Radiologic Findings of Splenic Abscess: A Report of Two Cases

Splenic abscess is a very rare entity with an incidence of about 0.14-0.7% in autopsy studies. Splenic abscess has been reported to have a mortality rate of about47%, it has various causes but usually associated with trauma and infections of the spleen. We report two cases of splenic abscess; a child with post traumatic cause and a middle aged lady with an unknown cause. Both cases were diagnosed of having the abscess following imaging; the first case had abdominopelvic ultrasound that showed an abscess cavity with a volume of about 11mls and subsequently resolved following medical treatment with intravenous antimicrobial agents. The second case however had an abdominopelvic computed tomography scan that showed a huge splenic abscess with a volume of about 1400mls; she had a successful splenectomy. We report the radiologic findings of these cases due to the rare nature of splenic abscess as reported in the literature.

Endovascular management of giant visceral artery aneurysms

Endovascular management of small visceral artery aneurysms is an established treatment with satisfactory outcomes. However, when size exceeds 5 cm visceral aneurysms are considered as “giant” (giant visceral artery aneurysms or GVAAs) and management is significantly more complex. Between August 2007 and June 2019 eleven cases of GVAAs that were endovascularly treated were retrospectively reviewed and included in this single center study. Mean size was 80 mm (± 26.3 mm) x 46 mm (+ \\-11.8 mm). Nine of the lesions were true aneurysms, and two were pseudoaneurysms. In 8 patients, the lesion was causing compression symptoms in the surrounding organs, one patient developed a contained rupture while 2 patients were completely asymptomatic. However, all patients were hemodynamically stable at the time of treatment. Technical success was defined as immediate complete exclusion of the aneurysmal sac, and clinical success as complete relief from clinical symptoms. Follow-up was performed with CT angiography, ultrasound and clinical examination. Mean follow-up was 45 months (range 6–84). Technical and clinical success were both 91%. Complications were one lack of control of contained rupture that was subsequently operated, one case of self-limiting non-target spleen embolization and one case of splenic abscess. Three patients died, one due to the contained rupture 15 days after procedure, the other two for other causes and occurred during the long-term follow-up. This series suggests that endovascular treatment of giant visceral artery aneurysms and pseudoaneuryms is a valid minimally invasive solution with very satisfactory immediate and long-term outcomes unless the aneurysm is already ruptured. A variety of endovascular tools may be required for successful treatment.

Key Clinical MessageSleeve gastrectomy (SG) is a surgical weight-loss procedure. Splenic abscess is a rare complication of SG. Four cases of splenic abscess after SG have been reported, all managed by surgical intervention. We report the first documented case of multiple splenic abscesses following SG managed conservatively by an integrated medical treatment.

Plasmodium vivax infection manifesting as splenic abscess: A rare case report and review of literature

Incidence of malaria across the globe has constantly been on a rise, especially the tropics. Hence, it is of utmost importance to know its clinical presentation and complications including rare ones and treat them at the earliest. Spleen is a major organ affected by plasmodium. The changes in spleen because of malaria can range from asymptomatic enlargement to complications, such as splenic infarct, rupture, hemoperitoneum, hypersplenism, torsion, cyst, or abscess formation all of which are life-threatening if not treated. Splenic abscess due to any cause is a serious complication due to high-incidence of morbidity and mortality associated with it. Spleen abscess as a complication of malaria has been rarely described in the literature. We present to you a rare case of splenic abscess due to vivax species of plasmodium which was successfully treated conservatively.

Splenic Abscess Caused by Streptococcus gallolyticus subsp. pasteurianus as Presentation of a Pancreatic Cancer

Splenic abscesses caused by Streptococcus bovis are rarely reported in the literature and are mainly seen in patients with endocarditis and associated colonic neoplasia/carcinoma. We report the first case of splenic abscess caused by Streptococcus gallolyticus subsp. pasteurianus (Streptococcus bovis biotype II/2) as presentation of a pancreatic cancer.

Splenic Abscess Due to Salmonella enteritidis

Splenic abscess is a very rare complication of non-typhoidal Salmonella infections. We report a case of splenic abscess caused by Salmonella enteritidis. The patient is a 63-year-old woman with diabetes mellitus and underwent splenectomy. This case suggests that the patients with comorbities are at increased risk for invasive infections in non-typhoidal Salmonella infections.

Urgent Splenectomy after Partial Splenic Embolization in Liver-Transplanted Patient: A Case Report

We describe a 51-year-old-male. Three months after liver transplantation due to hepatitis C virus (HCV) hepatopathy, an HCV relapse was detected, and partial splenic embolization (PSE) was performed prior to antiviral treatment. Eleven days after PSE emergency splenectomy was performed due to the development of a splenic abscess, which is a rare but severe complication of PSE. Between May 2002 and March 2012, 18 PSEs have been performed in transplant patients in our centre. The patient presented here is the only case of splenic abscess and the only one who has needed surgery after complications of PSE.

Splenic Abscess and Multiple Brain Abscesses Caused by Streptococcus Intermedius in a Young Healthy Man

We report a case of splenic abscess with multiple brain abscesses caused by Streptococcus intermedius in a healthy young man without any identifiable risk factors, which resolved with percutaneous drainage and antibiotics. Streptococcus intermedius, a member of the Streptococcus anginosus group, is a common commensal organism of the oral cavity and gastrointestinal tract, and it is a known cause of deep-seated infections. Suppurative infections caused by Streptococcus anginosus group are sometimes associated with bacteremia, but hematogenous spread of infection from an occult source leading to concurrent splenic abscess and multiple brain abscesses has never been previously reported in a healthy young individual.

胃癌に合併した脾膿瘍の１例

脾膿瘍は稀な疾患である．症例は66歳，男性．悪寒，動悸を主訴に来院した．39.8度の高熱と腹部全体に圧痛，筋性防御を認めた．血液学的検査では貧血と炎症の所見があり，腹部CTでは腹水の貯留と脾臓の裂傷がみられた．また胃壁は肥厚しており胃原発の悪性疾患を疑う所見を認めた．汎発性腹膜炎または脾臓破裂による腹腔内出血を疑い緊急手術を施行した．開腹所見は脾膿瘍による脾破裂による汎発性腹膜炎を呈していた．また，胃体部大彎を中心とした腫瘍性病変が触知され，この腫瘍と脾臓と膵臓は一塊となっていた．胃癌もしくは胃悪性リンパ腫が疑われ胃全摘，D1郭清，膵脾合併切除，Roux-Y再建施行した．病理所見から胃癌の脾臓浸潤と穿通が確認された．今回われわれは胃癌の脾臓浸潤による穿通のため脾膿瘍を合併し，膿瘍穿破による汎発性腹膜炎を発症した１例を経験したので報告する．

Splenic abscess caused by Lactobacillus paracasei

Splenic abscess is relatively uncommon. Infections caused by lactobacilli, which are usually one of the constituents of normal flora of human gut and vagina, are also very uncommon. Here we report a case of splenic abscess caused by Lactobacillus paracasei. We performed a literature review for this rare entity. Immunocompromised status of the patient, who had uncontrolled diabetes, could have contributed to the pathogenesis of this rare disease.

Higher incidence of major complications after splenic embolization for blunt splenic injuries in elderly patients

Background Nonoperative management (NOM) of blunt splenic injuries has been widely accepted, and the application of splenic artery embolization (SAE) has become an effective adjunct to NOM. However, complications do occur after SAE. In this study, we assess the factors leading to the major complications associated with SAE. Materials and Methods Focusing on the major complications after SAE, we retrospectively studied patients who received SAE and were admitted to 2 major referral trauma centers under the same established algorithm for management of blunt splenic injuries. The demographics, angiographic findings, and factors for major complications after SAE were examined. Major complications were considered to be direct adverse effects arising from SAE that were potentially fatal or were capable of causing disability. Results There were a total of 261 patients with blunt splenic injuries in this study. Of the 261 patients, 53 underwent SAE, 11 (21%) of whom were noted to have 12 major complications: 8 cases of postprocedural bleeding, 2 cases of total infarction, 1 case of splenic abscess, and 1 case of splenic atrophy. Patients older than 65 years were more susceptible to major complications after SAE. Conclusion Splenic artery embolization is considered an effective adjunct to NOM in patients with blunt splenic injuries. However, risks of major complications do exist, and being elderly is, in part, associated with a higher major complication incidence.

Tubercular splenic abscess in immunocompetent patients: a rare entity

Splenic abscess is an unusual, potentially life threatening condition associated with septiceamic conditions. Splenic abscess due to tuberculosis is a rare entity, specifically seen in immunocompromised individuals. However splenic tubercular abscess in immunocompetent individuals is rarer. We present two cases of splenic tubercular abscess in immunocompetent persons where a diagnosis of tuberculosis was made after splenectomy. The aim of presenting these cases is to stress the fact that tuberculosis should be considered as a differential diagnosis in all cases of splenic abscesses in areas where tuberculosis is endemic.

Abscesses of the spleen: Report of three cases

Abscess of the spleen is a rare discovery, with about 600 cases in the international literature so far. Although it may have various causes, it is most usually associated with trauma and infections of the spleen. The latter are more common in the presence of a different primary site of infection, especially endocarditis or in cases of ischemic infarcts that are secondarily infected. Moreover, immunosuppression is a major risk factor. Clinical examination usually reveals a combination of fever, left-upper-quadrant abdominal pain and vomiting. Laboratory findings are not constant. Imaging is a necessary tool for establishing the diagnosis, with a choice between ultrasound and computed tomography. Treatment includes conservative measures, and surgical intervention. In children and in cases of solitary abscesses with a thick wall, percutaneous catheter drainage may be attempted. Otherwise, splenectomy is the preferred approach in most centers. Here, we present three cases of splenic abscess. In all three, splenectomy was performed, followed by rapid clinical improvement. These cases emphasize that current understanding of spleen abscess etiology is still limited, and a study for additional risk factors may be necessary.

Combined Hepatic and Splenic Abscesses in a Patient with Ulcerative Colitis

Liver abscesses are very rare complications of ulcerative colitis, and furthermore, there has been only one case of splenic abscess in a patient with ulcerative colitis reported in the English literature. We recently encountered a patient with ulcerative colitis accompanied by both hepatic and splenic abscesses. The patient was treated with abscess drainage as well as sulfasalazine and antibiotics. Follow-up sonography of the abdomen showed complete resolution of the lesions. To our knowledge, this is the first report of combined case of multiple liver abscesses combined with splenic abscess in a patient with ulcerative colitis.

OBJETIVO: Relatar as características demográficas, clínicas, diagnóstica e terapêutica de pacientes com abscesso esplênico (AE) causado por Salmonella. MÉTODO: Análise retrospectiva de dados de pacientes atendidos no Serviço de Cirurgia Geral e Aparelho Digestivo do Hospital Universitário Gaffrée-Guinle no período de janeiro de 2001 a dezembro de 2005, a estes se incluiu um caso tratado em outro hospital em época anterior. RESULTADOS: Dentre 4823 pacientes recentemente atendidos, dois apresentaram AE causado por Salmonella enteritidis, enquanto o caso mais antigo o agente responsável foi a Salmonella typhi. Todos eram homens, com idade média de 45 anos. Em nenhum deles foi identificada condição predisponente à formação do abscesso; os exames de imagem foram capazes de diagnosticar o AE. Todos foram tratados por esplenectomia e antibioticoterapia e evoluíram para cura. CONCLUSÕES: A Salmonella, apesar de infrequente, pode ser o agente causal do AE. Caracteristicamente os abscessos eram grandes e apresentavam material necrótico em seu interior. Nesta condição, a esplenectomia associada a antibioticoterapia mostrou-se eficaz no tratamento.

A case of splenic abscess after radiofrequency ablation

Radiofrequency ablation (RFA) is an innovative technique used primarily for the palliative treatment of unresectable liver tumors. Its therapeutic indications however, have been expanded and now include various other organs and diseases. There is a paucity of data regarding technical details and complications of the use of RFA in the spleen. We report a case of partial splenectomy using radiofrequency ablation for splenic hydatid disease, complicated by an abscess formation.

Successful treatment of Paecilomyces variotii splenic abscesses: a rare complication in a previously unrecognized chronic granulomatous disease child

A case of splenic abscesses caused by Paecilomyces variotii in a 21-month-old child with previously undiagnosed chronic granulomatous disease (CGD) is described. An X-linked form of CGD with deficiency of gp91- phox was diagnosed. The fungal pathogen was isolated from the abscess contents. Cure was achieved by a partial splenectomy and a prolonged fluconazole and flucytosine combination treatment regimen. P. variotii is an emerging fungus in children with CGD. Every effort should be made to recover the possible underlying immunodeficiency in children with paecilomycosis.

Staphylococcus aureus Abscess of the Spleen in a ß-Thalassemia Patient

Splenic abscesses are rare among abdominal abscesses. We present a case of splenic abscess due to Staphylococcus aureus in a beta-thalassemia major patient. Such a complication may not be coincidental, as beta-thalassemia major patients have an increased susceptibility to infection, which is attributable to a number of immune abnormalities.
