BackgroundRecently, intestinal bacteria have attracted attention as factors affecting the prognosis of patients with cancer. However, the intestinal microbiome is composed of several hundred types of bacteria, necessitating the development of an analytical method that can allow the use of this information as a highly accurate biomarker. In this study, we investigated whether the preoperative intestinal bacterial profile in patients with esophageal cancer who underwent surgery after preoperative chemotherapy could be used as a biomarker of postoperative recurrence of esophageal cancer.MethodsWe determined the gut microbiome of the patients using 16S rRNA metagenome sequencing, followed by statistical analysis. Simultaneously, we performed a machine learning analysis using a random forest model with hyperparameter tuning and compared the data obtained.ResultsStatistical and machine learning analyses revealed two common bacterial genera, Butyricimonas and Actinomyces, which were abundant in cases with recurrent esophageal cancer. Butyricimonas primarily produces butyrate, whereas Actinomyces are oral bacteria whose function in the gut is unknown.ConclusionOur results indicate that Butyricimonas spp. may be a biomarker of postoperative recurrence of esophageal cancer. Although the extent of the involvement of these bacteria in immune regulation remains unknown, future research should investigate their presence in other pathological conditions. Such research could potentially lead to a better understanding of the immunological impact of these bacteria on patients with cancer and their application as biomarkers.

A Systematic Review: Fecal Bacterial Profile in Patients with Irritable Bowel Syndrome Analyzed with the GA-Map Dysbiosis Test Based on the 16S rRNA Gene of Bacterial Species or Groups.

The diagnosis of irritable bowel syndrome (IBS) is based on symptom-based criteria due to lack of reliable disease-specific biomarkers. Gut microbiota is perturbed in IBS and when comparing different methods used to analyze gut microbiota, the results might be obscured. Therefore, in this systematic review we aimed to investigate the profile of fecal bacterial markers and dysbiosis index (DI) in patients with IBS and IBS subgroups compared to healthy controls (HCs) conducted by the same method (GA-map Dysbiosis Test based on16S rRNA sequencing). We searched PubMed, EMBASE (Ovid) and Cochrane Library for case-control studies comparing fecal gut microbiota analyzed with the GA-map® Dysbiosis Test (Oslo, Norway) in patients with IBS and HCs. Our outcomes were the difference in fecal bacterial markers and DI in patients with IBS and IBS subgroups compared to HCs. The search identified 28 citations; five articles were included. Most studies evaluated fecal bacterial markers and DI in patients with diarrhea-predominant IBS (IBS-D). Results of fecal bacteria profile in IBS and IBS subgroups compared to HCs are inconsistent, however, two studies showed increased levels of Ruminococcus gnavus in IBS-D compared to HCs and results of DI indicated IBS and IBS subgroups (especially IBS-D) having higher DI compared to HCs. This systematic review revealed inconsistent findings in respect to differences in bacterial markers between IBS and IBS subgroups with HCs in studies using the GA-map Dysbiosis Test based on 16S rRNA sequencing. However, the test is quite novel, and few studies have used the method so far. More research comparing fecal microbiota profile differences in IBS and IBS subgroups compared to HCs utilizing the same method of analysis is needed to give us further insight into the gut bacteria profile in IBS and the clinical consequences of intestinal dysbiosis.

Comparison of Oral Aerobic Bacterial Profile in Patients with Oral Squamous Cell Carcinoma and Patients with Oral Pre Cancerous Lesions, Attending Tertiary Care Centre in Rajasthan

Background: More than 90 percent of oral cancers are squamous cell carcinomas, making them the most frequent kind of malignant tumor in the oral and maxillofacial area. Oral cancer rates are rising rapidly over the globe. Oral malignant tumors may develop when there is an incongruity between the microbiome and the host. Material & Methods: This prospective study was carried out in microbiology department, SMS Medical College, Jaipur. A total of 200 oral swab samples have been collected 100 for control group and 100 for case group and processed for bacterial identification according to standard microbiological guidelines. Results: Among the 100 samples studied from the oral squamous cell carcinoma patients, Pseudomonas species (n = 45) were the predominant isolate, followed by Klebsiella species (n = 26) and Staphylococcus species (n = 15). Of the 100 samples studied among the control group, Micrococci (24) and Diptheroids (23) is the predominant bacteria isolated. Conclusion: Patients with precancerous lesions had a more varied and abundant oral microbiome compared to those with oral cancer and healthy controls but that there was no difference between the groups in terms of overall microbiota composition. When comparing the oral bacterial profiles of those with cancer and those without, there were clear distinctions. Therefore, bacteria may be used as biomarkers in the diagnosis of oral cancer and as therapeutic targets in its therapy. Changing trend in the clinico- bacteriological profile of diabetic foot infection over a decade: observations from a tertiary care hospital of India

BACKGROUND: Diabetic foot infection (DFI) is a major challenge in podiatric surgery. Initiation of an appropriate antimicrobial drug is the determining factor in the prognosis in DFI.AIM: This study was undertaken to analyse the changing trends in the clinical profile of patients and the bacterial profile causing DFI for over a decade.MATERIALS AND METHODS: This was a retrospective observational study conducted at the department of microbiology and surgery of the tertiary teaching hospital. The clinical and bacterial profile of patients with DFI in 2009 and 2019 was collected and analysed for the changing trend.RESULTS: Amongst the 203 patients (95 in 2009 and 108 in 2019) included in the study, 113 (56%) were male patients. There was a shift in the age group of patients from 51–60 to 61–70 years over a decade. There was a change of most common pathogens from Pseudomonas aeruginosa to Klebsiella species. The resistance rates to third-generation cephalosporins decreased amongst the Gram-negative isolates such as P. aeruginosa and Escherichia coli, but an increase in resistance to carbapenems was observed during the study period. There was reduced incidence of infection with methicillin-resistant Staphylococcus aureus during the study period.CONCLUSION: There was a change in the demographic and bacteriological flora in the DFI patients over a decade. The antimicrobial resistance rate varied for various antimicrobial agents over a decade for a particular pathogen. Regular surveillance of the change in resistance pattern amongst the pathogens is essential for the initiation of appropriate empirical therapy to reduce the morbidity in DFI.

Circulating microbiota and metabolites: Insights into cardiovascular diseases.

In almost every country, cardiovascular diseases are the major cause of death, which are responsible for 17.7million deaths worldwide, or 54% of all deaths. However, the latest evidence has shown that non-communicable diseases such as obesity, diabetes, and cardiovascular events are significantly influenced by the blood microbiota and circulating metabolites. We searched online databases for the most recent related papers through the comprehensive international databases of the Institute of PubMed/ MEDLINE, ISI/WOS, and Scopus up to August 2022, using MESH terms and the related keywords in the English language. Considering the titles and abstracts, unrelated studies were excluded. The full texts of the remained studies were evaluated by authors, independently. Then, the studies' findings were assessed and reported. The study demonstrated that the bacterial profiles of patients with cardiovascular diseases and healthy individuals are significantly different. The diseased patients showed a significantly high abundance of phylum Proteobacteria, an important Proteobacterial component known as lipopolysaccharides that has been linked to the pathogenesis of cardiovascular disease, while phylum Firmicutes were found in healthy individuals. It suggests that Proteobacteria has a direct role in the onset of cardiovascular disease. We focused on the blood bacterial composition and circulating microbial metabolites in their relationship with the etiology and onset of cardiovascular disease. However, the various genera and species in the results reported were not always identical. Therefore, the microbial community structure of blood was more complicated and thus required a more in-depth exploration.

Bacterial co-infection in patients with SARS-CoV-2 in the Kingdom of Bahrain.

BACKGROUNDThe coronavirus disease 2019 (COVID-19) pandemic presents a significant challenge to the medical profession, increasing in the presence of microbial co-infection. Bacterial and Fungal co-infections increase the risk of morbidity and mortality in patients with COVID-19.AIMTo study the bacterial profile in patients with COVID-19 who needed admission to receive treatment in the main centres concerned with managing COVID-19 disease in the Kingdom of Bahrain.METHODSThe study was a retrospective observational analysis of the bacterial profile and the bacterial resistance in patients with confirmed COVID-19 disease who needed admission to receive treatment in the main centres assigned to manage patients with COVID-19 disease in the Kingdom of Bahrain from February to October 2020. We used the electronic patients’ records and the microbiology laboratory data to identify patients’ demographics, clinical data, microbial profile, hospital or community-acquired, and the outcomes.RESULTSThe study included 1380 patients admitted with confirmed COVID-19 disease during the study period. 51% were admitted from February to June, and 49% were admitted from July to October 2020, with a recurrence rate was 0.36%. There was a significant increase in bacterial and fungal co-infection in the second period compared to the first period. The most common isolated organisms were the gram-negative bacteria (mainly Klebsiella pneumoniae, Pseudomonas aeruginosa, multi-drug resistant Acinetobacter baumannii, and Escherichia coli), the gram-positive bacteria (mainly coagulase negative Staphylococci, Enterococcus faecium, Enterococcus faecalis, Staphylococcus aureus) and fungaemia (Candida galabrata, Candida tropicalis, Candida albicans, Aspergillus fumigatus, Candida parapsilosis, Aspergillus niger). The hospital-acquired infection formed 73.8%, 61.6%, 100% gram-negative, gram-positive and fungaemia. Most of the hospital-acquired infection occurred in the second period with a higher death rate than community-acquired infections.CONCLUSIONBacterial and fungal co-infections in patients admitted with confirmed COVID-19 disease pose higher morbidity and mortality risks than those without co-infections. We should perform every effort to minimize these risks.

Impact of Docosahexaenoic Acid Therapy on Subgingival Plaque Microbiota.

Oral docosahexaenoic acid (DHA) + aspirin therapy has been shown to reduce periodontal probing depth (PD) and local inflammatory mediators in gingival crevicular fluid (GCF) among patients with untreated chronic periodontal disease. Whether DHA + aspirin therapy influences specific bacterial burden in this setting is unknown. Thus, the aim of this study is to evaluate the impact of DHA with low-dose aspirin therapy on periodontal bacterial profile in patients with periodontitis. Fifty-five adults with moderate-to-severe periodontitis were enrolled in a randomized, 3-month double-masked, placebo-controlled trial of daily 2 g DHA or placebo capsules enriched with 81 mg aspirin; 46 enrollees completed the trial. In addition to clinical measurements and GCF sampling, subgingival plaque samples were collected from four posterior sites in all participants and analyzed by the checkerboard DNA-DNA hybridization technique. Presence of 40 periodontal bacterial species at baseline and 3 months was semiquantitatively estimated. Despite broad improvements in clinical parameters, total bacteria and individual species counts in dental plaque did not differ significantly between baseline and 3 months in either group (P >0.1 for all). A modest effect of DHA + aspirin on Porphyromonas gingivalis counts was associated with 14% (95% confidence interval: 3% to 35%) of the observed benefit of DHA on PD. DHA + aspirin had no significant effect on individual plaque bacterial counts in unadjusted models or those adjusted for age, sex, and race (P >0.1 for all). This pilot randomized, controlled trial suggests that DHA + aspirin therapy improves periodontitis largely by modulating host inflammatory response. Changes in individual species levels in subgingival plaque microbiota were not detectable; however, a small portion of the benefit appears to stem from changes in P. gingivalis levels in the DHA + aspirin treatment group. Whether this change in P. gingivalis levels leads to biofilm alteration with reversal of dysbiosis requires further longitudinal and more specific investigations.

Lobar Distribution in Non-Cystic Fibrosis Bronchiectasis Predicts Bacteriologic Pathogen Treatment

Non-cystic fibrosis bronchiectasis (NCFBr) is a major cause of morbidity due to frequent infectious exacerbations. We analyzed the influence of patient age and bronchiectasis location on the bacterial profile of patients with NCFBr. This retrospective cohort study included 339 subjects diagnosed with an infectious exacerbation of NCFBr during the 9-year period between January 2006 and December 2014. Bronchoalveolar lavage (BAL) cultures and high-resolution computed tomography scans (HRCT) were utilized to characterize the location of the bronchiectasis and bacteriologic pathogenic profile. In univariate logistic regression, the frequency of Haemophilus influenzae was higher in patients aged ≤64 years (OR = 0.969, p 64 years. The lobar distribution of bronchiectasis in the subjects was 25.9 % in the right middle lobe (RML), 20.7 % in the right lower lobe (RLL), 20.4 % in the left lower lobe (LLL), 13.8 % in the lingula, 13 % in the right upper lobe (RUL), and 6.2 % in the left upper lobe (LUL). In the lower lobes, H. influenzae was the dominant species isolated, whereas in the RUL it was P. aeruginosa and in the LUL it was non- tuberculous mycobacterium (NTM). H. influenzae was more prevalent in younger patients, whereas P. aeruginosa, Enterobacteriaceae and NTM predominated in older patients. Different pathogens were associated with different lobar distributions. The RML, RLL and LLL showed a greater tendency to develop bronchiectasis than other lobes.

Gut bacterial profile in patients newly diagnosed with treatment-na&amp;iuml;ve Crohn&amp;#39;s disease

Objectives:The aim of this study was to define the composition of the gut bacterial flora in Norwegian patients with early stage Crohn’s disease (CD).Methods:By using a nonselective metagenomics approach, the general bacterial composition in mucosal biopsies from the ileum and the colon of five subjects, four patients with different phenotypes of CD, and one noninflammatory bowel disease control, was characterized. After partial 16S ribosomal RNA (rRNA) gene sequencing, BLAST homology searches for species identification and phylogenetic analysis were performed.Results:An overall biodiversity of 106 different bacterial operational taxonomic units (OTUs) was detected in the cloned libraries. Nearly all OTUs belonged to the phylae Bacteroidetes (42% in CD, 71% in the control) or Firmicutes (42% in CD, 28% in the control), except for some OTUs that belonged to the phylum Proteobacteria (15% in CD, 0% in the control) and a few OTUs that could not be assigned to a phylum (2% in CD, 1% in the control).Conclusion:Based on the high incidence of inflammatory bowel disease (IBD) in Norway, this pilot study represents a relevant determination of the gut microbiota in Norwegian patients compared to previous findings in other countries. The bacterial profile of Norwegian CD patients was found to be similar to that of CD patients in other countries. The findings do not support a particular bacterial composition as a predominant causative factor for the high incidence of IBD that exists in some countries.

Molecular methodology to assess the impact of cancer chemotherapy on the oral bacterial flora: a pilot study

This pilot study determined the profile of the oral bacterial flora in an outpatient cancer population before and after chemotherapy using molecular techniques. We recruited 9 newly diagnosed breast cancer patients scheduled for induction chemotherapy. All were seen immediately before chemotherapy, and 7 to 14 days later. At both visits, we performed oral evaluations and obtained mucositis grading (with the World Health Organization [WHO] scale), absolute neutrophil counts (ANC), and bacterial samples from the buccal mucosa. Bacterial DNA was isolated, and 16S ribosomal RNA gene clonal libraries were constructed. Sequences of genes in the library were used to determine species identity by comparison to known sequences. After chemotherapy, WHO scores of 0 and 1 were in 3 and 6 patients, respectively, and mean ANC (+/-SD) dropped from 3326 +/- 463 to 1091 +/- 1082 cells/mm(3). From pre- and post-chemotherapy samples, 41 species were detected, with a predominance of Gemella haemolysans and Streptococcus mitis. More than 85% of species have not been previously identified in chemotherapy patients. Seven species appeared exclusively before chemotherapy and 25 after chemotherapy. After chemotherapy, the number of species per patient increased by a mean of 2.6 (SD = 4.7, P = .052). We identified species not previously identified in chemotherapy patients. Our results suggest a shift to a more complex oral bacterial profile in patients undergoing cancer chemotherapy.

Combination of 16S rRNA variable regions provides a detailed analysis of bacterial community dynamics in the lungs of cystic fibrosis patients

Chronic bronchopulmonary bacterial infections remain the most common cause of morbidity and mortality among patients with cystic fibrosis (CF). Recent community sequencing work has now shown that the bacterial community in the CF lung is polymicrobial. Identifying bacteria in the CF lung through sequencing can be costly and is not practical for many laboratories. Molecular techniques such as terminal restriction fragment length polymorphism or amplicon length heterogeneity-polymerase chain reaction (LH-PCR) can provide many laboratories with the ability to study CF bacterial communities without costly sequencing. The aim of this study was to determine if the use of LH-PCR with multiple hypervariable regions of the 16S rRNA gene could be used to identify organisms found in sputum DNA. This work also determined if LH-PCR could be used to observe the dynamics of lung infections over a period of time. Nineteen samples were analysed with the V1 and the V1_V2 region of the 16S rRNA gene. Based on the amplicon size present in the V1_V2 region, Pseudomonas aeruginosa was confirmed to be in all 19 samples obtained from the patients. The V1 region provided a higher power of discrimination between bacterial profiles of patients. Both regions were able to identify trends in the bacterial population over a period of time. LH profiles showed that the CF lung community is dynamic and that changes in the community may in part be driven by the patient's antibiotic treatment. LH-PCR is a tool that is well suited for studying bacterial communities and their dynamics.
