Which patient and doctor behaviours make a medical consultation more effective from a patient point of view. Results from a European multicentre study in 31 countries

ObjectiveTo assess European patients' preferences regarding seven aspects of doctor-patient communication. Methods6049 patients from 31 European countries evaluated 21 doctor and 12 patient behaviours, through a patient-generated questionnaire (PCVq). Multilevel models explored the effects of patient characteristics, contextual and cultural dimensions on preferences. ResultsPatients attributed more responsibility to doctors, by giving greater importance to doctor than to patient factors, in particular to Treating the patient as a partner and as a person and Continuity of care.Gender, age, education, the presence of chronic illness and two of Hofstede’s cultural dimensions, Individualism and Indulgence, showed differential evaluations among patients. Women gave greater importance to all seven communication aspects, older patients to being prepared for the consultation, lower educated patients to Treating patient as a person and Thoughtful planning. Patients from countries with an indulgent background rated all seven communication aspects of greater importance. A more individualistic orientation was related to lower importance regarding the four doctor's factors and the patient factor Open and Honest. ConclusionsTreating the patient as a person and providing continuity of care emerged as universal values. Practice implicationsThe findings should represent a landmark for the adaptation of patient-generated communication guidelines and programs in Europe.

Using Conversation Topics for Predicting Therapy Outcomes in Schizophrenia

Previous research shows that aspects of doctor-patient communication in therapy can predict patient symptoms, satisfaction and future adherence to treatment (a significant problem with conditions such as schizophrenia). However, automatic prediction has so far shown success only when based on low-level lexical features, and it is unclear how well these can generalize to new data, or whether their effectiveness is due to their capturing aspects of style, structure or content. Here, we examine the use of topic as a higher-level measure of content, more likely to generalize and to have more explanatory power. Investigations show that while topics predict some important factors such as patient satisfaction and ratings of therapy quality, they lack the full predictive power of lower-level features. For some factors, unsupervised methods produce models comparable to manual annotation.

Doctor-patient communication in radiology: a great opportunity for future radiology

This text discusses several aspects of doctor-patient communication in radiology, including the origins, advantages, and ethical and legal aspects. Over the last 10 years, radiologists have assumed increasing responsibilities towards patients and society. Patients, who are becoming better informed and more aware about medical issues, have a right to be given a timely diagnosis and want to receive as much information as possible from the radiologist. This has implications for several levels of everyday radiological practice, including the organisation of work, legal and ethical aspects and radiologist training. Better interaction with the patient helps to build a closer, more trusting, relationship with the result that the radiologist will be more motivated in his or her work. Until now, radiologists were not adequately trained to communicate the diagnosis directly and verbally to patients, especially when the diagnosis was unfavourable. It is important to emphasise the need for more specific and practical training in this respect, which is indispensable for future developments of the discipline.

The impact of discussion of non-medical problems in the physician's office.

To determine the impact of discussion of non-medical problems with the physician, patients with at least one chronic illness who were taking medication were interviewed in their family doctor's office. Of 149 patients interviewed, 90.6% reported at least one non-medical problem. Half (51%) of those patients with a problem had discussed it with their doctor. More than half (55%) of those patients discussing a problem reported that the discussion was helpful. Patient compliance and satisfaction were positively associated with 'helpful' discussion, but not with discussion per se. In particular, the two aspects of doctor-patient communication which were significantly associated with feeling helped were: 'Doctor tells me all I want to know about my illness' and 'Doctor gives me a chance to say what is really on my mind'. Both factors reflect care which is oriented to patient concerns. The study results provide support for a patient-centred approach to care.
