Growth pattern change of a benign clear cell ‘sugar’ tumor of the lung: Serial imaging surveillance over seven years

Clear cell tumor of the lung is a rare benign mesenchymal tumor and only sporadic cases have been previously reported. The present study reports the case of a 58-year-old man who presented with an incidentally detected solitary pulmonary nodule. Based on comparisons with serial previous chest and spine radiographs, a growth pattern change in the nodule (initial gradual growth over 4.5 years, then recent no growth over 2.5 years) was identified. The nodule was diagnosed as a benign clear cell tumor of the lung (CCTL) following surgical resection. Few studies have previously reported interval growth of CCTLs and the growth pattern of CCTL is not yet well known. To the best of our knowledge, this is the first reported case in which the growth pattern change of CCTL was identified via serial imaging over 7 years of follow-up.

Mimicry of sugar tumor and minute pulmonary meningothelial-like nodule to metastatic lung deposits in a patient with rectal adenocarcinoma

Several reports have described different lung lesions mimicking primary or metastatic neoplasms. In this paper, we describe the different features of two uncommon and benign lung lesions mimicking metastasis from a primary large bowel adenocarcinoma. Our patient is a 75-year old female with a history of invasive rectal adenocarcinoma. One month after her surgery, she started complaining of coughing and shortness of breath. Clear cell sugar tumor and minute meningothelial-like nodules had been found incidentally and simultaneously during her chest x-ray. The diagnosis had been made based on morphology and was supported by a positive staining to a panel of immunohistochemical stains including CD34, vimentin, HMB45, melan A and S100. An ultra-structural examination was also performed and confirmed the presence of melanosomes in sugar tumor. The coexistence of lung sugar tumor and minute pulmonary meningothelial-like nodules has never been reported in the literature and an awareness of these lesions is essential to correctly diagnose and stage patients.

A case of benign clear cell tumor of the lung with a past history of renal cell carcinoma

症例は乳癌・胃癌・腎癌の既往のある75歳女性．胸部CTで右肺下葉に異常陰影を認めるとのことで当科に紹介となった．胸部CTで右S8に8mm大の小結節影を認めた．retrospectiveにみると3年前の胸部CTで同部に3mm大の結節影を，2年前の胸部CTで4mm大の結節影を認めていた．増大傾向を認めており転移性肺腫瘍を疑われたため手術を行った．術中迅速組織診で腎癌肺転移と診断された．術後永久標本において豊富な淡明細胞質を有する細胞が薄壁性の類洞様血管を伴ってシート状に配列しており，免疫染色ではPAS・MelanA陽性，CD10・HMB45陰性であり淡明細胞腫と診断された．肺原発淡明細胞腫は比較的稀な腫瘍である．腎細胞癌肺転移との鑑別を要するが実際に腎細胞癌の既往を有する症例の報告は我々が検索した限り認められなかった．本症例の経過を若干の文献的考察を加えて報告する．

膀胱癌に併存した肺原発淡明細胞腫の1例

まれな肺原発の腫瘍である淡明細胞腫の1例を経験した．症例は69歳の男性．血尿を認め当院泌尿器科に紹介となった．経尿道的膀胱腫瘍切除（TUR-Bt）を行い膀胱癌と診断された．全身精査のために行った胸部CTで右肺下葉に充実性の結節を認め，気管支鏡下に擦過細胞診を行ったが確定診断はつかずcT2N0M1の診断で化学療法が施行された．化学療法終了後のTUR-Btで膀胱癌の残存を認めなかった．胸部CTの再検で肺結節の腫瘍径は不変であり，診断治療目的で当科紹介となった．胸腔鏡下肺部分切除術を施行し病理組織診断にて肺原発の淡明細胞腫と診断された．

Clear Cell "Sugar" Tumor of the Lung: A Well-Enhanced Mass with an Early Washout Pattern on Dynamic Contrast-Enhanced Computed Tomography

Clear cell tumor of the lung is a rare and very unusual benign pulmonary tumor. As clear cell tumor of the lung contains abundant cytoplasmic glycogen, this tumor is called "sugar tumor". We report a case of sugar tumor in a 64-yr-old man presenting as a round pulmonary nodule. On dynamic computed tomography (CT) scans, the solitary pulmonary nodule showed early wash-in enhancement with an early washout pattern like a lung malignancy. The patient underwent wedge resection for the tumor. Pathologic examination, including immunohistochemical studies, revealed that the nodule was a benign clear cell tumor, so-called "sugar tumor". Because only a small number of cases have been reported previously, clinical aspects, radiological characteristics on dynamic contrast-enhanced CT, and differential diagnosis of the tumor are not well established. Herein we present a clear cell tumor of the lung and discuss the clinical, radiological, and pathological features of the tumor.

Reported herein is a case of benign clear cell tumor of the lung in a 60-year-old man. Chest X-ray and CT examination revealed an abnormal nodule with homogenous density and a clear margin in the lower lobe of the left lung. The resected tumor was 13 mm in size, well-circumscribed and was graysh-white on cut surface. Histological examination showed a diffuse growth pattern of polygonal tumor cells with indented and pleomorphic nuclei, and clear abundant cytoplasm with a distinct cell border surrounded by thin-walled vascular spaces and sinusoid-type vessels. The clear cytoplasm contained numerous glycogen granules as demonstrated by PAS staining. In the present case there was focal immunoreactivity for S-100 protein, HMB-45, neuron-specific enolase, cathepsin B and melan A, which are consistent with reported immunohistochemical staining patterns of benign clear cell tumor. Based on these findings, the tumor was diagnosed as a benign clear cell tumor of the lung. Although most clear cell tumors are considered to belong to the family of neoplasms with perivascular epithelioid cell differentiation (PEComas), histogenesis of benign clear cell tumor of the lung has remained unclear. This first report of CD1a expression in this tumor might provide a new insight into its histogenesis and diagnosis.

Benign Clear Cell Tumor of the Lung

A 49-year-old woman presented with a solitary pulmonary nodule in the right lung. The tumor was well circumscribed and showed a reddish brown cut surface. It showed a diffuse growth pattern of polygonal cells with clear abundant cytoplasmand distinct cell border around thin-walled vascular spaces and sinusoid-type vessels. Based on morphological findings, the patient was diagnosed as having benign clear cell tumor of the lung. Silver impregnation and PAM stains showed fine reticular fibers continuously surrounding the vessels and individual neoplastic clear cells. Strong immunostaining for type IV collagen was observed surrounding all the individual clear cells. A few clear cells were positive for HMB-45 and NCAM 123C3 ( CD56 ). Electron microscopically, clear cells had numerous membrane-bound glycogen granules and a large amount of non—membrane-bound glycogen. The neoplastic cells were surrounded by external lamina and cytoplasmic processes. The neoplastic clear cells showed morphologic features seen in pericytes, melanocytic cells, and neuroendocrine cells.

Unusual lung tumors of uncertain origin have been described briefly concerning benign clear cell (sugar) tumor and, in more detail, concerning pneumocytoma (so-called sclerosing hemangioma). Melanocytic differentiation of clear cell tumor, which is peculiar to the lung, has been suggested recently. Sclerosing hemangioma, also unique to the lung, is now considered to be a tumor (neoplasm) related to alveolar epithelial cells. It shows invasive growth and multiplicity (or aerogenous metastasis), but is very rarely capable of lymph node metastasis. It should be designated as pneumocytoma.

Clear cell carcinoma of the lung: a case report and review of the literature

Clear cell carcinoma of the lung is an extremely rare type of lung cancer with doubtful etiology. This present case showed that the tumor cells contained abundant cytoplasmic glycogen with a diffusely distributed pattern and intracellular glandular structure electron microscopically. Immunohistochemical study revealed that the tumor cells expressed not only epithelial marker including cytokeratin and epithelial membrane antigen, but also Le y, one of stage-specific embryonic carbohydrate antigens. The diagnosis of pulmonary clear cell carcinoma should be done carefully, because benign clear cell tumor and secondary renal cell carcinoma show similarities in a histologic appearance but not in their management and prognosis.

“Clear cell” oncocytoma of salivary gland

For the most part, clear cell neoplasms of the salivary glands are adenocarcinomas of at least low-grade malignant potential. However, a rare benign clear cell tumor of major salivary glands can be distinguished as a histologic variant of oncocytoma and oncocytosis. Ten such cases have been identified in the files of the Armed Forces Institute of Pathology (Washington, DC). Eight patients were women, and nine of the lesions involved the parotid gland. All of the patients were middle-aged or older adults. The light-microscopic morphology and the phosphotungstic acid-hematoxylin (PTAH), PAS, and mucicarmine staining patterns were consistent with oncocytoma and oncocytosis. Transitions from typical eosinophilic oncocytes to clear cells were evident. Electron microscopy and histochemistry demonstrated that the clear cytoplasm seen by light microscopy was primarily due to artifact and intracytoplasmic glycogen. Mitochondria were the preponderant cytoplasmic organelles. Two patients were known to have experienced recurrent lesions.

Cytologic studies of lung metastasis of renal carcinoma

Renal carcinomas often metastasize to the endobronchial wall with no symptoms, so they are liable to be mistaken for primary lung tumors.Three cases of lung metastasis of renal carcinomas gave the following characteristic cytological indings: abundant cytoplasm with numerous vacuoles, eccentric but round nuclei and prominent nucleoli.Differentiation of lung metastasis of renal carcinoma from primary lung tumors especially “clear cell” carcinoma or benign clear cell tumor of the lung is discussed.

Benign clear cell tumor ("sugar tumor") of the lung.

The light microscopic and fine structural features of a benign clear cell tumor of the lung are reported. The tumor contained an abundance of glycogen which was present both free and membrane-bound. The resemblance of this glycogen distribution to that seen in type II glycogenosis (Pompe's disease) is noted. In some tumor cells, dense-core (neurosecretory) type granules and modification of the endoplasmic reticulum were noted similar to those reported in pulmonary carcinoid tumors. The fine structural features of this tumor may be helpful in its differential diagnosis. The pulmonary clear cell tumor appears to belong to that family of pulmonary neoplasms derived from the Kultschitzky cell.
