Asymmetrical Bilateral Traumatic Hip Dislocation with Segmental Femur Fracture

Introduction: Hip dislocations are extremely rare, but they are orthopedic emergencies that need to be immediately intervened. Traumatic hip dislocations are usually unilateral and occur toward the posterior region. Bilateral traumatic hip dislocations, on the other hand, are rarer. Asymmetric occurrence of bilateral traumatic hip dislocations is even rarer. The possibility of spontaneous hip dislocation in a healthy person is very unlikely. This is because ligaments wrapping the hip joint make it quite stable. Thus, hip dislocation usually occurs due to a high-energy trauma. Case Report: In this paper we report a unique female patient with asymmetric bilateral traumatic hip dislocation accompanied by a segmental fracture of the femur due to a traffic accident. The patient’s right hip was reduced under sedoanalgesia at the emergency department and the left hip was reduced under anesthesia at operating room. Avascular necrosis did not develop at her 2-year follow-up; however, she has been re-operated after developing nonunion in the left femoral segmental fracture. Conclusion: In conclusion, hip dislocations should be treated in first six hours due to the risk of avascular necrosis. Uncomplicated hip dislocations without accompanied fractures can be treated with sedoanalgesia in emergency service settings. However hip dislocations which are complicated with accompanied fractures should be reduced in operation rooms without delay.

Traumatic bilateral posterior hip dislocation in children. A 12 years follow up case report

Background. Bilateral traumatic hip dislocation in children is a very rare orthopedic emergency. Few case reports are available in literature.
 Clinical case. A 4-year-old male child case with a trivial mechanism of injury is presented. Closed reduction in emergency department was achieved after 24 hours of injury, he was placed on Bucks skin traction for 4 days and during 4 weeks in a spica cast. There was not either clinical sign nor images of early or late complications during follow up until he reached skeletal maturity.
 Discussion. Hip dislocation in children is a rare emergency with an incidence of 0.8 cases per million per year, its treatment should not be delayed to minimize late complications such as osteonecrosis, recurrent dislocations, osteoarthritis, neurological lesions, coxa magna and heterotopic ossification. The reported incidence for hip osteonecrosis is 36.4% for late (6 hours) and 8.2% after early (6 hours) reduction. After 12 years of follow up no complication was found although the reduction was made 24 hours later.
 Conclusions. After immediate reduction a 4 to 6 weeks immobilization period is an effective treatment. Close monitoring to timely identify and treat any further complication is mandatory.

Bilateral traumatic hip dislocation associated with sacro-iliac dislocation.

We report a rare injury in an 18-year-old woman who sustained posterior bilateral hip dislocation with sacro-iliac dislocation after a high energy motor vehicle accident. She was treated by closed reduction and skeletal traction. Bilateral traumatic hip dislocation is an uncommon occurrence. Rarer still is bilateral traumatic hip dislocation associated with sacro-iliac dislocation because it combines two different mechanisms of trauma. (Hip International 2002; 1: 47-9).

Bilateral Asymmetric Traumatic Hip Dislocation with Bilateral Acetabular Fracture: Case Report

Bilateral traumatic hip dislocation is a very rare condition. Simultaneous anterior and posterior traumatic dislocation of both hips is even more unusual. A case report of a bilateral asymmetrical hip joint dislocation with bilateral acetabular fracture in a 28-year-old man is presented.

Traumatic dislocation of bilateral hips and ankles

Abstract Bilateral traumatic hip dislocation is rare, and pure ankle dislocation without bony fracture is also rare. We present a patient who sustained bilateral hip and bilateral ankle dislocations without associated fracture after a fall from a significant height. Management consisted of immediate closed reduction of bilateral hips and bilateral ankles under conscious sedation in the emergency department, followed by immobilization of the ankles with short leg splints. After 2 weeks of bed rest with skin traction, the patient was initially mobilized in a wheelchair. He was allowed partial weight bearing after 4 weeks. Clinical and radiographic follow-up continued for 2 years after the accident. The patient reported complete disappearance of pain in hips and ankles. Good range of motion of the hips and ankles was also noted. There was no radiographic evidence of avascular necrosis, osteoarthritis, or calcification at 2-year follow-up.

Bilateral Asymmetric Hip Dislocation With Segmental Fracture Femur : An Unusual Case Report And Literature Review

Bilateral traumatic hip dislocation is rarely seen. A unique case is presented, consisting of bilateral asymmetric hip dislocation with associated segmental fracture femur, resulting from fall from train. This case represents an unusual, severe combination of injuries resulting from the fall from train under influence of alcohol. Traumatic hip dislocation represents a true orthopaedic emergency .Given the severity of associated complications, every effort should be made to ensure prompt diagnosis and immediate therapy. We report our experience in the management of this complex injury pattern and review the pertinent literature on this subject.

We report a 33-year-old trader with bilateral traumatic anterior hip dislocation following a road traffic accident. Both hip dislocations were reduced under general anesthesia followed by immobilization of the hips with skin traction. He discharged himself against medical advice one week after injury.

We report a rare injury in a 41-year-old man who sustained bilateral hip dislocations, anterior on the right side and posterior on the left, with concurrent acetabular fractures after a high - energy motor vehicle accident. He was treated with closed reduction and skeletal traction. One year after the injury the result was satisfactory without any signs of avascular necrosis.

We report a case of simultaneous asymmetrical bilateral traumatic hip dislocation, with one hip dislocated anteriorly and the other posteriorly, with ipsilateral acetabular fracture, suffered in a traffic accident by a 36-year-old man. Simultaneous asymmetric bilateral traumatic hip dislocation

A 25-year-old man presented to the emergency department with simultaneous right anterior and left posterior hip dislocations resulting from a motor vehicle accident. Simultaneous bilateral traumatic hip dislocation is an uncommon occurrence. More rare is asymmetric simultaneous bilateral hip dislocation, with only ten cases documented in the English-language literature. The importance of an anteroposterior pelvic radiograph after significant major trauma is emphasized. Several serious complications can be associated with this condition, so expeditious management in the ED is warranted.
