The primary stability of the femoral component in cemented single and twin peg Oxford unicompartmental knee arthroplasty under adverse conditions.

AimsThe cemented Oxford unicompartmental knee arthroplasty (OUKA) features two variants: single and twin peg OUKA. The purpose of this study was to assess the stability of both variants in a worst-case scenario of bone defects and suboptimal cementation.MethodsSingle and twin pegs were implanted randomly allocated in 12 pairs of human fresh-frozen femora. We generated 5° bone defects at the posterior condyle. Relative movement was simulated using a servohydraulic pulser, and analyzed at 70°/115° knee flexion. Relative movement was surveyed at seven points of measurement on implant and bone, using an optic system.ResultsAt the main fixation zone, the twin peg shows less relative movement at 70°/115°. At the transition zone, relative movements are smaller for the single peg for both angles. The single peg shows higher compression at 70° flexion, whereas the twin peg design shows higher compression at 115°. X-displacement is significantly higher for the single peg at 115°.ConclusionBony defects should be avoided in OUKA. Hybrid Oxford unicompartmental knee arthroplasty has lower residual cement extrusion than cemented arthroplasty in treating end-stage unicompartmental knee osteoarthritis

BackgroundHybrid Oxford unicompartmental knee arthroplasty (OUKA) consists of cementless femoral prostheses and cemented tibial prostheses. Although a hybrid OUKA has been used in clinical practice, the clinical outcome has not been reported. The purpose of this study was to compare the short-term clinical outcomes and rate of residual bone cement extrusion between hybrid and cemented prostheses and analyse the possible reasons for differences between outcomes.MethodsA total of 128 knees (118 patients) with end-stage osteoarthritis were included in this study, of which underwent consecutive operations using unicondylar Oxford phase 3 implants from July 2017 and September 2019 in our centre. Follow-up was performed at 6 weeks, 3 and 6 months, 1 year and every year after operation, and complications and changes in the Oxford knee score (OKS) were recorded. The OKS of the two groups was analysed by the generalized estimating equation approach. Prosthesis-based standard fluoroscopy was performed in a timely manner after each operation, and the rate of residual cement extrusion of the two groups was estimated using T-tests and a multivariate regression analysis.ResultsExcluding the cases that lost follow-up, a total of 120 knees (65 in hybrid group and 55 in cemented group) were included in the analysis. There was no statistically significant difference in patient characteristics between the two groups (p > 0.05). The average follow-up time was 23.4 months (and ranged from 12 to 38 months). As of the last follow-up, there were no complications, such as dislocation, fracture, prosthesis loosening and subsidence, but one patient in the cemented group experienced symptoms caused by residual loose cement. Postoperative OKS in both groups improved significantly (p < 0.001). There was no significant difference in the OKS at any point during the follow-up or in the improvement of the OKS between the two groups (p > 0.05). Residual cement was mainly extruded behind the tibial prosthesis. The rate of hybrid periprosthetic residual cement extrusion was significantly lower in the hybrid group than in the cemented group, and the difference was statistically significant (OR = 3.38; p = 0.014).ConclusionsHybrid OUKA is as effective as cemented OUKA in the short term after operation and can significantly reduce the residual cement extrusion rate around the tibial prosthesis.

Lower early revision rates after uncemented Oxford Unicompartmental Knee Arthroplasty (UKA) than cemented Oxford UKA: A meta-analysis

BackgroundUKA has been proved to offer good results in treating patients with unicompartmental knee osteoarthritis (OA). However, there is still a controversy about the better fixation mode in UKA procedure between cemented and uncemented prosthesis. Therefore, this meta-analysis was conducted to compare clinical and radiological outcomes of cemented versus uncemented Oxford UKA. HypothesisThe study surmised that uncemented Oxford UKA was associated with shorter operation time, higher function scores, lower revision rate and less radiolucency than cemented Oxford UKA. MethodsA meta-analysis to compare postoperative outcomes between cemented and uncemented Oxford UKA wsa conducted. The primary outcomes included Oxford knee score (OKS), revision rate, and incidence of radiolucency. The secondary outcomes included operation time, knee society score (KSS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), range of motion (ROM). PubMed, Embase, Web of Science, the Cochrane Library and China national knowledge infrastructure were searched until April, 2020 to identify studies for including. Relevant data were analyzed using RevMan v5.3. ResultsWe identified nine studies involving 901 patients meeting our inclusion criteria. No significant difference of OKS was found in both groups. Compared with cemented group, uncemented Oxford UKA group was associated with lower revision rate (95% CI: 0.90 to 3.73; OR=1.83) and less radiolucent lines (95% CI: 0.79 to 9.52; OR=2.75) after following up for at least 2years. The operation time was significantly shorter by 10.12minutes (95% CI: p<0.00001) in the uncemented group when compared against the cemented group. The KSS, WOMAC, ROM were not significantly different between two groups. ConclusionsUncemented Oxford UKA had lower revision rate, shorter operation time and less radiolucent lines than cemented Oxford UKA did. There is still need for more long follow-up clinical trials with high evidence level to determine which method of fixation is of preferable for Oxford UKA in the future. Level of evidenceI.

The long-term results of cemented Oxford unicompartmental knee arthroplasty: A single-center experience.

This study aims to investigate the long-term clinical outcomes, complications and survival rates of cemented Oxford medial unicompartmental knee arthroplasty (UKA) and to compare the findings with the reported outcomes of UKA and total knee arthroplasty (TKA) in the literature. In the study, 133 knees of 115 patients (16 males, 99 females; mean age 65.5±8.3 years; range, 50 to 88 years) who underwent Oxford medial UKA between May 2007 and August 2013 were followed-up for an average of 126 months. Prosthetic complications and revisions were evaluated. While Kaplan-Meier method was used to evaluate implant survival, Knee Society, Functional Knee Society and visual analog scale scores were used for determining final functional outcomes. Totally 29 prosthetic complications (21.6% of total patients) were observed and the most common one was insert dislocation observed in 17 knees (12.6%). Revision surgeries were performed for all of these patients. Among the 17 patients who underwent insert replacement surgeries, the inserts were stable in eight during the follow-up while insert dislocation reoccurred in nine. Revision surgeries were performed in a total of 21 patients (15.6%). There were statistically significant improvements in final follow-up functional knee scores of patients without complications compared to preoperative scores. According to our findings, while reoperation and early aseptic revision rates of Oxford medial UKA are high compared to the TKA results reported in the literature, functional results are satisfactory for patients with no revision.

The influence of the twin peg design on femoral Interface temperature and maximum load to failure in cemented Oxford unicompartmental knee arthroplasty

BackgroundThe twin peg femoral component was introduced for the cemented Oxford unicondylar knee to increase implant stability. The aim of this experimental study was to investigate the influence of the twin peg design on femoral interface temperature and maximum load to failure in comparison to the single peg design. MethodsIn this experimental study medial Oxford unicompartmental knee arthroplasty was performed in 12 pairs of fresh-frozen human knees. A cemented femoral single peg component was implanted on the one side (group A) and a cemented twin peg component on the other side (group B). Cement interface temperature was continuously monitored during the procedure. Maximum tensile forces of the femoral components were measured by pull-out tests. FindingsMaximum femoral interface temperatures did not reach critical values for heat necrosis of the bone in group A (mean 28.4, SD 1.2 °C) or group B (mean 27.6, SD 0.5 °C). The maximum load to failure was significantly higher in the twin peg group (mean 3628.41, SD 650.92 N) compared to the single peg group (mean 2979, SD 781 N) (P = 0.016). InterpretationOur experiments showed higher load to failure for the twin peg design compared to the single peg design without raising the risk of heat necrosis at the interfacial bone. The twin peg component offers a save alternative to the single peg component in a cadaveric setting.

The Oxford Medial Unicompartmental Knee Arthroplasty: The South African Experience

BackgroundThe Oxford unicompartmental knee arthroplasty (OUKA) is a successful treatment for endstage, symptomatic anteromedial osteoarthritis. This study reports the results of a cohort of consecutive cemented and cementless medial OUKAs from an independent center and aims to answer the following questions: what is the survival of OUKA in the hands of a nondesigner surgeon? Are there any differences in the survival of cementless and cemented OUKA? Are the failure modes any different with the cementless and cemented OUKA? MethodsOne thousand one hundred twenty consecutive OUKAs were implanted in a single center for the recommended indications. Patients were prospectively identified and followed up. Survival was calculated with revision as the end point. ResultsThere were 522 cemented and 598 cementless implants. The mean follow-up was 8.3 years for cemented implants (range 0.5-17, standard deviation [SD] 2.9) and 2.7 years (range 0.5-7, SD 1.8) for cementless implants. The Oxford knee score improved from a preoperative mean of 22 (SD 8.1) to 40 (SD 7.9) at the last follow-up (P < .001). There were 59 failures requiring revision surgery, with a 5.3% cumulative revision rate. The most common reason for failure was progression of osteoarthritis in the lateral compartment, occurred in 26 cases (2.3%). The cumulative 10-year survival was 91% (95% confidence interval 87.3-95.2). ConclusionThe results of this prospective, consecutive case series from the African continent demonstrated that excellent results are achievable with the OUKA in independent centers if the correct indications and surgical technique are used.

Cementless versus cemented Oxford unicompartmental knee arthroplasty: early results of a non-designer user group.

Although fewer tibial radiolucent lines are observed in cementless Oxford unicompartmental knee arthroplasty (UKA) compared with cemented Oxford UKA, an independent comparative study on this topic is lacking. In this multicentre retrospective study, a cohort of 60 consecutive cases of cementless Oxford UKA is compared with a cohort of 60 consecutive cases of cemented Oxford UKA. Radiolucent lines, survival, perioperative data and clinical results were compared. No complete tibial radiolucent lines were observed in either group. Seventeen per cent of partial tibial radiolucent lines were observed in the cementless group versus 21% in the cemented group (n.s.). The percentage of tibial radiolucent zones was 4 versus 9%, respectively (p=0.036). Survival rates were 90% at 34months for the cementless group and 84% at 54months for the cemented group (n.s.). Mean operation time was 10min shorter in the cementless group (p<0.001), and clinical results were not significantly different. Although no significant differences in radiolucent lines were found between both groups, they appear to be more common in the cemented group. This confirms previous results from reports by prosthesis designers. The presence of radiolucent lines after cemented Oxford UKA does not correlate with clinical outcome or survival. III.

The influence of the femoral force application point on tibial cementing pressure in cemented UKA: an experimental study

Aseptic loosening is the major cause for implant failure in cemented unicompartmental knee arthroplasty (UKA). Central positioning of the femoral pressure during the tibial cementation process is recommended to achieve equal pressure and a good cementation result. The aim of this study was to verify the central position of the femoral force application point (FFAP) at 45° flexion of the knee and to investigate the influence of ligament tension and cement penetration pressure (CPP) for UKA. Cemented Oxford UKAs were performed in 24 human legs. CPP and ligament tension forces (LTF) were measured. The FFAP was measured in a standardised manner in relation to the tibial implant length on lateral digital X-rays. The FFAP at 45° of knee flexion is located at 53.5 % and is not significantly different from the FFAP at 0° (p = 0.768). The CPP shows mean values at the anterior portion of 13.97 kPa (SD 16.11), at the implant keel of 24.34 kPa (SD 25.21) and at the posterior portion of 36.58 kPa (SD 26.51). The LTF shows a mean value of 194.35 N (SD 83.77). The central position of the FFAP for the investigated cemented UKA with single radius femoral component at 45° flexion of the knee could be confirmed. A flexion angle of <45° does not influence the position of the FFAP significantly. More than 45° of flexion should be avoided because the FFAP shifts backwards significantly and may cause increased pressures posteriorly and therefore tilting of the component occurs during the cementation process.

Femoral fixation pattern in cemented Oxford unicompartmental knee arthroplasty — An experimental cadaver study

Femoral component loosening is a rare but serious complication in cemented Oxford unicompartmental knee arthroplasty (OUKA). In a cadaver study, OUKA was performed in 24 knees to evaluate the femoral fixation pattern. Due to the geometry of bone and implant, three different zones were identified. Complete cement mantles and good interdigitation were found in the spherical part of the implant (zone 1) and around the peg (zone 3), which implies that these are most important for implant fixation. The posterior plane facet (zone 2) is the weak point of the interface due to incompleteness of the cement mantle and a lack of interdigitation. This study suggests that the cancellous bone in zone 2 and the drill holes in sclerotic bone areas should be filled with cement and pressurised prior to component seating.

Periprosthetic tibial fractures in unicompartmental knee arthroplasty as a function of extended sagittal saw cuts: An experimental study

Periprosthetic tibial plateau fractures (TPF) are rare but represent a serious complication of unicompartmental knee arthroplasty (UKA). As TPFs usually occur perioperatively, these can be associated with extended sagittal saw cuts during surgery. The aim of the study was to evaluate TPF as a function of extended sagittal saw cuts. The hypothesis was that extended sagittal saw cuts reduce the loading capacity of the tibial plateau and increase the risk of periprosthetic TPF. In a randomised study, standardised cemented Oxford UKA tibial component implantation was performed in six matched, paired fresh-frozen tibiae. In group A, a regular preparation of the tibial plateau was performed, whereas in group B a standardised extended sagittal saw cut was made at the dorsal cortex of the tibia. All tibiae were fractured under standardised conditions and fracture patterns and fracture loads were analysed. In group A, tibiae fractured with a mean load of F max = 3.9 (2.3–8.5) kN, whereas in group B fractures occurred at a mean load of F max = 2.6 (1.1–5.0) kN. The difference was statistically significant ( p < 0.05). Extended sagittal saw cuts in UKA weaken the tibial bone structure. Our results show that descendent extended sagittal saw cuts of 10° reduce fracture loads by about 30%. Surgeons should be aware of the potential pitfalls of an extended sagittal saw cut, as this can lead to reduced loading capacity of the tibial plateau and increase the risk of periprosthetic TPF.

Pulsed lavage reduces the incidence of radiolucent lines under the tibial tray of Oxford unicompartmental knee arthroplasty

The aim of the study was to determine whether the incidence of radiolucencies can be reduced using pulsed lavage before cementing the tibia in unicompartmental knee arthroplasty (UKA). We prospectively studied a consecutive series of 112 cemented Oxford UKA in 100 patients in two centres. In group A (n = 56) pulsed lavage and in group B (n = 56) conventional syringe lavage was used to clean the cancellous bone. The same standardised cementing technique was applied in all cases. At a minimum follow-up of one year patients were evaluated clinically and screened radiographs were obtained. The cement bone interface under the tibial plateau was divided into four zones and evaluated for the presence of radiolucent lines. All radiographs were evaluated (n = 112), and radiolucencies in all four zones were found in two cases in group A (4%) and in 12 cases in group B (22%) (p = 0.0149). Cement penetration showed a median of 2.6 mm (group A) and 1.5 mm (group B) (p < 0.0001). We recommend the routine use of pulsed lavage in Oxford UKA to reduce the incidence of radiolucency and to improve long-term fixation.
