Heart Rate Variability Applications in Strength and Conditioning: A Narrative Review.

Heart rate variability (HRV) is defined as the fluctuation of time intervals between adjacent heartbeats and is commonly used as a surrogate measure of autonomic function. HRV has become an increasingly measured variable by wearable technology for use in fitness and sport applications. However, with its increased use, a gap has arisen between the research and the application of this technology in strength and conditioning. The goal of this narrative literature review is to discuss current evidence and propose preliminary guidelines regarding the application of HRV in strength and conditioning. A literature review was conducted searching for HRV and strength and conditioning, aiming to focus on studies with time-domain measurements. Studies suggest that HRV is a helpful metric to assess training status, adaptability, and recovery after a training program. Although reduced HRV may be a sign of overreaching and/or overtraining syndrome, it may not be a sensitive marker in aerobic-trained athletes and therefore has different utilities for different athletic populations. There is likely utility to HRV-guided programming compared to predefined programming in several types of training. Evidence-based preliminary guidelines for the application of HRV in strength and conditioning are discussed. This is an evolving area of research, and more data are needed to evaluate the best practices for applying HRV in strength and conditioning.

Research Progress in Heart Rate Variability Applications in Exercise Rehabilitation for Cardiovascular Diseases

Heart rate variability (HRV) is an important marker for assessing the balance of the autonomic nervous system and clinical prognosis, because it can be non-invasively and easily measured, and the results are accurate and valuable. HRV is widely applied in cardiovascular disease fields. Exercise training is an important part of cardiac rehabilitation. Personalized sports rehabilitation therapy can effectively prevent the emergence of cardiovascular diseases, decrease the risk of recurrent cardiovascular events, and ameliorate dysfunction, as well as limitations in life, work, and social participation, caused by adverse cardiovascular events. However, sports rehabilitation can have drawbacks, in that improper training can cause injury, excessive fatigue, or even harmful cardiovascular events. To support future applications, this article reviews recent applications of HRV in sports rehabilitation for cardiovascular diseases.

Autonomic Modulation in Duchenne Muscular Dystrophy During a Computer Task: A Prospective Transversal Controlled Trial Assessment by Non-linear Techniques.

Introduction: Due to functional and autonomic difficulties faced by individuals with Duchenne Muscular Dystrophy (DMD), the use of assistive technology is critical to provide or facilitate functional abilities. The key objective was to investigate acute cardiac autonomic responses, by application of Heart Rate Variability (HRV), during computer tasks in subjects with DMD via techniques based on non-linear dynamics.Method: HRV was attained via a Polar RS800CX. Then, was evaluated by Chaotic Global Techniques (CGT). Forty-five male subjects were included in the DMD group and age-matched with 45 in the healthy Typical Development (TD) control group. They were assessed for 20 min at rest sitting, and then 5 min whilst performing the maze task on a computer.Results: Both TD and DMD subjects exhibited a significantly reduced HRV measured by chaotic global combinations when undertaking the computer maze paradigm tests. DMD subjects presented decreased HRV during rest and computer task than TD subjects.Conclusion: While there is an impaired HRV in subjects with DMD, there remains an adaptation of the ANS during the computer tasks. The identification of autonomic impairment is critical, considering that the computer tasks in the DMD community may elevate their level of social inclusion, participation and independence.

Is the Median Hourly Ambulatory Heart Rate Range Helpful in Stratifying Mortality Risk among Newly Diagnosed Atrial Fibrillation Patients?

Background: The application of heart rate variability is problematic in patients with atrial fibrillation (AF). This study aims to explore the associations between all-cause mortality and the median hourly ambulatory heart rate range () compared with other parameters obtained from the Holter monitor in patients with newly diagnosed AF. Material and Methods: A total of 30 parameters obtained from 521 persistent AF patients’ Holter monitor were analyzed retrospectively from 1 January 2010 to 31 July 2014. Every patient was followed up to the occurrence of death or the end of 30 June 2017. Results: was the most feasible Holter parameter. Lower was associated with increased risk of all-cause mortality (adjusted hazard ratio [aHR] for every 10-bpm reduction: 2.70, 95% confidence interval [CI]: 1.75–4.17, p < 0.001). The C-statistic of alone was 0.707 (95% CI: 0.658–0.756), and 0.697 (95% CI: 0.650–0.744) for the CHA2DS2-VASc score alone. By combining with the CHA2DS2-VASc score, the C-statistic could improve to 0.764 (95% CI: 0.722–0.806). While using 20 bpm as the cut-off value, the aHR was 3.66 (95% CI: 2.05–6.52) for patients with < 20 bpm in contrast to patients with ≥ 20 bpm. Conclusions: could be helpful for risk stratification for AF in addition to the CHA2DS2-VASc score.

As the application of heart rate variability extends to mobile and wearable devices, time required for analysis is becoming a major issue. There have been many research investigating whether ultra-short-term heart rate variability can replace traditional 5-minute heart rate variability, however, every study is limited to resting condition and are not suitable for application in mobile devices that lies on dynamic condition. The aim of this study is to evaluate the feasibility of the ultra-short-term heart rate variability in dynamic condition such as recovery after an exercise. We analyzed time- and frequency-domain variables of ultra-short-term heart rate variability at 240-, 180-, 120-, 60-, 30- and 10-second Electrocardiogram in recovery condition after exercise. Then, we compared the result of ultra-short-term heart rate variability with the results of 5-minute heart rate variability statistically. Results showed that more than 180-second is required for AVNN, 120-second is required for VLF, 60-second is required for SDNN, SDSD, RMSSD, pNN50, TP and LF, and 30-second is required for HF and LF/HF in recovery conditions.

Application of heart rate variability during blood pressure measurement in patients with somatic symptom disorder

ObjectiveThis study wants to test whether the heart rate variability (HRV) analysis using sphygmomanometers can effectively reflect the physical and emotional distress of somatic symptom disorder (SSD) cases and assist in further health management. MethodsThis study includes 66 female SSD cases and 32 male SSD cases. All cases received a 200-second blood pressure (BP) measurement, after which the numerical figure for the heart rate was obtained from the BP signal for HRV analysis. All cases also filled out Patient Health Questionnaire-15 (PHQ-15), Beck Depression Inventory-II (BDI-II), Beck Anxiety Inventory (BAI) to understand the extent of their somatic distress, depression, and anxiety. The relationship between psychological conditions and HRV indicators are examined statistically. Receiver operating characteristic (ROC) curve analysis was used to understand whether HRV indicators can assist in the determination of psychosomatic problems. ResultsConsidering all and female SSD cases, low-frequency power (LF) and PHQ-15 in the HRV index have the most significant correlations. In male SSD cases, root mean square of successive NN interval differences (RMSSD) and BDI-II have the highest correlations. The results of ROC curve analysis indicate that LF is useful to judge the severity of female SSD cases, while RMSSD has reference value in determining whether male patients of SSD cases have depression or not. In addition, physical stress index (PSI) can also help determine the degree of depression in male SSD cases. ConclusionThe use of BP in HRV analysis can help in the monitoring of somatic distress and depression issues in SSD cases. LF, RMSSD, PSI are potential physiological indicators.

Heart rate variability: Measurement and emerging use in critical care medicine.

Variation in the time interval between consecutive R wave peaks of the QRS complex has long been recognised. Measurement of this RR interval is used to derive heart rate variability. Heart rate variability is thought to reflect modulation of automaticity of the sinus node by the sympathetic and parasympathetic components of the autonomic nervous system. The clinical application of heart rate variability in determining prognosis post myocardial infarction and the risk of sudden cardiac death is well recognised. More recently, analysis of heart rate variability has found utility in predicting foetal deterioration, deterioration due to sepsis and impending multiorgan dysfunction syndrome in critically unwell adults. Moreover, reductions in heart rate variability have been associated with increased mortality in patients admitted to the intensive care unit. It is hypothesised that heart rate variability reflects and quantifies the neural regulation of organ systems such as the cardiovascular and respiratory systems. In disease states, it is thought that there is an 'uncoupling' of organ systems, leading to alterations in 'inter-organ communication' and a clinically detectable reduction in heart rate variability. Despite the increasing evidence of the utility of measuring heart rate variability, there remains debate as to the methodology that best represents clinically relevant outcomes. With continuing advances in technology, our understanding of the physiology responsible for heart rate variability evolves. In this article, we review the current understanding of the physiological basis of heart rate variability and the methods available for its measurement. Finally, we review the emerging use of heart rate variability analysis in intensive care medicine and conditions in which heart rate variability has shown promise as a potential physiomarker of disease.

Clinical Applications of Heart Rate Variability in Neurological Disorders

Heart rate variability (HRV) is a useful marker for predicting mortality and cardiovascular events in cardiac diseases. Various time- and frequency-domain parameters are used for assessing HRV. The time-domain measures include standard deviation of the NN intervals (SDNN), standard deviation of the average NN interval, root mean square of the successive differences SDNN index, NN50 count, and pNN50, while the power spectrum that is used the most widely in frequency-domain analyses is divided into low frequency (LF), high frequency (HF), LF norm, HF norm, and LF/HF. The HF band is modulated by the parasympathetic nervous system, while the LF band is modulated by both the sympathetic and parasympathetic nervous systems. Altered or reduced HRV parameters have been shown to be related to cardiovascular events in patients with various neurological disorders, such as parkinsonian syndromes, dementia, cerebrovascular disorder, and epilepsy. Furthermore, analyses of HRV have attempted to distinguish different diseases and evaluate the degree of disability. This article discusses the clinical use of HRV in the neurological field. Application of Heart Rate Variability in Diagnosis and Prognosis of Individuals with Diabetes Mellitus: Systematic Review.

The use of heart rate variability as a tool capable of discriminating individuals with diabetes mellitus is still little explored, as its use has been limited to comparing those with and without the disease. Thus, the purpose of this study was to verify the use of heart rate variability as a tool for diagnostic and prognostic evaluation in person with diabetes and to identify whether there are cutoff points generated from the use of this tool in these individuals. A search was conducted in the electronic databases MEDLINE, Cochrane Library, Web of Science, EMBASE, and LILACS starting from the oldest records until January 2015, by means of descriptors related to the target condition, evaluated tool, and evaluation method. All the studies were evaluated for methodological quality using the QUADAS-2 instrument. Eight studies were selected. In general, the studies showed that the heart rate variability is useful to discriminate cardiac autonomic neuropathy in person with diabetes, and the sample entropy, SD1/SD2 indices, SDANN, HF, and slope of TFC have better discriminatory power to detect autonomic dysfunction, with sensitivity and specificity values ranging from 72% to 100% and 71% to 97%, respectively. Although there are methodological differences in indices used, in general, this tool demonstrated good sensitivity and specificity and can be used as an additional and/or complementary tool to the conventional autonomic tests, in order to obtain safer and more effective diagnostic, collaborating for better risk stratification conditions of these patients.

Heart Rate Variability as a Method for Assessment of the Autonomic Nervous System and the Adaptations to Different Physiological and Pathological Conditions.

The autonomic nervous system controls the smooth muscles of the internal organs, the cardiovascular system and the secretory function of the glands and plays a major role in the processes of adaptation. Heart rate variability is a non-invasive and easily applicable method for the assessment of its activity. The following review describes the origin, parameters and characteristics of this method and its potential for evaluation of the changes of the autonomic nervous system activity in different physiological and pathological conditions such as exogenous hypoxia, physical exercise and sleep. The application of heart rate variability in daily clinical practice would be beneficial for the diagnostics, the outcome prognosis and the assessment of the effect of treatment in various diseases.

Heart Rate Variability and Traumatic Brain Injury (TBI): Clinical Applications

This paper discusses the clinical applications of heart rate variability (HRV) data in the treatment of clients who have experienced traumatic brain injuries (TBIs). In the authors' clinical practice, HRV data is collected at the initial assessment, at progress assessments, and again after the completion of a course of neurofeedback combined with HRV biofeedback treatment. This paper describes HRV seen in healthy individuals compared to HRV in individuals known to have experienced a TBI. Three clinical case examples are discussed that explore the changes in heart rate variability following traumatic brain injury as well as improvements noted during, and following, a course of neurofeedback combined with HRV biofeedback training. The cases illustrated in this paper demonstrate the impressive changes in heart rate variability that can occur following a traumatic brain injury and also highlight how neurofeedback combined with biofeedback training can be used to improve heart rate variability and ameliorate related cognitive symptoms.

Applied Technology in Feasibility Assessment of Photoplethysmograph Computing for Heart Rate Variability Application

In response to the problem of mental or physical disorders due to the pace of modern busy life, the heart rate variability analysis is often used as an indicator for assessment. This study indicates the use of electrocardiogram (ECG) and photoplethysmograph (PPG) to capture the heart rate and conduct heart rate variability comparative analysis. We want to use photoplethysmograph to replace ECG for retrieving heart rate of patients. In our experiment, the first observation is to record the autonomic nervous system modulation caused by listening to music with different rhythms and doing exercise. Then we observe the correlation among different heart rate variability under different physical conditions after that. According to the results of the analysis shown that PPG calculated heart rate variability signals are highly related to the results obtained by ECG.

Application of heart-rate variability in patients undergoing weaning from mechanical ventilation

IntroductionThe process of weaning may impose cardiopulmonary stress on ventilated patients. Heart-rate variability (HRV), a noninvasive tool to characterize autonomic function and cardiorespiratory interaction, may be a promising modality to assess patient capability during the weaning process. We aimed to evaluate the association between HRV change and weaning outcomes in critically ill patients.MethodsThis study included 101 consecutive patients recovering from acute respiratory failure. Frequency-domain analysis, including very low frequency, low frequency, high frequency, and total power of HRV was assessed during a 1-hour spontaneous breathing trial (SBT) through a T-piece and after extubation after successful SBT.ResultsOf 101 patients, 24 (24%) had SBT failure, and HRV analysis in these patients showed a significant decrease in total power (P = 0.003); 77 patients passed SBT and were extubated, but 13 (17%) of them required reintubation within 72 hours. In successfully extubated patients, very low frequency and total power from SBT to postextubation significantly increased (P = 0.003 and P = 0.004, respectively). Instead, patients with extubation failure were unable to increase HRV after extubation.ConclusionsHRV responses differ between patients with different weaning outcomes. Measuring HRV change during the weaning process may help clinicians to predict weaning results and, in the end, to improve patient care and outcome.

Clinical Application of Heart Rate Variability after Acute Myocardial Infarction

Heart rate (HR) variability has been extensively studied in patients surviving an acute myocardial infarction (AMI). The majority of studies have shown that patients with reduced or abnormal HR variability/turbulence have an increased risk of mortality within few years after an AMI. Various measures of HR dynamics, such as time-domain, spectral, and non-linear measures of HR variability, as well as HR turbulence, have been used in risk stratification of post-AMI patients. The prognostic power of various measures, except of those reflecting rapid R–R interval oscillations, has been almost identical, albeit some non-linear HR variability measures, such as short-term fractal scaling exponent, and HR turbulence, have provided somewhat better prognostic information than the others. Abnormal HR variability predicts both sudden and non-sudden cardiac death after AMI. Because of remodeling of the arrhythmia substrate after AMI, early measurement of HR variability to identify those at high risk should likely be repeated later in order to assess the risk of fatal arrhythmia events. Future randomized trials using HR variability/turbulence as one of the pre-defined inclusion criteria will show whether routine measurement of HR variability/turbulence will become a routine clinical tool for risk stratification of post-AMI patients.

This work investigated the application of heart rate variability (HRV) formonitoring and recording the states of autonomic arousal of a driver in interstate travel a 725 km away. The HRV signal is used as a tool for asses- sing the performance of the nervous system in heart rate control. They took into account environmental variables resulting from this task. At five different moments of the trip were measured signals R-R of ventri- cular rate (interval between two R waves matches the frequency of ventricular depola- rization) and simultaneously performed a reaction time test driver. The results expressed strong correlations among the low- frequency heart rate variability (LF – Low Frequency) and high frequency heart rate variability (HF– High Frequency) with the reaction time, which indicates the need to invest in a possible continuous monitoring of HRV the driver as an indicator of his condition to drive. Tool that would work in preventing traffic accidents. We suggest the use of restorative environments on the road to avoid accidents of this kind.

Clinical Applications of Heart Rate Variability in the Triage and Assessment of Traumatically Injured Patients

Heart rate variability (HRV) is a method of physiologic assessment which uses fluctuations in the RR intervals to evaluate modulation of the heart rate by the autonomic nervous system (ANS). Decreased variability has been studied as a marker of increased pathology and a predictor of morbidity and mortality in multiple medical disciplines. HRV is potentially useful in trauma as a tool for prehospital triage, initial patient assessment, and continuous monitoring of critically injured patients. However, several technical limitations and a lack of standardized values have inhibited its clinical implementation in trauma. The purpose of this paper is to describe the three analytical methods (time domain, frequency domain, and entropy) and specific clinical populations that have been evaluated in trauma patients and to identify key issues regarding HRV that must be explored if it is to be widely adopted for the assessment of trauma patients.

Hemodynamic Monitoring Today

Hemodynamic monitoring has been part of the routine management of intensive care patients and high risk surgical patients since the advent of the pulmonary artery catheter (PAC) more than thirty years ago. The growing availability of new less invasive devices over the past decades has now made it possible to monitor cardiac output (CO) more often in the operating room, as well as in new clinical settings such as the emergency department. In this special issue papers have been arranged that discuss different aspects of hemodynamic monitoring, from technical issues to new clinical applications. The first paper titled “Perioperative intravascular fluid assessment and monitoring: a narrative review of established and emerging techniques” is a review on perioperative intravascular fluid assessment and monitoring. How cardiovascular physiology can be monitored is explained, and different technologies are presented. The second paper titled “Cardiac output assessed by invasive and minimally invasive techniques” is a review on all available technologies to monitor CO. The authors start from the PAC and carry on to present the more recent less invasive devices. The review covers technical aspects as well as clinical validation and use. The third paper titled “The effect of a hyperdynamic circulation on tissue Doppler values: a simulation in young adults during exercise” presents research done on healthy individuals undergoing strenuous exercise, using left ventricular tissue Doppler velocity (TDI), giving us new data on the use of TDI in hyperdynamic circulation. The fourth paper titled “The effect of airway pressure release ventilation on pulmonary catheter readings: specifically pulmonary capillary wedge pressure in a swine model” investigates the effect of the airway pressure release ventilation (APRV) on the PAC readings in an animal model. This papers gives new insights into the heart-lung interaction when using this new mode of ventilation. In the fifth paper titled “Clinical applications of heart rate variability in the triage and assessment of traumatically injured patients” heart rate variability (HRV) is explored. HRV represents a way of looking at the function of autonomic nervous system during stress conditions and has been studied in different areas as a predictor of morbidity and mortality. In this paper the authors focus on the use of HRV in trauma patients. The sixth paper titled “Recommendations for haemodynamic and neurological monitoring in repair of acute type A aortic dissection” gives a perspective on how hemodynamic monitoring should be used in conjunction with other clinical strategies. In this review the authors describe how hemodynamic monitoring can be combined with neurological monitoring in order to optimize the circulation guaranteeing adequate cerebral blood flow. The seventh paper titled “Assessing the left ventricular systolic function at the bedside: the role of transpulmonary thermodilution-derived indices” is a review on the use of transpulmonary thermodilution to measure CO and other derived variables.
