Temporal corridors of forces and moments, and injuries to pelvis-lumbar spine in vertical impact simulating underbody blast

Pelvis and lumbar spine fractures occur in falls, motor vehicle crashes, and military combat events. They are attributed to vertical impact from the pelvis to the spine. Although whole-body cadavers were exposed to this vector and injuries were reported, spinal loads were not determined. While previous studies determined injury metrics such as peak forces using isolated pelvis or spine models, they were not conducted using the combined pelvis-spine columns, thereby not accounting for the interaction between the two body regions. Earlier studies did not develop response corridors. The study objectives were to develop temporal corridors of loads at the pelvis and spine and assess clinical fracture patterns using a human cadaver model. Vertical impact loads were delivered at the pelvic end to twelve unembalmed intact pelvis-spine complexes, and pelvis forces and spinal loads (axial, shear and resultant and bending moments) were obtained. Injuries were classified using clinical assessments from post-test computed tomography scans. Spinal injuries were stable in eight and unstable in four specimens. Pelvis injuries included ring fractures in six and unilateral pelvis in three, sacrum fractures in ten, and two specimens did not sustain any injuries to the pelvis or sacrum complex. Data were grouped based on time to peak velocity, and ± one standard deviation corridors about the mean of the biomechanical metrics were developed. Time-history corridors of loads at the pelvis and spine, hitherto not reported in any study, are valuable to assess the biofidelity of anthropomorphic test devices and assist validating finite element models.

Isometric, concentric, and eccentric neck strength in the sagittal and coronal planes of motion for adult females

Objective Assess strength in adult females using multiple positions, motions, and contraction types, to better understand strength production of young and non-symptomatic of adult female subjects to help assess and improve the biofidelity of anthropomorphic test devices and human body models. Methods Fifteen adult females (25.4 ± 6.3 years) were recruited for this study. Strength measurements were collected for the sagittal and coronal planes during isometric, concentric, and eccentric muscle contractions in neutral and mid-range of motion anatomical positions. Results For both planes, subjects were strongest during eccentric muscle contractions and weakest in concentric muscle activations. In the sagittal plane, subjects were stronger in extension for all muscle activation types and anatomical positions. In the coronal plane, there were no side differences in isometric nor concentric strength. Conclusions Neck strength of adult females depends on muscle activation type and anatomical positions. Future computational models should account for muscle activation type when quantifying responses of female subjects.

Cervical spine injury in rollover crashes: Anthropometry, excursion, roof deformation, and ATD prediction

While rollover crashes are rare, approximately one third of vehicle occupant fatalities occur in rollover crashes. Most severe-to-fatal injuries resulting from rollover crashes occur in the head or neck region, due to head and neck interaction with the roof during the crash. While many studies have used anthropomorphic test devices (ATDs) to predict head and neck injury, the biofidelity of ATDs in rollover has not been established. This study aims to build on previous research to compare the dynamic response and injuries sustained by four post mortem human surrogates (PMHS) to those predicted by six different ATDs in full-scale rollover crash tests. Additionally, this study evaluates injuries sustained by PMHS relative to possible contributing factors including occupant kinematics, occupant anthropometry, and vehicle roof deformation. While the vehicle kinematics and roof deformation were comparable for all tests, three out of the four PMHS sustained cervical spine injury, but only the tallest specimen sustained cervical spine fracture. Neck flexion at the time of head-to-roof contact appears to have affected cervical spine injury risk in these cases. Despite the injuries sustained in the PMHS, none of the six ATDs measured forces or accelerations that exceeded injury assessment reference values (IARVs), which adds to recent literature illustrating substantial differences between ATDs and PMHS in a rollover-like scenario.

Impact sites representing potential bruising locations associated with bed falls in children

Bruising can occur as a result of accidental or abusive trauma in children. Bruises are an early sign of child abuse and their locations on the body can be an effective delineator of abusive trauma. Since falls are often reported as false histories in abuse, the ability to predict potential bruising locations in falls could be valuable when attempting to differentiate between abuse and accident. In our study we used an anthropomorphic test device (ATD), a surrogate representing a 12 month old child, adapted with a custom developed force sensing skin to predict potential bruising locations during simulated bed falls. The sensing skin is made of custom resistive force sensors integrated into a conformable skin, adapted to fit the contours of the ATD. The sensing skin measured and displayed recorded force data on a computerized body image mapping system when sensors were activated. Simulated bed fall experiments were performed from two initial positions (FF – facing forward and FR – facing rearward) and two fall heights of 61cm (24 in) and 91cm (36 in) onto a padded carpet impact surface. Findings indicated potential bruising primarily in two planes of the ATD body. The majority of contact regions and greater forces were recorded in one plane, with fewer regions of contact and decreased force exhibited in an adjoining second plane. Additionally, no contact was recorded in the two planes opposite the impact planes. Differences in contact regions were observed for varying heights and initial position. Limitations of ATD biofidelity and soft tissue properties must be considered when interpreting these findings.

Normalized frontal impact biofidelity kinematic corridors using post mortem human surrogates.

Due to reducing cost and powerful computing resources and the ability of finite element human body models (FEHBM) to predict human body response more realistically, they are gaining acceptance to be a substitute for mechanical surrogates. Unlike mechanical surrogates, FEHBM can realistically simulate human kinematics and kinetics. Moreover, an array of quantities can be directly measured from FEHBMs. However, similar to Anthropomorphic Test Devices (ATDs), in order to evaluate the biofidelity, these models must be validated using PMHS response corridors. Therefore, availability of such PMHS corridors that can be used to validate both ATD and FEHBM kinematics is of primary importance. The current study presents normalized biofidelity corridors of head CG, T1, T12, and sacrum accelerations using PMHS frontal sled tests that were previously conducted. In addition, rotational accelerations and displacements of the head are also presented. The experimental data were collected using four specimens. Each specimens were tested with non-injurious pulses using two different velocities (low: 3.6m/s and medium: 6.9m/s). These data were normalized using mass-based technique to represent mid-sized United States population. Using the normalized data, average and plus/minus one standard deviation response corridors were generated that can be used to evaluate the biofidelity of ATDs and FEHBMs.

DEVELOPMENT AND BIOFIDELITY EVALUATION OF AN OCCUPANT BIOMECHANICAL MODEL OF A CHINESE 50TH PERCENTILE MALE FOR SIDE IMPACT

Finite element modeling has played a significant role in the study of human body biomechanical responses and injury mechanisms during vehicle impacts. However, there are very few reports on similar studies conducted in China for the Chinese population. In this study, a high-precision human body finite element model of the Chinese 50th percentile male was developed. The anatomical structures and mechanical characteristics of real human body were replicated as precise as possible. In order to analyze the model’s biofidelity in side-impact injury prediction, a global technical standard, ISO/TR 9790, was used that specifically assesses the lateral impact biofidelity of anthropomorphic test devices (ATDs) and computational models. A series of model simulations, focusing on different body parts, were carried out against the tests outlined in ISO/TR 9790. Then, the biofidelity ratings of the full human body model and different body parts were evaluated using the ISO/TR 9790 rating method. In a 0–10 rating scale, the resulting rating for the full human body model developed is 8.57, which means a good biofidelity. As to different body parts, the biofidelity ratings of the head and shoulder are excellent, while those of the neck, thorax, abdomen and pelvis are good. The resulting ratings indicate that the human body model developed in this study is capable of investigating the side-impact responses of and injuries to occupants’ different body parts. In addition, the rating of the model was compared with those of the other human body finite element models and several side-impact dummy models. This allows us to assess the robustness of our model and to identify necessary improvements.

Development and evaluation of a finite element model of the THOR for occupant protection of spaceflight crewmembers

New vehicles are currently being developed to transport humans to space. During the landing phases, crewmembers may be exposed to spinal and frontal loading. To reduce the risk of injuries during these common impact scenarios, the National Aeronautics and Space Administration (NASA) is developing new safety standards for spaceflight. The Test Device for Human Occupant Restraint (THOR) advanced multi-directional anthropomorphic test device (ATD), with the National Highway Traffic Safety Administration modification kit, has been chosen to evaluate occupant spacecraft safety because of its improved biofidelity.NASA tested the THOR ATD at Wright-Patterson Air Force Base (WPAFB) in various impact configurations, including frontal and spinal loading. A computational finite element model (FEM) of the THOR to match these latest modifications was developed in LS-DYNA software. The main goal of this study was to calibrate and validate the THOR FEM for use in future spacecraft safety studies.An optimization-based method was developed to calibrate the material models of the lumbar joints and pelvic flesh. Compression test data were used to calibrate the quasi-static material properties of the pelvic flesh, while whole body THOR ATD kinematic and kinetic responses under spinal and frontal loading conditions were used for dynamic calibration. The performance of the calibrated THOR FEM was evaluated by simulating separate THOR ATD tests with different crash pulses along both spinal and frontal directions. The model response was compared with test data by calculating its correlation score using the CORrelation and Analysis rating system. The biofidelity of the THOR FEM was then evaluated against tests recorded on human volunteers under 3 different frontal and spinal impact pulses.The calibrated THOR FEM responded with high similarity to the THOR ATD in all validation tests. The THOR FEM showed good biofidelity relative to human-volunteer data under spinal loading, but limited biofidelity under frontal loading. This may suggest a need for further improvements in both the THOR ATD and FEM. Overall, results presented in this study provide confidence in the THOR FEM for use in predicting THOR ATD responses for conditions, such as those observed in spacecraft landing, and for use in evaluating THOR ATD biofidelity.

Occupant Kinematics in Laboratory Rollover Tests: ATD Response and Biofidelity.

Rollover crashes are a serious public health problem in United States, with one third of traffic fatalities occurring in crashes where rollover occurred. While it has been shown that occupant kinematics affect the injury risk in rollover crashes, no anthropomorphic test device (ATD) has yet demonstrated kinematic biofidelity in rollover crashes. Therefore, the primary goal of this study was to assess the kinematic response biofidelity of six ATDs (Hybrid III, Hybrid III Pedestrian, Hybrid III with Pedestrian Pelvis, WorldSID, Polar II and THOR) by comparing them to post mortem human surrogate (PMHS) kinematic response targets published concurrently; and the secondary goal was to evaluate and compare the kinematic response differences among these ATDs. Trajectories (head, T1, T4, T10, L1 and sacrum), spinal segment (head-to-T1, T1-to-T4, T4-T10, T10-L1, and L1-to-sacrum) rotations relative to the rollover buck, and spinal segment extension/compression were calculated from the collected kinematics data from an optical motion tracking system. Response differences among the ATDs were observed mainly due to the different lateral bending stiffness of the spine from their varied architecture, while the additional thoracic joint in Polar II and THOR did not seem to provide more flexion/extension compliance than the other ATDs. In addition, the ATD response data were compared to PMHS response corridors developed from similar tests for assessing ATD biofidelity. All of the ATDs, generally, drifted outboard and upward during the tests similar to the PMHS. However, accompanied with this upward and outward motion, the ATD head and upper torso pitched forward (~10 degrees) while the PMHS' head and upper torso pitching rearward (~10 to ~15 degrees), due to the absence of flexion/extension compliance in the ATD spine. The differences in these pitch motions resulted in a difference of 130 mm to 160 mm in the longitudinal position of the head at 195 degrees of roll angle. Finally, substantially less lateral spinal bending was also observed in the ATDs compared to the PMHS. The results of the current study suggests there is greater upper spine flexion/extension, and lateral bending stiffness in all of the ATDs in comparison to the PMHS, and provided information for improvement of ATD biofidelity in future for rollover crashes.

Normalizing and scaling of data to derive human response corridors from impact tests

It is well known that variability is inherent in any biological experiment. Human cadavers (Post-Mortem Human Subjects, PMHS) are routinely used to determine responses to impact loading for crashworthiness applications including civilian (motor vehicle) and military environments. It is important to transform measured variables from PMHS tests (accelerations, forces and deflections) to a standard or reference population, termed normalization. The transformation process should account for inter-specimen variations with some underlying assumptions used during normalization. Scaling is a process by which normalized responses are converted from one standard to another (example, mid-size adult male to large-male and small-size female adults, and to pediatric populations). These responses are used to derive corridors to assess the biofidelity of anthropomorphic test devices (crash dummies) used to predict injury in impact environments and design injury mitigating devices. This survey examines the pros and cons of different approaches for obtaining normalized and scaled responses and corridors used in biomechanical studies for over four decades. Specifically, the equal-stress equal-velocity and impulse-momentum methods along with their variations are discussed in this review. Methods ranging from subjective to quasi-static loading to different approaches are discussed for deriving temporal mean and plus minus one standard deviation human corridors of time-varying fundamental responses and cross variables (e.g., force-deflection). The survey offers some insights into the potential efficacy of these approaches with examples from recent impact tests and concludes with recommendations for future studies. The importance of considering various parameters during the experimental design of human impact tests is stressed.

Effect of wheelchair headrest use on pediatric head and neck injury risk outcomes during rear impact

Comparative risks or benefits to wheelchair-seated pediatric occupants in motor vehicles associated with wheelchair headrest use during rear impact were evaluated using pediatric head and neck injury outcome measures. A Hybrid III 6-year-old anthropomorphic test device (ATD), seated in identical WC19-compliant pediatric manual wheelchairs, was used to measure head and neck response during a 25 km/h (16 mph), 11 g rear impact. ATD responses were evaluated across two test scenarios: three sled tests conducted without headrests, and three with slightly modified commercial headrests. Head and neck injury outcomes measures included: linear head acceleration, head injury criteria (HIC) values, neck injury criteria ( N ij ) values, and combined rotational head velocity and acceleration. Neck and head injury outcome measures improved by 34–70% in sled tests conducted with headrests compared to tests without headrests. Headrest use reduced N ij values and the likelihood of concussion from values above established injury thresholds to values below injury thresholds. Injury measure outcome reductions suggest lower head and neck injury risks for wheelchair-seated children using wheelchair-mounted headrests as compared to non-headrest users in rear impact. Use of relative comparisons across two test scenarios served to minimize effects of ATD biofidelity limitations.

Analysis of Nij in simulated real-world crashes with a 3-year-old Hybrid-III

Research and development in the field of child occupant crash protection relies heavily on the biofidelity of anthropomorphic test devices (ATDs) used in testing and the ability to relate measured parameters on the ATD to injury. However, there is very little live subject or cadaver testing with children to validate the biofidelity of the current state-of-the-art child ATDs. In an effort to bridge the gap, a study was conducted to determine whether the Hybrid-III 3-year-old ATD could reasonably re-create the dynamic response of a child in a real-world crash and whether the ATD could reliably predict injuries. In other words, does the data from testing with the ATD give results consistent with real-world experience? This paper contains an analysis of the neck load and moment data collected during this testing using Nij as an analysis tool to predict neck injury. This expands on prior analysis presented by the authors in 1999, which included a discussion of the initial results of a test program and analysis of individual force and moment data. At that time, Nij values could not be calculated and related to injury due to disagreement in the community as to the specific method for evaluating Nij as well as how the value would relate to injury.
