Costs, efficiency, and patient-reported outcomes associated with suture-mediated percutaneous closure for atrial fibrillation ablation: Secondary analysis of a randomized clinical trial.

To evaluate the cost and efficiency of suture-mediated percutaneous closure (SMC) compared to manual compression (MC) after atrial fibrillation (AF) ablation. SMC has been demonstrated to be efficacious in reducing hemostasis and bedrest times after AF ablation. To date, randomized data comparing the direct and indirect cost between the two techniques have not been described. We conducted a 1:1 randomized trial comparing SMC to MC following AF ablation. The primary endpoints have been previously published. However, secondary endpoints pertinent to indirect cost including complication rates, hospital utilization (i.e., delays in discharge, additional patient encounters, nursing utilization), pain, patient reported outcomes, as well as the direct costs of care associated with AF ablation were collected. We also performed secondary analysis of the primary endpoint to evaluate for a learning curve, and subgroups analysis comparing efficacy across different numbers of access sites and compared to those in the MC group with a figure-of-eight suture (Fo8), that could potentially have impacted the relative efficiency of the procedure. A total of 107 patients were randomized and included: 53 in the SMC group and 54 in MC. A learning curve was observed in the SMC group between the first and second half of the study group (p = 0.037), with no such difference in the MC group. After accounting for the number of access sites, time to hemostasis remained shorter in the SMC Group (p = 0.002). Compared to those in the Fo8 arm (n = 37), the time to hemostasis remained shorter in the SMC group (p = 0.001). Among those planned for same-day discharge, there were more delays in the MC group (31.5% vs. 11.3%, p = 0.0144). Rates of major and minor complications were similar between SMC and MC groups at discharge (p = 0.243) and 30 days (p = 1.00), as were nursing utilization, self-reported pain, and overall patient reported outcomes. The overall cost of care related to the procedure was similar between the MC and SMC groups ($56 533.65 [$45 699.47, $66 987.64] vs. $57 050.44 [$47 251.40, $66 426.34], p = 0.601). SMC has been shown to decrease time to hemostasis and ambulation and facilitate earlier same-day discharge after AF ablation without an increase in direct or indirect costs.

Activation patterns and electrophysiologic characteristics of Marshall bundle–related left atrial tachycardias after atrial fibrillation ablation

BackgroundEpicardial Marshall bundles (MBs) are frequently used in left atrial tachycardias (LATs) after atrial fibrillation (AF) ablation with pulmonary vein isolation and substrate modification. ObjectiveThis study sought to classify different activation patterns of MB-mediated LATs and the corresponding electrophysiologic characteristics. MethodsFrom 2019 to 2021, 28 cases of atrial tachycardias after AF ablation were diagnosed as MB-mediated LATs by ultrahigh-density mapping and entrainment. Cannulation and mapping in the vein of Marshall (VOM) and epicardial mapping in the MB region were also performed in selected cases to further prove the mechanism. ResultsThree activation patterns were identified with a critical isthmus through the MB: perimitral macroreentry (perimitral LAT; n = 20 [71.4%]); left atrial appendage–related reentry (n = 5 [17.9%]); and left pulmonary vein–related reentry (n = 3 [10.7%]). In 18 patients, a characteristic triple potential observed along the previously endocardial left atrial ridge block line was composed of near-field double potentials and far-field MB potential. These findings were further delineated in 24 patients with either cannulation in the VOM (19 patients) or epicardial mapping (5 patients). Ethanol infusion of the VOM resulted in atrial tachycardia termination in 20 of 28 patients. ConclusionDifferent types of MB-mediated LATs after AF ablation could be identified by ultrahigh-density mapping. Ethanol infusion within the VOM was effective in eliminating these tachycardias.

Efficacy and safety of Saireito (TJ-114) in patients with atrial fibrillation undergoing catheter ablation procedures: A randomized pilot study.

Early arrhythmia recurrences commonly occur after atrial fibrillation (AF) ablation because of irritability and inflammation of left atrium. We hypothesized that short-term use of Saireito would be effective in reducing frequent atrial tachyarrhythmias in the early-phase post-ablation. One hundred patients undergoing catheter ablation for symptomatic AF were randomly assigned to either a 30-day use of Saireito or control group. The primary endpoint was total number of episodes of frequent atrial tachyarrhythmias including definite recurrent AF and frequent premature atrial contractions during the 30-day treatment period post-ablation. Three (6.0%) out of 50 patients treated with Saireito discontinued the drug because of adverse symptoms. The Saireito group was associated with a numerically lower number of episodes of frequent atrial tachyarrhythmias than the control group (3.1 versus 5.2 times, P = 0.17). The mean daily episodes of frequent atrial tachyarrhythmias were significantly fewer in the Saireito group during Day-6 to Day-10 (0.12/day versus 0.27/day, P = 0.03), and during Day-11 to Day-15 (0.08/day versus 0.24/day, P = 0.001). The prevalence of adverse symptoms during the 30-day treatment period was significantly higher in the Saireito group (18.0% versus 2.0%, P = 0.005). Thirty-day use of Saireito following AF ablation was associated with a tendency toward reduced number of episodes of frequent atrial tachyarrhythmias during the treatment period, with more pronounced effect in the first two weeks.

It can be safe to discontinue oral anticoagulants after successful atrial fibrillation ablation: A systematic review and meta-analysis of cohort studies.

Current guidelines recommended that oral anticoagulants (OACs) should last for a minimum first 2 months after atrial fibrillation (AF) ablation and the long-term decision of anticoagulation after AF ablation should be based on the individual patient's risk of stroke rather than the rhythm status. There is controversy about the safety of discontinuing OACs in patients with atrial fibrillation after the blanking period due to the divergences between consensus recommendations and clinical practice. Electronic bibliographic sources (PubMed, Embase, and Web of Science) were searched until August 2023 to identify cohort studies about the safety of discontinuing OACs in patients with AF after the blanking period. The primary outcome was thromboembolism (TE). The secondary outcome was major bleeding events (MBEs). Two authors extracted articles independently using predefined data fields. The pooled odds ratios (ORs) and 95% confidence intervals (CIs) were calculated based on a random-effects model. A total of 16 studies (11 prospective cohorts and 5 retrospective cohorts) enrolling 23,942 patients (14,382 OFF-OAC and 9560 ON-OAC) were included in our analysis. No significant difference emerged in the risk of TE between OFF-OAC and ON-OAC patients following AF ablation after the banking period (OR = 0.66; 95%CI, 0.43-1.01). Similar results emerged in the patients with a high risk of TE after stratification by the risk level of TE (OR = 0.72; 95%CI, 0.25-2.08). A significant reduction in incidences of major bleeding was found in the OFF-OAC patients compared with the ON-OAC patients (OR = 0.23; 95%CI, 0.12-0.42). Subgroup analyses for TE found a reduction of incidences in the subgroups who switched to antiplatelet drugs and with a follow-up duration <3 years. Subgroup analyses for MBEs found a significant reduction of incidences in all subgroups. Our study suggests it can be safe to discontinue OACs after successful AF ablation. Discontinuation of OACs may reduce the risk of MBEs while not increasing the risk of TE.

The ABC-Death Score for Mortality Prediction in Patients With Atrial Fibrillation Undergoing Catheter Ablation

BackgroundData on the performance of risk scores in predicting mortality risk after atrial fibrillation (AF) ablations are limited. ObjectivesThe purpose of this study was to investigate the associations of mortality with preablation biomarkers and evaluate the performance of age, biomarker, and clinical history (ABC)-death score in patients with AF undergoing catheter ablation. MethodsPatients with AF undergoing catheter ablations between 2013 and 2019 in the Chinese Atrial Fibrillation Registry were enrolled. Biomarkers associated with ABC-death score were quantified from baseline blood samples collected before AF ablation. Clinical outcomes were all-cause mortality and cardiac mortality. Discrimination, reclassification, clinical use, and calibration were further evaluated. ResultsWe identified 4,218 patients with AF undergoing catheter ablations. During a median follow-up period of 4.0 years, 119 patients died due to all causes, with 49 dying due to cardiac causes. Biomarker levels were all independently associated with an increased risk of all-cause death and cardiac death. The ABC-death score was superior to the CHA2DS2-VASc score in predicting all-cause death (C index 0.73 vs 0.63; P = 0.001) and cardiac death (C index 0.83 vs 0.71; P = 0.007). Reclassification analysis revealed significant reclassification improvements of the ABC-death score compared with the CHA2DS2-VASc (cardiac failure or dysfunction, hypertension, age ≥75 [doubled], diabetes mellitus, stroke [doubled]—vascular disease, age 65 to 74 years and sex category [female]) score. Decision curve analysis showed the greater net benefit of use of the ABC-death score. Calibration plots presented an overestimation of the observed mortality event rate by ABC-death score. ConclusionsPreablation biomarkers associated with ABC-death score were independently related to increased all-cause and cardiac mortality risk. Despite the overestimation of the event rate, the ABC-death score outperformed the CHA2DS2-VASc score in discriminating and reclassifying mortality risk, especially for cardiac mortality.

Unipolar-voltage-based evaluation of left atrial tissue properties and ablation outcome in patients with atrial fibrillation.

The relationship between local unipolar voltage (UV) in the pulmonary vein (PV)-ostia and left atrial wall thickness (LAWT) and the utility of these parameters as indices of outcome after atrial fibrillation (AF) ablation remain unclear. Two-hundred seventy-two AF patients who underwent AF ablation were enrolled. Unipolar voltage of PV-ostia was measured using a CARTO system, and LAWT was measured using computed tomography. The primary endpoint was atrial tachyarrhythmia (ATA) recurrence including AF. The ATA recurrence was documented in 74 patients (ATA-Rec group). The UV and LAWT of the bilateral superior PV roof to posterior and around the right-inferior PV in the ATA-Rec group were significantly greater than in patients without ATA recurrence (ATA-Free group) (P < 0.001). The UV had a strong positive correlation with LAWT (R2 = 0.446, P < 0.001). The UV 2.7 mV and the corresponding LAWT 1.6 mm were determined as the cut-off values for ATA recurrence (P < 0.001, respectively). Multisite LA high UV (HUV, ≥4 areas of >2.7 mV) or multisite LA wall thickening (≥5 areas of >1.6 mm), defined as LA hypertrophy (LAH), was related to higher ATA recurrence. Among 92 LAH patients, 66 had HUV (LAH-HUV) and the remaining 26 had low UV (LAH-LUV), characterized by history of non-paroxysmal AF and heart failure, reduced LV ejection fraction, or enlarged LA. In addition, LAH-LUV showed the worst ablation outcome, followed by LAH-HUV and No LAH (log-rank P < 0.001). Combining UV and LAWT enables us to stratify recurrence risk and suggest a tailored ablation strategy according to LA tissue properties.

Photoplethysmography-documented atrial fibrillation in the first week after catheter ablation is associated with lower success rates.

To test the feasibility of postprocedural photoplethysmography (PPG) rhythm telemonitoring during the first week after atrial fibrillation (AF) ablation and its predictive value for later AF recurrence. PPG rhythm telemonitoring during the first week after the ablation procedure was offered to a total of 382 consecutive patients undergoing AF ablation. Patients were instructed to perform 1 min PPG recordings by a mobile health application 3 times per day and in case of symptoms. Clinicians assessed the PPG tracings via a secured cloud and the information was remotely integrated into the therapeutic pathway via teleconsultation (TeleCheck-AF approach). 119 patients (31%) agreed to perform PPG rhythm telemonitoring after ablation. Patients included in the TeleCheck-AF approach were younger compared to those who declined participation (58 ± 10 vs. 62 ± 10 years, p < 0.001). Median follow up duration was 544 (53-883) days. 27% of patients had PPG tracings suggestive of AF in the week following the ablation. In 24% of patients, the integration of PPG rhythm telemonitoring resulted in a remote clinical intervention during teleconsultation. During follow-up of one year, 33% of patients had ECG-documented AF recurrences. PPG recordings suggestive of AF in the week after ablation were predictive of late recurrences (p < 0.001). PPG rhythm telemonitoring during the first week after AF ablation often triggered clinical interventions. Due to its high availability, PPG-based follow-up actively involving patients after AF ablation may close a diagnostic and prognostic gap in the blanking period and increase active patient-involvement.

Predictive value of clinical risk scores for determination of left atrial low voltage area and arrhythmia recurrences in patients undergoing atrial fibrillation ablation

Background Presence of left atrial low voltage area (LVA) is an independent predictor of atrial tachyarhythmia (ATa) recurrences after atrial fibrillation (AF) ablation. Thus identifying patients with LVA by using non-invasive methods is an important endeavour. Purpose In this study; we investigated the predictors of LVA and postablation recurrences especially focusing on clinical risk scores. Methods We enrolled 328 consecutive patients who underwent initial AF ablation procedure using high density mapping. LVAs were assessed and segmental distribution was noted in each patient. CHA2DS2-VASc, HATCH, APPLE and SPEED scores were calculated. Predictive value and arrhythmia recurrences were evaluated. Results Two hundred and twenty one patients (67.37%) were paroxysmal AF and 107 (32.62%) patients were non-paroxysmal AF at the time of ablation. Mean CHA2DS2-VASc score was 1.88±1.66, mean APPLE score was 1.20±1.20, HATCH score was 0.93±1.00 and SPEED score was 1.54±1.27 in the whole study group. LVA was detected in 131 patients (39.93%). Female gender [OR:2.94, CI: 1.34-6.43, p=0.007), non-paroxsymal AF (OR: 2.49 CI:1.13-5.46, p=0.023), APPLE score ≥1 (OR:1.69 CI:1.12-2.55, p=0.012) and SPEED score ≥ 1 (OR: 1.47 CI:1.00-2.14, p=0.045] were independent predictors for the presence of LVA in multivariate analysis. Presence of LVA (OR:1.98; CI:1.13-4.15; p=0.017) and HT (OR:2.00; CI:1.19-3.38; p=0.009) were the independent predictors of recurrences. Conclusion Higher clinical scores are associated with more LVA which is associated with ATa recurrence. However the predictive value of these risk scores is limited. Better risk scoring systems are needed to precisely identify the underlying atrial substrate in this patient group.

Local conduction velocities determined by non-invasive electrocardiographic imaging predict arrhythmia-free survival after pulmonary vein isolation

As catheter ablation is primarily targeting pulmonary vein (PV) ectopic activity, it is not surprising that extra-PV arrythmogenic substrate is a key determinant of arrhythmia recurrence. Against this background, several studies have proposed assessment of extra-PV substrate in terms of atrial fibrosis or locally reduced conduction velocities (CV) to guide treatment. However, to date no non-invasive method directly assessing electrical arrhythmogenic substrate has been established in clinical practice, and treatment decisions are commonly based on crude surrogates like AF type or left atrial size. Here we establish and validate a novel non-invasive method based on electrocardiographic imaging (ECGi) to determine atrial arrhythmogenic substrate in terms of reduced local CVs and its predictive value regarding arrhythmia recurrence after PVI. Methods and results 52 consecutive patients scheduled for AF ablation (PVI-only) and 19 healthy controls were prospectively included and received ECGi to assess left and right atrial arrhythmogenic substrate. This ECGi-based method uses 64 electrodes placed on the torso. Subsequently, a 3D model of the torso is acquired as an anatomical reference using a 3D reconstruction camera. A personalised 3D atrial geometry is then derived from a database of human atria using an artificial intelligence-based algorithm. Finally, unipolar surface electrograms are projected onto the cardiac geometry and local CVs are estimated. Mean ECGi-determined atrial CVs were significantly lower in AF patients than in healthy controls, both in a global analysis (1.45±0.15 m/s vs. 1.64±0.15 m/s; p&amp;lt;0.0001) and a regional analysis of 19 predefined left and right atrial segments (Fig. 1). Considering only the segments with the lowest average CV in each patient, differences in CVs were more pronounced (0.80±0.22 vs. 1.08±0.26 m/s; p&amp;lt;0.0001). Multivariate logistic regression analyses combined with c-statistics including other previously proposed predictors found this mean CV of the "slowest" segment to be the strongest predictor of arrhythmia recurrence. A ROC analysis revealed that a cut-off for this variable of 0.72 m/s best discriminates PVI responders from non-responders: patients with a mean CV &amp;gt;0.72 m/s in all atrial regions showed a 6-months arrhythmia-free survival of 90.9%, whereas patients with one or more atrial regions with a mean CV &amp;lt;0.72 m/s had a poor outcome with an arrhythmia-free survival rate of only 57.9% (Fig. 2). Conclusion This was the first study to investigate local atrial CV non-invasively and to validate their predictive value regarding outcome after PVI. The absence of ECGi-determined areas of slow conduction well discriminated PVI responders from non-responders. Such non-invasive assessment of electrical arrhythmogenic substrate may guide treatment strategies and be an important step towards personalised AF therapy.

PO-04-155 LONG TERM POST-ABLATION ATRIAL FIBRILLATION MONITORING USING AUTOMATED SINGLE-LEAD ELECTROCARDIOGRAM ANALYSIS

Wearable and handheld consumer electrocardiogram devices may allow for noninvasive, frequent, and symptom driven monitoring after atrial fibrillation (AF) ablation. We sought to evaluate the performance of the KardiaMobile device and AliveCor algorithm in monitoring for arrythmia recurrence after AF ablation. Patients in a prospective randomized controlled trial of adjunctive therapy in AF ablation (NCT04625946) were provided with a KardiaMobile and asked to obtain symptomatic or at least weekly recordings before ablation and during 1 year of follow up. A total of 3174 recordings from 50 patients were obtained. The performance of the automated diagnosis was compared to the human interpretation which was considered the gold standard. A total of 772 tracings (24%) demonstrated AF occurring in 38/50 patients. The automated analysis had an overall sensitivity of 90.2% and specificity of 94.3% for the diagnosis of AF. The precision of the automated analysis had significant inter-patient variation (interquartile range 66.6-100%). In 3/12 patients without tracings of AF the automated analysis assigned a diagnosis of AF to one or more tracings. Inaccurate identifications of AF were most frequently adjudicated as sinus rhythm (47.7%), sinus rhythm with premature atrial contractions (23.5%), atrial flutter (8.8%), or uninterpretable (6.0%). A total of 58 tracings demonstrated atrial tachycardia or atrial flutter and were classified as ‘tachycardia’ (26%), AF (21%), or ‘unclassified’ (53%) by the automated analysis. The use of intermittent home electrocardiogram monitoring may facilitate the diagnosis of recurrent atrial arrythmias after AF ablation. Manual adjudication of the automated diagnosis or additional clinical event monitoring is likely required especially in patients with ectopy or postoperative atrial flutter.

PO-02-170 EFFECT OF LOW DOSE COLCHICINE USE ON RECURRENCE OF ATRIAL FIBRILLATION AFTER CATHETER ABLATION

Colchicine is commonly used early after atrial fibrillation (AF) ablation to reduce AF recurrence, but there is limited efficacy data. To evaluate the effect of colchicine use on AF recurrence after ablation. From 2013 to 2021, all AF ablations performed at a single tertiary care medical center were analyzed for colchicine use, clinical and procedural characteristics, and AF recurrence. The colchicine dose was 0.3-0.6mg once daily. The primary outcome was AF recurrence, defined as AF detection for more than 30 seconds after a 3 month blanking period. Propensity score matching (PSM, 1:1 match) was performed using the covariates of gender, age, BMI, AF classification, hypertension (HTN), and diabetes mellitus (DM) based on significant predictors of AF recurrence in prior studies. The minimum duration of follow-up was 6 months. Kaplan-Meier analysis was conducted to assess time to AF recurrence in the entire cohort and the PSM cohort. The study population consisted of 1544 AF ablations in 1231 patients (67% male, age 65 ± 7 years and mean follow up 34 ± 14 months); 78% of the patients received colchicine. Baseline characteristics in colchicine vs no colchicine group showed males (65 vs 69%), mean age (67 vs 63 years), BMI (30 vs 31), paroxysmal AF (70 vs 82%), DM (16 vs 17%), HTN (60 vs 54%), and obstructive sleep apnea (OSA) (18 vs 19%). Colchicine use was associated with decreased AF recurrence (HR 0.78, CI 0.63-0.96, p=0.022; Figure 1A). After PSM, there were 275 patients in each group. Baseline characteristics were not significantly different between both groups. AF recurrence was lower with colchicine (HR 0.71, CI 0.53-0.96, p=0.026; Figure 1B). Colchicine use was associated with decreased AF recurrence in patients with paroxysmal AF (HR 0.70, CI 0.50-0.98, p=0.036) but not persistent AF (HR 0.79, CI 0.39-1.59, p=0.5). Colchicine use was associated with decreased AF recurrence in patients undergoing repeat AF ablation (HR 0.55, CI 0.32-0.96, p=0.036) but not de novo ablation (HR 0.77, CI 0.54-1.1, p=0.2). Colchicine use was associated with lower AF recurrence in patients undergoing AF ablation.

PO-02-068 REAL WORLD COLCHICINE USE FOR PREVENTION OF PERICARDITIS AFTER ABLATION FOR ATRIAL FIBRILLATION

Colchicine is a commonly used anti-inflammatory medication for the treatment of acute pericarditis. The incidence of pericarditis after atrial fibrillation (AF) ablation has been estimated at up to 10%. Over the last decade, colchicine has been increasingly used for prevention of pericarditis after AF ablation. However, there are no established guidelines for preventative colchicine, no accepted dosage protocol, and no data regarding compliance with colchicine use. To estimate the usage of and compliance with colchicine for prevention of post-AF ablation pericarditis in real-world electrophysiology practices. The REAL-AF registry is the largest physician-driven registry of AF ablation in the United States, with a combined annual AF ablation volume of around 10,000 procedures by 80 operators. We circulated a Colchicine Use survey among all REAL-AF registry participants to estimate preventive colchicine usage as well as the dosage protocol and patient compliance. Sixty one (75%) registry participants responded to the survey. The majority (91.8%) used high power short duration (HPSD) ablation with 40-50 Watts in the left atrium (LA). Only 12 (21%) respondents had a pre-specified protocol for pericarditis prevention for all patients while another 12 (21%) operators used a selective protocol dependent upon extent of LA ablation beyond pulmonary veins. Colchicine was part of the preventive protocol for only 14 (24.1%) operators while another 10 (16.7%) used it selectively. Colchicine dosage varied between 0.3 mg BID (30.8%), 0.6 mg daily (38.4%) and 0.6 mg BID (38.4%), with duration of prescription varying widely from 1-4 weeks (Figure 1). As many as 44% of operators estimated that at least a third of their patients were unable to comply with complete colchicine protocol (Figure 1). Despite a general aura of regular use for prevention of post-AF ablation pericarditis, colchicine is actually used for this purpose by less than a third of high-volume operators in a real-world registry, with extreme variation in colchicine dosage protocol and a relatively high prevalence of patient non-compliance. The utility of preventive colchicine needs to be studied in a systematic manner to establish a universal usage recommendation.

PO-02-067 PREDICTORS AND IMPACT OF PULMONARY VEIN NON-RECONNECTION IN THE SECOND PROCEDURE AFTER ABLATION INDEX-GUIDED PULMONARY VEIN ISOLATION FOR ATRIAL FIBRILLATION

Reconnection of the pulmonary vein (PV) is the primary mechanism of atrial tachyarrhythmia (AT) recurrence after atrial fibrillation (AF) ablation. In patients with AT recurrence after the first ablation index (AI)-guided AF ablation, we examined the predictors of PV non-reconnection and its impact on the outcome after the second procedure. We aimed to elucidate the predictor of PV non-reconnection and its impact on the outcome after the second ablation procedure. Among the consecutive 2089 patients undergoing the first AI-guided AF ablation, we studied 289 with AT recurrence undergoing the second procedure. We compared clinical characteristics and outcomes between the patient groups with and without PV reconnection at the beginning of the second procedure. Of the 289 patients, 134 (46%) had non-reconnection, while the other 155 (54%) had reconnection at least in one PV. Patients' characteristics, including AF type (paroxysmal AF, 41% versus 48%; persistent AF, 28% versus 26%; long-standing persistent AF, 31% versus 26%, respectively, P=0.50), were similar between the two groups. The percentage of first-pass PV isolation (FPI) in both PV sides in the first ablation procedure was significantly higher in PV non-reconnection than in those with reconnection group (77% versus 43%, respectively, p<0.001). Multivariate logistic regression analysis revealed that FPI at the first procedure (odds ratio, 0.23 (95% confidence interval (CI), 0.13-0.38), p<0.001) was the only independent predictor of PV reconnection. After the second procedure, Kaplan-Meier analysis revealed that AT recurrence rate was significantly higher in patients with PV non-reconnection than in those with reconnection (Log-rank test, p=0.005). Multivariate Cox proportional hazard model analysis revealed that PV non-reconnection (hazard ratio (HR), 1.71 (95%CI, 1.14-2.55), p=0.008), left atrial dimension (1.04 (1.01-1.07), p=0.006), and left ventricular ejection fraction (0.98 (0.96-0.99), p=0.005) were independent predictors of AT recurrence. However, AF type was not: HR to paroxysmal AF (reference) was 1.10 (95%CI, 0.67-1.83) (p=0.70) in persistent AF and 1.30 (0.82-2.03) (p=0.26) in long-standing persistent AF. FPI in the first procedure was the only independent predictor of PV non-reconnection. After the second procedure, PV non-reconnection was an independent worsening predictor of AT recurrence, suggesting a need for an aggressive search for non-PV mechanisms.

Causes of Early Mortality After Catheter Ablation of Atrial Fibrillation.

Recognition of the causes of early mortality after atrial fibrillation (AF) catheter ablation is essential for the improvement of patient safety. This study sought to determine the causes of early mortality (≤90 days) after AF ablation. We performed a retrospective analysis of AF ablation from January 1, 2013, to December 1, 2021 at the Mayo Clinic (Rochester, Phoenix, and Jacksonville). Causes of death were identified through a comprehensive chart review of the electronic health record from within the Mayo Clinic system and outside records when available. A total of 6723 patients were included in the study. The 90-day all-cause mortality rate was 0.22% (n=15). Among all 90-day deaths, majority of the deaths (73.3%) did not have a direct relationship with the procedure. Sudden death was the most common cause of early death (20%), followed by peri-procedural stroke (13%), respiratory failure (13%), atrioesophageal fistula (13%), infection (7%), heart failure (7%), and traumatic brain injury (7%). The 90-day mortality rate directly due to AF ablation procedural complications was 0.06% (n=4). AF ablation procedure has a 90-day mortality of 0.22%, and the most common cause of early mortality was sudden death. The majority (73.3%) of early mortality was not directly associated with a procedural complication, and the mortality rate due to complications associated with the AF ablation procedure was low at 0.06%. Further studies are required to investigate causes and risk factors associated with sudden death in this patient population.

Vein of Marshall ethanol infusion: First-step or adjunctive choice for perimitral atrial tachycardia?

Perimitral atrial tachycardia (PMAT) is the most frequent type of iatrogenic atrial tachycardia (AT) after atrial fibrillation (AF) ablation. Vein of Marshall ethanol infusion (EIVOM) is a promising technique in mitral isthmus (MI) ablation. A total of 165 patients with PMAT were divided into three groups according to ablation strategies, including RF only group (n=89), RF-EIVOM group (initial RF ablation with adjunctive EIVOM, n=28), and EIVOM-RF group (first-step EIVOM with touch-up RF ablation, n=48). Acute and follow-up procedure outcomes were evaluated. PMAT terminated in 89.9%, 89.3%, and 93.7% of patients in RF only, RF-EIVOM and EIVOM-RF groups, respectively (p=.715), with complete MI block achieved in 80.9%, 89.3%, and 95.8% of patients (EIVOM-RF vs. RF only, p=.012). First-step utilization of EIVOM was associated with a significant shortening of RF ablation time at MI (EIVOM-RF 2.1±1.3min, RF only 7.9±5.9min, RF-EIVOM 6.8±5.8min; p<.001) and a decrease in the proportion of patients need ablation within coronary sinus (CS, EIVOM-RF 14.6%, RF only 61.8%, RF-EIVOM 64.3%; p<.001). After a mean follow-up of 12.1±6.2 months, AF/AT recurred in 39 (43.8%), 6 (21.4%), and 12 (25.0%) patients in RF only, RF-EIVOM, and EIVOM-RF group (RF-EIVOM vs. RF only, p=.026; EIVOM-RF vs. RF only, p=.022). EIVOM was associated with an enhanced acute MI block rate as well as reduced AF/AT recurrence. First-step utilization of EIVOM promises to significantly simplify the RF ablation process. PMAT is the most common type of iatrogenic AT after AF ablation procedures. EIVOM contributed to a higher acute MI block rate and lower arrhythmia recurrence risk during follow-up. First-step utilization of EIVOM significantly reduced the need for radiofrequency ablation at MI and inside CS with the advantage of creating a homogenous, transmural lesion and eliminating epicardial connections.

Sudden death was the most common cause of early death (20%), followed by peri-procedural stroke (13%), respiratory failure (13%), atrioesophageal fistula (13%), infection (7%), heart failure (7%), and traumatic brain injury (7%). The 90-day mortality rate directly due to AF ablation procedural complications was 0.06%. Most of the early deaths (93.3%) occurred after discharge from index hospitalization, among which 64.3% were readmitted before their death, 14% died at home, and 7% were discharged on comfort care and died in hospice. The main reason for readmission leading to death was respiratory failure (33%). Conclusion: Atrial fibrillation ablation procedure has a 90-day mortality of 0.22% and the most common cause of early mortality was sudden death. The majority (73.3%) of early mortality was not directly associated with a procedural complication, and the mortality rate due to complications associated with the AF ablation procedure was 0.06%.

Evaluating the value of the timing of recurrence during blanking period after atrial fibrillation ablation

Abstract Introduction In the first weeks after atrial fibrillation (AF) ablation, the arrythmia may recur theoretically due to transient local inflammation and not due to treatment failure. This is defined as the blanking period, with a proposed duration of 3 months. Recently, this time period has been brought into question. The aim of this work was to evaluate the correlation between the timing of blanking recurrence and late AF recurrence. Methods This was a single-centre retrospective study including patients without structural heart disease that underwent first AF ablation and were subsequently enrolled in the post ablation structured program between 2018 and 2021. Patients were excluded if they had &amp;lt;6 months follow-up. Appointment with ECG and Holter monitoring was performed at 1, 3, 6 and 12 months after ablation. Results We included a total of 193 patients (56% male, mean age 63±12 years). Of these, 79% had paroxysmal AF and mean left atrial volume index was 58±18 mL/m2. During the 3-month blanking period, there were 39 (21%) recurrences, 18 (9%) of which in the first month. After blanking period, at 6 months, 25 (13%) patients had AF recurrence, 56% of which had already recurred during blanking period. AF recurrence in the 2nd and 3rd month of blanking increased the odd of recurrence at 6-month by more than 5-fold (odds ratio (OR) 8,944; CI 95% 2,817–28,400, p&amp;lt;0.001 and OR 5,591; 95% CI 1,173–26,651; p=0.031). On the other hand, recurrence of AF during the 1st month of blanking was not associated with increased chance of 6-month AF recurrence (OR 2,095, 95% CI 0,630–6,964, p=0.227) (figure 1). There were no significant differences in clinical variables, including LA volume, between patients with 1-month recurrence and patients without recurrences. However, patients with AF recurrence in the 2nd and 3rd month of blanking had significantly increased LA volume. Conclusion Our study suggests that patients with AF recurrence in the 2nd and 3rd month of blanking have structurally different atria and are at a significantly higher risk of post blanking AF recurrence, in contrast with patients with AF recurrence in the 1st month of blanking, thus questioning the appropriate duration of the blanking period. Effects of early recurrence and extra-PV triggers on long-term recurrence after catheter ablation for atrial fibrillation

Abstract Background Although early recurrence (ER) within 3 months after atrial fibrillation (AF) catheter ablation (AFCA) was recently reported to be a reliable predictor of late recurrence (LR), the mechanism is not clear. Purpose We explored the characteristics of patients with ER and compared the long-term late recurrence (LR) pattern depending on the existence of extra-pulmonary vein trigger (ExPV-trigger). Methods Among 3643 patients who underwent de novo AFCA, we included 1249 patients (59.2±11.0 years old, 31.3% persistent AF) who underwent isoproterenol provocation and regular follow-up over three years after AFCA. We evaluated the risk factors for ER and compared the patients with ER alone (10.1%), LR alone (16.6%), and ER+LR (15.9%), and the outcome of repeat procedure. Results Overall ER (ER alone and ER+LR) was independently associated with persistent AF (OR 1.58 [1.16–2.14], p=0.003), extra-PV triggers (OR 2.80 [1.90–4.13], p&amp;lt;0.001), and empirical extra-PV ablation (OR 1.54 [1.15–2.07], p=0.004). Overall LR (LR alone and ER+LR) risk was significantly higher in the ER with ExPV-trigger group than in ER without ExPV-trigger or no ER groups (Log-rank p&amp;lt;0.001). The rhythm outcome of the second procedure did not differ between ER+LR and LR alone groups (Log-rank p=0.160), but was worse in the ER+LR ExPV-trigger than in ER+LR without ExPV-trigger or LR alone groups (Log-rank p=0.005). Conclusion ER was independently associated with LR after de novo AFCA. ExPV-trigger played crucial roles in ER and LR after de novo AFCA and worse rhythm outcome after redo AFCA. Alcohol Consumption Is Associated With Postablation Recurrence but Not Changes in Atrial Substrate in Patients With Atrial Fibrillation: Insight from a High-Density Mapping Study.

BackgroundThe association between alcohol consumption, atrial substrate, and outcomes after atrial fibrillation (AF) ablation remains controversial. This study evaluated the impacts of drinking on left atrial substrate and AF recurrence after ablation.Methods and ResultsWe prospectively enrolled 110 patients with AF without structural heart disease (64±12 years) from 2 institutions. High‐density left atrial electroanatomic mapping was performed using a high‐density grid multipolar catheter. We investigated the impact of alcohol consumption on left atrial voltage, left atrial conduction velocity, and AF ablation outcome. Patients were classified as abstainers (<1 drink/wk), mild drinkers (1–7 drinks/wk), or moderate‐heavy drinkers (>7 drinks/wk). High‐density mapping (mean 2287±600 points/patient) was performed on 49 abstainers, 27 mild drinkers, and 34 moderate‐heavy drinkers. Low‐voltage zone and slow‐conduction zone were identified in 39 (35%) and 54 (49%) patients, respectively. There was no significant difference in the proportions of low‐voltage zone and slow‐conduction zone among the 3 groups. The success rate after a single ablation was significantly lower in drinkers than in abstainers (79.3% versus 95.9% at 12 months; mean follow‐up, 18±8 months; P=0.013). The success rate after a single or multiple ablations was not significantly different among abstainers and drinkers. In multivariate analysis, alcohol consumption (P=0.02) and the presence of a low‐voltage zone (P=0.032) and slow‐conduction zone (P=0.02) were associated with AF recurrence after a single ablation, while low‐voltage zone (P=0.023) and slow‐conduction zone (P=0.024) were associated with AF recurrence after a single or multiple ablations.ConclusionsAlcohol consumption was associated with AF recurrence after a single ablation but not changes in atrial substrate.

Outcomes of atrial fibrillation ablation in patients with or without silent pulmonary veins from prior ablation procedure

Pulmonary venous (PV) electrical recovery underlies most arrhythmia recurrences after atrial fibrillation (AF) ablation. Little is known about procedural profiles and outcomes of patients with electrically silent PVs upon redo ablation for AF. In a prospectively maintained registry, we enrolled 838 consecutive patients (2013-2016) undergoing redo ablation procedures. Ablation procedures targeted the PVs, the PV antra, and non-PV sites at operators' discretion. Procedural profiles and clinical outcomes were assessed. The primary outcome was freedom from AF after a 3-monthblanking period. The secondary outcome was improvement in quality of life. Most patients undergoing redo AF ablation (n = 684, 82%) had PV reconnection while the remaining 154 (18%) had electrically silent PVs. Patients with recurrent AF and electrically silent PVs were older (66 vs. 64 years, p = .02), had more prior ablation procedures (median 2 IQR 1-3 vs 1 IQR 1-2 p = .001), were more likely to have non-paroxysmal AF (62% vs. 49%, p = .004) and atrial flutter (48%vs. 29%, p = .001) and had significantly larger left atrial volumes (89 vs. 81 ml, p = .003). Patients with silent PVs underwent a more extensive non-PV ablation strategies with antral extension of prior ablation sets in addition to ablation of the roof, appendage, inferior to the right PVs, peri-mitral flutter lines, cavotricuspid isthmus lines and ablation in the coronary sinus. Upon one year of follow-up, patients with electrically silent PVs were less likely to remain free from recurrent atrial arrhythmias (64% vs. 76%, p = .008). Regardless of PV reconnection status, redo ablation resulted in improvement in quality of life. Rhythm control with extensive ablation allowed maintenance of sinus rhythm in about two thirds of patients with silent PVs during redo AF ablation procedures. Regardless of PV reconnection status, redo ablation resulted in improvement in quality of life. This remains a challenging group of patients, highlighting the need to better understand non-PV mediated AF.
