A rare case of acute traumatic complicated right axillary artery pseudoaneurysm with quadrangular space syndrome

Axillary artery pseudoaneurysms are relatively rare, with few reported cases found in the literature. There have been high rates of rupture and compression-related complications in trauma-related pseudoaneurysms of the upper limb. Prompt diagnosis with non-invasive imaging modalities such as ultrasound, magnetic resonance imaging, and computed tomography. Angiography is required for immediate intervention and prevention of complications. Here, we present a case of an extremely rare right-sided acute traumatic axillary artery pseudoaneurysm with compression, resulting in quadrangular space syndrome in a 31-year-old male patient. We performed a critical literature review of the same and identified only a few previously reported cases.

Percutaneous treatment of a Right Axillary Pseudoaneurysm

We present a very rare case of a 62-year-old male who was presented to our department with a large pulsatile mass on the upper lateral part of the right hemi thorax. He had undergone multiple vascular surgeries in the past months, last one including right axillary-femoral artery bypass grafting (AFBG). Last few days a large, growing pulsatile mass was identified. Contrast computed tomography identified 6x8 cm pseudoaneurysm of the right axillary artery. An open surgical repair was unsuccessful. He underwent an endovascular treatment using a stent graft via left femoral access. The patient had an uneventful recovery and remained well on follow-up.

A 27-year-old male patient with a wound due to a gunshot in right upper thoracic area who was diagnosed as a pseudoaneurysm of right axillary artery underwent open resection and bypass graft with saphenous vein. Today, most vascular surgeons tend to treat these complications with endovascular procedure, but because of this patient’s young age and his occupation and the necessity for brachial plexus repair, we decided the open procedure method.

Ruptured axillary artery pseudoaneurysm following an anterior glenohumeral dislocation: A case report.

Vascular injuries following anterior shoulder dislocations are rare, with an estimated incidence of 1-2%. The formation of an axillary artery pseudoaneurysm secondary to vascular trauma is a possible late complication and frequently underdiagnosed since it may remain asymptomatic for many years. A rupture of a pseudoaneurysm may occur either from the dislocation itself or after forceful reduction attempts. A ruptured pseudoaneurysm of the axillary artery is a medical emergency and may result in significative upper-limb morbidity or even patient mortality. Nowadays, endovascular techniques have progressively gained ground for the treatment of such lesion, especially in an emergency context. In the present article, the authors present the case of a 77-years-old male patient with a rupture of a pseudoaneurysm of the left axillary artery after repeated forceful reductions of an anterior glenohumeral dislocation and its treatment with percutaneous endovascular stenting.

A Challenging Surgical Case of Axillary Artery Pseudoaneurysm Weeks after Blunt Trauma

Axillary artery injury is a rare complication of blunt upper extremity trauma and is reported in the literature on only a few cases. The usual treatment is an open operation with the exclusion of the aneurysm and interposition grafting. Vascular injury in the elderly population after trauma is especially “dangerous” owing to a large cohort of these patients being on anticoagulation. For this reason, any hematoma after blunt trauma must be fully “evaluated” and a careful vascular exam must be undertaken. When the vascular injury is acute as a result of trauma, the typical interventions include an open repair either with an interposition graft or reverse saphenous vein graft. We report a case of a delayed traumatic axillary artery dissection with resultant pseudoaneurysm treated with endovascular stent-grafting.

Axillary Artery Pseudoaneurysm After Revision Plate Osteosynthesis of a Midshaft Clavicle Fracture Nonunion: A Case Report.

A patient with history of nonunion repair of traumatic midshaft clavicle fracture was referred to our orthopaedic trauma clinic for new-onset shoulder pain and paresthesias involving the ipsilateral arm. Computed tomography angiography revealed an axillary artery pseudoaneurysm adjacent to the instrumentation and recurrent nonunion of the fracture site. The patient underwent coil embolization of the pseudoaneurysm and nonunion repair. This case demonstrates that iatrogenic neurovascular injury during clavicle fracture nonunion repairs can present in a delayed fashion requiring more thorough clinical and imaging evaluation to achieve successful treatment.

A Rare Case of Axillary Artery Pseudoaneurysm Due to Shoulder Dislocation Treated With Percutaneous Thrombin Injection

The incidence of combined neurovascular injuries among patients with high-energy shoulder trauma ranges from 27% to 44%. However, the presentation of an axillary artery pseudoaneurysm (PSA) due to shoulder dislocation without an associated osseous fracture is a very rare condition. Moreover, treatment of combined neurologic and vascular injuries of the shoulder remains controversial. Additionally, minimally invasive treatments such as thrombin injection have been mainly evaluated in patients with iatrogenic femoral artery PSAs. Therefore, we aim to report a rare case of axillary artery PSA associated with brachial plexus injury after shoulder dislocation treated with percutaneous thrombin injection.

Traumatic pseudoaneurysm of axillary artery combined with brachial plexus injury.

Traumatic pseudoaneurysm of the axillary artery combined with brachial plexus injury is extremely rare. The factors that influence the symptoms and functional recovery related to this condition are unclear. Nine patients who had sustained this trauma were surgically treated at our unit between June 1999 and November 2010. The cause of trauma, symptoms, signs and examinations of neurological and vascular deficits, and the surgical findings of the involved nerves and vessels were recorded in detail. The functional recovery of vessels and nerves, as well as the extent of pain, were evaluated, respectively. The average length of patient follow-up was 4.5 years (range, 24 months to 11.3 years). After vessel repair, whether by endovascular or operative treatment, the distending, constant, and pulsating pain was relieved in all patients. Furthermore, examination of the radial artery pulse on the repaired side appeared normal at last follow-up. All patients showed satisfactory sensory recovery, with motor recovery rated as good in five patients and fair in four patients. The symptom characteristics varied with the location of the damage to the axillary artery. Ultrasound examination and computed tomography angiography are useful to evaluate vascular injury and provide valuable information for operative planning. Surgical exploration is an effective therapy with results related to the nerve injury condition of the brachial plexus.

Traumatic axillary artery pseudoaneurysm treated with intravascular balloon occlusion and percutaneous thrombin injection

Axillary artery pseudoaneurysms are relatively rare, with few reported cases found in the literature. Furthermore, treatment with percutaneous thrombin injection has not yet been reported. We report the case of a 59-year-old man with a large (10 cm) post-traumatic pseudoaneurysm of the left axillary artery found five weeks after a motorcycle crash. The patient sustained multiple injuries, including fractures of the left scapula and clavicle. Edema was observed at the time of diagnosis. Arteriography with successful ultrasound-guided percutaneous thrombin injection was undertaken. The patient experienced no complications after the procedure.

Supraclavicular Approach for Repair of an Axillary Artery Pseudoaneurysm after Axillobifemoral Bypass

A 61-year-old woman underwent right axillobifemoral bypass using a reinforced expanded polytetrafluoroethylene T-shaped graft for high aortic occlusion. One year later, anastomotic pseudoaneurysm of the axillary artery was noted. We performed pseudoaneurysmectomy and graft interposition at the same anastomotic site through an infraclavicular approach. Unfortunately, the pseudoaneurysm recurred four months later. Therefore, we performed a second reoperation through a supraclavicular approach, in addition to the infraclavicular one. We were able to achieve better exposure from the axillary artery to the distal part of the subclavian artery, and reconstruct the bypass with secure and safe arterial clamping and reanastomosis.

Axillary artery pseudoaneurysms are rare. We report on a 30-year-old patient with a 6.5-cm post-traumatic pseudoaneurysm of the left axillary artery two months after a knife stab wound of the shoulder. The patient showed axillary fullness and signs of severe brachial plexus compression. A surgical repair was undertaken. The aneurysm was excluded and a saphenous vein interposition was performed. The early and late postoperative periods were uneventful. This is probably not only the largest axillary artery pseudoaneurysm ever reported, but also the first secondary to a stab wound.

An unusual presentation of traumatic pseudoaneurysm of axillary artery mimicking soft tissue tumor.

Traumatic pseudoaneurysm of the axillary artery is a rare sequela of injury to shoulder region. We report here a unique case of delayed presentation of axillary artery pseudoaneurysm after a blunt injury, clinically mimicking soft tissue tumor without evidence of gross bony injury. The gentleman presented after six months of injury with a progressively growing mass in left axillary region and neurological deficit. Ultimate management of the lesion was surgical resection and Saphenous vein graft interpositioning. Unfortunately the lack of subsequent neurological recovery parallels some of the findings in the literature, from cases where decompression of the brachial plexus was not undertaken soon enough.
