Patients with Invasive Tumors and eNOS Gene Polymorphisms with Subarachnoid Hemorrhage Tend to Have Poorer Prognosis.

Context Endothelial nitric oxide synthase ( eNOS) gene polymorphisms are found to predict predisposition to aneurysmal rupture and development of vasospasm in a patient of subarachnoid hemorrhage (SAH). eNOS gene polymorphisms are also found to predict invasiveness of malignant cells. Studies are not available in literature to describe the effect of eNOS gene polymorphisms and correlation between aneurysm and carcinoma. This study aims to investigate whether positive cancer history influences clinical outcome following SAH and eNOS gene polymorphisms. Materials and Methods The eNOS gene polymorphisms were analyzed in seven consecutive patients (mean age, 52.28 ± 20 years) with a diagnosis of invasive systemic tumors from 2011 to 2017. The eNOS 4a/4b eNOS -786T> eNOS 894G > T polymorphisms of the eNOS gene were determined by polymerase chain reaction and restriction fragment length polymorphism. Results Seven patients of aneurysmal SAH in association with malignancies were studied for eNOS polymorphisms expression and outcome. Three patients had carcinoma cervix: one patient of carcinoma breast and one each of transitional cell carcinoma of urinary bladder, spindle cell carcinoma of left kidney, and untreated patient of atypical pituitary (adenoma). A genotype study of eNOS gene polymorphisms in these patients shows common polymorphisms are involved in the determination of disease progression in malignancies and aneurysmal SAH. Conclusion Patients who expressed 4ab, eNOS -786T > TT/CC/TC, eNOS 894G > T GG/GT polymorphisms did better than patients who expressed only 4bb, though both were associated with poor prognosis.

Eosinophilic vacuolated tumour of the kidney

Eosinophilic vacuolated tumour (EVT) is one of the emerging entities of renal cell carcinoma. We present a case which we believe is second ever reported case of eosinophilic and vacuolated renal tumour (EVT) arising in setting of tuberous sclerosis syndrome. A 44-year-old male patient with past history (2017) of unclassified renal cell carcinoma of left kidney and known tuberous sclerosis syndrome receiving ongoing tyrosine kinase inhibitor therapy and undergoing radiological surveillance was found to have a 3 cm right renal mass.

Thyroid malignancies are commonest endocrine malignancy. It can be primary, can be secondary also.Though secondary thyroid malignancies are rare it happens sporadically from kidney, skin, breast andhead-neck tumor. Here we described a case of metastatic papillary thyroid carcinoma. This case wasdiagnosed as a case of papillary cell carcinoma of left kidney 18 months back, underwent radical nephrectomyand received a course of Tyrosine Kinase Inhibitor also and remain symptom free for one year. Laparoscopic management renal cell carcinoma in left kidney with tumor caval thrombus

Background. Treatment of level II-III inferior vena cava (IVC) tumor thrombus for left renal cell carcinoma is among the most challenging open urologic oncologic surgeries with 38 % complications and 4—10 % mortality rate. There are increasing numbers of centers using a right side laparoscopic radical nephrectomy with thrombectomy at last years but only few reports about laparoscopic management of IVC thrombus from left kidney. The study objective is to demonstrate reproducibility and relative safety of laparoscopic radical nephrectomy with thrombectomy of IVC tumor thrombus originating from the left kidney. Materials and methods . We describe the method and present the initial series of full laparoscopic level II-III IVC thrombectomy in three patients with tumor in left kidney. One patient had been diagnosed distant metastases before operation, the another — pancreas involvement in the tumor process. The follow up time consist 4—26 months after surgery. Results . All procedures completed without conversion to open surgery. Tumor sizes were 5—16 cm, length of thrombus in IVC — 2.4—7.0 cm. Volume of blood loss ranged from 300 to 2500 ml. One patient received blood transfusion after surgery. One patient have died because of distant metastases 5 months after surgery. Two others were alive at 4 and 26 months follow-up without signs of progression. Conclusion. Laparoscopic IVC tumor thrombectomy for level II-III thrombi in cases of left kidney cancer is feasible, safe. This kind of pro­cedure doesn’t lead to serious perioperative complications and can gain a quicker recovery after surgery. It needs more operations and longer follow-up for evaluation oncological efficiency.

Metastatic renal cell carcinoma in the head and neck region

<p>We describe 3 rare cases of metastatic renal cell carcinoma in the head and neck region. Our first case was a 72 years old male presented with profuse bleeding from the left ear. On examination, proliferative, pink, friable mass was present in the left external auditory canal. On eliciting the detailed history, it was found that patient had renal cell carcinoma in the left kidney four years back and underwent left radical nephrectomy. Patient underwent left lateral temporal bone resection with cul-de-sac closure. Histopathological examination of the specimen showed metastatic renal cell carcinoma. Our second case was a 64 years old male presented with complaints of growth in the oral cavity of 1 month duration. On examination, 5×3 cms proliferative growth was present in the hard palate. Biopsy from the growth in the hard palate showed metastatic renal cell carcinoma. Patient gave history of renal cell carcinoma in the past for which he did not receive treatment. Our third case was a 45 years old male who presented to us with a diffuse swelling in the left temporal region of 2 months duration. On examination, 3×2 cms swelling was present in the left temporal region. Patient had history of renal cell carcinoma of left kidney and underwent left nephrectomy one year prior to the presentation of the left temporal swelling. Ultrasonography guided biopsy of the left temporal region showed metastatic renal cell carcinoma.</p>

Diagnosis and treatment of adrenal medullary hyperplasia: experience from 12 cases.

Objective. To dissect the characteristics of adrenal medullary hyperplasia (AMH) and share our experience of diagnosis and treatment of AMH. Methods. From 1999 to 2013, 12 cases of AMH have been pathologically diagnosed after operation in our hospital. The clinical characteristics, process of diagnosis, treatment, and prognosis during follow-up of all patients are summarized retrospectively. Results. Four cases were trended to be AMH and 6 cases were trended to be pheochromocytoma before operation; moreover, the other two patients were diagnosed accidentally. All patients, except for the patient with mucinous tubular and spindle cell carcinoma of left kidney by open surgery, experienced a smooth laparoscopic adrenalectomy, including 2 with radical nephrectomy, 10 of which experienced unilateral adrenalectomy, 1 was bilaterally partial adrenalectomy, and the remaining one was unilaterally complete removal and then 2/3 partially contralateral excision. After a medium follow-up of 6.5 years, it demonstrated a satisfactory outcome of 8 cured patients and 4 symptomatic improved patients. Conclusions. AMH presents a mimicking morphology and clinical manifestation with pheochromocytoma. Surgery could be the only effective choice for the treatment of AMH and showed a preferable prognosis after a quite long follow-up.

Adult Wilms tumour: A rare entity

WILMS TUMOR (nephroblastoma) in adults is rare. Only 3% of Wilms tumors are reported in adults. Most adult patients are diagnosed unexpectedly following nephrectomy for presumed renal cell carcinoma. We present a 40 yr old female patient who came with complaints of lump and pain in left lumbar and hypochondrium region. After examination and workup, provisionally diagnosed as renal cell carcinoma of left kidney. Radical nephrectomy was done and histopathology revealed Wilms tumor.

Bleeding Gastric Polyp: A Rare Manifestation of Renal Cell Carcinoma 7 years after Nephrectomy

Purpose: Renal cell carcinoma can notoriously recur in unusual locations many years after initial cure and presentation as upper GI bleeding from a gastric polyp is rare. Case: An 83 year old diabetic woman was evaluated for one week of exertional dyspnea, fatigue and melena. She had history of clear cell type renal cell carcinoma of left kidney and underwent left nephrectomy seven years ago. Six months before presentation, she was evaluated for worsening iron deficiency anemia. An EGD showed three non-bleeding gastric ulcers and colonoscopy was normal. Patient was started on twice daily proton pump inhibitor. On current presentation, physical examination was within normal limits except melena in rectum. Laboratory examination showed: hemoglobin 8.6 g/dL (range= 12-15) and GFR 40 ml/min/1.73 m2 (normal= >59). She required multiple transfusions with packed red cells. An EGD was performed which revealed 3 ulcerated semi-pedunculated bleeding polyps in fundus and melanic patches in the antral region. All polyps were removed without complications. Histopathology revealed metastatic clear cell carcinoma. Melanic patches were not biopsied and there was no cutaneous or oral pigmentation. CT chest revealed multiple bilateral lung nodules biopsy of which confirmed late recurrence of renal cell carcinoma. Conclusion: High index of suspicion should be maintained for GI metastasis of renal cell carcinoma in patients presenting with GI bleeding and history of this malignancy.Figure: GD showing bleeding gastric polyp and antral melanosis.

多発皮膚転移による発見された腎癌の１例

A 74-year-old female with skin metastasis of renal cell carcinoma is described.She noticed her cutaneous lesion on her left lower abdomen about three years before visiting our dermatological outpatient clinic. The lesion gradually enlarged and multiple nodule developed on the other parts of the body.The biopsy was taken from the lesion on the right arm. On microscopic findings, the metastasis of renal cell carcinoma was suggested. An examination of whole body was performed and renal cell carcinoma of left kidney was discovered. Systemic and multi-organ involvement were also observed.This case is rare because the patient had a large number of metastatic skin lesions up to 62. In addition, she lived at least 3 years after the metastasis of the skin. It is relatively a long period for the patient with metastatic renal cell carcinoma.
