Does Consciousness Level Determine the Timing of Successful Removal of Urinary Catheters at Neurosurgery Intensive Care Unit-Implication to Nosocomial Infection Prevention

Objective This study aimed to investigate whether the level of consciousness can be used as an indicator to determine the successful removal of urinary catheters in post-stroke patients admitted to the neurosurgical intensive care unit (NICU). Methods The study included 97 patients who had urinary catheters in NICU and were divided into three groups based on their level of consciousness. Our analyses was performed by various statistical methods, including the chi-square test, one-way ANOVA, univariate and multivariate regression. Results The overall success rate of catheter removal was found to be 62.9%. On average, the catheters were retained for 6.3 ± 3.6 days. The presence of diabetes mellitus was identified as the only factor that significantly influenced the successful removal of catheters (Adjusted Odds Ratio = 0.236, p = 0.035). No significant differences were observed among patients with different degrees of consciousness impairment. Implementing a nursing-driven circle strategy and bladder sonography scan protocol significantly could decrease the catheter-associated urinary infection (CAUTI) rate from 10.25/1,000 catheter days to 6.69/1,000 catheter days. Conclusions Based on our findings, we recommend removing urinary catheters five days after initial placement for patients with a Glasgow Coma Scale (GCS) score greater than eight and for non-intubated patients, as well as for intubated patients and aphasic patients in the NICU with a GCS score greater than 4. Active Surveillance and Contact Precautions for Preventing Methicillin-Resistant Staphylococcus aureus Healthcare-Associated Infections During the COVID-19 Pandemic.

Statistically significant decreases in methicillin-resistant Staphylococcus aureus (MRSA) healthcare-associated infections (HAIs) occurred in Veterans Affairs (VA) hospitals from 2007 to 2019 using a national policy of active surveillance (AS) for facility admissions and contact precautions for MRSA colonized (CPC) or infected (CPI) patients, but the impact of suspending these measures to free up laboratory resources for testing and conserve personal protective equipment for coronavirus disease 2019 (COVID-19) on MRSA HAI rates is not known. From July 2020 to June 2022 all 123 acute care VA hospitals nationwide were given the rolling option to suspend (or re-initiate) any combination of AS, CPC, or CPI each month, and MRSA HAIs in intensive care units (ICUs) and non-ICUs were tracked. There were 917 591 admissions, 5 225 174 patient-days, and 568 MRSA HAIs. The MRSA HAI rate/1000 patient-days in ICUs was 0.20 (95% confidence interval [CI], .15-.26) for facilities practicing "AS + CPC + CPI" compared to 0.65 (95% CI, .41-.98; P < .001) for those not practicing any of these strategies, and in non-ICUs was 0.07 (95% CI, .05-.08) and 0.12 (95% CI, .08-.19; P = .01) for the respective policies. Accounting for monthly COVID-19 facility admissions using a negative binomial regression model did not change the relationships between facility policy and MRSA HAI rates. There was no significant difference in monthly facility urinary catheter-associated infection rates, a non-equivalent dependent variable, in the policy categories in either ICUs or non-ICUs. Facility removal of MRSA prevention practices was associated with higher rates of MRSA HAIs in ICUs and non-ICUs.

Active surveillance and contact precautions for preventing MRSA healthcare-associated infections during the COVID-19 pandemic

Background: Statistically significant decreases in methicillin-resistant Staphylococcus aureus (MRSA) healthcare-associated infections (HAIs) occurred in Veterans Health Administration (VA) facilities from 2007 to 2019 using active surveillance for facility admissions and contact precautions for patients colonized (CPC) or infected (CPI) with MRSA, but the value of these interventions is controversial. Objective: To determine the impact of active surveillance, CPC, and CPI on prevention MRSA HAIs, we conducted a prospective cohort study between July 2020 and June 2022 in all 123 acute-care VA medical facilities. In April 2020, all facilities were given the option to suspend any combination of active surveillance, CPC, or CPI to free up laboratory resources for COVID-19 testing and conserve personal protective equipment. We measured MRSA HAIs (cases per 1,000 patient days) in intensive care units (ICUs) and non-ICUs by the infection control policy. Results: During the analysis period, there were 917,591 admissions, 5,225,174 patient days, and 568 MRSA HAIs. Only 20% of facilities continued all 3 MRSA infection control measures in July 2020, but this rate increased to 57% by June 2022. The MRSA HAI rate for all infection sites in non-ICUs was 0.07 (95% CI, 0.05–0.08) for facilities practicing active surveillance plus CPC plus CPI compared to 0.12 (95% CI, 0.08–0.19; P = .01) for those not practicing any of these strategies, and in ICUs the MRSA HAI rates were 0.20 (95% CI, 0.15–0.26) and 0.65 (95% CI, 0.41–0.98; P &lt; .001) for the respective policies. Similar differences were seen when the analyses were restricted to MRSA bloodstream HAIs. Accounting for monthly COVID-19 admissions to facilities over the analysis period using a negative binomial regression model did not change the relationships between facility policy and MRSA HAI rates in the ICUs or non-ICUs. Catheter-Associated Urinary Infections and Consequences of Using Coated versus Non-Coated Urethral Catheters-Outcomes of a Systematic Review and Meta-Analysis of Randomized Trials.

Coated urethral catheters were introduced in clinical practice to reduce the risk of catheter-acquired urinary tract infection (CAUTI). We aimed to systematically review the incidence of CAUTI and adverse effects in randomized clinical trials of patients requiring indwelling bladder catheterization by comparing coated vs. non-coated catheters. This review was performed according to the 2020 PRISMA framework. The incidence of CAUTI and catheter-related adverse events was evaluated using the Cochran–Mantel–Haenszel method with a random-effects model and reported as the risk ratio (RR), 95% CI, and p-values. Significance was set at p < 0.05 and a 95% CI. Twelve studies including 36,783 patients were included for meta-analysis. There was no significant difference in the CAUTI rate between coated and non-coated catheters (RR 0.87 95% CI 0.75–1.00, p = 0.06). Subgroup analysis demonstrated that the risk of CAUTI was significantly lower in the coated group compared with the non-coated group among patients requiring long-term catheterization (>14 days) (RR 0.82 95% CI 0.68–0.99, p = 0.04). There was no difference between the two groups in the incidence of the need for catheter exchange or the incidence of lower urinary tract symptoms after catheter removal. The benefit of coated catheters in reducing CAUTI risk among patients requiring long-term catheterization should be balanced against the increased direct costs to health care systems when compared to non-coated catheters.

How to Prevent Catheter-Associated Urinary Tract Infections: A Reappraisal of Vico's Theory-Is History Repeating Itself?

New, contextualized modern solutions must be found to solve the dilemma of catheter-associated urinary infection (CAUTI) in long-term care settings. In this paper, we describe the etiology, risk factors, and complications of CAUTI, explore different preventive strategies proposed in literature from the past to the present, and offer new insights on therapeutic opportunities. A care bundle to prevent CAUTI mainly consists of multiple interventions to improve clinical indications, identifying a timeline for catheter removal, or whether any alternatives may be offered in elderly and frail patients suffering from chronic urinary retention and/or untreatable urinary incontinence. Among the various approaches used to prevent CAUTI, specific urinary catheter coatings according to their antifouling and/or biocidal properties have been widely investigated. Nonetheless, an ideal catheter offering holistic antimicrobial effectiveness is still far from being available. After pioneering research in favor of bladder irrigations or endovesical instillations was initially published more than 50 years ago, only recently has it been made clear that evidence supporting their use to treat symptomatic CAUTI and prevent complications is needed.

Inhibition of bacterial adherence to biomaterials by coating antimicrobial peptides with anionic surfactant

Medical devices are widely used in modern medicine, but their utilities are often limited by the biofilm formation of bacteria that are tolerant to most antibiotics. In this report, antimicrobial peptides (AMPs) were coated onto biomaterials by the aid of surfactant through hydrophobic interactions. To increase the coating efficiency, stability of AMPs in body fluids and spectrum of antimicrobial activity, pairs of AMPs were coated simultaneously onto various substrates, such as silicone, polyurethane and titanium, which are commonly used components of biomedical devices. These coated AMPs exhibited very low cytotoxicity and hemolytic activities because they were gradually released into urine or serum. The AMP pairs, such as T9W + SAAP159 and T9W + RRIKA, coated onto the silicone discs were able to inhibit in vitro bacterial adherence in urine. Most importantly, AMP pairs coated onto the silicone tubing by surfactant SDBS could prevent bacterial adherence to mouse bladder and the silicone tubing implanted within it. These results provide a promising approach towards circumventing urinary catheter-associated infections caused by bacterial adherence.

Microbiological profile of catheter associated urinary tract infection in ICUs of a tertiary care hospital Bhubaneswar, Odisha, India

Among nosocomial infections catheter associated urinary infection (CA-UTI) is one of the most common infection. Uro- pathogens isolated from CAUTI were more multi-drug resistant than from community acquired urinary tract infection (UTI).To isolate micro-organisms responsible for CA-UTI in ICUs, to find out antibiotic sensitivity pattern of the isolates and to know the Impact of CAUTI care bundle on reducing CAUTI rate.A retrospective study was conducted from September 2017 to August 2018, urine samples were collected from 300 catheerised patients which were processed microbiologically and antimicrobial sensitivity test was performed. Out of 300 catheterised paients , 76 patients developed CAUTI. So the incidence rate of CAUTI is 21.7%. A sum total of 38,067 catheter days were obtained in the study period from the month of September 2017 to August 2018. CAUTI rate was found to be 1.9 per 1000 catheter days over a period of 1 yearOut of 76 total isolates 56 were Gram negative and 20 were Gram positive bacteria. Gram negative bacteria included Escherichia coli 19(25%), followed by Klebsiella 14(19%), Proteus8 (11%) Pseudomomas 6(8%), Acinetobacter 4(8%). Among gram positive Enterococcus species is 17(22%) followed by staphylococcus spp.03 (4%). Enterobacteriaceae showed high resistant to commonly used antimicrobials like Gentamycin, Ceftriaxone, Ofloxacin, Ciprofloxacin but were highly sensitive to Amikacin, ceftazidime, pipercillin Tazobactum, Imepenem, Meropenem. Enterococcus and staphylococcus were sensitive to Tigecyclin , Vancomycin, Teicoplanin and linezolid.The most common organism responsible for CAUTI is Escherichia coli followed by Klebsiella spp. and Enerococcus spp. Members of enterobacteriaceae are highly sensitive to Amikacin, ceftazidime, pipercillin Tazobactum, Imepenem, Meropenem. Enterococcus and staphylococcus are sensitive to Tigecyclin, Vancomycin, Teicoplanin and linezolid. Strict insertion and maintainance CAUTI care bundle reduces CAUTI rate. Prevention of infections attributable to these devices should be an important goal of health-care infection prevention

Urinary tract infections are the most common type of health care-associated infection, and greater than 75% of them are attributed to an indwelling urinary catheter. A catheter-associated urinary infection may lead to a longer hospital length of stay by as many as 4 days. A new patient care standard requiring twice-daily chlorhexidine cleansing from umbilicus to knees was implemented on all patients of the pilot unit with a urinary catheter. This same technique was used after a patient with a urinary catheter had an incontinent bowel movement. The 9-month average catheter-associated urinary infection rate decreased from 3.06/1000 urinary catheter days to 0.46/1000 urinary catheter days after implementation of the new standard. The use of chlorhexidine for routine urinary catheter care and after bowel movements from umbilicus to knees for patients with urinary catheters may significantly decrease catheter-associated urinary tract infections when compared with the standard of care using soap and water. Standards for Quality Improvement Reporting Excellence guidelines were used in reporting these data.

Prevention of urinary catheter-associated infections by coating antimicrobial peptides from crowberry endophytes

Urinary catheters are extensively used in hospitals, being responsible for about 75% of hospital-acquired infections. In this work, a de novo designed antimicrobial peptide (AMP) Chain201D was studied in the context of urinary catheter-associated infections. Chain201D showed excellent antimicrobial activity against relevant ATCC strains and clinical isolates of bacteria and yeast and demonstrated high stability in a wide range of temperatures, pH and salt concentrations. Moreover, the bactericidal activity of Chain201D immobilized on a model surface was studied against Escherichia coli (E. coli) and Staphylococcus aureus (S. aureus), some of the most prevalent strains found in urinary catheter-associated infections. Chain201D was successfully tethered to ((1-mercapto-11-undecyl)-(tetra(ethylene glycol) (EG4)) terminated self-assembled monolayers (SAMs), (EG4-SAMs), activated by 1,1′-Carbonyldiimidazole (CDI) at different concentrations. Chain201D surfaces can bind and kill by contact a high percentage of adherent bacteria. These achievements are obtained without any peptide modification (for chemoselective conjugation) and without the use of a spacer. Moreover, increased amounts of immobilized AMP lead to higher numbers of adhered/dead bacteria, revealing a concentration-dependent behaviour and demonstrating that Chain201D has excellent potential for developing antimicrobial urinary catheters.

Effect of clean water perineum nursing on prevention of urinary tract urinary catheter associated infection

Objective To observe the effect of clean water perineum nursing in preventing urinary catheter-associated urinary tract infection. Methods From October 2015 to October 2016, 160 patients with indwelling urinary catheter were selected from the first affiliated Hospital of Shantou University Medical College, ICU and neurosurgery ICU. According to the order of admission, the patients were randomly divided into experimental group and control group. A total of 80 patients in the experimental group received clean water perineal nursing, while 80 patients in the control group received routine perineum nursing with 0.5% iodophor. The incidence of urinary tract infection was compared between the two groups on day 3rd, 7th and14th after indwelling urethral catheter. Results In the control group, the incidence of urinary tract infection on day 3rd, 7th and14th was 0, 1.3%(1/80) and 5.0%(4/80), respectively. In the experimental group, the incidence of urinary tract infection on day 3, 7 and 14 was 0, 2.5% (2/80) and 3.8%(3/80). The difference was not statistically significant (χ2=0.340, 0.149, P> 0.05). Conclusions Using clean water perineum nursing does not increase the incidence of urinary tract infection. This method is simple, effective, and has no adverse reactions. It is worth popularizing in clinic because it can reduce the consumption of iodophor disinfectant and reduce the workload of nurses. Catheter-Associated Bacteria Urinary Tract Infection and Antibiotic Susceptibility Pattern in a Tertiary Hospital, in Ghana

Background: This study seeks to identify the prevalence of catheter associated urinary infection and the type of bacteria that are associated with this infection, as well as the antibiotic susceptibility patterns of the organisms isolated. This would guide the choice of antibiotics when there is catheter associated urinary tract infection. Method: From 1 November 2015-31 April 2016 a cross-sectional study was conducted among patients with urinary catheter in-situ. Urine samples collected were processed and cultured on CLED agar plates. Pure colonies of isolated organism were Gram and Biochemically characterized. A disc diffusion antibiotic susceptibility determined by Kirby-Bauer disc diffusion method was performed on each uropathogen isolated. Data obtained was cleaned, analyzed and presented. Result: There were 122 study subjects of which, 73 (59.8%) were males and 49 (40.2%) were females. Their median age was 42.5 (range 33 - 65) years. Significant bacterial growth was obtained in 88 (72.1%) of the urine specimen cultured of which males constituted 48 (54.5%) and females 40 (45.5%). The most prevalent uropathogens isolated were Escherichia coli 41 (46.6%), Klebsiella spp. 18 (20.6%), Pseudomonas aeruginosa 10 (11.4%), Enterobacter spp. 6 (6.8%) and Staphylococcus aureus 5 (5.8%). Bacterial isolates showed some susceptibility to Amikacin 73 (83.0%), Levofloxacin 34 (38.6%) and Ciprofloxacin 26 (29.5%) respectively. The uropathogens were least susceptible to Gentamicin 3 (3.4%), Ampicillin 3 (3.4%) and Cefuroxime 1 (1.1%) respectively. Conclusion: Catheter associated bacterial urinary tract infection (CABUTI) is prevalent at the Tamale Teaching Hospital. Micro bacterial isolates demonstrated substantial decrease in susceptibility to antibiotics commonly used. Understanding the local antibiotic susceptibility pattern could guide the choice of antibiotics used in treating CABUTI.

2109. Reducing Catheter-associated Urinary Infections (CAUTI) in the Intensive Care Unit (ICU): Changing the Culture of Culturing

BackgroundCAUTIs are one of the most common preventable adverse events in hospitalized patients. Prior to the start of this intervention, CAUTI rates in our ICU’s throughout our multicenter acute care system were significantly above the national average. Our hypothesis was that we could decrease CAUTI’s by daily review of the urine culture orders for patients with a Foley catheter.MethodsWe implemented guidelines for appropriate ordering of urine culture for patients with Foley catheters in 2014. The culture was deemed appropriate if the patient had a fever >100.4°F within 48 hours or leukocytosis with no other identifiable source of infection and has one or more of the following: costovertebral angle/flank pain, suprapubic pain, increase in urinary frequency; urgency, frequency or dysuria after catheter removal, acute mental status change, worsening of clinical status. The ordering prescriber was called to cancel the order that did not meet the urine culture ordering guideline. If the ordering prescriber questioned the guidelines, Epidemiologists intervened to explain the rationale. This was a prospective, observational study. Chi-squared analysis was used to compare the reduction of CAUTIs.ResultsThe data showed sustained improvement. Compared with the 2012 baseline rate of 4.28 cases per 1,000 device days, the 2013 rate was 2.70 (P = 0.085), the 2014 rate was 1.38 (P = 0.00046), the 2015 rate was 0.73 (P < 0.0001), and the 2016 rate was 0.63 (P < 0.0001).ConclusionWe found that using guidelines combined with an Infection Preventionist review to determine the appropriateness of urine cultures was associated with a significant reduction in the rate of ICU CAUTI’s. Real-time culture order review is a sustainable process that has continued the success of our CAUTI reduction program.Disclosures All authors: No reported disclosures.

Catheter-Associated Urinary Tract Infections in a Burn Unit: Epidemiological Study

Introduction: Besides burn wound infections, burned patients are also more susceptible to other types of nosocomial infections. Catheter-associated urinary tract infections (CA-UTI) are one of the most common infections in this context, responsible for high morbidity, increased hospital stay and associated costs. The aim of this study was to characterize catheter-associated urinary tract infections in hospitalized burn patients and evaluate the frequency of microbiologic agents responsible for these infections.&#x0D; Material and Methods: Retrospective study, performed in a Burn Center (Coimbra Burns Unit) of a University Hospital (Centro Hospitalar e Universitário de Coimbra, Portugal – CHUC), based in the clinical data and urine cultures of burned patients who have performed at least once this exam between 1 January 2010 and 31 December 2014. Different variables such as date of infection, general characteristics of the population and the responsible pathogen were analyzed. Infections were further categorized taking into account the existence of previous episodes of CA-UTI, thereby defining primary infection, re- infection, relapse and over-infection.&#x0D; Results: Between January 2010 and December 2014, 213 CA-UTI were diagnosed in 143 patients. The most common uropathogens were E. coli (27.2%), Enterococcus faecalis (20.2%), Pseudomonas spp. (13.1%), Candida spp. (12.1%), Klebsiella spp. (10.8%) and Acinetobacter baumannii (9.9%). The most common microorganisms varied significantly depending on the gender of the patient. The CA-UTI analyzed corresponded to 143 primary infections, 44 reinfections, 17 relapses and nine over-infections. Relapse corresponded to 11% of infections in males and 5.7% in females and was significantly more frequent in infections due to Acinetobacter baumannii.&#x0D; Discussion/Conclusion: Catheter-associated urinary infections are common in intensive care units, particularly at Burn Units. The most common pathogens identified were similar to those reported in the literature. Pathogens responsible for polymicrobial infections were similar to those in monomicrobial infections, probably due to the short-term nature of urinary catheterization. Infections by Acinetobacter baumannii showed high susceptibility to relapse, which is probably related to its multi-drug resistance, common in this pathogen. The high relapse rate detected in males is probably related to the greater frequency of Acinetobacter baumannii infections in this gender. Candiduria was more frequent in the context of reinfection and over-infection, probably due to disruption of bacterial flora secondary to previous systemic antibiotics.

Using A Criteria-Based Reminder To Reduce Use Of Indwelling Urinary Catheters And Decrease Urinary Tract Infections

Duration of indwelling urinary catheterization is an important risk factor for urinary tract infection. To determine whether a reminder approach reduces the use of urinary catheters and the incidence of catheter-associated urinary tract infections. A randomized control trial was performed in 2 respiratory intensive care units in a 2990-bed tertiary referral medical center. Patients who had urinary catheters in place for more than 2 days from April through November 2008 were randomly assigned to either the intervention group (use of a criteria-based reminder to remove the catheter) or the control group (no reminder). A total of 278 patients were recruited. Utilization rate of indwelling urinary catheters was decreased by 22% in the intervention group compared with the control group (relative risk, 0.78; 95% CI, 0.76-0.80; P < .001). The intervention significantly shortened the median duration of catheterization (7 days vs 11 days for the control group; P < .001). The success rate for removing the catheters in the intervention group by day 7 was 88%. The reminder intervention reduced the incidence of catheter-associated infections by 48% (relative risk, 0.52; 95% CI, 0.32-0.86; P = .009) in the intervention group compared with the control group. Use of a criteria-based reminder to remove indwelling urinary catheters can diminish the use of urinary catheterization and reduce the likelihood of catheter-associated urinary infections. This reminder approach can prevent catheter-associated urinary infections, and its use should be strongly considered as a way to enhance the safety of patients.

Prevention and Treatment of Urinary Catheter-Associated Infections

Catheter-associated urinary tract infections (CA-UTIs) are the most common nosocomial infection worldwide. Prolonged catheterization is the most important risk factor for CA-UTIs. As is the case with all device-related infections, the biofilm plays a central role in the pathogenesis of CA-UTIs. The diagnosis is often difficult, given the lack of good diagnostic tests. The most effective way to prevent infection is to limit catheter use and discontinue the catheter when no longer needed. Catheter removal or exchange is also useful in management. This review summarizes and analyzes the results of published studies of CA-UTIs and assesses the role of prevention approaches and management strategies.

Objective The prevention effect of CAUTI by long-acting antibacterial material JUC was evaluated by meta analysis.Methods China digital hospital library full-text database,Chinese biomedical literature CD database,China hosptial knowledge database (CHKD) in China national knowledge internet (CNKI),VIP information Chinese science and technology periodical database were retrieved to fully collect the literature from 1995 to 2012 regarding JUC's prevention effect of CAUTI.The inclusion and exclusion standard of literature was made,and the effect index was made from the literature.Two researchers collected the literatures independently,and the included literatures were meta-analyzed by Review Manager 4.2 software,and the risk difference of infection rate and 95％ CI were calculated in both the treatment group and control group.Those literatures which could not be meta-analyzed were performed descriptive analysis to evaluate the efficacy of JUC objectively.Results 20 related articles were retrieved,including 1 multicenter randomized control study,18 monocentric randomized control studies,and 1 uncontrolled study.3172 cases were included with 1526 cases in control group (usual care) and 1646 cases in treatment group (usual care with JUC).The difference between the two groups is significant (RD =-0.15,95％ CI:-0.18 ～-0.13,P ＜ 0.05).Conclusions Long-acting antibacterial material JUC can diminish the incidence of CAUTI significantly.
