From images to insights: a neuroradiologist's practical guide on white matter fiber tract anatomy and DTI patterns for pre-surgical planning.

Diffusion tensor imaging (DTI) is a valuable non-invasive imaging modality for mapping white matter tracts and assessing microstructural integrity, and can be used as a "biomarker" in diagnosis, differentiation, and therapeutic monitoring. Although it has gained clinical importance as a marker of neuropathology, limitations in its interpretation underscore the need for caution. This review provides an overview of the principles and clinical applicability of DTI. We focus on major white matter fiber bundles, detailing their normal anatomy and pathological DTI patterns, with emphasis on tracts routinely requested in our neurosurgical department in the preoperative context (uncinate fasciculus, arcuate fasciculus, pyramidal pathway, optic radiation, and dentatorubrothalamic tract). We guide neuroradiologists and neurosurgeons in defining volumes of interest for mapping individual tracts and demonstrating their 3D reconstructions. The intricate trajectories of white matter tracts pose a challenge for accurate fiber orientation recording, with each bundle exhibiting specific characteristics. Tracts adjacent to brain lesions are categorized as displaced, edematous, infiltrated, or disrupted, illustrated with clinical cases of brain neoplasms. To improve structured reporting, we propose a checklist of topics for inclusion in imaging evaluations and MRI reports. DTI is emerging as a powerful tool for assessing microstructural changes in brain disorders, despite some challenges in standardization and interpretation. This review serves an educational purpose by providing guidance for fiber monitoring and interpretation of pathological patterns observed in clinical cases, highlighting the importance and potential pitfalls of DTI in neuroradiology and surgical planning.

Toward diffusion tensor imaging as a biomarker in neurodegenerative diseases: technical considerations to optimize recordings and data processing.

Neuroimaging biomarkers have shown high potential to map the disease processes in the application to neurodegenerative diseases (NDD), e.g., diffusion tensor imaging (DTI). For DTI, the implementation of a standardized scanning and analysis cascade in clinical trials has potential to be further optimized. Over the last few years, various approaches to improve DTI applications to NDD have been developed. The core issue of this review was to address considerations and limitations of DTI in NDD: we discuss suggestions for improvements of DTI applications to NDD. Based on this technical approach, a set of recommendations was proposed for a standardized DTI scan protocol and an analysis cascade of DTI data pre-and postprocessing and statistical analysis. In summary, considering advantages and limitations of the DTI in NDD we suggest improvements for a standardized framework for a DTI-based protocol to be applied to future imaging studies in NDD, towards the goal to proceed to establish DTI as a biomarker in clinical trials in neurodegeneration.

Ultrafast diffusion tensor imaging based on deep learning and multi-slice information sharing

Objective. Diffusion tensor imaging (DTI) is excellent for non-invasively quantifying tissue microstructure. Theoretically DTI can be achieved with six different diffusion weighted images and one reference image, but the tensor estimation accuracy is poor in this case. Increasing the number of diffusion directions has benefits for the tensor estimation accuracy, which results in long scan time and makes DTI sensitive to motion. It would be beneficial to decrease the scan time of DTI by using fewer diffusion-weighted images without compromising reconstruction quality. Approach. A novel DTI scan scheme was proposed to achieve fast DTI, where only three diffusion directions per slice was required under a specific direction switching manner, and a deep-learning based reconstruction method was utilized using multi-slice information sharing and corresponding T 1-weighted image for high-quality DTI reconstruction. A network with two encoders developed from U-Net was implemented for better utilizing the diffusion data redundancy between neighboring slices. The method performed direct nonlinear mapping from diffusion-weighted images to diffusion tensor. Main results. The performance of the proposed method was verified on the Human Connectome Project public data and clinical patient data. High-quality mean diffusivity, fractional anisotropy, and directionally encoded colormap can be achieved with only three diffusion directions per slice. Significance. High-quality DTI-derived maps can be achieved in less than one minute of scan time. The great reduction of scan time will help push the wider application of DTI in clinical practice.

Clinical applications of skeletal muscle diffusion tensor imaging.

Diffusion tensor imaging (DTI) may allow the determination of new threshold values, based on water anisotropy, to differentiate between healthy muscle and various pathological processes. Additionally, it may quantify treatment monitoring or training effects. Most current studies have evaluated the potential of DTI of skeletal muscle to assess sports-related injuries or therapy, and training monitoring. Another critical area of application of this technique is the characterization and monitoring of primary and secondary myopathies. In this manuscript, we review the application of DTI in the evaluation of skeletal muscle in these and other novel clinical scenarios, with emphasis on the use of quantitative imaging-derived biomarkers. Finally, the main limitations of the introduction of DTI in the clinical setting and potential areas of future use are discussed.

A variety of DTI parameters have been studied in various articles. The standard parameters are fractional anisotropy (FA) values, apparent diffusion coefficient (ADC) values, radial diffusivity values, and axial diffusivity values, followed by tractography. FA and ADC values are the most commonly used parameters. The findings observed in most of the studies are increased FA and reduced ADC values following injury to the spinal cord. DTI data metrics possess the potential to become a potent clinical tool in patients with SCI. It is helpful for diagnosis, prognosis, and treatment planning, as well as for evaluating recovery. Nonetheless, to overcome the limitations and determine its liability clinically, more research has to be performed.

Application of diffusion tensor imaging and functional alterations in evaluating brain alterations related to heatstroke in a rat model

ObjectivesTo assess the alterations in resting-state functions and neural structures in the brain of a heatstroke rat model and explore the underlying relationship. MethodsIn total, 17 male Sprague Dawley rats were randomly divided into a control group (CTRL, n = 7) and a heatstroke group (HS, n = 10). All rats underwent 7.0 T magnetic resonance imaging (MRI). T2-weighted imaging, resting-state functional MRI (rs-fMRI), and diffusion tensor imaging (DTI) were obtained. On day 25, the surviving HS group rats (the follow-up group, FU, n = 7) were scanned again. ResultsHeatstroke resulted in functional alterations and structural damage in the cerebellar molecular layer (CML), right perirhinal area (PA), pretectal region (PR), right dentate gyrus, and external cortex of the inferior colliculus (ECIC). Further functional changes occur in the right temporal associative cortex (TAC), left retrosplenial cortex (RC), and CML during convalescence. The fractional anisotropy values were significantly positively correlated with the amplitude of low-frequency fluctuation (ALFF) (HS–CML: r = 0.746, p = 0.034; right PR: r = 0.648, p = 0.049; FU–right PA: r = 0.817, p = 0.025)/regional homogeneity (ReHo) ratio (HS–CML: r = 0.833, p = 0.008; ECIC: r = 0.678, p = 0.045) and negatively correlated with the ALFF (FU–left RC: r = −0.818, p = 0.024; right TAC: r = −0.813, p = 0.049). ConclusionDTI and rs-fMRI allow meticulous monitoring of the progression of neurological and functional alterations in the brain after heatstroke.

Applications of diffusion tensor imaging integrated with neuronavigation to prevent visual damage during tumor resection in the optic radiation area.

BackgroundIntracranial tumors involving the temporo-occipital lobe often compress or destroy the optic radiation (OpR), resulting in decreased visual function. The aim of this study is to explore the value of diffusion tensor imaging (DTI) tractography integrated with neuronavigation to prevent visual damage when resecting tumors involving the OpR and find potential factors affecting patients’ visual function and quality of life (QOL).MethodsOur study is a cross-sectional study that included 28 patients with intracranial tumors in close morphological relationship with the OpR recruited between January 2020 and February 2022. The surgical incision and approach were preoperatively designed and adjusted according to the DTI tractography results and visual function scores. All patients underwent examinations of visual acuity (VA) and visual field index (VFI) and completed visual function and QOL scales at admission and 2 months after discharge. Logistic regression and linear regression analysis were conducted to evaluate clinical factors potentially affecting pre/postoperative OpR morphology, VA, VFI, visual function, and QOL.ResultsLesion size was the main factor found to affect visual function (β = -0.74, 95%CI: -1.12~-0.36, P = 0.05), VA (left: β = -0.11, 95%CI: -0.14~-0.08, P < 0.001; right: β = -0.15, 95%CI: -0.17~-0.13, P < 0.001), and VFI (left: β = -0.11, 95%CI: -0.14~-0.08, P < 0.001; right: β = -0.14, 95%CI: -0.16~-0.12, P < 0.001). Lesion size, edema, and involvement of the lateral ventricle temporal horn were factors affecting OpR morphology and QOL. The 28 patients showed significantly improved VA, VFI, visual function, and QOL results (P < 0.05) 2 months after discharge.ConclusionsCombining DTI of OpR mapping and microscopic-based neuronavigation aided precise mapping and thus preservation of visual function in patients undergoing tumor resection. Potential clinical factors affecting patients’ visual function and QOL scores were identified which are useful for assessing a patient’s condition and predicting prognosis.

Application of DTI and fMRI in moyamoya disease.

Moyamoya disease (MMD) is a chronic and progressive cerebrovascular stenosis or occlusive disease that occurs near Willis blood vessels. Diffusion tensor imaging (DTI) and functional magnetic resonance imaging (fMRI) are used to detect the microstructure of white matter and the function of gray matter, respectively. The damage of these structures will lead to the change of cognitive level in patients with moyamoya disease. In this paper, the principles of DTI and fMRI, their applications and challenges in moyamoya disease are reviewed.

Quantitative evaluation of intraorbital optic nerve in optic atrophy using diffusion tensor imaging

The aim of this study is to quantitatively investigate the microstructural properties of the optic nerve (ON) in vivo using diffusion tensor imaging (DTI) in patients with unilateral optic atrophy (OA) and to determine their association with retinal nerve fiber layer (RNFL) thickness of the optic nerve head (ONH). Six patients with unilateral OA and 11 control subjects underwent DTI. ONs from ONH to the orbital apex were tracked. Fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (AD), and radial diffusivity (RD) were computed in both ONs and their correlation with RNFL thickness measured using optical coherence tomography was also analyzed. FA of atrophic ON was lower than that of non-affected and control ONs (atrophic [A], 0.136 ± 0.059; non-affected [N], 0.384 ± 0.048; control [C], 0.389 ± 0.053). MD and RD of atrophic ONs were higher than those of non-affected and control ONs (MD, A, 0.988 ± 0.247; N, 0.658 ± 0.058; C, 0.687 ± 0.079; RD, A, 0.920 ± 0.247; N, 0.510 ± 0.054; C, 0.532 ± 0.078). All DTI measures of atrophic ON except for AD showed a significant correlation with RNFL thickness of ONH; FA showed the strongest correlation, followed by RD and MD (FA, R2 = 0.936, P < 0.001; RD, R2 = 0.795, P < 0.001; MD, R2 = 0.655, P = 0.001). This study reports quantitative analysis of the ON using DTI and differences in DTI measures between atrophic and normal ONs. The significant correlation between DTI measures and RNFL thickness suggests the applicability of DTI as a clinical tool to evaluate the ON.

Clinical Application of Diffusion Tensor Imaging for a Brachial Plexus Injury.

Brachial plexus injuries are commonly diagnosed clinically, as conventional imaging has a low sensitivity. In recent years, diffusion tensor imaging has established a clinical role in the study of the central nervous system and, while still presenting some limitations due to the technical complexity of the acquisition method, is showing promising results when applied to peripheral nerves. Moreover, deterministic fiber tracking with the Euler’s method and multishell acquisition are two novel advances in the field which contribute to enhancing the reliability of the technique reducing the respiratory and inhomogeneity artifacts in this “magnetically complex” region, and better isolating the fibers in a heterogeneous territory. Here, we report a case of brachial plexus traumatic injury, a healthy reference subject, and details on the acquisition protocol of the reconstruction algorithm.

Diffusion tensor imaging in injuries of the cervical spine in children

Introduction. The technique of diffusion tensor imaging (DTI) is widely used in brain examination. However, DTI application for examining the spinal cord, especially in children, is not easy: small dimensions of the spinal cord with a typical low signal-to-noise ratio, artifacts from the respiration and pulsation of the heart and large vessels, as well as from swallowing movements. EPI sequences used to obtain diffusion indices cause eddy current distortions.Objective. To study changes in DTI parameters of the spinal cord in children with cervical spinal cord injury.Material and methods. 56 children aged 2–17 years with cervical spine injury (CSI) and 20 children without CSI were examined with Phillips Achieva3 T magnetic resonance scanner. The protocol consisted of sagittal STIR, sagittal and axial T1- and T2WI SE and axial DTI. To assess DTI, the following parameters were calculated: apparent diffusion coefficient (ADC), fractional anisotropy (FA), axial (AD) and radial (RD) diffusion coefficients.Results. By ASIA criteria, 29 (51%) children out of 56 had CSI with neurological complications; 27 (49%) had no complications. Neurological dysfunction of degree A was in 13 patients; B – in 3; C – in 9; D – in 4; E – in 27. Average values of diffusion in patients were: ADC = 0.74 ± 0.12 • 10−3 mm2/s−1, FA = 0.36 ± 0.07, BP = 1.15 + 0.28 • 10−3 mm2/s−1, RD = 0.52 + 0.32 • 10−3 mm2/s−1.Conclusion. DTI can detect changes which cannot be detected by conventional MRI. Low ADC values in the acute injury may indicate spinal cord injury and predict a negative functional outcome.

Application of Diffusion Tensor Imaging Based on Automatic Fiber Quantification in Alzheimer's Disease.

Neuroimaging suggests that white matter microstructure is severely affected in Alzheimer's disease (AD) progression. However, whether alterations in white matter microstructure are confined to specific regions and whether they can be used as potential biomarkers to distinguish normal control (NC) from AD are unknown. In this cross-sectional study, 33 cases of AD and 25 cases of NC were recruited for automatic fiber quantification (AFQ). A total of 20 fiber bundles were equally divided into 100 segments for quantitative assessment of fractional anisotropy (FA), mean diffusivity (MD), volume and curvature. In order to further evaluate the diagnostic value, the maximum redundancy minimum (mRMR) and LASSO algorithms were used to select features, calculate the Radscore of each subject, establish logistic regression models, and draw ROC curves, respectively, to assess the predictive power of four different models. There was a significant increase in the MD values in AD patients compared with healthy subjects. The differences were mainly located in the left cingulum hippocampus (HCC), left uncinate fasciculus (UF) and superior longitudinal fasciculus (SLF). The point-wise level of 20 fiber bundles was used as a classification feature, and the MD index exhibited the best performance to distinguish NC from AD. These findings contribute to the understanding of the pathogenesis of AD and suggest that abnormal white matter based on DTI-based AFQ analysis is helpful to explore the pathogenesis of AD.

Prospects and Challenges for the Spatial Quantification of the Diffusion of Fluids Containing 1H in the Pore System of Rock Cores

AbstractNuclear magnetic resonance (MR) (NMR) is commonly used for the determination of rock porosity, as well as pore size distribution (PSD) and tortuosity, required to handle hydrocarbon exploitation. Emerging technologies for NMR equipment now enable the visualization of porosity using magnetic resonance imaging (MRI). Currently, single‐point imaging is the most common MRI technique employed, but diffusion‐weighted imaging has been attracting the attention of geologists and geophysicists. In this study, a more advanced technique, diffusion tensor imaging (DTI), is proposed for the characterization of rock core samples. The main purpose of this paper is to demonstrate the usefulness of DTI in the petrophysical characterization of rocks and discuss its strengths. As an example, a carbonate core sample was examined using DTI. The diffusion tensor (DT) was obtained and DT parameters (mean diffusivity, MD; fractional anisotropy, FA; three eigenvalues; DT components; and proton density, PD) were visualized in 2D and 3D. Each parameter was described and its utility in terms of pore space characterization was analyzed. In addition, a new parameter, principal diffusion tracts, was introduced based on the DT tractography performed in the study. The analysis is summarized in a set of DT parameters and a resultant ellipsoid that delivers complementary information about the sample's pore network microgeometry, including referential measurements of anisotropic phantoms. The applications of DTI for the determination of pore size distribution, tortuosity and conductivity are also shown. The study ends with a consideration of the potential prospects and challenges for DTI‐based examination of rock core samples.

Clinical application of diffusion tensor imaging and fiber tractography in the management of brainstem cavernous malformations: a systematic review.

This study aimed to systematically review the literature to determine the clinical utility and perspectives of diffusion tensor imaging (DTI) in the management of patients with brainstem cavernous malformations (BSCMs). PubMed, Embase, and Cochrane were searched for English-language articles published until May 10, 2021. Clinical studies and case series describing DTI-based evaluation of patients with BSCMs were included. Fourteen articles were included. Preoperative DTI enabled to adjust the surgical approach and choose a brainstem safe entry zone in deep-seated BSCMs. Preoperatively lower fractional anisotropy (FA) of the corticospinal tract (CST) correlated with the severity of CST injury and motor deficits. Postoperatively increased FA and decreased apparent diffusion coefficient (ADC) corresponded with the normalization of the perilesional CST, indicating motor improvement. The positive (PPV) and negative predictive value (NPV) of qualitative DTI ranged from 20 to 75% and from 66.6 to 100%, respectively. The presence of preoperative and postoperative motor deficits was associated with a higher preoperative resting motor threshold (RMT) and lower FA. A higher preoperative CST score was indicative of a lower preoperative and follow-up Medical Research Council (MRC) grade. DTI facilitated the determination of a surgical trajectory with minimized risk of WMTs' damage. Preoperative FA and RMT might indicate the severity of preoperative and postoperative motor deficits. Preoperative CST score can reliably reflect patients' preoperative and follow-up motor status. Due to high NPV, normal CST morphology might predict intact neurological outcomes. Contrarily, sparse and relatively low PPV limits the reliable prediction of neurological deficits.

A callosal biomarker of behavioral intervention outcomes for autism spectrum disorder? A case-control feasibility study with diffusion tensor imaging.

Tentative results from feasibility analyses are critical for planning future randomized control trials (RCTs) in the emerging field of neural biomarkers of behavioral interventions. The current feasibility study used MRI-derived diffusion imaging data to investigate whether it would be possible to identify neural biomarkers of a behavioral intervention among people diagnosed with autism spectrum disorder (ASD). The corpus callosum has been linked to cognitive processing and callosal abnormalities have been previously found in people diagnosed with ASD. We used a case-control design to evaluate the association between the type of intervention people diagnosed with ASD had previously received and their current white matter integrity in the corpus callosum. Twenty-six children and adolescents with ASD, with and without a history of parent-managed behavioral intervention, underwent an MRI scan with a diffusion data acquisition sequence. We conducted tract-based spatial statistics and a region of interest analysis. The fractional anisotropy values (believed to indicate white matter integrity) in the posterior corpus callosum was significantly different across cases (exposed to parent-managed behavioral intervention) and controls (not exposed to parent-managed behavioral intervention). The effect was modulated by the intensity of the behavioral intervention according to a dose-response relationship. The current feasibility case-control study provides the basis for estimating the statistical power required for future RCTs in this field. In addition, the study demonstrated the effectiveness of purposely-developed motion control protocols and helped to identify regions of interest candidates. Potential clinical applications of diffusion tensor imaging in the evaluation of treatment outcomes in ASD are discussed.

Diffusion-tensor magnetic resonance imaging of the human heart in health and in acute myocardial infarction using diffusion-weighted echo-planar imaging technique with spin-echo signals.

Originally thought unsuitable due to proneness to myocardial motion and susceptibility artefacts, spin-echo echo planar imaging (SE-EPI) has gained attention for the cardiac diffusion-weighted imaging (DWI) and diffusion tensor imaging (DTI) offering higher SNR and lower achievable echo time (TE). The application of DTI for patients with acute myocardial infarction (AMI) using our methodology developed on the basis of the SE-EPI sequence. Twelve patients with AMI and six healthy controls were enrolled in the preliminary DTI study within the CIRCULATE STRATEGMED 2 project. Our method relied on a pilot ECG-triggered DTI examination, based on which the initial evaluation was possible and allowed proper manipulation of TE (64/47 ms for patients/control), repetition time (TR) and ECG trigger delay in the consecutive DTI. The study demonstrated that by using our algorithm it was possible to obtain DWI images showing infarct zones identified on T1-weighted images with late gadolinium-enhancement (LGE) with division into subtle and severe damage. Quantitative DTI showed increased mean diffusivity (MD) and decreased fractional anisotropy (FA) in the infarct compared to remote tissue. The application of B-matrix spatial distribution (BSD) calibration allowed the improvement of FA. Our algorithm is suitable for qualitative assessment of infarction zones with different severity. The analysis of the quantitative DTI showed that despite the lack of motion compensation blocks in the applied SE-EPI sequence, it was possible to approach the diffusion tensor parameter values reported for the myocardium.

Development of DTI Based Probabilistic Tractography Methods to Characterize Arm Muscle Architecture in Individuals Post Hemiparetic Stroke.

A hemiparetic stroke may lead to changes in muscle structure that further exacerbate motor impairments of the paretic limb. Cadaveric measurements have previously been used to study structural parameters in skeletal muscles but has several limitations, including ex vivo fixation. Here, we present novel application of diffusion tensor imaging (DTI) based probabilistic tractography methods, in comparison to the traditional deterministic approach, with respect to cadaveric dissection to quantify in vivo muscle fascicles in the biceps brachii. Preliminary results show that probabilistic tractography yields longer fascicle lengths that are more consistent with cadaveric measurements, albeit with higher variability, while deterministic tractography identifies shorter fascicle lengths, but with less variability. Results suggest that DTI tractography techniques can capture fascicles consistent with previously published cadaveric measurements and can identify interlimb differences in fascicle lengths in an individual with stroke.Clinical Relevance- The methods proposed here describe a non-invasive way to quantify heterogeneous musculoskeletal parameters such as across upper arm muscles in individuals with hemiparetic stroke. This will expand the current knowledge of macro- and micro-structural muscle changes that occur after stroke and may lead to more effective rehabilitation strategies to prevent such changes in individuals with stroke.

Application of Diffusion Tensor Imaging in Obstructive Nephropathy

Background: Obstructive nephropathy is a common clinical disease. Objectives: To explore the value of diffusion tensor imaging (DTI) in obstructive nephropathy. Materials and Methods: Forty healthy Sprague-Dawley (SD) rats were examined in this study. Thirty-two animals underwent complete obstruction of the left ureter, while eight animals underwent a sham surgery. Magnetic resonance imaging (MRI) was performed before surgery and within different intervals after surgery. Eight rats from the experimental group and two rats from the sham group were used in each interval. Following MRI, the animals were sacrificed and sent for medical examinations. The scanning sequences included positioning, transverse T2-weighted (T2W), coronal, and coronal DTI sequences. Image postprocessing was performed after DTI to measure DTI parameters, including apparent diffusion coefficient (ADC) and fractional anisotropy (FA), and to reconstruct DTI fiber traces. One-way analysis of variance was used to compare the parameters between the cortex and medulla and between different intervals. Results: The fiber tracing showed that the obstructed renal fiber bundles were sparse and disordered. The ADC and FA values of the renal cortex, extrarenal medulla, and inner medulla decreased with prolonged hydrops and were negatively correlated with the expression of alpha-smooth muscle actin (α-SMA) and the renal tubulointerstitial lesion grade (r &lt; 0, P &lt; 0.001). Comparison of the cortex, extrarenal medulla, and inner medulla showed the following trends for the ADC and FA values: cortex &gt; extrarenal medulla &gt; inner medulla and cortex &lt; extrarenal medulla &lt; inner medulla, respectively. Conclusions: DTI in obstructive nephropathy not only can reflect the degree of renal interstitial fibrosis and accurately indicate the renal function, but also can provide information regarding renal blood perfusion, water metabolism, and ultrastructural changes.

A time-course study of actively stained mouse brains: Diffusion tensor imaging parameters and connectomic stability over 1 year.

While the application of diffusion tensor imaging (DTI), tractography, and connectomics to fixed tissue is a common practice today, there have been limited studies examining the effects of fixation on brain microstructure over extended periods. This mouse model time-course study reports the changes of regional brain volumes and diffusion scalar parameters, such as fractional anisotropy, across 12 representative brain regions as measures of brain structural stability. The scalar DTI parameters and regional volumes were highly variable over the first 2 weeks after fixation. The same parameters were consistent over a 2-8-week window after fixation, which means confounds from tissue stability over that scanning window were minimal. Quantitative connectomes were analyzed over the same time with extension out to 1 year. While there was some change in the scalar metrics at 1 year after fixation, these changes were sufficiently small, particularly in white matter, to support reproducible connectomes over a period ranging from 2-weeks to 1-year post-fixation. These findings delineate a scanning period, during which brain volumes, diffusion scalar metrics, and connectomes are remarkably consistent.
