Deep Learning-Enabled Quantification of 99mTc-Pyrophosphate SPECT/CT for Cardiac Amyloidosis.

Transthyretin cardiac amyloidosis (ATTR CA) is increasingly recognized as a cause of heart failure in older patients, with 99mTc-pyrophosphate imaging frequently used to establish the diagnosis. Visual interpretation of SPECT images is the gold standard for interpretation but is inherently subjective. Manual quantitation of SPECT myocardial 99mTc-pyrophosphate activity is time-consuming and not performed clinically. We evaluated a deep learning approach for fully automated volumetric quantitation of 99mTc-pyrophosphate using segmentation of coregistered anatomic structures from CT attenuation maps. Methods: Patients who underwent SPECT/CT 99mTc-pyrophosphate imaging for suspected ATTR CA were included. Diagnosis of ATTR CA was determined using standard criteria. Cardiac chambers and myocardium were segmented from CT attenuation maps using a foundational deep learning model and then applied to attenuation-corrected SPECT images to quantify radiotracer activity. We evaluated the diagnostic accuracy of target-to-background ratio (TBR), cardiac pyrophosphate activity (CPA), and volume of involvement (VOI) using the area under the receiver operating characteristic curve (AUC). We then evaluated associations with the composite outcome of cardiovascular death or heart failure hospitalization. Results: In total, 299 patients were included (median age, 76 y), with ATTR CA diagnosed in 83 (27.8%) patients. CPA (AUC, 0.989; 95% CI, 0.974-1.00) and VOI (AUC, 0.988; 95% CI, 0.973-1.00) had the highest prediction performance for ATTR CA. The next highest AUC was for TBR (AUC, 0.979; 95% CI, 0.964-0.995). The AUC for CPA was significantly higher than that for heart-to-contralateral ratio (AUC, 0.975; 95% CI, 0.952-0.998; P = 0.046). Twenty-three patients with ATTR CA experienced cardiovascular death or heart failure hospitalization. All methods for establishing TBR, CPA, and VOI were associated with an increased risk of events after adjustment for age, with hazard ratios ranging from 1.41 to 1.84 per SD increase. Conclusion: Deep learning segmentation of coregistered CT attenuation maps is not affected by the pattern of radiotracer uptake and allows for fully automatic quantification of hot-spot SPECT imaging such as 99mTc-pyrophosphate. This approach can be used to accurately identify patients with ATTR CA and may play a role in risk prediction.

Generative adversarial network-based attenuation correction for 99mTc-TRODAT-1 brain SPECT.

Attenuation correction (AC) is an important correction method to improve the quantification accuracy of dopamine transporter (DAT) single photon emission computed tomography (SPECT). Chang's method was developed for AC (Chang-AC) when CT-based AC was not available, assuming uniform attenuation coefficients inside the body contour. This study aims to evaluate Chang-AC and different deep learning (DL)-based AC approaches on 99mTc-TRODAT-1 brain SPECT using clinical patient data on two different scanners. Two hundred and sixty patients who underwent 99mTc-TRODAT-1 SPECT/CT scans from two different scanners (scanner A and scanner B) were retrospectively recruited. The ordered-subset expectation-maximization (OS-EM) method reconstructed 120 projections with dual-energy scatter correction, with or without CT-AC. We implemented a 3D conditional generative adversarial network (cGAN) for the indirect deep learning-based attenuation correction (DL-ACμ) and direct deep learning-based attenuation correction (DL-AC) methods, estimating attenuation maps (μ-maps) and attenuation-corrected SPECT images from non-attenuation-corrected (NAC) SPECT, respectively. We further applied cross-scanner training (cross-scanner indirect deep learning-based attenuation correction [cull-ACμ] and cross-scanner direct deep learning-based attenuation correction [call-AC]) and merged the datasets from two scanners for ensemble training (ensemble indirect deep learning-based attenuation correction [eDL-ACμ] and ensemble direct deep learning-based attenuation correction [eDL-AC]). The estimated μ-maps from (c/e)DL-ACμ were then used in reconstruction for AC purposes. Chang's method was also implemented for comparison. Normalized mean square error (NMSE), structural similarity index (SSIM), specific uptake ratio (SUR), and asymmetry index (%ASI) of the striatum were calculated for different AC methods. The NMSE for Chang's method, DL-ACμ, DL-AC, cDL-ACμ, cDL-AC, eDL-ACμ, and eDL-AC is 0.0406 ± 0.0445, 0.0059 ± 0.0035, 0.0099 ± 0.0066, 0.0253 ± 0.0102, 0.0369 ± 0.0124, 0.0098 ± 0.0035, and 0.0162 ± 0.0118 for scanner A and 0.0579 ± 0.0146, 0.0055 ± 0.0034, 0.0063 ± 0.0028, 0.0235 ± 0.0085, 0.0349 ± 0.0086, 0.0115 ± 0.0062, and 0.0117 ± 0.0038 for scanner B, respectively. The SUR and %ASI results for DL-ACμ are closer to CT-AC, Followed by DL-AC, eDL-ACμ, cDL-ACμ, cDL-AC, eDL-AC, Chang's method, and NAC. All DL-based AC methods are superior to Chang-AC. DL-ACμ is superior to DL-AC. Scanner-specific training is superior to cross-scanner and ensemble training. DL-based AC methods are feasible and robust for 99mTc-TRODAT-1 brain SPECT.

DuDoSS: Deep-learning-based dual-domain sinogram synthesis from sparsely sampled projections of cardiac SPECT.

Myocardial perfusion imaging (MPI) using single-photon emission-computed tomography (SPECT) is widely applied for the diagnosis of cardiovascular diseases. In clinical practice, the long scanning procedures and acquisition time might induce patient anxiety and discomfort, motion artifacts, and misalignments between SPECT and computed tomography (CT). Reducing the number of projection angles provides a solution that results in a shorter scanning time. However, fewer projection angles might cause lower reconstruction accuracy, higher noise level, and reconstruction artifacts due to reduced angular sampling. We developed a deep-learning-based approach for high-quality SPECT image reconstruction using sparsely sampled projections. We proposed a novel deep-learning-based dual-domain sinogram synthesis (DuDoSS) method to recover full-view projections from sparsely sampled projections of cardiac SPECT. DuDoSS utilized the SPECT images predicted in the image domain as guidance to generate synthetic full-view projections in the sinogram domain. The synthetic projections were then reconstructed into non-attenuation-corrected and attenuation-corrected (AC) SPECT images for voxel-wise and segment-wise quantitative evaluations in terms of normalized mean square error (NMSE) and absolute percent error (APE). Previous deep-learning-based approaches, including direct sinogram generation (Direct Sino2Sino) and direct image prediction (Direct Img2Img), were tested in this study for comparison. The dataset used in this study included a total of 500 anonymized clinical stress-state MPI studies acquired on a GE NM/CT 850 scanner with 60 projection angles following the injection of 99m Tc-tetrofosmin. Our proposed DuDoSS generated more consistent synthetic projections and SPECT images with the ground truth than other approaches. The average voxel-wise NMSE between the synthetic projections by DuDoSS and the ground-truth full-view projections was 2.08%±0.81%, as compared to 2.21%±0.86% (p<0.001) by Direct Sino2Sino. The averaged voxel-wise NMSE between the AC SPECT images by DuDoSS and the ground-truth AC SPECT images was 1.63%±0.72%, as compared to 1.84%±0.79% (p<0.001) by Direct Sino2Sino and 1.90%±0.66% (p<0.001) by Direct Img2Img. The averaged segment-wise APE between the AC SPECT images by DuDoSS and the ground-truth AC SPECT images was 3.87%±3.23%, as compared to 3.95%±3.21% (p=0.023) by Direct Img2Img and 4.46%±3.58% (p<0.001) by Direct Sino2Sino. Our proposed DuDoSS is feasible to generate accurate synthetic full-view projections from sparsely sampled projections for cardiac SPECT. The synthetic projections and reconstructed SPECT images generated from DuDoSS are more consistent with the ground-truth full-view projections and SPECT images than other approaches. DuDoSS can potentially enable fast data acquisition of cardiac SPECT.

Data Management and Network Architecture Effect on Performance Variability in Direct Attenuation Correction via Deep Learning for Cardiac SPECT: A Feasibility Study.

Attenuation correction (AC) is important for accurate interpretation of SPECT myocardial perfusion imaging (MPI). However, it is challenging to perform AC in dedicated cardiac systems not equipped with a transmission imaging capability. Previously, we demonstrated the feasibility of generating attenuation-corrected SPECT images using a deep learning technique (SPECTDL) directly from non-corrected images (SPECTNC). However, we observed performance variability across patients which is an important factor for clinical translation of the technique. In this study, we investigate the feasibility of overcoming the performance variability across patients for the direct AC in SPECT MPI by proposing to develop an advanced network and a data management strategy. To investigate, we compared the accuracy of the SPECTDL for the conventional U-Net and Wasserstein cycle GAN (WCycleGAN) networks. To manage the training data, clustering was applied to a representation of data in the lower-dimensional space, and the training data were chosen based on the similarity of data in this space. Quantitative analysis demonstrated that DL model with an advanced network improves the global performance for the AC task with the limited data. However, the regional results were not improved. The proposed data management strategy demonstrated that the clustered training has potential benefit for effective training.

Deep learning-guided attenuation correction in the image domain for myocardial perfusion SPECT imaging

Abstract We investigate the accuracy of direct attenuation correction (AC) in the image domain for myocardial perfusion SPECT (single-photon emission computed tomography) imaging (MPI-SPECT) using residual (ResNet) and UNet deep convolutional neural networks. MPI-SPECT 99mTc-sestamibi images of 99 patients were retrospectively included. UNet and ResNet networks were trained using non-attenuation-corrected SPECT images as input, whereas CT-based attenuation-corrected (CT-AC) SPECT images served as reference. Chang’s calculated AC approach considering a uniform attenuation coefficient within the body contour was also implemented. Clinical and quantitative evaluations of the proposed methods were performed considering SPECT CT-AC images of 19 subjects (external validation set) as reference. Image-derived metrics, including the voxel-wise mean error (ME), mean absolute error, relative error, structural similarity index (SSI), and peak signal-to-noise ratio, as well as clinical relevant indices, such as total perfusion deficit (TPD), were utilized. Overall, AC SPECT images generated using the deep learning networks exhibited good agreement with SPECT CT-AC images, substantially outperforming Chang’s method. The ResNet and UNet models resulted in an ME of −6.99 ± 16.72 and −4.41 ± 11.8 and an SSI of 0.99 ± 0.04 and 0.98 ± 0.05, respectively. Chang’s approach led to ME and SSI of 25.52 ± 33.98 and 0.93 ± 0.09, respectively. Similarly, the clinical evaluation revealed a mean TPD of 12.78 ± 9.22% and 12.57 ± 8.93% for ResNet and UNet models, respectively, compared to 12.84 ± 8.63% obtained from SPECT CT-AC images. Conversely, Chang’s approach led to a mean TPD of 16.68 ± 11.24%. The deep learning AC methods have the potential to achieve reliable AC in MPI-SPECT imaging.

Preparing for the Artificial Intelligence Revolution in Nuclear Cardiology.

A major opportunity in nuclear cardiology is the many significant artificial intelligence (AI) applications that have recently been reported. These developments include using deep learning (DL) for reducing the needed injected dose and acquisition time in perfusion acquisitions also due to DL improvements in image reconstruction and filtering, SPECT attenuation correction using DL without need for transmission images, DL and machine learning (ML) use for feature extraction to define myocardial left ventricular (LV) borders for functional measurements and improved detection of the LV valve plane and AI, ML, and DL implementations for MPI diagnosis, prognosis, and structured reporting. Although some have, most of these applications have yet to make it to widespread commercial distribution due to the recency of their developments, most reported in 2020. We must be prepared both technically and socio-economically to fully benefit from these and a tsunami of other AI applications that are coming.

Direct and indirect strategies of deep-learning-based attenuation correction for general purpose and dedicated cardiac SPECT.

Deep-learning-based attenuation correction (AC) for SPECT includes both indirect and direct approaches. Indirect approaches generate attenuation maps (μ-maps) from emission images, while direct approaches predict AC images directly from non-attenuation-corrected (NAC) images without μ-maps. For dedicated cardiac SPECT scanners with CZT detectors, indirect approaches are challenging due to the limited field-of-view (FOV). In this work, we aim to 1) first develop novel indirect approaches to improve the AC performance for dedicated SPECT; and 2) compare the AC performance between direct and indirect approaches for both general purpose and dedicated SPECT. For dedicated SPECT, we developed strategies to predict truncated μ-maps from NAC images reconstructed with a small matrix, or full μ-maps from NAC images reconstructed with a large matrix using 270 anonymized clinical studies scanned on a GE Discovery NM/CT 570c SPECT/CT. For general purpose SPECT, we implemented direct and indirect approaches using 400 anonymized clinical studies scanned on a GE NM/CT 850c SPECT/CT. NAC images in both photopeak and scatter windows were input to predict μ-maps or AC images. For dedicated SPECT, the averaged normalized mean square error (NMSE) using our proposed strategies with full μ-maps was 1.20 ± 0.72% as compared to 2.21 ± 1.17% using the previous direct approaches. The polar map absolute percent error (APE) using our approaches was 3.24 ± 2.79% (R2 = 0.9499) as compared to 4.77 ± 3.96% (R2 = 0.9213) using direct approaches. For general purpose SPECT, the averaged NMSE of the predicted AC images using the direct approaches was 2.57 ± 1.06% as compared to 1.37 ± 1.16% using the indirect approaches. We developed strategies of generating μ-maps for dedicated cardiac SPECT with small FOV. For both general purpose and dedicated SPECT, indirect approaches showed superior performance of AC than direct approaches.

Dr. Eiichi Tanaka's Achievement in the Research of Image Reconstruction

In memoriam of Dr. Eiichi Tanaka who passed away on August 21, 2021, I review his achievement in the research of tomographic image reconstruction. Tomographic image reconstruction is an important research area which has wide applications including X-ray CT, nuclear medicine imaging such as PET and SPECT, and electron microscopy. Since 1970's, Dr. Tanaka has worked on numerous important topics in tomographic reconstruction fields aiming at using them in image reconstruction for PET and SPECT. Among them, in this paper, I will introduce his research on Filtered BackProjection (FBP) method, analytical attenuation correction in SPECT, image reconstruction in Time-of-Flight PET, image reconstruction for 3-D PET imaging, and iterative image reconstruction method called Dynamic Row-Action Maximum Likelihood Algorithm (DRAMA).

Does Single Computed Tomography Attenuation Correction able to Surrogate Serial Computed Tomography Attenuation Correction in Single-Photon Emission Computed Therapy Imaging for Peptide Receptor Radionuclide Therapy Dosimetry Result?

Peptide Receptor Radionuclide Therapy (PRRT) is the administration of a radionuclide, such as the Lu-177 label, along with a pharmaceutical agent to destroy the lesion cell. The first cycle of Lu-177 DOTA-TATE is an excellent way to estimate radionuclide uptake for organs at risk. To estimate the absorbed dose for a kidney, serial SPECT-CT imaging with up to five sets is required. In general, serial CT on patients would result in additional external exposure and extra time consuming, especially for low dose CT such as cone beam CT technology. However, by introducing a new method, such as optimized single CT (24 h) to perform with serial SPECT attenuation correction, additional external exposure from serial CT exposure could be reduced.Aims:The purpose of this study is to assess the agreement between single CT scanning as CT attenuation Correction with serial CT scanning for serial SPECT imaging as well as estimated absorbed dose to the organ at risk.Settings and Design:During the first cycle of Lu177-DOTA-TATE, all patients should undergo internal dosimetry technique using SPECT-CT imaging with a PHILIP Brightview XCT gamma camera. The quantifications of SPECT images are being used to measure the uptake activity to the organ. In this investigation, Partik's categorical grading criteria are being used to convert the numeric value of Lin's concordance coefficient into an ordinal scale.Method:Total of 9 patients at our institution was administered with Lu177-DOTA-TATE enroll in this study. SPECT-CT data were acquired using automatic body contouring with a total of 48 angular views at steps of 6° (15 s/projections). The pixel size is 4.66 x 4.66 x 4.66 mm, the images size set to one, and SPECT data has been acquired for three-bed positions extending from the abdomen to the thorax region. Low-dose CT imaging performed at an X-ray tube voltage of 120 kVp.Results:Our current result demonstrated by single CT scanning for SPECT attenuation shown the excellent agreement with standard serial CT imaging for organs at risk such as kidney 0.999, spleen 0.9951, liver 0.9951 and bladder 0.9972.Conclusions:When compared to the previous method, patients benefit significantly more from this study, such as lower CT exposure.

Noise heterogeneity in attenuation-corrected cardiac SPECT images increases perfusion value uncertainty near the base of the heart

BackgroundDedicated cardiac SPECT cameras which employ multi-pinhole detectors have variable photon sensitivity within the camera’s field-of-view such that a lower number of photon counts is typically detected from the base of the heart than from the apex. Consequently, the noise in a reconstructed image is expected to be higher at the base than at the apex of the heart. MethodsPatient emission images were resampled to create statistical replicates which were reconstructed with and without attenuation correction. Noise images were computed using one standard deviation of the replicated images. These were evaluated for 93 patients with normal study results, each imaged with both a dual-headed parallel-hole camera and a multi-pinhole camera. Statistics for a normal database (NDB) of images from the 93 patients were also calculated. ResultsImage noise (1.7-fold) and NDB uncertainty (1.3-fold) increase significantly from the apex-to-the base of the heart in attenuation-corrected multi-pinhole SPECT images. The differences for non-attenuation-corrected images or those acquired with a parallel-hole camera were not significant. ConclusionsFor best interpretation of attenuation-corrected images acquired with multi-pinhole cameras, knowledge of NDB uncertainty gradients should be taken into consideration.

Attenuation correction using deep learning for brain perfusion SPECT images.

Non-uniform attenuation correction using computed tomography (CT) improves the image quality and quantification of single-photon emission computed tomography (SPECT). However, it is not widely used because it requires a SPECT/CT scanner. This study constructs a convolutional neural network (CNN) to generate attenuation-corrected SPECT images directly from non-attenuation-corrected SPECT images. We constructed an auto-encoder (AE) using a CNN to correct the attenuation in brain perfusion SPECT images. SPECT image datasets of 270 (44,528 slices including augmentation), 60 (5002 slices), and 30 (2558 slices) cases were used for training, validation, and testing, respectively. The acquired projection data were reconstructed in three patterns: uniform attenuation correction using Chang's method (Chang-AC), non-uniform attenuation correction using CT (CT-AC), and no attenuation correction (No-AC). The AE learned an end-to-end mapping between the No-AC and CT-AC images. The No-AC images in the test dataset were loaded into the trained AE, which generated images simulating the CT-AC images as output. The generated SPECT images were employed as attenuation-corrected images using the AE (AE-AC). The accuracy of the AE-AC images was evaluated in terms of the peak signal-to-noise ratio (PSNR) and the structural similarity metric (SSIM). The intensities of the AE-AC and CT-AC images were compared by voxel-by-voxel and region-by-region analysis. The PSNRs of the AE-AC and Chang-AC images, compared using CT-AC images, were 62.2, and 57.9, and their SSIM values were 0.9995 and 0.9985, respectively. The AE-AC and CT-AC images were visually and statistically in good agreement. The proposed AE-AC method yields highly accurate attenuation-corrected brain perfusion SPECT images.

Automatic attenuation map estimation from SPECT data only for brain perfusion scans using convolutional neural networks

In clinical brain SPECT, correction for photon attenuation in the patient is essential to obtain images which provide quantitative information on the regional activity concentration per unit volume (kBq.). This correction generally requires an attenuation map ( map) denoting the attenuation coefficient at each voxel which is often derived from a CT or MRI scan. However, such an additional scan is not always available and the method may suffer from registration errors. Therefore, we propose a SPECT-only-based strategy for map estimation that we apply to a stationary multi-pinhole clinical SPECT system (G-SPECT-I) for 99mTc-HMPAO brain perfusion imaging. The method is based on the use of a convolutional neural network (CNN) and was validated with Monte Carlo simulated scans. Data acquired in list mode was used to employ the energy information of both primary and scattered photons to obtain information about the tissue attenuation as much as possible. Multiple SPECT reconstructions were performed from different energy windows over a large energy range. Locally extracted 4D SPECT patches (three spatial plus one energy dimension) were used as input for the CNN which was trained to predict the attenuation coefficient of the corresponding central voxel of the patch. Results show that Attenuation Correction using the Ground Truth maps (GT-AC) or using the CNN estimated maps (CNN-AC) achieve comparable accuracy. This was confirmed by a visual assessment as well as a quantitative comparison; the mean deviation from the GT-AC when using the CNN-AC is within 1.8% for the standardized uptake values in all brain regions. Therefore, our results indicate that a CNN-based method can be an automatic and accurate tool for SPECT attenuation correction that is independent of attenuation data from other imaging modalities or human interpretations about head contours.

Direct Attenuation Correction Using Deep Learning for Cardiac SPECT: A Feasibility Study.

Dedicated cardiac SPECT scanners with cadmium-zinc-telluride cameras have shown capabilities for shortened scan times or reduced radiation doses, as well as improved image quality. Since most dedicated scanners do not have integrated CT, image quantification with attenuation correction (AC) is challenging and artifacts are routinely encountered in daily clinical practice. In this work, we demonstrated a direct AC technique using deep learning (DL) for myocardial perfusion imaging (MPI). Methods: In an institutional review board-approved retrospective study, 100 cardiac SPECT/CT datasets with 99mTc-tetrofosmin, obtained using a scanner specifically with a small field of view, were collected at the Yale New Haven Hospital. A convolutional neural network was used for generating DL-based attenuation-corrected SPECT (SPECTDL) directly from noncorrected SPECT (SPECTNC) without undergoing an additional image reconstruction step. The accuracy of SPECTDL was evaluated by voxelwise and segmentwise analyses against the reference, CT-based AC (SPECTCTAC), using the 17-segment myocardial model of the American Heart Association. Polar maps of representative (best, median, and worst) cases were visually compared to illustrate potential benefits and pitfalls of the DL approach. Results: The voxelwise correlations with SPECTCTAC were 92.2% ± 3.7% (slope, 0.87; R2 = 0.81) and 97.7% ± 1.8% (slope, 0.94; R2 = 0.91) for SPECTNC and SPECTDL, respectively. The segmental errors of SPECTNC scattered from -35% to 21% (P < 0.001), whereas the errors of SPECTDL stayed mostly within ±10% (P < 0.001). The average segmental errors (mean ± SD) were -6.11% ± 8.06% and 0.49% ± 4.35% for SPECTNC and SPECTDL, respectively. The average absolute segmental errors were 7.96% ± 6.23% and 3.31% ± 2.87% for SPECTNC and SPECTDL, respectively. Review of polar maps revealed successful reduction of attenuation artifacts; however, the performance of SPECTDL was not consistent for all subjects, likely because of different amounts of attenuation and different uptake patterns. Conclusion: We demonstrated the feasibility of direct AC using DL for SPECT MPI. Overall, our DL approach reduced attenuation artifacts substantially compared with SPECTNC, justifying further studies to establish safety and consistency for clinical applications in stand-alone SPECT systems suffering from attenuation artifacts.

Quantitative imaging of bone remodeling in patients with a unicompartmental joint unloading knee implant (ATLAS Knee System)\u2014effect of metal artifacts on a SPECT-CT-based quantification

BackgroundSPECT-CT using radiolabeled phosphonates is considered a standard for assessing bone metabolism (e.g., in patients with osteoarthritis of knee joints). However, SPECT can be influenced by metal artifacts in CT caused by endoprostheses affecting attenuation correction. The current study examined the effects of metal artifacts in CT of a specific endoprosthesis design on quantitative hybrid SPECT-CT imaging.The implant was positioned inside a phantom homogenously filled with activity (955 MBq 99mTc). CT imaging was performed for different X-ray tube currents (I = 10, 40, 125 mA) and table pitches (p = 0.562 and 1.375). X-ray tube voltage (U = 120 kVp) and primary collimation (16 × 0.625 mm) were kept constant for all scans. The CT reconstruction was performed with five different reconstruction kernels (slice thickness, 1.25 mm and 3.75 mm, each 512 × 512 matrix). Effects from metal artifacts were analyzed for different CT scans and reconstruction protocols. ROI analysis of CT and SPECT data was performed for two slice positions/volumes representing the typical locations for target structures relative to the prosthesis (e.g., femur and tibia). A reference region (homogenous activity concentration without influence from metal artifacts) was analyzed for comparison.ResultsSignificant effects caused by CT metal artifacts on attenuation-corrected SPECT were observed for the different slice positions, reconstructed slice thicknesses of CT data, and pitch and CT-reconstruction kernels used (all, p < 0.0001). Based on the optimization, a set of three protocols was identified minimizing the effect of CT metal artifacts on SPECT data. Regarding the reference region, the activity concentration in the anatomically correlated volume was underestimated by 8.9–10.1%. A slight inhomogeneity of the reconstructed activity concentration was detected inside the regions with a median up to 0.81% (p < 0.0001). Using an X-ray tube current of 40 mA showed the best result, balancing quantification and CT exposure.ConclusionThe results of this study demonstrate the need for the evaluation of SPECT-CT protocols in prosthesis imaging. Phantom experiments demonstrated the possibility for quantitative SPECT-CT of bone turnover in a specific prosthesis design. Meanwhile, a systematic bias caused by metal implants on quantitative SPECT data has to be considered.

Direct Image-Based Attenuation Correction using Conditional Generative Adversarial Network for SPECT Myocardial Perfusion Imaging.

Attenuation correction (AC) is important for an accurate interpretation and quantitative analysis of SPECT myocardial perfusion imaging. Dedicated cardiac SPECT systems have invaluable efficacy in the evaluation and risk stratification of patients with known or suspected cardiovascular disease. However, most dedicated cardiac SPECT systems are standalone, not combined with a transmission imaging capability such as computed tomography (CT) for generating attenuation maps for AC. To address this problem, we propose to apply a conditional generative adversarial network (cGAN) for generating attenuation-corrected SPECT images (SPECTGAN ) directly from non-corrected SPECT images (SPECTNC ) in image domain as a one-step process without requiring additional intermediate step. The proposed network was trained and tested for 100 cardiac SPECT/CT data from a GE Discovery NM 570c SPECT/CT, collected retrospectively at Yale New Haven Hospital.The generated images were evaluated quantitatively through the normalized root mean square error (NRMSE), peak signal to noise ratio (PSNR), and structural similarity index (SSIM) and statistically through joint histogram and error maps. In comparison to the reference CT-based correction (SPECTCTAC ), NRMSEs were 0.2258±0.0777 and 0.1410±0.0768 (37.5% reduction of errors); PSNRs 31.7712±2.9965 and 36.3823±3.7424 (14.5% improvement in signal to noise ratio); SSIMs 0.9877±0.0075 and 0.9949±0.0043 (0.7% improvement in structural similarity) for SPECTNC and SPECTGAN , respectively. This work demonstrates that the conditional adversarial training can achieve accurate CT-less attenuation correction for SPECT MPI, that is quantitatively comparable to CTAC. Standalone dedicated cardiac SPECT scanners can benefit from the proposed GAN to reduce attenuation artifacts efficiently.

Randomized Comparison of Clinical Effectiveness of Pharmacologic SPECT and PET MPI in Symptomatic CAD Patients

ObjectivesThis study compared the clinical effectiveness of pharmacologic stress myocardial perfusion imaging (MPI) plus positron emission tomography (PET) with single-photon emission computed tomography (SPECT) in patients with known coronary artery disease (CAD) presenting with symptoms suggestive of ischemia. BackgroundAlthough PET MPI has been shown to have higher diagnostic accuracy in detecting hemodynamically significant CAD than SPECT MPI, whether this impacts downstream management has not been formally evaluated in randomized trials. MethodsThis study consisted of a single-center trial in which patients with known CAD and suspected ischemia were randomized to undergo PET or attenuation-corrected SPECT MPI between June 2009 and September 2013. Post-test management was at the discretion of the referring physician, and patients were followed for 12 months. The primary endpoint was diagnostic failure, defined as unnecessary angiography (absence of ≥50% stenosis in ≥1 vessel) or additional noninvasive testing within 60 days of the MPI. Secondary endpoints were post-test escalation of antianginal therapy, referral for angiography, coronary revascularization, and health status at 3, 6, and 12 months. ResultsA total of 322 patients with an evaluable MPI were randomized (n = 161 in each group). At baseline, 88.8% of patients were receiving aspirin therapy, 76.7% were taking beta-blockers, and 77.3% were taking statin therapy. Diagnostic failure within 60 days occurred in only 7 patients (2.2%) (3 [1.9%] in the PET group and 4 [2.5%] in the SPECT group; p = 0.70). There were no significant differences between the 2 groups in subsequent rates of coronary angiography, coronary revascularization, or health status at 3, 6, and 12 months of follow-up (all p values ≥0.20); however, when subjects were stratified by findings on MPI in a post hoc analysis, those with high-risk MPI on PET testing had higher rates of angiography and revascularization on follow-up than those who had SPECT MPI, whereas those undergoing low-risk PET studies had lower rates of both procedures than those undergoing SPECT (interaction between randomized modality ∗high-risk MPI for 12-month catheterization [p = 0.001] and 12-month revascularization [p = 0.09]). ConclusionsIn this contemporary cohort of symptomatic CAD patients who were optimally medically managed, there were no discernible differences in rates of diagnostic failure at 60 days, subsequent coronary angiography, revascularization, or patient health status at 1 year between patients evaluated by pharmacologic PET compared with those evaluated by SPECT MPI. Downstream invasive testing rates with PET MPI were more consistent with high-risk features than those with SPECT MPI. Model Image-Based Metal Artifact Reduction for Computed Tomography.

Metal implants often produce severe artifacts in the reconstructed computed tomography (CT) images, causing information and image detail loss and making the CT images diagnostically unusable. In order to eliminate the metal artifacts and enhance the diagnostic value of the reconstructed CT images, a post-processing metal artifact reduction algorithm, based on a tissue-class model segmented by thresholding and k-means clustering with spatial information, is proposed. The image inpainting technique is incorporated into the algorithm to improve the segmentation accuracy for CT images severely corrupted by metal artifacts. A study of a water phantom and of two sets of clinical CT images was performed to test the algorithm performance. The proposed method effectively eliminates typical metal artifacts, restores the average CT numbers of different tissues to the proper levels, and preserves the edge and contrast information, thus allowing the accurate reconstruction of the tissue attenuation map. The quality of the artifact-corrected CT images allows them to be subsequently used in other clinical applications, such as three-dimensional rendering, dose estimation for radiotherapy, attenuation correction for PET and SPECT, etc. The algorithm does not rely on the use of the raw sinogram and so is not limited by the proprietary format restrictions.

Improved diagnosis of the number of stenosed coronary artery vessels by segmentation with scatter and photo-peak window data for attenuation correction in myocardial perfusion SPECT

BackgroundAttenuation correction using segmentation of scatter and photo-peak window data (SSPAC) enables an evaluation of the attenuation map in a patient-specific manner without additional radiation exposure. We compared the accuracy of SSPAC and non-corrected myocardial perfusion scintigraphy methods for diagnosing the number of stenosed coronary artery vessels. Methods and ResultsWe retrospectively reviewed the data from 183 consecutive patients who underwent 99mTc-tetrofosmin stress/rest SPECT examination and a coronary angiography within 3 months. The MPS images were reconstructed with and without SSPAC attenuation correction. We examined the accuracy of the quantitative interpretation using summed differential score in the detection of coronary artery disease (CAD). The attenuation maps were successfully determined in 179 of 183 patients (98%). In terms of the vessel-based diagnostic ability, sensitivity, specificity, positive predictive and negative predictive values of the SSPAC and non-correction methods for diagnosing CAD in individual coronary territories were 77%*, 89%, 74%*, and 90%* vs 51%, 87%, 62%, and 82%, respectively (*P < .05). In 35 patients with multi-vessel CAD, those values were 78%*, 81%, 93%, and 55%* vs 49%, 81%, 89%, and 34%, respectively (*P < .05; AUC: 0.82 vs 0.62, P < .05). ConclusionSSPAC-corrected SPECT myocardial perfusion images exhibit improved accuracy in the detection of the number of stenosed coronary artery vessels, even in patients with multi-vessel CAD.
