Assessing the potential efficacy of 830-nanometer low-level laser therapy in cats: Extraoral applications

Background and Aim: Low-level laser therapy (LLLT) has shown benefits as an alternative treatment of feline chronic gingivostomatitis by reducing pain and inflammation within the oral cavity. Extraoral application technique in cats provides more comfort compared to intraoral application. However, the efficacy of LLLT through buccal tissue is still controversial. This study aimed to investigate the penetration efficacy of LLLT using 830 nm continuous waves with various settings and different application techniques. Materials and Methods: Twenty-four healthy cats were included in this study. The wavelength of LLLT was 830 nm with an output power of 200 mW through extraoral application, using fluences of 2 and 6 J/cm2 in continuous-wave mode. This study compared different distances (contact and non-contact) and three different transmission media (absent media, alcohol, and normal saline solution). Measurement of the laser power within the oral cavity is represented as the mean output power (MOP). Results: Penetration efficacy was detectable for all fluences, distances, and transmission media, with an average buccal thickness of 2.68 mm. MOP did not differ between fluences of 2 and 6 J/cm2 (p = 0.19). In the absence of media, MOP was significantly higher compared with alcohol (p < 0.05) but was not significantly different from normal saline solution (p = 0.26). Conclusion: Extraoral application of LLLT demonstrated penetration efficacy through the buccal tissue with both contact and non-contact skin (<10 mm). This is a potential alternative treatment for oral diseases in clinical practice. However, there is a need for further study on the efficacy of treatment in clinical practice. Evaluation of the Impact of Near-Infrared Multiwavelength Locked System Laser Therapy on Skin Microbiome in Atopic Dogs.

Photobiomodulation (PBM) is a newly adopted consensus term to replace the therapeutic application of low-level laser therapy. It has been suggested that PMB influences the microbiome which, in turn, has increasingly been shown to be linked with health and disease. Even though the use of PBM has also grown dramatically in veterinary medicine, there is still a lack of evidence supporting its effect in vivo. Our objective was to investigate the impact of a dual-wavelength near-infrared laser source (Multiwavelength Locked Laser System, MLS®) on the skin microbiome in atopic dogs. Twenty adult-client-owned atopic dogs were enrolled in the study. The dogs were treated with MLS® laser therapy on one half of the abdominal region, whereas the contralateral side was left untreated and served as a control. Skin microbiome samples were collected before and after MLS® treatments, and then subjected to NGS-based ITS and 16S rRNA analysis. The results showed that while microbiome composition and diversity were not significantly affected, PBM could play a role in modulating the abundance of specific bacterial species, in particular Staphylococcus, that represent a major skin pathogenic strain. To the best of the authors' knowledge, this is the first study to investigate the potential impact of MLS® laser therapy on the skin microbiome in atopic dogs.

A brief literature review of low-level laser therapy for treating amyotrophic lateral sclerosis and confirmation of its effectiveness.

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease with a steadily progressive course due to the death of central and peripheral motor neurons responsible for voluntary movements. Low-level laser therapy (LLLT) is a treatment method unique in its universality and efficacy, particularly for neurodegenerative diseases. In this review, we discuss the effect and application of LLLT in the treatment of ALS. A literature search for English and Russian publications for the keywords "Amyotrophic Lateral Sclerosis", "Low-Level Laser Therapy" was performed using PubMed, Scopus, Google Scholar, Web of Science and Russian Science Citation Index (RSCI) databases. The article provided a brief literature review, substantiated the potential use of low-level laser therapy for ALS. The particular techniques of LLLT were developed. Based on the results of several studies and many years of successful experience with low-level laser therapy in Russia we conclude that a LLLT technique, including intravenous laser blood illumination (ILBI), noninvasive laser blood illumination (NLBI), and local exposure, is a promising treatment method for ALS.

Prevention and Treatment of Oral Mucositis in Pediatric Patients: Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Oral mucositis (OM) is a common and serious side effect of cancer treatment. The incidence of chemotherapy-induced OM in pediatric patients can reach up to 91.5% and has a major impact on patients' quality of life. The aim of the study was to assess the efficacy of current interventions and agents for the management of OM in children undergoing chemo/radiotherapy or hematopoietic stem cell transplantation (HSCT). A systematic search of randomized controlled trials (RCTs) was conducted in the MEDLINE and Scopus databases from January 2000 until March 2023. Thirty-four randomized studies meeting the inclusion criteria were identified and five RCTs investigating the efficacy of Low Level Laser Therapy (LLLT) intervention or the agent honey were included in the meta-analysis. The meta-analysis of two RCTs indicated that topical application of honey on oral mucosa was effective in shortening the mean duration of hospital stay in children with severe OM (MD=-4.33, p=0.002). However, LLLT was not found to be effective for the prevention or treatment of OM grade ≥II (RR=0.99, p=0.99). Moreover, the therapeutic application of LLLT did not show significant benefit for lower risk of OM grade ≥II (RR=0.48, p=0.58). Various interventions and agents were examined in the present study for the management of OM. Honey could be a promising candidate for the treatment of OM in pediatric patients. Further high-quality RCTs are required to enhance our findings.

The effectiveness of interventions in reducing pain related to orthodontic separation: a systematic review and meta-analysis.

Pain is an unpleasant experience and annoying sensation. To control this pain during orthodontic separation, different pharmacological and non-pharmacological methods have been used. This systematic review and meta-analysis aimed to critically assess the evidence of the effectiveness of pharmacological and non-pharmacological methods in reducing pain induced by orthodontic separation. An electronic search was conducted using the following databases: PubMed® (Medline), Scopus®, EMBASE®, Web of ScienceTM, Google ScholarTM, ProQuest, and Cochrane Central Register of controlled trials (CENTRAL) searching for the studies published between January 2012 and April 2023. Only randomized controlled trials (RCTs) were included, each experimental group included patients who received elastomeric separators and one kind of pharmacological or non-pharmacological interventions for pain reduction during the separation stage. Cochrane's risk of bias tool (RoB2 tool) was applied. The Grading of Recommendations Assessment, Development, and Evaluation [GRADE] approach was used to evaluate the strength of the evidence. Thirty-one studies (RCTs) were included in this systematic review. Nineteen of them were appropriate for quantitative synthesis and used VAS for pain assessment. Meta-analysis showed that low-level laser therapy (LLLT) was an effective approach for pain relief after separators placement with standard mean difference of 13.79 mm (95% confidence interval (CI): -15.64, -11.94) at 6 h and 23.34 mm at 24 h (95% CI: -25.91, -20.77). LLLT was also effective when applied in split-mouth and the standard mean difference was 8.9 mm at 6 h (95% CI: -12.86, -3.33) and 17.15 mm at 24 h (95% CI: -30.12, -4.17). Ibuprofen had a pain control effect at 6 h and at 24 h compared with the placebo group. The standard mean difference was 14.37 mm (95% CI: -20.54, -8.19) and 20.46 mm (95% CI: -27.79, -13.13), respectively. There was no difference in pain control between ibuprofen and acetaminophen. Naproxen had lower visual analog scale scores in pain perception at 6 h and the standard mean difference was 7.03 mm (95% CI: -12.67, -1.40). The application of LLLT decreased the pain induced by the separation during the first day of teeth separation; the pain reduction showed an increase from 6 h to the end of the 24 h. However, the evidence is weak to moderate. The analgesics reduced the pain compared to placebo; this pain reduction had shown an increase from 6 h to the end of the 24 h. The strength of the evidence is moderate. Naproxen gel effectively reduced the pain compared to placebo; the evidence in this regard is moderate. Naproxen gel effectively reduced the pain compared to placebo, but it was less effective than the oral intake of non-steroidal anti-inflammatory drugs. However, the evidence in this regard is moderate. This systematic review was registered with PROSPERO (CRD42022335553) during the first stages of its conduction.

Patient-Reported Outcome Measures of Flapless Corticotomy with Low-Level Laser Therapy in En Masse Retraction of Upper Anterior Teeth: A Three-Arm Randomized Controlled Trial.

(1) Background: This study aimed to compare patient-reported outcome measures when accelerating en masse retraction between the piezocision procedure and the subsequent application of low-level laser therapy (FC+LLLT), with the piezocision alone (FC), and in a control group. (2) Methods: A three-arm randomized controlled trial (RCT) was conducted involving 60 patients (41 females and 19 males) with Class II division I malocclusion. The en masse retraction was performed using NiTi closed coil springs attached to miniscrews. The LLLT was performed using an 808 nm Ga-Al-As diode laser. Patient responses regarding pain, discomfort, swelling, and chewing difficulties were reported at ten assessment points. (3) Results: The greatest pain levels were observed 24 h after the application of force during the first and third months of retraction. The mean pain, discomfort, swelling, and chewing difficulties were significantly smaller in the control group than in the FC and FC+LLLT groups. High satisfaction levels were reported in all three groups (p < 0.05). (4) Conclusions: The accelerated en masse retraction via piezocision, followed by a small course of LLLT, was accompanied by significantly fewer pain, discomfort, and chewing difficulties than the control group. LLLT is a valuable addition to piezocision, with an improved patient experience.

A Brief Review of Low-Level Light Therapy in Depression Disorder.

Introduction: Low-level laser therapy (LLLT), also called Photobiomodulation, has gained widespread acceptance as a mainstream modality, particularly in the form of photobiostimulation (PBM). Here in our review, we aim to present the application of LLLT to help with depression, explore potential action mechanisms and pathways, discuss existing limitations, and address the challenges associated with its clinical implementation. Methods: In biological systems, the visible light with a wavelength range of 400-700 nm activates photoreceptors involved in vision and circadian rhythm regulation. The near-infrared (NIR) light with a wavelength range of 800-1100 nm exhibits superior tissue penetration capabilities compared to the visible light, which enables the non-invasive application of LLLT to various tissues. Results: By enhancing adenosine triphosphate (ATP) production using the respiratory chain, LLLT is able to enhance blood flow, reduce inflammation, support repair and healing, and enhance stem cell growth and proliferation. Preclinical studies using animal models have shown promising neuroprotective effects of the LLLT method on central nervous system (CNS) diseases, suggesting potential improvements in brain function for patients suffering from Alzheimer's disease. In addition, it helps Parkinson's patients with their movement problems and ameliorates mental disorders in individuals with depression. Conclusion: patients' quality of life can be significantly enhanced. A comprehensive understanding of the protective effects and underlying mechanisms of LLLT will facilitate its therapeutic application in the future.

Comparison of three different dosages of low-level laser therapy on expression of cell proliferation and inflammatory markers following ovariohysterectomy in rats

The objective of the current study was to evaluate Low-level laser therapy (LLLT) on the healing of incisional wounds following ovariohysterectomy in rats, by means of subjective histopathological and immunohistochemical analysis. A total of 72 female Wistar rats were categorised into four treatment groups (Group I; sacrification 4 hours following only one LLLT application, Group II; sacrification 7 days following only one LLLT application, Group III; sacrification 4 hours after two LLLT applications, and Group IV; sacrification 7 days after two LLLT applications). Each group was further divided into four different doses subgroups (Group Control [C, off mode LLLT application], L1 [1 J/cm2], L3 [3 J/cm2], and L6 [6 J/cm2]), with equal representation in each subgroup. Ovariohysterectomy was employed using two 2-cm-length midline abdominal incisions in the left and right sides of line alba. The Group C was assigned to the left side incision to each rat in the study. After irradiation, the tissue was subjected to histopathological analysis to determine the extent of mononuclear cell infiltration, edoema, and epithelialization. Additionally, immunohistochemical analysis was performed to evaluate the expression of proliferating cell nuclear antigen (pCNA) and inducible nitric oxide synthase (iNOS). Group L1 and L3 significantly decreased mononuclear cell infiltration compared with Group C in all treatment groups (p < 0.05). Group L3 significantly decreased edoema compared with Group C in all groups except for treatment Group I (p < 0.05). Group L2 and L3 significantly increased epithelization in treatment Group IV (p < 0.05). Moreover, Group L2 and L3 significantly increased pCNA in all groups, while L2 and L3 significantly decreased iNOS expression in treatment Group II, III, and IV (p < 0.05). However, no statistical difference was found between subgroups of treatment Group I in iNOS expiration (p > 0.05). The results of the current examination demonstrated that LLLT can modulate mononuclear cell infiltration and edoema, and improve epithelization, as well as increase pCNA expression, whereas decrease iNOS expression during the wound healing process, therefore enhancing wound healing following ovariohysterectomy in rats.

Effectiveness of photobiomodulation with low-level lasers on the acceleration of orthodontic tooth movement: a systematic review and meta-analysis of split-mouth randomised clinical trials.

Although several studies have evaluated the effect of low-level laser therapy (LLLT) on orthodontic movement acceleration, results are still inconsistent. Such inconsistencies may be attributed to the differences in the LLLT application protocols, especially in terms of wavelength ranges. Objective: (i) to assess the clinical effects of LLLT on the acceleration of orthodontic movement and (ii) to establish the most effective LLLT wavelength to accelerate tooth movement during orthodontic treatments. MEDLINE (PubMed), Scopus, ScienceDirect, and LILACS were searched from inception to October 2022. Inclusion criteria: Split-mouth randomised clinical trials (RCTs) on systemically healthy patients reporting the effect of LLLT in accelerating orthodontic movements, specifically retraction of canines. The risk of bias was assessed using RoB-2. A random effect model was applied. Nineteen RCTs met the inclusion criteria for qualitative synthesis, and eighteen RCTs were included in the quantitative synthesis. Seventeen studies were rated as at some concerns of bias and two studies were classified as having a low risk of bias. In general terms, this systematic review and meta-analysis presents a moderate risk of bias. Findings of this systematic review and meta-analysis point to a tendency for faster orthodontic dental movement in the groups receiving LLLT treatment during the first (OR of 0.28 95% CI (0.07 to 0.48)), second (OR of 0.52 95% CI (0.31 to 0.73)), and third (OR of 0.41 95% CI (0.03 to 0.79)) month follow-up. Wavelengths ≤ 810nm and energy density values ≤ 5.3J/cm2 were associated with faster orthodontic tooth movement.

Different Effects of Low-Level Laser Therapy on the Proliferation of HT29 Cells in Culture and Xenograft Models.

Introduction: Different kinds of treatments have been developed to fight cancers. Low-level laser therapy (LLLT), also known as photobiomodulation therapy (PBMT), is a low-power monochromatic and coherent light that has been used successfully for healing injuries and combating malignancies. However, there are concerns about the application of LLLT to cancers due to the increased proliferation of some cancer cells after LLLT. Methods: This study investigated the effects of 650 nm and 870 nm lasers on the proliferation of HT29 colorectal cancer cell lines in vitro and in vivo. Results: The results showed that the laser with a wavelength of 870 nm did not meaningfully alter the proliferation of cultured cells. However, cell proliferation was promoted when the laser was applied within a wavelength of 650 nm. Treatment of HT29-derived tumors in nude mice with the 650 nm laser resulted in the decline of the tumor progression rate compared to controls. This result was inconsistent with the proliferative effects of the laser on the cultured cells. Conclusion: Cell behavior in response to LLLT might be different between cell culture and xenograft models.

Low-Level Laser Therapy for the Treatment of Oral Mucositis Induced by Hematopoietic Stem Cell Transplantation: A Systematic Review with Meta-Analysis.

Oral mucositis is a common and debilitating side effect induced by stem cell transplantation that is experienced by cancer patients undergoing chemotherapy or radiation therapy. This condition involves inflammation and ulceration of the oral mucosa, leading to pain, difficulty with eating and speaking, and an increased risk of infections. Mucositis not only compromises the quality of life for cancer patients, but also affects treatment outcomes and may necessitate dose reductions or treatment delays. This scientific article provides a comprehensive overview of mucositis. The purpose of this literature review with a meta-analysis is to evaluate the efficacy of laser therapy in treating post-transplant mucositis. Materials and methods: A search of the literature from 3 May 2023 was carried out on three online databases, PubMed, Scopus, and Web of Science. Only studies that treated patients with laser therapy were considered; only studies with the placebo-treated control group were considered. Review Manager version 5.2.8 (Cochrane Collaboration) was used for the pooled analysis. We measured the std. mean difference between the two groups (laser and placebo). Results: There were 230 papers included in this review. Two hundred twenty-seven were excluded. Furthermore, a manual search was performed. After the search phase, three articles were considered in the study. The overall effect showed differences in the degree of mucositis in the laser-treated patients compared with the placebo group. The meta-analysis shows a reduction in the degree of mucositis in the patients treated with laser therapy (std. mean difference -1.34 [-1.98; -0.98]; C.I. 95%). Conclusions: The application of laser therapy results in decreased severity of oral mucositis from radiation and chemotherapy. Our study shows that the application of low-level laser therapy in the treatment of transplant mucositis has excellent efficacy in relieving the symptoms and severity of mucositis.

Exploring the Effects of Low-Level Laser Therapy on the Cytocompatibility and Osteo/Odontogenic Potential of Gingival-Derived Mesenchymal Stem Cells: Preliminary Report

Numerous tissue engineering uses for gingival-derived mesenchymal stem cells (GMSCs) have been demonstrated. Recently, low-level laser therapy (LLLT) has been projected as a factor that can improve MSCs’ regeneration capacity. Therefore, the aim of this research was to examine the impact of LLLT at 1.5 J/cm2 and 3 J/cm2 on the viability and osteo/odontogenic potential of GMSCs. An MTT assay was performed to detect viability. Osteo/odontogenic differentiation was evaluated using Alizarin Red S staining and qRT-PCR for the evaluation of the RUNX2, OC, DMP1, and DSPP genes. A two-way ANOVA with Tukey’s post hoc test was used to determine the statistical significance between groups. The results revealed that LLLT of both energy densities had no cytotoxic effect on GMSC viability. LLLT of 1.5 J/cm2 demonstrated better viability than the higher energy density (3 J/cm2). Furthermore, the osteo/odontogenic differentiation potential was promoted following LLLT radiation, where both groups exhibited mineralized nodule formation, with the low-energy laser having a significantly higher Alizarin Red S stain level. A qRT-PCR analysis revealed higher expression levels of osteogenic and odontogenic markers in the LLLT groups compared to the control group. In conclusion, this study showed the potential application of LLLT as a non-toxic and effective strategy to enhance the regenerative capacity of GMSCs for tissue engineering and clinical treatments in the oral and craniofacial fields.

The Condylar Effects of Mesenchymal Stem Cells, Low-Level Laser Therapy and Grape Seed Extract on Functional Mandibular Advancement of the Rat Mandible.

Functional treatment of Class II malocclusion is expected to lead to adaptation in the condyle. This study aimed to evaluate the effects of adipose tissue-derived mesenchymal stem cells (ADMSCs), low-level laser therapy (LLLT), and grape-seed extract (GSE) on condylar growth after functional mandibular advancement. Forty-five rats were randomly divided into 8 groups. Functional appliances were applied to all groups (n=6) except the control group (n=3). One group was treated with appliances only; the other six groups received various combinations of ADMSCs, LLLT, and GSE. Analyses for new osteoblasts and new bone formation, vascular endothelial growth factor, and Type II collagen were performed on condylar tissues, after an experimental period of four weeks. The quantitative data obtained from the results of the experiments were evaluated by H-score and analyzed using One-Way ANOVA by Tukey-Kramer multiple comparisons test (p≤0.05). Levels of all investigated parameters increased in all groups (p≤0.05). The highest increases were achieved by a combined application of functional appliance, ADMSCs, LLLT and GSE (p≤0.05). Single LLLT administrations or single GSE applications did not create a statistical difference from appliance alone (p>0.05). A positive effect of ADMSCs or LLLT on osteoblast formation, neovascularization, and Type II collagen level was apparent (p≤0.05), however, neither affected new bone formation (p>0.05). This study shows that ADMSCs with LLLT and GSE applications provide differing levels of new osteoblast and bone formation, new vascular formation, and Type II collagen formation in rat condyles after functional mandibular advancement.

Comparative Evaluation of the Effects of Low-Level Laser Therapy and Ozone Application on Wound Healing After Gingivectomy and Gingivoplasty: A Randomized Control Clinical Trial

AIM: The aim of this randomized controlled clinical trial was to clinically evaluate and compare the effect of ozonated water and Low level laser therapy (LLLT) on the early wound healing period of gingivectomy and gingivoplasty wounds by clinical analysis. Materials &amp; Methods: Forty-five patients were randomly allocated into 3 groups, test group 1 (ozone therapy n=15), test group 2 (LLLT application n=14) and control group (no treatment n=15). Ozone irrigation was done on the surgical site with a concentration of 4ppm for 5-10 minutes, LLLT application was performed with diode LASER (810 nm) immediately after the surgical therapy and on day 3, day 7, day 21. Wound surface area, wound healing index, quality of life, plaque index, sulcus bleeding index were investigated. Results: At day 21 statistically significant smaller wounds were observed in both ozone and LLLT groups compared to control group. However, intergroup comparison between the ozone and LLLT group did not show statistically significant difference in wound surface area evaluated by staining technique and image J analysis. The wound healing index, VAS score OHIP -14 questionnaire, analgesics consumed, plaque index, sulcus bleeding index showed a statistically significant reduction from baseline to day 21in all groups. Conclusion: Ozone therapy is safe and effective in promoting wound healing during the 1st 3 weeks post-surgery. Ozone therapy and LLLT application enhanced wound healing after gingivectomy and gingivoplasty procedures, accompanied by better quality of life and reduced pain .

Investigation of effects of quercetin and low-level laser therapy in cisplatin-induced in vitro peripheral neuropathy model.

Chemotherapy-induced peripheral neuropathy (CIPN) is one of the dose-dependent side effects of cisplatin. The loss of sensory neurons is observed in CIPN. There are many methods to minimalize CIPN symptoms such as pharmacological agents and photobiostimulation but the mechanisms of these methods are unclear. Our study is aimed at determining the effects of quercetin and low-level laser therapy (LLLT) in undifferentiated and nerve growth factor-differentiated PC12 cells in cisplatin-induced peripheral neuropathy. PC12 cells with cisplatin were co-treated with quercetin and LLLT (diode pumped all-solid-state laser, 670 nm, output 500 mW, and the laser beam surface area was 1.96 cm2). The effects of quercetin and LLLT on GAP-43 and Synapsin I expressions were analyzed by real-time PCR, cell viability was assessed by MTT assay, Annexin and dead assay measured the induction of apoptosis, the alterations in mitopotential were assessed by mitopotential assay, and lactate dehydrogenase activity in cells was analyzed. All experiment data were analyzed by the Tukey test and applied as a post hoc test, and statistical evaluation was made. Our results indicated that cisplatin increased apoptosis (24,210 ± 2189, 46,504 ± 8246) cells, mitochondrial dysfunction (44,312 ± 0.751, 68,788 ± 1271), and LDH activity (62,821 ± 8245, 87,838 ± 8116). Furthermore, it decreased cell viability (42,447 ± 1780, 36,140 ± 3682) and inhibited GAP-43 and Synapsin I genes in undifferentiated and differentiated PC12 cells. However, apoptosis, the alterations in mitopotential, and lactate dehydrogenase activity decreased by applications of quercetin and LLLT. It has been recommended that quercetin and low-level laser therapy roles on cisplatin-induced peripheral neuropathy should be investigated in vivo, and the relationship between quercetin and low-level laser therapy should be molecular.

Assessment of the effect of frequency of low-level laser therapy exposure at different intervals on orthodontic tooth movement – A systematic review and meta-analysis

To assess the optimal intervals of exposure of low-level LASER therapy (LLLT) that would optimally accelerate orthodontic tooth movement. Second objective was to quantitatively analyze the difference in the time taken for alignment of anterior teeth with and without the application of LASER.PROSPERO database registry was done (CRD42020196472) and review was conducted based on PRISMA guidelines. A search was systematically conducted in five major electronic databases without restrictions up to June 2020 along with a hand search of selected journals. The quality of evidence was assessed using the Grading of Recommended Assessment, Development, and Evaluation tool, risk of bias using Cochrane risk of bias tool, and meta-analysis was carried out using RevMan 5.4 software.Ten randomized controlled trials which met the inclusion criteria were evaluated and tabulated. A random-effects meta-analysis demonstrated that there is a statistically significant increase in the orthodontic tooth movement when patients were exposed to minimum of four intervals of LLLT in the first month, at P =0.03 by a standard mean difference of 0.46 mm with an overall heterogeneity of I2 = 0% at 95% confidence interval. There was a statistically highly significant reduction in the number of days taken for alignment of anterior teeth with the application of LASER (P <.00001).Application of LLLT for minimum of four irradiations in the first month has yielded better results in accelerating orthodontic tooth movement than application of LLLT once a month.

Serum Level of Il-2 in Patients with Type 2 Diabetes Mellitus and Periodontopathy

The purpose of this study was to investigate the influence of low-level laser therapy (LLLT) in patients with type 2 diabetes mellitus (T2DM) and chronic periodontopathy (ChP) on serum levels of IL-2. Methods and Results: A total of 80 patients aged 35-60 years were followed; all of them had T2DM diagnosed (HbA1C≤7.5%) with ChP, where clinical attachment loss (CAL) was ≥4mm on at least 50% of affected teeth. All participants are divided into two groups. Group A included 40 patients who underwent conservative (non-surgical) periodontal treatment supplemented with LLLT. Group B included 40 patients who underwent only conservative therapy. Patients used oral antidiabetic medications to control glycemia: Metformin (Alkaloid, Skopje S. Macedonia) 500 mg two times a day. LLLT (Laser HF® comfort, Hager. Werken, Duisburg, Germany) was applied (660 nm, 10 mW, 8 min/day) with contact to the gingiva for five consecutive days. Serum IL-2 was determined by ELISA in 3 time intervals: at the first examination, 6 weeks, and 3 months after treatment in both groups. In Group A and Group B, at the first examination, 6 weeks after therapy, and 3 months after treatment, the serum IL-2 was 17.20±0.54 pg/ml and 17.22±0.66 pg/ml, 17.12±0.63 pg/ml and 17.17±0.63 pg/ml, and 17.03±0.64 pg/ml and 16.98±0.65 pg/ml, respectively. In Group A, there was a significant difference between the serum IL-2 values in specified time points (first examination, 6 weeks, and 3 months after the therapy) (Friedman’s ANOVA: χ² (n=40, df=2) = 17.22 and P=0.0002). In Group B, between the serum IL-2 levels, there also was a significant difference in specified time points (Friedman’s ANOVA: χ² (n=40, df=2) = 42.33 and P=0.0000). The intergroup analysis, according to the temporal dynamics of the measurements, showed an evident difference between the two groups, but the serum IL-2 values in the two groups treated with and without LLLT were close, and no statistical significance was recorded between them. Conclusion: No significant differences were recorded in the serum IL-2 levels in T2DM patients with ChP non-surgically treated with and without the application of LLLT.

Evaluation of Adjunctive Low Level Laser Therapy in Postoperative Management of Pain, Swelling, Trismus Following Surgical Removal of Impacted Mandibular Third Molars

Our aim is to evaluate the effectiveness of Low Level Laser Therapy (LLLT) in the control of postoperative pain, swelling, and trismus associated with the surgical removal of impacted mandibular third molars. This study was carried out as a single centre, prospective study with a sample size of 30 patients to evaluate the effectiveness of LLLT following surgical removal of impacted third molars. Patients were randomly divided into two groups of 15 each. Group 1(Study /LLLT group) consisted of patients undergoing LLLT and with the use of postoperative analgesics and antibiotics. Group 2 (Control) included patients who were administered postoperative analgesics and antibiotics without the concurrent use of LLLT. The predictor variable was the LLLT application following mandibular third molar impaction surgery. The outcome variables namely pain, swelling, and trismus were evaluated on the day of surgery and 1st, 3rd, and 7th postoperative days (POD).Results:The pain was highest on POD 1 and gradually reduced by POD 7 in the study group when compared to the control group. Swelling showed a steep increase on POD 1 and thereafter a gradual reduction was observed on POD 7, when compared to the control group, the study group showed a significant decrease in swelling. Mouth opening was the lowest on POD1 and gradually increased by POD 7 in the study group than in the control group. Conclusion: The results of the study suggest that the application of LLLT to impacted mandibular third molar sockets helps eliminate/or reduce postoperative pain, swelling, and trismus.

Clinical application of low-level laser therapy (Photo-biomodulation therapy) in the management of breast cancer-related lymphedema: a systematic review

BackgroundBreast cancer-related lymphedema (BCRL) is a frequent issue that arises after mastectomy surgery in women and compromises physical and mental function. Previously published studies have shown positive effects with the use of Low-level laser therapy in another term Photo-biomodulation therapy (PBM). This research investigated the efficacy of clinical use of LLLT (PBM) in the treatment of metastatic breast cancer-related lymphedema.MethodsPubMed, PEDro, Medline, and the Cochrane Library were searched for LLLT clinical trials published before October 2021. The methodological quality of randomized trials and the effectiveness of Laser Therapy for BCRL were evaluated. The primary objectives were arm circumference or arm volume, whereas the secondary goals were to assess shoulder mobility and pain severity.ResultsEight clinical trials were analyzed in total. Typically, the included RCTs had good research quality. At four weeks, there was a considerable reduction in arm circumference/volume, and this continued with long-term follow-up. However, no statistically significant change in shoulder mobility or pain severity was seen between the laser and placebo groups at 0-, 1-, 2-, and 3-month short-term follow-up.ConclusionsThe findings of this comprehensive study demonstrated that LLLT (PBM) was successful in diminishing arm circumference and volume than improving shoulder mobility and pain. Data indicates that laser therapy (PBM) may be a beneficial treatment option for females with PML.
