KRAS Protein Expression in Oral Squamous Cell Carcinoma: A Potential Marker for Progression and Prognosis.

Emerging evidence suggests that KRAS could play an important role in squamous cell carcinoma; however, its role in oral squamous cell carcinoma (OSCC) is largely unknown. The aim of the current study was to investigate the expression of KRAS, Ki-67, Cyclin D1, and Bcl2 in OSCC and their association with clinicopathological features. Forty paraffin blocks of retrospective histologically diagnosed cases of OSCC and 20 blocks of oral leukoplakia with epithelial dysplasia were obtained from two hospitals between 2018 and 2021. The paraffin-embedded tissue was analyzed for the expression of KRAS for oral epithelial dysplasia and OSCC, and ki-67, Cyclin D1, and bcl2 were analyzed only for OSCC. The results were correlated with each other and with different clinicopathological features and were statistically analyzed. KRAS expression was significantly associated with histological tumor grade, tumor extent, presence of nodal and distant metastasis, pathological stage, and the presence of lymphovascular invasion (P=<0.001, 0.001, 0.001, 0.009, <0.001, and <0.001, respectively). The KRAS expression was positively correlated with the histological grade, tumor extent, nodal status, and the pathological stage (r=0.712, 0.649, 0.646, and 0.865, respectively). A positive correlation was also found with the expression of Bcl2, Cyclin D1, and Ki-67 (r=0.81, 0.723, and 0.698, respectively). The KRAS expression in oral epithelial dysplasia was significantly lower than that in OSCC (P=0.003). KRAS may be a potential prognostic marker for OSCC and may play a role in its progression.

The expression of Bcl-2 in oral squamous cell carcinoma—A review

Currently, oral cancer is one of the most alarming health problems facing mankind. More than 90% of all oral cancers are oral squamous cell carcinoma (OSCC). BCL-2 proteins are one of the most prominent anti-apoptotic proteins expressed in OSCC. They contribute to cancer development and mediate resistance to current anticancer treatments. Various studies have been conducted to evaluate the expression of Bcl-2 in OSCC. This article emphasizes the importance of Bcl-2 expression as a prognostic indicator in OSCC.

Possible Role of Bcl-2 Expression in Metastatic and Non Metastatic Oral Squamous Cell Carcinoma.

Oral carcinoma is a global health problem. India accounts for one-third of world burden with high prevalence rate of 2.5 million and mortality rate of 5.5 lacs per year. Oral carcinogenesis is a multistage process that frequently involves invasion and metastasis that needs early detection and treatment to improve the overall quality of life. The ability of Oral Squamous Cell Carcinoma (OSCC) to metastasize to lymph nodes does not always show a relationship with clinical staging. A spectrum of molecular mechanisms is involved in carcinogenesis, where defect in the regulation of apoptosis is assumed to contribute to the pathogenesis and progression of cancer. Apoptosis regulatory genes include the antiapoptotic protein such as B-cell lymphoma-2 (Bcl-2) gene that might be used as a molecular marker to evaluate the biological behaviour of oral cancer. To evaluate and correlate the Bcl-2 expression in OSCC patients with lymphnode metastasis and without metastasis. The study comprised of 30 samples, 15 cases of metastatic and 15 non-metastatic primary OSCC. All the cases were stained for routine Hematoxylin and Eosin (H&E), and Bcl-2 antibody by immunohistochemistry. The H&E stained sections were evaluated for Lymphocytic Infiltrate (LI) and Pattern Of Invasion (POI). Bcl-2 stained sections were evaluated for staining intensity and distribution. The differences between the groups were statistically analysed using chi-square test. The histopathological parameters, LI and POI did not show any statistical difference between the study groups. Expression of Bcl-2 in OSCC was 33.3% among the study groups, where metastatic group showed a positive expression of 13.3% and 20% in non-metastatic OSCC that did not show statistically significance among the study groups. There was no significant difference in the expression of Bcl-2 between the study groups. Apoptosis is regulated by interaction among the Bcl-2 gene family. Hence, evaluation of Bcl-2 along with other apoptotic regulating proteins could define the role in pathogenesis and prognosis of metastatic and non-metastatic OSCC.

Expression and clinical significance of nuclear factor κB/B cell lymphoma-2 signal pathway and glycogen synthase kinase 3β in oral squamous cell carcinoma

This study aims to examine the expression of nuclear factor κB (NF-κB)/B cell lymphoma-2 (Bcl-2) signal pathway and glycogen synthase kinase 3β (GSK-3β) in oral squamous cell carcinoma (OSCC) and provide references for the early diagnosis and prognosis evaluation of OSCC. A total of 55 cases of OSCC and 10 cases of paracan- cerous mucosa were examined in this study. Their expressions of GSK-3β, NF-κB and Bcl-2 were detected using the SP me- thod immunohistochemistry. The correlation between their expression in OSCC and the clinical and pathological peculiarity of OSCC was analyzed. The positive expression of GSK-3β, NF-κB, and Bcl-2 in OSCC were significantly higher than that in paracancerous mucosa (P < 0.01). The expression of GSK-3β, NF-κB, and Bcl-2 had no obvious relationship with patient's age, sex, and clinical stages of the disease (P > 0.05). The expression of Bcl-2 was significantly correlated with the degree of tumor differentiation (P < 0.05), whereas the expression of GSK-3β and NF-κB in OSCC had no obvious relation- ship with the degree of tumor differentiation (P > 0.05). Strong positive correlations were observed among the expressions of GSK-3β, NF-κB, and Bcl-2 (P < 0.05). The positive expression of GSK-3β, NF-κB, and Bcl-2 in OSCC are sig- nificantly higher than that in paracancerous mucosa. Detecting GSK-3β, NF-κB, and Bcl-2 in OSCC may have implications in the early diagnosis and prognosis evaluation of OSCC.
