The following text is the de-identified and edited transcript of an invited presentation by Professor Clare Delany on the topic of 'How clinical ethics discussions can be a model for accommodating and incorporating plural values in paediatric and adult healthcare settings.' Professor Delany's presentation formed part of the Conference on Accommodating Plural Values in Healthcare and Healthcare Policy, which was held in Melbourne, Australia, on Monday, October 30, 2023. This conference was a key output of the Australian Research Council Discovery Project grant DP190101597, 'Religion, pluralism, and healthcare practice: A philosophical assessment'. Professor Delany's presentation was introduced by Doctor Lauren Notini, Research Fellow and Lecturer at Monash Bioethics Centre, Monash University.

Encompassing medical ethics within the medical humanities?

As medicine becomes ever more technologically advanced, "human skills" are becoming increasingly important. Medical ethics or bioethics and medical humanities may have been formally introduced into the curriculum about the same time around the 1970s in certain developed nations. However, in many developing nations, medical/health humanities is much more recent and only came into prominence during the first two decades of the twenty-first century. The term "bioethics" was coined by Potter in 1970 [1]. During the ensuing five decades, however, medical ethics has become the dominant discipline of the two, globally. Medical ethics is more well-known and has greater resources allotted. Medical ethics may be a less radical and more comfortable concept and the study of ethical issues in medical practice may not challenge the traditional knowledge and power structures inherent in medicine. While we have a Centre for Bioethics and Humanities at my present university, I feel a more logical and correct name would be Centre for Humanities and Bioethics, emphasising the greater scope of the humanities.

Joining Humanity and Science: Medical Humanities, Compassionate Care, and Bioethics in Medical Education.

Educating medical students to be physicians involves many dimensions. But in an educational culture where science and technology dominate the curriculum, these subjects also too often dominate the academic value system as well. While a firm grasp of scientific knowledge and technical skill is essential, cultivating humanistic virtues is at the core of all good medical care and the full well-being of those within it. This article describes a formative educational process that points towards compassionate flourishing and unfolds through dialogue and reflection on the human aspects of patient care and the student experience, a process coequal in value to scientific development. This educational process has been successfully implemented at the Center for Medical Humanities, Compassionate Care, and Bioethics at Stony Brook. When supported by a broader institutional culture through an ongoing reflective group process in residencies and other clinical settings, this process fosters professional flourishing, which leads to deeper meaning and compassionate care of patients.

Policy vs. Ethics.

Editor's Note: As Catholic ethics increasingly diverge from those of other religious traditions and from secular principles, it is important to explore how-if at all-Catholic physicians and institutions can work with those whose view of life and faith are very different. This article presents not only an historical vision of how the Ethical and Religious Directives came to be, it raises questions about just how Catholics go about moral reasoning when there are competing interests and principles at play. The author of the article, Warren T. Reich, was, at the time it was written, a Senior Research Scholar in Medical Ethics at the Kennedy Center for Bioethics, Georgetown University. He went on to be a founding member of the Kennedy Institute of Ethics. He was also founder and director of the Project for the History of Care, and is Distinguished Research Professor of Religion and Ethics in the Georgetown University Theology Department and Professor Emeritus of Bioethics in the Georgetown University School of Medicine. The ethical and legal considerations of young people and their parents using a hospital patient portal: Hospital Ethics Committee members perspectives

Background In April 2019, our hospital transitioned to an electronic patient record system and patient portal (MyGOSH). MyGOSH enables young people aged 12 years or older and their parents to access results, documentation, appointments, and to communicate with their care team. Aims A focus group was conducted to explore the ethical and legal considerations of young people and their parents using a patient portal from the perspective of hospital Ethics Committee members. Participants and research context Members of the hospital Paediatric Bioethics Centre were recruited ( n = 7). Written informed consent was provided by all participants. Research design The focus group discussion was recorded, transcribed verbatim and analysed using thematic analysis. Ethical considerations Health Research Authority Research Ethics Committee (REC) Approval was granted by London Southeast REC (IRAS Project ID: 248793; REC Reference: 18/LO/1945). Results Four themes, and sub-themes, were identified: (1) granting access – complexities of assessing competence/capacity, exclusion from MyGOSH due to non-disclosure of diagnosis, and truth-telling; (2) inequalities resulting in inequities – complexities of inequity in access; (3) responding to change – providing support and managing expectations; (4) engagement, empowerment and understanding – data safety, empowering young people to take ownership of their health data, a one-size fits all system may not meet the needs of all families, and early engagement with young people/parents. Discussion Giving families portal access can increase engagement, empowering them to be more involved in care, yet poses challenges for all. Complexities exist in the assessment for portal access, and supporting and managing user expectations whilst providing an equitable service. Conclusions Ongoing support is required for all stakeholders following patient portal implementation to navigate the ethical and legal complexities, and to promote equity and portal utility for patient benefit.

Herstory as an Important Force in Bioethics.

This tribute celebrates the life and work of Marian Gray Secundy (1938-2002), who was the first director of the National Center for Bioethics in Research and Health Care, a passionate advocate for health equity, a visionary scholar, and a skilled editor and collaborator. Within the tribute are several short remembrances in which individuals describe their association with Secundy to honor her as a leader, friend, scholar, advocate, and teacher in bioethics.

Being Catholic in the Public Square

The nature of the Catholic faith often places practitioners at odds with established order and the specificity of our values may cause us to run afoul of secular sensibilities. What follows is a collection of writings by National Catholic Bioethics Center President, Dr. Joseph Meaney, exploring our place in the public square, the proper way to respond to government driven injustice, and some specific instances in which the current administration has infringed or threatened to infringe upon the conscience and religious rights of not just faithful Catholics, but faithful members of many other religions as well.

Response—Belonging, Interdisciplinarity, and Fragmentation: On the Conditions for a Bioethical Discourse Community

I have been invited to reflect on "Discourse communities and the discourses of experience" a paper co-authored by Little, Jordens, and Sayers and discuss how their analysis of discourse communities has influenced the development of bioethics and consider its influence now and potential effects in the future. Their paper examines the way different discourse communities are shaped by different experiences and desires. The shared language and experiences can provide a sense of belonging and familiarity. These can be positive aspects of a discourse community, but there are also risks restricting the voices and experiences that can be heard and recognized. In this essay I hope to weave together three threads: an analysis of Little, Jordens, and Sayer's research on discourse communities; the narrative interview I conducted with Little about his own experiences in establishing a bioethics centre; and my own experience of the bioethical discourse community established by Little.
