Common cause of massive pleural effusions (MPE) are malignancy, tuberculosis and parapneumonic effusion,. Parasitic infections are very rare causes of (MPE). Echinococcosis or hydatid disease is a major public health problem in poor hygienic country in the world. The liver is most common organ involved and lungs are the secound most frequently involved organs. Pulmonary hydatid cyst appears to be more common in younger individuals. Echinococcus granulosus is most common type and Echinococcus multilocularis hydatid cyst causes alveolar hydatid cyst, which accounts for <5%of all cases of hydatid disease of liver and less frequently cause of lung cyst. Here, we present an unusual and very rare case of echinococcosis pulmonary hydatid cysts mimicking and present as (MPE) with delay diagnosis. After antero-lateral thoracotomy. Diagnosis of hydatid cyst confirmed, patient was discharged with good condition.

The evaluation of pulmonary hydatic cyst and their surgical results in our region

The demographic characteristics, clinical and radiological findings, surgical procedures and surgical results of pulmonary hydatid cyst patients who underwent surgical treatment between 2009 and 2013 in our clinic were evaluated. Pulmonary hydatid cyst cases who underwent surgical treatment between 2009-2013 were analyzed retrospectively. A total of 84 (48.8%), 41 women and 43 (51.19%) men with pulmonary hydatid cysts were included in the study. Diagnosis was made by radiological evaluation in all hydatid cyst cases. In our study, one-way analysis of variance test and Chi-square test were used as statistical methods with the SPSS statistics package program. Of all 84 patients who were decided to undergo surgical intervention due to pulmonary hydatid cyst, 69 (82.14%) of them had single and 15 (17.85%) of them had multiple pulmonary hydatid cysts. Hydatid cyst was observed in the left lung in 30 patients (38%), in the right lung in 37 patients (44%), and bilaterally in 15 patients (18%). The most common (88.09%) preferred surgical procedure was cystotomy+capitonnage. In our study, no mortality related to hydatid cyst was observed. Hydatid cyst disease is an endemic disease in our country. It should be kept in mind in suspicious clinical and radiology situations as it may also progress without symptoms. The curative treatment of pulmonary hydatid cysts is surgical, and the cystotomy+capitonnage technique which we preferred the most in our study is a reliable procedure and radical parenchyma resections such as pneumonectomy, lobectomy or segmentectomy should be avoided as much as possible.

Cystic echinococcosis or hydatid disease is caused by genus echinococcus and is transmitted to human through intermediate host (sheep and cattle). In children pulmonary involvement is common than hepatic involvement. Presentation is usually non-specific but sometimes patient can present ē symptoms of mass effect or cyst complications. Early detection and prompt treatment prevent mortality. We report a case of pulmonary hydatid cyst, who was initially presented with fever & cough for one month & later came with shortness of breath. Final diagnosis was done on CXR, USG chest, CT and echinococcus antibodies and was treated successfully with anthelminthic drugs.

A Single‐center Large‐volume Experience in the Surgical Management of Hydatid Disease of the Lung With and Without Extrapulmonary Involvement

We compared the number of incisions, surgical procedures, hospital duration, and complications in hydatid cyst patients with unilateral or bilateral thoracic involvement and concomitant involvement of the extrathoracic organs. A total of 76 hydatid cyst cases surgically treated between the years 2007 and 2012 were divided into three groups according to radiological evidence of other organ involvement and surgical procedures: group 1 had only unilateral thoracic involvement and a single incision; group 2 had additional involvement of the contralateral thoracic side or extrathoracic organs and at most two incisions were performed at the same session; and group 3 had two or more incisions performed at separate sessions in addition to the involvement features of group 2. We had 46 (60.5 %) cases with only thoracic involvement and 30 others (39.5 %) with extrathoracic organ involvement. Complications were seen in only one patient each in the first and second groups, and in 6 patients in the third group. Duration of hospital stay was 7.04 ± 0.86 (5-9) days in group 1.8.33 ± 1.87 (7-13) days in group 2, and 13.95 ± 2.03 (9-18) days in group 3. Although multiple session surgery is used to decrease the risk of complications, contamination, and infection in multiple or bilateral pulmonary hydatid cyst cases, or in patients with other organ involvement, single-session surgery can be used in selected cases taking into account the operative trauma, financial consequences, and psychological profile.

Surgical treatment of pulmonary hydatid cysts, which perforated to the pleura.

Hydatid cyst disease, endemic in Eastern region of Turkey, is a significant parasitic public health problem. In this study, pleural complications of hydatid cysts were presented in 76 cases. In our study, of the 412 pulmonary hydatid cyst cases operated on between 2003 and 2011, 76 cases had ruptured into the pleura for various reasons, and the different clinicoradiological presentations were evaluated retrospectively. The age distribution of the cases was between 7 and 56 years, and the mean age was 26.20±13.04. The most frequent symptom due to pleural rupture in patients was dyspnea (44 cases, 57.8%). Etiologically, iatrogenic perforation was detected in four cases and thoracic trauma in nine cases (six car accidents and three falls from a height). An anthelmintic drug use history was found in three cases of ruptured pleura. Spontaneous perforation was detected in the other 60 (78.9%) cases. Two cases that were admitted to the emergency unit and were immediately administered a tube thoracostomy developed tension pneumothorax. In addition, 21 cases had hydropneumothorax, 17 had pneumothorax, and 36 had pleurisy. Morbidity was observed in 30 cases (39.4%). Atelectasis was the most frequent cause of morbidity in these patients (10 cases). The mean duration of hospitalization was determined to be 12.26±2.90 days. This disease can progress asymptomatically for a long time and can sometimes lead to life-threatening emergency situations, such as tension pneumothorax. The delayed admission of a patient to a physician causes the disease to become more complicated and to increase the morbidity and mortality rates. Treatment of the disease is in the form of surgery. Possible parenchymal protection should be applied in surgical treatment, and anatomic resection should not be performed unless necessary.

Social factors associated with pulmonary hydatid cyst in Aegean, Turkey

Pulmonary hydatid cysts are caused by larval forms of the tapeworm Echinococcus granulosus. Hydatid disease, which is endemic in Turkey, is an important health problem in agricultural and stockbreeding areas where there are insufficient environmental health and preventive medicine services. Previous studies on this topic reported that hydatid disease was frequently associated with stockbreeding in rural areas. In this retrospective clinical study, it is aimed to evaluate the social factors associated with pulmonary hydatid cyst in the Aegean region of Turkey. Seventy-one pulmonary hydatid cyst cases reported between 1998 and 2010 in four hospitals in the Aegean region for which data were available on social and lifestyle factors were reviewed. In contrast to expectations, we found that pulmonary hydatid disease in the Aegean region was associated with city rather than rural life and with occupations other than stockbreeding. The findings are attributed to the presence of uncontrolled dogs in the city and urban environmental pollution. The offices of the public administration have a responsibility to inform citizens about the control of street animals, environmental health, and food hygiene.

Our results in surgical treatment of hydatid cyst of the lungs

Aim: The cases of pulmonary hydatid cysts are frequently seen especially in Eastern Anatolia Region because the living source is stock-breeding. We aimed to discuss 102 patients operated with the diagnosis of pulmonary hydatid cyst in 2.5 years’ period. The patients with hydatid cyst disease who were applied to our clinic, were evaluated in light of literature. Methods: 102 patients were diagnosed with hydatid cyst clinically and radiologically between June 2002 and January 2005. Their ages ranged between 4-65 years with the mean age of 23 years. Thoracotomy was applied to all patients. Results: Cyst localization was unilateral in 96 and bilateral in 6 of them. 10 patients had also cyst in the liver. Cyst was perforated in 56% of the patients. Enucleation was applied in 3 patients, cystectomy in 3 patients and cystotomy+capitonnage was applied to the remaining. In bilateral cases, operation was applied to the other side after one month from the first operation. Percutaneous drainage was applied in coordination with radiology department in the presence of hepatic hydatid cyst. In these cases and in the presence of multipl cysts, albendazole treatment was applied. Lower lobectomy (2 right, 1 left) was performed in 3 cases. No mortality or recurrence was observed. Conclusion: Cystotomy+capitonnage is still the most valid surgical method in pulmonary hydatid cyst cases because it is a method preserving the parenchyma.

Thoracoscopic treatment of pulmonary hydatid cyst in a child

Hydatid disease has a wide geographic distribution around the world. In human, the lungs are the second most commonly affected sites. Pulmonary hydatidosis is much more frequently encountered in children than in adults. Surgical treatment has remained the standard option in the management of hydatid disease. However, surgeons were able to replicate the principles of conventional surgery using minimally invasive techniques. Herein, we report a case of pulmonary hydatid cyst in a 9-year-old girl treated successfully using the thoracoscopic approach.

Pulmonary hydatid cyst is exceptional in our country. The diagnosis is generally established on the basis of imaging. Surgical treatment is indicated in most patients. We report a case of pulmonary hydatid cyst in a 74-year-old smoker (11 package-years) who presented cough with mucopurulent expectoration and occasional hemoptysis. The chest x-ray demonstrated a poorly delimited heterogeneous opacity in the upper third of the left lung which persisted after antibiotic therapy. Bronchial fibroscopy revealed a hydatid membrane in the left superior bronchus which was totally removed by aspiration. A control radiography after the fibroscopy demonstrated a clear regression of the opacity. A thoracic CT-scan was performed and revealed a residual cavity in the left upper lobe with bronchial dilatation. The indication for surgery was not retained. Treatment of pulmonary hydatid cyst by endoscopy is an exceptional event. Our exceptional case illustrates an atypical radiological presentation and the use of "therapeutic" fibroscopy.

Surgical treatment of pulmonary hydatid cysts in pediatric patients.

Hydatid cyst is a zoonotic infection that is caused by Echinococcus granulosus and alveolaris, and that can result in endemics in rural regions. Infections caused by Echinococcus granulosus are more commonly seen in our country. In this retrospective study, we aimed at discussing the experience we have accumulated on pediatric pulmonary hydatid cyst cases over the last 6 years. Between 1995-2001, in the Department of Thoracic and Cardiovascular Surgery in Gaziantep University, 38 cases underwent 39 operations due to pulmonary and hepatic hydatid cysts. Of these, 24 were males and 14 were females, with an age range of 2-16 years. Operations were frequently performed via thoracotomy. Nineteen cysts were intact, and 26 were perforated. One case had sternotomy, another had sequential thoracotomy, and the rest of the cases underwent thoracotomy, which resulted in cystotomy-capitonnage. In the postoperative period, 2 patients had sustained air leakage, and 2 other cases developed skin infections. Average hospital stay was 5 days. After the operation, the patients received chemotherapy (albendazole 10 mg/kg/day) for 1.5 months. Early or late deaths or recurrences were not observed. We conclude that cystotomy-capitonnage is a successful treatment for pediatric pulmonary hydatid cysts, as it preserves the parenchyma. Chemotherapy in the postoperative period is beneficial in preventing the recurrences.

Repeated Unintentional Transthoracic Needle Aspiration of a Pulmonary Hydatid Cyst

Transthoracic needle aspiration is not a recommended diagnostic modality in hydatid disease. Percutaneous aspiration of a suspected hydatid cyst is believed to be associated with the risk of allergic reactions which can result in systemic anaphylaxis and possible spreading of the cyst contents. We present herein a illustrative case of pulmonary hydatid cyst and multiple mediastinal lymphadenopathies which was diagnosed after repeated transthoracic fine needle aspirations.
