Management of Atypical Suicidal Cut Throat Injury in a Private Tertiary Hospital

Cut throat injuries are potentially life threatening due to the peculiarity of the structures traversing this region of the body. These injuries could be accidental, homicidal or suicidal and the use of a sharp weapon is the least frequent suicidal method in most cases reported. In cases of attempted suicide, hesitant marks are a common finding. Attempting suicide by cutting the throat without hesitant marks is a very rare occurrence. Generally, cases of cut throat injuries are largely under reported as many are fatal before presenting to the hospital therefore a definite incidence is not available. These patients need emergency multidisciplinary care to prevent rapid deterioration, avoidable morbidity and mortality. Adequate airway management skill in intubating and securing the airway to allow for wound exploration and surgical repair is essential for a good outcome. We present to you a rare case of suicidal cut throat injury in a 38 year old male with depressive type of schizoaffective disorder not associated with hesitant marks at Babcock University Teaching Hospital, a private tertiary hospital in south west Nigeria. The rarity of the case is the reason for presenting this case report.

A study of incidence, causes and management of cut throat injuries

<p class="abstract"><strong>Background:</strong> Cut throat injuries is trauma that needs prolonged hospitalization, high cost of health care and reduced quality of life and above all death. The aim of the study was to study on cut throat of injuries including its incidence factors provoked and management.</p><p class="abstract"><strong>Methods:</strong> it is prospective study conducted for a period of 2 years. A total of 30 cases of cut throat injuries were included in the study of patients from age group 5-70 yrs. </p><p class="abstract"><strong>Results:</strong> Male to female ratio was (9:1). Cut throat injury was more common in males who came from rural area. Most of them were unemployed and of low socioeconomic group and of low education level. Cut throat injuries were common in Hindu religion followed by Muslims. The most common cause of cut throat injury in our study was homicidal 17(56.6%) followed by suicidal 9 (30%) and accidental 4(13.3%). In our study 11(36.6%) cut throats were superficial and 19 (63.33%) were deep. 19 (63.33) cases sustained grieveous injury with 7 patients were dead. In all the cases (100%) skin, soft tissue and small vessels were severed. The laryngotracheal injury was present in 12 cases. The majority of patients reached the hospital between 6-10 hours. Simple wound closure was done in 17 cases. 30 patients 23 patients had improved in follow up with no complaint of alteration in voice. 12 (40%) patients had stay of around 0–10 days and with only 10 (33.33%).</p><p class="abstract"><strong>Conclusions:</strong> with the advent of newer diagnostic modalities management of cut throat injuries which allow more precise and better management of cut throat injuries. The management of cut throat injuries require a multidisciplinary approach requiring the close collaboration of the otolaryngologist, the anaesthetist and the psychiatrist.</p>

Life threatening cut throat injury due to kite string

Kite flying is a popular sport in Indian subcontinent. The string or manjha, used for flying kites is coated with ground glass and glue, which makes it very sharp. This abrasive string can give rise to various types of injuries ranging from minor cuts to life threatening injuries and cause severe disabilities and death. Accidental Cut Throat: A Case Report

Injuries by sharp weapon to neck region are possible by many ways but the dilemma to determine its manner of infliction produces mind-numbing challenges in front of autopsy surgeons. Growing urbanization and increased use of locomotives have definitely increased the health hazards especially that of vehicular accidents and deaths. Additional cause of increased vehicular accidents is negligence of driver towards the traffic rules. Cut throat injuries are one of the well known methods of homicide and suicide but accidental cut throat injuries are not infrequent. The manner of causation of cut throat injuries is very crucial part of investigation and many times false allegations and situations are put forth in such cases. The situation further worsens in case of undisturbed crime scene, absence of suicide note and suspicious circumstances. We present here a case of cut throat injury caused by vehicular accident in which manner of death was decided by detailed history, autopsy findings and accurate information about crime scene.

An autopsy study of 74 cases of cut throat injuries

Abstract: The present prospective autopsy study was carried out during the period July 2009 to May 2012 in Kingston, Jamaica. A total of 74 cases of cut throat injury were studied. All the cut throat injuries irrespective of those directly or indirectly contributing to the death were studied. Males dominated the list of victims, contributing to 71.62% (n = 53). Majority of those were in the age group 21–30 (n = 25) and 31–40 (n = 26) which contributed to 33.78% and 35.13% respectively. Majority of the cases were homicides contributing to 97.29% (n = 72) of cases, only 2.7% were suicides and accidental cut throat were never reported. Males dominated the homicides category contributing to 72.22% (n = 52) of the cases. Gang and relationship crisis (homosexual and heterosexual) were the major motivating factors, each contributing to 39.19% (n = 29) and 32.43% (n = 24) of the cases, respectively. The disease suffered by individuals were the least motivating factors contributing to 1.35% of cases (n = 01). The most common cause of death was exsanguinations in 49.95% (n = 34) of cases followed closely by asphyxia due to aspiration of blood i.e. 36.49% (n = 27) of cases and air embolism was the least cause of death, contributing to 4.05% (n = 03) of cases. Cut throat injury was associated with other injuries like gunshot wound, chop wounds and stab wounds in 13.52% of (n = 10) autopsies. Chop injuries contributed to maximum number of other injuries in homicides (n = 46). The most preferred place for the crime was an open field or farm (n = 26). Majority of the cut throat wounds were situated in zone II level contributing to 66.21% (n = 49) of cases. Majority of the wounds were directed from left to right in 75.68% (n = 56) of cases and the cervical vertebra was affected in 8.11% (n = 06) of cases. Major weapon of choice was machete contributing to 83.78% (n = 62) of injuries. The low income group was the most affected group contributing to 91.89% (n = 68) of cases.

Primary Tracheal Reconstruction in Cut Throat Injury of a 4 Years Old Child

Cut throat injuries in children are rare, but they constitute a significant management challenge. Such injuries are rarely reported in the literature. Evaluation and treatment of such injuries is a tough task. Cervical tracheal injuries are relatively uncommon and are frequently associated with esophageal, vascular or spinal injuries. We hereby report a case of cut throat injury with incomplete transection of trachea and its management in a four years old male child.

Cut - Throat Injuries from Deliberate Self Harm and Homicide in Sokoto, Nigeria

The act of terminating human life may be suicidal or homicidal, involving violent or non violent methods. Cut throat as a violent method in terminating human life is discussed. All cases of cut throat injuries that were from deliberate infliction over a 9year period(September 1999- August 2008) and the circumstances surrounding such injuries were reviewed from a tertiary health institution in north western Nigeria. A total number of 15 cases were seen. All were males with age range of 18 to 60 years(Mean age 32.47years± SD 11.351) . Ten (66.7%) cases were from deliberate self harm(DSH)(Age range:20-60years, mean age 36.9years±SD 13.017) but were known psychiatric patients who had defaulted in follow-up psychiatric care. Five (33.3%) were homicidal(Age range 18-35years ,mean age 23.8years±SD 8.258) as a result of conflicts, three of whom were under the influence of cannabis. Surgical repair and psychiatric treatment were offered concurrently. One required a tracheostomy to prevent upper airway obstruction from laryngeal trauma. Repair was carried out under local anaesthesia in all but one case that required general anaesthesia. Psychosocial disturbances may lead to DSH, suicide or homicidal cut throat injuries. Psychiatric patients who are not on regular medications accounted for 66.7% of the cases of DSH in this study while 33.3% of the cases were homicidal. Therefore continued follow up of psychiatric patients must be emphasized to prevent these injuries in addition to addressing psychosocial issues that may lead to DSH, suicide, or homicide in the society.Key words: Cut throat injury, Deliberate self harm, Homicide,
