Parent Explanatory Model Personalization as a Method of Reducing Risk for Poor Engagement and Outcomes in PCIT among Culturally Diverse Families.

Background/Objectives: Evidence supports the efficacy of Behavioral Parent Training (BPT) interventions such as Parent-Child Interaction Therapy (PCIT) for treating child behavior problems; however, treatment engagement and outcomes vary across ethnic groups. Risk for poor treatment engagement and outcomes may be attributed in part to misalignment between parent explanatory model components (PEMs) and the traditional BPT model, including treatment expectations, etiological explanations, parenting styles, and family support for treatment. The present study aims to examine whether personalized treatment adaptations addressing these PEM-BPT misalignments reduce risk for poor treatment engagement and outcomes. Methods: The authors previously utilized the PersIn framework to develop a personalized version of PCIT (MY PCIT) that assesses these PEMs in order to identify families at risk for poor treatment engagement and outcomes. Families were identified as high risk (due to PEM-BPT misalignment) and low risk (meaning those without identified PEM-BPT misalignment) for specific PEMs. Families at elevated risk then received tailored treatment materials designed to improve alignment between the parental explanatory model and the PCIT treatment explanatory model. A recent pilot trial of MY PCIT demonstrated positive treatment outcomes; however, the extent to which adaptations were successful in reducing the underlying risk factors has not yet been examined. Results: Findings demonstrate that the personalization approach was effective in reducing indicators of risk, and that families who were initially at high and low risk during pre-treatment reported similar levels of treatment engagement and outcomes by post-treatment. Conclusions: The findings suggest that this personalized approach has the potential to reduce risk associated with poor treatment engagement and outcomes for culturally diverse families.

The added value of virtual reality to parent–child interaction therapy: a qualitative study with parents and therapists

BackgroundVirtual reality (VR) is one of the most rapidly growing technological advancements being studied, developed, and implemented in mental health care. It offers a wide array of possibilities that go beyond the traditional ways of providing mental health care services. Many interventions require clients to learn new skills in order for change to occur. Relevant to this study, behavioral parent-training (BPT) interventions are focused on the acquisition of parenting skills in order to change child behavior. VR was added to the Parent–Child Interaction Therapy (PCIT) – a known BPT intervention — as an additional practice tool. In the present study, which is part of a larger study, the qualitative experiences of (PCIT-)VR from both parents’ and therapists’ perspectives are evaluated, as well as their suggestions for implementation.MethodSemi-structured interviews were conducted with eleven parents. Five therapists participated in a focus group and three additional semi-structured interviews were conducted with therapists. To analyze, a combination of deductive and inductive thematic analysis was used.ResultsResponses from interviews were divided into three categories: (1) parents’ evaluation, (2) therapists’ evaluation, and (3) suggestions for implementation from both parents and therapists. Next to a positive overall experience with PCIT, a large number of parents were also positive about the addition of VR to treatment. Both parents and therapists experienced VR as a useful additional practice tool. The additional practice in VR led to a boost in confidence in using positive parenting skills. There were mixed experiences regarding the ease and immersion of the tool. Suggestions for implementation from both parents and therapists were to make the tool more appealing and realistic, to vary more in VR modules and to improve certain practical aspects.ConclusionAdding VR to PCIT is still a pioneering piece of technology within PCIT. The value of this VR tool lies in the provision of the possibility to practice with skills outside of the therapeutic encounter, which has the potential to instill confidence in using those skills. Given the positive experiences regarding PCIT-VR, it is important to further develop the VR tool. We believe that ultimately, PCIT-VR can potentially serve as a customized way for parents to practice what they are taught during the treatment.

Implementing a Child Mental Health Intervention in Child Welfare Services: Stakeholder Perspectives on Feasibility.

This pilot study integrated quantitative and qualitative data to examine the feasibility of implementing a modified version of a multiple family group behavioral parent training intervention (The 4Rs and 2Ss for Strengthening Families Program [4Rs and 2Ss]) in child welfare (CW) placement prevention services, from the perspectives of participating caregivers (n = 12) and CW staff (n = 12; i.e., 6 caseworkers, 4 supervisors, and 2 administrators). Quantitative surveys were administered to caregivers and CW staff followed by semi-structured interviews to examine the feasibility of implementing the modified 4Rs and 2Ss program, as well as factors impacting feasibility. Results indicated that quantitative benchmarks for high feasibility were met in all assessed areas (e.g., family recruitment, caseworker fidelity ratings, CW staff feasibility ratings) except for family attendance, which was markedly lower than desired. Factors facilitating feasibility included agency and research support, intervention ease-of-use, perceived benefits to existing CW practice, as well as logistical support (e.g., food, transportation, childcare) promoting attendance. Factors hindering feasibility included conflicts between research-based eligibility criteria and existing client population demographics, research-related processes resulting in delays, CW staff role conflicts, added workload burden, complex family issues, and power differentials inherent to CW services which complicated families' voluntary participation.

Study Protocol: Type 1 Hybrid Effectiveness-Implementation Trial of a Behavioral Parent Training Intervention for Parents of Young Children Who Are Deaf or Hard of Hearing.

Children who are deaf or hard of hearing (DHH) and who use hearing aids or cochlear implants are more likely than their peers with typical hearing to exhibit behavior problems. Although multiple evidence-based interventions for child behavior problems exist, they are rarely delivered to children who are DHH, and no rigorous randomized controlled trials have been conducted to determine their effects with this population. This protocol describes a study aiming to test the effectiveness of an evidence-based behavioral parent training intervention adapted for parents of young children who are DHH and simultaneously to assess key implementation outcomes and multilevel contextual factors influencing implementation. The protocol for a Type 1 hybrid effectiveness-implementation trial of a behavioral parent training intervention for parents of young children who are DHH is presented, including details of the study design, participants, assessments, and analyses. Using a stakeholder-engaged, mixed-methods approach, we will test the effects of the intervention versus treatment as usual on parenting behaviors, child behaviors, and a range of secondary effectiveness outcomes, including adherence to using hearing aids and cochlear implants as well as measures of child speech and language. We will assess the acceptability, feasibility, fidelity, and costs of the intervention from the perspectives of peer coaches who deliver the intervention, hearing health care clinicians (including audiologists and speech-language pathologists), and administrators of programs serving young children who are DHH. Results of this trial will inform future efforts to close the gap between prevalence of behavioral problems in young children who are DHH and access to and use of evidence-based interventions to prevent and treat them. If effective, this intervention could be widely implemented using strategies informed by the findings of this study to benefit young children who are DHH and followed in hearing health care and their families.

33.1 Targeted Behavioral Parent Training Interventions for Clinical Encounters

Evidence-based behavior management training for caregivers is associated with sustained behavioral improvements among children with oppositional and disruptive behaviors, as well as reduced use of psychotropic medications. Unfortunately, these behavioral training programs are not easily accessible. Mental health medication management visits with child and adolescent psychiatrists offer a unique opportunity to provide caregivers with brief, targeted behavioral intervention strategies to families and their children who are struggling with challenging behaviors, thereby increasing the impact of these evidence-based interventions. The authors were unable to identify any standard skills trainings that focus on these interventions within the time constraints of medication management appointments or clinical encounters within the primary care setting. Our objectives were to address this gap with one 4-hour virtual interactive workshop designed for clinicians, with the goal to improve knowledge, confidence, and application of basic behavioral parent/caregiver training skills and strategies. We created an interactive, virtual workshop to teach strategies based on the principles of parent management training (PMT), which included identifying antecedents of challenging behaviors, applying consequence-based behavioral reinforcers, and developing an individual caregiver plan to maintain prosocial behaviors. Information regarding clinician demographics, discipline, prior knowledge, and training was collected. Acquisition of knowledge and skills was measured by administration of a Likert scale before and after the workshop. A total of 138 participants, including trainees and faculty from a variety of disciplines in community and academic settings, within primary care and behavioral health across the United States, attended the workshop. Our results demonstrate a statistically significant increase in each dimension measured (p < 0.05; n = 90), including both confidence of knowledge and skill acquisition of behavioral parent training. These findings suggest that this pilot workshop can serve as a model for future trainings focused on brief, targeted behavioral parent training interventions during clinical encounters in the outpatient setting.

ADHD Symptoms do not Moderate Outcomes to Behavioral Parent Training Delivered in the Voluntary Sector

The purpose of the study was to determine the moderating role of baseline levels of attention-deficit/hyperactivity disorder symptoms on parent- and child-level outcomes following the Caring in Chaos behavioral parent training intervention at immediate post-intervention and follow-up assessment. One-hundred sixty-one children between the ages of 3–9 (Mean age = 7.6; 73% male) with parental concerns of attention-deficit/hyperactivity disorder were randomly assigned to the Caring in Chaos behavioral parent training intervention, a task-shifted intervention delivered by a volunteer workforce across 12 community-based settings in Denmark, or to a wait-list control condition. Parent report of parenting behavior, sense of competence, stress, depressive symptoms as well as child attention-deficit/hyperactivity disorder symptoms and child noncompliance were collected at baseline, immediate post-intervention and at follow-up assessment points. Analyses indicated no moderating effect of baseline attention-deficit/hyperactivity disorder symptoms on parent- and child-outcomes, with no to large effects at post-treatment with maintenance of effects at follow-up assessment. The results of these analyses suggest that the Caring in Chaos behavioral parent training intervention can be utilized with young children with varying levels of attention-deficit/hyperactivity disorder symptoms. These findings further support that community-based interventions delivered by nonprofessionals may serve as a beneficial option to increase availability and access to behavioral parent training for the treatment of attention-deficit/hyperactivity disorder.

Patterns of Parental Adherence and the Association to Child and Parenting Outcomes Following a Multicomponent School-Home Intervention for Youth With ADHD

The goal of the present study was to evaluate the role of parent adherence in the Collaborative Life Skills (CLS) program, a multicomponent school-home intervention, for predicting child and parenting outcomes. A sample of 129 children (63% male; M age = 8.22, SD = 1.10; grades 2–5) with attention-deficit/hyperactivity disorder (ADHD) and their parents participated in CLS, which included 10 weekly behavioral parent training group sessions. Each week, parents provided information on their CLS skill use between sessions (at home) as part of the intervention. Outcome measures included parent and teacher ratings of child behavior and parenting at post-intervention and 6 months follow-up. Growth mixture models examining weekly parent skill use trajectories throughout the intervention significantly predicted parent- and teacher-reported outcomes including parent-rated child behavior, teacher-rated academic competence, and positive parenting behaviors. Fifty-two percent of parents displayed moderate skill use throughout the intervention, whereas the remaining parents had either low (20%) or high (28%) initial levels of use but demonstrated high skill utilization by the middle of the intervention. Results highlight the importance of examining individual differences in parents between session strategy use for behavioral parent training interventions targeting child and parenting outcomes.

Do Parents' ADHD Symptoms Affect Treatment for their Children? The Impact of Parental ADHD on Adherence to Behavioral Parent Training for Childhood ADHD.

Nearly half of all youth with Attention-Deficit Hyperactivity Disorder (ADHD) have at least one parent who also meets criteria for the disorder, and intergenerational ADHD is a significant risk factor for poor outcomes following evidence-based behavioral parent training (BPT) programs. Given that BPT is predicated on consistent parental involvement, symptoms of ADHD in parents may be a significant barrier to effective engagement with BPT treatment. In the present investigation, we examine the effect of parental ADHD symptoms on BPT treatment engagement for children with ADHD-predominantly inattentive presentation (N = 148, ages 7-11). We examine the following parent- and clinician-rated treatment engagement domains: between-session skill adherence, in-session participation, perceived skill understanding, treatment-engagement attitudes, and session attendance. Parent- and clinician-rated between-session adherence was the only treatment engagement domain related significantly to parental ADHD symptoms. This finding was robust and remained even after accounting for symptoms of parental anxiety and depression, child ADHD symptom severity, and various sociodemographic factors (parental education level, household income, employment status, and being a single parent). These findings suggest that targeting parental ADHD symptoms in the context of parenting interventions may be a promising approach for improving adherence and treatment outcomes for BPT interventions.

The effect of behavioral parent training of children with attention deficit hyperactivity disorder on parents’ mental health

BackgroundParents’ mental health can be impacted by sleep problems experienced by children diagnosed with ADHD. This study aimed to investigate the effect of group-based behavioral parent training (BPT) related to sleep problems of children with ADHD on parents’ mental health. MethodsA parallel randomized controlled trial design was used in this study. Participants included 58 parents of school-age children with ADHD who had at least one sleep problem and who were known to medical services in a psychiatric clinic in Rasht city, Iran. Participants were randomly allocated to the intervention or control groups equally. Participants in the intervention group received a 5-week BPT intervention program related to sleep problems of ADHD children including 3 sessions group-based training and 2 telephone follow-up. Data were collected before and two months after the intervention. Data were analyzed using descriptive and inferential statistics via the SPSS software. ResultsThe findings revealed two months after the intervention, participants in the intervention group had a significant improvement in the anxiety (p = 0.01) and stress (p = 0.02) levels compared to the control group. ConclusionThe results suggest that group-based BPT related to sleep problems of children with ADHD could be an effective strategy in improving parents' mental health.

Personalizing Behavioral Parent Training Interventions to Improve Treatment Engagement and Outcomes for Culturally Diverse Families.

Behavioral Parent Training (BPT) interventions are efficacious for young children with externalizing behavior problems. However, not all families benefit, and ethnic minority families in particular are less likely to enroll, engage, and improve in BPT. Versions of BPT interventions tailored for specific ethnic groups have been successful at improving engagement and outcomes for ethnic minorities; however, the specificity of these models presents challenges for broad dissemination. This article presents a personalization approach (PersIn) that utilizes cultural assessment results to tailor treatment protocols to the characteristics of individual families. We believe this approach has the potential to maximize cultural sensitivity while preserving generalizability to both minority and non-minority ethnic groups. We further propose that personalization on Parent Explanatory Model (PEM) parameters that have been found to vary across ethnic groups and to impact treatment engagement and/or outcomes is a promising approach to decreasing disparities in BPTs. We describe examples of evidence-supported PEMs that present good targets for personalization and provide examples from MY PCIT to illustrate how PersIn can be applied to Parent-Child Interaction Therapy (PCIT).

Risk factors for intergenerational child maltreatment: The role of temperament

Behavioral parent training programs have documented efficacy for improving behaviors among parents and their children and are frequently used by child welfare agencies to prevent removal of a child from the parental home or to facilitate reunification. Although an ideal time for parent training might be during supervised visits where parents may practice skills with their children under the guidance and support of a therapist or caseworker, this is not typically the case. Most often, parents within the child welfare system receive parent training in small groups without their children present, and to date, few studies have examined effects of behavioral parent training interventions during supervised visitation. In this study, concurrent multiple baseline across behaviors design was used to examine effects of a behavioral parent training program, Filming Interactions to Nurture Development (FIND), on parental skill acquisition with four mothers who had lost custody of their children but were being considered for reunification. Children emitted little or no problem behaviors during baseline or intervention, so parenting behavior was the primary dependent variable. Results obtained across participants documented a clear functional relation between implementation of the FIND intervention and increases in developmentally supportive parenting behaviors. Results of social validity and contextual fit measures suggest the intervention was perceived by mothers to be positive, feasible, and appropriate within the child welfare context. Practical and conceptual implications, limitations of this study, and directions for future research are discussed.

Violence Victimization and Parenting Among Black South African Mothers.

Black South African women and girls face high rates of violence victimization, including physical, sexual, and emotional abuse in childhood and intimate partner violence (IPV) in adulthood. U.S.-based research suggests that violence victimization predicts parenting difficulties, but this relation has not been examined in a South African context. Among a sample of 99 Black South African female caregivers, we examined rates of child abuse, IPV, and cumulative trauma, and we explored the associations between caregivers' victimization history and current parenting quality (i.e., parent-child relationship quality & parental involvement with child). Caregivers completed self-report questionnaires using ACASI software. Hierarchical regression analyses revealed that caregivers reporting childhood physical or sexual abuse were less involved with their children. Caregivers with a history of emotional abuse reported significantly worse parent-child relationship quality. Cumulative trauma was associated with worse parental involvement and parent-child relationship quality. Results reveal relatively high rates of interpersonal violence victimization among this convenience sample of Black South African women, as well as unique relations between forms of victimization and specific parenting practices. The findings highlight the importance of behavioral parent training interventions with a focus on specific assessment of parents' victimization experiences.

Benefits of Child Behavior Interventions for Parent Well‐Being

ObjectiveTo explore the direct and indirect associations among GenerationPMTO (i.e., the “Oregon model” of a parent management training program), child behavior problems, and parent outcomes.BackgroundThe behavioral parent training program GenerationPMTO is designed to improve child behavior outcomes. Although parent well‐being is not targeted, previous studies have found collateral benefits on parental well‐being for GenerationPMTO in preventive low‐risk samples and high‐risk samples.MethodBootstrapped structured equation models were used to analyze 2 randomized trials that evaluate parent well‐being (mental health, somatic health, and vitality) in 2 versions of GenerationPMTO: Brief Parent Training (BPT) for a preventive sample (N = 216) and GenerationPMTO, which was higher‐risk and a mix of a clinical and a preventive sample (N = 137). We examined direct and indirect effects of treatment condition for parent well‐being via change in children's behavior problems, change in parenting practices, and level of parenting self‐efficacy.ResultsSix months after treatment, results revealed positive effects on targeted child and parent outcomes in both samples, whereas only the BPT intervention had direct effects on parent well‐being.ConclusionCollateral benefits in the low‐risk BPT sample suggest that helping children and their families at earlier stages of negative development may increase the potential for immediate collateral benefits.ImplicationsThe results highlight the preventive potential of a brief behavioral parent training intervention based on the social interaction learning model.

A Recovery Capital and Stress-Buffering Model for Post-deployed Military Parents.

We tested a recovery capital model for military families employing the After Deployment, Adaptive Parenting Tools (ADAPT) randomized control trial, a longitudinal preventive intervention study of 336 post-deployed military parents. Recovery resources included measures of social capital (parenting support, observed partner support behaviors), personal capital (parenting efficacy, education), and community capital (the ADAPT behavioral parent-training intervention). We hypothesized higher levels of recovery capital would buffer the negative impact of military stress on growth in post-traumatic stress disorder (PTSD) symptoms for deployed and civilian parents. Outcome data were evaluated with three waves across 2-years. Hypotheses were tested with latent growth models in a structural equation modeling framework. Military stress was assessed by reports of exposure to combat and battle aftermath. Recovery capital was measured by reported support for parenting and direct observation of behavioral interactions during problem-solving discussions of deployment-related stressors. Fathers had higher levels of military-related stress and PTSD symptoms over time compared to mothers. Growth curve models showed that fathers were characterized by individual differences in 2-year average levels of PTSD symptoms while mothers were characterized by individual differences in initial status and linear growth trajectories. Results supported a recovery capital model. Higher levels of parenting efficacy and parenting support were associated with lower PTSD symptoms, representing common pathways for both mothers and fathers. Similarly, parenting support operated as a moderating buffer for both parents. That is, effects of military trauma exposure on psychological distress were lower for mothers and fathers with higher levels of parenting support relative to parents with lower levels. Regions of significance indicated that half a standard deviation above the mean of support was beneficial for mothers, while one and half standard deviations were needed to impact the effects of trauma on fathers’ PTSD. For mothers assigned to the ADAPT parent training intervention – but not fathers – the intervention was associated with linear reductions in PTSD symptoms over 2 years. The recovery capital model explained 36% of the variance in father outcomes and 46% for mothers. The intervention obtained a medium effect size in reducing mothers’ symptoms (d = 0.41). Implications for prevention and treatment within a recovery capital model are discussed.

A randomized clinical trial of interventions for improving well-being in custodial grandfamilies.

Despite the rising cultural phenomenon of grandparents parenting grandchildren on a full-time basis due to problems within the birth parent generation, intervention studies with these families have been scarce, methodologically flawed, and without conceptual underpinnings. We conducted a randomized clinical trial (RCT) with 343 custodial grandmothers recruited from across 4 states to compare the effectiveness of behavioral parent training (BPT), cognitive-behavioral therapy (CBT), and information-only control (IOC) conditions at lowering grandmothers' psychological distress, improving their parenting practices, and reducing the internalizing and externalizing difficulties of target grandchildren between ages 4 and 12. These outcomes were derived conceptually from the family stress model and modeled as latent constructs with multiple indicators. Each RCT condition was fully manualized and delivered across 10 sessions within groups led jointly by trained professionals and peer facilitators in community settings. Multidomain second-order latent difference score models were performed on a full intent-to-treat basis to compare the 3 RCT conditions on changes in the above outcomes from baseline to postintervention and from baseline to 6 months postintervention. In general, while CBT and BPT interventions were both superior to IOC at both times of measurement on most outcomes, they differed little from each other. Effect sizes were generally in the moderate to large range and similar to those found in prior studies of BPT and CBT with traditional birth parents. Service needs of adolescent parents in child welfare: Is an evidence-based, structured, in-home behavioral parent training protocol effective?

SafeCare is an evidence-based behavioral parent training intervention that has been successfully implemented in multiple state child welfare systems. A statewide implementation in Oklahoma established the effectiveness of SafeCare with a diverse group of parents, which included adolescent parents under 21 years of age, a particularly at-risk group. The current study examined whether SafeCare is also effective for this subsample of 294 adolescent parents with regard to child welfare recidivism, depression and child abuse potential, and attainment of service goals. Post-treatment adolescent parent ratings of program engagement and satisfaction were also examined. Among the subsample of adolescent parents, the SafeCare intervention did not result in significantly improved outcomes in terms of preventing recidivism or reduction in risk factors associated with child abuse and neglect as compared to child welfare services as usual. Further, no significant differences in program engagement and satisfaction between SafeCare and services as usual were detected. These findings shed light on the potential differences in program effectiveness between adolescent and adult parents, and the need for future research to rigorously evaluate the effectiveness of behavioral parenting programs with adolescent parents.

Young Children With Behavior Disorders in Low-Income Families: The Role of Clinic Observations in the Assessment of Parenting

ABSTRACTChildren from low-income families are at elevated risk of meeting diagnostic criteria for behavior disorders. Although mastery-based behavioral parent training (BPT) programs have a robust evidence base for reducing disruptive behaviors of young children, socioeconomically disadvantaged families are more likely to drop out of treatment and, in turn, are less likely to benefit. Therefore, efficient assessment and treatment may be needed. Less research, however, has addressed the value of time-consuming behavioral observations relative to caregiver-report of problem behaviors in the assessment of this at-risk and underserved group. In the current study, 43 low-income parents and their clinic-referred child were observed during a standard mastery-based BPT interaction task (Child’s Game) at baseline. Findings revealed that parents were observed to display an extremely high proportion of maladaptive behaviors targeted for reduction by subsequent treatment (Instructions plus Questions) relative to behaviors that the treatment was designed to increase (Attends plus Rewards). In contrast, parents reported they utilized relatively high and low rates of positive and negative parenting, respectively. Behaviorally observed parenting was not correlated with caregiver-reported parenting. The findings suggest that baseline observations of behaviors targeted in mastery-based BPT interventions are an important part of assessment in parenting programs for low-income families with young disruptive children.

Bending the Curve: A Community-Based Behavioral Parent Training Model to Address ADHD-Related Concerns in the Voluntary Sector in Denmark.

Limited access to evidence-based behavioral parent training (BPT) for addressing attention deficit/hyperactivity disorder (ADHD) has been a growing concern internationally. Models to improve access to BPT are needed, particularly those that can be readily implemented in community settings and that leverage the potential workforce to increase capacity to deliver BPT. The purpose of this study was to evaluate a BPT model which included oft-used content, methods, processes of BPT (common-elements), non-professionally delivered (task-shifted/shared) BPT intervention, and an efficient ancillary support system (training, fidelity, and supervision methods) for families of youth with parental concerns about ADHD. In a randomized controlled trial of 161 families of children (79% male; mean age 7.04 [1.55]), the Caring in Chaos (CiC) BPT model, delivered by community volunteers across 12 community-based sites in Denmark, was compared to a wait-list control condition on key child and parent outcomes at immediate post-treatment and 4-month follow-up assessment points. Results suggested that the CiC model led to significantly greater improvement in parenting behavior, parenting sense of competence, child functional impairment, parental stress and parental depressive symptoms compared to the wait list condition at immediate post-treatment, with maintenance of gains in most of these areas at follow-up assessment. No effect of intervention was found on ADHD symptoms. The results of this study suggest that developing efficient BPT intervention models, such as the CiC model, can result in readily implemented interventions by a variety of individuals in community settings. Such models are necessary to bend the curve on addressing unmet needs of families of youth with concerns about ADHD.

Behavioral Parent Training in Infancy: What About the Parent–Infant Relationship?

Behavioral parent training (BPT) and attachment interventions have demonstrated efficacy in improving outcomes for young children. Despite theoretical overlap in these approaches, the literature has evolved separately, particularly with respect to outcome measurement in BPT. We examined the impact of the Infant Behavior Program (IBP), a brief home-based adaptation of Parent–Child Interaction Therapy, on changes in attachment-based caregiving behaviors (sensitivity, warmth, and intrusiveness) at postintervention and 3- and 6-month follow-ups during a videotaped infant-led play. Sixty mother–infant dyads were randomly assigned to receive the IBP (n = 28) or standard care (n = 30). Infants were an average age of 13.52 months and predominately from ethnic or racial minority backgrounds (98%). We used bivariate correlations to examine the association between attachment-based caregiving behaviors and behaviorally based parenting do and don’t skills and structural equation modeling to examine the direct effect of the IBP on attachment-based caregiving behaviors and the indirect effect of behaviorally based parenting skills on the relation between intervention group and attachment-based caregiving behaviors. Behaviorally based parenting do and don’t skills were moderately correlated with attachment-based caregiving behaviors. Results demonstrated a direct effect of the IBP on warmth and sensitivity at postintervention and 3- and 6-month follow-ups. The direct effect of the IBP on warmth and sensitivity at the 3- and 6-month follow-ups was mediated by increases in parenting do skills at postintervention. Findings suggest that behaviorally based parenting skills targeted in BPT programs have a broader impact on important attachment-based caregiving behaviors during the critical developmental transition from infancy to toddlerhood.

Sequenced neurocognitive and behavioral parent training for the treatment of ADHD in school-age children

ABSTRACTThe present study examines the potential of sequencing a neurocognitive intervention with behavioral parent training (BPT) to improve executive functions (EFs), psychiatric symptoms, and multiple indices of functional impairment in school-age children aged 7 to 11 years who have been diagnosed with attention-deficit/hyperactivity disorder (ADHD). Specifically, in a randomized controlled trial design, 85 children were assigned to either Cogmed Working Memory Training (CWMT) followed by an empirically supported, manualized BPT intervention, or to a placebo version of CWMT followed by the same BPT intervention. Working memory maintenance (i.e., attention control/short-term memory), working memory processing and manipulation, ADHD and oppositional defiant disorder (ODD) symptoms, impairment in parent–child dynamics, familial impairment, and overall functional compromise were evaluated as outcomes. The results suggest specific effects of the combined CWMT and BPT program on verbal and nonverbal working memory storage and nonverbal working memory processing and manipulation but no incremental benefits in regard to ADHD symptoms, ODD symptoms, and functional outcomes. The present findings do not support the hypothesis regarding the complementary and augmentative benefits of sequenced neurocognitive and BPT interventions for the treatment of ADHD. These results, the study’s limitations, and future directions for research are further discussed.
