Analysis of a Series of 27 Cases of Darier-Ferrand Dermatofibrosarcoma at the Mohammed VI University Hospital in Marrakech

Darier Ferrand dermatofibrosarcoma is a low-grade superficial fibroblastic tumor with a storiform architecture. It is characterized by its local aggressiveness and low metastatic potential. We report a retrospective study spread over a period from January 2019 to July 2024 and involving 27 cases collected at the Mohammed VI University Hospital in Marrakech. Our series showed a sex ratio M/F of 1.4 with an average age of our patients at the time of diagnosis was 31.7 years with extremes of age between 68 and 21 years. The buttock and thigh were the predominant location observed in 55.6% (15 patients), the thorax 18.5% (5 cases), the leg 14.8% (4 cases), the face 3 ,7% (1 case), the back (1 case) and the vulva (1 case). The aim of this work is to report the experience of the Mohammed VI University Hospital and to discuss the related literature on the epidemiological and anatomopathological characteristics.

Could the mitotic count improve personalized prognosis in melanoma patients?

A number of studies have indicated that the mitotic rate may be a predictive factor for poor prognosis in melanoma patients. The aim of this study was to investigate whether the mitotic rate is associated with other prognostic clinical and anatomopathological characteristics. After adjusting for other anatomopathological characteristics, we then verified the prognostic value of the number of mitoses, determining in which population subgroup this variable may have greater prognostic significance on 3-year mortality. The Veneto Cancer Registry (Registro Tumori del Veneto-RTV), a high-resolution population-based dataset covering the regional population of approximately 4.9 million residents, served as the clinical data source for the analysis. Inclusion criteria included all incident cases of invasive cutaneous malignant melanoma recorded in the RTV in 2015 (1,050 cases) and 2017 (1,205 cases) for which the number of mitoses was available. Mitotic classes were represented by Kaplan-Meier curves for short-term overall survival. Cox regression calculated hazard ratios in multivariable models to evaluate the independent prognostic role of different mitotic rate cut-offs. The results indicate that the mitotic rate is associated with other survival prognostic factors: the variables comprising the TNM stage (e.g., tumor thickness, ulceration, lymph node status and presence of metastasis) and the characteristics that are not included in the TNM stage (e.g., age, site of tumor, type of morphology, growth pattern and TIL). Moreover, this study demonstrated that, even after adjusting for these prognostic factors, mitoses per mm2 are associated with higher mortality, particularly in T2 patients. In conclusion, these findings revealed the need to include the mitotic rate in the histological diagnosis because it correlates with the prognosis as an independent factor. The mitotic rate can be used to develop a personalized medicine approach in the treatment and follow-up monitoring of melanoma patients.

Isolated splenic peliosis: a case report

BackgroundPeliosis is a rare condition with anatomopathological characteristics that affect the liver. However, splenic peliosis is even more unique and rare. Patients with such abnormality usually exhibit no symptoms. Moreover, this is a lethal condition due to the high probability of splenic rupture accompanied by shock.Case presentationWe present a case of a 29-year-old Arab female who was admitted to the hospital with severe upper abdominal pain that started 1 week from the date of admission, associated with nausea, anorexia, low-grade fever, and vomiting, with no past medical history or comorbidities. A computerized tomography scan with contrast showed intraperitoneal free fluid along with multiple hypodense splenic cysts. Hence, an emergent exploratory laparotomy with splenectomy was performed. Splenic peliosis was confirmed by the histopathological examination.ConclusionFurther investigations are warranted if peliosis is confirmed in one organ, for example, the liver, to detect its presence in any other potential organs that can be affected by peliosis. Splenic peliosis is extraordinarily rare. Furthermore, such a disease has no established management plan. Definitive treatment is surgical. Many aspects of splenic peliosis remain puzzling requiring more research in the near future.

Epidemiology, pathological characteristics and survival of retroperitoneal soft‑tissue sarcomas compared with non‑retroperitoneal soft tissue sarcomas.

Retroperitoneal soft-tissue sarcomas (RPS) are rare forms of mesenchymal tumors that account for ~0.15% of all malignancies. The purpose of the present study was to determine the differences between RPS and non-RPS anatomopathological and clinical features and to analyze whether the hazard ratio for short-term mortality differs between patients with RPS and non-RPS, after adjusting for differences in baseline anatomopathological and clinical features. The Veneto Cancer Registry, a high-resolution population-based dataset spanning the regional population, was used as a data source for the analysis. The current analysis focuses on all incident cases of soft-tissue sarcoma recorded by the Registry from January 1, 2017 to December 31, 2018. A bivariate analysis was carried out to compare demographic and clinical characteristics in RPS and non-RPS. Short-term mortality risk was analyzed by primary tumor site. The significance of variations in survival by site group was determined using Kaplan-Meier curves and the Log-rank test. Finally, Cox regression was used to assess the hazard ratio for survival by sarcoma group. RPS accounted for 22.8% of the total sample (92 out of 404 cases). The mean age at diagnosis was 67.6 years for RPS vs. 63.4 for non-RPS; 41.3% of RPS were >150 mm vs. 5.5% for non-RPS. Stages III and IV were more prevalent in RPS (53.2 vs. 35.6%), despite the fact that, in both groups, advanced stages are the most common onset at diagnosis. Regarding surgical margins, the present study showed that R0 is the most prevalent in non-RPS (48.7%), while R1-R2 is the most frequent in patients with RPS (39.1%). The 3-year mortality rate for retroperitoneum was 42.9 vs. 25.7%. Comparing RPS and non-RPS, the multivariable Cox model showed a hazard ratio of 1.58 after adjusting for all other prognostic factors. RPS clinical and anatomopathological characteristics differ from those of non-RPS. Overall, despite adjusting for other prognostic factors, the retroperitoneum site was an independent prognostic factor associated with a worse overall survival in sarcoma patients compared with other sites.

Osteonecrosis of the jaw under palbociclib: A case series description.

Cases of osteonecrosis of the jaw have been reported by dental surgeons to the pharmacovigilance center in Rennes, France, occurring among patients treated with palbociclib, a cyclin-dependent kinase 4/6 inhibitor. Although this event was not expected with the drug, a safety signal was raised. Describing a local case series, the aim of our study was to identify specific patterns that might suggest a triggering role for these drugs, and to discuss pathophysiological hypotheses. A retrospective case series of patients exposed to cyclin-dependent kinase 4/6 inhibitors between 2016 and 2020 with a diagnosis of osteonecrosis of the jaw at the Rennes Dental Care Center was analyzed. The descriptive analysis was conducted on patient demographics, breast cancer characteristics, osteonecrosis of the jaw, biological data, and exposure to cyclin-dependent kinase 4/6 inhibitors. We identified eight cases, most of them at stages 0-1 (62.5%). Four patients were still exposed to palbociclib at the time of diagnosis and four had discontinued the treatment before the diagnosis. Chronological imputability could not be excluded given the drug's half-life and the variable intervals of dental monitoring from one patient to another. All patients had at least one dental osteonecrosis risk factor (including dental extraction, dentures, and denosumab exposure at the time of diagnosis). Neutropenia and mucositis were not systematically reported at the time of diagnosis. The anatomopathological characteristics were nonspecific. We did not identify a specific pattern that could suggest a triggering role of palbociclib in the development of ONJ.

Étude de l’expression de LAT-1 et de la fixation de la 18F-FDOPA dans les tumeurs cérébrales. Illustration par une série de cas

IntroductionPET imaging of amino acid analogues, in particular with 18F-FDOPA, is increasingly used for the diagnosis and post-therapeutic evaluation of brain tumors. The mechanism of 18F-FDOPA uptake in brain tumors is still debated in the literature, the main hypothesis being uptake via L-amino-transporter 1 (LAT-1). MethodWe present 4 cases of patients with primary or secondary brain lesions who underwent 18F-FDOPA PET/CT or PET/MRI for the evaluation of suspected tumor recurrence and who underwent surgical removal with immunostaining labeling of LAT-1 transporters within 3 months. For each of these patients, a correspondence between semi-quantitative PET parameters (SUVmax, TBRmax and TSRmax) and anatomopathological characteristics was performed. ResultsAll patients had a significant and intense tumor uptake on 18F-FDOPA PET. In contrast, the expression level of LAT-1 receptors was very variable from one patient to another (LAT-1 score ranging from 0 to 193). ConclusionThis article reflects the persistent questioning of the link between the LAT-1 receptor and the uptake intensity on 18F-FDOPA PET, especially in patients who have received a first line of treatment.

Oral lichen planus pemphigoid : a case report of a rare disease treated by topical Tacrolimus mouthwash and Hydroxychloroquine

Introduction: Oral Lichen Planus Pemphigoid (LPP) is the rare association of lichen planus and Mucous Membrane Pemphigoid (MMP) whose therapeutic lines are poorly defined. Bilateral and symmetrical reticulated white lesions characteristic of lichen planus (LP) most often precede the painful post-bullous ulcerations of pemphigoid (P). The oral LPP is very rare and the treatment is not codified. This report described a rare case of oral LPP, successfully treated with an original combo of topical Tacrolimus and hydroxychloroquine. Observation: A 72 yr old patient was referred to the unit of the oral mucosa pathology and oro-facial pain, of the oral surgery service of Bordeaux hospital. He has painful mouth ulcerations accompanied by reticulated and symmetrical white lesions. A biopsy to perform an anatomopathological examination and a direct immunofluorescence founds the anatomopathological characteristics of LP and P.As first line of treatment, local and general corticosteroid therapy was undertaken to reduce inflammation. The result was not totally satisfactory. As second line of treatment topical Tacrolimus and hydroxychloroquine made it possible to reduce drastically the patient's erosive and ulcerated lesions. Conclusion: The combination of topical tacrolimus and HCQ should be considerated as second line of treatment for LPP resistant to corticotherapy.

Laryngeal Cancers at the Pathological Anatomical Laboratory (ACP) in Dakar about 215 Cases

Introduction: Laryngeal cancer is the first cancer of upper aerodigestive tract. Dysphonia, dyspnea and dysphagia are evocative signs. Diagnosis is histological and squamous cell carcinoma is the most common type. The objective of our study was to discuss epidemiological and anatomo-pathological characteristics of laryngeal cancers diagnosed in different pathological anatomy and cytology (ACP) laboratories of Dakar hospitals. Material and method: This was a retrospective study spanning from January 2013 to December 2018 at the pathological anatomy laboratories of Fann, Aristide Le Dantec and Idrissa Pouye hospitals. All patients with laryngeal cancer confirmed at histology were included. Data collection was based on clinical records of the patients and archives of histological reports of the pathological laboratories. Data analysis was performed under the Excel software. Results: We collected 215 cases of laryngeal cancer. The average age was 58.01 years with extremes of 07 and 94 years. The male sex was predominant with 183 patients (85.12%) against 32 women (14.88%). The most noted risk factor was tobacco which affected 14 patients, or 60.87%. Three patients (13.04%) did not present any alcohol-smoking impregnation. Clinically, dysphonia was noted in 22 patients (26.51%). It was associated with dyspnea in 0.48% of cases and dysphagia in 3.61%. Laryngoscopy was performed in 62 patients or 28.83% of cases, nasofibroscopy in 6.45% of cases. Cord arythenoid fixity was noted in two patients (3.23%) and hypo-mobility in 1 patient (1.61%). The most frequent local extension was involvement of the piriform sinus with 11.29% of cases. Palpable lymphadenopathy reported in 17 patients (20.48%). Pathological examination was performed in all patients after post endoscopic biopsy in 92 patients (42.79%), and after obtaining the operative specimen in 123 cases (57.21%). The three floors affected 76 patients (35.35%). The ulcerative budding aspect most noted concerned 108 patients (60.97%). Histologically, it was a squamous cell carcinoma in 205 patients (95.34%). The most site invaded by the tumor was cricoid cartilage.T4 type was most found (45 patients or 40.90%) followed by the T3 type with 34.55%. There was no lymphadenopathy invasion (Type N0) in 74 patients (67.27%), capsular rupture was reported in five patients (6.77%), no evaluable metastasis in 110 patients (97.27%), and stage IVA predominated in 66 patients (60%). Conclusion: laryngeal cancer is a reality in our contexts, however, its incidence is poorly understood in Senegal. The main risk factors remain tobacco and alcohol. Histology confirms the diagnosis. Its management is multidisciplinary and must be early.

Rosai-Dorfman-Destombes disease: case series and literature review.

Rosai-Dorfman-Destombes disease (RDD) is a rare histiocytic disorder with a prevalence of 1:200 000 people in the United States. The characteristic clinical presentation is a large and painful bilateral cervical mass in young adults, but more than a quarter of cases show extranodal manifestations. Skin, paranasal sinuses, soft tissue, bone, salivary glands, oral cavity, and central nervous system can be affected, resulting in a very distinct constellation of symptoms in each patient. Systemic manifestations such as fever and leukocytosis are also frequent. The usual course is benign, with spontaneous regression in many cases. However, a minority of patients suffer from a progressive disease, occasionally resulting in death, usually due to local infiltration of vital structures. 
 This study aims to describe the clinical, radiologic, and anatomopathological characteristics as well as individualized therapeutic procedures and outcomes of five cases of RDD disease that have been diagnosed in a reference hospital in São Paulo and compare these findings with recent bibliography on this rare disease.

Brachytherapy’s Clinical-Imagery Correlates and Tumor’s Reduction Outputs Among Patients with Cervical Cancer

Aim: The aim of this study was to investigate the clinical-imagery correlates of cervical cancer before and after brachytherapy, as well as the effects of tumor reduction resulting from the use of this radiation therapy. Methods: A prospective case series study was conducted during 2009-2012 in the University Hospital Center: "Mother Teresa", Oncology Service. Sampling technique included 202 consecutive women diagnosed with cervical cancer, presented at the hospital and met the study protocol’s inclusion criteria. The instrument used for cervical cancer imaging data was Computerized Tomography, which was used at different times, after applying the respective doses of brachytherapy. The paired t-test was used to estimate the differences between two variables for the same subject, such as imaging data after applying three doses of brachytherapy. Results: In terms of the distribution of signs and symptoms in cervical cancer patients, 27.2% of them had pain in the lower back, 33.2% referred for pain during sex, 6.4% had vaginal bleeding during sex, 24.8% referred changes to the vaginal discharge, and 30.2% had blood in urine. Only 5.8% of the patients have performed brachytherapy without combined forms of chemo-radiotherapy, since most of the patients (51%) were in stage 2 of cancer. The reductions of the mean tumor sizes in different application of brachytherapy doses such as (dose 3 vs. dose vs. dose 1 vs. before application) were 9.6%, 4.93% and 4.93%, respectively. All these reduction’s results (obtained after paired t-tests) were statistically significant, p<0.001. Conclusions: It is important to be aware of the clinical, imaging and anatomopathological characteristics of cervical cancer that are used to diagnose and treat this disease. The application of brachytherapy with or without external beam radiation therapy leads to a gradual reduction size of about 5 to 10% after each dose, until the tumour mass might fully diminished. Non-accidental head injury of infants: legal implications for the pediatric neurosurgeon.

Though the neurosurgeon's role in non-accidental head injury (NAHT) is the prompt recognition and care of the inflicted injuries, he/she should be aware of the possible legal implications related to this particular neurosurgical condition. Based on published data and their own clinical and medico-legal practice, the authors review the role of the neurosurgeon in NHAT. Besides the contribution that can be given by the neurosurgeon to a correct differential diagnosis, he/she is likely to be involved in the interpretations of the pathological findings in the case where the judge may request for a medical opinion concerning etiology, circumstances, severity, and consequences. As a member of a multidisciplinary team, usually the neurosurgeon is only requested for information regarding the lesions he was called to recognize and treat. Nevertheless, such information may have a pivotal part in the evaluation process. Consequently, the neurosurgeon should be able to reach a correct differential diagnosis of NAHT among all the events that may share similar clinical and anatomo-pathological characteristics and be aware of the ongoing scientific controversies related to the diagnosis and pathophysiology. In practical terms, the pediatric neurosurgeon is called to individuate and record all the precise details of the condition to be eventually offered to the judge in case of controversy. Whereas the diagnosis of NAHT should be evoked at the initial phase in order not to leave the child unprotected, all possible alternative hypotheses should be ruled out "beyond reasonable doubt" for the court. The medical file should be maintained in the record carefully and be accessible even after a long time.

P306 Scedosporium spp. and Lomentospora prolificans, fungal agents with unexpected vascular tropism

Poster session 2, September 22, 2022, 12:30 PM - 1:30 PM ObjectivesInvasive scedosporiosis and lomentosporiosis are deadly fungal infections due to Scedosporium spp. and Lomentospora prolificans. The Scedosporiosis/lomentosporiosis Observational Study (S.O.S.) highlighted for the first time a frequent vascular involvement in these infections including aortitis and peripheral arteritis (PA). We here describe the clinical, microbiological, radiological and anatomopathological characteristics of these vascular infections.MethodsWe retrospectively reviewed cases of arteritis (with the exception of central nervous system arteritis) from the S.O.S. cohort and from the literature.ResultsSeven cases of vascular infections were identified from the S.O.S. cohort representing 24% (7/29) of the disseminated scedosporiosis/lomentosporiosis. Four cases had both aortitis and PA, 2 patients were diagnosed with PA and one patient with aortitis. A total of 9 aortitis and 4 PA cases were identified from the literature. All 20 cases were proven scedosporiosis/lomentosporiosis. The main species was S. apiospermum [60% of cases (12/20)] followed by L. prolificans [35% of cases (7/20)]. One infection was caused by both species. An underlying immunosuppression was present in 70% of the cases (14/20, with 10 cases of solid organ transplantation and 3 cases of hematologic malignancies). The main risk factor in immunocompetent patients was a previous cutaneous trauma (4/6). Interestingly, vascular involvement was identified at diagnosis of the scedosporiosis/lomentosporiosis in only half of the cases. Aortitis was mainly abdominal (8/13). Various PA localizations were reported with frequent iliac or femoral involvement (4/10). Arteritis was the only localization in only 10% (2/20) of the scedosporiosis/lomentosporiosis, other sites involved being mainly osteoarticular (10/20), and pulmonary (9/20) followed by central nervous system (5/20), cutaneous localizations (4/20), and endocarditis (4/20). Of note, three-quarters of the cases were disseminated. Aneurysmal lesion was the most frequent imaging aspect (8/11 of aortitis and 6/10 of PA) which was complicated by a rupture in half of the aortitis (4/8) and only one PA (1/6). Vascular wall thickening (2/11 of aortitis and 1/10 of PA) and perivascular abscess (1/11 and 1/10, respectively) were more rarely described. Hypermetabolism was constant on PET-CT scan when performed (6/6). When available (11/20), pathological analysis showed an invasion of the artery wall by fungal hyphae (10/11), particularly in the media and the adventice. A total of 3-months of mortality related to infection was 44% (8/18), rising to 71% (5/7) in case of fungemia.ConclusionThe vascular tropism of Scedosporium spp. and L. prolificans underlies the necessity of vascular imaging in the management of these infections, especially in case of dissemination seeking in particular aneurysmal lesions of the abdominal aorta and iliofemoral arteries.

An effective algorithm to detect the possibility of being MSI phenotype in endometrial cancer given the BMI status and histological subtype: a statistical study

PurposeIn endometrial cancer, the incidence of mutations in mismatch repair genes (MMR) is estimated at 17–30%. Patients with alterations at this level (MSI) are known to have different clinical and anatomopathological characteristics than those without this genetic alteration (MSS). In this study, we aim to identify the MSI phenotype in patients who underwent hysterectomy for endometrial cancer. We assessed the correlation of this phenotype with anatomoclinical parameters such as obesity and histological subtype.Methods/patientsClinical and anatomopathological data were collected from 147 patients diagnosed with endometrial cancer and an immunohistochemical study of MMR system proteins was performed. PMS2 and MSH6 proteins were evaluated as primary screening and subsequent evaluation of MLH1 and MSH6, respectively, if the former were negative. Statistical association between the anatomopathological data and the immunohistochemical result was analyzed.Results and conclusions22.4% of our patients were MSI phenotype. We obtained statistically significant differences by multivariate analysis between endometrioid subtype and higher FIGO classification grade with MSI phenotype and obesity with MSS phenotype. Given these statistical results, we propose a function for predicting the probability of being MSI phenotype taking into account the histological subtype (endometrioid/non-endometrioid carcinoma) and FIGO grade as well as obesity. This prediction may be useful prior to hysterectomy, for genetic study of the MLH1 promoter and subsequent genetic counseling.

Prognostic analysis of lymph node ratio of patients with disease recurrence previously submitted to cervical dissection surgery for head and neck cancer.

the variable lymph node ratio has recently been studied as a possible influencer in the survival of patients diagnosed with head and neck cancer. to analyze the correlation between lymph node density and survival of recurred disease patients previously submitted to cervical dissection surgery due to head and neck squamous cell carcinoma. we retrospectively analyzed 71 medical records of patients treated at the Head and Neck Surgery Service of the Pontifícia Universidade Católica de Campinas who had undergone cervical dissection surgery and presented tumor recurrence between 2006 and 2019. Patient and tumor data such as age, gender, skin color, smoking, alcohol consumption, location of the primary tumor, anatomopathological characteristics and lymph node status were correlated with the survival time. we found a predominance of males and the mean age was 59.5 years. The most frequent primary site was the oral cavity followed by the larynx and oropharynx. The mortality rate was 53.52% and the mean lymph node ratio 0.28. We found influence on survival with statistical significance for the parameters: lymph node ratio, number of dissected and affected lymph nodes, T and N staging, type of treatment proposed (palliative or surgical), presence of compromited margins in the primary tumor and lymph node extravasation. the calculation of lymph node density in patients with recurred disease after cervical dissection surgery by head and neck squamous cell carcinoma should be taken into account during therapeutic planning and prognostic evaluation due to its direct influence on the survival.

Natural evolution of perforating wounds of 30% extension of the left diaphragm and the anatomopathological characteristics of its healing. Experimental Study.

diaphragmatic injury is a challenge for surgeons. It is an injury that can be isolated. It is frequent in penetrating thoracoabdominal trauma. It represents a diagnostic challenge and the ideal approach is not yet well established. The occurrence of spontaneous healing of these injuries is still much discussed and even more, if it does, what is the healing mechanism? to macroscopically and histologically evaluate the natural evolution of perforation and cutting wounds equivalent to 30% of the left diaphragm. 50 specimens of rats underwent a surgical procedure and, after 30 days, were euthanized and those that presented scar tissue in the diaphragm, the samples were submitted to histopathological study, using the hematoxylin and eosin stains, Massons trichrome and Picrosirius to assess the presence of collagen or muscle fibers (hyperplasia) in the scar. it was found that healing occurred in diaphragmatic injuries in 90% of rats. We also observed the presence of fibrosis in all analyzed samples. Spontaneous healing occurred in most diaphragmatic injuries and the inflammatory reaction represented by the presence of fibrosis and collagen deposition was observed in all our samples. Muscle fiber hyperplasia did not occur.

Nutritional and immunological parameters as prognostic factors in patients with advanced oral cancer

ObjectiveThe aim of the present study was to analyze the prognostic relationship of weight loss and preoperative hematological indexes in patients surgically treated for pT4a squamous cell carcinoma of the oral cavity. MethodsA retrospective cohort study. ResultsPercent weight loss greater than 10% was identified in 49 patients (28.2%), and any weight loss in relation to the usual weight occurred in 140 patients (78.7%). Percent weight loss greater than 10% (HR = 1.679), Red cell distribution width (RDW) values greater than 14.3% (HR = 2.210) and extracapsular spread (HR = 1.677) were independent variables associated with risk of death. ConclusionPatients with advanced squamous cell carcinoma of the oral cavity present significant weight loss and as significantly immunocompromised. Increased values of RDW and higher percentages of weight loss in relation to the individual's usual weight, together with extracapsular spread of metastatic lymph nodes, were risk factors for lower survival, regardless of other clinical and anatomopathological characteristics. TRIPLE NEGATIVE BREAST CANCER, EXPERIENCE OF MILITARY HOSPITAL RABAT: ABOUT 52 CASES

Objective: to review the epidemiological, clinical, anatomopathological, biological, therapeutic and prognostic characteristics of this histo-prognostic subgroup, namely triple-negative breast cancer Material and Method: Our work consists of a retrospective study carried out at the military hospital of instruction Med v rabat between January 2010 and December 2017 on 52 cases of invasive breast carcinoma with a triple negative phenotype. Result: From an epidemiological point of view: a peak in frequency was noted between 45 and 50 years old, and 48.1% of patients still in genital activity. On the anatomo-pathological level: invasive ductal carcinoma is the most predominant type representing 68.3% of cases with an average tumor size of 30mm. Histopronostic grades II and III each represent 43.2% and 56.8% of cases. Therapeutically: more than 42.3% received conservative treatment and 57.7% underwent radical surgery of the Pateytype.Adjuvant systemic chemotherapy was given in 86.5% of cases and 5.8% received neoadjuvant chemotherapy On the evolutionary level, 17.3% or 9 cases developed systemic metastases with predilection for the lungs. Conclusion: Despite advances in treatments and the emergence of targeted therapies, breast cancer remains the leading cause of death in women. Current clinical and histological classifications do not fully establish the prognostic and predictive parameters of response to treatment.

Unilateral Sinonasal Opacifications: A Histo-Radiological Correlation

AbstractVarious pathological entities may manifest on imaging as unilateral or bilateral nasal and/or sinus opacity. The vast majority is represented by inflammatory pathologies, tumors are rare, but they are dominated mainly by benign tumors. Malignant diseases are uncommon, accounting for 3% of tumors of the head and neck. Advances in imaging using preoperative computed tomography and magnetic resonance imaging have been significantly marked in the diagnostic approach to sinonasal pathologies. Surgical modalities are influenced by preoperative knowledge of the nature and topography of the tumor. The aim of this work is to describe the clinical, radiological, and anatomopathological characteristics of sinonasal pathologies expressed by unilateral sinonasal opacity in imaging, to identify the clinicoradiological variables likely to predict malignancy, and to make a correlation between the radiological images and the anatomopathological result.

Placental Findings in Preterm and Term Preeclampsia: An Integrative Review of the Literature.

Preeclampsia (PE) is a pregnancy complication associated with increased maternal and perinatal morbidity and mortality. The disease presents with recent onset hypertension (after 20 weeks of gestation) and proteinuria, and can progress to multiple organ dysfunction, with worse outcomes among early onset preeclampsia (EOP) cases (< 34 weeks). The placenta is considered the root cause of PE; it represents the interface between the mother and the fetus, and acts as a macromembrane between the two circulations, due to its villous and vascular structures. Therefore, in pathological conditions, macroscopic and microscopic evaluation can provide clinically useful information that can confirm diagnosis and enlighten about outcomes and future therapeutic benefit. To perform an integrative review of the literature on pathological placental findings associated to preeclampsia (comparing EOP and late onset preeclampsia [LOP]) and its impacts on clinical manifestations. Cases of EOP presented worse maternal and perinatal outcomes, and pathophysiological and anatomopathological findings were different between EOP and LOP placentas, with less placental perfusion, greater placental pathological changes with less villous volume (villous hypoplasia), greater amount of trophoblastic debris, syncytial nodules, microcalcification, villous infarcts, decidual arteriolopathy in EOP placentas when compared with LOP placentas. Clinically, the use of low doses of aspirin has been shown to be effective in preventing PE, as well as magnesium sulfate in preventing seizures in cases of severe features. The anatomopathological characteristics between EOP and LOP are significantly different, with large morphological changes in cases of EOP, such as hypoxia, villous infarctions, and hypoplasia, among others, most likely as an attempt to ascertain adequate blood flow to the fetus. Therefore, a better understanding of the basic macroscopic examination and histological patterns of the injury is important to help justify outcomes and to determine cases more prone to recurrence and long-term consequences.
