by 13.85% during lateral bending. This is the first biomechanical finite element study comparing PEACD with PCF for the treatment of CSR. Our results showed that PEACD led to hypermobility with high IDP within the cervical segment undergone surgery, whereas the ROM and IDP changed slightly after PCF. The variations of the contact stress indicated that both procedures changed the transmission path of the force on the facet joint and may accelerate the degeneration of the facet joint. PCF may be a better choice for the treatment of CSR compared with PEACD.

Ayurvedic management of cervical spondylosis radiculopathy

Radiculopathy, commonly referred as pinched nerve, refers to a set of conditions in which one or more nerves are affected and their functioning is hampered. This phenomenon is termed as neuropathy. This can result in pain or more precisely in radicular pain. In a radiculopathy, the problem occurs at or near the root of the nerve, shortly after its exit from the spinal cord. However, the pain or other symptoms often radiate to the part of the body served by that nerve. The management available in the current era of medicine is not satisfactory. Here, we are presenting a case of cervical spondylosis (CS) radiculopathy which was treated with a combination of Panchkarma procedures and Ayurvedic oral drugs. Greeva stambh was considered as the Ayurvedic diagnosis for the case. The treatment protocol designed for the particular case was Panchatikta ksheera Basti (enema with medicated milk) for 16 days along with Shalishastic pind sweda. The oral medication prescribed was Ashwagandha churna (Withania somnifera Dunal), Shatavari churna (Asparagus recemosus Wild), Gokshur churna (Tribulus terrestris L.) each in a dose of 2 g, Sarpgandha churna (Rauvolfia serpentina (L) Benth. Ex Kurz) 1 grms, Shankha bhasma 500 mg with Luke warm water, Yograj guggulu-3 tablet with Dashmool kwath in a dose of 40 ml, Ashwagandha avaleha 10 g with lukewarm water. All these drugs were prescribed for twice a day. Visual analog scale and range of motion were the parameter used to assess patient and considerable improvement was noticed in the case after a month of treatment which sustained in follow-up duration of 2 months. The case study demonstrate that CS radiculopathy may be successfully managed with Ayurvedic treatment.

Acupuncture and moxibustion based on meridian differentiation for cervical spondylosis radiculopathy:a randomized controlled trial

To observe the difference of clinical effects of acupoints selected based on meridian differen-tiation and conventional method for cervical spondylosis radiculopathy(CSR) treated with acupuncture and moxibustion. Sixty patients with CSR were randomly divided into an observation group and a control group,30 cases in each one. Ashi points of the injured meridians in the neck,namely six-he points,were selected in the observation group,matched with luo-connecting points of the injured meridians on the same side and the interiorly-exteriorly correlated meridians on the opposite side. Conventional acupuncture was used on Fengchi(GB 20),Jiaji(EX-B 2) of the neck,Tianzhu(BL 10),Jianjing(GB 21),Houxi(SI 3),Hegu(LI 4) and Waiguan(TE 5) on the affected side in the control group. Treatment was given once every other day,three times a week and total 12 times in the two groups. Twenty subscales for CSR and visual analogue scale(VAS) were observed before and after treatment,and total effects were evaluated after treatment. The effective rate of the observation group was 93.3%(28/30),which was better than 66.7%(20/30) of the control group(P<0.05). The scores of 20 subscales and VAS after treatment were all improved compared with those before treatment in the two groups(all P<0.01),with more apparent change in the observation group(both P<0.05). The effect of acupuncture and moxibustion based on meridian differentiation is obvious,and superior to that of conventional acupoints selection.

Comparison of efficacy between electroacupuncture and the combination of collateral bloodletting, cupping, and acupoint application for cervical spondylosis radiculopathy (CRS)

ObjectiveTo observe the efficacy of treating young patients with cervical spondylosis radiculopathy (CRS) by collateral bloodletting, cupping, and acupoint application. MethodsSixty-one CRS patients were randomly divided into an acupoint application group (group A, 31 cases) and an electroacupuncture group (group B, 30 cases). Both sides of the spine and scapula were treated with collateral bloodletting, cupping, and acupoint application over Ashi points in group A. The following points received electroacupuncture in group B: Jiāji (▪ EX-B2), Fēngchí (▪ GB 20), Jiānjing (▪ GB 21), jiānyú (▪ LI 15), Wàiguān (▪ TE 5), Hòuxī (▪ SI 3) and DàzhuT (▪ GV 14). Patients in the two groups were compared in terms of scores for total symptoms and signs, and the theraputic effect of each method was analyzed. ResultsTotal score of symptoms and signs of the two groups were both more significantly improved compared to that before treatment (9.96 ± 2.02 vs 15.87 ± 1.84, P<0.05 in group A; 10.02 ± 1.76 vs 13.59 ± 1.52, P<0.05 in group B). The differences in two groups before and after treatment were 5.91 ± 1.95 in group A, 3.53 ± 1.68 in group B, they were statistically significant (P<0.05). The total effective rate of group A [93.6% (29/31)] was better than that of group B [83.3% (25/30)], and the differences were statistically significant (P<0.05). ConclusionGood therapeutic effects can be achieved in treating young CRS patients with collateral bloodletting, cupping, and acupoint application.

Simultaneous determination of nineteen major components in Qi She Pill by ultra-high-performance liquid chromatography–tandem mass spectrometry

Qi She Pill (QSP) is a traditional Chinese medicine (TCM) prescription that has been used in treating cervical spondylosis radiculopathy for many years. In this study, a simple and sensitive method using ultra-high-performance liquid chromatography–tandem mass spectrometry (UHPLC–MS/MS) on a reverse-phase C18 column was developed for the simultaneous determination of the 19 major components in QSP. We found that the optimum mobile phase for gradient elution was 0.1% formic acid and methanol. The correlation coefficients of all calibration curves were greater than 0.99. Recoveries measured at three concentration levels varied from 95.43% to 102.35%. Relative standard deviations of intra- and inter-day precisions were less than 4.45%. After successfully validating our method, we then applied it to the quantification of 19 components in QSP products to show that this method provides a new standard in quality assessment of TCM prescriptions containing multiple bioactive components.
