GRANULAR CELL AMELOBLASTOMA OF MANDIBLE: REVIEW OF A RARE VARIANT OF AMELOBLASTOMA WITH PRESENTATION OF 3 INTERESTING CASES OF THE SAME.

Ameloblastoma is a benign odontogenic neoplasm derived from odontogenic epithelium with mature brous stroma without ectomesenchyme. Based on its clinical,radiographic and histological features,it has been mainly classied under four different typessolid/multicystic,unicystic,peripheral and desmoplastic ameloblastoma. The granular cell ameloblasatoma,a rare histological variant mainly seen in association with plexiform or follicular ameloblastoma of the solid/multicystic type, has been reported in 5% of the cases. The aim of this paper is to present a series of 3 interesting cases of granular cell ameloblastoma with a detailed review on clinical and histopathological features of the same.

Granular cell type of ameloblastoma

Ameloblastoma is a locally invasive tumor derived from odontogenic epithelium. An uncommon variant of ameloblastoma is granular cell type, which cannot distinguish from other ameloblastoma subtypes by clinical and radiographic findings alone. Only review of it's microscopic features allows distinction from other subtypes. The purpose of this article is to present a case of granular cell ameloblastoma. This subtype should be distinguished from the other histopathologic subtypes because of it's higher recurrence rate and more aggressive biological behavior. Radiographic and histologic findings as well as treatment are also discussed.

Granular cell ameloblastoma : A case report with a brief note on review of literature

Ameloblastomas are tumours of odontogenic epithelial origin with varied microscopic patterns that occur either singly or in combination. Granular cell pattern is rarely seen in ameloblastoma, and is characterised by nests of large eosinophilic granular cells. This article describes a case of granular cell ameloblastoma in a 29year old male patient with clinical, radiology and histological findings along with a short on review of literature.

Granular cell ameloblastoma: A diagnostic dilemma for histopathologist

AbstractA rare case of granular cell ameloblastoma in the posterior mandible of a 45-year-old female has been studied both histologically and immunohistochemically. Histopathologically, hematoxylin and eosin and Periodic acid Schiff (PAS) prepared sections of granular cell ameloblastoma showed granular neoplastic cells residing within the tumor follicles. To characterize the granular cells in ameloblastoma, we examined the expression of low molecular weight cytokeratin and S-100 protein using immunohistochemistry. Granular cells exhibited positivity only with cytokeratin while S-100 protein was negative. This result allowed easy distinction of granular cell ameloblastomas from similar tumors exhibiting granular cell changes, and indicated that the granularity in ameloblastoma cells is consequent to lysosomal overload.

Investigation of basement membrane proteins in a case of granular cell ameloblastoma

Granular cell ameloblastoma is a rare, benign neoplasm of the odontogenic epithelium. A case of massive granular cell ameloblastoma in a 44-year-old Thai female is reported. Histopathological features displayed a follicular type of ameloblastoma with an accumulation of granular cells residing within the tumor follicles. After treatment by partial mandibulectomy, the patient showed a good prognosis without recurrence in a 2-year follow-up. To characterize the granular cells in ameloblastoma, we examined the expression of basement membrane (BM) proteins, including collagen type IV, laminins 1 and 5 and fibronectin using immunohistochemistry. Except for the granular cells, the tumor cells demonstrated a similar expression of BM proteins compared to follicular and plexiform ameloblastomas in our previous study, whereas the granular cells showed strong positivity to laminins 1 and 5 and fibronectin. The increased fibronectin expression in granular cells suggests a possibility of age-related transformation of granular cells in ameloblastoma.

Metastasizing granular cell ameloblastoma

Ameloblastoma is a slow growing odontogenic epithelial tumor of jaw. It accounts for 1% of all tumors and cysts arising in maxilla and mandible. Although it is locally invasive and has a marked tendency to recur, metastasis is rare. Of the various histological patterns of ameloblastoma, the granular cell type is extremely rare accounting for 4% of ameloblastomas. We report a case of granular cell ameloblastoma with metastasis to the cervical lymph node presenting in a 40-year-old Indian female.

Granular cell ameloblastoma mimicking oncocytoma

Granular cell ameloblastoma is a variant of ameloblastoma where cells located in the central portion of the follicles have granular eosinophilic cytoplasm and the peripheral cells resemble ameloblasts. A case of granular cell ameloblastoma of the mandible having very similar histopathological features of oncocytoma (oxyphilic adenoma) is reported where tumor cells were arranged in cords, sheets and follicles and their cytoplasm was full of eosinophilic granules.

In some areas, the tumor was very similar to an oncocytoma (oxyphilic adenoma). Almost all tumor cells were full of eosinophilic granules, whereas in the common type of granular cell ameloblastoma, only the cells located in the central portion of the tumor are granular.

Case reports was made on five cases of granular cell ameloblastoma. Age of the patients ranged from 17 to 53 years and mean age was 31. One case was in male and four were females. All cases occurred in the mandible. Four lesions involved the mandibular body or the ascending ramus and one involved the symphyseal region. Radiographic examination revealed unilocular appearance in four cases. Only one case indicated multilocular type. Histologically many of the granular cells were restrictively shown in the stellate reticulum of the epithelial nests. Granular cells were suspected to change from the stellate cells. The granules were ultrastructurally similar to the lysosomes. Although the granular cells indicated various stages of breakdown, in which the granules were released extracellularly, the nuclei retained normal pattern. These findings rejected the possibility of degenerative change of the cells.

Intracytoplasmic crystalloids in granular cell ameloblastoma

A case of granular cell ameloblastoma with numerous intracytoplasmic crystalloids is reported. The intracytoplasmic crystalloids are linear in shape and up to 15 microns in length, and stained similar to the intracytoplasmic granules of granular cell ameloblastomas which have been identified histochemically as lysosomes. The intracytoplasmic crystalloids in the granular cell ameloblastoma presented here probably reflect a variant type of lysosome, possibly due to cellular degeneration.

Granular Cell Ameloblastoma of the Mandible with Metastasis to the Third Thoracic Vertebra

A 62-year-old housewife had a metastasis of granular cell ameloblastoma to the third thoracic vertebra. She had suffered at least four local recurrences during the 38-year duration of the right mandibular tumor and was treated by posterior decompression of the spinal cord with instrumentation. Histologically, the granular cells appeared to be of epithelial origin. Ultrastructurally, the granules were identified as lysosomes, including some that were autophagic. Of 45 reported cases of granular cell ameloblastoma, there have been only three cases with pathologically confirmed metastasis. Their characteristics are long duration of the tumor, multiple local recurrences, and predominance in blacks.

Ultrastructural and histochemical studies of granular-cell ameloblastoma.

Four cases of granular-cell ameloblastoma were studied by light and electron microscopy, histochemistry and electron enzyme histochemistry. Microscopically, granular cells commonly occurred in a follicular pattern, but, in one case, they were in a plexiform pattern. The high activity of acid phosphatase and electron microscopic features revealed that the cytoplasmic granularity was ascribed to the high content of lysosomes. It was speculated that granular-cell ameloblastoma occurs due to the altered dysfunction of tumor cells, and the age factor is related to the pathogenesis of this tumor.

Granular Cell Ameloblastoma: Electron Microscopic Observations

A case of granular cell ameloblastoma was studied by light and electron microscopy. Histologically, the lesion resembled simple ameloblastoma, but differed from the latter in that it contained many cells with granular eosinophilic cytoplasm. Ultrastructurally, the cytoplasmic granules were identified as lysosomes. These organelles were particularly abundant in cells situated in the center of islands and clusters of neoplastic cells. Since no cytoplasmic components could be identified in any of these lysosomes, it was concluded that they are not engaged in autophagy or in remodeling of the neoplastic cells.
