PROGNOSTIC FACTORS IN DEEP NECK INFECTION

Objective: The present study aimed to evaluate the prognostic importance of hematologic test findings in addition to the neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, monocyte-to-lymphocyte ratio, systemic inflammation response index, and C-reactive protein in the patients with deep neck infection and to compare these results with healthy subjects.Material and Methods: The study included 32 patients diagnosed as deep neck infection and treated by surgical intervention and parenteral antibiotics. As none of the patients had life-threatening complications, the prognosis was determined by the length of stay in the hospital. The effect of age, sex, pre-treatment blood parameters, abscess culture results, and medical comorbidities on prognosis were analysed. Pre- and post-treatment blood parameters were compared in each patient. Blood parameters were also compared between healthy subjects and patients.Results: When comparing patients with length of stay in the hospital ≤7 days and >7 days, significant differences were observed for age (t=-2.568, p=0.015) and red blood cell distribution width values obtained preoperatively (Z=-2.343, p=0.019). The correlation analysis revealed a positive correlation between age and length of stay in the hospital. (r=0.450, p=0.010), and between length of stay in the hospital and comorbidity (r=0.366, p=0.039). Logistic Regression analysis revealed that age could be the best marker in the prediction of the patients with worse prognosis (percentage of predicting patients with worse prognosis=73.3%, B=0.054, Wald=4.967, p=0.026).Conclusion: The present study confirmed that the prognosis was worse in older patients and patients with comorbid disease whereas no relation was observed between the inflammatory markers and prognosis.

Predicting postoperative ischemic stroke problems in patients following coronary bypass surgery using neutrophil-lymphocyte ratio, platelet-lymphocyte ratio, and red blood cell distribution width values.

IntroductionCoronary artery bypass grafting (CABG) plays an important role in the revascularization of ischemic heart disease. However, stroke is a rare but extremely serious complication after CABG.AimWe investigated the relationship between platelet-lymphocyte ratio (PLR), neutrophil-lymphocyte ratio, red blood cell distribution width (RDW) values, and postoperative ischemic stroke by examining the preoperative complete blood count (CBC) parameters in patients who underwent CABG.Material and methodsA total of 1240 patients who underwent CABG between September 2016 and June 2019 were included in this retrospective observational study. The diagnosis of ischemic stroke was made in the postoperative period by neurology consultation and radiological imaging. The CBC data used as preoperative values for each case were obtained the day before surgery.ResultsIschemic stroke was observed in 40 patients. In patients with stroke, the mean age was higher, the hemoglobin (HB) value and lymphocyte count were lower (p < 0.001), and there were also higher neutrophil to lymphocyte ratio (NLR), PLR, and RDW values (p < 0.001), number of mechanical ventilator days, length of stay in the hospital, and length of stay in the ICU (p < 0.001). Statin and acetylsalicylic acid use were statistically significantly higher in patients without ischemic stroke after CABG (p < 0.001); the use of other antiaggregants was found to be higher in stroke patients (p < 0.05). However, age, operation time, PLR, NLR, and RDW values were determined as independent risk factors for ischemic stroke.ConclusionsIn patients undergoing CABG, high preoperative PLR, NLR, and RDW values can be used as useful and independent risk factors for the prediction of postoperative ischemic stroke.

Aim: Anemia is a public health problem worldwide. Cost effectiveness and efficient use of resources are vitally important. Red blood cell distribution width, which can be obtained from a standard complete blood count, is a measure of the variability in size of circulating erythrocytes. The present study was performed to investigate whether red blood cell distribution width can be used to predict response to iron therapy. Methods: This study was conducted in 50 patients admitted to hematology and family medicine clinics. Complete blood count and reticulocyte count were determined on day 5; complete blood count was examined 1 month after commencement of therapy. Results: Statistically significant differences were detected between hemoglobin levels and red blood cell distribution width values at the time of diagnosis and on day 5 and after 1 month of therapy. A significant positive correlation was found between the increase in red blood cell distribution width and the increase in hemoglobin. Conclusion: Red blood cell distribution width may be used in place of reticulocyte count to predict response to iron therapy. Red blood cell distribution width is the best biomarker for this purpose as a component of complete blood count, and therefore it may be accepted as superior to reticulocyte count.

A cross-sectional comparison between cardiorespiratory fitness, level of lesion and red blood cell distribution width in adults with chronic spinal cord injury

ObjectivesTo assess; (1) differences in red blood cell distribution width between individuals with chronic (>1year), motor-complete cervical (n=21), upper-thoracic (n=27) and thoracolumbar (n=15) spinal cord injury and, (2) associations between red blood cell distribution width and cardiorespiratory fitness. DesignProspective multi-center, cross-sectional study. MethodsPeak oxygen uptake was determined using an upper-body arm-crank exercise test to volitional exhaustion and red blood cell distribution width was measured using an automated hematology system. ResultsThere were significant (p<0.009) differences between groups classified by level of injury in absolute and relative peak oxygen uptake, peak power output and red blood cell distribution width. A significant (p<0.001) large negative association (r = −0.524) was found between relative peak oxygen uptake and red blood cell distribution width. Unbiased recursive partitioning, while revealing study site specific differences in red blood cell distribution width, identified homogenous subgroups based specifically on cardiorespiratory fitness irrespective of additional demographic and injury characteristics. ConclusionThe strong negative association between cardiorespiratory fitness and red blood cell distribution width in individuals with paraplegia parallel those previously observed in non-disabled individuals. Higher red blood cell distribution width values are an independent risk factor for increased cardiovascular mortality, heart failure, and coronary heart disease and may reflect several underlying exacerbated metabolic responses such as oxidative stress and systemic inflammation. These data emphasize the importance of maintaining a high aerobic capacity following spinal cord injury.

Red Blood Cell Distribution Width in Quarter Horses: A Comparison Between Healthy and Hospitalized Animals

Red blood cell distribution width (RDW) is a hematimetric index used to diagnose anisocytosis. In human medicine, it has been widely used as a predictor and biomarker of various diseases, but there are few comparable studies in equine medicine. We aimed to compare RDW values between healthy and hospitalized Quarter Horses (QH). A total of 176 QH were evaluated, 95 healthy animals reared on 27 farms, and 81 animals treated at a Veterinary School Hospital. Blood samples were evaluated using an automated hematological analyzer to obtain RDW. Red blood cell distribution width values in hospitalized horses (18.6%) were significantly higher than in healthy ones (17.8%, P = .0006). Values from our healthy group can be used as a reference for QH. We suggest that further studies are needed to evaluate the use of RDW as a biomarker or predictor of various disorders or diseases in equines.

Fournier Gangrene: Association of Mortality with the Complete Blood Count Parameters

The authors studied the alterations in mean platelet volume, neutrophil-to-lymphocyte ratio, and red blood cell distribution width values together with the platelet count in hospitalized patients diagnosed with Fournier gangrene to determine their association with disease prognosis. Records of patients diagnosed with Fournier gangrene were analyzed retrospectively. Seventy-four patients (49 men and 25 women) with a mean age of 57.60 ± 15.34 years (range, 20 to 95 years) were included. Sixty-eight participants were discharged and six died during follow-up. In the discharged group, during hospitalization, there was a trend downward in neutrophil-to-lymphocyte ratio and mean platelet volume values, whereas platelet count increased significantly. In the nonsurvivor group, the neutrophil-to-lymphocyte ratio and mean platelet volume after first débridement and at the end of hospitalization were significantly higher; platelet counts at admission, after the first débridement, and at the end of hospitalization were significantly lower compared with the survivor group (p < 0.05). In correlation analysis, mortality rate was negatively correlated with platelet count at admission and after first débridement and positively correlated with the neutrophil-to-lymphocyte ratio and mean platelet volume after first débridement. Regarding the receiver operating characteristic curve analyses, a platelet count of 188,500/µl at admission and 196,000/µl after the first débridement, a neutrophil-to-lymphocyte ratio of 13.71, and a mean platelet volume of 9.25 fl after the first débridement were defined as the cutoff levels having the best sensitivities and specificities. This study suggests that platelet count at admission and platelet count, mean platelet volume, and neutrophil-to-lymphocyte ratio after first débridement and during discharge may be included among the prognostic scores of Fournier gangrene. The authors defined some threshold values that can be used during patient follow-up. Larger prospective studies are warranted to determine the exact role of those parameters in the prognosis of Fournier gangrene. Risk, III.

Are neutrophil, platelet and eosinophil-to-lymphocyte ratio and red blood cell distribution width can be used for nasal polyposis?

The aim of this study is to explore the relationship of neutrophil, platelet and eosinophil-to-lymphocyte ratios and red blood cell distribution width values with nasal polyposis, and whether this could be a predictive parameter that can be used for the severity and recurrence risk of the disease. Neutrophil/lymphocyte, platelet/lymphocyte, eosinophil/lymphocyte ratios and red blood cell distribution width values were compared between nasal polyposis patients who were reviewed in a retrospective manner and the control group comparable for age and sex. Moreover, the nasal polyposis group was grouped for nasal polyposis severity and recurrence, and these subgroups were statistically compared, too. When the nasal polyposis patient group and the control group are compared, no statistically significant difference was found between two groups except for eosinophil/lymphocyte ratio. While only the difference between nasal polyposis severity and eosinophil/lymphocyte ratio is statistically significant, there was no parameter with statistically significant difference for recurrence. We think that caution should be exercised when using these new hematological parameters which can be affected by many factors for the presence, severity and recurrence risk of nasal polyposis, and literature data should be proven by conducting different and objective studies on this subject.

Can whole-blood parameters be used in follow-up of children with rheumatic valvular heart disease?

The aim of the present study was to investigate the relationships between red blood cell distribution width, platelet distribution width, and mean platelet volume and the presence and severity of valvular involvement in patients with rheumatic heart disease. Between April, 2012 and December, 2015, 151 patients who were admitted to the Pediatric Cardiology Unit with diagnosis of rheumatic heart disease and 148 healthy children were included to our study. Transthoracic echocardiography for all children was performed, and the values of red blood cell distribution width, platelet distribution width, and mean platelet volume, besides other blood count parameters, erythrocyte sedimentation rate, and C-reactive protein levels were recorded. Red blood cell distribution width, platelet distribution width, mean platelet volume, and C-reactive protein levels were significantly higher in patients with rheumatic heart disease when compared with healthy controls (p0.05). This is the first study in children with rheumatic heart disease that demonstrated significantly increased red blood cell distribution width, platelet distribution width, and mean platelet volume levels, as well as evaluated all three parameters together. Furthermore, red blood cell distribution width values in the chronical period of acute rheumatic fever, due to the positive correlation with the other chronic inflammatory markers, may help make the diagnosis in children.

Effect of Left Ventricular Assist Devices on Red Blood Cell Distribution Width.

Red blood cell distribution width is a measure of the variability in the size of circulating erythrocytes, which is calculated by automated blood cell counters as part of a routine blood cell count analysis. Our aim was to examine whether continuous flow left ventricular assist devices affected red blood cell distribution width. Our study included 16 of 24 patients who received a HeartWare HVAD Pump (HeartWare Inc, Framingham, MA, USA) implant (continuous flow left ventricular assist device) between April 2012 and February 2015 at our institution. The mean age of the patients was 46.6 years. We compared patient erythrocyte, leucocyte, and platelet counts, hemoglobin and hematocrit levels, mean corpuscular volume, and red blood cell distribution width values before and 6 months after implant. Hemoglobin level (P = .008), hematocrit level (P = .027), and mean corpuscular volume (P = .003) were significantly decreased; however, we observed no significant change in red blood cell distribution width. Although we did not find significant results in our group of patients with left ventricular assist device implant, a larger group of patients may show more significant results, and an increase in red blood cell distribution width can be used as an indicator of a negative prognosis in these patients.

The relationship between complete blood count parameters and Fontaine’s Stages in patients with peripheral arterial disease

The aim of the present study is to evaluate whether blood count parameters differ according to the stages of Fontaine's classification and to investigate the relationship between hemogram parameters and the severity of the disease. Eighty-two peripheral arterial disease patients were examined prospectively. Patients were classified according to the Fontaine classification system. Fifty newly diagnosed patients were included in the study. The neutrophil-to-lymphocyte ratio, mean platelet volume, and red blood cell distribution width values were recorded. Mean neutrophil-to-lymphocyte ratio values were found to be 3.31 ± 1.1% in Stage I, 3.11 ± 1.3% in Stage II, and 3.48 ± 1.1% in Stage III (p > 0.05). Mean platelet volume values were found to be 7.8 ± 0.6 fl (Stage I), 8.2 ± 1.0 fl (Stage II), and 9.0 ± 0.9 fl (Stage III) (p < 0.05). Red blood cell distribution width values were found to be 13.6 ± 1.0% in Stage I, 14.8 ± 1.7% in Stage II, and 15.4 ± 2.3% in Stage III, being significantly different among all three stages (p < 0.05). Both red blood cell distribution width and mean platelet volume are found to be associated with the severity of atherosclerotic disease in patients with peripheral arterial disease. This finding hypothesizes that complete blood counting parameters may serve as a beneficial and cost-effective method for monitoring atherosclerotic peripheral disease.

The Influence of Erythropoietin (EPO T→G) and α-Actinin-3 (ACTN3 R577X) Polymorphisms on Runners' Responses to the Dietary Ingestion of Antioxidant Supplementation Based on Pequi Oil (Caryocar brasiliense Camb.): A Before-After Study

Background/Aims: As diet can affect an individual's genes and these can affect response to supplementation, we aimed to investigate the influence of erythropoietin (EPO T→G) and α-actinin-3 (ACTN3 R577X) polymorphisms on plasma lipid peroxidation, hemogram and biochemical dosages of creatine kinase, aspartate aminotransferase, alanine aminotransferase and C-reactive protein (including high-sensitivity C-reactive protein) of runners (n = 123) before and after 14 days of 400 mg pequi oil supplementation, a natural carotenoid-rich oil, after races under closely comparable conditions. Methods/Results: Blood samples were taken immediately after racing to perform the tests. Before pequi oil supplementation, EPO polymorphism influenced erythrogram and plateletgram results, suggesting an aerobic advantage for the TG genotype and a disadvantage for the GG genotype as regards possible microvascular complications, while no association was found for ACTN3 polymorphism with endurance performance. Both polymorphisms influenced the runners' response to pequi oil: significant responses were observed for the EPO TT genotype in erythrocyte, hematocrit, mean corpuscular hemoglobin and mean corpuscular hemoglobin concentration values, and for the TT and TG genotypes in red blood cell distribution width values. Significant differences were also observed in the plateletgram for the TT and TG genotypes. ACTN3 mainly influenced aspartate aminotransferase and creatine kinase values: heterozygotes had a significant reduction in aspartate aminotransferase values and homozygous individuals (XX) in creatine kinase values after pequi oil supplementation. Conclusion: These results emphasize the importance of studying nutrigenomic effects on athletes' performance.

Haematological changes in elite kayakers during a training season

This study monitored haematological markers in response to training load in elite kayakers during a training season. The sample comprised eight elite kayakers aged 22 ± 4.2 years with a 77.2 ± 6.7 kg body mass and a 177.5 ± 5.6 cm stature. The initial [Formula: see text]O(2max) was 61.2 ± 5.5 mL·kg(-1)·min(-1). The control group consisted of six healthy males, aged 18.6 ± 1.1 years, with an 81.3 ± 13.8 kg body mass and a 171.9 ± 4.5 cm stature. Blood samples were collected at the beginning of the training season after an off-training period of six weeks (t(0)), at the 11th week after the application of high training volumes (t(1)), at the 26th week after an intense training cycle (t(2)), and at the 31st week at the end of a tapering phase (t(3)). Differences between time points were detected using ANOVA and the Bonferroni post hoc test. Significant changes were found after the intense training cycle (t(2)), lymphocytes decreased while haemoglobin, mean corpuscular volume, mean corposcular haemoglobin, mean concentration of corpuscular hemoglobin concentration, platelets distribution width, and red blood cell distribution width values increased when compared with baseline values. At t(3), a reduction in monocyte numbers and an increase in mean platelet volume compared with baseline values were seen. By reducing the volume and intensity of training, many variables returned to values close to those at baseline. Although many athletes had accumulated responses over time due to training, they still suffered transient changes that appear to be influenced by training load. Haemorheology monitoring may help detect health risks, especially during times of intensified training.

Red Blood Cell Distribution Width (RDW) and its Association with Coronary Atherosclerotic Burden in Patients with Stable Angina Pectoris

Although there are several studies regarding the association between RDW and the vascular events, information is scant about possible role of RDW in cardiovascular system. We aimed to investigate whether RDW is related with the severity and extent of angiographically assessed coronary artery disease (CAD). Two hundred ninety and six stable eligible patients who had undergone coronary angiography with a suspicion of CAD were enrolled consecutively. Two hundred and nine (71%) of 296 patients had CAD (men 70%, mean age±SD: 61±11yrs) and 87 patients (29%) had normal coronary arteries (NCA) without any atherosclerotic lesion (men 48%, 52±11yrs). Red blood cell distribution width values were significantly different among the subgroups determined for the severity and extent of CAD. When 14.8% was accepted as the cut-off value for RDW, the sensitivity and the specificity for the detection of CAD was 68% and 52%. When we performed multiple logistic regression analysis to determine the independent predictors of CAD, we found a positive independent relationship between age, gender, family history of CAD and RDW and CAD. Our results show that RDW has a significant relationship with CAD independent of nonspecific inflammation and circulating inflammatory cells. Although we cannot conclude the underlying pathologic process of RDW, we believe that these findings may pave the way for further studies searching the role of RDW in atherosclerosis.
