Cutaneous findings are a prominent feature of many rare infectious diseases, and recognition of these findings is often essential to diagnosis. During my residency, I have seen 3 rare infectious diseases as dermatologic consultations which have included rat-bite fever (RBF), leprosy, and generalized vaccinia. The case of RBF was diagnosed in a 34-year-old woman who presented with a 2-day history of malaise, fever/chills, polyarthralgia, and a tender and pruritic vesiculopapular rash of extremities and oral mucosa beginning 2 days after being bitten by a rat.

BackgroundOnly three other cases of rat bite fever caused by Streptobacillus notomytis in humans have been reported since this species was identified in 2015. Data specific to the differences in clinical features and geographic distribution between S. notomytis infection and S. moniliformis infection are scarce. All previous cases of human S. notomytis infection were reported from Japan. This is the first case of S. notomytis infection reported from outside of Japan.Case presentationA 72-year-old Thai woman was admitted to Siriraj Hospital (Bangkok, Thailand)—Thailand’s largest university-based national tertiary referral center—in August 2020 with fever, myalgia, and polyarthralgia for 3 days, and gradually decreased consciousness for the past 1 day. Physical examination and laboratory investigations revealed septic arthritis of both knee joints, meningitis, and hepatitis. She was initially misdiagnosed as rheumatoid arthritis in the elderly since the initial investigations were unable to detect a causative pathogen. However, S. notomytis infection was later confirmed by polymerase chain reaction amplification of a part of the 16S rRNA gene and sequencing from synovial fluid. Her clinical course was also complicated by spondylodiscitis and epidural abscess caused by S. notomytis, which was detected from tissue biopsy. Therefore, rat bite fever in this patient manifested as meningitis, septic polyarthritis, hepatitis, and spondylodiscitis. The patient was treated with intravenous ceftriaxone then switched to oral amoxicillin with complete recovery.ConclusionsThe clinical manifestations of S. notomytis infection are similar to those demonstrated in S. moniliformis infection. This case also showed that arthritis caused by S. notomytis mimics rheumatoid arthritis, and that meningitis and spondylodiscitis are potential coexisting complications that can be found in S. notomytis infection.

Rat bite fever with osteomyelitis and discitis: case report and literature review

BackgroundRat bite fever (RBF) is a rare systemic febrile illness transmitted by rats. Streptobacillus moniliformis is a pleomorphic Gram-negative bacillus which is the usual etiologic organism for rat bite fever in the United States.Case presentationHere we present a case of rat bite fever complicated by vertebral osteomyelitis and discitis. The patient revealed an exposure history of being bitten by pet rats. The patient’s symptoms dramatically improved with a six-week course of cephalexin therapy.ConclusionsIt is important to obtain a thorough zoonotic exposure history and maintain rat bite fever in the differential when considering potential causes of discitis and osteomyelitis.

Rat Bite Fever Resembling Rheumatoid Arthritis.

Rat bite fever is rare in Western countries. It can be very difficult to diagnose as blood cultures are typically negative and a history of rodent exposure is often missed. Unless a high index of suspicion is maintained, the associated polyarthritis can be mistaken for rheumatoid arthritis. We report a case of culture-positive rat bite fever in a 46-year-old female presenting with fever and polyarthritis. The clinical presentation mimicked rheumatoid arthritis. Infection was complicated by discitis, a rare manifestation. We discuss the diagnosis and management of this rare zoonotic infection. We also review nine reported cases of rat bite fever, all of which had an initial presumptive diagnosis of a rheumatological disorder. Rat bite fever is a potentially curable infection but can have a lethal course if left untreated.

Prosthetic Valve Endocarditis Caused by Streptobacillus moniliformi s: a Case of Rat Bite Fever

We report a case of rat bite fever caused by Streptobacillus moniliformis in Taiwan. It manifested as prosthetic valve endocarditis, which was cured by cardiac valve replacement and antimicrobial therapy. The DNA sequence of the 16S rRNA gene of S. moniliformis was detected in valve specimens by PCR and nucleotide sequencing.

An enzyme-linked immunosorbent assay (ELISA) for monitoring rodent colonies for Streptobacillus moniliformis antibodies.

An enzyme-linked immunosorbent assay (ELISA) to measure Streptobacillus moniliformis antibodies in mice and rats was developed. Twelve S. moniliformis strains originating from cases of rat-bite fever and Haverhill fever in man and from various rodent species, showed considerable serological relationship. The ELISA appeared specific since antibodies to S. moniliformis were absorbed by autologous and homologous antigen, but not by heterologous bacterial antigens. Acholeplasma laidlawii showed partial serological relationship with S. moniliformis. The ELISA was validated using experimental infections in mice and rats. These studies and observations in naturally infected feral rats, confirmed that S. moniliformis is difficult to grow on primary isolation, and that the ELISA for S. moniliformis antibodies revealed more contaminated animals than culture.

Subacute bacterial endocarditis due to streptobacillus moniliformis.

Cases of rat bite fever due to Streptobacillus moniliformis have been extensively reported in recent years, and this disease has been distinguished from that due to Spirillum minus. 1 Bacterial endocarditis due to S. moniliformis has been reported rarely, a survey of the literature revealing but four cases of this complication. 2 An additional unpublished case has come to our knowledge. 3 All these cases have ended fatally. Three of them occurred in the prepenicillin era, and two occurred soon after the use of penicillin was begun and prior to the recognition of the importance of massive doses of the drug in the treatment of endocarditis. The value of penicillin in S. moniliformis infections was experimentally demonstrated by Heilman, 4 and this drug has been shown to be clinically effective by many workers. 5 The present case is reported because it is the first apparent cure of subacute bacterial endocarditis

EXPERIMENTAL STREPTOBACILLUS MONILIFORMIS ARTHRITIS IN THE CHICK EMBRYO

1. A strain of Streptobacillus moniliformis isolated from a case of rat bite fever in man has been found to produce infection of the developing chick embryo following inoculation of the chorio-allantois. 2. The disease in embryos is characterized by invasion of the blood stream and an almost exclusive localization of the infectious process to the synovial lining of the joints. 3. In the early stages of the development of the joint lesions the Streptobacillus moniliformis behaves as a facultative intracellular parasite within the cytoplasm of the synovial lining cells. Conditions favorable for the growth of the microorganisms are maintained only temporarily. The infection appears to be self-limiting in nature.

Summary The cases of ratbite fever in the United States are reviewed in which Spirillum minus was demonstrated, and another such case is reported. The methods of laboratory diagnosis are briefly discussed.

Rat-bite fever: Report of three cases with a review of the literature

Summary 1. Three cases of rat-bite fever are reported because of their rarity, their unusual symptoms, and serology. 2. All three gave histories of having been bitten by a rat; and two had typical local swelling, lymphadenitis, typical recurring periodic temperatures. All had positive Kahn and Wassermann reactions during the height of their illness. Two improved under sulpharsphenamine treatment and were discharged as cured and with negative serology. One patient did not report back until recently when the Wassermann and Kahn reactions were positive. She will receive further treatment. No claim is made that all cases will show a positive serology. 3. Any form of arsphenamine may be used in treatment. We prefer sulpharsphenamine for use in children. The prognosis is excellent under a very short period of treatment. 4. We believe that rat-bite fever occurs more frequently than is generally believed and should be brought to the attention of the profession.

Rat-Bite Fever Associated with Sporothrix

occurs sporadically in all parts of the world. It most commonly follows the bite of a rat, less commonly that of a weasel, ferret, cat or squirrel. Ordinarily the local wound heals in about five days. This is followed after from five to fourteen days by regional lymphadenopathy, paroxysms of fever rising to from 103 to 105 F., prostration, pain in the back, weakness in the limbs and loss of appetite. The paroxysms may last from two to four days, during which a bluish exanthem may appear on the body. This is followed by a remission, and in the course of from five to eight days another paroxysm occurs. Thus there are alternate paroxysms and remissions for a variable period of time. At first these alternations occur at weekly intervals; later the afebrile periods grow longer and the febrile shorter and less severe. A considerable number of widely different organisms have been assigned an etiologic role in this disease. Ogata 1 described an aspergillus-like organism (Sporozoa muris) which he isolated from the blood, lymph glands and wound in two cases of rat-bite fever. He inoculated rabbits and guinea-pigs with a piece of lymph gland from one of the patients and was able to recover the protozoan in different stages of development from the ulcers, blood, lymph glands and spleens of the inoculated animals.
