Recruiting refugees to reduce labour shortages in health care professions: experimental evidence on the potential of foreign-language outreach on social media

BackgroundMany high-income countries are grappling with severe labour shortages in the healthcare sector. Refugees and recent migrants present a potential pool for staff recruitment due to their higher unemployment rates, younger age, and lower average educational attainment compared to the host society's labour force. Despite this, refugees and recent migrants, often possessing limited language skills in the destination country, are frequently excluded from traditional recruitment campaigns conducted solely in the host country’s language. Even those with intermediate language skills may feel excluded, as destination-country language advertisements are perceived as targeting only native speakers. This study experimentally assesses the effectiveness of a recruitment campaign for nursing positions in a German care facility, specifically targeting Arabic and Ukrainian speakers through Facebook advertisements.MethodsWe employ an experimental design (AB test) approximating a randomized controlled trial, utilizing Facebook as the delivery platform. We compare job advertisements for nursing positions in the native languages of Arabic and Ukrainian speakers (treatment) with the same advertisements displayed in German (control) for the same target group in the context of a real recruitment campaign for nursing jobs in Berlin, Germany. Our evaluation includes comparing link click rates, visits to the recruitment website, initiated applications, and completed applications, along with the unit cost of these indicators. We assess statistical significance in group differences using the Chi-squared test.ResultsWe find that recruitment efforts in the origin language were 5.6 times (Arabic speakers) and 1.9 times (Ukrainian speakers) more effective in initiating nursing job applications compared to the standard model of German-only advertisements among recent migrants and refugees. Overall, targeting refugees and recent migrants was 2.4 (Ukrainians) and 10.8 (Arabic) times cheaper than targeting the reference group of German speakers indicating higher interest among these groups.ConclusionsThe results underscore the substantial benefits for employers in utilizing targeted recruitment via social media aimed at foreign-language communities within the country. This strategy, which is low-cost and low effort compared to recruiting abroad or investing in digitalization, has the potential for broad applicability in numerous high-income countries with sizable migrant communities. Increased employment rates among underemployed refugee and migrant communities, in turn, contribute to reducing poverty, social exclusion, public expenditure, and foster greater acceptance of newcomers within the receiving society.

Childhood migration and educational attainment: Evidence from Indonesia

Millions of families migrate every year in search of better opportunities. Whether these opportunities materialize for the children brought with them depends on the quality of the destination that their parents selected. Exploiting variation in the age of migration, I analyze the impact of destination quality on the educational outcomes of childhood internal migrants in Indonesia. Using Population Census microdata from 2000 and 2010, I show that children who spend more time growing up in districts characterized by higher average educational attainment among permanent residents tend to exhibit greater probabilities of completing primary and secondary schooling. Moreover, educational outcomes of migrants converge with those of permanent residents at an average rate of 1.7 to 2.2 percent annually, with children from less educated households benefiting more from additional exposure. My findings suggest substantial heterogeneity of returns to childhood migration with respect to destination.

Gender based disparities in Medicare physician reimbursement persist across years and specialty.

The gender pay gap is wide in medicine but the extent of this disparity across specialties and over time have not been elucidated. Here we evaluate differences in Medicare reimbursement between men and women physicians over time and by specialty, controlling for physician and practice characteristics. The Centers for Medicare & Medicaid Services Payment Data was used to determine total reimbursements and number of services submitted by physicians practicing in the US between 2013 and 2019. Data from the American Community Survey (ACS) were used to determine average income, unemployment rates, poverty rates, income, and educational attainment levels by zip code for each physician's practice location. Among the 3,831,504 physicians included in this analysis from 2013-2019, 2,712,545 (70.8%) were men and 1,118,859 (29.2%) were women. Overall, men received more in Medicare reimbursements ($58,815 ± $104,772 vs. $32,205 ± $60,556, p<0.001) and billed more services (864 ± 1,780 vs. 505 ± 1,007, p<0.001) compared to women. The median Medicare reimbursement for men decreased from 2013 to 2019 from $59,710 to $57,874, while the median Medicare reimbursement for women increased from $30,575 to $33,456. Men were reimbursed more than women across all specialties with the greatest disparity in procedure-heavy specialties. The specialties with the highest difference in median Medicare reimbursement between men and women were ophthalmology ($99,452), dermatology ($84,844), cardiology ($64,112), nephrology ($62,352), and pulmonary medicine ($47,399). In linear regression models controlling for calendar year, years of experience, total number of services, and ACS zip-code-level variables, men received a higher amount of Medicare reimbursement in all specialties, as compared to women (p<0.01 for all). The percentage of top earning men (range: 65.0%-99.5%) surpassed the proportion of men in each specialty (range: 46.1%-94.6%), except public health and preventive medicine. Women physicians continue to receive lower total Medicare reimbursements than men physicians, particularly in procedure-heavy specialties. Lower clinical volume and fewer procedural services among women physicians partially contribute to the disparities in reimbursement.

Varieties occupational change in Europe after the great recession

ABSTRACT This paper empirically explores occupational change in Europe after the 2008-crisis (the Great Recession). During this period, which has remained relatively unexplored by the literature so far, many European economies have implemented profound institutional changes in their labour markets and transformed their growth models. Using individual-level data of 18 economies, I build three indicators of job quality -the average educational attainment, the median earnings and an index of job instability based on the contractual characteristics of the job- and analyse relative employment growth of jobs. The findings suggest that there is not just one pattern of occupational change in Europe, in opposition to the mainstream view of pervasive polarisation. On the contrary, I detect a variety of occupational change profiles, which even differ within the same country depending on the indicator employed.

Correlation of Job Satisfaction of Poultry Farm Workers and Product Compliance Sustainability

The study aimed to scrutinize the relationships between Job Satisfaction and Product Compliance indicators such as Supervisor's Relations, Co-workers' Relations, Nature of Work, Working Conditions, Pay and promotions, Product Attributes, Product Standards, and Product Disclosure. The samples were poultry farm workers from the province of Bulacan, Philippines. In addition to employing random sampling, the research instruments comprised rating scale-equipped questionnaires. The research utilized Mann-Whitney U and Kruskal-Wallis to test the significant difference, Spearman's Rho to test the relationship, and SEM to analyze the structural relationship. The results showed that pay and promotion have significant differences when grouped according to sex, family size, and length of service. Co-worker relations significantly differ when grouped according to average monthly income and educational attainment. Product disclosure has significant differences when grouped according to age and educational attainment. Product attributes significantly differ when grouped according to average monthly income, educational attainment, length of service, and number of training attended. Product standards have significant differences when grouped according to the number of training attended. Product disclosures have significant differences when grouped according to average monthly income, length of service, and number of training attended. Co-workers’ relations have a significant relationship with product attributes. SEM shows that supervisor relations, product attributes, and product disclosures are correlated. Job satisfaction can be enhanced through the provision of higher pay, training, and good working conditions. Future research should be qualitative for a richer qualitative interpretation.

The Impact of Long COVID on Cognitive Performance and Sleep Quality: An Analysis of the Rancagua Chilean Study (RACHIS).

Background Infection with the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) can lead to prolonged symptoms post-recovery, commonly known as long-term coronavirus disease 2019 (COVID-19) or "long COVID." Neuropsychiatric consequences of long COVID include cognitive dysfunction and sleep disturbances, which significantly impair daily living. This study aimed to explore the impact of long COVID on cognitive performance and sleep quality in patients receiving outpatient care. Material and methods This study involved a random sample of 138 of 363 patients, corresponding to 38% of the cohort, who tested positive for SARS-CoV-2 via polymerase chain reaction (PCR) between May 2020 and April 2021. These unvaccinated, non-hospitalized individuals, predominantly exhibiting mild disease symptoms, were prospectively assessed 11 months post-positive PCR test. After informed consent, demographic data, memory, and concentration impairment levels were collected through interviews. Participants reporting cognitive symptoms underwent the Mini-Mental State Examination (MMSE), the Montreal Cognitive Assessment (MOCA), and the Pittsburgh Sleep Quality Index. Statistical analyses were conducted, including Student's t-test, Chi-square, Fisher's test, Kruskal-Wallis test, and Pearson correlation coefficient, with a significance threshold set at p<0.05. Results Of the 138 participants, 76 (55.1%) were female and 62 (44.9%) were male. The mean age was 45.9 years (± 13.0), with an average educational attainment of 10.4 years (± 3.7). Roughly 50% of the patients reported significant memory and concentration issues (p<0.001). Thirty-three participants underwent detailed cognitive assessments, revealing a 2:1 female-to-male ratio and a significantly higher prevalence of depression in female participants. Cognitive deficits were diagnosed in five (15.2%) participants via the MMSE and in 26 (78.8%) via the MOCA test, with notable deficits in visuospatial/executive functions, language repeat, and deferred recall (p<0.001). A lower educational level was correlated with higher cognitive deficits (p=0.03). Conclusion The study findings reveal that cognitive impairments, as a consequence of COVID-19, can persist up to 11 months post-infection. The MOCA test proved more effective in diagnosing these deficits and requires adjustments based on educational background. Sleep parameters remained largely unaffected in this cohort, likely attributed to the mild nature of the initial symptoms and the outpatient management of the disease.

Assessing Teachers’ Readiness for Mathematics Instruction: Insights from Student Perspectives

This study evaluated the perceived readiness of Mathematics teachers at Luuk National High School from the perspective of their students. Utilizing a descriptive-correlational research design, the study included 100 student respondents selected through purposive sampling. Key findings are as follows: The majority of the 100 student respondents are female, within the typical age range for junior high school, and evenly distributed across the four grade levels. Their parents generally have average educational attainment and low-income levels. Students perceive their teachers as frequently prepared to teach Mathematics across three main areas: understanding of student needs in Mathematics, strategies for engaging students, and the implementation of instructional practices. No significant differences were observed in perceived teacher readiness based on students' age, gender, parental education, or family income, except grade level. There is a significant positive correlation among the subcategories of teacher readiness, except between understanding student needs in Mathematics and engagement strategies. The study offers several recommendations: School administrators should enhance support and resources for Mathematics teachers through professional development, mentoring, feedback, and recognition. Mathematics teachers are encouraged to deepen their understanding of student needs, refine their instructional strategies, and collaborate with colleagues to share best practices. Parents should actively support their children's Mathematics education, especially given the low income and educational levels within the community. Students should value and leverage the readiness and resources provided by their teachers. Future research should investigate additional factors influencing teacher readiness, such as school culture, curriculum, assessment methods, and technological integration.

Ecosistema de la Economía Violeta en la Ciudad de Cuenca, una alternativa económica con enfoque de Género

This research provides an in-depth analysis of the current labor conditions for women, focusing on gender inequality, occupational segregation, and the persistent wage gap. The primary objective was to ascertain the level of job satisfaction among women in Cuenca by integrating the concept of the violet economy, which emphasizes the inclusion of rural parishes. A non-experimental, quantitative methodology was utilized. Data were collected via a survey conducted with 96 women operating formal businesses under the auspices of the Prefecture of Azuay in May and June 2023. Survey responses were operationalized into variables such as age, average monthly income, educational attainment, tenure, work hours, affiliation, and credit access. Additional variables were assessed using a Likert scale, with multiple responses analyzed through SPSS software. The findings reveal that entrepreneurial women generally lack fixed incomes or earnings above the basic salary, constraining their access to adequate living conditions. The overall job satisfaction level was classified as 'Moderately Satisfied.' Following the enactment of the violet economy law, women's participation becomes pivotal in narrowing the gender gap, empowering women, and achieving economic stability. This engagement is instrumental in fostering human development and reinforcing the nation's economic framework.

Modern services led growth and development in a structuralist dual economy: Long‐run implications of skilled labor constraint

AbstractMotivated by the South Asian experience, this paper examines the role of expansion of skilled labor force for modern services led growth and development in economies with low average educational attainment. The model economy consists of two capital‐using sectors—a service sector that employs only skilled labor and an industry sector that does not require skilled labor. The service sector represents modern skilled‐labor intensive services whereas the industry sector represents a typical South Asian industry. Supply of unskilled labor is unlimited but skilled labor is relatively scarce and grows at a finite rate. Increase in the skill premium only partially explains growth of skilled‐labor supply while the rest depends on autonomous factors, which may be influenced by education policies of the government. The main result specifies a set of necessary and sufficient conditions for existence of a unique steady state characterized by balanced sectoral growth at a rate determined by the autonomous part of skilled labor growth. A low wage share and stagnant investment conditions in the industry sector are conducive for both existence and local stability of this steady state. The model shows that supply‐side factors may completely determine steady state growth rates in structuralist models despite presence of unemployed resources.

Examining the Black Gender Gap in Educational Attainment: The Role of Exclusionary School Discipline &amp; Criminal Justice Contact

Abstract Black men and women have different levels of average educational attainment, yet few studies have focused on explaining how and why these patterns develop. One explanation may be inequality in experiences with institutional punishment through exclusionary school discipline and criminal justice exposure. Drawing on intersectional frameworks and theories of social control, I examine the long-term association between punishment and the Black gender gap using data from the Children of the National Longitudinal Survey of Youth 1979 cohort (NLSY-C). Decomposition analyses reveal that about one third of the gender gap can be explained by gender differences in experiences with institutional punishments, net of differences in observed behaviors. These measures are predictive at key educational transition points, including finishing high school and earning a 4-year college degree. Though Black boys and girls have similar family backgrounds and grow up in similar neighborhoods, results suggest that Black girls have a persistent advantage in educational attainment due in part to their lower levels of exposure to exclusionary school discipline and the criminal justice system. In addition, I find that gender differences in early achievement, early externalizing behavioral problems, school experiences, and substance use in adolescence and early adulthood are associated with gender differences in educational attainment. Taken together, these results illustrate the importance of punishment disparities in understanding disparate educational outcomes over the life course of Black men and women.

Population-Level Risk Factors Related to Measles Case Fatality: A Conceptual Framework Based on Expert Consultation and Literature Review.

A better understanding of population-level factors related to measles case fatality is needed to estimate measles mortality burden and impact of interventions such as vaccination. This study aimed to develop a conceptual framework of mechanisms associated with measles case fatality ratios (CFRs) and assess the scope of evidence available for related indicators. Using expert consultation, we developed a conceptual framework of mechanisms associated with measles CFR and identified population-level indicators potentially associated with each mechanism. We conducted a literature review by searching PubMed on 31 October 2021 to determine the scope of evidence for the expert-identified indicators. Studies were included if they contained evidence of an association between an indicator and CFR and were excluded if they were from non-human studies or reported non-original data. Included studies were assessed for study quality. Expert consultation identified five mechanisms in a conceptual framework of factors related to measles CFR. We identified 3772 studies for review and found 49 studies showing at least one significant association with CFR for 15 indicators (average household size, educational attainment, first- and second-dose coverage of measles-containing vaccine, human immunodeficiency virus prevalence, level of health care available, stunting prevalence, surrounding conflict, travel time to major city or settlement, travel time to nearest health care facility, under-five mortality rate, underweight prevalence, vitamin A deficiency prevalence, vitamin A treatment, and general malnutrition) and only non-significant associations for five indicators (antibiotic use for measles-related pneumonia, malaria prevalence, percent living in urban settings, pneumococcal conjugate vaccination coverage, vitamin A supplementation). Our study used expert consultation and a literature review to provide additional insights and a summary of the available evidence of these underlying mechanisms and indicators that could inform future measles CFR estimations.

Susceptibility to Breast Cancer Misinformation Among Chinese Patients: Cross-sectional Study.

Currently, breast cancer is the most commonly diagnosed cancer and the sixth-leading cause of cancer-related deaths among Chinese women. Worse still, misinformation contributes to the aggravation of the breast cancer burden in China. There is a pressing need to investigate the susceptibility to breast cancer misinformation among Chinese patients. However, no study has been performed in this respect. This study aims to ascertain whether some demographics (age, gender, and education), some health literacy skills, and the internal locus of control are significantly associated with the susceptibility to misinformation about all types of breast cancers among randomly sampled Chinese patients of both genders in order to provide insightful implications for clinical practice, health education, medical research, and health policy making. We first designed a questionnaire comprising 4 sections of information: age, gender, and education (section 1); self-assessed disease knowledge (section 2); the All Aspects of Health Literacy Scale (AAHLS), the eHealth Literacy Scale (eHEALS), the 6-item General Health Numeracy Test (GHNT-6), and the "Internal" subscale of the Multidimensional Health Locus of Control (MHLC) scales (section 3); and 10 breast cancer myths collected from some officially registered and authenticated websites (section 4). Subsequently, we recruited patients from Qilu Hospital of Shandong University, China, using randomized sampling. The questionnaire was administered via wenjuanxing, the most popular online survey platform in China. The collected data were manipulated in a Microsoft Excel file. We manually checked the validity of each questionnaire using the predefined validity criterion. After that, we coded all valid questionnaires according to the predefined coding scheme, based on Likert scales of different point (score) ranges for different sections of the questionnaire. In the subsequent step, we calculated the sums of the subsections of the AAHLS and the sums of the 2 health literacy scales (the eHEALS and GHNT-6) and the 10 breast cancer myths. Finally, we applied logistic regression modeling to relate the scores in section 4 to the scores in sections 1-3 of the questionnaire to identify what significantly contributes to the susceptibility to breast cancer misinformation among Chinese patients. All 447 questionnaires collected were valid according to the validity criterion. The participants were aged 38.29 (SD 11.52) years on average. The mean score for their education was 3.68 (SD 1.46), implying that their average educational attainment was between year 12 and a diploma (junior college). Of the 447 participants, 348 (77.85%) were women. The mean score for their self-assessed disease knowledge was 2.50 (SD 0.92), indicating that their self-assessed disease knowledge status was between "knowing a lot" and "knowing some." The mean scores of the subconstructs in the AAHLS were 6.22 (SD 1.34) for functional health literacy, 5.22 (SD 1.54) for communicative health literacy, and 11.19 (SD 1.99) for critical health literacy. The mean score for eHealth literacy was 24.21 (SD 5.49). The mean score for the 6 questions in the GHNT-6 was 1.57 (SD 0.49), 1.21 (SD 0.41), 1.24 (SD 0.43), 1.90 (SD 0.30), 1.82 (SD 0.39), and 1.73 (SD 0.44), respectively. The mean score for the patients' health beliefs and self-confidence was 21.19 (SD 5.63). The mean score for their response to each myth ranged from 1.24 (SD 0.43) to 1.67 (SD 0.47), and the mean score for responses to the 10 myths was 14.03 (SD 1.78). Through interpreting these descriptive statistics, we found that Chinese female patients' limited ability to rebut breast cancer misinformation is mainly attributed to 5 factors: (1) lower communicative health literacy, (2) certainty about self-assessed eHealth literacy skills, (3) lower general health numeracy, (4) positive self-assessment of general disease knowledge, and (5) more negative health beliefs and lower levels of self-confidence. Drawing on logistic regression modeling, we studied the susceptibility to breast cancer misinformation among Chinese patients. The predicting factors of the susceptibility to breast cancer misinformation identified in this study can provide insightful implications for clinical practice, health education, medical research, and health policy making.

Abstract P1-06-06: Access to medical coverage in newly diagnosed Breast cancer patients a minority population during the COVID 19 pandemic

Abstract Background: Hispanics are among the most significant minorities in the United States, and assessing their health needs is vital in formulating health policies. The social, environmental, and biological background affect their morbidity and mortality, with cancer being the leading cause of mortality in Hispanics. Although implementing the Affordable Care Act improved access to health care, disparities and challenges in Hispanics’ access to health care remain. (1) In our study, we are trying to assess the COVID-19 pandemic implications on the socioeconomic status of Hispanics with newly diagnosed breast cancer and how that is affecting their outcome. Methods: This is a retrospective study demonstrating the nature of breast cancer screening, diagnosis, and management during COVID-19 lockdown and post-vaccine compared to the pre-COVID-19 era. The restrospective review was conducted at Mays Cancer Center in San Antonio, IRB approved by UT Health San Antonio. The data were abstracted from the medical record system (EPIC). Pearson’s Chi-squared and logistic regression tests were used to determine the relationship between time and stage at diagnosis and association with funding status Result: 696 subjects with newly diagnosed breast cancer were identified and divided into three cohorts: Cohort A: 264 pts in a pre-covid era: From 2018-February 2020, Cohort B: 171 patients during the covid-19 lockdown, and Cohort C: from April 2020-Dec-2020 with 261 patients in the post-vaccine era. A higher percentage of patients had Tis identified in Cohort A (26%) compared to Cohort B (19%) and Cohort C(12%) (p0.003). These numbers have changed to an increasing percentage of patients with T1 or T2. N0 or N1in Cohort B (62%) and Chohort C (68%), p0.003). Presentation of locally advanced (T3/T4 with any N) and metastatic disease remained relatively similar in all cohorts, with more metastatic cases at presentation in Cohort A (p0.003). Many patients of Hispanic ethnicity (OR 1.47, p0.1) were not funded by commercial insurance or Medicare. (OR 2.33, 95% CI 1.43, 3.77, p&amp;lt; 0.001). A high proportion of Hispanic ethnicity in post covid era (cohort B and C) OR 1.78, 95% CI (1.16,2.79) (p0.009) required neoadjuvant chemotherapy suggesting more advanced and possibly aggressive cancer histology. We observed that unfunded patients are less likely to have hormonal receptor-positive breast cancer (p0.001). Conclusion: Population health is likely affected by socioeconomic status, which eventually affects their health care. In a minority Hispanic population, low average income and educational attainment are likely the obstacles to getting timely appropriate health.(2) A higher proportion of Hispanic patients were not insured when compared to non-Hispanics leading possibly to more advanced and aggressive histologies at presentation.

An Analysis of the Pattern of Farmer’s Income in Gopalganj District: An Empirical Investigation

Aims: Although the income of the people of other sectors in the economy is relatively stable but the income of farmers are comparatively unstable. In addition, most of the previous studies analyzed partially the determinants of farmer’s income.Thus, the aim of the current study is to investigate the pattern of farmers’ income in Gopalganj district of Bangladesh.&#x0D; Place and Duration of Study: The present study was conducted in Gopalganj district of Bangladesh where 10 unions, 20 villages and 400 respondents were selected randomly. Data were collected during the period from January to May in 2022.&#x0D; Methodology: A multistage sampling technique was applied for the study where district and upazilas were selected purposively while unions, villages and respondents were selected using simple random sampling techniques. Primary data were collected from 400 farmers using a well-structured questionnaire. Descriptive statistics and the log linear regression model were used as the analytical tools.&#x0D; Results: Result found from the descriptive statistics indicates that mean age of the respondents was 49.02 years whereas average educational attainment of the farmers in the study area was 5.72. In regards to the farming experience, it is found that average farming experience of the farmer was 5.07 years with maximum and minimum values were 25 years and 3 years, respectively. Regarding the household size of the respondents, the study showed that the mean family member was 4.82 with a standard deviation of 1.02. The study also indicates that 35.12% average income of the farmer obtained from non-farm sources while 64.88% average income obtained from farm sources.In regards to the farm income, it is found from the regression analysis that farm income is positively related with age, household size, education, farm size, agricultural training, access to credit facilities, membership of agricultural cooperative and distance to nearest market whereas the same is influenced by non-farm income. The findings suggest that variables like household size, education, household expenditure, access to internet and access to technical and vocational training has positive impact on non-farm income. In contrast, variables like age, farm size, distance from town, farm income have negative impact on non-farm income of the respondents in the study area. Finally, some policy recommendations are made based on the findings obtained from the study.

Development of social convoys: Trajectories of convoy structure and composition from childhood through adulthood.

Personal networks undergo changes in structure and composition throughout the life span, adapting to developmental transitions and changing circumstances in a dynamic way. This study examines stability and change in social convoys from childhood to adulthood and variation in trajectories of convoy characteristics by gender and race. Multilevel models for convoy structure and composition characteristics were estimated using three waves of longitudinal data spanning 23 years. The regionally representative sample included 193 children aged 8 to 12 in Wave 1 (1992) who were surveyed again in their 20s (Wave 2) and 30s (Wave 3). The Wave 1 sample comprised 52% girls, 32% Black children, and 59% White children with average maternal educational attainment of 13 years. Overall, changes in composition, proximity, and contact frequency were observed at each wave. Between Waves 1 and 2, the changes reflect age-normative trends toward network diversification typical of the transition to adulthood, whereas between Waves 2 and 3, the changes were consistent with those expected as young adults settle into stable roles. We also identified convoy characteristics that differed between men and women and between Black and White individuals, emphasizing the importance of considering personal characteristics to fully understand form and function of social relations. Social convoy trajectories early in the life span provide direction for more in-depth examinations of the implications of social ties during these critical life periods. Determinants of Livelihood Diversification among the Thadou-Kukis of Manipur, India

This paper examined livelihood diversifications among the Thadou-Kuki tribes of Manipur using the Simpson index and also finds out the determinant of livelihood diversification using multiple regression. The study uses household-level data collected from a census survey of 15 villages scattered across three Thadou-Kuki dominated districts. The mean Simpson Diversity Index (SDI) value is 0.42 that indicates most of the households diversify moderately and livelihood diversifications are mainly for meeting their subsistence need rather than for accumulation. The multiple regression results show that the distance of the village from the market, number of farm household members, number of non-farm household members, wet-land area, and access to forest resources positively impacts the livelihood diversification, and average educational attainment of workers and cash cropland negatively impacts the livelihood diversification

Fringe Benefits, Self-Selection, and the Public/Private Compensation Differential

Across developing countries, many workers strive for public sector jobs. Is this because public sector workers receive higher compensation than comparable private sector workers? This study addresses this question looking at broad measures of compensation that encompass wages as well as non-wage fringe benefits such as paid leave and access to social insurances. Using detailed labour force data for Bhutan, it combines Oaxaca type decompositions of compensation differentials into characteristics and coefficients effects with multinomial logit models for self-selection into labour force participation and the public or private sector. The study finds that public/private wage differentials are sizeable but can entirely be accounted for by observable characteristics (in particular by differences in the average educational attainment and industrial structures). A sizeable public/private differential in fringe benefits related to leave can also be accounted for by differences in observable characteristics between public and private sector workers. However, this is not the case for the differential in non-leave-related fringe benefits. Instead, differentials in non-leave-related fringe benefits are largely due to coefficients effects, strongly suggesting that preferences for public sector jobs are at least in part the result of pronounced intersectoral differentials in such benefits.

The burden of ischaemic heart disease in Brazil from 1990 to 2019 and the association between temporal changes and socioeconomic level

Abstract Introduction The burden of ischaemic heart disease (IHD) has declined in several countries, although IHD remains the leading cause of death globally. Brazil is a large country with high inequality in income distribution across different regions. Purpose This study sought to evaluate the burden of IHD in Brazil from 1990 to 2019, as well as the relationship between temporal changes and socioeconomic level of the Federative Units. Methods Estimates of prevalence, mortality, and disability-adjusted life-years (DALYs) due to IHD were retrieved from the Global Burden of Disease Study 2019. We used databases from the Ministry of Health of Brazil to obtain the number of hospitalisations from acute coronary syndrome (ACS) and acute myocardial infarction (AMI) in the context of the public service. The Socio-demographic Index (SDI), a composite indicator of income per capita, average educational attainment, and fertility rate in females younger than 25 years, was used as a measure of socioeconomic status. Rates with 95% uncertainty intervals are reported. Results In 2019, the age-standardised prevalence rate of IHD was 1,709 (1,466–1,994) per 100,000 inhabitants. This rate remained stable from 1990 to 2019 (percent change: −1.1 [−2.6–0.5]). The estimate of deaths from IHD in 2019 was 171,246 (156,180–180,511), corresponding to 12% (11%-13%) of total deaths in the country (the leading cause) and 43% of all cardiovascular deaths. In 2019, the age-standardised mortality rate due to IHD was 75 (68–79) per 100,000. The unadjusted mortality rate due to IHD mildly increased from 2005 to 2019, while the age-standardised rate continuously decreased from 1990 to 2019 (cumulative percent change: −53 (−55 to −50, Figure 1). A negative correlation was observed between the change in age-standardised mortality rate from IHD in the period and the 2019 SDI (Figure 2). In 2019, the age-standardised DALY rate due to IHD was 1,563 (1,472–1,636) per 100,000. This DALY rate was equivalent to 5.7% (5.1%-6.3%) of total DALYs, meaning that IHD was the second most common cause of DALYs in Brazil in females and males. From 1990 to 2019, the crude DALY rate per 100,000 remained fairly stable, and the age- standardised DALY rate per 100,000 gradually declined (−50% [−52% to −48%]). There was a negative correlation between the change in age-standardised DALY rate from IHD in the period and the SDI (r2 0.59, p-value &amp;lt;0.01). The number of hospital admissions for ACS remained stable from 2008 to 2019 (33 per 100,000 in 2019). The number of hospitalisations due to AMI increased from 25 per 100,000 in 2008 to 39 per 100,000 in 2019 (percent change: 52%). Conclusions While age-standardised mortality, DALY rates continuously decreased from 1990 to 2019, the burden of IHD in Brazil remains high, probably due to ageing and population growth. Global, regional, and national mortality among young people aged 10–24 years, 1950–2019: a systematic analysis for the Global Burden of Disease Study 2019

SummaryBackgroundDocumentation of patterns and long-term trends in mortality in young people, which reflect huge changes in demographic and social determinants of adolescent health, enables identification of global investment priorities for this age group. We aimed to analyse data on the number of deaths, years of life lost, and mortality rates by sex and age group in people aged 10–24 years in 204 countries and territories from 1950 to 2019 by use of estimates from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019.MethodsWe report trends in estimated total numbers of deaths and mortality rate per 100 000 population in young people aged 10–24 years by age group (10–14 years, 15–19 years, and 20–24 years) and sex in 204 countries and territories between 1950 and 2019 for all causes, and between 1980 and 2019 by cause of death. We analyse variation in outcomes by region, age group, and sex, and compare annual rate of change in mortality in young people aged 10–24 years with that in children aged 0–9 years from 1990 to 2019. We then analyse the association between mortality in people aged 10–24 years and socioeconomic development using the GBD Socio-demographic Index (SDI), a composite measure based on average national educational attainment in people older than 15 years, total fertility rate in people younger than 25 years, and income per capita. We assess the association between SDI and all-cause mortality in 2019, and analyse the ratio of observed to expected mortality by SDI using the most recent available data release (2017).FindingsIn 2019 there were 1·49 million deaths (95% uncertainty interval 1·39–1·59) worldwide in people aged 10–24 years, of which 61% occurred in males. 32·7% of all adolescent deaths were due to transport injuries, unintentional injuries, or interpersonal violence and conflict; 32·1% were due to communicable, nutritional, or maternal causes; 27·0% were due to non-communicable diseases; and 8·2% were due to self-harm. Since 1950, deaths in this age group decreased by 30·0% in females and 15·3% in males, and sex-based differences in mortality rate have widened in most regions of the world. Geographical variation has also increased, particularly in people aged 10–14 years. Since 1980, communicable and maternal causes of death have decreased sharply as a proportion of total deaths in most GBD super-regions, but remain some of the most common causes in sub-Saharan Africa and south Asia, where more than half of all adolescent deaths occur. Annual percentage decrease in all-cause mortality rate since 1990 in adolescents aged 15–19 years was 1·3% in males and 1·6% in females, almost half that of males aged 1–4 years (2·4%), and around a third less than in females aged 1–4 years (2·5%). The proportion of global deaths in people aged 0–24 years that occurred in people aged 10–24 years more than doubled between 1950 and 2019, from 9·5% to 21·6%.InterpretationVariation in adolescent mortality between countries and by sex is widening, driven by poor progress in reducing deaths in males and older adolescents. Improving global adolescent mortality will require action to address the specific vulnerabilities of this age group, which are being overlooked. Furthermore, indirect effects of the COVID-19 pandemic are likely to jeopardise efforts to improve health outcomes including mortality in young people aged 10–24 years. Relationship of Coping Strategies and Quality of Life: Parallel and Serial Mediating Role of Resilience and Social Participation among Older Adults in Western Philippines.

Growing old is frequently linked with various challenges. Hence, it is important to understand how to age successfully. Drawing on the concept that an individual’s quality of life (QOL) is influenced by their demographics, coping strategies, resilience, and social participation, the current study reports on the findings of these variables among older adults in the Western Philippines. A total of 392 volunteer older adults were surveyed. Aside from the demographics such as age, gender, marital status, average monthly income, educational attainment, and health status, the data collected also included the 30-item coping strategies for the elderly, 14-item resilience scale, 12-item social participation scale, and 35-item older people’s QOL scale. Structural equation modeling was used to verify the parallel and serial mediating role of resilience and social participation within the relationship between coping strategies and QOL. Findings show that coping strategies alone are not enough to improve QOL. The only way to improve QOL is through resiliency, as well as the ability to participate in social activities. In essence, the QOL of older adults can be improved by encouraging them to have more social participation, and at the same time, understand how it takes to become resilient.
