Clinical effectiveness of small needle knife in autologous fat grafting for treatment of atrophic scar

Objective To explore the clinical effect of using small needle knife in autologous fat grafting for the treatment of atrophic scar. Methods Twelve patients with atrophic scar were included in this study during March 2014 and December 2016 in the Hangzhou First People＇s Hospital, Zhejiang University School of Medicine. We used small needle knife during fat grafting to treat the atrophic scar. Observer Scar Assessment Scale (POSAS) was used to assess the clinical effect of the patients. Results Aesthetic and functional improvements were observed. Resolution of pain and increases in scar elasticity were objectively assessable. Improvement of both clinical evaluation and patient perception was obtained. Conclusions Use of small needle knife during fat grafting is a good alternative for the treatment of atrophic scars. The Use of Autologous Fat Grafting for Treatment of Scar Tissue and Scar-Related Conditions: A Systematic Review.

Scar tissue can cause cosmetic impairments, functional limitations, pain, and itch. It may also cause emotional, social, and behavioral problems, especially when it is located in exposed areas. To date, no gold standard exists for the treatment of scar tissue. Autologous fat grafting has been introduced as a promising treatment option for scar tissue-related symptoms. However, the scientific evidence for its effectiveness remains unclear. This systematic review aims to evaluate the available evidence regarding the effectiveness of autologous fat grafting for the treatment of scar tissue and scar-related conditions. A systematic literature review was performed using MEDLINE, Cochrane Library, EMBASE, and Web of Science. No language restrictions were imposed. Twenty-six clinical articles were included, reporting on 905 patients in total. Meta-analysis was not performed because of the heterogeneous methodology demonstrated among the articles. Main outcome measures were scar appearance and skin characteristics, restoration of volume and/or (three-dimensional) contour, itch, and pain. All publications report a beneficial effect of autologous fat grafting on scar tissue. There is statistical significant improvement of the scar appearance, skin characteristics, and pain. Itch and restoration of volume and three-dimensional contour also improved. Autologous fat grafting is used to improve a variety of symptoms related to scar tissue. This systematic review suggests that autologous fat grafting provides beneficial effects with limited side effects. However, the level of evidence and methodological quality are quite low. Future randomized controlled trials with a methodologically strong design are necessary to confirm the effects of autologous fat grafting on scar tissue and scar-related conditions.

Is ASJ Still a “Clinical Journal”?

It was with some trepidation that, at a recent meeting of the Aesthetic Surgery Journal ( ASJ ) Editorial Board, I asked how our editors felt about the inclusion of bench research—aka “rat studies”—in the Journal. This was by no means a hypothetical question. As I am sure you have noticed, we already do publish select bench research in the pages of ASJ . For example, in this issue, we have an excellent article from Brazil, “Autologous Fat Grafting for Treatment of Breast Implant Capsular Contracture: A Study in Pigs.”1 There are a number of other laboratory research articles published already, or scheduled for publication, in 2014. Several of these studies deal with preparation of and applications for autologous fat; this is an area of research already suggesting significant clinical benefits, with the promise of more to come.2,3 I was aware, however, of certain minor “rumblings” among Board members—concerns that ASJ might be in danger of losing its focus as a “clinical journal.” That was why I wanted to give each member an opportunity to voice his or her opinion on the subject in an open forum. As we went around the boardroom table, I was relieved to find that everyone could agree on one thing: bench research may not always be the most entertaining reading experience, but it does have a place in ASJ . Autologous fat grafting for treatment of facial atrophy in Behcet's disease: A case report

Behcet's disease (BD) is an autoimmune & autoinflammatory disease of unclear etiology characterized by recurrent oral & genital ulcers as well as other systemic manifestations. A key pathogenesis is excessive inflammatory wound healing response. While descriptions of the cutaneous manifestations of disease are limited to short-term consequences such as extensive pustule and papule formation in response to minor tissue injury, the long-term consequences are significant fibrosis and scarring of epithelial tissue. We describe the case of a patient with Behcet's disease who presented with unilateral facial atrophy secondary to minor trauma to the oral mucosa. She was treated with autologous fat grafting. Though a rare disease, plastic surgeons should be aware of the entity of Behcet's disease and its complications of tissue atrophy that may require reconstructive surgery.
