Box Hill Hospital has approximately 750 stroke presentations per annum and in 2023, 99 patients received thrombolysis and 40 were transferred out for thrombectomy. Thrombolysis rates, for patients with ischaemic stroke that present within 4.5 hours of symptom onset, was at 37%, with a median door-to-needle (DNT) of 41 minutes, 86% of cases being treated under 60 minutes of arrival and our fastest DNT being 16 minutes. Meidan DIDO was 47 minutes with the fastest recorded 17 minutes. Reperfusion time metrics have improved significantly over the years with the biggest impact being from a direct to CT protocol, organisational coordination and the presence of an Acute Stroke Nurse (ASN) for patients receiving thrombolysis.

COBRA: COde Blue Retrospective Audit in a metropolitan hospital.

Inhospital cardiac arrest (IHCA) is an uncommon but challenging problem. To investigate the management and outcomes of IHCA, and to investigate the effect of introducing a medical emergency team (MET) on IHCA prevalence. Retrospective medical record review of 176 adult IHCA episodes at Box Hill Hospital, a university-affiliated public hospital in metropolitan Melbourne, from July 2012 to June 2017. Inpatients receiving cardiopulmonary resuscitation for IHCA, in inpatient wards, intensive care unit, cardiac catheterisation laboratory and operating theatres were included. Data collected included demographics, resuscitation management and outcomes. Average treatment effect (ATE) was derived from margins estimates and linear regression fitted to hospital outcome, adjusted for IHCA factors. An exponentially weighed moving average control chart was used to explore IHCA prevalence over time. There were 65.3% of IHCA patients who died in hospital. IHCA prevalence was unchanged after the introduction of a dedicated MET service. Factors associated with higher likelihood of survival to discharge were initial cardiac of rhythm ventricular tachycardia (VT) (ATE 0.10 (95% CI = -0.03 to 0.25)) or ventricular fibrillation (VF) (ATE 0.28 (95% CI=0.11-0.46)), cardiac monitoring at the time of arrest (ATE 0.06 (95%CI = -0.04 to 0.16)) and time to return of spontaneous circulation (ATE 0.023 (95% CI=0.015-0.031)). IHCA is uncommon and is associated with high mortality. IHCA prevalence was unchanged after the introduction of a dedicated MET service. Factors associated with improved survival to hospital discharge were initial rhythm VT or VF, cardiac monitoring and shorter resuscitation times.

Impact of SARS-CoV-2 transmission-based precautions on inpatient management of general medical patients.

This audit reviewed the impact on access to routine medical care and adverse outcomes in patients with suspected SARS‐CoV‐2 infection managed on a ‘COVID‐19’ (CV) ward compared with a general medicine ward at Box Hill Hospital, Victoria. Data were collected at two time points to capture changes associated with onsite testing. We found no healthcare delays from admission to CV wards and observed faster exits from CV wards with improved testing efficiency. This critical finding is relevant as Victoria manages a third wave of infections.

‘A City’s Paradise’: preserving the remainder of Box Hill, voluntary social action and Country Life, 1919–1936

ABSTRACT Land was first purchased at Box Hill, Surrey, on the North Downs, to save a famous viewpoint in 1914. Safeguarding the remainder of Box Hill from development necessitated public appeals in 1919, 1923, 1926 and 1935. Led by the largely autonomous Box Hill Management Committee, and often supported by Country Life, voluntary social action in Surrey and London was mobilised to preserve adjoining land for the National Trust by accretion, intent on avoiding spoliation by villas on winding drives, extensive tree felling, streets of bungalows, and highway construction. Sub-national piecemeal protection and voluntary vigilance sustained delight in the country by subscribers who affirmed their familiarity with Box Hill, where views, trees and sequestered spaces on low and high ground offered quiet enjoyment amid common nature. Using sources which originated in the hill’s management, complemented by Country Life and newspaper reports, the article evaluates the interrelationship of locality and nation during the subscription appeals, with reference to private acts of informal benevolence and personal sense-impressions of Box Hill. The importance of providing respite from congested districts, on unembellished former wooded pasture in its natural state, is explored before sufficient national political consensus arose for the statutory protection of open country. In 1944 the Greater London Plan demanded that remaining unspoiled chalk country should be taken into public possession.

A Guzhangian (late Middle Cambrian) fauna from the Gidgealpa 1 drillhole, Warburton Basin, South Australia

A Cambrian Series 3 (Guzhangian, Boomerangian) fauna is described from Core 12 of the Gidgealpa 1 drillhole from the Warburton Basin, northeastern South Australia. There are four agnostoid taxa: Ammagnostus laiwuensis, Lejopyge calva, L. armata and an unassigned pygidium, and six polymerid trilobite taxa: Fuchouia sp., Dorypyge sp., Solenoparia changi sp. nov., Pianaspis sp., an unassigned cranidium and an unassigned pygidium. This fauna suggests a correlation with the Goniagnostus nathorsti Zone or the lower part of the Lejopyge laevigata Zone (Boomerangian on the northern Australian biostratigraphic scheme), with the latter preferred. These are the first published descriptions of fossils of this age from the Warburton Basin. Developing independent learners

Box Hill School is an independent school in Surrey, England. In 2008 the English curriculum was abandonedin favour of the International Baccalaureate Diploma (IBD). A library is a statutory requirement of theInternational Baccalaureate Organisation (IBO) endorsement who also recommend this be managed by aqualified teacher-librarian. In May 2008, I was employed to create a library and develop independentlearning throughout the school. This paper considers the rationale behind the physical design. It outlines theimplementation of independent learning in an international school community where no infrastructure topromote this style of teaching and learning previously existed.

A layered investigation of Chinese in the linguistic landscape

Abstract Increased attention to urban diversity as a site of study has fostered the recent development of linguistic landscape studies. To date, however, much of the research in this area has concerned the use and spread of English to the exclusion of other global languages. In a case study situated in Box Hill, a large suburb of Melbourne, we adopted a layered approach to investigate the role of Chinese language in Australia. Our data set consisted of hundreds of photographs of street signage in one square block area of the shopping district. Results of our analyses show that signage portrays a variety of code preferences and semiotic choices that in turn reveal insights into the identities, ideologies, and strategies that help to structure the urban environment. As demonstrated in our study, such complexity requires a renewed and situated understanding of key principles of linguistic landscape research (Ben-Rafael & Ben-Rafael, 2015).

Tunnels in Arcadia: Isambard Kingdom Brunel's Portal Designs for the Great Western Railway

AbstractFrom the 1830s, the British landscape was transformed by the development of the steam-hauled railway system, which necessitated bridges, viaducts and tunnels. Of such structures, tunnel entrances feature little in serious studies of railway architecture. However, rich archival evidence exists relating to the designs of Isambard Kingdom Brunel for the tunnel portals on the Great Western Railway between London and Bristol, including numerous pencil and ink drawings in sketchbooks held by the Brunel Archive, University of Bristol, and watercolour elevations in the Network Rail Archive in York, as well as lithographs of the portals by John Cooke Bourne for his History and Description of the Great Western Railway (1846). Brunel's drawings, unique among nineteenth-century engineers, range from the classical style for Box and Middle Hill tunnels in Wiltshire, through the Gothic for Twerton in Somerset, to the Romanesque for Brislington on the edge of Bristol, his so-called ‘Tunnel No. 1’. In their variety and careful design, Brunel's portals represent an important part of Britain's railway and industrial architectural heritage.

Chinese migration, consumption, and housing in twenty‐first century Australia

AbstractInternational migration has changed the demographics and dynamics of Australia's population since European settlement. It continues to be a principal driver in the growth of its cities, including a projected doubling of major city populations by mid‐century. The influence of migrants on metropolitan housing markets—and urban consumption more generally—has been significant, and the socio‐spatial influence of migration in Australia has been the focus of regular geographical research. Less researched is the impact that “importing” half of the nation's population growth each year has on consumption of urban resources and environmental impact, including via housing consumption practices. Evidence has also been lacking on changed consumption behaviours of migrants moving from developing to developed, high‐income societies—a barometer, perhaps, of a future trajectory of consumption within developing societies. The largest cohort of recent immigrants to Australian cities has been from China, and this trend can be expected to continue. This paper focuses on a comparison of urban consumption patterns (with particular focus on housing) among a group of 61 China‐born households prior to their migration to Melbourne post‐1995 and following their settlement in an established middle‐ring suburb of Melbourne (Box Hill). Using ecological footprint as the metric of consumption, results are striking. Not only do China‐born migrants “catch up” to Australian levels of urban consumption but they also surpass it in many cases. Findings are examined in relation to housing and urban context and a range of household sociocultural factors.

Is there a difference in adenoma detection rates between gastroenterologists and surgeons?

AIMTo compare the adenoma detection rate (ADR) between gastroenterologists and colorectal surgeons at Box Hill Hospital, Melbourne, Australia.METHODSA total of 300 colonoscopies performed by gastroenterologists and colorectal surgeons at Box Hill Hospital were retrospectively reviewed from May 2016 to June 2017. Exclusion criteria were: Patients ≤ 50 years old, colonoscopies with failure of caecal intubation, patients who previously had colon cancer and/or a colonic resection, history of polyposis syndromes or inflammatory bowel disease, or a colonoscopy within the last 10 years. Patient demographics, indications, symptoms and procedural-related outcomes were measured.RESULTSThe ADR was not significantly different between gastroenterologists and colorectal surgeons (34% vs 34.67%; P = 0.90). The adjusted odds ratio correcting for gender, age, 1st degree relative with colorectal cancer, previous colonoscopy, trainee involvement and caecal or terminal ileum intubation rate was 1.19 (0.69-2.05).CONCLUSIONBoth specialties at our institution exceed benchmark standards suggested by published Australian and American guidelines. An association between endoscopist specialty and ADR was not observed.

Beyond the Ethnic Enclave: Interethnicity and Trans‐spatiality in an Australian Suburb

AbstractThis ethnographic study investigates a suburban place in metropolitan Melbourne, Australia. It aims to reconsider the spatial formation of ethnic Chinese in a Western immigrant society beyond conventional notions about a Chinatown, an enclave, or an ethnoburb. In public enunciations, this local district, Box Hill, is depicted as an emerging Chinese enclave. In contrast, this study sheds light on multicultural engagements, interethnic interactions, and translocal and transnational mobility among the subjects who are integral to the production of the Box Hill space, through their narratives and experiences that are all in some way linked to Box Hill. This study explores everyday accommodations of multiculturalism, and yet expands this notion by underlining intersecting concepts beyond culture which together produce narratives about a suburban Chinatown. The conceptual framework of interconnectivity suggests relations and links as well as interstices and discontinuities that undermine enclave theories and to which place‐making practices across ethnicity and space are integral. Doating on Faults in Jane Austen’s Emma

ABSTRACTThis article examines the novel’s interest in powerless female characters and claims that through women’s lack the public experiences a communal loss. Emma famously attempts to repair the lack of socially dispossessed women. Engaging in a fantasy of making up for lack by marrying her friends off to wealthy men, Emma appears to embrace the reactionary side of fantasy, which attempts to escape the trauma of loss by emphasizing wholeness and full satisfaction. However, as psychoanalytic theory makes clear, fantasy has another function: it inadvertently brings the subject closer to the trauma of loss. While much of the novel’s action is focused on repairing women’s lack particularly by emphasizing the marriage plot, fantasy in the novel also leads to the insistent public encounter with loss, culminating in the Box Hill episode. The novel insists on an enjoyment in lack itself. Furthermore, for most critics, the marriage at the novel’s end between Knightley and Emma seems to function as a reactionary kind of fantasy resolution. This article, however, by focusing on the psychoanalytic concepts of fantasy and drive, emphasizes how the protagonists’ union is motivated by a desire for the experience of loss and the acknowledgment of social authority’s groundlessness.

Chinese Activities and Chinese Culture Within Community Arts in Melbourne

The purpose of this paper is to analyze current problems in the development of Chinese activities in community arts in Melbourne, including publicity issues. It can be a big subject to find out the way for Box Hill Community Arts Centre to attract Chinese and others to participate in activities. The more this is done, the more Chinese people might expect Chinese culture to be better integrated into individual communities and across communities. Community arts can have an important role to play in this.

The impact of transit-oriented development (TOD) on residential property prices: the case of Box Hill, Melbourne

Transit-oriented design (TOD) – an increase in density around transit stations – has arisen in many of Australia’s capital cities as a way to encourage mass transit ridership as well as to efficiently utilize the increase in foot and vehicle traffic that transit stations create. However, the implementation of TODs in Melbourne has faced strong opposition due to residents’ perception that the disamenities of a TOD will outweigh the benefits resulting in negative impacts on property prices. This research analyzes the relationship between proximity to a TOD and residential home prices. Results indicate that proximity to a TOD is positively related to property prices, even after controlling for neighborhood factors such as street connectivity and overall land use mix. By testing a variety of transformations of distance, we find that the benefits of TOD proximity extend approximately 1250 m from the Box Hill station. From a methodological standpoint, we find that more flexible treatments of distance variables in spatial autoregressive and spline models produce better model fit and lead to results more in line with urban economic theory.
