A systematic review on the effectiveness of objective structured clinical examination (OSCE) in comparison with the traditional method of practical examination

Clinical skills and practice play the main roles in the training of professional courses like nursing. Since its inception, the OSCE has been widely and increasingly used. Studies have shown that it is an effective evaluation tool to assess practical skills for professional courses like nursing. In many occurrences, the OSCE process has been used to test trainees from different disciplines in healthcare settings. The application of OSCE has numerous benefits for both medical and nursing students. It can help to assess the knowledge and performance of clinical skills easily, which is important for nursing practices. An extensive review of Objective Structured Clinical Examination (OSCE) was conducted in comparison with the traditional method of practical examination to find out its effectiveness and identify some barriers and issues related to it. Data were collected from electronic databases including MEDLINE, Pub Med, ISI Web of Science, and CINHAL in four stages, Identification, Screening, Selection, and Inclusion. In this process, 19 studies have been selected and 52 studies have been rejected due to non-meeting of inclusion criteria. In each and every study, the scores for OSCE, along with student satisfaction scores, are high, as some of the studies identify some key issues in carrying out OSCE. The key issues are identified by the teachers who are on the check list, which should be valid, the requirement for expensive models to carry out OSCE, the requirement for more examiners and the space and care that must be taken in organising the stations.

Application of OSCE for Stage Assessment in Standardized Training for Oral Residents.

The aim of this study was to establish and implement an objective structured clinical examination (OSCE) for stage assessment in standardized training for oral residents, evaluating its validity and suitability. An OSCE was established for stage assessment of 158 residents in a program in Jiangsu Province, China. Its validity and suitability were evaluated using indices such as reliability and discrimination coefficients of assessment results. The established OSCE had eight stations: interpretation of auxiliary examination results, basic knowledge, doctor-patient communication and clinical examination, clinical case analysis, medical record and prescription writing, public skills, first aid skills, and specialist's operation skills. The mean overall score and consistency coefficient (Cronbach's alpha) for the test subjects were 83.64±3.69 points and 0.732, respectively. The developed OSCE was reasonably established, with credible assessment results, and indices such as mean and discrimination coefficient of test scores from each station were appropriate. Therefore, the proposed protocol was found to be reliable and suitable.

Application of objective structured clinical examination (OSCE) for completion of general practice residency training

Objective To analysis the implementation of the objective structured clinical examination (OSCE) for completion of general practice residency training in Xinjiang Uygur autonomous region. Methods Ninety eight general practice residents took training-completion examination applying OSCE in Xinjiang Uygur Autonomous Region. The examiners (including SP examiners) and examinees were interviewed with questionnaire after the OSCE. Results All 60 examiners and 98 examinees were generally in favor of using OSCE in general practice residency training completion examination; 100.0% (60/60) examiners and 86.7% (85/98) examinees thought that it was necessary; 98.3% (59/60) examiners and 89.8% (88/98) examinees thought that the OSCE was able to evaluate the clinical competence of general practice; 95.0% (57/60) examiners and 78.6% (77/98) examinees thought the OSCE examination stations reasonable; 40.8% (40/98) examinees thought that the difficulty degree of the exam was moderate, 35.7% (35/98) and 20.4% (20/98) examinees thought that it was difficult or very difficult, particularly for history writing and case analysis. Conclusions The examiners and examinees recognize the advantages of OSCE in assessment of clinical competence. This study provides useful information for application of OSCE in general practice residency training completion assessment. An Innovative Evaluation in Fundamental Nursing Curriculum for Novice Nursing Students: An Observational Research.

<Fundamental Nursing> is the first specialized curriculum for nursing students, laying a foundation for nursing education. <Fundamental Nursing> includes many important basic nursing skills, which suggests the traditional examination could not evaluate how well students learn. Since the OSCE (objective structured clinical examination) developed, it has been used among newly graduated nurses, postgraduate nursing students and achieved some success. OSCE might be a better way to evaluate and reflect students' fundamental nursing skills and comprehensive abilities, although few studies have put it into practice. To build an examination model relating to <Fundamental Nursing> based on OSCE, and put this examination model into practice, and then evaluate the reliability and validity of this examination model. This study is to design an examination model based on literature and teaching purposes of <Fundamental Nursing>. A quantitative descriptive survey methodology was used to obtain the students' outcome and the views of the students on the process and outcome. This study was conducted in the nursing school of one comprehensive university in Beijing, China. 99 undergraduate nursing students in second year participated in this study. The OSCE consisted of 4 interactive simulated patient stations, 2 operating stations without standard patients, 2 theoretical stations. We designed 4 standardized process-based case scenarios and corresponding scripts pertinent to each station. 4 standard patients and 8 teachers of <Fundamental Nursing> were recruited, trained and evaluated before the implementation of objective structured clinical examination. The average score of overall fundamental nursing skills was 90.51 ± 4.00. The Cronbach alpha was 0.665, demonstrating the OSCE is reliable. The content validity of each station varies from 0.90-1, and the criterion validity was promising (R = 0.661, p = 0.000). Also, most students were in favor of OSCE to evaluate in <Fundamental Nursing>. The OSCE is a reliable and valid assessment tool in <Fundamental Nursing>. Meanwhile, it could inspire students' critical thinking and promote students' communication skills. Through successive application of OSCE in nursing education from the very beginning, it might also have some potential effect on the clinical performance.

Brief Analysis of Application of Objective Structured Clinical Examination (OSCE) in Graduation Exams of Clinical Medical Students

This article gives a brief introduction to the Objective Structured Clinical Examination (OSCE) and analyzesdevelopmental progress of OSCE at both home and abroad and standardized patients’ application in OSCE. Also,this article expounds application of OSCE in graduation exam of clinical medical students. Finally, this articlesummarizes pre-exam preparation work and flow of OSCE exam in Affiliated Hospital of Clinical Medical School,analyzes OSCE and puts forward suggestions to improve OSCE.

Study and exploration of stage assessment of standardized resident training

Objective To explore and establish the effective model of assessment for residen- cy training. Methods Totally 181 residents completed general specialist stage of resident training of Liaoning province in 2010. Assessment process was designed and implemented for general specialist training stage, including theory exam （basic knowledge, professional knowledge and public knowledge） and objective structured clinical examination （OSCE） （standardized patient（SP）, computer-based case simulations（CCS） and clinical skills operation）. T test and variance analysis were used and P〈~O.05 stands for statistically significant difference. Results The total results consisted of two parts, theory examination resuhs （40.00%） and OSCE results （60.00%） and two students failed the examination. The mean score of OSCE was up to standard （66.58 ± 5.50）. The mean score of SP and CCS was close to standard. The mean score of clinical skills operation reached a good level. In terms of the OSCE, SP examination and CCS examination, the performance of residents from the two universities had sig- nificant difference （t=3.818, P=0.000; t=5.581, P=0.000; t=5.419, P=0.013） and the performance of residents from the five hospitals also had significant differences. Conclusions The assessment of standardized resident training is improving gradually. Through the widespread application of OSCE in the assessment; it could be helpful to the clinical training of residents.
