Auditory Effects of Acoustic Noise From 3-T Brain MRI in Neonates With Hearing Protection.

Neonates with immature auditory function (eg, weak/absent middle ear muscle reflex) could conceivably be vulnerable to noise-induced hearing loss; however, it is unclear if neonates show evidence of hearing loss following MRI acoustic noise exposure. To explore the auditory effects of MRI acoustic noise in neonates. Prospective. Two independent cohorts of neonates (N = 19 and N = 18; mean gestational-age, 38.75 ± 2.18 and 39.01 ± 1.83 weeks). T1-weighted three-dimensional gradient-echo sequence, T2-weighted fast spin-echo sequence, single-shot echo-planar imaging-based diffusion-tensor imaging, single-shot echo-planar imaging-based diffusion-kurtosis imaging and T2-weighted fluid-attenuated inversion recovery sequence at 3.0 T. All neonates wore ear protection during scan protocols lasted ~40 minutes. Equivalent sound pressure levels (SPLs) were measured for both cohorts. In cohort1, left- and right-ear auditory brainstem response (ABR) was measured before (baseline) and after (follow-up) MRI, included assessment of ABR threshold, wave I, III and V latencies and interpeak interval to determine the functional status of auditory nerve and brainstem. In cohort2, baseline and follow-up left- and right-ear distortion product otoacoustic emission (DPOAE) amplitudes were assessed at 1.2 to 7.0 kHz to determine cochlear function. Wilcoxon signed-rank or paired t-tests with Bonferroni's correction were used to compare the differences between baseline and follow-up ABR and DPOAE measures. Equivalent SPLs ranged from 103.5 to 113.6 dBA. No significant differences between baseline and follow-up were detected in left- or right-ear ABR measures (P > 0.999, Bonferroni corrected) in cohort1, or in DPOAE levels at 1.2 to 7.0 kHz in cohort2 (all P > 0.999 Bonferroni corrected except for left-ear levels at 3.5 and 7.0 kHz with corrected P = 0.138 and P = 0.533). A single 40-minute 3-T MRI with equivalent SPLs of 103.5-113.6 dBA did not result in significant transient disruption of auditory function, as measured by ABR and DPOAE, in neonates with adequate hearing protection. 2. Serum Prestin Level May Increase Following Music Exposure That Induces Temporary Threshold Shifts: A Pilot Study.

To determine if blood prestin level changes after exposure to music at high sound pressure levels, and if this change is associated with temporary threshold shift (TTS) and/or changes in distortion product (DP) amplitude. Participants were exposed to pop-rock music at 100 dBA for 15 min monaurally through headphones. Pure-tone audiometry, DP amplitude, and blood prestin level were measured before and after exposure. Fourteen adults (9 women; age range: 20 to 54 years, median age = 31 [Interquartile ratio = 6.75]) with normal hearing were included in the study. Mean prestin level increased shortly after exposure to music, then returned to baseline within 1 week, although this trend was not observed in all participants. All participants presented TTS or a decrease in DP amplitude in at least one frequency after music exposure. There was a statistically significant average threshold elevation at 4 min postexposure. Statistically significant DP amplitude shifts were observed at 4 and 6 kHz, 2 min following exposure. Mean baseline serum prestin level (mean: 140.00 pg/mL, 95% confidence interval (CI): 125.92 to 154.07) progressively increased following music exposure, reaching a maximum at 2 hr (mean: 158.29 pg/mL, 95% CI: 130.42 to 186.66) and returned to preexposure level at 1 week (mean: 139.18 pg/mL, 95% CI: 114.69 to 163.68). However, after correction for multiple comparisons, mean prestin level showed no statistically significant increase from baseline at any timepoint. No correlation between maximum blood prestin level change and average TTS or distortion product otoacoustic emission amplitude shift was found. However, in an exploratory analysis, TTS at 6 kHz (the frequency at which maximum TTS occurred) decreased significantly as baseline blood prestin level increased. The results suggest that blood prestin level may change after exposure to music at high sound pressure levels, although statistical significance was not reached in this relatively small sample after correction. Baseline serum prestin level may also predict the degree of TTS. These findings thus suggest that the role of baseline serum prestin level as a proxy marker of cochlear susceptibility to intense music exposure should be further explored.

A phase I/IIa safety and efficacy trial of intratympanic gamma-secretase inhibitor as a regenerative drug treatment for sensorineural hearing loss

Inhibition of Notch signalling with a gamma-secretase inhibitor (GSI) induces mammalian hair cell regeneration and partial hearing restoration. In this proof-of-concept Phase I/IIa multiple-ascending dose open-label trial (ISRCTN59733689), adults with mild-moderate sensorineural hearing loss received 3 intratympanic injections of GSI LY3056480, in 1 ear over 2 weeks. Phase I primary outcome was safety and tolerability. Phase lla primary outcome was change from baseline to 12 weeks in average pure-tone air conduction threshold across 2,4,8 kHz. Secondary outcomes included this outcome at 6 weeks and change from baseline to 6 and 12 weeks in pure-tone thresholds at individual frequencies, speech reception thresholds (SRTs), Distortion Product Otoacoustic Emissions (DPOAE) amplitudes, Signal to Noise Ratios (SNRs) and distribution of categories normal, present-abnormal, absent and Hearing Handicap Inventory for Adults/Elderly (HHIA/E). In Phase I (N = 15, 1 site) there were no severe nor serious adverse events. In Phase IIa (N = 44, 3 sites) the average pure-tone threshold across 2,4,8 kHz did not change from baseline to 6 and 12 weeks (estimated change −0.87 dB; 95% CI −2.37 to 0.63; P = 0.252 and −0.46 dB; 95% CI −1.94 to 1.03; P = 0.545, respectively), nor did the means of secondary measures. DPOAE amplitudes, SNRs and distribution of categories did not change from baseline to 6 and 12 weeks, nor did SRTs and HHIA/E scores. Intratympanic delivery of LY3056480 is safe and well-tolerated; the trial’s primary endpoint was not met.

Comparison of Two Clinical Devices for the Measurement of Distortion Product Otoacoustic Emissions in Normal-Hearing Adults.

Otoacoustic emissions (OAEs) are low-intensity sounds generated by the cochlea and associated with the function of the outer hair cells. Since OAE measurements do not require active participation of a listener, OAEs are considered an objective measure of cochlear function. While distortion-product OAEs (DPOAEs) are commonly used in clinical practice, limited information is available on the performance of various clinical devices. This study compared two commercial clinical devices, Titan and Eclipse from Interacoustics, and collected normative data for DPOAEs in people with normal hearing. The data collection and analysis were focused on signal and noise amplitudes as well as signal-to-noise ratios (SNRs). Sixty-three participants with normal hearing (age 23.2±2.0 years) were included in the study. DPOAEs were measured at 31 frequencies ranging from 500 to 10,000 Hz. DPOAE amplitude differed across frequencies. Additionally, a high number of unreliable responses were observed at the edge frequencies (<828 Hz and >6,072 Hz). There were no significant differences between the two devices in terms of DPOAE amplitudes, but the recorded-noise levels (amplitude) differed significantly. SNRs of the recorded OAEs were frequency dependent (higher frequencies showed larger SNRs), and significant differences in terms of SNR were found between the two devices. Despite the above-mentioned differences, the OAEs recorded with both devices met the pass criteria for the SNR (≥6 dB) consistently across frequencies, and thus the differences do not compromise the test outcomes. The frequency dependence of the OAE amplitudes and the corresponding SNRs may be relevant for clinical practice.

Development and Validation of an Efficient and Safe Loud Music Exposure Paradigm.

The purpose of this study was to develop a time-efficient music exposure and testing paradigm that safely creates temporary cochlear dysfunction that could be used in future temporary threshold shift (TTS) studies. A 30-min audio compilation of pop rock music tracks was created. Adult volunteers with normal hearing were then exposed to this music material monaurally through headphones for 30 min at 97 dBA or 15 min at 100 dBA. Levels were measured from the ear of a manikin and are considered to provide an equivalent daily noise dose based on a 3-dB exchange. We assessed the changes in their hearing, by means of distortion product otoacoustic emission (DPOAE) testing, and standard and extended high-frequency pure-tone audiometry before and after exposure. There were 17 volunteers in total. In the first trial, eight volunteers (four females; Mdnage = 31 years [interquartile range, IQR = 4.25]) were included. Although TTS was observed in all eight participants for at least one frequency, a large variation in affected frequencies was observed. To address this issue, the audio material was further remastered to adjust levels across the different frequency bands. Fourteen adults (nine newly recruited and five from the first trial; seven females; Mdnage = 31 years [IQR = 5]) were exposed to the new material. All but two of 17 participants presented clinically significant TTS or decrease in DPOAE amplitude in at least one frequency. Statistically significant average TTS of 7.43 dB was observed at 6 kHz. There were statistically significant average DPOAE amplitude shifts of -2.55 dB at 4 kHz, -4.97 dB at 6 kHz, and -3.14 dB at 8 kHz. No participant presented permanent threshold shift. A monaural music paradigm was developed and shown to induce statistically significant TTS and DPOAE amplitude shifts, without evidence of permanent loss. Effect of Head Position on Distortion Product Otoacoustic Emissions

Abstract Objectives The study aimed to explore the impact of various head positions on the distortion product otoacoustic emissions (DPOAE) amplitudes, identifying the specific DPOAE frequencies showing the effect was also of interest. Materials and Methods DPOAEs were recorded from the right ears of 50 normal-hearing individuals in six head positions, supine, and five sitting positions (head erect, roll left, roll right, pitch backward, and pitch forward). DPOAEs were averaged and measured for their overall and frequency-specific amplitudes at 1 kHz, 1.5 kHz, 2 kHz, 3 kHz, 4 kHz, 6 kHz, and 8 kHz. Statistical Analysis To investigate the statistical significance of the observed mean differences, the data was tested using repeated measures of analysis of variance (ANOVA) following the Shapiro–Wilk normality test. The pair-wise comparison was tested using the Bonferroni post-hoc test and one-sided Bayesian paired sample t-test. Results The results of ANOVA revealed a significant main effect of head posture only at 1 kHz. The Bonferroni post-hoc test and one-sided Bayesian paired sample t-test results showed significantly higher DPOAE amplitude in the head erect compared to the supine position. There were no significant differences between other pairs of head postures. Conclusion Head posture is a trivial influencing factor of DPOAEs and hence, does not require consideration in interpreting DPOAEs.

Distortion Product Otoacoustic Emission and Tinnitus: Role of Outer Hair Cells in the Genesis of Tinnitus

Background: This study aims to detect outer hair cell (OHC) function in patients with tinnitus, by comparing the otoacoustic emissions (OAEs) in tinnitus patients with normal hearing and normal-hearing subjects without tinnitus, thereby revealing the role of OHC in the generation of tinnitus. Methods: After approval by the local research and ethics committee, this study was done in the department of otolaryngology and associated hospitals, from April 2022 to January 2023. Consent was obtained from all the participants in the present work after explaining the test procedure. The study group included 60 subjects with bilaterally normal hearing to participate. They were classified into two groups: Group 1, i.e., the control group, comprised 30 subjects who were unaffected by tinnitus, whereas Group 2, i.e., the study group, comprised 30 subjects who were suffering from tinnitus. Basic audiological tests and OAE test were done on the respondents of both the groups followed by a comparison of outcomes. Results: The authors of the present study found a statistically significant difference between the amplitude and signal-to-noise ratio (SNR) of distortion product OAE (DPOAE) in tinnitus patients and the control group without tinnitus, suggesting that OHCs have a role in the generation of tinnitus. Conclusion: It can be concluded that OHC activity was reduced in patients with tinnitus, as detected by reduced DPOAE amplitude and SNR, suggesting that the dysfunction of OHCs can be attributable to the generation of tinnitus.

Similar Tuning of Distortion-Product Otoacoustic Emission Ratio Functions and Cochlear Vibrations in Mice.

When elicited by two stimulus tones (at frequencies f1 and f2, f2 > f1), the amplitudes of specific distortion-product otoacoustic emission (DPOAE) components exhibit a characteristic bandpass shape as the ratio between f2 and f1 is varied. This bandpass shape has been attributed to various mechanisms including intracochlear resonance, suppression, and wave interference, and has been proposed to be related to cochlear frequency tuning. While human studies suggest modest correlations between psychophysical tuning and the tuning of DPOAE amplitude vs. f2/f1 ratio functions, a relationship between the latter and the tuning of cochlear mechanical responses has yet to be established. This was addressed here through direct comparisons of DPOAEs and cochlear vibrations in wild-type CBA/CaJ mice. DPOAEs were elicited using a fixed-f2, swept-f1 paradigm, and optical coherence tomography was used to measure displacements from cochlear locations with characteristic frequencies near f2. The tuning sharpness of 2f1-f2 DPOAE ratio functions was found to be remarkably similar to that of basilar membrane and/or tectorial membrane responses to single tones, with the tuning sharpness of all responses increasing similarly with decreasing stimulus level. This relationship was observed for f2 frequencies ranging from ~8 to 22 kHz. Intracochlear distortion products did not exhibit a bandpass shape as the f2/f1 ratio was varied, indicating that interference between distortion products traveling to the stapes may be responsible for the tuning of the DPOAE ratio function. While these findings suggest that DPOAE ratio functions could be used to noninvasively infer cochlear tuning, it remains to be determined whether this relationship holds in other species and for lower frequency regions.

Comprehensive behavioral and physiologic assessment of peripheral and central auditory function in individuals with mild traumatic brain injury

Auditory complaints are frequently reported by individuals with mild traumatic brain injury (mTBI) yet remain difficult to detect in the absence of clinically significant hearing loss. This highlights a growing need to identify sensitive indices of auditory-related mTBI pathophysiology beyond pure-tone thresholds for improved hearing healthcare diagnosis and treatment. Given the heterogeneity of mTBI etiology and the diverse peripheral and central processes required for normal auditory function, the present study sought to determine the audiologic assessments sensitive to mTBI pathophysiology at the group level using a well-rounded test battery of both peripheral and central auditory system function. This test battery included pure-tone detection thresholds, word understanding in quiet, sentence understanding in noise, distortion product otoacoustic emissions (DPOAEs), middle-ear muscle reflexes (MEMRs), and auditory evoked potentials (AEPs), including auditory brainstem responses (ABRs), middle latency responses (MLRs), and late latency responses (LLRs). Each participant also received magnetic resonance imaging (MRI). Compared to the control group, we found that individuals with mTBI had reduced DPOAE amplitudes that revealed a compound effect of age, elevated MEMR thresholds for an ipsilateral broadband noise elicitor, longer ABR Wave I latencies for click and 4 kHz tone burst elicitors, longer ABR Wave III latencies for 4 kHz tone bursts, larger MLR Na and Nb amplitudes, smaller MLR Pb amplitudes, longer MLR Pa latencies, and smaller LLR N1 amplitudes for older individuals with mTBI. Further, mTBI individuals with combined hearing difficulty and noise sensitivity had a greater number of deficits on thalamic and cortical AEP measures compared to those with only one/no self-reported auditory symptoms. This finding was corroborated with MRI, which revealed significant structural differences in the auditory cortical areas of mTBI participants who reported combined hearing difficulty and noise sensitivity, including an enlargement of left transverse temporal gyrus (TTG) and bilateral planum polare (PP). These findings highlight the need for continued investigations toward identifying individualized audiologic assessments and treatments that are sensitive to mTBI pathophysiology.

Effects of Body Position on Cochlear Function in Infants: An Otoacoustic Emission Study.

Otoacoustic Emission (OAE) is frequently recorded in various body positions for infants. However, little is available about whether these deviations will produce non-pathological effects on the clinical results. The current study assessed body position's effect on infants' inner ear function. Sixty normally hearing infants participated in an analytical cross-sectional study. Distortion-product OAEs (DPOAEs) were measured in the supine, side-lying, and upright positions. The DPOAE amplitude and signal-to-noise ratio (SNR) were recorded across the 1500 to 6000Hz range. The mean DPOAE amplitude and SNR values were significantly greater in the upright position than supine and side-lying positions (p < 0.05). These differences were more pronounced in the 3000 to 6000Hz range. The effects of gender and ear asymmetry on DPOAEs were not statistically significant. Our findings suggested that the upright position could be regarded as the best position for assessing DPOAEs in infants.

Normal linear and non-linear cochlear mechanisms and efferent system functioning in individuals with misophonia.

Misophonia, a condition characterized by heightened sensitivity and strong emotional reactions to specific sounds, has sparked considerable interest and debate regarding its underlying auditory mechanisms. The study aimed to understand the auditory underpinnings of two such potential inner ear systems, non-linear and linear outer hair cell functioning along with auditory efferent functioning in individuals with misophonia. 40 ears with misophonia (20 participants) and 37 ears without misophonia (20 participants), both having normal hearing sensitivity were included in this study. Transient evoked otoacoustic emissions (TEOAEs) and distortion product otoacoustic emissions (DPOAEs) were obtained in two conditions (with and without contralateral noise). Results of independent-samples t-test showed no statistically significant difference (p > 0.05) in the absolute amplitudes of both TEOAEs and DPOAEs between the individuals with and without misophonia. There was no statistically significant difference (p > 0.05) observed in the magnitude of suppression amplitude between the two groups for in both TEOAEs and DPOAEs between individuals with and without misophonia. These results suggest that the cochlear and efferent auditory underpinnings examined in this study may not be major contributors to the development or manifestation of misophonia.

Otoacoustic emission measurements: a test–retest reliability study

ObjectivesOtoacoustic emissions (OAEs) are an important part of the audiological test battery and have many clinical uses. This study aims to determine the amplitude changes in the test–retest condition of distortion product otoacoustic emissions (DPOAEs) and transient-evoked otoacoustic emissions (TEOAEs), which are widely used in clinical settings.DesignDPOAE and TEOAE measurements were taken in 110 ears of 55 adults aged 18–35 years with normal hearing during three sessions. The repeatability of the measurements was evaluated by very short-term measurements taken 20 min after the first measurement and by short-term measurements taken 20 days after the first measurement.ResultsThere was no statistically significant difference between the three measurements in which DPOAE and TEOAE amplitudes were evaluated. The weakest reliability for TEOAEs was determined at frequencies of 1.0 kHz and 1.5 kHz, and the weakest reliability for DPOAEs was determined at 6728 Hz.ConclusionsThe current findings indicate that DPOAE and TEOAE measurements are reliable for monitoring cochlear function over time. The data obtained from this study could help clinicians correctly interpret OAE changes and distinguish between physiological and pathological changes.

Cochlear Function in Individuals with and without Spontaneous Otoacoustic Emissions.

This study investigated the status of spontaneous otoacoustic emissions (SOAEs) on cochlear function in a cohort of male/female participants with a wide age range. It examined whether there was a correlation between the presence of SOAEs and measurements of transient evoked otoacoustic emissions (TEOAEs), distortion product otoacoustic emissions (DPOAEs), SOAEs and extended high-frequency (EHF) hearing thresholds. 463 participants (222 male, 241 female; age range 20-59 years) with pure-tone thresholds ≤25 dB HL for octave frequencies of 500-8000 Hz were included in the study, divided into three age groups (20-29, 30-39, and 40-59 years). Evaluations included EHF (9000-16,000 Hz) hearing thresholds and TEOAE, DPOAE and SOAE measures. Multiple regression models showed that participants with SOAEs had larger expected amplitudes and signal-to-noise ratios (SNRs) for TEOAE and DPOAE responses than participants without SOAEs, holding gender and age variables constant. Spearman correlation tests identified deterioration in TEOAE and DPOAE amplitudes and SNRs, and EHF hearing thresholds with age in participants without SOAEs. Among participants with SOAEs, no significant decreases in TEOAE and DPOAE measures were shown in participants with older age. Nonetheless, as expected, EHF hearing thresholds did become worse with age, with or without SOAEs. Participants with identifiable SOAEs had greater TEOAE and DPOAE amplitudes and SNRs than participants without SOAEs. SOAEs appear to be a useful marker of cochlear health in adults.

Generation of distortion product otoacoustic emissions in infants with a combined air and bone conduction stimulus.

Evaluation of Distortion Product Otoacoustic Emissions (DPOAEs) by combining Air Conduction (AC) and Bone Conduction (BC) stimuli in infants. Measurements were performed in 19 normal hearing infants, and in 23 adults serving as a control group. The stimulus consisted either of two AC tones, or of combined AC/BC tones. DPOAEs were measured for f2 at 0.7, 1, 2, 4kHz, and a constant ratio of f2/f1=1.22. Sound pressure level of the primary stimulus L1 was held constant at 70dB SPL, while the level of L2 was decreased in 10dB steps from 70 to 40dB SPL. A response was included for further analysis when DPOAEs reached a Signal to Noise Ratio (SNR) of ≥6dB. Additional DPOAE responses of <6dB SNR were included when visual inspection of the measurements indicated clear DPOAEs. DPOAEs could be elicited in infants at 2 and 4kHz for the AC/BC stimulus. DPOAE amplitudes evoked by the AC/AC stimulus were larger than those by the AC/BC stimulus, with the exception of 1kHz. The highest amplitudes of DPOAEs were registered for a stimulation level of L1=L2=70dB, with the exception of AC/AC at 1kHz, where the highest amplitudes were with L1-L2=10dB. We demonstrated that DPOAEs can be generated in infants by a combined AC/BC stimulus at 2 and 4kHz. The high noise floor needs to be further reduced to achieve more valid measurements in frequencies <2kHz.

Distortion product otoacoustic emissions in newborn babies with and without late-term maternal iron deficiency anaemia

BackgroundStudies on animals have demonstrated that maternal iron deficiency anaemia (IDA) could result in decreased cochlear sensory hair cells and reduced amplitudes of distortion-product otoacoustic emissions (DPOAEs) of young guinea pigs. Thus, it is essential to study the functioning of cochlear hair cells using DPOAEs in human newborn babies with maternal IDA. The current study explores maternal IDA's effect on DPOAEs in newborn babies. MethodA total of 110 newborn babies with gestational age ≥34 weeks were considered and a ‘between-subjects’ design was used. The participants were divided into 3 groups- “Normal” (61 babies without maternal IDA), “Mild” (28 babies with mild maternal IDA) and “Moderate” (21 babies with moderate maternal IDA). The cord blood was collected and the DPOAEs were recorded for each baby for a range of frequencies (1 k – 8 kHz) and a range of intensities (70–40 dB SPL in 10 dB steps). ResultsThe analysis of both DP-gram and DP input-output (I/O) function showed that there was no significant difference (p > 0.05) across the normal, mild, and moderate groups in the overall presence of DPOAEs as well as the amplitude across frequencies or intensities (70–40 dB SPL). Also, the overall correlation of RBC indices with DPOAE amplitude across frequencies as well as the slope of the I/O function showed no relationship. ConclusionThe current study concludes that there is no effect of late-term maternal IDA on the DPOAEs of newborn babies.

ABO blood group and cochlear function: evidence from a large sample size study

Objective The present study investigated the effect of blood group on cochlear function in a large participant sample across different age groups. The study hypothesis was that participants with blood group O would show relatively reduced cochlear function as reflected in otoacoustic emission (OAE) measures. Design Data were collected from transient evoked otoacoustic emission (TEOAE), distortion product otoacoustic emission (DPOAE), DPOAE input/output (I/O) function, and spontaneous otoacoustic emission (SOAE) recordings. Study sample Four hundred and sixty-three normal hearing adults aged 20–59 years among the four ABO blood groups participated in the study. Results TEOAE and DPOAE amplitudes did not reveal significant differences for participants with blood group O compared with participants with non-O blood groups. No significant differences in I/O function categories were found among participants with different blood groups. SOAE prevalence was also not significantly different across blood groups. However, previously reported age and gender differences for OAE variables were confirmed. Conclusions Participants with blood group O were not found to have significantly reduced cochlear function, based on OAE measures. Results from the current study do not support the hypothesis that normal hearing individuals with different ABO blood groups differ in level of cochlear function.

Peripheral Auditory Function in Tanzanian Children Living With HIV With Clinically Normal Hearing

Despite normal audiometry, adults living with HIV have lower distortion product otoacoustic emissions (DPOAEs) compared with HIV-negative controls, but the degree of these differences in children living with HIV is unknown. If subclinical auditory deficits are present, results could affect developmental outcomes in children living with HIV (CLWH). To compare DPOAEs and auditory brainstem responses (ABR) between 2 age- and sex-matched groups of younger children with normal audiometry, 1 infected with HIV and the other uninfected. Cohort study in an infectious disease center in Dar es Salaam, Tanzania. Participants included 340 Tanzanian children aged 3 to 9 years with clinically normal hearing, type A tympanograms bilaterally, and air-conduction thresholds of 20 dB HL or less from 0.5 to 8 kHz. Participants in the cohort repeated testing approximately every 6 months (approximately 2.2 sessions per participant) for a total of 744 total observations. Data were analyzed from March 2020 to January 2022. DPOAE amplitudes from 1.5 to 8 kHz using an f2 to f1 ratio of 1.2 and L1/L2 values of 65/55 dB sound pressure level and click-evoked ABR using a slow (21.1/s) and fast (61.1/s) click rate. A total of 141 CLWH (70 female participants [49.3%]; mean [SD] age, 7.24 [1.67] years) and 199 HIV-negative individuals (99 female participants [49.7%]; mean [SD] age, 7.26 [1.44] years) participated in the study. The groups did not differ significantly in age, static immittance, or air-conduction thresholds. HIV status was independently associated with approximately 1.4 dB (95% CI, -3.28 to 0.30 dB) to 3.8 dB (95% CI, 6.03 to -1.99 dB) lower DPOAE amplitudes at 6 and 8 kHz bilaterally and 0.28 μV (95% CI, 0.01 to 0.33 μV) lower ABR wave V amplitudes in the right ear. Consistent with previous findings in young adults, CLWH had slightly, but reliably, lower DPOAEs and ABR wave V amplitudes than HIV-negative controls. The magnitude of these differences was small, but results suggest an early and consistent association between HIV infection or treatment and outer hair cell and auditory brainstem responses in children as young as 3 years. These subclinical changes suggest tracking both auditory function and development outcomes in CLWH is warranted.

Impact of endolymphatic hydrops on DPOAE in subjects with normal to mild hearing loss.

The increased endolymph volume affects a shift in the organ of Corti and basilar membrane in ears with endolymphatic hydrops (EH), which might affect distortion-product otoacoustic emissions (DPOAE) by altering the operating point of the outer hair cells. We investigated how changes in DPOAE are related to the distribution site of EH. Prospective study. Among 403 patients with hearing or vestibular symptoms who underwent contrast-enhanced magnetic resonance imaging (MRI) for the diagnosis of EH and subsequent DPOAE testing, subjects whose hearing levels on pure tone audiometry were ≤35dB at all frequencies were included in this study. In patients with EH on MRI, the presence and amplitude of DPOAE were evaluated between groups with hearing levels of ≤25dB at all frequencies versus hearing levels of >25dB at one or more frequencies. There were no differences in the distribution of EH between groups. The amplitude of DPOAE had no clear correlation with the presence of EH. However, in both groups, there was a significantly higher probability of the presence of a DPOAE response from 1001 to 6006Hz in cases with EH in the cochlea. Among patients whose hearing levels were ≤35dB at all frequencies, better responses on DPOAE testing were found in subjects with EH in the cochlea. Alteration of DPOAEs in the early stages of hearing impairment could indicate morphological changes in the inner ear with altered basilar membrane compliance due to EH. 4.

Development of an audiological assessment and diagnostic model for high occupational noise exposure.

To explore the diagnostic auditory indicators of high noise exposure and combine them into a diagnostic model of high noise exposure and possible development of hidden hearing loss (HHL). We recruited 101 young adult subjects and divided them according to noise exposure history into high-risk and low-risk groups. All subjects completed demographic characteristic collection (including age, noise exposure, self-reported hearing status, and headset use) and related hearing examination. The 8kHz (P = 0.039) and 10kHz (P = 0.005) distortion product otoacoustic emission amplitudes (DPOAE) (DPs) in the high-risk group were lower than those in the low-risk group. The amplitudes of the summating potential (SP) (P = 0.017) and action potential (AP) (P = 0.012) of the electrocochleography (ECochG) in the high-risk group were smaller than those in the low-risk group. The auditory brainstem response (ABR) wave III amplitude in the high-risk group was higher than that in the low-risk group. When SNR = -7.5dB (P = 0.030) and -5dB (P = 0.000), the high-risk group had a lower speech discrimination score than that of the low-risk group. The 10kHz DPOAE DP, ABR wave III amplitude and speech discrimination score under noise with SNR = -5dB were combined to construct a combination diagnostic indicator. The area under the ROC curve was 0.804 (95% CI 0.713-0.876), the sensitivity was 80.39%, and the specificity was 68.00%. We expect that high noise exposure can be detected early with this combined diagnostic indicator to prevent HHL or sensorineural hearing loss (SNHL). ChiCTR2200057989, 2022/3/25.

Subclinical Hearing Deficits in Noise-Exposed Firefighters.

Noise-induced hearing loss (NIHL) is the most prevalent occupational disease in the world and firefighters are at increased risk of NIHL due to their frequent exposure to hazardous levels of noise during service. Adverse effects of NIHL include acceleration of age-related hearing loss and an increased risk of cognitive decline. A critical challenge in addressing NIHL is the delayed clinical presentation of symptoms and lack of sensitive tools for early detection. To study the early clinical symptoms of NIHL in this high-risk group, we collected hearing function data including behavioral audiometric thresholds and distortion product otoacoustic emissions (DPOAEs) in 176 firefighters during annual physical assessments. Results revealed significant deficits in cochlear outer hair cell function in the presence of normal audiograms. Additionally, 55% of firefighters self-reported changes in hearing, while 20% self-reported concerns about their balance. This study is the first to characterize DPOAEs in firefighters who display decreased DPOAE amplitudes with increasing years in the fire service. These effects were observed even when controlling for hearing loss and age and are suggestive of a link between hearing loss and occupational exposure to hazardous noise.
