The involvement of clinicians in the most highly cited publications on artificial intelligence in ophthalmology indexed journals

PurposeSignificant advances in artificial intelligence (AI) have led to promising applications in ophthalmology. This study highlights the involvement of clinicians in the most cited ophthalmology publications on AI in ophthalmology journals indexed by Web of Science. MethodsArticles examining AI in ophthalmology journals were processed from Web of Science. After selecting relevant articles, we performed bibliometric analyses at the article and author levels as of March 2024. The primary outcome measure was the number of citations per article. Secondary outcomes included article measures (publication year, subspecialties, article type, databases, imaging) and author attributes (gender, academic metrics, location). ResultsThe top 100 publications were cited between 58 and 734 times, with a median of 91 citations. Publication reprint addresses were mainly based in America (44) and in Europe (22). Common subspecialties were retina (60), glaucoma (44) and cornea (18). Most imaging modalities were fundus photography (47), optical coherence tomography (47) and visual fields (19). 76 studies were aimed at the development and evaluation of a diagnostic technology. Some private databases (44 %) and public databases (40 %) were specified. Among the 399 men and 163 women authors, 297 were physicians (52.9 %). Women and men had significantly different h-indexes (women: 23 [interquartile range (IQR): 13–46] vs. men: 38.5 [17–65]; P = 0.02) and number of published documents (women: 104 [32–277] vs. men: 188.5 [63.5–394]; P = 0.03). ConclusionThe most influential articles in AI and ophthalmology by number of citations predominantly used AI for image recognition and improving diagnostic technology in retina followed by glaucoma. Physicians had a predominant role in these, highlighting the continued importance of clinician involvement in this research.

Design of an Iterative Method for Enhanced Retinal Image Analysis Using Stacked Deep Learning Operations

The necessity for advanced diagnostic techniques in ophthalmology has become increasingly evident, particularly for conditions such as glaucoma and diabetic retinopathy, which necessitate precise retinal image analysis. Current methodologies, while effective to a degree, fall short in terms of accuracy, efficiency, and adaptability, especially in semantic segmentation and disease classification tasks. These limitations underscore the need for more sophisticated and reliable approaches to retinal image analysis. In response, this work introduces a comprehensive framework leveraging Fully Convolutional Neural Networks (FCNNs) for optic disc segmentation and Cup-to-Disc Ratio (CDR) estimation. The selection of FCNNs is predicated on their demonstrated proficiency in semantic segmentation, ability to discern spatial dependencies, and capacity to learn intricate structures within fundus images without relying on manually engineered features. This approach aims to surpass existing segmentation accuracy benchmarks, targeting over 95% Intersection over Union (IoU) while reducing the mean absolute error in CDR estimation to below 10%. To address the challenges of data variability and quality, the framework incorporates Contrast Limited Adaptive Histogram Equalization (CLAHE) for synthetic data generation, enhancing local contrast while preserving image integrity levels. This method is expected to improve contrast in augmented images by 30%, simultaneously enhancing image quality metrics. Furthermore, the introduction of an overlapping sliding window technique with adaptive patch sizes ensures meticulous coverage and analysis of fundus images, significantly elevating lesion detection sensitivity and reducing false positives. Lastly, the framework employs an innovative multi-disease classification strategy utilizing ensemble learning and stacking of CNN architectures. This synergistic approach amalgamates multiple base models to diminish overfitting risks and enhance generalization capabilities, aiming for a classification accuracy surpassing 95% in binary assessments and 85% for specific retinal diseases. The proposed model not only addresses the current gaps in retinal image analysis but also sets a new standard for precision, reliability, and efficiency in diagnosing and managing ocular diseases, marking a significant step forward in the application of artificial intelligence in ophthalmology.

Advancements and turning point of artificial intelligence in ophthalmology: A comprehensive analysis of research trends and collaborative networks.

Artificial intelligence (AI) has emerged as a transformative force with great potential in various fields, including healthcare. In recent years, AI has garnered significant attention due to its potential to revolutionise ophthalmology, leading to advancements in patient care such as disease detection, diagnosis, treatment and monitoring of disease progression. This study presents a comprehensive analysis of the research trends and collaborative networks at the intersection of AI and ophthalmology. In this study, we conducted an extensive search of the Web of Science Core Collection to identify articles related to 'artificial intelligence' in ophthalmology published from 1968 to 2023. We performed co-occurrence keywords and co-authorship network analyses using VOSviewer software to explore the relationships between keywords and country collaboration. We found a remarkable surge in articles applying AI in ophthalmology after 2017, marking a turning point in the integration of AI within the medical field. The primary application of AI shifted towards the diagnosis of ocular disease, which was particularly evident through keywords such as glaucoma, diabetic retinopathy and age-related macular degeneration. Analysis of the collaboration networks of countries revealed a global expansion of ophthalmology-related AI research. This study provides valuable insights into the evolving landscape of AI integration in ophthalmology, indicating its growing potential for enhancing disease detection, diagnosis, treatment planning and monitoring of disease progression. In order to translate AI technologies into clinical practice effectively, it is imperative to comprehend the evolving research trends and advancements at the intersection of AI and ophthalmology.

Dr. Google vs. Dr. ChatGPT: Exploring the Use of Artificial Intelligence in Ophthalmology by Comparing the Accuracy, Safety, and Readability of Responses to Frequently Asked Patient Questions Regarding Cataracts and Cataract Surgery

ABSTRACT Purpose Patients are using online search modalities to learn about their eye health. While Google remains the most popular search engine, the use of large language models (LLMs) like ChatGPT has increased. Cataract surgery is the most common surgical procedure in the US, and there is limited data on the quality of online information that populates after searches related to cataract surgery on search engines such as Google and LLM platforms such as ChatGPT. We identified the most common patient frequently asked questions (FAQs) about cataracts and cataract surgery and evaluated the accuracy, safety, and readability of the answers to these questions provided by both Google and ChatGPT. We demonstrated the utility of ChatGPT in writing notes and creating patient education materials. Methods The top 20 FAQs related to cataracts and cataract surgery were recorded from Google. Responses to the questions provided by Google and ChatGPT were evaluated by a panel of ophthalmologists for accuracy and safety. Evaluators were also asked to distinguish between Google and LLM chatbot answers. Five validated readability indices were used to assess the readability of responses. ChatGPT was instructed to generate operative notes, post-operative instructions, and customizable patient education materials according to specific readability criteria. Results Responses to 20 patient FAQs generated by ChatGPT were significantly longer and written at a higher reading level than responses provided by Google (p < .001), with an average grade level of 14.8 (college level). Expert reviewers were correctly able to distinguish between a human-reviewed and chatbot generated response an average of 31% of the time. Google answers contained incorrect or inappropriate material 27% of the time, compared with 6% of LLM generated answers (p < .001). When expert reviewers were asked to compare the responses directly, chatbot responses were favored (66%). Conclusions When comparing the responses to patients’ cataract FAQs provided by ChatGPT and Google, practicing ophthalmologists overwhelming preferred ChatGPT responses. LLM chatbot responses were less likely to contain inaccurate information. ChatGPT represents a viable information source for eye health for patients with higher health literacy. ChatGPT may also be used by ophthalmologists to create customizable patient education materials for patients with varying health literacy.

In ophthalmology, the use of artificial intelligence is very promising, given that the diagnosis and therapeutic monitoring of eye diseases often depend heavily on the correct interpretation of images. The use of artificial intelligence in ophthalmology focuses on eye diseases that lead to vision loss, such as age-related macular degeneration, diabetic retinopathy, glaucoma and cataract. Over the past few years, artificial intelligence has reached tremendous successes in the practice of ophthalmology. Many studies have shown that artificial intelligence performance is equal to and even exceeds the capabilities of ophthalmologists in many diagnostic and prognostic tasks. However, there is still a lot of work to be done before introducing artificial intelligence into routine clinical practice. Issues such as real-world performance, generalizability, and interpretability of artificial intelligence systems are still poorly understood and will require more attention in future research. Most artificial intelligence-based systems are used in developed countries, and some require further study. High costs and a shortage in doctors and equipment in some regions of the Russian Federation and rural areas make it difficult to screen for eye diseases. Although the field of artificial intelligence is underdeveloped, we hope that artificial intelligence will play an important role in the future of ophthalmology by making healthcare more efficient, accurate and accessible, especially in regions where staffing problems exist.

Impact of Artificial Intelligence on Diagnosing Eye Diseases – A Meta-analysis

The application of artificial intelligence (AI) in the field of ophthalmology has garnered significant attention for its potential to enhance the accuracy of eye disease diagnosis. This systematic review and meta-analysis aimed to comprehensively assess the impact of AI on diagnosing eye diseases through the synthesis of existing research. A systematic search of electronic databases was conducted to identify relevant studies in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses protocol. Eligible studies were those that reported the diagnostic accuracy of AI in ophthalmic image diagnosis. The standardized mean difference (SMD) and mean difference (MD) were utilised as the effect size measures to evaluate AI performance. A total of 18 studies meeting the inclusion criteria were selected for the quantitative synthesis. Further, the meta-analysis revealed that AI exhibited a substantial positive impact on the accuracy of diagnosing eye diseases. The overall SMD across various diagnostic parameters indicated a statistically significant improvement (SMD = 0.88, 95% confidence interval [CI]: 0.71–1.05). Moreover, the MD of diagnostic values demonstrated significant enhancements, with an overall MD of −10.2 (95% CI: −12.1 to −8.3). The selected studies consistently demonstrated that AI achieved high accuracy levels, reinforcing its potential as a valuable tool in ophthalmic diagnosis. This study provides significant evidence supporting the significant positive impact of AI on diagnosing eye diseases. The synthesis of the selected studies underscores the high accuracy achieved by AI in ophthalmic image diagnosis, as indicated by the substantial SMD and MD improvements. These findings highlight the promising role of AI in ophthalmology, offering the potential to revolutionise the field and improve patient care through enhanced diagnostic precision.

The goal of this review is to detail how AI can be utilized in making diagnostic predictions to enhance the clinical setting. It is crucial to keep improving methods that emphasize clarity in AI models. This makes it possible to evaluate the information obtained from ocular imaging and easily incorporate it into therapeutic decision-making procedures. This will contribute to the wider acceptance and adoption of AI-based ocular imaging in healthcare settings combining advanced machine learning and deep learning techniques with new developments. Multiple studies were reviewed and evaluated, including AI-based algorithms, retinal images, fundus and optic nerve head (ONH) photographs, and extensive expert reviews. In these studies, carried out in various countries and laboratories of the world, it is seen those complex diagnoses, which can be detected systemic diseases from ophthalmological images, can be made much faster and with higher predictability, accuracy, sensitivity, and specificity, in addition to ophthalmological diseases, by comparing large numbers of images and teaching them to the computer. It is now clear that it can be taken advantage of AI to achieve diagnostic certainty. Collaboration between the fields of medicine and engineering foresees promising advances in improving the predictive accuracy and precision of future medical diagnoses achieved by training machines with this information. However, it is important to keep in mind that each new development requires new additions or updates to various social, psychological, ethical, and legal regulations.

Application of artificial intelligence in ophthalmology for solving the problem of semantic segmentation of fundus images

The paper presents main aspects of the application of artificial intelligence in ophthalmology for the diagnosis and treatment of eye diseases, considering the problem of semantic segmentation of fundus images as an example. The classic approach to semantic segmentation on the basis of textural features is compared to the proposed approach based on neural networks. Basic problems of using the neural network approach in biomedicine are formulated. We propose a new method for selecting an optimal zone of laser exposure for laser coagulation based on two neural networks. The first network is used for detecting anatomical objects in the fundus and the second one is used for selecting the area of macular edema. The region of interest is formed from the edema area while taking into account the location of anatomical objects in it. A comparative analysis of sev-eral architectures of neural networks for solving the problem of selecting the edema area is carried out. The best results in the selection of the edema area are shown by the neural network architecture of Unet++.

Guidelines on clinical research evaluation of artificial intelligence in ophthalmology (2023).

With the upsurge of artificial intelligence (AI) technology in the medical field, its application in ophthalmology has become a cutting-edge research field. Notably, machine learning techniques have shown remarkable achievements in diagnosing, intervening, and predicting ophthalmic diseases. To meet the requirements of clinical research and fit the actual progress of clinical diagnosis and treatment of ophthalmic AI, the Ophthalmic Imaging and Intelligent Medicine Branch and the Intelligent Medicine Committee of Chinese Medicine Education Association organized experts to integrate recent evaluation reports of clinical AI research at home and abroad and formed a guideline on clinical research evaluation of AI in ophthalmology after several rounds of discussion and modification. The main content includes the background and method of developing this guideline, an introduction to international guidelines on the clinical research evaluation of AI, and the evaluation methods of clinical ophthalmic AI models. This guideline introduces general evaluation methods of clinical ophthalmic AI research, evaluation methods of clinical ophthalmic AI models, and commonly-used indices and formulae for clinical ophthalmic AI model evaluation in detail, and amply elaborates the evaluation methods of clinical ophthalmic AI trials. This guideline aims to provide guidance and norms for clinical researchers of ophthalmic AI, promote the development of regularization and standardization, and further improve the overall level of clinical ophthalmic AI research evaluations.

Recent evidence of economic evaluation of artificial intelligence in ophthalmology.

Health economic evaluation (HEE) is essential for assessing value of health interventions, including artificial intelligence. Recent approaches, current challenges, and future directions of HEE of artificial intelligence in ophthalmology are reviewed. Majority of recent HEEs of artificial intelligence in ophthalmology were for diabetic retinopathy screening. Two models, one conducted in the rural USA (5-year period) and another in China (35-year period), found artificial intelligence to be more cost-effective than without screening for diabetic retinopathy. Two additional models, which compared artificial intelligence with human screeners in Brazil and Thailand for the lifetime of patients, found artificial intelligence to be more expensive from a healthcare system perspective. In the Thailand analysis, however, artificial intelligence was less expensive when opportunity loss from blindness was included. An artificial intelligence model for screening retinopathy of prematurity was cost-effective in the USA. A model for screening age-related macular degeneration in Japan and another for primary angle close in China did not find artificial intelligence to be cost-effective, compared with no screening. The costs of artificial intelligence varied widely in these models. Like other medical fields, there is limited evidence in assessing the value of artificial intelligence in ophthalmology and more appropriate HEE models are needed.

DR is a common complication of Diabetes Mellitus (DM) that affects the eyes, and early detection and treatment is crucial in preventing vision loss, especially over the working populations [3]. The use of AI in DR screening involves analyzing digital fundus images to detect any signs of DR, including microaneurysms, dot and blot haemorrhages, cotton-wool spots, venous beading, intraretinal microvascular anomalies [4]. Currently, there are 3 US FDA approved systems on DR screening, IDx-DR (Digital Diagnostics), EyeArt (Eyenuk, Inc) and AEYE-DS (AEYE Health, Inc) [5-7]. AI has several advantages over traditional screening methods, including usage by non-ophthalmologically trained medical personnel, accessible and stable performance over whatever time and place, increased speed of clinical workflow, these might potentially help to improve outcomes for patients with DM. In the era of rapidly advancing technology, the use of AI in ophthalmology is an exciting development that might transform our future practice [8]. In this appraisal, we focus on the 3rd US FDA approved AI algorithms for DR.

Explainable artificial intelligence in ophthalmology.

Despite the growing scope of artificial intelligence (AI) and deep learning (DL) applications in the field of ophthalmology, most have yet to reach clinical adoption. Beyond model performance metrics, there has been an increasing emphasis on the need for explainability of proposed DL models. Several explainable AI (XAI) methods have been proposed, and increasingly applied in ophthalmological DL applications, predominantly in medical imaging analysis tasks. We summarize an overview of the key concepts, and categorize some examples of commonly employed XAI methods. Specific to ophthalmology, we explore XAI from a clinical perspective, in enhancing end-user trust, assisting clinical management, and uncovering new insights. We finally discuss its limitations and future directions to strengthen XAI for application to clinical practice.

Artificial Intelligence in Ophthalmology: A Comparative Analysis of GPT-3.5, GPT-4, and Human Expertise in Answering StatPearls Questions.

Importance Chat Generative Pre-Trained Transformer (ChatGPT) has shown promising performance in various fields, including medicine, business, and law, but its accuracy in specialty-specific medical questions, particularly in ophthalmology, is still uncertain. Purpose This study evaluates the performance of two ChatGPT models (GPT-3.5 and GPT-4) and human professionals in answering ophthalmology questions from the StatPearls question bank, assessing their outcomes, and providing insights into the integration of artificial intelligence (AI) technology in ophthalmology. Methods ChatGPT's performance was evaluated using 467 ophthalmology questions from the StatPearls question bank. These questions were stratified into 11 subcategories, four difficulty levels, and three generalized anatomical categories. The answer accuracy of GPT-3.5, GPT-4, and human participants was assessed. Statistical analysis was conducted via the Kolmogorov-Smirnov test for normality, one-way analysis of variance (ANOVA) for the statistical significance of GPT-3 versus GPT-4 versus human performance, and repeated unpaired two-sample t-tests to compare the means of two groups. Results GPT-4 outperformed both GPT-3.5 and human professionals on ophthalmology StatPearls questions, except in the "Lens and Cataract" category. The performance differences were statistically significant overall, with GPT-4 achieving higher accuracy (73.2%) compared to GPT-3.5 (55.5%, p-value < 0.001) and humans (58.3%, p-value < 0.001). There were variations in performance across difficulty levels (rated one to four), but GPT-4 consistently performed better than both GPT-3.5 and humans on level-two, -three, and -four questions. On questions of level-four difficulty, human performance significantly exceeded that of GPT-3.5 (p = 0.008). Conclusion The study's findings demonstrate GPT-4's significant performance improvements over GPT-3.5 and human professionals on StatPearls ophthalmology questions. Our results highlight the potential of advanced conversational AI systems to be utilized as important tools in the education and practice of medicine.

Artificial intelligence in ophthalmology. Do we need risk calculators for glaucoma development and progression?

Artificial intelligence (AI) is rapidly entering modern medical practice. Many routine clinical tasks, from imaging and automated diagnostics to robotic surgery, cannot be imagined without the use of AI. Neural networks show impressive results when analyzing a large amount of data obtained from standard automated perimetry, optical coherence tomography (OCT) and fundus photography. Currently, both in Russia and abroad mathematical algorithms are being developed that allow detection of glaucoma based on certain signs. This article analyzes the advantages and disadvantages of employing artificial intelligence in ophthalmological practice, discusses the need for careful selection of the criteria and their influence on the accuracy of calculators, considers the specifics of using mathematical analysis in suspected glaucoma, as well as in an already established diagnosis. The article also provides clinical examples of the use of glaucoma risk calculator in the routine practice of an ophthalmologist.

Tracking Online Interest in Artificial Intelligence in Ophthalmology Using Google Trends

ABSTRACT Purpose To examine trends in internet search queries related to artificial intelligence (AI) in ophthalmology and determine the correlation between online interest in AI, capital investment in AI, and peer-reviewed indexed publications regarding AI and ophthalmology. Methods Online search trends for “AI retina”, “AI eye”, and “AI healthcare” were obtained via Google Trends from 2016 to 2022 on a relative interest scale in 1-week intervals. Global venture financing of AI- and machine learning (ML)-focused companies in healthcare was tracked from 2010 to 2019 from the consulting company, Klynveld Peat Marwick Goerdeler (KPMG), and the technology market intelligence company, CB Insights. Citation count from pubmed.gov was determined using the search query “artificial intelligence retina” from 2012 to 2021. Results An increasingly linear growth in online search trends for “AI retina”, “AI eye”, and “AI healthcare” keyword searches was observed between 2016 and 2022. Global venture financing of AI and ML companies in healthcare also increased exponentially over the same time frame. There was an exponential increase in citations with nearly a 10-fold increase as reported by PubMed from 2015 onwards for the “artificial intelligence retina” search query. There was a significant and positive correlation between online search trends and investment trends (correlation coefficients of 0.98–0.99 and p-values <0.05) and between online search trends and citation count trends (correlation coefficients of 0.98–0.99 and p-values <0.05). Conclusions These results demonstrate that the applications of AI and ML in ophthalmology are increasingly being investigated, financed, and formally researched, suggesting a prominent role for AI-derived tools in ophthalmology clinical practice in the near future.
