Lung Ultrasound in Congenital Cardiac Abnormality: ALCAPA.

Anomalous origin of left coronary artery from the pulmonary artery (ALCAPA) is a life-threatening ischemic congenital cardiac disease, resulting in myocardial dysfunction and heart failure within the first year of life. The authors report the case of a 1-mo-old infant with a radiological diagnosis of pneumonia handled with point-of-care lung ultrasound (POCUS) due to the persistence of dyspnea, despite a week of antibiotic therapy. Lung ultrasound showed a non-univocal interpretation of the lesion and gave indication to the second level instrumental diagnostics that confirmed the presence of ALCAPA. There is no case of lung ultrasound aiding to the early diagnosis of ALCAPA and no indirect ecographic signs of ALCAPA that have been described in literature yet.

Mid-term outcomes of surgical repair for anomalous origin of the left coronary artery from the pulmonary artery: In infants, children and adults.

Background:Anomalous origin of left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital malformation. We sought to evaluate in-hospital and mid-term outcomes of patients with a diagnosis of ALCAPA who underwent surgical repair.Objectives:The objective of this study is to evaluate the mid-term outcomes of surgical repair of ALCAPA at our center and to analyze the surgical techniques used.Materials and Methods:In a retrospective study, we analyzed early and mid-term clinical and echocardiographic data to determine the outcomes of patients who underwent surgical repair of ALCAPA in our institution between 2005 and 2015.Results:Twenty-one patients underwent surgical repair for ALCAPA using aortic reimplantation (n = 10, 47.6%), ostial closure (n = 8, 38.1%), or ligation (n = 3, 14.3%). The median age of patients was 24 months (range 22 days to 51 years). There were 2 (9.5%) in-hospital mortalities in infants undergoing the reimplantation technique. All patients were followed up for a median of 21 months (range 1–60 months). No patients required reoperation, and there was no mortality from discharge to mid-term follow-up. Severe early postoperative mitral regurgitation (MR) was associated with composite end-point, defined as a combination of mortality after surgery, moderate to severe MR, and moderate to severe left ventricular dysfunction at late follow-up (P = 0.019) while mitral valve repair was not (P = 0.469).Conclusion:The surgical management of ALCAPA can be associated with good in-hospital and mid-term outcomes regardless of the age, at which the patient has been operated.

Adult ALCAPA Syndrome-Anomalous Origin of Left Coronary Artery from Pulmonary Artery: A Rare Coronary Artery Anomaly Diagnosed by 256-Slice Computed Tomography

We are presenting a rare case of anomalous origin of left coronary artery from the pulmonary artery (ALCAPA) in an adult patient. The patient underwent coronary CT angiography for exertional dyspnoea, suspected as coronary artery disease; however there were no atherosclerotic plaques. There was anomalous origin of left coronary artery from pulmonary artery. Conventional catheter angiography confirmed the origin of left coronary artery from pulmonary artery, which was retrogradely filling through right coronary artery.

Objectives: We evaluated mid-term results of two different repair strategies of Anomalous Origin of Left Coronary Artery from Pulmonary Artery (ALCAPA) in two surgical centers.

Clinical application of dual-source CT in the diagnosis of ALCAPA

Objective To explore the clinical usefulness of anomalous origin of left coronary artery from the pulmonary artery (ALCAPA) by dual-source CT (DSCT) angiography. Methods A total of 10 patients (mean age 5.1±5.0 yrs, age range 0.2-15 yrs| male 5) with ALCAPA who underwent DSCT angiography and echocardiography were retrospectively included.Surgery was performed in 7 patients, and conventional cardiac angiography (CCA) in 4 patients.The subjective image quality was evaluated on a four-point scale by two radiologists.Radiation dose values were calculated. Results ALCAPA in all patients were accurately displayed with DSCT with the accuracy of 100%.The left coronary arteries originate from the inferior-left wall of the pulmonary artery in 1 patients, from left lateral wall in 3 patients, from right lateral wall in 4 patients and posterior wall in 2 patients, respectively.In all cases, the origins of the right coronary artery were normal and originated from the right sinus of Valsalva of aorta,the right coronary artery was turtuous and dilated in 5 patients.All DSCT images were diagnosable.The average subjective image quality score was 2-4 (3.2±0.8).Two radiologists made good agreement (k=0.79).The ED was (0.56±0.28) mSv.Seven cases were diagnosed correctly by echocardiography while 3 cases was misdiagnosed as dilated cardiomyopathy, left coronary artery-pulmonary artery fistula and endocardial fibroelastosis, respectively. Conclusion Low-dose DSCT angiography provides high diagnostic accuracy for the assessment of ALCAPA. Key words: Coronaryarterydisease; Tomography,X-raycomputed; Echocardiography

A Gratifying Surgical Repair of Anomalous Left Coronary Artery From Pulmonary Artery: Three Decades of Follow-Up

Establishing a two coronary system is the best treatment option for anomalous origin of left coronary artery from the pulmonary artery (ALCAPA). We describe a case of ALCAPA with three decades of follow up after an end-to-end anastomosis of left subclavian artery and left coronary artery with mitral valve replacement. This technique of establishing a two coronary system was done in an era when coronary transfer technique was not established and myocardial protection techniques were not available. This case is also the longest follow up of prosthetic mitral valve replacement in a child with ALCAPA.

Imaging diagnosis study of anomalous origin of coronary artery from the pulmonary artery

目的 探讨冠状动脉(简称冠脉)起源于肺动脉(ACAPA)影像学诊断方法的价值.方法回顾性分析11例ACAPA的影像学表现.11例均行X线胸片、超声心动图(Echo)和心血管造影检查,其中1例行电子束CT(EBCT)检查.结果10例为左冠脉起源于肺动脉,1例为右冠脉起源于肺动脉.11例胸片均未确诊,Echo诊断3例,EBCT诊断1例.心血管造影全部诊断正确,其中左冠脉异常起源者左冠脉均发自主肺动脉后窦或后壁,通过扩张的右冠脉藉侧支逆行充盈;右冠脉异常起源者右冠脉从主肺动脉右窦发出.手术与造影所见相同.3例前乳头肌缺血性纤维化,二尖瓣环扩大,前叶脱垂致二尖瓣关闭不全.结论X线胸片诊断受限,Echo简便、无创,但操作技术及认知水平有待提高.心血管造影仍是术前确诊的金标准。
