Effects of electroconvulsive therapy on functional brain networks in patients with schizophrenia

BackgroundSchizophrenia is a kind of intractable brain disorder. Electroconvulsive therapy (ECT) has been used to rapidly improve the clinical symptoms of patients with schizophrenia, but the effect of ECT on topological attributes of brain functional network in patients with schizophrenia has not been clear. The purpose of this study was to investigate the brain functional network mechanism of ECT against schizophrenia.MethodsThirty-one patients with schizophrenia and fifty healthy controls matching age, gender, and years of education were included. All participants underwent general data collection and magnetic resonance imaging scanning before ECT, and clinical symptoms were assessed using the Positive And Negative Syndrome Scale (PANSS). MRI and clinical symptoms were collected again after the first and eighth ECT application. The functional brain network was constructed on the basis of magnetic resonance imaging, and the global and node topological properties were analyzed. Repeated measure variance analysis was used to explore the changes of the topological attribute values and clinical symptom scores before and after ECT, and Bonferroni post hoc analysis was performed. The independent sample t-test was used to compare the differences in the topological attribute values between patients and healthy controls at three time points before and after ECT. Partial correlation analysis was performed for topological attribute values and clinical symptom scores of abnormal brain regions in the patient groups and their changes during ECT. A general linear regression model was used to predict the outcome after the final eighth ECT using the patient's response to the first ECT.Results(1) One ECT can restore the gamma(γ), lamuda(λ), sigma(σ), nodal global efficiency (Ne) of right insular gyrus ventral agranular insula (INS_R_vIa) and nodal local efficiency (NLe) of bilateral fusiform gyrus medioventral area37 (FuG_A37mv). Eight ECT can also restore the NLe of cortex rostral lingual gyrus (MVOcC _R_rLinG). Eight ECT did not improve the Ne of right superior parietal lobule rostral area 7 (SPL_R_A7r) and NLe of left superior frontal gyrus medial area 6 (SFG_L_A6m). (2) Even after only the first use of ECT, total PANSS scores began to decrease (mean ΔPANSSECT1 was 11.7%; Range, 2%-32.8%), decreased significantly after the eighth application (mean ΔPANSSECT8 was 86.0%; Range,72.5% to 97.9%). Five patients met the response criteria after ECT1 (20% reduction in PANSS total score), and all patients met the response criteria after ECT8. (3) Linear regression analysis showed that ΔPANSSECT1 was a significant predictor of ΔPANSSECT8 (F=5.387, P=0.028), and ΔPANSSECT1 explained 15.7% of the variance of ΔPANSSECT8 (R2=0.157).ConclusionsECT was able to normalize γ, λ, σ, Ne of INS_R_vIa, NLe of bilateral FuG_A37mv in SZ patients after the first treatment, and NLe of MVOcC_R_rLinG after the eighth ECT. ECT significantly alleviates psychotic symptoms in patients with SZ, and its efficacy after eight sessions can be predicted by the patient's response to the first session of ECT.

Effects of electroconvulsive therapy on cerebral A1 adenosine receptor availability: a PET study in patients suffering from treatment-resistant major depressive disorder.

Sleep deprivation and electroconvulsive therapy (ECT) effectively ameliorate symptoms in major depressive disorder (MDD). In rodents, both are associated with an enhancement of cerebral adenosine levels, which in turn likely influence adenosinergic receptor expression. The aim of the current study was to investigate cerebral A1 adenosine receptor (A1AR) availability in patients with MDD as a potential mediating factor of antidepressant effects of ECT using [18F]CPFPX and positron emission tomography (PET). Regional A1AR availability was determined before and after a series of ECT applications (mean number ± SD 10.4 ± 1.2) in 14 subjects (4 males, mean age 49.5 ± 11.8 years). Clinical outcome, measured by neuropsychological testing, and ECT parameters were correlated with changes in A1AR availability. ECT had a strong antidepressive effect (p < 0.01) while on average cerebral A1AR availability remained unaltered between pre-and post-ECT conditions (F = 0.65, p = 0.42, mean difference ± SD 3.93% ± 22.7%). There was no correlation between changes in clinical outcome parameters and regional A1AR availability, although individual patients showed striking bidirectional alterations of up to 30-40% in A1AR availability after ECT. Solely, for the mean seizure quality index of the applied ECTs a significant association with changes in A1AR availability was found (rs = -0.6, p = 0.02). In the present study, therapeutically effective ECT treatment did not result in coherent changes of A1AR availability after a series of ECT treatments. These findings do not exclude a potential role for cerebral A1ARs in ECT, but shift attention to rather short-termed and adaptive mechanisms during ECT-related convulsive effects.

Catatonia in depressive disorder, more usual than it is supposed to be

IntroductionCatatonia is a psychomotor syndrome characterized by various motor, affective and behavioral symptoms. It can occur as a cause of various underlying organic and psychiatric disorders. In Psychiatric nosology is used to specify a subtype of the disorder underlying.Unlike what was assumed in the past, today it is accepted that catatonia is more frequent in affective disorders than in schizophrenia. But despite this, diagnosis and treatment are still late in affective cases on many occasions.Objectives -A case of catatonia is presented to review the diagnostic difficulties that can sometimes entail.-Review treatment algorithm.MethodsWe present the case of a 62-year-old woman, initially diagnosed of major depressive symptoms with psychotic symptoms, showing no response to different treatments, evolving to catatonia, which is diagnosed after screening for neurological and medical diseases.ResultsThe patient had an adequate evolution after the withdrawal of antipsychotics and the application of ECT (Electroconvulsive therapy).Conclusions- It is important to carry out an adequate screening, because many times the symptoms are caused by medical or neurological diseases.-Catatonia has a good prognosis with an early treatment, but it may increase the risk of mortality after 5 days from the onset of symptoms.-It is important to avoid the use of antipsychotics or other dopamine blockers. Our anesthesia management in a bipolar pregnant case in third trimester

Bipolar disorder is one of the most common psychiatric disorders during pregnancy. Electroconvulsive therapy (ECT) is a potent treatment option for pregnant bipolar cases who do not respond to medical treatment and have a high risk of suicide. In this report, we aimed to present the anesthesia management of a 34-week pregnant woman with bipolar disorder who underwent ECT due to the increase in the frequency of manic episodes despite medical treatment. Induction of labor remains an important risk in the application of ECT in third-trimester pregnant women. Further large-scale randomized controlled studies using different anesthetic agents are needed.

Progressive Changes in Brain Regional Homogeneity Induced by Electroconvulsive Therapy Among Patients With Schizophrenia.

Electroconvulsive therapy (ECT) has significant effects on improving psychotic symptoms in schizophrenia (SZ), but the changes of brain function induced by it are unclear. The purpose of the study was to explore progressive ECT-induced changes in regional homogeneity (ReHo) at multiple time points before, during, and after a course of ECT. The 27 in-patients with SZ (SZ group) who met the recruitment criteria accepted clinical evaluations and resting-state functional magnetic resonance imaging scans before the first ECT (pre-ECT), after the first ECT (ECT1), and after the eighth ECT (ECT8), all conducted within 10 to 12 hours. Forty-three healthy controls (HCs; HC group) who matched well with the patients for age, sex, and years of education were recruited. For Positive and Negative Syndrome Scale (PANSS) and ReHo, progressive changes were examined. Pair-wise comparisons of patient pre-ECT, ECT1, and ECT8 ReHo values with HC ReHo values revealed that ECT normalized the ReHo values in bilateral superior occipital gyrus (SOG), right lingual gyrus (LG), left medial prefrontal cortex. Furthermore, improved ReHo in bilateral SOG and right LG appeared after the first ECT application. The ReHo values in right middle occipital gyrus, right middle temporal gyrus, and right inferior parietal lobule were not significantly altered by ECT. The total PANSS score was lower even after the first ECT application (mean ΔPANSSECT1, 11.7%; range, 2%-32.8%) and markedly reduced after the eighth application (mean ΔPANSSECT8, 86.3%; range, 72.5%-97.9%). The antipsychotic effects of ECT may be achieved through regulating synchronization of some regions such as bilateral SOG, right LG, and left medial prefrontal cortex. Furthermore, the enhanced synchronizations also take place in other regions.

Image testing in psychiatrics: a bibliografic review

IntroductionPsychoradiology is a term that describes a growing interest in relating psychiatry and radiological images, proposing a radiological approach in the management of major psychiatric illnesses. This includes the diagnosis, the planning of the treatment and the study of the clinical course.ObjectivesThe objective of this communication is to review the current status of the importance and indications of neuroimaging tests in psychiatry.MethodsA literature review has been carried out to review this issue.Results In schizophrenia, longitudinal studies have been carried out that compare the anatomical structures between a first psychotic episode and in a chronic state, locating regional changes that progress as the disease does. Anatomical alterations have also been detected among patients with a predominance of positive symptoms or negative symptoms. Although more and more studies demonstrate a certain common genetic and radiological basis between bipolar disorder and schizophrenia, imaging techniques can also show specific findings that differentiate one pathology from the other. The neuroimaging tests used in psychiatry are: • Brain CT, recommended when a first psychotic episode is suspected. • MRI: recommended in processes of cognitive deterioration, to evaluate white matter and for pregnant patients. It is also recommended to evaluate injuries that could have a poor prognosis with the application of electroconvulsive therapy. • Functional tests (PET and SPECT) are often used to screen some types of dementia such as Alzheimer’s or for research.ConclusionsNew advances and knowledge in psychiatry and radiology must be integrated for better clinical practice.DisclosureNo significant relationships.

Turkish Validity and Reliability Study of the ECT Perception and Knowledge Scale.

Electroconvulsive Therapy (ECT) is an effective and safe treatment method used in the treatment of various psychiatric diseases. However, negative attitudes associated with ECT are common. This causes many negative consequences, from the treatment preference to treatment response and stigma. In this study, we aimed to carry out a validity-reliability analysis of the ECT Perception and Knowledge Scale (ECT-PK), which was developed to determine the perception and knowledge levels related to ECT, and adapt it to Turkish. The Turkish adaptation of the ECT-PK was made using the translation-retranslation method. Our study included 50 patients with schizophrenia, 50 patients with bipolar disorder, 50 patients with major depression who met the remission criteria determined separately for each disorder, and 150 healthy controls.. To measure test-retest reliability, the scale was re-applied to 30 patients randomly selected from the patient group 14-21 days after the first application of the scale. In our study, a significant difference was found in both the patient and control groups in terms of the history of ECT application in the past and the status of accepting ECT application when recommended, and the perception and knowledge subscales of the ECT-PK. These results support the construct and criterion validity of the ECT-PK. Cronbach's alpha coefficient was found to be 0.85 for the perception subscale and 0.78 for the knowledge subscale. The intra-class correlation coefficient used to evaluate the test-retest reliability was 0.86 for the perception scale and 0.83 for the knowledge subscale. It has been shown that the ECT-PK is a valid and reliable measurement tool that can be used to measure the perception and knowledge levels related to ECT in both clinical and non-clinical groups.

Risk Assessment of Electroconvulsive Therapy in Clinical Routine: A 3-Year Analysis of Life-Threatening Events in More Than 3,000 Treatment Sessions.

Background: Extensive research has reported that electroconvulsive therapy (ECT) can be highly effective in approximately 80% of patients suffering from depression. Its clinical use is mainly limited by historical objections and the concern about unwanted adverse effects (AEs), including serious and potentially life-threatening adverse events (pLTAEs), induced either by ECT or by anesthesia. Objective risk estimation is, therefore, a decisive factor in determining an indication for ECT.Methods: This paper presents a retrospective analysis of 3-year safety protocols and patient files of 157 patients who received a total of 3,106 ECT applications in a psychiatric inpatient setting at a psychiatric community hospital. This patient group comprises 5.3% of inpatients admitted with comparable diagnoses. Adverse events were analyzed from standardized safety protocols and patient files with a focus on pLTAEs.Results: Adverse events were reported for 30 (19.1%) of the 157 participants during 39 (6.1%) of 641 hospital stays. Serious pLTAEs occurred during three electroconvulsive stimulations in three patients, who needed action through the administration of medication or mechanical respiration. No patient suffered permanent damage to health, and no patient died. The incidence of these and other AEs was independent of sex, age, and diagnosis of patients, and anesthesia medication. Minor AEs occurred more often with higher stimulus doses and an increasing number of treatments.Conclusion: The low incidence rate of 0.097% of serious pLTAEs that require medical action may allow the conclusion that ECT is a rather safe treatment when performed in a controlled setting. The beneficial risk profile of ECT performed in the standard care of psychiatric hospitals suggests a more generous indication of this treatment method. We recommend that ECT facilities collect individual safety data to allow a reliable judgment of their institutional ECT risk profile.

Electroconvulsive therapy: a clinical case study and review

Electroconvulsive therapy is deemed one of the most effective treatments for alleviating symptoms in individuals diagnosed with severe depression, yet it remains one of the most controversial psychiatric treatments in contemporary mental health nursing practice. As such, its use and efficacy continue to be questioned. The following article analyses the case of Albert, a fictional character who represents the presentation of several patients who have received electroconvulsive therapy. The application of electroconvulsive therapy is evaluated, by referring to the most recent policy and national guidelines. Despite the associated stigma, electroconvulsive therapy can save lives, as portrayed by Albert. However, further research is required and updated guidance from National Institute for Health and Care Excellence is needed, specifically with regards to maintenance and relapse prevention. It is important to be aware of the implications of side effects, just as with other interventions. Overall, this article highlights the importance of safe, recovery-focused and person-centred care.

Use of electroconvulsive therapy in adolescents - A retrospective survey on 12- to 17-year-old patients at three university hospitals in Germany Abstract. Abstract. Background: Electroconvulsive therapy (ECT) is a highly effective and well-researched therapy in adult psychiatry and has been successfully used especially as a treatment for severe depressive, catatonic, and psychotic disorders. Although severe disease progressions also occur in child and adolescent psychiatry, ECT is used much less frequently there. This may be because hardly any data have been collected on the use, effectiveness, and tolerability of ECT in child and adolescent psychiatric patients. This article outlines the application, effectiveness, and tolerability of ECT when applied to young adolescents in Germany. Methods: A retrospective survey on ECT in 29 patients under 18 years of age was conducted at three German university centers. All documented cases were recorded and evaluated for effectiveness and tolerability. In addition, a comprehensive PubMed-based database search was carried out. Results and conclusions: Internationally, there are no meta-analyses or randomized controlled studies and hardly any published cases on electroconvulsive therapy in German child and adolescent psychiatry. Our data on ECT show high efficacy in previously treatment-resistant and severely ill patients. Side effects occurred rarely. There was no evidence of differences between adults and adolescents in indication (depression, catatonia, schizophrenia), effectiveness, tolerability, and negative predictors of response to ECT. The results also suggest that the use of ECT in adolescents should be considered earlier in the treatment course.

Electroconvulsive Therapy Applications on Children in the 1940s: The Italian Case.

The existence of pediatric applications of electroconvulsive therapy (ECT) in the 1940s in Italy has been neglected by international literature. However, 2 case reports by Ferdinando Accornero and Mario Anderson, both assistants of Ugo Cerletti at the Sapienza Clinic for Nervous and Mental Diseases, should be brought to the attention of historians of ECT. The work presented therein began in Rome in September 1940, approximately at the same time of the first reported ECT administration on a child in Bristol, United Kingdom. The 2 reports described applications on 19 children and adolescents during 3 different sessions (1940-1941, 1944-1947, and 1947-1949), with a 3-year-long interruption due to the circumstances of World War II. Unfortunately, this research appeared belatedly in 2 articles from 1948 and 1950, respectively, in an Italian journal with limited distribution, when more extensive research contributions on pediatric ECT had already received international exposure. The Italian reports may cast light on the progress made by the early generation of ECT researchers in the 1940s toward the refinement of ECT techniques and the subsequent identification of diagnostic indicators for ECT among children.

Electroconvulsive Therapy and Movement Disorders. New Perspectives on A Time-Tested Therapy.

Electroconvulsive therapy (ECT) has been a very well known therapy in Psychiatry for over 80 years. ECT is considered useful in treating acute mania, severe depression and other psychiatric conditions. Over time, this therapy has also been used in several movement disorders including Parkinson disease (PD) and Huntington disease (HD). In this brief review, I summarize the recent History and evolution of ECT, its proven and potential applications in movement disorders as well as its potential mechanisms.

201 - Behavioral and Psychological Symptoms of Dementia: a symposium of the IPA BPSD Shared Interest Forum

Section A. Psychotropic drug use (20 minutes)1) Sytse Zuidema: The RID (Reduction of inappropriate psychotropic drugs in dementia) study Nursing home residents with dementia and neuropsychiatric symptoms are prescribed psychotropic drugs too often and also sometimes inappropriate. Multidisciplinary interventions can help to facilitate better care by using psychosocial interventions and help to decrease inappropriate psychotropic drug use. We supported implementation of such interventions through participatory action research, to facilitate nursing homes to design, and implement their own local interventions. The results of the RCT (including a process evaluation) will be presented.2) Tzung-Jeng Hwang: dextromethorphan for treating agitationAgitation is highly prevalent in patients with dementia and associated with significant impairment of quality of life, early institutionalization, and higher mortality. Although no pharmacotherapy has been approved by the US FDA for dementia patients with agitation, off-label use of antipsychotic agents is common. But antipsychotic agents are clearly associated with increased adverse effects, including stroke and death. Dextromethorphan (DXM) is a low-affinity uncompetitive N-methyl-D-aspartate (NMDA) receptor antagonist, a high-affinity sigma-1 receptor agonist, a serotonin and norepinephrine reuptake inhibitor, and a nicotinicα3β4 receptor antagonist. The combination of DXM and low dose quinidine (Q) has been approved for the treatment of pseudobulbar affect in 2010. A recent study found that DXM/Q may be effective for the management of agitation in patients with Alzheimer disease (AD). In this talk, a pilot study on using DXM alone to treat agitation in dementia will be reported. Overall, the use of DXM 60-240 mg/day resulted in a high proportion of improvement (CGI-agitation: 69% much improved, 25% minimally improved). No severe adverse events were found. The results support DXM may be efficacious and safe.3) Marie Andree Bruneau: OPUS-APOPUS-AP aims at improving resident care through increased staff’s knowledge and competency, resident-centered approaches, nonpharmacologic interventions, and antipsychotic deprescribing in inappropriate indications. OPUS-AP is implemented through integrated knowledge translation and mobilization activities. Antipsychotic, benzodiazepine, antidepressant prescriptions and BPSD were evaluated every 3 months for 9 months. Phase 2 of OPUS-AP was conducted in 129 LTC centres in Quebec, Canada, from April to December 2019. At baseline (April 2019), 10,601 residents were admitted on OPUS-AP participating wards from which 74% had a diagnosis of major neurocognitive disorder (MNCD) and 47% an antipsychotic prescription. The follow-up cohort included 4,087 residents with both MNCD and antipsychotic prescription. Among the 1381 residents in whom antipsychotic deprescribing was attempted between baseline and 9 months and still included at 9 months, successful deprescribing was achieved in 79,2% (cessation 44,8% or dose decrease 34,5%)). No increase in benzodiazepine or antidepressant prescriptions nor worsening of BPSD were observed.Section B. In some severe cases, pharmacological and other interventions are not effective in improving these symptoms. A recent systematic review suggested that electroconvulsive therapy (ECT) could be an effective treatment for severe and treatment-refractory agitation and aggression in dementia, with few adverse consequences. We need to have controlled studies, address the stigma of ECT and develop professional guidelines for the application of ECT for agitation and aggression in dementia, particularly because most patients are not able to provide informed consent.2) Discussion and making plan for international survey on application of ECT in BPSD

The Effects of Ultrasound, Infrasound, and Electroconvulsive Stimulations on Depression-like Behavior in Mice

Abstract Introduction: In the present study, the effects of ultrasound, infrasound, and electroconvulsive stimulation on depression-like behavior was assessed in animal models. Method: For carrying out this study, 60 male BALB/c mice (mean age of 60 days, weight range of 25 – 30 g) were randomly selected. Depression was induced using reserpine 0.1 mg/kg i.p. for 30 days. They were allocated to three groups of experimental (ultrasound, infrasound, and electroconvulsive stimulation) and one group of control animals, each including 15 mice. Experimental animals received ultra- or infrasound 0.5 hours or 1 electroconvulsive pulse, daily for 10 days. Finally the Forced Swim Test was carried out. Results: There was a statistically significant difference between the groups regarding the duration of immobility posture [F(3,54) = 99.54, P = 0.001]. Mean immobility time was significantly longer in the control group compared to the other groups. Also, immobility was significantly longer for electroconvulsive compared to the ultra- and infrasound groups. Group ultrasound showed longer immobility than group infrasound; however, the difference was not significant. Conclusion: Ultrasound and infrasound stimulations are capable of decreasing depression-like behavior in mice. The results of this study were also compatible with the application of electroconvulsive therapy. However, a more successful response can be exploited with ultra- and infrasound stimulations.

Electroconvulsive Therapy in Germany: Development Over 8 Years With a Background of 4 Decades.

The use of electroconvulsive therapy (ECT) in Europe varies broadly between the extremes of Sweden on one side and Italy on the other. Here, we document the frequency, practice and standards of ECT in Germany in 2016, compare it with a survey of 2008, and describe the historical background. A questionnaire was sent electronically to the 396 psychiatric institutions in Germany. Repeated follow-up calls yielded a complete list of hospitals performing ECT and a high rate of detailed responses. Forty-five percent (178/396) of the psychiatric hospitals performed ECT. Of those, 73.6% (131/178) completed the questionnaire. Based on 2 procedures, we estimated an incidence of 60,000 single treatments and 5700 patients treated, with a rate of 6.9 patients per 105 inhabitants. As in 2008, regional differences proved to be significant. The rate at which patients are referred to ECT hospitals from other clinics for the special purpose of ECT has increased since 2008. Maintenance treatments and the use of ketamine for anesthesia have gone up considerably while fewer hospitals offered outpatient treatments. The application of ECT has increased markedly in the last 8 years, showing an exponential growth over the last 4 decades in Germany. The accessibility to the treatment seems to have improved and some global trends have been well received. Electroconvulsive therapy has become a modern, increasingly used medical intervention.

Vagus Nerve Stimulation and External Defibrillation during Resuscitation; a Letter to Editor.

Dear Editor; External defibrillation in patients with implanted neuromodulatory devices is a crucial therapeutic challenge. We report a 63-year-old male patient with refractory epilepsy (RE) after recurrent ischaemic strokes in the middle cerebral artery and in the anterior cerebral artery territory 26 and 23 years ago. He received various therapeutic interventions to achieve seizure control with insufficient success. Therefore, vagus nerve stimulation (VNS) (model Pulse 102, Aspire SR, Cyberonics Inc, Houston, Texas) was applied via insertion of a pulse generator in the left upper chest in 2011. VNS stimulation settings were: output current 1.0 mA, pulse width 500 µs, frequency 30 Hz, 30s ON, 3.0 minute OFF. A reduction of seizure frequency was achieved. Last antiepileptic therapy consisted of levetiracetam 1500 mg td, valproate 1000 mg td, and eslicarbazepine 800 mg td. In 2016, he was admitted because of generalized seizure and aspiration pneumonia. During hospital stay he suffered a cardiac arrest (CA) with pulseless ventricular tachycardia (VT) caused by fulminant pulmonary artery embolism. After successful resuscitation, the patient experienced return of spontaneous circulation (ROSC). During resuscitation, biphasic electric shocks were applied using 150 Joule and subsequently 360 Joule with patches placed approximately 10 cm parasternal and at the left chest below the VNS. After successful ROSC the VNS was checked again and no malfunction could be detected. Stimulation settings were not changed. Impedance was normal. Seizures were not observed during the remaining day. Unfortunately, the patient died within one day after successful resuscitation and ROSC because of therapy refractory circulatory insufficiency. VNS is an established therapeutic approach in treatment of TRE in children and adults to achieve reduction of seizure frequency with proven safety and efficacy. The safety of VNS in emergency situations like cardiac arrest due to VF with need of external defibrillation and application of large amounts of electrical energy is not clear. External defibrillation in VNS patients may potentially be harmful. The literature concerning external defibrillation during resuscitation or external cardioversion (EC) in patients with implanted electronic devices in neurological disorders is sparse. In patients with cardiac pacemakers external defibrillation may damage the cardiac device. EC applied to deep brain stimulation (DBS) patients may cause thalamotomy or DBS failure. Application of electroconvulsive therapy in psychiatric disorders seems to be safe. Two cases of electroconvulsive therapy (ECT) in VNS have been reported by Sharma et al. The first patient was a 66-year old female with major depression and the second one, a 57-year-old male with a history of bipolar disorder. Both had VNS for therapy refractory psychiatric illness. ECT was applied because of further worsening of the mental state without malfunction of the VNS device. To our knowledge, this is the first case of external defibrillation in a patient with VNS for TRE without alteration of the neurostimulator’s function. External defibrillation applied to VNS patients seem to be safe and effective. Several steps should be taken to minimize the electrical current flowing through the neurostimulator. First, position the patches as far away as possible from the VNS at least 10 cm each. Second, position the patches perpendicular to the VNS; use the lowest clinically appropriate output setting, and, finally, confirm that the VNS is functioning properly after defibrillation.

Electroconvulsive Therapy in the Treatment of Mood Disorders: One-Year Follow-up.

Electroconvulsive therapy (ECT) is known to be an effective option in the treatment of mood disorders, especially resistant depression. However, the remission achieved by ECT was reported to be not long lasting enough. The aim of the present study was to investigate the relapse/recurrence rates and associated risk factors during the first year after ECT in patients diagnosed with mood disorders. In a naturalistic observation, patients diagnosed with unipolar depressive disorder or a depressive episode of bipolar disorder and who had achieved remission by ECT were followed up for at least one year. The patients were evaluated with structured interviews during the follow-up period. The relapse/recurrence rates were the primary outcome measurements, while hospitalization and suicide attempts were the secondary outcome measurements. The remitted and non-remitted patients were compared regarding the clinical features, ECT, and pharmacological variables. Fifty of 62 patients who had achieved remission with ECT completed the one year follow-up period. Thirty-three patients (66%) had relapse/recurrence, while 17 (34%) patients remained in remission. The relapse rates were similar in patients with unipolar depression and bipolar disorders. The mean number of ECT sessions was higher in relapsed patients with bipolar disorders. Multiple episodes were more frequent in non-remitted patients with unipolar depression. Comorbid psychiatric diagnosis was higher in non-remitted patients with unipolar and bipolar disorders. The relapse/recurrence rate was found to be fairly high in the first year of follow-up in patients who had achieved remission with ECT. ECT decisions should be made carefully in patients with comorbid psychiatric diagnosis and multiple episodes as these are more risky. The ECT application procedure and successive maintenance treatment (maintenance ECT, pharmacotherapy, and psychotherapy) should be planned to sustain the remission for patients with mood disorders in long-term follow-up.

MODELLING GAIT SYNDROME IN HUNTINGTON’S DISEASE: THE GENETIC ALGORITHM APPROACH

Huntington’s disease (HD) which usually affects the patients at middle age results from malfunctioning of the basal ganglia. It is characterized by cognitive impairment, involuntary movements, neuropsychiatric and psychological disturbances. Early motor signs of Huntington’s disease typically include the gradual onset of clumsiness, balance difficulties, and brief, random, fidgeting movements. A popular approach to solving symptoms arising from HD has been through the administration of drugs. But drugs debase human activities, thus the application of electroconvulsive therapy. This work proposes a genetic algorithmic (GA) simulation of chorea in HD patient as a pedestal for the design of a therapeutic device aimed at managing the phenomenon. Earlier efforts have led us to the formulation of such models in recent past. Clinical factors related to acute electroconvulsive therapy outcome for patients with major depressive disorder.

The aim of this study was to explore the significant predictors associated with electroconvulsive therapy (ECT) outcome for patients with major depressive disorder. Major depressive disorder inpatients (N=130) requiring ECT were recruited from a major psychiatric center in South Taiwan. ECT was generally performed for a maximum of 12 sessions. Symptom severity was assessed using the 17-item Hamilton Depression Rating Scale (HAMD-17) and Clinical Global Impression of Severity (CGI-S) before ECT, after every three ECT sessions, and after the last ECT. The generalized estimating equations method was used to analyze the influence of potential variables over time on the HAMD-17 and CGI-S, respectively. Fourteen patients not completing the first three sessions of ECT were excluded. The remaining 116 patients were included in the analysis. Patients with treatment-resistant depression, longer duration of the current depressive episode, and higher levels of pain were more likely to have less symptom reduction after acute treatment with ECT, irrespective of how the depressive symptoms were rated using HAMD-17 or CGI-S. To improve efficacy, earlier application of ECT and pain control should be considered during an acute course of ECT. Other clinical predictors related to ECT outcome require further investigation in future studies.

Electroconvulsive therapy management in benzodiazepine-resistant catatonic syndrome: A Case report

Catatonia is a rare but potentially lethal neuropsychiatric syndrome. Despite its historical association with schizophrenic disorders, it is more frequent in affective ones, and is currently considered an independent pathological entity. The basis of the treatment, regardless of the cause, is the use of benzodiazepines and electroconvulsive therapy (ECT), without a clear consensus on the combined treatment. Regarding ECT, the frequency and number of effective sessions has not been clearly established. Therefore, clinical evolution is the main factor to be considered in order to determinate the appropriate treatment regimen, although the daily application of ECT is preferred, at least for the first week. We report the case of a 41-year-old patient with paranoid schizophrenia, who presented with a benzodiazepine resistant catatonic syndrome. The clinical picture included stupor, mutism, negativism, severe stiffness, catalepsy, waxy flexibility and diaphoresis, with slight CPK increase but with no other extrapyramidal symptoms, fever more than 39 and hemodynamic instability, which allowed to exclude a neuroleptic malignant syndrome. A blood analysis, lumbar puncture, CT, EEG and viral serologies were performed with inconclusive results. The patient required ICU admission and ECT treatment and we used the Bush-Francis Catatonia Rating Scale to evaluate the evolution of symptoms. Six daily treatments with ECT led to an almost full recovery of the patient. Further case series regarding the clinical management of this syndrome are needed, in order to reach consensus on an effective ECT regimen.Disclosure of interestThe authors have not supplied their declaration of competing interest.
