Implementation and evaluation of the AAPM TG 111 for radiation dosimetry in a state-of-the-art C-arm cone beam CT system

The aim of this study was to assess the dosimetric characteristics of a state-of-the-art C-arm cone beam computed tomography (CBCT) system using the methodology proposed by the American Association of Physicists in Medicine (AAPM) Task Group (TG) 111. The dose measurement methodology described in AAPM TG 111 for wide cone beam acquisitions without table translation was employed to estimate equilibrium beam length () and equilibrium dose () in various interventional task-specific protocols with different tube arc projection geometries. Dose profiles were derived from point dose measurements in the centre and peripheral locations of the ICRU/AAPM and standard polymethyl methacrylate (PMMA) body phantom. Dose measurements were performed in phantom and free-in-air using a solid-state point detector. Monte Carlo (MC) based simulation dosimetry was performed to quantify the inhomogeneous dose patterns imparted in the phantoms. Estimated and on the ICRU/AAPM phantom was up to 49.4 cm and 6.17 mGy/100 mAs, respectively. Corresponding values determined on the PMMA phantom were 139 cm and 8.8 mGy/100 mAs, respectively. Free-in-air dose measurement ranged from 1.43 mGy/100 mAs to 5.93 mGy/100 mAs. Per cent difference in and between MC simulation and solid-state point detector measurement methods in the ICRU/AAPM phantom were within 16% and 18%, respectively. Manufacturers can use the presented methodology to characterize the dosimetric properties of C-arm CBCT systems. Clinical medical physicists may follow this methodology to verify corresponding data provided by the manufacturer and check for C-arm CBCT system performance dosimetric consistency.

Source-detector trajectory optimization for CBCT metal artifact reduction based on PICCS reconstruction.

Precise instrument placement plays a critical role in all interventional procedures, especially percutaneous procedures such as needle biopsies, to achieve successful tumor targeting and increased diagnostic accuracy. C-arm cone beam computed tomography (CBCT) has the potential to precisely visualize the anatomy in direct vicinity of the needle and evaluate the adequacy of needle placement during the intervention, allowing for instantaneous adjustment in case of misplacement. However, even with the most advanced C-arm CBCT devices, it can be difficult to identify the exact needle position on CBCT images due to the strong metal artifacts around the needle. In this study, we proposed a framework for customized trajectory design in CBCT imaging based on Prior Image Constrained Compressed Sensing (PICCS) reconstruction with the goal of reducing metal artifacts in needle-based procedures. We proposed to optimize out-of-plane rotations in three-dimensional (3D) space and minimize projection views while reducing metal artifacts at specific volume of interests (VOIs). An anthropomorphic thorax phantom with a needle inserted inside and two tumor models as the imaging targets were used to validate the proposed approach. The performance of the proposed approach was also evaluated for CBCT imaging under kinematic constraints by simulating some collision areas on the geometry of the C-arm. We compared the result of optimized 3D trajectories using the PICCS algorithm and 20 projections with the result of a circular trajectory with sparse view using PICCS and Feldkamp, Davis, and Kress (FDK), both using 20 projections, and the circular FDK method with 313 projections. For imaging targets 1 and 2, the highest values of structural similarity index measure (SSIM) and universal quality index (UQI) between the reconstructed image from the optimized trajectories and the initial CBCT image at the VOI was calculated 0.7521, 0.7308 and 0.7308, 0.7248 respectively. These results significantly outperformed the FDK method (with 20 and 313 projections) and the PICCS method (20 projections) both using the circular trajectory. Our results showed that the proposed optimized trajectories not only significantly reduce metal artifacts but also suggest a dose reduction for needle-based CBCT interventions, considering the small number of projections used. Furthermore, our results showed that the optimized trajectories are compatible with spatially constrained situations and enable CBCT imaging under kinematic constraints when the standard circular trajectory is not feasible.

Assessment of abdominal organ dose and image quality in varying arc trajectory interventional C-arm cone beam CT

PurposeThe aim of this study was to investigate the effect of varying arc exposure trajectory on radiation dose to radiosensitive organs and to assess image quality in abdominal C-arm cone beam computed tomography (CBCT) interventional procedures using a latest generation system. MethodsAn anthropomorphic phantom that simulates the average adult individual was used. Individual-specific Monte Carlo (MC) simulation dosimetry was performed to estimate organ doses (OD) in abdominal C-arm CBCT. Seven examination protocols prescribed by the system for vascular and soft tissue CBCT, were simulated. These protocols are differentiated in the range of the arc exposure trajectory and the level of radiation dose delivered to the patient. OD was estimated for liver, adrenals, kidneys, pancreas, stomach, gall bladder, spleen, bone and skin. Image noise, signal to noise ratio (SNR), contrast to noise ratio (CNR) and in-plane spatial resolution were assessed using CT-specific image quality assessment phantoms. ResultsOD was found to depend on the range of arc trajectory and was higher for posterior located organs. In vascular protocols OD ranged from 4.75 mGy for skin to 0.60 mGy for bone. Image noise was higher in vascular protocols than in soft tissue ones. SNR and CNR were significantly modified among different soft tissue protocols (P < 0.05). In-plane spatial resolution was found 0.80 lp/mm in vascular as opposed to 0.41 lp/mm in soft tissue protocols. ConclusionsThe current results may be used to estimate OD for different examination protocols and enable operators choose the appropriate acquisition protocol on the preprogrammed interventional task.

The Impact of the Use of C-Arm Cone-Beam CT During Chemoembolization for Hepatocellular Carcinoma.

The objective of the study was to investigate the consequences of using CArm Cone-Beam computed Tomography (CBCT) on super-selective catheterization of Hepatic Artery (HA) branches during chemoembolization of hepatocellular carcinoma. Two groups of patients were created according to the dates of their treatment sessions. Group A and Group B included patients who had their treatment sessions in 2004 - 2005 and 2008 - 2010, respectively. The sessions performed in 2006 and 2007 were excluded to allow for the adoption and incorporation of CBCT imaging into clinical practice. All chemoembolized HA branches were categorized according to selection order (0-1, 2, or ≥3). Other procedure variables were documented. A total of 58 and 183 sessions were included in Groups A and B, respectively, for 144 patients. C-arm CBCT was used in 2 (3%) sessions and 142 (78%) sessions in groups A and B, respectively. The average number of vessels treated was significantly higher in group B (1.8) compared to group A (1.3) (P < .0001). A shift to an increased selection order in group B (0-1, 44 [24%]; 2, 85 [46%]; ≥3, 54 [30%]) was more significant (P = .0004) than that in group A (0-1, 32 [55%]; 2, 18 [31%]; ≥3, 8 [14%]). The average duration of the procedure was significantly longer in group B (P = .0002). Using C-arm CBCT during chemoembolization has a positive impact on increasing the number and order of HA selected and chemoembolized. This comes at the expense of an increase in the duration of the procedure.

C-Arm Cone-Beam CT Virtual Navigation versus Conventional CT Guidance in the Transthoracic Lung Biopsy: A Case-Control Study.

C-arm cone-beam computed tomography (CBCT) virtual navigation-guided lung biopsy has been developed in the last decade as an alternative to conventional CT-guided lung biopsy. This study aims to compare the biopsy accuracy and safety between these two techniques and explores the risk factors of biopsy-related complications. A total of 217 consecutive patients undergoing conventional CT- or C-arm CBCT virtual navigation-guided lung biopsy from 1 June 2018 to 31 December 2019 in this single-center were retrospectively reviewed. Multiple factors (e.g., prior emphysema, lesion size, etc.) were compared between two biopsy techniques. The risk factors of complications were explored by using logistic regression. The patients’ median age and male-to-female ratio were 63 years and 2.1:1, respectively. Eighty-two (82) patients (37.8%) underwent conventional CT-guided biopsies, and the other 135 patients (62.2%) C-arm CBCT virtual navigation-guided biopsies. Compared with patients undergoing C-arm CBCT virtual navigation-guided lung biopsies, patients undergoing conventional CT-guided lung biopsies showed higher needle repositioning rate, longer operation time, and higher effective dose of X-ray (52.4% vs. 6.7%, 25 min vs. 15 min, and 13.4 mSv vs. 7.6 mSv, respectively; p < 0.001, each). In total, the accurate biopsy was achieved in 215 of 217 patients (99.1%), without a significant difference between the two biopsy techniques (p = 1.000). The overall complication rates, including pneumothorax and pulmonary hemorrhage/hemoptysis, are 26.3% (57/217), with most minor complications (56/57, 98.2%). The needle repositioning was the only independent risk factor of complications with an odds ratio of 6.169 (p < 0.001). In conclusion, the C-arm CBCT virtual navigation is better in percutaneous lung biopsy than conventional CT guidance, facilitating needle positioning and reducing radiation exposure. Needle repositioning should be avoided because it brings about more biopsy-related complications.

Survival impact of C-Arm cone-beam computed tomography on hepatocellular carcinoma patients undergoing chemoembolization

Objectives: The objectives of the study were to evaluate the use of C-arm cone-beam computed tomography (CBCT) for tumor targeting for transarterial chemoembolization (TACE) and its impact on overall survival (OS) in hepatocellular carcinoma patients. Material and Methods: Two groups were retrospectively evaluated according to the date of the first TACE session before and after C-arm CBCT installation in late 2005 (group A [n = 34], 2004–2005; group B [n = 104], 2008+). The years 2006 and 2007 were excluded to allow for the incorporation of this new imaging technology into clinical practice. The vessel selection order was recorded for all TACE sessions. Univariate and multivariate analyses were performed to assess the impact on and predictors of survival. Results: The average TACE selection order for each patient was significantly higher in group B than in group A (P &lt; 0.0001). The median OS was significantly longer in group B (29.34 months) than in group A (19.65 months; P = 0.0088), and the difference in duration was most pronounced in patients with tumor burdens &lt; 25% (n = 93; P = 0.0075), in whom the 3-year survival rate was 56.1% in group B and 15.3% in group A. In these 93 patients, the OS was significant longer (P = 0.018) for high (41.07 months) versus low (19.65 months) vessel selection order across both groups. In multivariate analyses, both the period in which TACE was performed (P = 0.022) and the use of C-arm CBCT (P = 0.0075) were significant predictors of improved OS. Conclusion: Use of advanced C-arm CBCT during TACE enhances the operating physician’s ability to deliver targeted, effective therapy for hepatocellular carcinoma, an aggressive approach that favorably impacts survival.

Is dual-phase C-arm CBCT sufficiently accurate for the diagnosis of colorectal cancer liver metastasis during liver intra-arterial treatment?

This study aimed to estimate the accuracy of dual-phase C-arm cone beam computed tomography (CBCT) for the detection of colorectal cancer liver metastases, as compared with multidetector computed tomography (MDCT). Between March 2014 and December 2016, 49 consecutive patients referred for intra-arterial treatment for colorectal cancer liver metastases were enrolled in a single-center observational study. All patients were examined with MDCT and with dual-phase C-arm cone beam computed tomography performed after iodine injection in the proper hepatic artery before intra-arterial treatment. Two blinded observers independently reviewed all examinations. Diagnostic accuracy was determined using both a six-cell matrix method and a "worst-case scenario." Readers identified at MDCT 264 colorectal liver metastases and 43 other liver lesions. The early and late arterial phase showed 240 and 277 liver lesions respectively. A certainty of the diagnosis was obtained in 63% and 85% at the early (EAP) and late arterial phase (LAP), respectively. Streak artifacts or liver segment truncation, or inadequate enhancement was responsible for the inability to see or to correctly adjudicate a lesion to a diagnosis in 27% and 15% of the cases at the EAP and LAP. The "worst-case scenario" yielded a Se and Sp of 58% and 51%, respectively, at EAP and 84% and 70%, respectively, at LAP. On CBCT, EAP showed limited accuracy. LAP provided the best tumor detectability. • The early arterial phase (EAP) yielded poor accuracy: Se = 58% and Sp = 51% (p < 0.0001). • The late arterial phase (LAP) phase yielded good accuracy: Se = 84% and Se = 70% (p = 0.02). • The probability of a correct diagnosis at the EAP was 60%.

ESTIMATION OF PATIENT LENS DOSE ASSOCIATED WITH C-ARM CONE-BEAM COMPUTED TOMOGRAPHY USAGE DURING INTERVENTIONAL NEURORADIOLOGY.

The purpose of this study was to investigate the dose distribution and lens doses associated with C-arm cone-beam computed tomography (CBCT), using a head phantom, and to estimate the contribution ratio of C-arm CBCT to each patient's lens dose during interventional neuroradiology ('lens dose ratio') in 109 clinical cases. In the phantom study, the peak skin doses and respective right and left lens doses of C-arm CBCT were as follows: 63.0 ± 1.9 mGy, 19.7 ± 1.4 mGy and 21.9 ± 0.8 mGy in whole brain C-arm CBCT and 39.2 ± 1.4 mGy, 4.7 ± 0.9 mGy and 3.6 ± 0.3 mGy in high-resolution C-arm CBCT. In the clinical study, the lens dose ratios were 25.4 ± 8.7% in the right lens and 19.1 ± 9.8% in the left lens. This study shows that, on average, ~25% of patients' total lens dose was contributed by C-arm CBCT.

Reduction of Angularly-Varying-Data Truncation in C-Arm CBCT Imaging.

C-arm cone-beam computed tomography (CBCT) has been used increasingly as an imaging tool for yielding 3D anatomical information about the subjects in surgical and interventional procedures. In the clinical applications, the limited field-of-view (FOV) of C-arm CBCT can lead to significant data truncation, resulting in image artifacts that can obscure low contrast tumor embedded within soft-tissue background, thus limiting the utility of C-arm CBCT. The truncation issue can become serious as most of the surgical and interventional procedures would involve devices and tubes that are placed outside the FOV of C-arm CBCT and thus can engender angularly-varying-data truncation. Existing methods may not be adequately applicable to dealing with the angularly-varying truncation. In this work, we seek to reduce truncation artifacts by tailoring optimization-based reconstruction directly from truncated data, without performing pre-reconstruction data compensation, collected from physical phantoms and human subjects. The reconstruction problem is formulated as a constrained optimization program in which a data-derivative-ℓ2-norm fidelity is included for effectively suppressing image artifacts caused by the angularly-varying-data truncation, and the generic Chambolle-Pock algorithm is tailored to solve the optimization program. The results of the study suggest that an appropriately designed optimization-based reconstruction can be exploited for yielding images with reduced artifacts caused by angularly-varying-data truncation.

Investigations of organ and effective doses of abdominal cone-beam computed tomography during transarterial chemoembolization using Monte Carlo simulation

BackgroundTo investigate the organ dose, effective dose (ED), conversion factor, and the C-arm rotation angle effects on dose variations of abdominal C-arm cone-beam computed tomography (CBCT) during transarterial chemoembolization (TACE).MethodsThe organ doses and EDs for abdominal C-arm CBCT were retrospectively calculated according to a Monte Carlo technique for 80 patients. Dose variations from projections, ED to dose–area product (DAP) ratios, and effects of body mass index (BMI) on the ED and ED to DAP ratios were also analyzed.ResultsThe kidney received the highest dose (14.6 ± 1.2 mSv). Organ dose deviations among C-arm rotation angles was highest for stomach (CV = 0.71). The mean ED of the the CBCT run during TACE was 3.5 ± 0.5 mSv, and decreased with increased BMI (R2 = 0.45, p < 0.001). The mean ED to DAP ratio was 0.27 ± 0.04 mSv·Gy− 1·cm− 2 and tended to decrease with increased BMI (R2 = 0.55, p < 0.001). The mean ED to DAP ratios were 0.29 ± 0.02, 0.26 ± 0.02, and 0.23 ± 0.03 mSv·Gy− 1·cm− 2 for patients with BMI < 25 kg/m2, 25–30 kg/m2, and ≥30 kg/m2, respectively.ConclusionsSuitable conversion factors for C-arm CBCT facilitate the use of DAPs for estimating the ED. The patient dose can be varied by adjusting the CBCT rotation angle setting, and dose reduction strategies can be further manipulated.

Percutaneous radiofrequency ablation of lung metastases from colorectal carcinoma under C-arm cone beam CT guidance.

The aim of this study was to assess the feasibility, safety and efficacy of percutaneous radiofrequency ablation of lung metastases from colorectal carcinoma using C-arm cone beam computed tomography (CBCT) guidance. This single-center prospective observational study was performed from August 2013 to August 2016, and included consecutive patients referred for radiofrequency ablation of lung metastases from colorectal cancer. Radiofrequency ablation procedures were performed under C-arm CBCT guidance. Feasibility was assessed by probe accuracy placement, time to accurate placement and number of C-arm CBCT acquisitions to reach the target lesion. Safety was assessed by the report of adverse event graded using the common terminology criteria for adverse events (CTCAE-V4.0). Efficacy was assessed by metastases response rate using RECIST 1.1 and 18FDG-PET-CT tumor uptake at 6months. Fifty-four consecutive patients (32 men, 22 women) with a mean age of 63±8 (SD) years (range: 51-81years) with a total of 56 lung metastasis from colorectal metastases were treated in a single session. The mean tumor diameter was 25.6±4.5 (SD)mm (range: 17-31mm). Median time to insert the needle into the target lesion was 10min (range: 5-25min). Median number of needles repositioning and C-arm CBCT acquisition per patient was 1 (range: 0-3) and 4 (range: 3-6) respectively. The accuracy for radiofrequency ablation probe placement was 2±0.2 (SD)mm (range: 0-9mm). Pneumothorax requiring chest tube placement occurred in one patient (CTCAE-V4.0 grade 3). At 6months, all patients were alive with tumor response rate of -27% and had no significant activity on the 18FDG-PET CT follow-up. Percutaneous radiofrequency ablation of lung metastases from colorectal cancer under C-arm CBCT guidance is feasible and safe, with immediate and short-term results similar to those obtained using conventional CT guidance.

Optimization-based image reconstruction with artifact reduction in C-arm CBCT

We investigate an optimization-based reconstruction, with an emphasis on image-artifact reduction, from data collected in C-arm cone-beam computed tomography (CBCT) employed in image-guided interventional procedures. In the study, an image to be reconstructed is formulated as a solution to a convex optimization program in which a weighted data divergence is minimized subject to a constraint on the image total variation (TV); a data-derivative fidelity is introduced in the program specifically for effectively suppressing dominant, low-frequency data artifact caused by, e.g. data truncation; and the Chambolle–Pock (CP) algorithm is tailored to reconstruct an image through solving the program. Like any other reconstructions, the optimization-based reconstruction considered depends upon numerous parameters. We elucidate the parameters, illustrate their determination, and demonstrate their impact on the reconstruction. The optimization-based reconstruction, when applied to data collected from swine and patient subjects, yields images with visibly reduced artifacts in contrast to the reference reconstruction, and it also appears to exhibit a high degree of robustness against distinctively different anatomies of imaged subjects and scanning conditions of clinical significance. Knowledge and insights gained in the study may be exploited for aiding in the design of practical reconstructions of truly clinical-application utility.

Automatic bone removal for 3D TACE planning with C-arm CBCT: Evaluation of technical feasibility

Purpose To evaluate the technical feasibility of automatically removing the ribs and spine from C-arm cone-beam computed tomography (CBCT) images acquired during transcatheter arterial chemoembolization (TACE). Material and methods Fifty-eight patients (45.8 ± 5.0 years) with unresectable hepatocellular carcinoma (HCC) underwent transcatheter arterial chemoembolization and had intraprocedural CBCT imaging. Automatic bone removal was performed using model-based segmentation of the ventral cavity. Two interventional radiologists independently evaluated the performance of bone removal, remaining soft tissue retention, and general usability (where both the bone is appropriately removed while retaining soft tissue) for 3D TACE planning on a four-level (complete/excellent, adequate/good, incomplete/questionable, insufficient/bad) score. The proportion of inter-reader agreement was calculated. Results For ribs and spine removal, 98.3–100% and 100% of cases showed complete or adequate performance, respectively. In 96.6% of the cases, soft tissue was at least adequately retained. 91.3–93.1% of the cases demonstrated good or excellent general usability for TACE planning. Satisfactory inter-reader agreement proportion was achieved in ribs (93.1%) and spine removal (89.7%), soft tissue retention (84.5%), and general usability for TACE planning (72.4%). Conclusion Intraprocedural automatic bone removal on CBCT images is technically feasible and offers good removal of ribs and spine while preserving soft tissue. Its clinical value needs further assessment.

Role of C-arm cone-beam CT in chemoembolization for hepatocellular carcinoma.

With the advent of C-arm cone-beam computed tomography (CBCT), minimally-invasive procedures in the angiography suite made a new leap beyond the limitations of 2-dimensional (D) angiography alone. C-arm CBCT can help interventional radiologists in several ways with the treatment of hepatocellular carcinoma (HCC); visualization of small tumors and tumor-feeding arteries, identification of occult lesion and 3D configuration of tortuous hepatic arteries, assurance of completeness of chemoembolization, suggestion of presence of extrahepatic collateral arteries supplying HCCs, and prevention of nontarget embolization. With more improvements in the technology, C-arm CBCT may be essential in all kinds of interventional procedures in the near future.

Planning Evaluation of C-Arm Cone Beam CT Angiography for Target Delineation in Stereotactic Radiation Surgery of Brain Arteriovenous Malformations

Stereotactic radiation surgery (SRS) is one of the therapeutic modalities currently available to treat cerebral arteriovenous malformations (AVM). Conventionally, magnetic resonance imaging (MRI) and MR angiography (MRA) and digital subtraction angiography (DSA) are used in combination to identify the target volume for SRS treatment. The purpose of this study was to evaluate the use of C-arm cone beam computed tomography (CBCT) in the treatment planning of SRS for cerebral AVMs. Sixteen consecutive patients treated for brain AVMs at our institution were included in this retrospective study. Prior to treatment, all patients underwent MRA, DSA, and C-arm CBCT. All images were coregistered using the GammaPlan planning system. AVM regions were delineated independently by 2 physicians using either C-arm CBCT or MRA, resulting in 2 volumes: a CBCT volume (VCBCT) and an MRA volume (VMRA). SRS plans were generated based on the delineated regions. The average volume of treatment targets delineated using C-arm CBCT and MRA were similar, 6.40 cm(3) and 6.98 cm(3), respectively (P=.82). However, significant regions of nonoverlap existed. On average, the overlap of the MRA with the C-arm CBCT was only 52.8% of the total volume. In most cases, radiation plans based on VMRA did not provide adequate dose to the region identified on C-arm CBCT; the mean minimum dose to VCBCT was 29.5%, whereas the intended goal was 45% (P<.001). The mean volume of normal brain receiving 12 Gy or more in C-arm CBCT-based plans was not greater than in the MRA-based plans. Use of C-arm CBCT images significantly alters the delineated regions of AVMs for SRS planning, compared to that of MRA/MRI images. CT-based planning can be accomplished without increasing the dose to normal brain and may represent a more accurate definition of the nidus, increasing the chances for successful obliteration.

A novel technique for ventriculoperitoneal shunting by flat panel detector CT-guided real-time fluoroscopy

Background:Surgical placement of a ventriculoperitoneal shunt (VPS) is the main strategy to manage hydrocephalus. However, the failure rate associated with placement of ventricular catheters remains high.Methods:A hybrid operating room, equipped with a flat-panel detector digital subtraction angiography system containing C-arm cone-beam computed tomography (CB-CT) imaging, has recently been developed and utilized to assist neurosurgical procedures. We have developed a novel technique using intraoperative fluoroscopy and a C-arm CB-CT system to facilitate accurate placement of a VPS.Results:Using this novel technique, 39 consecutive ventricular catheters were placed accurately, and no ventricular catheter failures were experienced during the follow-up period. Only two patients experienced obstruction of the VPS, both of which occurred in the extracranial portion of the shunt system.Conclusion:Surgical placement of a VPS assisted by flat panel detector CT-guided real-time fluoroscopy enabled accurate placement of ventricular catheters and was associated with a decreased need for shunt revision.

Radiation dose evaluation in tomosynthesis and C‐arm cone‐beam CT examinations with an anthropomorphic phantom

The objective of this study was to evaluate organ dose and the effective dose to patients undergoing tomosynthesis (TS) and C-arm cone-beam computed tomography (CBCT) examinations and to compare the doses to those in multidetector CT (MDCT) scans. Patient doses were measured with small sized silicon-photodiode dosimeters, 48 in number, which were implanted at various tissue and organ positions within an anthropomorphic phantom. Output signals from photodiode dosimeters were read out on a personal computer, from which organ and effective doses were computed. The doses in head, chest, abdomen, and hip-joint TS, and in head and abdomen C-arm CBCT were evaluated for routine protocols on Shimadzu TS and C-arm CBCT systems, and the doses in MDCT with the same scan regions as in TS and CBCT were on Toshiba 64-detector-row CT scanners. In TS examination of the head, chest, abdomen, and hip-joint, organ doses for organs within scan ranges were 1-4 mGy, and effective doses were 0.07 mSv for the head scan and around 1 mSv for other scans. In C-arm CBCT examinations of the head and abdomen, organ doses within scan range were 2-37 mGy, and effective doses were 1.2 mSv for the head scan and 4-5 mSv for abdominal scans. Effective doses in TS examinations were approximately a factor of 10 lower, while the doses in CBCT examinations were nearly the same level, compared to the doses in the corresponding MDCT examinations. TS examinations with low doses and excellent resolutions in coronal images compared to recent MDCT would widely be used in tomographic examinations of the chest, abdomen, pelvis, skeletal-joints, and knee instead of MDCT examinations with significantly high doses. Since patient dose in C-arm CBCT was nearly the same level as that in recent MDCT, the same consideration for high radiation dose would be required for the use of CBCT.

Intraoperative adaptive brachytherapy of iodine-125 prostate implants guided by C-arm cone-beam computed tomography–based dosimetry

Purpose ( 1) To demonstrate the feasibility of C-arm cone-beam computed tomography (CBCT)-based postplanning and subsequent adaptation of underdosed critical areas by adding remedial seeds during the transrectal ultrasound (TRUS)-guided implantation of 125I seeds and ( 2) to assess the duration of this procedure. Methods and materials After finishing the implant, three fiducial markers were implanted and a TRUS study was performed to delineate the prostate. A C-arm CBCT unit with isocentric design was used to generate a CT data set to localize the seeds. The TRUS and CBCT data sets were coregistered by the radiation oncologist to assess the dosimetry of the implant. If underdosages existed at critical areas, dosimetry was adapted by adding remedial seeds while the patient was still under anesthesia. Results Of 20 patients studied, 9 demonstrated underdosage in critical areas. On average four additional seeds were implanted, resulting in a mean D 90 of 100.7% (increase 4.9%) and 117.5% (increase 17.8%) of the prescribed dose of 145 and 110 Gy, respectively. The average additional time involved in performing the adaptation procedure was less than 30 min. Conclusions C-arm CBCT-guided intraoperative postplanning during TRUS-guided brachytherapy for prostate cancer is both feasible and time efficient. The adaptation resulted in improved dosimetry of the prostate implants.
