A Case of Foreign Body Granuloma Due to the Presence of a Silk Surgical Thread after Resection of Local Recurrence of Descending Colon Cancer

症例は65歳の女性で，下行結腸癌（pT3pN0M0，pStage II：大腸癌取扱い規約第8版）に対し腹腔鏡下結腸左半切除術を施行した．術後22か月目にCEA 10.1 mg/mlの上昇，FDG-PET/CTで吻合部近傍にSUVmax 5.5の異常集積を伴う腫瘤を認めた．下行結腸癌術後局所再発と診断し開腹結腸部分切除術を施行した．病理所見は管状腺癌の浸潤性増殖で大腸癌局所再発であった．再発手術後12か月目の造影CTで吻合部近傍に腫瘤を認め，FDG-PET/CTでSUVmax 9.7の異常集積があり大腸癌再々発と診断し手術を施行した．腫瘤は小腸浸潤も認め，開腹結腸部分切除，小腸部分合併切除を行った．病理所見は異型細胞を認めず，縫合糸による異物性肉芽腫と診断した．大腸癌局所再発切除後の異物性肉芽腫の報告ははじめてであり，PET陽性であった場合異物性肉芽腫も鑑別にあげる必要があると考える．

Curious case of mesh associated foreign body granuloma- a case report

Background: Foreign body granuloma represents granulomatous response of the tissue to any foreign body. Many cases of foreign body granuloma with various foreign bodies have been reported. However, mesh acting as a foreign body is quite a rare occurrence.
 Summary: Here we report a case of a 52 yrs old lady who presented to us with complaints of swelling over abdomen in supraumbilical area since 6 months. Patient had a history of umbilical hernioplasty done 4 yrs back. On clinical evaluation, the provisional diagnosis of an ?abdominal wall tumor ?recurrent hernia was made. Ultrasound and computed tomography of the abdomen was done which was suggestive of a midline abdominal wall abscess. Excision of the swelling and consecutive histopathological analysis suggested it to be a foreign body granuloma associated with the mesh.
 Conclusion: Thus, it is very important to keep in mind foreign body granuloma as a differential diagnosis in patients with history of hernia surgery presenting with swelling in the same region.

Delayed Foreign Body Granulomas in the Orofacial Region after Hyaluronic Acid Injection.

Objective: To evaluate and analyse granulomatous reaction caused by intradermal injections with hyaluronic acid (HA) fillers in the orofacial region. Methods: A retrospective review of 11 cases of foreign body granulomas caused by HA fillers was performed. Demographic data, clinical symptoms, imaging features, treatments, pathology results, history of facial cosmetic procedures and prognosis were reviewed. Results: Most of the cases appeared as painless palpable nodules with no significant growth, located in the cheeks, chin, lips or temples. The nodules were excised, and pathological examination revealed amorphous basophilic material surrounded by foreign body giant cells and macrophages. No patient’s clinical and pathological diagnosis was linked to HA during the first appointment. During follow-up, all patients admitted that they had received dermal filler injections from 3 to 10 years previously. Most of the patients had a favourable prognosis; one patient complained of facial asymmetry and another reported mild pain in the upper lip after surgery. Conclusion: The increase in the number of cases showing delayed complications caused by HA fillers merits closer clinical attention. A thorough understanding of the patient’s medical history and biopsy specimen are necessary to make a definite diagnosis and offer appropriate treatment.

Diagnostic Tools to Use When We Suspect an Allergic Reaction to a Tattoo: A Proposal Based on Cases at Our Hospital

IntroductionTattooing has become a popular practice in western countries, particularly among younger populations. Tattoos, however, can cause complications, such as infections, allergic or foreign-body reactions, and even systemic inflammatory responses. Patients and methodsWe conducted a retrospective study of all patients seen for tattoo-related complications at our skin allergy unit between January 2002 and December 2016. ResultsWe studied 23 patients. Nine of these experienced early complications, all related to infection. The other 14 patients developed late reactions. Ten were diagnosed with probable allergic contact dermatitis to ink, but the suspect allergen was identified in just 3 cases and confirmed in just 1 of these. There were 2 cases of cutaneous sarcoidosis, 1 case of foreign body granuloma, and 1 case of neuropathy. ConclusionsComplications resulting from tattoos are relatively common dermatology complaints. Drawing from our experience, we propose a diagnostic algorithm designed to guide dermatologists in evaluating different reactions to tattoos and prescribing appropriate treatment.

Granuloma formation is a specific type of chronic inflammation initiated by infectious and noninfectious agents with an aggregation of multinucleated giant cells. Foreign bodies can penetrate soft tissues in connection with accidents through open wounds and lacerations. Foreign body giant cells are most commonly observed at the tissue interface where the size of foreign particulate is too large to permit macrophage phagocytosis. A case of foreign body granuloma in the mandible is presented.

Granuloma due to sweet almond oil injection: Difficulties of diagnosis and treatment

<p><span lang="EN-US">Foreign body granuloma reaction is a tissue response to some extraneous materials which incite a characteristic pattern of granulomatous reaction. Several cases of foreign body granulomas on the face have been reported, especially after the injection of dermal fillers. Oleoma or paraffinoma is defined as a foreign body granuloma resulting from the injection of oily substances into the skin or subcutaneous tissue. We report a case of an adult woman who had developed foreign body granulomas due to a self-injection of sweet almond oil into the glabella and periorbital area. The diagnosis was based on a thorough interrogation, clinical features and histopathological findings. Treatment of foreign body granuloma is challenging. At first, oral prednisone was initiated with the improvement of the inflammatory signs and reduction of the lesions’ dimension, but recurrence of the lesions occurred when the dose was decreased. After research of literature, a low-dose minocycline regime was prescribed for its beneficial effects in granulomatous diseases, with encouraging results. <strong></strong></span></p>

Spontaneous lung pathology in a captive common marmoset colony (Callithrix jacchus).

Data on spontaneous pathology are substantially scarce for common marmosets, compared to other laboratory animals, but is essential for the interpretation of histological findings in the context of toxicological and experimental studies. Especially if common marmosets are used as experimental animals in respiratory research, detailed knowledge on the spectrum, occurrence, and incidence of spontaneous histopathological pulmonary lesions in this non-human primate species is required. In this study, lung tissue of 638 common marmosets from the marmoset colony of the German Primate Center was examined histologically. The analysis revealed a high incidence of predominantly mild and multifocal interstitial pneumonia (32.99 %) of unknown etiology in most cases. Only few marmosets exhibited lobar pneumonia (1.41 %) and bronchopneumonia (0.94), which were mainly caused by bacterial pathogens such as Bordetella bronchiseptica and Klebsiella pneumoniae. Lung immaturity and atelectasis were common histological findings in newborn marmosets. Typical background lesions included anthracosis (8.15 %), hemosiderosis (1.72 %), extramedullary hematopoiesis (11.6 %), mineralization (10.97 %), and inflammatory cell foci (10.34 %). In addition, three cases of pulmonary arteriopathy (0.47 %) and 1 case of foreign-body granuloma (0.16 %) were detected in the marmoset study cohort. The high prevalence of circulatory disturbances (congestion, edema, hemorrhage) and changes in air content (secondary atelectasis, alveolar emphysema) could partly be explained by euthanasia-related artifacts or agonal changes. The present study provides a comprehensive overview of the range and incidence of spontaneous pulmonary histopathology in common marmosets, serving as valuable reference data for the interpretation of lung lesions in toxicological and experimental marmoset studies.

Pilomatricoma is a benign skin neoplasm that arises from hair follicle matrix cells. It occurs on the head, neck, and upper extremities, and less frequently on the trunk and lower extremities. The etiology of pilomatricoma is subject to controversy. Trauma, insect bites, or surgery are thought to be antecedent events to the onset of the tumors. Influenza vaccination is a common annual event among individuals. Complications are usually mild and self-limiting. Prophylactic influenza vaccinations rarely cause cutaneous complications. Few cases of foreign body granuloma, infectious reaction, and other cutaneous complications have been reported. In this article, we describe a case of pilomatricoma arising at an influenza vaccination site. Although there has been a report of an association between pilomatricoma and bacille Calmette-Guerin (BCG) vaccination [1], an association with influenza vaccination has not been reported previously. The clinical manifestations and progress of the case are presented in this report. A 7-year-old girl received her routine influenza vaccination in the left upper arm. One month later, a sensation of warmth and redness developed on the vaccination site. Serous discharge was drained with a small amount of blood. Three months later, the patient was referred to the plastic surgery outpatient department, where she presented with a pinkish ulcerative nodule on her left upper arm at the site of administration of the influenza vaccine. At that time, the nodule was covered with a dressing, but it gradually became harder to the touch and larger in size.

Treatment of Foreign Body Granuloma of the Hand Associated with Unregulated Material Injection for Aesthetic Purpose

Recently, rejuvenation of the aging hand has gained popularity. Various medical fillers have recently become popular, and patients have more options to rejuvenate their aging hands. However, in our country, unregulated material injections by unlicensed practitioners are still wide spread and its related complications have been considered as a serious problem. The aim of this retrospective study is to introduce our experiences regarding 17 cases of foreign body granuloma following hand rejuvenation using unregulated material. A retrospective review from December 2002 to October 2012 was performed. We analyzed data for 17 patients who underwent perilesional surgical excision for treatment of a foreign body granuloma of the hand associated with unregulated material injection by unlicensed practitioners. Injected materials include paraffin, silicone oil, and other unknown materials. Four patients were injected with paraffin, 3 patients with silicone oil, and remaining 10 patients with unknown materials. The main symptom was palpable mass and contour deformity. The patients were asked to rate their overall satisfaction following treatment. The average patient satisfaction score was 3.7. To prevent foreign body granuloma following unregulated medical practice, it is very important to inform the public about the disastrous consequence of unregulated material injections by unlicensed practitioners. Long-Term Complications of Polyethylene Glycol Injection to the Face

Currently, filling, smoothing, or recontouring the face through the use of injectable fillers is one of the most popular forms of cosmetic surgery. Because these materials promise a more youthful appearance without anesthesia in a noninvasive way, various fillers have been used widely in different parts of the world. However, most of these fillers have not been approved by the Food and Drug Administration, and their applications might cause unpleasant disfiguring complications. This report describes a case of foreign body granuloma in the cheeks secondary to polyethylene glycol injection and shows the possible complications associated with the use of facial fillers.

Clinical outcome in a series of 173 cases of foreign body granuloma: Improved outcomes with a novel surgical technique

Soft-tissue filler injections have become popular, and injections of even illegal materials are widespread. Complications such as foreign body granuloma often occur in such cases, and appropriate treatment is mandatory but no optimal treatment has been established. We treated 173 patients who underwent surgical excision of foreign body granulomas via direct approach to the lesion (n=121) or injection therapies (n=52) at Kangbuk Samsung Hospital over a period of 7 years from April 2004 to February 2011. A retrospective chart review found that 104 patients had a history of treatment failure at other hospitals. Among these 104 patients, 83 had a history of prior injection therapy and 21 patients had a history of prior surgical therapy. Patient satisfaction was evaluated at our hospital on a scale of 1-5 using an in-house questionnaire. Comparisons between patients receiving injection therapy and patients receiving surgical therapy were made using the Mann-Whitney test. Patients who underwent surgical therapy via direct approach to the lesion reported statistically higher satisfaction scores 4(3-4) than those who underwent injection therapy 3(2-3) (p<0.001). Our novel surgical technique results in better outcomes and patient satisfaction in cases of severe foreign body granuloma.

Horsley wax® is an efficient bone haemostatic agent commonly used in bone surgery. But it is non resorbable and surgeons should be aware of possible complications, which can appear many years later.We report three cases of foreign body granulomas due to bone wax. A 14-years-old female patient with an Apert syndrome underwent Le Fort III osteotomy and distraction osteogenesis; 3 years later, an inflammatory granuloma appeared in the temporal area. A 23-years-old female patient, traffic accident casualty, complained about pain in the hip 1 year after undergoing cervical spine surgery with iliac bone graft. A 35-years-old female patient underwent rhinoplasty with anterior nasal aperture surgery. She presented with an inflammatory reaction requiring several subsequent surgeries.Using Horsley wax® must be performed according to recommendations so as to prevent inflammatory or infectious complications, and to allow good bone healing. Surgeons must mention its use in the surgical report to avoid a delayed diagnosis in case of complications. There are alternatives to bone wax but they are less effective.

Foreign Body Granuloma Associated With Dura-Cranioplasty After Resection of Convexity Meningioma With Extracranial Extension -Case Report-

A 72-year-old man presented with a space-occupying lesion at the site of the prior craniotomy one year after removal of a convexity meningioma with an extracranial extension. The lesion had grown outside the duraplasty with extracranial extension through the degenerative cranioplasty, and was removed. The histological diagnosis was granulation. The original dura-cranioplasty had been performed using Goretex dura substitute, hydroxyapatite cement, and fibrin glue-bonded autologous bone dust. This rare case of foreign body granuloma occurring after craniotomy with dura-cranioplasty indicates that detailed preoperative evaluation of tissue destruction based on neuroimaging is essential for construction of a suitable cranioplasty.

F-18 FDG PET of Foreign Body Granuloma

Retained textiles with foreign body granulomatous reaction can form mass-mimicking soft tissue tumors, which may cause diagnostic, clinical, and medicolegal problems. In this study, we present 3 cases of foreign body granuloma by retained textiles, and demonstrate FDG PET findings with detailed pathologic correlation. We also provide a review of previous literatures. Three patients (all females; age range, 48-61 years) with histopathologic diagnosis of mass-forming foreign body granuloma caused by retained textiles were retrospectively studied. They underwent an FDG PET scan in our institute. The mass was located in the abdominopelvic cavity in all cases. The time interval between the prior surgery and the PET acquisition was 11 months, 15 or 19 years, and 26 years, respectively. Intense FDG uptake with a ring-shaped pattern was observed in 2 cases, while no uptake in 1 case. Microscopically, 2 with ring-shaped FDG uptake showed a cellular foreign body reaction peripherally, and artificial textile fibers and acellular eosinophilic amorphous materials in the central portion. In the one without FDG uptake, the mass was composed of mainly necrotic debris surrounding by a thick layer of well-formed collagen fibers. In previous case reports describing the accumulation of FDG in foreign body granulomas, 9 of 10 cases showed intense FDG uptake with a ring-shaped pattern. The ring-shaped pattern of FDG uptake seems to well characterize pseudotumor with foreign body granulomas. However, the pattern of FDG accumulation in retained textile with foreign body granulomas may vary due to histologic features.
