Quick Arthroscopic Repair of Ulnar-Sided Triangular Fibrocartilage Complex Tears: Technical Note.

Triangular fibrocartilage complex (TFCC) tears can cause ulnar-sided wrist pain. Arthroscopy is important in treatment and diagnosis, and arthroscopic repair of TFCC tears is indicated after failure of nonsurgical treatments such as cast immobilization, splinting, and administration of nonsteroidal anti-inflammatory drugs for more than 3 months. Several arthroscopic procedures have been described, including inside-out, outside-in, and all-arthroscopic techniques. However, these arthroscopic procedures are time-consuming and technically demanding. This article presents a straightforward technique of arthroscopic inside-out repair that uses double-loop sutures for ulnar-sided TFCC tears.

Second look arthroscopic evaluation of repaired radial/oblique tears of the midbody of the lateral meniscus in stable knees.

Radial/oblique tears of the midbody of the lateral meniscus significantly impair the ability of the meniscus to withstand the tibiofemoral load, requiring meniscal repair. However, healing status after meniscal repair has not been fully elucidated. This study aimed to evaluate arthroscopic findings after inside-out suture repair for isolated radial/oblique tears of the midbody of the lateral meniscus. From 2011 to 2015, 18 consecutive patients with isolated radial/oblique tears of the midbody of the lateral meniscus underwent arthroscopic inside-out repair with the tie-grip suture technique. All knees were stable with no previous surgery. All patients were evaluated by second-look arthroscopy at six months postoperatively. Activities including jogging were not allowed until meniscal status was evaluated arthroscopically. To analyze factors associated with healing rates, age, time from injury to initial surgery, and tear zone were compared. Second-look arthroscopy revealed complete healing in four (22%) patients, partial healing in seven (39%), and failure to heal in seven (39%). Significant differences were observed for tear zone (p<0.0001), but not for age and timing of repair. Arthroscopic evaluation revealed that inside-out repair with the tie-grip suture technique for isolated radial/oblique tears of the midbody of the lateral meniscus achieved complete or partial healing only in 61% of patients. Satisfactory results were observed particularly in patients with tears extending to the vascular zone, whereas those with tears in the avascular zone failed to achieve healing. Therefore, the operative indication of inside-out repair for radial/oblique tears of the midbody of the lateral meniscus might be limited to tears extending into the vascular zone. Given that 39% of cases were arthroscopically considered a failure even if patients complained of no symptoms in daily life, decisions should be made carefully to allow patients to return to sports activities.

Arthroscopic Inside-Out Repair of a Meniscus Bucket-Handle Tear Augmented With Bone Marrow Aspirate Concentrate

Bucket-handle tears of the meniscus comprise nearly 10% of all meniscus tears and commonly affect the young male population. Displacement of the free segment can lead to significant pain and disability, necessitating reduction and surgical treatment. General contraindications include malalignment, severe arthritis, significant comorbidities, or chronic asymptomatic tears, but otherwise repair should almost always be performed. Options for surgical treatment include partial meniscectomy and arthroscopic repair using an all-inside, outside-in, or inside-out technique. The purpose of this Technical Note is to detail our arthroscopic inside-out repair technique augmented with bone marrow aspirate concentrate.

Inside-Out Repair of Meniscal Ramp Lesions

Meniscal ramp lesions have been reported to be present in 9% to 17% of patients undergoing anterior cruciate ligament reconstruction. Detection at the time of arthroscopy can be accomplished based upon clinical suspicion and careful evaluation without the use of an accessory posteromedial portal. Options for surgical treatment include arthroscopic repair using an all-inside or inside-out technique. The purpose of this Technical Note is to detail our arthroscopic inside-out repair technique for meniscal ramp lesions.

Clinical outcome of meniscus repair for isolated meniscus tear in athletes

ObjectiveTo examine the clinical and functional outcomes for a series of patients who underwent meniscal repair for isolated meniscal tears focusing the study population on athletes. MethodsThis study represents a case series of 46 athletes who underwent repair of isolated meniscal lesions of the knee from 2010 to 2015. Cases of discoid meniscal lesions and combined ligament injuries were excluded. The mean age of the patients was 22.9 years ranging from 12 to 50 years. Arthroscopic inside-out repair was primarily a procedure of option. For repair of tears with degeneration and inferior vascularity, autogenous fibrin clot was implanted to the repair site for healing enhancement. The mean follow-up period of all patients was 19.8 ± 6.8 months (range; 12 months–33 months). ResultsIn total, 37 of 46 patients (80%) could go back to their original sports activities. During the follow-up period, re-tear was encountered in 4 of 46 knees (8.7%). No significant differences in clinical/functional outcomes and re-tear rate were detected between the medial and lateral meniscal repairs. ConclusionIn our expanded repair indication for isolated meniscus repair for athletes, the rate of satisfactory return to sports was 91.3% in total (88.9% for the medial meniscus group; 92.9% for the lateral meniscus group). During the follow-up period ranging from 12 to 33 months (mean, 19.8 months), re-tear of the repaired site was encountered in 4 of the 46 knees (8.7%).

Arthroscopic inside-out repair of complete radial tears of the meniscus with a fibrin clot

To report on the results of 12 complete radial tears of the meniscus treated using arthroscopic inside-out repair with fibrin clots, the results showed good meniscal healing and excellent clinical outcomes. From 2007 to 2009, 12 patients with complete radial tears of the meniscus were treated by arthroscopic inside-out repair with fibrin clots. In all patients, the International Knee Documentation Committee (IKDC) subjective knee form and Lysholm score were determined pre- and post-operatively. We performed magnetic resonance imaging (MRI) and if indicated, we performed a second-look arthroscopic examination. At a mean of 30 ± 4 postoperative months, the Lysholm score and IKDC subjective knee score had improved from 65 ± 6 and 57 ± 7 to 94 ± 3 and 92 ± 3, respectively. Eleven of 12 cases showed complete healing on follow-up MRI. Six of 7 patients undergoing a second-look arthroscopic examination had healed completely. This study shows successful meniscal repairs using fibrin clots in complete radial tears. This surgical procedure appears to be a good treatment method for complete radial tear of the meniscus. Operative technique for inside-out repair of the triangular fibrocartilage complex

A technique for arthroscopic inside-out repair of peripheral traumatic (type 1B) tears of the triangular fibrocartilage complex is reported. The technique can be performed using zone-specific cannulas that are commonly used for repairing meniscal tears in the knee. Anatomic dissections were used to show the safe regions around the TFCC where tears are amenable to this type of repair.
