Socioeconomic differences in antibiotic use for common infections in pediatric urgent-care centers-A quasi-experimental study.

To investigate differences in the rate of firstline antibiotic prescribing for common pediatric infections in relation to different socioeconomic statuses and the impact of an antimicrobial stewardship program (ASP) in pediatric urgent-care clinics (PUCs). Quasi-experimental. Three PUCs within a Midwestern pediatric academic center. Patients aged >60 days and <18 years with acute otitis media, group A streptococcal pharyngitis, community-acquired pneumonia, urinary tract infection, or skin and soft-tissue infections who received systemic antibiotics between July 2017 and December 2020. We excluded patients who were transferred, admitted, or had a concomitant diagnosis requiring systemic antibiotics. We used national guidelines to determine the appropriateness of antibiotic choice in 2 periods: prior to (July 2017-July 2018) and following ASP implementation (August 2018-December 2020). We used multivariable regression analysis to determine the odds ratios of appropriate firstline agent by age, sex, race and ethnicity, language, and insurance type. The study included 34,603 encounters. Prior to ASP implementation in August 2018, female patients, Black non-Hispanic children, those >2 years of age, and those who self-paid had higher odds of receiving recommended firstline antibiotics for all diagnoses compared to male patients, children of other races and ethnicities, other ages, and other insurance types, respectively. Although improvements in prescribing occurred after implementation of our ASP, the difference within the socioeconomic subsets persisted. We observed socioeconomic differences in firstline antibiotic prescribing for common pediatric infections in the PUCs setting despite implementation of an ASP. Antimicrobial stewardship leaders should consider drivers of these differences when developing improvement initiatives.

Antibiotic Use for Common Infections in Pediatric Emergency Departments: A Narrative Review.

Antibiotics are one of the most prescribed medications in pediatric emergency departments. Antimicrobial stewardship programs assist in the reduction of antibiotic use in pediatric patients. However, the establishment of antimicrobial stewardship programs in pediatric EDs remains challenging. Recent studies provide evidence that common infectious diseases treated in the pediatric ED, including acute otitis media, tonsillitis, community-acquired pneumonia, preseptal cellulitis, and urinary-tract infections, can be treated with shorter antibiotic courses. Moreover, there is still controversy regarding the actual need for antibiotic treatment and the optimal dosing scheme for each infection.

Antibiotic assemblages and their implications for the prevention of antimicrobial resistance

Individual antibiotic use for common infections is a focus for public health efforts seeking to prevent antimicrobial resistance (AMR). These approaches employ a binary opposition of responsible and irresponsible antibiotic use with a focus on the knowledge, behaviours and intentions of the individual. To overcome these unhelpful tendencies and reveal new entry points for AMR prevention, we adopted assemblage theory to analyse personal experience narratives on individual antibiotic use in community settings. Antibiotic use was irregular, situationally diverse and shaped by factors not always under personal control. Individuals were focussed on preventing, moderating and treating infections that threatened their health. Our analysis shows that antibiotic assemblages are both cause and effect of individual efforts to manage infections. We suggest that AMR prevention needs to look beyond the antibiotic as object and the (ir)responsible use binary to engage with the antibiotic effects individuals seek in order to manage infectious diseases. This antibiotic assemblage orientation is likely to be more meaningful for individuals seeking out methods for promoting their health in the face of common infections.

P47 Infographics about antibiotics: making facts accessible

BackgroundPublic misconceptions about antibiotic use persist despite the efforts of antibiotic awareness campaigns. These campaigns have often followed a top-down approach and have not sought input from the public. Communities need to see antibiotic campaign messages as relevant, accessible and important in order to have an influence on health-seeking behaviour and antibiotic use.ObjectivesTo develop a series of evidenced-based infographics (EBIs) on antibiotic use for common infections in children and to evaluate their effectiveness at increasing parents' understanding of antibiotic use and antibiotic resistance.MethodsThere were three phases to this research. In Phase 1 we set out to identify and summarize scientific evidence for the use of antibiotics for three common infections in children (sore throat, acute cough and otitis media). Phase 2 focused on co-design of a series of prototype EBIs for each infection in focus groups with parents of young children and graphic designers, to test the face and content validity. Phase 3 tested the feasibility of EBIs in increasing parents' understanding about antibiotic use and the perceived relevance of antibiotic resistance in an online survey.ResultsWe iteratively co-produced 10 prototype EBIs. Parents found the evidence displayed in the EBIs novel and relevant to their families. Parents did not favour EBIs that were too medically focused. The way the information was displayed influenced their understanding. Parents preferred one health message per EBI. We included eight EBIs in a national survey of parents (n = 998). EBIs improved knowledge by more than a third across the board (34%, IQR 20%–46% P<0.001). Respondents confirmed that EBIs were novel and potentially useful, corroborating our focus group findings.ConclusionsCo-designed EBIs have the potential to succinctly change parents' perceptions about antibiotics for acute respiratory tract infections in children. Further research should test EBIs in real-world settings to assess their reach as a potential public-facing intervention.

143. Antibiotic Use for Common Infections in British Columbia and Ontario: A Review of Outpatient Prescribing to Seniors from 2000 – 2018

Abstract Background Antimicrobials remain among the most prescribed medications in Canada, with over 90% prescribed in outpatient settings. Older adults (aged ≥65 years) prescribed antimicrobials are particularly vulnerable to adverse drug events and antimicrobial resistance. This study compared annual rates of indication-associated, outpatient prescribing to seniors across two Canadian provinces. Methods All outpatient, oral antimicrobials dispensed to older adults (≥65 years) were identified from administrative health databases, from 2000 to 2018. Antimicrobials were limited to outpatient use only and linked to an indication using a 3-tiered diagnostic hierarchy. When possible, a record of dispensation was matched to a tier 1 indication (always require antibiotics) first. In the absence of a tier 1 indication, priority was given to tier 2 (sometimes require antibiotics), then 3 (never require antibiotics). Prescription rates were calculated per 1000 population, and trends were examined overall, by drug class, and patient demographics. Results Our study included over 18 million individuals (aged ≥65 years) with a total of 23,773,552 antibiotic prescriptions issued to seniors, for common infections. In both provinces, prescribing for tier 1 diagnoses increased over the study period (BC: 44%; ON: 28%). Urinary tract infections accounted for most prescriptions within this tier (ON: 89 prescriptions/1000, BC: 129 prescriptions/1000 population by 2018). Pneumonia-associated prescribing increased by roughly 10% in both provinces. In any given study year, for both provinces, tier 3 diagnosis was the most common reason for antibiotic use, accounting for 50% of all indication-associated antibiotic prescribing. As diagnoses within this tier do not warrant prescribing all antibiotics issued are therefore inappropriate prescriptions. Figure 1. 214. Antibiotic Use for Common Infections in British Columbia: A Review of Outpatient Prescribing from 2000 - 2018

BackgroundAntimicrobial resistance continues to jeopardize the future of modern medicine; as 92% of all antibiotics are used in the community, it is imperative to parse outpatient prescribing. In British Columbia (BC), efforts to curb the use of these essential medications have included: stewardship campaigns, practitioner guidelines, and vaccine scheduling amendments. This study reviews the trends in antibiotic use over the past two decades to identify new targets for Provincial stewardship and intervention.MethodsAntibiotic prescription information was extracted from PharmaNet, a centralized data system that links all pharmacies with prescriptions dispensed in the community setting. The Medical Service Plan records reimbursement claims submitted by physicians for services provided, including diagnostic codes. Antibiotic prescriptions were extracted from PharmaNet and then matched to the billing system using anonymized patient identifiers. Prescription rates were calculated, and trends were examined by major anatomical therapeutic chemical (ATC) classification.ResultsOur study included 3,564,258 individuals over an 18-year period, with a total of 26,108,576 antibiotic prescriptions issued, for common infections. Overall antibiotic utilization decreased 18% (from 228 to 187 prescriptions per 1000 population) over the course of the study period. This trend was reflected in both Beta-Lactam (-37%) and Macrolide (-50%) antibiotics; two of the most common classes prescribed in the outpatient setting. A significant outlier was the J01X class of Other Antibacterials, which increased by a staggering 218%, by 2018. Further analyses are currently underway to stratify these changes in magnitude by demographic variables to identify specific, new targets for stewardship.Rates of outpatient antibiotic prescriptions, for common infections, per 1000 population, by major ATC class, over time. ConclusionOutpatient antibiotic prescribing has decreased steadily since 2000. These promising results can be ascribed to the various Provincial initiatives to quell the misuse of these medications. However, many of the indications tied to these prescriptions do not warrant the use of antibiotics, and further analyses are necessary to evaluate prescribing quality to fully delineate the state of antibiotic use in BC.
