Modification of heart rate variability induced by focal muscle vibration in patients with severe acquired brain injury

ABSTRACT Background/Purpose Heart rate variability (HRV) is a biomarker of autonomic nervous system (ANS) reaction in persons with severe acquired brain injury (sABI) who undergo a rehabilitation treatment, such as focal muscle vibration (FMV). This study aims to evaluate if and how FMV can modulate HRV and to compare potential differences in FMV modulation in HRV between patients with sABI and healthy controls. Methods Ten patients with sABI and seven healthy controls have been recruited. Each individual underwent the same stimulation protocol (four consecutive trains of vibration of 5 minutes each with a 1-minute pause). HRV was analyzed through the ratio of frequency domain heart-rate variability (LF/HF). Results In the control group, after performing FMV, a significant LF/HF difference was observed in the in the second vibration session compared to the POST phase. Patients with SABI treated on the affected side showed a statistically significant LF/HF difference in the PRE compared to the first vibration session. Conclusion These preliminary results suggest that FMV may modify the cardiac ANS activity in patients with sABI.

Mast cell stabilizer, an anti-allergic drug, reduces ventricular arrhythmia risk via modulation of neuroimmune interaction.

Mast cells (MCs) are important intermediates between the nervous and immune systems. The cardiac autonomic nervous system (CANS) crucially modulates cardiac electrophysiology and arrhythmogenesis, but whether and how MC-CANS neuroimmune interaction influences arrhythmia remain unclear. Our clinical data showed a close relationship between serum levels of MC markers and CANS activity, and then we use mast cell stabilizers (MCSs) to alter this MC-CANS communication. MCSs, which are well-known anti-allergic agents, could reduce the risk of ventricular arrhythmia (VA) after myocardial infarction (MI). RNA-sequencing (RNA-seq) analysis to investigate the underlying mechanism by which MCSs could affect the left stellate ganglion (LSG), a key therapeutic target for modulating CANS, showed that the IL-6 and γ-aminobutyric acid (GABA)-ergic system may be involved in this process. Our findings demonstrated that MCSs reduce VA risk along with revealing the potential underlying antiarrhythmic mechanisms.

Alleviation of acute stress response by black pepper aroma administration

BackgroundBlack pepper is one of the most popular spices globally. As black pepper essential oil has not yet been used in the context of aromatherapy, this study examined the effect of black pepper aroma on cardiac and peripheral autonomic nervous system (ANS) activity under stressful conditions using an olfactometer to administer aroma in a precise and controlled manner to ensure reproducibility.MethodsA within-participant design experiment was conducted with 20 male university students who performed a 30-min calculation task as a short-term stressor under three aroma conditions: black pepper, ginger, and dipropylene glycol (DPG) (scentless air as a control). Each aroma was sporadically delivered (first 20 s of each 1-min interval) with the olfactometer during the task. Electrocardiograms and skin conductance level (SCL) were measured to evaluate ANS's physiological acute stress response. Subjective evaluations for the given stressful task and impressions on the types of aromas were assessed.ResultsThe physiological acute stress response induced by the short-term stressor, which is characterized by the enhancement of the heart rate (HR) and SCL and decreases in the heart rate variability (HRV), was suppressed with black pepper: the increase in HR and reduction in HRV from the baseline were 38.9% (p = 0.048 when compared with DPG) and 32.9% smaller (p = 0.002 for multiple comparisons) than those in DPG, respectively, and the increase of SCL was 15.5% smaller (p = 0.005 for multiple comparisons) than that in ginger. However, there was no significant difference in subjective scores among the conditions.ConclusionAlthough black pepper is a stimulative agent, the study findings showed that black pepper aroma alleviated the physiological acute stress response, which can be beneficial in aromatherapy under stressful conditions.

Heart Rate Variability Biofeedback in Adults with a Spinal Cord Injury: A Laboratory Framework and Case Series.

Heart rate variability biofeedback (HRV-F) is a neurocardiac self-regulation therapy that aims to regulate cardiac autonomic nervous system activity and improve cardiac balance. Despite benefits in various clinical populations, no study has reported the effects of HRV-F in adults with a spinal cord injury (SCI). This article provides an overview of a neuropsychophysiological laboratory framework and reports the impact of an HRV-F training program on two adults with chronic SCI (T1 AIS A and T3 AIS C) with different degrees of remaining cardiac autonomic function. The HRV-F intervention involved 10 weeks of face-to-face and telehealth sessions with daily HRV-F home practice. Physiological (HRV, blood pressure variability (BPV), baroreflex sensitivity (BRS)), and self-reported assessments (Fatigue Severity Scale, Generalised Anxiety Disorder Scale, Patient Health Questionnaire, Appraisal of Disability and Participation Scale, EuroQol Visual Analogue Scale) were conducted at baseline and 10 weeks. Participants also completed weekly diaries capturing mood, anxiety, pain, sleep quality, fatigue, and adverse events. Results showed some improvement in HRV, BPV, and BRS. Additionally, participants self-reported some improvements in mood, fatigue, pain, quality of life, and self-perception. A 10-week HRV-F intervention was feasible in two participants with chronic SCI, warranting further investigation into its autonomic and psychosocial effects.

Fit-Fat Index is better associated with heart rate variability compared to fitness and fatness alone as indicators of cardiometabolic human health.

Cardiorespiratory fitness and fatness indicators have been related to heart rate variability (HRV) parameters. The Fit-Fat Index (FFI) is a single index combining cardiorespiratory fitness and fatness indicators. To the best of our knowledge, no studies have previously analyzed whether FFI are related to cardiac autonomic nervous system activity assessed through HRV parameters. This study aimed (i) to examine the association of cardiorespiratory fitness, fatness indicators, and FFI with HRV parameters; and (ii) to report what of the different fatness indicators included in FFI is better associated with HRV parameters in sedentary adults. One hundred and fifty healthy adults (74 women; 76 men), aged between 18 and 65 years old, participated in this cross-sectional study. We measured cardiorespiratory fitness (maximal oxygen consumption) and fatness indicators (waist-to-height ratio [WHR], fat mass percentage [FM%] and visceral adipose tissue [VAT]). Three FFIs were calculated as the quotient between cardiorespiratory fitness and one out of three possible fatness indicators: Fit-Fat Index calculated waist-to-height ratio (FFIWHR ), Fit-Fat Index calculated with FM% (FFIFM% ), and Fit-Fat Index calculated with VAT (FFIVAT ). HRV parameters were measured in resting conditions using a Polar RS800CX. FFIWTHR , FFIFM% and FFIVAT were related to different HRV parameters (β ranges between -0.507 and 0.529; R2 ranges between 0.096 and 0.275; all p < .001) and the association was stronger with HRV parameters than the isolated fitness or fatness indicators (β ranges between -0.483 and 0.518; R2 ranges between 0.071 and 0.263; all p < .001). FFIVAT was the index more consistently associated with HRV parameters (β ranges between -0.507 and 0.529; R2 ranges between 0.235 and 0.275; all p < .001). Our study suggests that compound FFIs are better predictors of HRV parameters than either cardiorespiratory fitness or fatness indicators alone. The FFIVAT was the best index in terms of its association to HRV.

0801 Predictive value of heart rate variability on BP outcomes in OSA status – a 10yr longitudinal study

Abstract Introduction The apnoea/hypopnoea index (AHI) is commonly used as the indicator of OSA severity, but it cannot fully describe the illness severity, comorbidities, or treatment response. Heart rate variability (HRV) is a non-invasive way of detecting cardiac autonomic nervous system activity variations. It adds a new dimension to help understand the interplay between cardiac and nervous systems involvement in OSA. Aims: To explore the associations of HRV with OSA status in subjects from a longitudinal study where measurements were performed at 4-yr and 10-yr follow-up visits. Methods The cohort was established in 2003 to examine the prevalence of OSA in primary school children. During the 4-yr follow-up, HRV data in the time domain and frequency domain for 243 subjects when they were 10-17 years old were collected. Participants were invited to return for the 10-yr follow-up visit to undergo anthropometric measurements, overnight sleep study, and 24-hour ambulatory blood pressure (ABP) monitoring. Results Two hundred forty-three participants returned for the 10-yr follow-up. Participants who completed the HRV measurements at the 4-yr follow-up were included in this preliminary analysis. There were 122 participants (48.4% with OSA). Participants with OSA (AHI &amp;gt;=5 event/h) at the 10-yr follow-up had a higher high frequency (HF) norm, a kind of HRV parameter, at the 4-yr follow-up than those without OSA (40.5 ±22.1n.u.) vs (33.7 ±15.2n.u.) (P = 0.030). Conclusion HRV parameters combined with OAHI at 4yr likely partially explain SBP, and it can better predict SBP at 10yr than independent OAHI. HRV is prone to have an interaction effect, instead of a mediation effect, to help us predict SBP. Support (if any)

Effects of man-made electromagnetic fields on heart rate variability parameters of general public: a systematic review and meta-analysis of experimental studies.

The effects of man-made electromagnetic fields (EMFs) on the cardiovascular system have been investigated in many studies. In this regard, the cardiac autonomic nervous system (ANS) activity due to EMFs exposure, assessed by heart rate variability (HRV), was targeted in some studies. The studies investigating the relationship between EMFs and HRV have yielded conflicting results. We performed a systematic review and meta-analysis to assess the data's consistency and identify the association between EMFs and HRV measures. Published literature from four electronic databases, including Web of Science, PubMed, Scopus, Embase, and Cochrane, were retrieved and screened. Initially, 1601 articles were retrieved. After the screening, 15 original studies were eligible to be included in the meta-analysis. The studies evaluated the association between EMFs and SDNN (standard deviation of NN intervals), SDANN (Standard deviation of the average NN intervals for each 5 min segment of a 24 h HRV recording), and PNN50 (percentage of successive RR intervals that differ by more than 50 ms). There was a decrease in SDNN (ES=-0.227 [-0.389,-0.065], p=0.006), SDANN (ES=-0.526 [-1.001,-0.05], p=0.03) and PNN50 (ES=-0.287 [-0.549,-0.024]). However, there was no significant difference in LF (ES=0.061 (-0.267, 0.39), p=0.714) and HF (ES=-0.134 (0.581, 0.312), p=0.556). In addition, a significant difference was not observed in LF/HF (ES=0.079 (-0.191, 0.348), p=0.566). Our meta-analysis suggests that exposure to the environmental artificial EMFs could significantly correlate with SDNN, SDANN, and PNN50 indices. Therefore, lifestyle modification is essential in using the devices that emit EMs, such as cell phones, to decrease some signs and symptoms due to EMFs' effect on HRV.

Nine Months of Hybrid Intradialytic Exercise Training Improves Ejection Fraction and Cardiac Autonomic Nervous System Activity.

Cardiovascular disease is the most common cause of death in hemodialysis (HD) patients. Intradialytic aerobic exercise training has a beneficial effect on cardiovascular system function and reduces mortality in HD patients. However, the impact of other forms of exercise on the cardiovascular system, such as hybrid exercise, is not clear. Briefly, hybrid exercise combines aerobic and strength training in the same session. The present study examined whether hybrid intradialytic exercise has long-term benefits on left ventricular function and structure and the autonomous nervous system in HD patients. In this single-group design, efficacy-based intervention, twelve stable HD patients (10M/2F, 56 ± 19 years) participated in a nine-month-long hybrid intradialytic training program. Both echocardiographic assessments of left ventricular function and structure and heart rate variability (HRV) were assessed pre, during and after the end of the HD session at baseline and after the nine-month intervention. Ejection Fraction (EF), both assessed before and at the end of the HD session, appeared to be significantly improved after the intervention period compared to the baseline values (48.7 ± 11.1 vs. 58.8 ± 6.5, p = 0.046 and 50.0 ± 13.4 vs. 56.1 ± 3.4, p = 0.054 respectively). Regarding HRV assessment, hybrid exercise training increased LF and decreased HF (p < 0.05). Both conventional Doppler and tissue Doppler imaging indices of diastolic function did not change after the intervention period (p > 0.05). In conclusion, long-term intradialytic hybrid exercise training was an effective non-pharmacological approach to improving EF and the cardiac autonomous nervous system in HD patients. Such exercise training programs could be incorporated into HD units to improve the patients' cardiovascular health.

Sex differences in estimates of cardiac autonomic function using heart rate variability: effects of dietary capsaicin.

Heart rate variability (HRV) estimates the autonomic nervous system (ANS) influence on the heart and appears sex-specific. Sensory afferents exhibit sex-specificity; although, it is unknown if Capsaicin, an agonist for transient receptor potential vanilloid channel-1 (TRPV1), alters cardiac ANS activity in a sex-dependent manner, which could be important given the predictive nature of HRV on risk of developing hypertension. Thus, we explored if there was sex-specificity in the effect of capsaicin on estimated cardiac ANS activity. HRV was measured in 38 young males (M: n = 25) and females (F: n = 13), in a blinded-crossover design, after acute ingestion of placebo or capsaicin. Resting HR, RR-interval, root-mean-square of successive differences (RMSSD), natural log-transformed RMSSD (LnRMSSD), standard deviation of n-n intervals (SDNN), number of pairs of successive n-n intervals differing by > 50ms (NN50), and percent NN50 (PNN50) were obtained using standard techniques. Significant sex differences were observed in mean HR (M: 59 ± 9.3 vs. F: 65 ± 12 beats/min, p = 0.036, η2 = 0.098), minimum HR (M: 47 ± 8.3 vs. F: 56 ± 12 beats/min, p = 0.014, η2 = 0.124), and NN50 (M: 177 ± 143 vs. F: 29 ± 17, p < 0.001, η2 = 0.249). There was a significant interaction of sex*treatment (p = 0.02, η2 = 0.027) for RMSSD, where males increased (78 ± 55 vs. 91 ± 64ms), and females decreased (105 ± 83 vs. 76 ± 43ms), placebo vs. capsaicin. This controlled study recapitulates sex differences in HR and HRV, but revealed a sexual dimorphism in the parasympathetic response to capsaicin, perhaps due to differing TRPV1-afferent sensitivity, highlighting a potential mechanism for differential regulation of hemodynamics, and CVD risk, and should beconsidered in future studies.

This study aimed to investigate the effect of flavored mouthwash on peripheral and cardiac autonomic nervous system activity. In a within-subject experimental design, 10 male and 10 female healthy adults went through two types of mouthwashes, one was with citrus mint flavor and another was with spice mint flavor, right after performing a 20-minute calculation task given as a cognitive stressor. Along with subjective evaluation, skin conductance and electrocardiogram was recorded throughout the experiment. Although there was no difference in the subjective stress levels between the two conditions, the level of skin conductance decreased more remarkably and significantly during the recovery period when the participants used citrus mint mouthwash. The findings suggest that a particular flavored mouthwash may provide physiological effect on user regardless of its subjective sensation and the citrus mint type mouthwash might be better suited for use in need of a relaxation.

Effects of an 18-Week Integrated Yoga Program on Cardiac Autonomic Function in Breast Cancer Patients Undergoing Adjuvant Chemotherapy: A Randomized Controlled Trial.

Cardiotoxicity is a commonly observed adverse effect seen in breast cancer (BC) patients undergoing chemotherapy with attributes toward cardiac autonomic dysfunction (CAD). Yoga, a mind-body system of medicine that has been shown to improve cardiac autonomic nervous system (ANS) activity in various health conditions, could be an effective adjuvant approach in addressing CAD. This study aims to investigate the protective effects of Integrated Yoga Therapy (IYT) on ANS functioning, assessed using Heart rate variability (HRV) in breast cancer patients undergoing chemotherapy. A total of 68 (stage I-III) BC patients were randomly assigned into 2 groups: Treatment as Usual group (TAU) and TAU with Yoga Therapy group (TAUYT). All patients underwent anthracycline-based adjuvant chemotherapy for a total of 6 cycles with 21 days/cycle. During chemotherapy, the TAUYT group received IYT 5 days a week for 18 weeks, compared with usual care alone in the TAU group. Resting heart rate (RHR) and HRV, measured in both the time and frequency domains, were used to assess the cardiac ANS function of each patient before and after 6 cycles of chemotherapy. A total of 30 subjects in the TAU group and 29 subjects in the TAUYT group were included in the analysis. At baseline (before chemotherapy), there were no significant differences between the TAU and TAUYT groups in terms of RHR and HRV indices. However, after chemotherapy, patients in the TAU group had a significantly higher average RHR (P < .02) and lower HRV indices with reduced parasympathetic indices: RMSSD (P < .01), pNN50% (P < .04), high-frequency power (P < .001) and increased sympathetic indices: low-frequency power (P < .001) with sympathovagal imbalance: LF/HF (P < .001) compared with patients in the TAUYT group. The study showed the protective effects of yoga therapy on CAD in patients receiving anthracycline-based chemotherapy for BC, proposing yoga as a potential adjuvant intervention in improving cardiac health and preventing cardiovascular-related morbidities. This trial is registered with the Clinical Trials Registry-India (CTRI) database (CTRI/2020/10/028446; October 16, 2020).

Alterations in heart rate variability before and after percutaneous coronary intervention in patients with ischaemic heart disease

Objectives: Heart rate variability (HRV) is the beat-to-beat variation of heart rate and an important non-invasive tool to study the cardiac autonomic nervous system activity. Ischaemic heart disease (IHD) patients are often considered to have a state of sympathetic predominance. Elimination of myocardial ischaemia with percutaneous coronary intervention (PCI) could restore autonomic balance. Hence, this study aimed to evaluate HRV measures, after successful revascularisation among IHD patients. Materials and Methods: A total of 145 male IHD patients planned for coronary angiography were recruited. Fifteen minutes of electrocardiogram recording was done. Short-term HRV analyses of all the patients were acquired and time-domain indices (standard deviation of normal-to-normal intervals [SDNN], root-mean-squared differences of successive R-R intervals [rMSSD]) and frequency-domain indices (low-frequency [LF] power, high-frequency [HF] power and total power [TP]) were analysed. Before and after PCI, all measurements were made. Results: This study included 145 male IHD patients with the mean age of 54.57 ± 10.81 years. There was a significant decrease in SDNN, rMSSD, LF, HF and TP whereas increase in LF/HF ratio following revascularisation. These changes in measures of neurocardiac regulation correlated with risk factors and medication on regression analysis. Conclusion: Increase in LF/HF ratio with the decrease in SDNN, rMSSD, LF, HF and TP in patients with IHD who underwent coronary intervention infers changed cardiac autonomic balance with this surgical procedure. These measures could be potentially used as prognostic measures in the future studies.

Effect of Algorithmic Music Listening on Cardiac Autonomic Nervous System Activity: An Exploratory, Randomized Crossover Study.

It is proven that music listening can have a therapeutic impact in many clinical fields. However, to assume a curative value, musical stimuli should have a therapeutic logic. This study aimed at assessing short-term effects of algorithmic music on cardiac autonomic nervous system activity. Twenty-two healthy subjects underwent a crossover study including random listening to relaxing and activating algorithmic music. Electrocardiogram (ECG) and non-invasive arterial blood pressure were continuously recorded and were later analyzed to measure Heart Rate (HR) mean, HR variability and baroreflex sensitivity (BRS). Statistical analysis was performed using a general linear model, testing for carryover, period and treatment effects. Relaxing tracks decreased HR and increased root mean square of successive squared differences of normal-to-normal (NN) intervals, proportion of interval differences of successive NN intervals greater than 50 ms, low-frequency (LF) and high-frequency (HF) power and BRS. Activating tracks caused almost no change or an opposite effect in the same variables. The difference between the effects of the two stimuli was statistically significant in all these variables. No difference was found in the standard deviation of normal-to-normal RR intervals, LFpower in normalized units and LFpower/HFpower variables. The study suggests that algorithmic relaxing music increases cardiac vagal modulation and tone. These results open interesting perspectives in various clinical areas.

Effects of a Combined Intradialytic Exercise Training Program and Music on Cardiac Autonomic Nervous System Activity in Hemodialysis Patients.

This study aimed to examine the effect of an intradialytic exercise program in combination with music on heart rate variability (HRV) indices and functional capacity in patients on maintenance hemodialysis (HD). Methods: Forty HD patients were randomized to four training groups for six months: the combined music and exercise group (Group A), the exercise group (Group B), the music group (Group C), and the control group (Group D). At baseline and after 6 months, all participants underwent both short- (for 30 min) and long- (for 24 h) term measurements of HRV and functional capacity assessment with a 6 min walking test (6MWT). Patients of groups A and C listened to preferred music. Results: Long-term HRV analysis showed that standard deviation of all normal-to-normal RR intervals (SDNN) and the square root of the mean squared differences of successive RR intervals (rMSSD) were significantly higher at the end of the study in groups A (by 13.2% and 47.3%), B (by 15.1% and 50%), and C (by 9.0% and 30.1%), compared to group D (p < 0.05). Values of rMSSD and percentage of RR intervals differing by more than 50 ms from the preceding RR interval (pNN50) were elevated in groups A (by 35.6% and 142.9%), B (by 36.1% and 75%), and C (by 15.2% and 28.6%), compared to baseline measurements (p < 0.05). Also, pNN50 was increased in group A compared to groups B (by 21.4%), C (by 88.9%), and D (by 142.9%) (p < 0.05). Similar results were noted by short-term HRV analysis. Functional capacity was improved at the end of the 6-month study in groups A (by 20.3% and 25.7%) and B (by 15.8% and 21.1%) compared to groups C and D (p < 0.05). Conclusions: Intradialytic exercise combined with music-listening can improve the functional capacity and cardiac autonomic nervous system activity in hemodialysis patients.

Sex Differences in Estimates of Cardiac Autonomic Function Using Time Domain based Method of Heart Rate Variability: Effects of Oral Capsaicin

Heart rate variability (HRV) estimates autonomic nervous system (ANS) influence on the heart and is sex‐specific. Sensory afferents exhibit sex‐specificity; although, there is a paucity of data on the potential effects of Capsaicin, an agonist for transient receptor potential vanilloid channel‐1 (TRPV1), on cardiac ANS activity and if the effect is sex‐dependent. Given the predictive nature of HRV on risk of developing hypertension, understanding the sex differences in factors governing HRV is paramount.PURPOSETherefore, this study sought to determine the sex‐specificity in the effect of capsaicin on cardiac autonomic function estimated through HRV. The physiological ramifications of these findings, specifically in the regulation of blood pressure and ultimately cardiovascular disease risk remain to be explored, especially in the transition to menopause in women.

Obesity and heart rate variability: A cross-sectional study in obese young adults

CONTEXT: Obesity is one of the important causative factors for multiple co-morbid conditions that can lead to metabolic and cardiac disorders. Many research studies indicate the association of obesity and sudden cardiac deaths.AIMS: This study was aimed to evaluate the cardiac autonomic nervous system activity, given by the changes in heart rate variability (HRV) in healthy obese young adults.SETTINGS AND DESIGN: This cross-sectional study included thirty obese young adult males of 18–25 years and 30 age-matched healthy adult males. Power spectral analysis of HRV was used to determine the autonomic imbalance.SUBJECTS AND METHODS: HRV was performed using HRV software (Lab Chart 7). Numerical data of absolute values of low frequency (LF) and high frequency (HF) (in ms2), normalized units of LF and HF (in nu) and LF/HF ratio were expressed as mean ± standard deviation and analyzed.RESULTS: On statistical analysis, it was found that HF in normalized units was significantly low in obese young adults compared to age-matched controls. LF (nu) and LF/HF ratio were higher in obese subjects compared to controls, though they were not statistically significant. Although there was negative correlation between HRV parameters and body mass index in obese individuals, it was not significant.CONCLUSIONS: These results suggest that obese young adults present with changes in the autonomic nervous system, characterized by decrease in parasympathetic activity.

Motor Imagery and Frontal Head Oxygenation: An fNIRS Study.

Motor imagery (MI) is a manifestation of mental movements, but it cannot be identified visually. Therefore, to a large extent, MI assessment has not yet been established. The present study aimed to investigate whether frontal oxy-Hb changes and cardiac autonomic nervous system activity during MI are associated with the psychometric scale assessment of MI and clarify the utility of each index in MI assessment. Thirty-one healthy men and women were included in this study, and Pocket NIRS Duo was used to assess frontal oxygenated hemoglobin levels during walking MI. Simultaneously, heart rate and sympathetic index (low and high frequency (LF/HF) during MI were evaluated using Chiryou Meijin, a heart rate frequency analyser. In addition, a psychometric scale evaluation was carried out in MC and VAS, and its correlation with oxy-Hb levels, heart rate (HR), and LF/HF was investigated. HRs and LF/HF during MI were significantly increased compared with those at rest. However, oxy-Hb levels during MI were not increased. There was a significant correlation between right oxy-Hb levels and mental chronometry (MC) during MI (r=-0.3, p<0.05). HR and LF/HF were not correlated with MC. VAS was not correlated with oxy-Hb levels, HR, or LF/HF. The results of this study confirm an association between MI performance and frontal oxy-Hb changes and that brain activity is not necessarily elevated during MI. HR were significantly increased but did not show any association with MC.

Heart rate variability in patients with tetralogy of Fallot

Abstract Background Heart rate variability (HRV) can be used to evaluate cardiac autonomic nervous system activity. In the adult population decreased HRV has been associated with increased mortality in patients with cardiac disease, however, application of HRV in patients with congenital heart disease is not well established. Purpose To describe and compare HRV parameters obtained by clinical Holter monitoring in patients with Tetralogy of Fallot (TOF) and healthy controls and to evaluate its association with different clinical outcomes. Methods Data from 489 patients were obtained; 150 had undergone repair of TOF and 339 were healthy controls. Using propensity score matching (PSM), 149 TOF patients were matched for sex, age at Holter,body mass index, use of beta-blockers, and ectopic beats with 149 healthy patients. Minimum, maximum, average heart rate and time domain HRV parameters (ASDNN5, SDANN5, SDNN, RMSSD, PNN50) and clinical outcomes (arrhythmias, hospital admission for cardiac disease, and functional class) were compared between groups using simple and multiple linear regression models as well as logistic regression analysis. Results No significantdifference was found in time domain HRV parameters between the two groups, though average (−7.0253, p=0.000) and maximum (−15.8585, p=0.000) heart rate values were significantly lower in patients with TOF compare with controls. Logistic regression analysis showed an association between maximum heart rate values and the development of arrhythmias (OR=0.9817, p=0.013), as well as the functional class (OR=0.9706, p=0.03). Conclusion Average and maximum heart rates are significantly lower in patients with repaired TOF compared to the control population. Time domain HRV parameters were not associated with clinical outcomes in patients with TOF when compared to healthy controls. Further investigation assessing frequency domain parameters may provide further insight. Funding Acknowledgement Type of funding sources: None.

Effortful listening: Sympathetic activity varies as a function of listening demand but parasympathetic activity does not

Research on listening effort has used various physiological measures to examine the biological correlates of listening effort but a systematic examination of the impact of listening demand on cardiac autonomic nervous system activity is still lacking. The presented study aimed to close this gap by assessing cardiac sympathetic and parasympathetic responses to variations in listening demand. For this purpose, 45 participants performed four speech-in-noise tasks differing in listening demand—manipulated as signal-to-noise ratio varying between +23 dB and -16 dB—while their pre-ejection period and respiratory sinus arrythmia responses were assessed. Cardiac responses showed the expected effect of listening demand on sympathetic activity, but failed to provide evidence for the expected listening demand impact on parasympathetic activity: Pre-ejection period reactivity increased with increasing listening demand across the three possible listening conditions and was low in the very high (impossible) demand condition, whereas respiratory sinus arrythmia did not show this pattern. These findings have two main implications. First, cardiac sympathetic responses seem to be the more sensitive correlate of the impact of task demand on listening effort compared to cardiac parasympathetic responses. Second, very high listening demand may lead to disengagement and correspondingly low effort and reduced cardiac sympathetic response.

Cardiac Autonomic Nervous System Activity during Slow Breathing in Supine Position.

The purpose of this study is to clarify cardiac autonomic nervous system activity during slow breathing exercises in a supine position. Eighteen healthy young males were participated. Heart rate variability was measured for 5 minutes at rest, 5 minutes at slow breathing, and then 5 minutes at rest. As a result, the LF/HF ratio increased with slow breathing, but HF value did not change. We suggest that the increased LF/HF ratio may be due to increased airway resistance. Cardiac autonomic nervous system activity during slow breathing in the supine position was revealed.
