Introduction: Shoulder dislocations, common in emergency departments, often include anterior dislocations. Bilateral dislocations, particularly posterior, are rare and are generally caused by sporting accidents or convulsions. Bilateral anterior dislocations are even rarer and often of traumatic origin. Case report: We analyzed three new cases of bilateral anterior shoulder dislocation, examining the mechanisms, risks and treatments Discussion: Bilateral dislocation, often posterior but sometimes anterior, is rare and can result from high-energy trauma or convulsions. A careful clinical examination is crucial to avoid diagnostic errors. Approximately 15% of patients have greater tuberosity fractures. Arthroscopic Bankart and open Latarjet procedures are often recommended for the treatment of residual instability.

Bilateral Anterior Shoulder Dislocation: A Systematic Review.

To compile the existing literature on bilateral anterior shoulder dislocation (BASD) and analyze patient demographics, mechanisms of injury, injury characteristics, management, and outcome. This systematic review was conducted in accordance with Preferred Reporting Items for Systematic review and Meta-Analyses (PRISMA) guidelines. Online databases, including Ovid Medline 1946-, Embase.com 1947-, Scopus 1960-, Cochrane Central, and Clinicaltrials.gov were systematically queried. Studies eligible for inclusion were case reports or case-series, documenting BASD. Two reviewers independently screened and applied a set of a priori exclusion criteria to each returned study. Data were extracted, compiled, and synthesized from each reported case of BASD. Contingency tables/Chi-Square Analyses, T-tests, and univariate regression analyses were conducted to assess relationships between different variables. Eighty-one studies (87 cases of BASD) were included. Patients were 41.1 (SD± 19.5) years old and most were male (n=63; 72.4%). Around a quarter of patients (28.7%) had a history of epilepsy/seizures or were being worked-up for such. Younger males were more likely to have BASD due to a seizure or electrocution (P<0.05). Close to a third of cases (n=27; 31.0%) were delayed in presentation. Those sustaining seizures or electrocutions were more likely to be delayed in presentation (P=0.013). Most events resulted in simple dislocations that were closed reduced successfully. BASD resulting from seizures or electrocutions were more likely to be fracture-dislocations (P=0.018); and in younger patients with fracture-dislocations, closed reduction was more often to fail or not be attempted (P<0.05). Median follow-up was 6 months (IQR: 3 months - 12 months). Seven patients (10.6%) had complications and 4 (2.3%) demonstrated recurrent instability. In young males presenting with BASD without known trauma, suspicion should be high for a convulsant event. In patients with a known seizure disorder who present with chronic bilateral shoulder or arm pain, BASD should be considered and work-up should be expedited to avoid misdiagnosis.

Pure anterior bilateral shoulder dislocations are rare clinical features, especially in traumatic forms. They are most often posterior, occurring during an epileptic seizure. Few cases are described in the literature, and the mechanism varies from case to case. We report a specific case of pure bilateral anterior shoulder dislocation in a 29-year-old judo player following an accident during his training and discuss the circumstances, mechanism, treatment, and prognosis.

Central Cord Syndrome Mimic-Bilateral Anterior Shoulder Dislocations: A Case Report

Unilateral anterior shoulder dislocations are the most common major joint dislocation. However, bilateral shoulder dislocations are rare, and when present are mostly posterior. Given the rarity of this clinical entity, many emergency physicians would suspect central cord syndrome in a patient who presents with disproportionate weakness of the arms compared to the legs post trauma. This case report describes a 65-year-old woman presenting with inability to move both arms after she tripped. She displayed signs consistent with an initial diagnosis of central cord syndrome. Appropriate investigations led to the correct and timely diagnosis of bilateral anterior shoulder dislocations. Bilateral shoulder dislocations are uncommon. Central cord syndrome can mimic similar clinical presentations, and vice versa. It is important for emergency physicians to recognise these pitfalls in diagnosis, and to increase awareness of this clinical entity to avoid future late or missed diagnoses.

Bilateral Anterior Shoulder Dislocation after an Episode of Grand-Mal Seizure: A Case Report and Literature Review

Background: Among all joint dislocations, dislocations of the shoulder are the most common. Although posterior shoulder dislocation is infrequent, it is more common following seizures. Trauma, electrocution, and seizures can cause bilateral shoulder dislocation. Anterior shoulder dislocations can be accompaniment by greater tuberosity (GT) fractures, Bankart fractures, and other complications such as rotator cuff tears and neuro-vascular injuries.&#x0D; Case Report: This article describes a 19-year-old boy who suffered from bilateral anterior shoulder dislocation due to an episode of generalized seizure, which happened after taking tramadol.&#x0D; Conclusion: Any sign of shoulder pain in a patient who has experienced a generalized tonic-clonic seizure should warrant full radiographic evaluation to prevent the complications.

Bilateral Simultaneous Asymmetrical Anterior Shoulder Dislocation With a Fracture

In this case report, we present the case of a 19-year-old male who presented to the emergency department with bilateral shoulder pain and significantly decreased range of motion. His X-rays showed a bilateral dislocation of the glenohumeral joints, along with a proximal humeral fracture. Shoulder dislocations were manipulated, while the proximal humeral fracture was conservatively managed. Bilateral simultaneous asymmetric anterior shoulder dislocations are uncommon in such a young age group, especially when associated with a sports injury.

Bilateral Anterior Shoulder Dislocation Occurring Separately from the Same Injury: A Case Report

Bilateral anterior shoulder dislocation is a relatively rare condition. In a patient presenting with bilateral shoulder pain, a posteroanterior radiograph of the shoulders easily reveals the diagnosis. A 76-year-old woman presented to our emergency room with complaints of bilateral shoulder pain. She had fallen forward with both elbows hitting the ground and also hit her right shoulder. A posteroanterior radiograph revealed a right anterior shoulder dislocation with no remarkable changes noted on the left shoulder. The right shoulder dislocation was resolved, and she was sent home. Six hours later, she was brought back to our emergency room with left shoulder pain. A repeat posteroanterior radiograph of the left shoulder revealed left shoulder dislocation. Bilateral anterior shoulder dislocation occurring after 6 hours is an extremely rare condition. We report this case with a review of the literature.

Bilateral Anterior Shoulder Fracture-Dislocation: About a Case

Bilateral anterior dislocations of the shoulder are rare lesions. They are even rarer when combined with a bilateral fracture of major tubercles. We report the case of a 28-year-old patient admitted to trauma emergencies after trauma by falling from the top of a stool. Clinical and radiographic findings revealed bilateral anterior shoulder dislocation associated with bilateral fracture of major tubercles. The patient was treated orthopedically by a reduction of dislocations under General Anesthesia, immobilization of the shoulders for 6 weeks followed by reeducation. The functional result after 12 months was satisfactory with a UCLA score of 30 points.

Spaso technique for reduction of bilateral Anterior Dislocations of Shoulder

Spaso technique has been in use for anterior shoulder reductions less then a decade. Kocher`s manoeuvre, Hippocratic &amp; Scapular methods for reduction of anterior dislocation of shoulder are well known conventional methods in practice. All have a high success rate. They dislocations are rare. We report a patient with bilateral anterior shoulder dislocations following an assault. Spaso technique was used to reduce dislocated shoulders.

Bilateral Shoulder Combined with Unilateral Knee Dislocation After a Fall

A 54-year-old overweight woman presented to the emergency department (ED) complaining of severe pain and lack of movement in both shoulders and her left knee. Her medical history revealed that the episode began after slipping on the wet floor in the bathroom. The first dislocation of the knee joint occurred during the fall when she lost her balance, and the second occurred when she tried to use her arms to avoid hitting her head on the wet floor. There was no history of connective tissue disease, seizure, or prior dislocations. During clinical examination, there was no evidence of neurovascular deficit for upper extremities and left knee. Radiographs confirmed bilateral anterior glenohumeral dislocation combined with anterior left knee dislocation (Figure 1). Bilateral anterior shoulder dislocations are reported less often. A recent review of 70 patients demonstrated that the causes of anterior injuries were trauma (50%), muscle contractions (37%) due to seizures or electrocution, and atraumatic events (13%). Because of various etiologies, dislocation mechanisms are different. Lever effect is a type of dislocation mechanism that forces abduction and external rotation of the arm. With the greater tuberosity pressing against the acromion, the humeral head is forced anteriorly out of the glenohumeral joint (1, 2). The patient, in this case, had low-energy trauma as well as left knee dislocation. Complete dislocation of the knee causes devastating extremity injuries, particularly in the popliteal artery and peroneal nerve. The incidence of nerve and artery injury due to knee dislocation is 10%–40%. Even worse, when a vascular injury secondary to a traumatic knee dislocation cannot be repaired within 8 h, the amputation rate increases to nearly 85%. Therefore, vascular assessment should include examination of both the dorsalis pedis and posterior tibial pulses, ankle-brachial index measurement, and computerized tomography (CT) angiography or arteriography. The patient had no clinical signs of vascular and nerve injury at the time of admission and observation, so she did not urgently need further radiological study such as CT angiography (3, 4).

Concurrent cases of bilateral anterior shoulder dislocation: our observations in three cases

Bilateral anterior shoulder dislocations are rarely seen and usually occur due to the same mechanism arising from traumatic injuries. In here, we discussed three cases of traumatic bilateral anterior shoulder dislocation , one had an additional patella fracture, and the other one had bilateral tuberculum majus fracture. All of the patients were female and our first case that presented here was 65-year-old and given a closed reduction for isolated bilateral anterior shoulder dislocation that occurred as a result of falling due to an epileptic seizure. The second patient was 70-year-old and had bilateral shoulder dislocation and patella fracture (right leg) after of falling down the stairs due to syncope. Closed reduction was applied to the bilateral shoulder dislocation. Patella fracture on the right leg was fixated using open reduction internal fixation method. The third patient, who was 57-year-old, fell down from the stairs and had bilateral anterior shoulder dislocation accompanied by bilateral tuberculum majus fracture, and she was given closed reduction. For all three of the cases, Hippocrates method was used as the closed reduction method.

Simultaneous bilateral anterior shoulder dislocation: report of two cases and review of the literature

AbstractBilateral shoulder dislocations arerare and almost always occur in the posterior direction. Simultaneous bilateral anterior shoulder dislocation is even rarer and only a few cases are stated in the literature. The most interesting part of a bilateral shoulder dislocation is about its injury mechanism as a synchronous and simultaneous force is needed to result in it. In cases of epilepsy or electrocution, the mechanism is different and the forceful contractions of the selective group of muscles result in the dislocation. This article reports two cases of bilateral simultaneous anterior shoulder joint dislocation that occurred after a road side accident and after an episode of convulsion in an epileptic patient. The dislocations were diagnosed early and reduced immediately with a proper postreduction rehabilitation. During their follow-up, bothpatients had satisfactory functional outcome. This article emphasizes on the importance of shoulder examination in road side accident victims and epileptic patients. All orthopedic surgeonsand emergency physicians should be aware of such unusual possibilities to have an early diagnosis and treatment. An early reduction and appropriate rehabilitation can provide satisfactory functional outcome. This article also briefly discusses the injury mechanisms, diagnoses and treatments of bilateral shoulder dislocation as reported in the literature.

Bilateral Anterior Shoulder Dislocations due to Seizure

During assessment, he had a witnessed generalized tonic-clonic seizure that terminated after intravenous lorazepam administration. He was admitted to the Intermediate Care Unit for ongoing close monitoring due to persistently altered mental status. When more alert the following day he complained of bilateral shoulder pain with markedly restricted range of movement. An initial screening chest radiograph demonstrated bilateral anterior shoulder dislocations with an associated fracture of the left greater tuberosity (Figure 1). On further discussion a history of multiple bilateral shoulder dislocations with minimal force was elicited. He was reviewed by the orthopedic service that elected to perform closed reduction under general anesthesia and he was placed in bilateral shoulder immobilizers for 2 weeks. Operative repair was recommended.

Introduction: Shoulder dislocation is the most common presentation of joint dislocation in emergency department (ED). Bilateral shoulder dislocation seldom happens and often is posterior type. It usually occurs due to seizure and electric shock. Trauma is among the common causes of bilateral anterior shoulder dislocation. Here, we present a case of bilateral anterior shoulder dislocation due to seizure attack. Case Presentation: A 21-year-old man presented to ED with complaint of bilateral shoulder pain and restricted range of motion. The patient was under therapy for epilepsy, but discontinued his therapy in recent weeks. He had a seizure attack while sleeping and thereafter he was unable to move his arms. Neurovascular examination of both upper extremities was normal. Shoulder X-ray images revealed bilateral anterior dislocation. Reduction was performed in ED with sedation by adduction and external rotation method. Neurovascular systems were rechecked and reduction was confirmed by radiologic images. Both shoulders were immobilized by sling and swathe, and patient discharged from ED with follow-up recommendation. Conclusions: Bilateral anterior shoulder dislocation is rare. It could be undiagnosed, especially when there is no clear history of major trauma. By awareness of such unusual possibility, early diagnosis, treatment, and reduction can provide desirable outcome

Simple bilateral anterior shoulder dislocation: A case report and review of the literature

Bilateral shoulder dislocations are usually of the posterior type, which is commonly associated with seizures, electrocution or electroconvulsive therapy. Bilateral anterior shoulder dislocations are very rare and usually result from significant trauma. Simple bilateral anterior shoulder dislocations without associated fracture are much rarer and not many cases have been reported in the literature. We present a traumatic bilateral simple anterior shoulder dislocation in a 24-year-old male without any features of ligamentous laxity, and review the international literature on simple bilateral anterior shoulder dislocations. From the review we conclude that seizures are as important an aetiology of simple bilateral anterior shoulder dislocation as is trauma. Moreover, this injury is not as rare as was considered in the past.

Bilateral Traumatic Anterior Shoulder Dislocation

Aim: The aim of this case report is to discuss the evaluation of potential complications and treatment options associated with bilateral anterior shoulder dislocation. Material and Method: The data for this rarely encountered case was collected, during the consultation and treatment phases for a 66-year-old female patient who first presented at the emergency department of Bezmiaiem Vakif University Medical Center with restriction of movements in her both shoulders after falling down the stairs in her home. Results: Evaluation of the patient’s pre-operative X-rays and computed tomography (CT scan) images and post operative evaluation of the shoulders was achieved using DASH and the Constant shoulder score. Discussion: Osteoporotic elderly patients and their relatives should be informed of possible complications when considering closed reduction of severe glenoid and humeral head fractures. Instead, an associated risk will most likely necessitate open reduction in this patient population after anesthesia has been administered for closed reduction. Hence, obtaining consent and educating the patient about a possibile need for internal fixation and prosthetic joint replacement using surgical implants should be included in the preoperative consultation and treatment planning session before the patient receives sedation.

A case of neglected bilateral anterior shoulder dislocation: a rare entity with unusual mechanism of injury.

Bilateral shoulder dislocations are rare, and if they occurred, posterior type of dislocations is common. Bilateral anterior shoulder dislocations are very rare and occur due to trauma with unique mechanism of injury. We report a case of unreduced simultaneous bilateral anterior dislocations of shoulder without associated fractures in a forty-year-old man following a unique mechanism of injury; both hands of the patient were pulled from either side. To the best of our knowledge, this unusual mechanism of injury has not been reported in the literature.
