Energy window optimization for Y-90 Bremsstrahlung SPECT imaging: A Monte Carlo simulation study

Introduction: In Y-90 imaging, image quality is highly dependent on choosing the energy window and collimator design because, the Y-90 bremsstrahlung photons have a continuous and broad energy distribution. The aim of this work was to optimize the bremsstrahlung energy window setting and collimator to improve both resolution and sensitivity. Materials and methods: We simulate the Siemens Medical System Symbia using the SIMulation of Imaging Nuclear Detectors (SIMIND) Monte Carlo program. The SIMIND was used to generate the Y-90 bremsstrahlung Single Photon Emission Computed Tomography (SPECT) projection of the point source. In order to assess the effect of the energy window on the resolution, six energy windows settings and two collimators denoted Medium Energy (ME) and High Energy (HE) were used. Results: The experimental measurement and simulation results exhibited a similar pattern in point spread functions (PSFs) with energy window. The simulation data show that, the geometric component reaches 73% for 51-120 keV energy window with HE collimator. In addition, the results showed that, the Full Width at Half Maximum (FWHM) and Full Width at Tenth Maximum (FWTM) (FWHM =7 mm + FWTM =35mm) was better in this window relative to other window. Conclusion: The optimal energy window for Y-90 bremsstrahlung SPECT imaging was 51- 120 keV. The obtained optimal energy window and optimal HE collimator has the potential to improve the image resolution and sensitivity of Y-90 SPECT images.

Evaluation the effect of different collimators and energy window on Y-90 bremsstrahlung SPECT imaging by SIMIND Monte Carlo program.

Recently, the treatment efficiency of Yttrium 90 (Y-90) and providing reliable estimates of activity by single photon emission computed tomography (SPECT) imaging of bremsstrahlung radiation released during beta therapy have been evaluated. In the Y-90 bremsstrahlung SPECT imaging, the resulting energy spectrum is very complex and continuous, which creates many difficulties in the imaging protocol and image reconstruction. Furthermore, image quality and quantitative accuracy in the bremsstrahlung SPECT imaging are affected by collimator penetration and scatter. So, the collimator type and its geometry have impressive effects on the spatial resolution, system sensitivity and image contrast. Hereby, in this paper, we evaluated the effect of the energy window (three energy windows: 60 to 160 keV, 160 to 400 keV, and 60 to 400 keV) and the commercial parallel-hole collimators with different geometric parameters on the Y-90 bremsstrahlung spectrum and the image quality of the liver tumors based on criteria such as system sensitivity and image contrast. SIMIND Monte Carlo simulation code was used to generate the Y-90 bremsstrahlung SPECT images of the liver tumor with different diameters: 1.36, 2.04, 2.72, 3.4, 4.08, and 4.76 cm by use of the digital Zubal phantom. Furthermore, the tumor size was estimated by evaluating pixel intensity profile on the line drawn through the activity distribution image. Our results showed that the collimator choice and energy window setting in the bremsstrahlung SPECT imaging have significant effects on the image quality and tumor size estimation. Optimal image quality could be acquired by the energy window of 60 to 400 keV and the SPECT system equipped with a Medium-Energy General-Purpose (MEGP) collimator of Millennium VG Kameran (GV) Company. Moreover, the estimation of distribution size was close to the actual value for tumor sizes larger than 2.04 cm, especially by using the SPECT system equipped with the GV-MEGP collimator in the wide energy window. We found an optimal collimator to be more appropriate for improving the imaging quality of Y-90 bremsstrahlung photons, which can be used for reliable activity distribution estimates after radiation therapy.

Energy Window and Contrast Optimization for Single-photon Emission Computed Tomography Bremsstrahlung Imaging with Yttrium-90.

Purpose:In yttrium-90 (Y-90) single-photon emission computed tomography (SPECT) imaging, the choice of the acquisition energy window is not trivial, due to the continuous and broad energy distribution of the bremsstrahlung photons. In this work, we investigate the effects of the energy windows on the image contrast to noise ratio (CNR), in order to select the optimal energy window for Y-90 imaging.Materials and Methods:We used the Monte Carlo SIMIND code to simulate the Jaszczak phantom which consists of the six hot spheres filled with Y-90 and ranging from 9.5 to 31.8 mm in diameter. Siemens Symbia gamma camera fitted with a high-energy collimator was simulated. To evaluate the effect of the energy windows on the image contrast, five narrow and large energy windows were assessed.Results:The optimal energy window obtained for Y-90 bremsstrahlung SPECT imaging was 120–150 keV. Furthermore, the results obtained for CNR indicate that the high detection is only for the three large spheres.Conclusion:The optimization of energy window in Y-90 bremsstrahlung has the potential to improve the image quality.

Comparison of TOF-PET and Bremsstrahlung SPECT Images of Yttrium-90: A Monte Carlo Simulation Study.

Objective(s):Yttrium-90 (90Y) is a beta particle nuclide used in targeted radionuclide therapy which is available to both single-photon emission computed tomography (SPECT) and time-of-flight (TOF) positron emission tomography (PET) imaging. The purpose of this study was to assess the image quality of PET and Bremsstrahlung SPECT by simulating PET and SPECT images of 90Y using Monte Carlo simulation codes under the same conditions and to compare them.Methods:In-house Monte Carlo codes, MCEP-PET and MCEP-SPECT, were employed to simulate images. The phantom was a torso-shaped phantom containing six hot spheres of various sizes. The background concentrations of 90Y were set to 50, 100, 150, and 200 kBq/mL, and the concentrations of the hot spheres were 10, 20, and 40 times of those of the background concentrations. The acquisition time was set to 30 min, and the simulated sinogram data were reconstructed using the ordered subset expectation maximization method. The contrast recovery coefficient (CRC) and contrast-to-noise ratio (CNR) were employed to evaluate the image qualities.Results:The CRC values of SPECT images were less than 40%, while those of PET images were more than 40% when the hot sphere was larger than 20 mm in diameter. The CNR values of PET images of hot spheres of diameter smaller than 20 mm were larger than those of SPECT images. The CNR values mostly exceeded 4, which is a criterion to evaluate the discernibility of hot areas. In the case of SPECT, hot spheres of diameter smaller than 20 mm were not discernable. On the contrary, the CNR values of PET images decreased to the level of SPECT, in the case of low concentration.Conclusion:In almost all the cases examined in this investigation, the quantitative indexes of TOF-PET 90Y images were better than those of Bremsstrahlung SPECT images. However, the superiority of PET image became critical in the case of low activity concentrations.

Collimator and energy window optimization for 90Y bremsstrahlung SPECT imaging: A SIMIND Monte Carlo study

Treatment efficacy of radioembolization using Yttrium-90 (90Y) microspheres is assessed by the 90Y bremsstrahlung single photon emission computed tomography (SPECT) imaging following radioembolization. The radioisotopic image has the potential of providing reliable activity map of 90Y microspheres distribution. One of the main reasons of the poor image quality in 90Y bremsstrahlung SPECT imaging is the continuous and broad energy spectrum of the related bremsstrahlung photons. Furthermore, collimator geometry plays an impressive role in the spatial resolution, sensitivity and image contrast. Due to the relatively poor quality of the 90Y bremsstrahlung SPECT images, we intend to optimize the medium-energy (ME) parallel-hole collimator and energy window. The Siemens e.cam gamma camera equipped with a ME collimator and a voxelized phantom was simulated by the SImulating Medical Imaging Nuclear Detectors (SIMIND) program. We used the SIMIND Monte Carlo program to generate the 90Y bremsstrahlung SPECT projection of the digital Jaszczak phantom. The phantom consist of the six hot spheres ranging from 9.5 to 31.8mm in diameter, which are used to evaluate the image contrast. In order to assess the effect of the energy window on the image contrast, three energy windows ranging from 60 to 160KeV, 160 to 400KeV, and 60 to 400KeV were set on a 90Y bremsstrahlung spectrum. As well, the effect of the hole diameter of a ME collimator on the image contrast and bremsstrahlung spectrum were investigated. For the fixed collimator and septa thickness values (3.28cm and 1.14mm, respectively), a hole diameter range (2.35–3.3mm) was chosen based on the appropriate balance between the spatial resolution and sensitivity. The optimal energy window for 90Y bremsstrahlung SPECT imaging was extended energy window from 60 to 400KeV. Besides, The optimal value of the hole diameter of ME collimator was obtained 3.3mm. Geometry of the ME parallel-hole collimator and energy window are indeed important indicators of the image quality in 90Y bremsstrahlung imaging. The obtained optimal ME collimator and optimal energy window have the potential to improve the image contrast of 90Y bremsstrahlung images. Subsequently, high quality 90Y bremsstrahlung images can provide reliable estimate of the 90Y microsphere activity distribution after radioembolization.

A collimator optimization method for quantitative imaging: application to Y-90 bremsstrahlung SPECT.

Post-therapy quantitative 90Y bremsstrahlung single photon emission computed tomography (SPECT) has shown great potential to provide reliable activity estimates, which are essential for dose verification. Typically 90Y imaging is performed with high- or medium-energy collimators. However, the energy spectrum of 90Y bremsstrahlung photons is substantially different than typical for these collimators. In addition, dosimetry requires quantitative images, and collimators are not typically optimized for such tasks. Optimizing a collimator for 90Y imaging is both novel and potentially important. Conventional optimization methods are not appropriate for 90Y bremsstrahlung photons, which have a continuous and broad energy distribution. In this work, the authors developed a parallel-hole collimator optimization method for quantitative tasks that is particularly applicable to radionuclides with complex emission energy spectra. The authors applied the proposed method to develop an optimal collimator for quantitative 90Y bremsstrahlung SPECT in the context of microsphere radioembolization. To account for the effects of the collimator on both the bias and the variance of the activity estimates, the authors used the root mean squared error (RMSE) of the volume of interest activity estimates as the figure of merit (FOM). In the FOM, the bias due to the null space of the image formation process was taken in account. The RMSE was weighted by the inverse mass to reflect the application to dosimetry; for a different application, more relevant weighting could easily be adopted. The authors proposed a parameterization for the collimator that facilitates the incorporation of the important factors (geometric sensitivity, geometric resolution, and septal penetration fraction) determining collimator performance, while keeping the number of free parameters describing the collimator small (i.e., two parameters). To make the optimization results for quantitative 90Y bremsstrahlung SPECT more general, the authors simulated multiple tumors of various sizes in the liver. The authors realistically simulated human anatomy using a digital phantom and the image formation process using a previously validated and computationally efficient method for modeling the image-degrading effects including object scatter, attenuation, and the full collimator-detector response (CDR). The scatter kernels and CDR function tables used in the modeling method were generated using a previously validated Monte Carlo simulation code. The hole length, hole diameter, and septal thickness of the obtained optimal collimator were 84, 3.5, and 1.4 mm, respectively. Compared to a commercial high-energy general-purpose collimator, the optimal collimator improved the resolution and FOM by 27% and 18%, respectively. The proposed collimator optimization method may be useful for improving quantitative SPECT imaging for radionuclides with complex energy spectra. The obtained optimal collimator provided a substantial improvement in quantitative performance for the microsphere radioembolization task considered.

Optimization of energy window for 90Y bremsstrahlung SPECT imaging for detection tasks using the ideal observer with model‐mismatch

In yttrium-90 ((90)Y) microsphere brachytherapy (radioembolization) of unresectable liver cancer, posttherapy (90)Y bremsstrahlung single photon emission computed tomography (SPECT) has been used to document the distribution of microspheres in the patient and to help predict potential side effects. The energy window used during projection acquisition can have a significant effect on image quality. Thus, using an optimal energy window is desirable. However, there has been great variability in the choice of energy window due to the continuous and broad energy distribution of (90)Y bremsstrahlung photons. The area under the receiver operating characteristic curve (AUC) for the ideal observer (IO) is a widely used figure of merit (FOM) for optimizing the imaging system for detection tasks. The IO implicitly assumes a perfect model of the image formation process. However, for (90)Y bremsstrahlung SPECT there can be substantial model-mismatch (i.e., difference between the actual image formation process and the model of it assumed in reconstruction), and the amount of the model-mismatch depends on the energy window. It is thus important to account for the degradation of the observer performance due to model-mismatch in the optimization of the energy window. The purpose of this paper is to optimize the energy window for (90)Y bremsstrahlung SPECT for a detection task while taking into account the effects of the model-mismatch. An observer, termed the ideal observer with model-mismatch (IO-MM), has been proposed previously to account for the effects of the model-mismatch on IO performance. In this work, the AUC for the IO-MM was used as the FOM for the optimization. To provide a clinically realistic object model and imaging simulation, the authors used a background-known-statistically and signal-known-statistically task. The background was modeled as multiple compartments in the liver with activity parameters independently following a Gaussian distribution; the signal was modeled as a tumor with a Gaussian-distributed activity parameter located randomly with equal probability at one of three positions. The IO test statistics (i.e., likelihood ratios) were estimated using Markov-chain Monte Carlo methods. The authors realistically modeled human anatomy using a digital phantom code, and realistically simulated (90)Y bremsstrahlung SPECT imaging with a clinical SPECT system and typical imaging parameters using a previously validated Monte Carlo bremsstrahlung simulation method. Model-mismatch was included by modeling image formation process in the calculation of IO test statistics using an analytic modeling method previously developed for quantitative (90)Y bremsstrahlung SPECT. To demonstrate the effects of the model-mismatch on the detection task, the authors optimized the energy window both with and without model-mismatch included in the IO. For all the energy windows, the AUC values for the IO-MM were smaller than that for the IO. The optimal windows for the IO-MM and the IO were 80-180 and 60-400 keV, respectively. The authors have demonstrated the degradation of the ideal performance due to model-mismatch and optimized the energy window for (90)Y bremsstrahlung SPECT for detection tasks by accounting for the effects of the model-mismatch. The obtained optimal window was much narrower when taking into account the model-mismatch and similar to that obtained previously for estimation tasks.

Comparison of positron emission tomography/CT and bremsstrahlung imaging following Y-90 radiation synovectomy

The aim of this study is to compare the results of positron emission tomography (PET)/CT with bremsstrahlung imaging following Y-90 radiation synovectomy. All patients referred to our institution for Y-90 radiation synovectomy between July 2011 and February 2012 underwent both PET/CT and bremsstrahlung planar (± single photon emission computed tomography (SPECT) or SPECT/CT) imaging at 4 or 24 h following administration of Y-90 silicate colloid. PET image acquisition was performed for between 15 and 20 min. In patients who underwent SPECT, side-by-side comparison with PET was performed and image quality/resolution scored using a five-point scale. The distribution pattern of Y-90 on PET and bremsstrahlung imaging was compared with the intra- or extra-articular location of Y-90 activity on fused PET/CT. Thirteen joints (11 knees and two ankles) were imaged with both PET/CT and planar bremsstrahlung imaging with 12 joints also imaged with bremsstrahlung SPECT. Of the 12 joints imaged with SPECT, PET image quality/resolution was superior in 11 and inferior in one. PET demonstrated a concordant distribution pattern compared with bremsstrahlung imaging in all scans, with the pattern classified as diffuse in 12 and predominantly focal in one. In all 12 diffuse scans, PET/CT confirmed the Y-90 activity to be located intra-articularly. In the one predominantly focal scan, the fused PET/CT images localised the Y-90 activity to mostly lie in the extra-articular space of the knee. PET/CT can provide superior image quality compared with bremsstrahlung imaging and may enable reliable detection of extra-articular Y-90 activity when there are focal patterns on planar bremsstrahlung imaging.

A method for energy window optimization for quantitative tasks that includes the effects of model-mismatch on bias: application to Y-90 bremsstrahlung SPECT imaging

Quantitative Yttrium-90 (90Y) bremsstrahlung single photon emission computed tomography (SPECT) imaging has shown great potential to provide reliable estimates of 90Y activity distribution for targeted radionuclide therapy dosimetry applications. One factor that potentially affects the reliability of the activity estimates is the choice of the acquisition energy window. In contrast to imaging conventional gamma photon emitters where the acquisition energy windows are usually placed around photopeaks, there has been great variation in the choice of the acquisition energy window for 90Y imaging due to the continuous and broad energy distribution of the bremsstrahlung photons. In quantitative imaging of conventional gamma photon emitters, previous methods for optimizing the acquisition energy window assumed unbiased estimators and used the variance in the estimates as a figure of merit (FOM). However, for situations, such as 90Y imaging, where there are errors in the modeling of the image formation process used in the reconstruction there will be bias in the activity estimates. In 90Y bremsstrahlung imaging this will be especially important due to the high levels of scatter, multiple scatter, and collimator septal penetration and scatter. Thus variance will not be a complete measure of reliability of the estimates and thus is not a complete FOM. To address this, we first aimed to develop a new method to optimize the energy window that accounts for both the bias due to model-mismatch and the variance of the activity estimates. We applied this method to optimize the acquisition energy window for quantitative 90Y bremsstrahlung SPECT imaging in microsphere brachytherapy. Since absorbed dose is defined as the absorbed energy from the radiation per unit mass of tissues in this new method we proposed a mass-weighted root mean squared error of the volume of interest (VOI) activity estimates as the FOM. To calculate this FOM, two analytical expressions were derived for calculating the bias due to model-mismatch and the variance of the VOI activity estimates, respectively. To obtain the optimal acquisition energy window for general situations of interest in clinical 90Y microsphere imaging, we generated phantoms with multiple tumors of various sizes and various tumor-to-normal activity concentration ratios using a digital phantom that realistically simulates human anatomy, simulated 90Y microsphere imaging with a clinical SPECT system and typical imaging parameters using a previously validated Monte Carlo simulation code, and used a previously proposed method for modeling the image degrading effects in quantitative SPECT reconstruction. The obtained optimal acquisition energy window was 100–160 keV. The values of the proposed FOM were much larger than the FOM taking into account only the variance of the activity estimates, thus demonstrating in our experiment that the bias of the activity estimates due to model-mismatch was a more important factor than the variance in terms of limiting the reliability of activity estimates.

Development and evaluation of an improved quantitative 90Y bremsstrahlung SPECT method

Yttrium-90 ((90)Y) is one of the most commonly used radionuclides in targeted radionuclide therapy (TRT). Since it decays with essentially no gamma photon emissions, surrogate radionuclides (e.g., (111)In) or imaging agents (e.g., (99m)Tc MAA) are typically used for treatment planning. It would, however, be useful to image (90)Y directly in order to confirm that the distributions measured with these other radionuclides or agents are the same as for the (90)Y labeled agents. As a result, there has been a great deal of interest in quantitative imaging of (90)Y bremsstrahlung photons using single photon emission computed tomography (SPECT) imaging. The continuous and broad energy distribution of bremsstrahlung photons, however, imposes substantial challenges on accurate quantification of the activity distribution. The aim of this work was to develop and evaluate an improved quantitative (90)Y bremsstrahlung SPECT reconstruction method appropriate for these imaging applications. Accurate modeling of image degrading factors such as object attenuation and scatter and the collimator-detector response is essential to obtain quantitatively accurate images. All of the image degrading factors are energy dependent. Thus, the authors separated the modeling of the bremsstrahlung photons into multiple categories and energy ranges. To improve the accuracy, the authors used a bremsstrahlung energy spectrum previously estimated from experimental measurements and incorporated a model of the distance between (90)Y decay location and bremsstrahlung emission location into the SIMIND code used to generate the response functions and kernels used in the model. This improved Monte Carlo bremsstrahlung simulation was validated by comparison to experimentally measured projection data of a (90)Y line source. The authors validated the accuracy of the forward projection model for photons in the various categories and energy ranges using the validated Monte Carlo (MC) simulation method. The forward projection model was incorporated into an iterative ordered subsets-expectation maximization (OS-EM) reconstruction code to allow for quantitative SPECT reconstruction. The resulting code was validated using both a physical phantom experiment with spherical objects in a warm background and a realistic anatomical phantom simulation. In the physical phantom study, the authors evaluated the method in terms of quantitative accuracy of activity estimates in the spheres; in the simulation study, the authors evaluated the accuracy and precision of activity estimates from various organs and compared them to results from a previously proposed method. The authors demonstrated excellent agreement between the experimental measurement and Monte Carlo simulation. In the XCAT phantom simulation, the proposed method achieved much better accuracy in the modeling (error in photon counts was -1.1 %) compared to a previously proposed method (errors were more than 20 %); the quantitative accuracy of activity estimates was excellent for all organs (errors were from -1.6 % to 11.9 %) and comparable to previously published results for (131)I using the same collimator. The proposed (90)Y bremsstrahlung SPECT reconstruction method provided very accurate estimates of organ activities, with accuracies approaching those previously observed for (131)I. The method may be useful in verifying organ doses for targeted radionuclide therapy using (90)Y.

Fast simulation of yttrium-90 bremsstrahlung photons with GATE

Multiple investigators have recently reported the use of yttrium-90 (90Y) bremsstrahlung single photon emission computed tomography (SPECT) imaging for the dosimetry of targeted radionuclide therapies. Because Monte Carlo (MC) simulations are useful for studying SPECT imaging, this study investigates the MC simulation of 90Y bremsstrahlung photons in SPECT. To overcome the computationally expensive simulation of electrons, the authors propose a fast way to simulate the emission of 90Y bremsstrahlung photons based on prerecorded bremsstrahlung photon probability density functions (PDFs). The accuracy of bremsstrahlung photon simulation is evaluated in two steps. First, the validity of the fast bremsstrahlung photon generator is checked. To that end, fast and analog simulations of photons emitted from a 90Y point source in a water phantom are compared. The same setup is then used to verify the accuracy of the bremsstrahlung photon simulations, comparing the results obtained with PDFs generated from both simulated and measured data to measurements. In both cases, the energy spectra and point spread functions of the photons detected in a scintillation camera are used. Results show that the fast simulation method is responsible for a 5% overestimation of the low-energy fluence (below 75 keV) of the bremsstrahlung photons detected using a scintillation camera. The spatial distribution of the detected photons is, however, accurately reproduced with the fast method and a computational acceleration of approximately 17-fold is achieved. When measured PDFs are used in the simulations, the simulated energy spectrum of photons emitted from a point source of 90Y in a water phantom and detected in a scintillation camera closely approximates the measured spectrum. The PSF of the photons imaged in the 50-300 keV energy window is also accurately estimated with a 12.4% underestimation of the full width at half maximum and 4.5% underestimation of the full width at tenth maximum. Despite its limited accuracy, the fast bremsstrahlung photon generator is well suited for the simulation of bremsstrahlung photons emitted in large homogeneous organs, such as the liver, and detected in a scintillation camera. The computational acceleration makes it very useful for future investigations of 90Y bremsstrahlung SPECT imaging.
