The Department of Medicine at the Albert Einstein College of Medicine, 1955-1980

After several years of planning, the new Albert Einstein College of Medicine (AECOM) recruited its first class, of just 56 students, and began their instruction in the fall of 1955. This event took place in a not-yet-fully-outfitted Forchheimer building, which was then the only one of what later became a cluster of AECOM buildings. Across the street was the Bronx Municipal Hospital Center (BMHC, now Jacobi Medical Center, JMC), which was the original (and, for years, only) teaching hospital of the College. The evolution of the Department of Medicine at this new College, from its early stages – with 56 medical students, a small outstanding young faculty, one science building and one teaching hospital – to a world-renowned medical research and educational institution, with over 800 medical and doctoral students, numerous post-doctoral fellows and trainees, a large outstanding faculty, four major science buildings, and six major teaching hospital affiliates, is described.

Presentation of the 1999 Jean Hamburger Award to Barry M. Brenner

It is a great privilege and pleasure to introduce my former mentor, the recipient of the Jean Hamburger Award in 1999, Dr. Barry M. Brenner. Although Dr. Brenner's name is readily recognized from his research, theories and textbooks, not many have had my good fortune of being able to observe him so closely and for so long. It was the summer of 1974, when I was visiting from Tokyo, that Dr. Brenner granted me my first interview in his small office at the VA hospital on Clement Street in San Francisco, which overlooks the magnificent Golden Gate Bridge. During the next seven years of working with him, Dr. Brenner taught me nephrology, medicine, and importantly for me, English, as well. I believe that the many significant benefits that I received from working with him have earned me the right to introduce Dr. Brenner today.

Height of Temperature: A Febrile Seizure Risk Factor

Risk factors were identified in 69 children with a first febrile seizure compared to 99 matched controls seen in a three year period at the Bronx Municipal Hospital Center, Montifiore Medical Center, and North Central Bronx Hospital, and Albert Einstein College of Medicine, New York.

Femoral vein pulmonary artery catheterization in the intensive care unit.

To evaluate the success and complications of pulmonary artery catheterization via the femoral vein without the use of fluoroscopy, in the medical ICU. We retrospectively reviewed the charts of all patients receiving pulmonary artery catheters through the femoral vein in the medical ICU between July 1, 1988 and June 30, 1992. Medical ICU at Bronx Municipal Hospital Center. Thirty-three patients were studied (20 male and 13 female). The patients ranged in age from 24 to 83 years, with the mean age of 56 years. The condition at the time of catheterization was classified as septic shock (n = 22), gastrointestinal bleed (n = 7), cardiogenic shock (n = 1), cardiac tamponade (n = 1), preoperative (n = 1), or drug overdose (n = 1). In each case, pulmonary artery pressure tracings, electrocardiographic recordings, procedure, and nursing notes were reviewed from the time of catheter insertion. Charts were also analyzed for evidence of catheter-related septicemia, deep vein thrombosis, pulmonary embolism, or problems developing at the insertion site. Femoral vein pulmonary artery catheterization was performed successfully in 37 of 39 attempts (95 percent success rate) without the use of fluoroscopy. Five of the 33 patients studied had more than one femoral vein pulmonary artery catheter during their hospital course. Four of them had two femoral vein pulmonary artery catheters and the fifth patient had three femoral vein pulmonary artery catheters. Overall, 39 attempts at femoral vein pulmonary artery catheterization were made in the 33 patients studied. Three patients had 10- to 19-beat ventricular tachycardia (7.7 percent incidence) precluding catheter insertion via this approach in two instances. There was no clinical evidence of major hematomas or hemorrhages, catheter-related septicemia, deep vein thrombosis, or pulmonary emboli in any of our patients. Femoral vein pulmonary artery catheterization without the use of fluoroscopy is safe and effective.

YOUTHS SET HOMELESS MAN AFIRE IN SUBWAY

A 31-year-old homeless man was seriously injured early yesterday after four teen-agers set him on fire as he lay sleeping in a Bronx subway station, the transit police said. A 25-year-old man on his way to work grabbed a fire extinguisher from a token booth and doused the flames. The victim, whose name was not released, was listed in critical condition at Bronx Municipal Hospital Center with burns over 75 percent of his body... Fourteen homeless people were set on fire from January through April in the city's subways, the transit police said. One of them died.

Tuberculosis in Anergic HIV-Infected Drug Users

To the Editor. —Selwyn et al 1 report important information for the prevention of tuberculosis in addicts infected with the human immunodeficiency virus (HIV). We would like to make the following comments based on our experience at the Albert Einstein College of Medicine Methadone Maintenance Clinic at Bronx (NY) Municipal Hospital Center, which was established 26 years ago specifically for the treatment of addicts with tuberculosis. In 1985, we reported that 27% of our patients were positive for purified protein derivative (PPD) and 9% were anergic. 2 In 1989, again 97 (27%) of 355 patients who were tested were PPD-positive. Of these 97 patients, 20 (20%) had a history of treated tuberculosis. Of the remaining 77 PPD-positive patients, 61 (80%) had received preventive isoniazid treatment. Sixteen (20%) had no preventive treatment for one of the following reasons: they were HIV-negative and over 35 years of age; they had elevated, unstable

INCIDENCE OF INTESTINAL PARASITIC DISEASE IN AN ACQUIRED IMMUNODEFICIENCY SYNDROME DAY-CARE CENTER

In June, 1986, the Bronx Municipal Hospital Center opened an acquired immunodeficiency syndrome day-care center to provide a quality educational experience for children infected with the human immunodeficiency virus. A major concern was the possibility of increasing secondary infections among these immunocompromised children by placing them in a group environment. One particular worry was intestinal parasitic disease, a serious public health problem in day-care centers throughout the United States. To minimize the risk of parasitic infections, scrupulous hygienic and monitoring procedures were instituted at the acquired immunodeficiency syndrome day-care center. This study reports the incidence of intestinal parasitic disease at the acquired immunodeficiency syndrome day-care center during its first 40 months of operation, encompassing 669 child-months of enrollment, with 131 stool specimens examined for ova and parasites. There were 2 cases of parasitic infection: Entamoeba histolytica in an asymptomatic 6-year-old and Giardia intestinalis in a 7-year-old with diarrhea. In neither case was there any secondary spread. None of the 15 children in diapers had a positive specimen, and we found no Cryptosporidium. Our experience suggests that with appropriate precautions human immunodeficiency virus-infected children can participate in a group day-care program without excessive risk for parasitic disease. Strict adherence to hygienic procedures may also decrease the risk of intestinal parasitic disease among healthy children attending day-care centers.

Quality Assurance for HIV-Related Care

Now that models of care for numerous HIV-related conditions have been defined, a knowledge base exists for developing protocols of appropriate, high-quality care for HIV-infected patients. Bronx Municipal Hospital Center in New York City plans to implement a monitoring and evaluation program for HIV-related care that would establish protocols and indicators of quality and appropriateness, monitor compliance with protocols, and generate recommendations for improving care. At present, Bronx Municipal's AIDS consultation service has drafted guidelines for developing indicators of appropriate diagnosis, clinical course, and drug usage for HIV-infected patients, as well as guidelines for reviewing medical records.

In 1983 the North American Hyperbaric Center (affiliated with Bronx Municipal Hospital Center) was designated to provide Hyperbaric oxygen (HBO) for carbon monoxide (CO) patients meeting Emergency Medical System (EMS) criteria: 1. Unconscious or CNS derangement, any carboxyhemoglobin level [( COHb]); 2. [COHb] 25% or more; 3. Pregnant, any [COHb]. Through 1984, 39 CO patients received HBO; in 1985 81 were treated including 8 pregnant and 16 pediatric. CO sources were: fire, 43; heater, 21; engine, 17. 42 of 59 acute patients were initially in coma; 16 required CPR. Time to HBO averaged 4.5 hrs. Mean hospital [COHb] was 31% for acute, 28% for subacute/chronic, 15% for pregnant; mean maximum [COHb] was estimated: 51% for acute, 40% for subacute/chronic patients. Mean half life for [COHb] was 2.1 hrs. HBO typically was 46 min at 3ATA (2 [COHb] half lives), presented few problems, and gave rapid clinical improvement. 13 of 19 patients comatose before HBO were responsive after HBO (mean [COHb] was 1.8%). Four pediatric deaths occurred; brain damage was noted in two other patients. EMS efforts to make HBO available for CO is a success.

Plasma vitamin C and uterine cervical dysplasia

Plasma concentrations of vitamin C were determined in a case-control study of women (n = 80) who had sought a Papanicolaou test in the Bronx Municipal Hospital Center, Controls (n = 34) were women having negative cytologic tests, negative colposcopic findings, and no known gynecologic dysfunction. Cases (n = 46) were defined as women who had either one positive or two consecutive suspicious Papanicolaou smears in a 12-month period. The mean concentration of vitamin C in the plasma was significantly lower in the cases than in the controls (0.36 versus 0.75 mg/dl, p < 0.0001). Cases were further stratified according to the histopathologic diagnosis. The data direct attention to a possible etiologic association of vitamin C in human cervical epithelial abnormalities. A clinical trial with vitamin C intervention is suggested.

Recurrent diabetic ketoacidosis

This report presents and analyzes certain clinical and laboratory findings in 17 patients who were hospitalized repeatedly for diabetic ketoacidosis on a municipal hospital medical service during the early 1970s and the early 1980s. The 17 patients had a total of 92 hospitalizations for ketoacidosis during the survey periods. During the 1970s, most of the frequent recurrences occurred in young women, as has often been noted by others. In recent years, however, the number of patients with frequent recurrences hospitalized at Bronx Municipal Hospital Center has declined, and most of the patients have been young men. There is no obvious explanation for the latter trend. The laboratory findings were analyzed in order to learn whether they were similar in each patient during his or her recurrent admissions. In a few patients, blood pH, hemoglobin concentration, serum glucose level, anion gap, and osmoiarity tended to be similar during their repeated hospitalizations. In the majority of the patients, however, there was significant variation in most of these laboratory indexes, except for the serum osmolarities and blood hemoglobin concentrations. The similarity of the serum osmolarities and hemoglobin concentrations in sequential episodes in most of the patients suggested that a certain severity of dehydration may have been the main factor that led them to seek hospitalization.

Who Should Provide Primary Health Care to Children: Pediatricians or Family Medicine Physicians?

INTRODUCTION —Robert A. Hoekelman, MD This debate is one of a series entitled "Controversies in Child Health and Pediatric Practice." In 1978 and 1979, the Department of Pediatrics at the University of Rochester conducted 12 such debates, which were published in book form in 1981.1 It is interesting that of these 12 controversies, not one has been resolved. That may prove to be the case for the controversy presented in this debate, "Who should provide primary health care to children: pediatricians or family medicine physicians?" Our guest speakers are Dr Michael Klein and Dr James E. Strain. Dr Klein is Professor of Family Medicine and Assistant Professor of Pediatrics at McGill University School of Medicine. He is also Director of the Department of Family Medicine and of the Herzl Family Practice Center at the Sir Mortimer B. Davis Jewish General Hospital in Montreal. Dr Klein received his medical degree at Stanford University in 1966, and he was his class's recipient of the pediatric Harold K. Kaiser Award. Dr Klein served his internship at Bronx Municipal Hospital Center at Albert Einstein University and his pediatric residency at Montreal Children's Hospital. During his last year, he served as Senior Resident in Neonatology under Robert Usher at the Royal Victoria Hospital. He then became a fellow in biochemical genetics under Charles Scriver and the Chief Medical Resident at Montreal Children's Hospital under Mary Ellen Avery. In 1970, he came to Rochester as a Fellow in Ambulatory Pediatrics. Dr Klein's practice experience is extensive. He was the pediatric coordinator of the St Jacques Clinic in Montreal during the late 1960s and the medical director of the Westside Health Services in Rochester from 1971 to 1975, after which he assumed his current position.
