Vertebral Compression Fracture in the Middle of Fused Segments Without a History of Injury

A case report and review of the literature. To report a rare case of a vertebral compression fracture in the middle of fused segments in the absence of a history of trauma, and to investigate the contributory factors. Few articles have been published on vertebral fractures among fused segments. However, several articles have addressed fractures at juxtafused segments after implant removal. To the best of the authors' knowledge, only one case of compression fracture within fused segments has been previously reported. This study involved the case of a 62-year-old woman with vertebral compression fracture of L4 within a solid fused segment region after implant removal. An investigation was conducted to identify the factors that contributed to this compression fracture. Plain radiographs and bone scanning showed a compression fracture of L4 within a region of fused segments (L3-S1) after implant removal without the history of injury. BMD revealed osteopenia of -2.3 SD on the neck of the femur. The patient's symptoms were relieved by conservative treatment. The contributory factors may have been persistent anterior motion, osteopenia and/or osteoporosis, and a subcortical substance defect associated with screw tracks after implant removal. Vertebral compression fractures in solid fused segments may occur as a complication of implant removal. Accordingly, if spinal implant removal is needed for revision surgery, surgeons should be aware of this significant complication.

Rheumatoid involvement of the cervical spine is considered relatively frequent, but there are relatively few references in the literature about the involvement of the thoracic and lumbar spine in the course of rheumatoid arthritis. Radiological Study of the lumbar spine in 68 patients with rheumatoid arthritis showed that:1. Osteoporosis was seen in 47 patients (69.1%) and compression fractures in 18 patients (26.5%).2. Osteoporosis and compression fractures were frequent in the RA patient who was older than 60-year-old female and more than Class 2 in Stage III and IV.3. Incomplete paralysis occurred in 4 cases of compression fractures and one of them seemed related to rheumatoid involvement.

Experimentelle Untersuchungen über das Frakturverhalten menschlicher Fersenbeine unter vertikalem Druck

In 60 cases typical compression fractures of the calcaneus were achieved by means of static compression of the test preparation. Afterwards the load bearing capacity of the fractured calcaneus was studied. In case of pure vertical compression the calcaneus does not loose much stability. The load bearing capacity of the studied bones after fracture achieved on an average still 63% of the initial value. In adequate cases of compression fractures, not only the early mobilization but the early weight bearing has to be discussed.
