Imaging the pediatric retina: An overview.

Recent decade has seen a shift in the causes of childhood blinding diseases from anterior segment to retinal disease in both developed and developing countries. The common retinal disorders are retinopathy of prematurity and vitreoretinal infections in neonates, congenital anomalies in infants, and vascular retinopathies including type 1 diabetes, tumors, and inherited retinal diseases in children (up to 12 years). Retinal imaging helps in diagnosis, management, follow up and prognostication in all these disorders. These imaging modalities include fundus photography, fluorescein angiography, ultrasonography, retinal vascular and structural studies, and electrodiagnosis. Over the decades there has been tremendous advances both in design (compact, multifunctional, tele-consult capable) and technology (wide- and ultra-wide field and noninvasive retinal angiography). These new advances have application in most of the pediatric retinal diseases though at most times the designs of new devices have remained confined to use in adults. Poor patient cooperation and insufficient attention span in children demand careful crafting of the devices. The newer attempts of hand-held retinal diagnostic devices are welcome additions in this direction. While much has been done, there is still much to do in the coming years. One of the compelling and immediate needs is the pediatric version of optical coherence tomography angiography. These needs and demands would increase many folds in future. A sound policy could be the simultaneous development of adult and pediatric version of all ophthalmic diagnostic devices, coupled with capacity building of trained medical personnel.

Differential association of elevated inflammatory cytokines with postoperative fibrous proliferation and neovascularization after unsuccessful vitrectomy in eyes with proliferative diabetic retinopathy.

BackgroundPars plana vitrectomy is the only treatment for advanced proliferative diabetic retinopathy (PDR). However, vitrectomy is not always successful despite current progress in vitreoretinal surgical techniques. The aim of our study was to investigate whether the vitreal concentrations of MCP-1, IL-6, IL-8, and VEGF are elevated after unsuccessful vitrectomy in patients with PDR and to investigate whether the altered levels of these cytokines are associated with the cause for the reoperation.Patients and methodsVitreous samples were collected from 263 eyes of 233 patients: PDR (n=129 eyes), proliferative vitreoretinopathy (PVR; n=24 eyes) and nondiabetic controls (n=110 eyes) prior to vitrectomy. Vitreous samples were also collected from 14 eyes of 14 patients with PDR before vitrectomy and from the same 14 eyes before a second vitrectomy for reoperation. The levels of MCP-1, IL-6, IL-8, and VEGF were measured by flow cytometry using a cytometric bead array (CBA) assay.ResultsThe mean concentrations of vitreal MCP-1, IL-6, IL-8, and VEGF were significantly higher in patients with PDR and PVR (P<0.01). There were significantly high correlations among the concentrations of MCP-1, IL-6, and IL-8, whereas the correlation of VEGF with the other 3 cytokines was lower. Among the 14 patients who required reoperation, the mean vitreal concentrations of MCP-1, IL-6, and IL-8 were higher than that at the time of the initial vitrectomy (P<0.01). At the time of the reoperation vitrectomy, the mean vitreous level of MCP-1, IL-6, and IL-8 in eyes with fibrous proliferation was higher than in those without fibrous proliferation (P<0.05). In contrast, VEGF in eyes with neovascular glaucoma (NVG) or anterior hyaloidal fibrovascular proliferation (AHFVP) was higher than in the eyes without NVG and AHFVP (P<0.05).ConclusionThe elevated levels of MCP-1, IL-6, and IL-8 may be the cause of the postoperative fibrous proliferation. In contrast, VEGF may be the cause of the neovascularization after unsuccessful vitrectomy in the eyes of PDR patients.

Characteristics and outcomes of anterior hyaloidal fibrovascular proliferation in lasered retinopathy of prematurity. The Indian Twin Cities Retinopathy of Prematurity Study (ITCROPS) report number 4

To describe the characteristics and treatment outcomes of an unreported, late vitreous hemorrhage due to anterior hyaloidal fibrovascular proliferation in laser-regressed retinopathy of prematurity (ROP). Interventional case series. In the ongoing Indian Twin Cities ROP study database, consecutive cases with isolated late vitreous hemorrhage at least one year after laser-regressed disease were analyzed retrospectively. Anterior hyaloidal fibrovascular proliferation was diagnosed primarily using scleral depression. Anterior retinal cryopexy with adjunctive treatments was performed. The main outcome measure was clinical resolution of new vessels with no recurrent hemorrhage over a 1-year period. Vitreous hemorrhage, at two to eight years of age, developed in three eyes of three children out of 1,168 ROP lasered eyes. All had received laser for zone I disease as neonates, with no subsequent sequelae. Evaluation revealed filiform new vessels at the posterior vitreous base involving inferior 180° with absence of any other source of hemorrhage. All underwent anterior retinal cryopexy to the affected area. Simultaneous additional treatment, based on intraoperative findings, included one case each of peripheral laser photocoagulation, lens-sparing vitrectomy and intravitreal bevacizumab. All three showed successful regression and non-recurrence of vitreous hemorrhage with improvement of vision >20/40 at an intermediate follow-up of two years. Anterior hyaloidal fibrovascular proliferation is an unreported and rare cause of vitreous hemorrhage, appearing years after laser-regressed ROP. It has a good response to interventional treatment. Meticulous scleral depression of the vitreous base under anesthesia is useful to detect this rare source of vitreous hemorrhage.

Vitreous levels of vascular endothelial growth factor in eyes with anterior hyaloidal fibrovascular proliferation

To determine the intravitreal levels of vascular endothelial growth factor (VEGF) in eyes with anterior hyaloidal fibrovascular proliferation (AHFVP). Three eyes of three patients who underwent vitrectomy for proliferative diabetic retinopathy (PDR) and subsequently developed an AHFVP (AHFVP group) were studied. We measured the level of VEGF in vitreous samples collected at the primary and following operations by enzyme-linked immunosorbent assay. The vitreous levels of VEGF in 25 eyes of 22 patients with PDR were also studied as controls (PDR group). The averaged VEGF level in the samples collected at the primary surgery was 1.98 ± 2.23 ng/mL in the PDR group, and it was 9.07, 1.94, and 8.07 ng/mL in the AHFVP cases. After the primary surgery, the VEGF level rose up to 49.50, 15.60, and 50.60 ng/mL at the subsequent surgeries for respective cases of the AHFVP group. These levels of VEGF were more than five times higher than the baseline at the primary surgery. The subsequent increase of the VEGF level after the primary surgery in eyes with an AHFVP suggests that the vitreous levels of VEGF are associated with the development of the AHFVP although only three eyes were studied.

Anterior Hyaloidal Fibrovascular Proliferation (AHFVP) in a Diabetic after Cataract Extraction, Resulting in Hyphaema and Vitreous Haemorrhage during YAG Laser Capsulotomy

Aim: To report a rare case of anterior hyaloidal fibrovascular proliferation (AHFVP) which occurred after cataract surgery on a non-vitrectomized diabetic eye. This complication resulted in vitreous haemorrhage and hyphaema following a YAG laser capsulotomy procedure.Method: A case report of a patient managed at the above eyedepartment. Clinical records as indicated in the patient’s case note were reported.Conclusion: Anterior hyaloidal fibrovascular proliferation is a complication which an eye surgeon should bear in mind when attending to diabetic eyes after cataract surgery. The potential danger of vitreous haemorrhage following YAG laser capsulotomy should also be borne in mind, as more centres inthe country aspire to acquire a YAG laser machine to performcapsulotomy.

PERIPHERAL 360° RETINECTOMY IN COMPLEX RETINAL DETACHMENT

To report the functional and anatomical results and complications of 360 degrees peripheral retinectomy for management of complicated retinal detachment. Patients with complicated retinal detachment underwent pars plana vitrectomy, 360 degrees retinectomy, intraoperative endolaser, and internal tamponade with silicone oil. Postoperative visual acuity, intraocular pressure, retinal status, need for reoperation, and complications are presented. Twenty eyes of 19 patients with a mean age of 32.4 years (8-75 years) underwent pars plana vitrectomy and 360 degrees peripheral retinectomy for complicated retinal detachment due to anterior proliferative vitreoretinopathy, unstable edge of retinal break, anterior hyaloidal fibrovascular proliferation, retinal incarceration in scleral wound, and 300 degrees giant retinal tear. Intraoperative reattachment was achieved in 18 eyes. Mean postoperative follow-up time was 24.2 months (2-70 months). Retina was attached in 14 eyes (70%) in the last visit. Eight eyes (40%) had 5/200 or greater visual acuity. Preoperative and postoperative visual acuities did not have significant correlation (Spearman correlation coefficient = 0.291). There was no relation between diagnosis and anatomical outcome (P > 0.2). Relaxing peripheral 360 degrees retinectomy is an effective procedure for flattening the retina in complicated retinal detachments when no other option is available.

Advantage of combined procedure in vitreous surgery

Lens preservation facilitates the progression of nuclear cataracts after vitreous surgery among patients over 45 years old, and postoperative nuclear cataracts frequently necessitate intraocular lens surgery. Once the lens is removed, the fundus from the posterior pole to the ciliary process becomes recognizable and surgically reachable, thus making total vitreous removal possible. The complete removal of the incarcerated vitreous to the scleral wound prevents postoperative vitreous base tears, and anterior hyaloidal fibrovascular proliferation. Simultaneous vitreous surgery plus intraocular lens surgery (triple surgery) will prevent these complications before they arise. Triple surgery for patients over 45 years of age is reasonable even if they have amplitude of accommodation, and is an effective surgical strategy in that it improves both the result of the vitreous surgery and the patient’s quality of life.

Ultrasound biomicroscopy of sclerotomy sites after pars plana vitrectomy for diabetic vitreous hemorrhage

Objective This study was aimed at assessing changes at the sclerotomy site using the ultrasound biomicroscope (UBM) in eyes that underwent primary pars plana vitrectomy for complications of proliferative diabetic retinopathy. Design Prospective, observational case series. Participants Eighty-six eyes of 84 patients with vitreous hemorrhage caused by proliferative diabetic retinopathy. Intervention Three-port pars plana vitrectomy followed by UBM evaluation of all sclerotomy sites between 6 and 8 weeks after surgery. Correlation with intraoperative findings done in case of reoperations. Forty-one eyes had repeat UBM at 6 months. Main outcome measures The changes at the sclerotomy site were classified into six groups: well healed, gape, plaque, vitreous incarceration, fibrovascular proliferation, and anterior hyaloidal fibrovascular proliferation (AHFVP). The UBM characteristics of each of the groups were defined. The findings at 6 months were compared with those at 6 to 8 weeks. Results At 6 to 8 weeks after surgery, most sclerotomies were well healed or had either moderate to high reflective plaques bridging the site. Wound gape was seen in 22.1% of active ports, 32.6% of light ports, and 25.6% of infusion ports. Vitreous incarceration was seen most often at the infusion port (18.6% of eyes). Fibrovascular proliferation was seen in 9.3% of active ports, 12.8% of light ports, and 15.1% of infusion ports. Thirteen eyes had recurrent vitreous hemorrhage 6 to 8 weeks after surgery. Cases with rebleeding and no fibrovascular proliferation at the sclerotomy on UBM did well with outpatient fluid-air exchange (two eyes) or observation only (three eyes). Those with fibrovascular proliferation on UBM (eight eyes) required more extensive surgery. Conclusions UBM is helpful in detecting complications at the sclerotomy sites after pars plana vitrectomy and is an invaluable tool in the assessment of the patient before reoperation.

Ultrasound Biomicroscopy in a Case of Anterior Hyaloidal Fibrovascular Proliferation

The authors describe the use of ultrasound biomicroscopy for the diagnosis and preoperative evaluation of anterior hyaloidal fibrovascular proliferation (AHFVP). Ultrasound biomicroscopy was performed on a 62-year-old man who presented after diabetic vitrectomy with a hyphema, vitreous hemorrhage, and hypotony. Images in the temporal and nasal meridians revealed thickened tissue bands extending from the peripheral retina to the ciliary body, and from the pars plicata to the posterior surface of the iris. A ciliary body epithelium detachment was seen in the nasal meridian. Ultrasound biomicroscopy demonstrated to be a potential tool in the diagnosis and surgical management of AHFVP.

Pars plana incisions of four patients: histopathology and electron microscopy.

The pathology of pars plana incisions of four patients is described: three with light microscopy and one with light and electron microscopy. Two eyes were removed because of choroidal melanoma, immediately and 8 days after vitrectomy and transvitreous retinal biopsy. Considerable disruption of tissues surrounding the pars plana incisions was observed. Vitreous was incarcerated in the wounds, which healed with granulation tissue. One eye was examined 4 months after vitrectomy for diabetic retinopathy and a failed pars plana filtering operation. It contained fibrovascular ingrowth from all the incisions, infiltrating the vitreous base with granulation tissue and causing vitreous haemorrhage and retinal detachment. One eye was removed 1 year after vitrectomy for anterior hyaloidal fibrovascular proliferation and early phthisis. The wound had fibrous ingrowth histologically and evidence of active fibroplasia.

Anterior hyaloidal fibrovascular proliferation

Anterior hyaloidal fibrovascular proliferation was first described in phakic eyes with florid proliferative diabetic retinopathy after standard three-port pars plana vitrectomy. Severe vision-threatening complications of this entity are the development of complicated cataracts, vitreous hemorrhage, traction retinal detachment, ciliary body detachment, and finally phthisis bulbi. It was a recent observation that anterior hyaloidal fibrovascular proliferation can occur after uncomplicated extracapsular cataract extraction with an intraocular lens implant in proliferative diabetic eyes. After cataract surgery the course of the disease process seems to be much less severe; however, there is a risk of visual impairment and it may be useful to detect anterior hyaloidal fibrovascular proliferation early to treat it most effectively. The most important differential diagnosis of anterior hyaloidal fibrovascular proliferation after cataract extraction is rubeosis capsulare. Rubeosis capsulare develops with direct spread from iris neovascularization and thus the distinction between the two entities is made by observing whether the neovascularization is visible on the anterior or posterior surface of the posterior lens capsule.

Anterior Hyaloidal Fibrovascular Proliferation After Extracapsular Cataract Extraction in Diabetic Eyes

Four patients with proliferative diabetic retinopathy developed anterior hyaloidal fibrovascular proliferation after extracapsular cataract extraction and posterior chamber lens implantation. This complication of cataract extraction has been described after vitrectomy in diabetic eyes. Risk factors for this entity after extracapsular cataract extraction include proliferative diabetic retinopathy, iris neovascularization, and anterior ischemic retina. Anterior hyaloidal fibrovascular proliferation was observed an average of 12 months postoperatively and affected vision in one of four patients. No progression was seen within an average of six months of follow-up, and no complications such as traction retinal detachment and vitreous hemorrhage developed.

Vitrectomy in the management of diabetic eye disease

Vitrectomy techniques including endolaser photocoagulation allow visual rehabilitation in many eyes that are otherwise untreatable. Discerning the indications and timing for diabetic vitrectomy is increasingly important as the treatment of complications of diabetic retinopathy continues to undergo modification and redefinition. The most common indications for diabetic vitrectomy include: 1) severe nonclearing vitreous hemorrhage; 2) traction retinal detachment recently involving the macula; 3) combined traction and rhegmatogenous detachment; 4) progressive fibrovascular proliferation; and 5) rubeosis iridis and vitreous hemorrhage for eyes in which the media opacity has prevented adequate laser photocoagulation. Other less common indications in selected cases include dense premacular hemorrhage, ghost cell glaucoma, macular edema with premacular traction, cataract preventing treatment of severe, proliferative diabetic retinopathy, anterior hyaloidal fibrovascular proliferation, and fibrinoid syndrome with retinal detachment. The rationale and surgical objectives are discussed and results are summarized.

Efficacy of Fluid-Air Exchange for Postvitrectomy Diabetic Vitreous Hemorrhage

We reviewed the records of 33 fluid-air exchanges to assess the efficacy of fluid-air exchange in the management of recurrent vitreous cavity hemorrhage after vitrectomy for proliferative diabetic retinopathy. Fluid-air exchange alone was successful in clearing the vitreous cavity in ten of 20 eyes after a mean of 1.5 exchanges per eye. Repeat vitrectomy was required in the remaining ten eyes and anterior hyaloidal fibrovascular proliferation was frequently found. Hemorrhages that occurred in the late postoperative period (more than nine months) appeared more likely to be successfully treated with fluid-air exchange alone. Failure of the initial fluid-air exchange to induce clearing immediately after the procedure appeared to be associated with subsequent exchange failures and need for surgical intervention. Complications from the exchange procedure were infrequent with the development of peripheral retinal detachment in one eye. Our current recommendation for nonclearing recurrent postvitrectomy diabetic vitreous hemorrhage is to perform a fluid-air exchange, provided no other high-risk characteristics are present. If clearing occurs in the immediate postexchange period but rebleeding occurs at a later period, we recommend a second fluid-air exchange. If clearing does not occur in the immediate postexchange period, we recommend proceeding directly to revision of vitrectomy.

Echographic Diagnosis of Anterior Hyaloidal Fibrovascular Proliferation

High-resolution contact B-scan echographic imaging of the ciliary body and peripheral retina was performed on five eyes with anterior hyaloidal fibrovascular proliferation and media opacity by means of a wide (58 degrees) scanning arc. This technique determined the circumferential and anteroposterior extent of peripheral traction retinal detachment associated with anterior hyaloidal fibrovascular proliferation, which correlated highly with findings at subsequent vitrectomy. Final visual acuity of 20/400 or better was achieved in the two eyes (50%) with more limited peripheral traction detachment. In the presence of media opacity, anterior echographic imaging may allow early detection of traction retinal detachment associated with anterior hyaloidal fibrovascular proliferation and may be useful in characterizing the severity of this condition.

Silicone Oil for Progressive Anterior Ocular Neovascularization After Failed Diabetic Vitrectomy

We have performed revision of vitrectomy and silicone oil injection in 18 eyes that developed advanced iris neovascularization and/or anterior hyaloidal fibrovascular proliferation complicated by retinal detachment or media opacity after vitreous surgery for the complications of proliferative diabetic retinopathy. Stabilization or regression of the anterior ocular neovascular changes was noted in 83% of the eyes. Sustained retinal attachment was achieved in 56% of the eyes, with visual acuity improving to 20/400 or better in 28% of the eyes. Posterior segment vascular and avascular reproliferation was the major cause of anatomic failure, while generalized retinal vascular ischemia was felt to be the principal cause of poor visual results despite retinal attachment. Revision of vitrectomy with silicone oil injection is of limited value in the management of complicated and advanced anterior ocular neovascularization after unsuccessful vitrectomy for diabetic retinopathy.

Anterior Hyaloidal Fibrovascular Proliferation After Diabetic Vitrectomy

Vitrectomy was performed to treat 74 consecutive eyes for complications of diabetic retinopathy. Eight (13%) of 61 eyes followed up for an average of 12 months developed anterior hyaloidal fibrovascular proliferation. This was the most common postoperative complication, whose features included recurrent hemorrhages into the vitreous cavity or anterior vitreous, or both; vessels or fibrovascular tissue on the posterior lens capsule; anterior extraretinal vascularization extending toward the lens on the anterior hyaloid; traction detachment of the peripheral retina or ciliary body; and hypotony. Patients who developed this complication tended to be young males with severe retinal neovascularization and extensive retinal ischemia; traction retinal detachment as an indication for surgery; placement of a scleral buckle; postoperative rubeosis iridis, recurrent vitreous hemorrhages, and retinal detachment; and multiple surgeries. Four eyes progressed to atrophia bulbi. Early recognition followed by additional surgery in two patients and extensive additional photocoagulation in two other patients was successful in preserving good visual function.
