Evaluation of orthodontic treatment need in school children of shimla using index of orthodontic treatment need

: The objective of this survey was to assess the orthodontic treatment need in school children of Shimla aged 12-16 years using Index of orthodontic treatment need.The study included 576 children studying in Shimla, Himachal Pradesh, India who were assessed for orthodontic treatment need using IOTN and results were expressed in percentages. The difference in orthodontic treatment need for males and female and the orthodontist and subjects perception of orthodontic treatment need was calculated by χ² test. Pearson’s correlation assessed the correlation between the Dental Health Component and Aesthetic Component of IOTN. 10.7% (62) of the children had definite need for orthodontic treatment (AC) while 31.9% (184) had definite need for orthodontic treatment (DHC) with statistically insignificant difference between male and female. The difference between orthodontist perception of treatment need and that of subject was statistically significant. A strong and positive correlation between DHC and AC of IOTN was found which was statistically significant.31.9% (184) and 10.8% (62) of the schoolchildren had definite need for orthodontic treatment according to the dental health component and aesthetic component of index of orthodontic treatment need respectively. 49.7% (286) of the subjects had knowledge about orthodontic treatment. 45.8% (264) of the subjects expressed desire to undergo orthodontic treatment. Orthodontist was more critical in aesthetic evaluation as compared to the schoolchildren.

Prevalence and factors influencing bullying among Egyptian schoolchildren.

To evaluate the prevalence of bullying among schoolchildren and to study the correlation between bullying and a specific type of malocclusion, self-esteem, oral health-related quality of life (OHRQoL), the need for orthodontic treatment and socioeconomic status. A cross-sectional study. Low, middle and high socioeconomic-class schools in the same educational district in Cairo, Egypt. A total of 324 Egyptian schoolchildren aged 11-14 years. Validated questionnaires were used to assess self-reported bullying frequency and intensity, self-esteem and OHRQoL. The Index of Orthodontic Treatment Need (IOTN) was used to assess the orthodontic treatment need and the participants' occlusion was classified according to the British Standard Incisors (BSI) classification. Bullying was found to be prevalent in 61.7% of the sample. Regression analysis revealed that participants with a Class II division 1 incisor relationship had the highest odds ratio (OR) for predicting bullying (OR = 1.86, 95% confidence interval [CI] = 1.38-2.15). Being bullied was significantly associated with lower self-esteem (OR = 0.47, 95% CI = 0.13-0.78), worse OHRQoL (OR = 2.73, 95% CI = 1.95-3.15) and a high need for orthodontic treatment, as determined by the IOTN Dental Health Component score (OR = 1.33, 95% CI = 1.05-1.95) and IOTN Aesthetic Component score (OR = 1.52, 95% CI = 1.12-1.88). Moreover, participants in high and middle socioeconomic-class groups had a lower OR for predicting bullying than those in the low socioeconomic-class group (OR = 0.27, 95% CI = 0.13-0.54). The bullying prevalence among Egyptian schoolchildren aged 11-14 years was very high. The prevalence of bullying was higher in schoolchildren with higher orthodontic treatment need and Class II division 1 incisor relationship. Bullying was correlated to poor self-esteem as well as poor OHRQoL. In addition, low socioeconomic status was found to be a potential predictor for bullying.

Aesthetic Treatment Needs for Buccally Displaced Canines in Orthodontics

Background and Objectives: Aesthetic and related psychosocial impairment are the major ill effects of malocclusion. Many studieshave concluded that aesthetics, especially alignment of the visible anterior teeth, is often a main motivating factor for see kingorthodontic treatment. The objective of the study was to assess the aesthetic treatment need of orthodontic patients with buccallydisplaced maxillary canines using the “Aesthetic Component of Index of Orthodontic Treatment Need (AC-IOTN)”.Methodology: We recruited 158 subjects at Khyber College of Dentistry (KCD), with a chronological age range of 12 to 30 years.Age, gender, aesthetic grade and category of treatment need were noted for all the patients. Version 20.0 of the Statistical Packagefor Social Sciences was used to enter and analyze the data.Results: Our sample consisted of 71 (44.9%) males and 87 (55.1%) females. The mean age of the overall sample was 18.51± 4.2years. There were 82 (51.9%) patients in the “no or little need” category, 25 (15.8%) patients in the “mod erate” treatment needcategory, and 51 (32.8%) patients in the “definite” treatment need category, where most of the patients had a grade 8 need on IOTNAC.Conclusion: On aesthetic need, majority of patients in definite treatment need category fell in the grade 8. Therefore it can beconcluded that buccally displaced canine is one of the primary motivating factors for people to seek orthodontic treatment.

Prevalence of orthodontic treatment needs in permanent dentition in the population of Gulf Cooperation Council countries: A systematic review and meta-analysis of observational studies.

The aim of this systematic review and meta-analysis was to comprehensively analyze the existing information on the prevalence of the need for orthodontic treatment in the permanent dentition stage among populations in Gulf Cooperation Council (GCC) countries. For observational studies in GCC countries, the key terms were electronically searched in Science Direct, PubMed, Embase, Cochrane Reviews, Google Scholar, and Sage databases (1990-2021). The bias risk for the selected studies was evaluated using the modified Strengthening the Reporting of Observational Studies in Epidemiology statement. Thirteen studies reported on the prevalence of orthodontic treatment needs among 33,134 children in GCC countries in permanent dentition with an age range of 11-19 years satisfied the inclusion criteria. Out of the 13 studies, 9 reported on the prevalence of malocclusion, 11 reported on the prevalence of occlusal traits, and 12 reported on the prevalence of orthodontic treatment needs as per the Dental Health Component (DHC) of Index of Orthodontic Treatment Need (IOTN), 4 reported as per both DHC and Aesthetic Component (AC) of IOTN, and 1 reported as per only AC of IOTN. Meta-analysis of the included studies indicated that the pooled malocclusion prevalence rate was 10.60% (confidence interval [CI] 95%: 0.093-0.076) with 8.58% Class I (CI 95%: 0.074-0.188), 2.09% Class II (CI 95%: 0.014-0.058), and 0.93% Class III (CI 95%: 0.005-0.018) malocclusions. The most prevalent type of occlusal trait was spacing (13.10%, CI 95%: 0.018-0.169), followed by crowding (4.96%, CI 95%: 0.017-0.091). The pooled prevalence of borderline and definite needs for orthodontic treatment based on DHC and AC of IOTN was 0.82% (CI 95%: 0.014-0.035), 1.13% (CI 95%: 0.011-0.091), 4.08% (CI 95%: 0.009-0.114), and 2.06% (CI 95%: 0.002-0.048), respectively. The findings were heterogeneous (P < 0.05). These findings indicated that the prevalence of malocclusion and orthodontic treatment needs was not high in the GCC population.

Comparison of Self-Perception of Esthetics and Orthodontic Treatment Needs among Dental and Non-Dental Students with Assessment by Orthodontist

Objectives: The present study was conducted to compare the self-perception of esthetics and wish for seeking orthodontic treatment among dental and non-dental students. The realistic orthodontic treatment needs were confirmed by expert orthodontist. Methods: A cross-sectional study was conducted among 255 male students (Dental—75, Medical—93 and Pharmacy—87 students). The questionnaire included 20 items covering dental esthetics self-perception (10 questions), self-confidence and psychological impact (5 questions), and the need for seeking orthodontic treatment (5 questions) on a five-point Likert scale. The realistic orthodontic treatment needs were confirmed by photographs using aesthetic component of IOTN and clinical examination for the participants and analysis of jaw models using Dental aesthetic index (DAI). Kruskal-Wallis ANOVA was used to check the significant difference between groups. Results: The overall average esthetic self-perception score for total sample was 3.83. The overall average psychological impact score for the whole sample was 2.67. The total percentage of participants who either agree or strongly agree with the question related to their perception of need for seeking orthodontic treatment was 70%. According to the assessment of orthodontic needs using AC-IOTN, 19.8% required definitive treatment. According to the assessment of orthodontic treatment needs based on DAI, 20.3% and 9.9% needed highly desirable (DAI 31 - 35) and definite need of treatment (DAI ≥ 36) respectively. Conclusion: It can be concluded that there was no difference between the esthetic perception of dental and non-dental students. The self-perception for seeking orthodontic treatment was over-reported by participants compared to real requirement as assessed by orthodontic specialist. Dental esthetics has no psychological impact on academic performance and self-confidence as perceived by study participants.

The Correlation between Orthodontic Treatment Need and Psychosocial Impact of Dental Aesthetics among Patients Seeking Orthodontic Treatment

Background: The influence of orthodontic treatment on self-confidence and quality of life is a major reason for patients to seek treatment. This study was designed to assess the relationship between orthodontic treatment need and psychosocial impact of dental aesthetics in patients seeking orthodontic treatment. Methods: This descriptive analytical cross-sectional study was performed on 165 patients referred to orthodontic clinics in Kerman to start treatment. Patients were selected by census method to reach the specified sample size. The data collection tools included 3 forms: demographic information form, Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ) and a form for recording information obtained from clinical examination to determine orthodontic treatment need based on Index of Orthodontic Treatment Need (IOTN). Data analysis was performed through SPSS 22 and using t test, ANOVA, and linear regression at a significance level of 0.05. Results: The educational level of subjects and their parents and economic status showed no significant relationship with total PIDAQ score and any of its domains (P&gt;0.05). In psychological impact domain, women’s score was significantly higher (P=0.016) and there was a significant relationship between the treatment need based on aesthetic component (AC) and the domains of dental self-confidence (P=0.003), social impact (P=0.049) and psychological impact (P=0.066), as well as the total score of questionnaire (P=0.012). Treatment need based on dental health component had no statistically significant relationship with PIDAQ score (P&gt;0.05). Conclusion: AC of IOTN had significant relationship with dental self-confidence, social impact and psychological impact as well as the total score of PIDAQ. In orthodontic treatments, in addition to malocclusion, the psychological aspects should also be considered.

An investigation into the reliability of a mobile app designed to assess orthodontic treatment need and severity

Aim To investigate reliability of the Easy IOTN app between clinicians with different levels of experience in determining Index of Orthodontic Treatment Need (IOTN) Dental Health Component (DHC) and Aesthetic Component (AC) scores from study models. The accuracy of each clinician in discriminating treatment need using the app against the 'gold standard' conventional assessment at the threshold of treatment acceptance criteria was also explored.Materials and methods In total, 150 sets of pre-treatment study models were assessed by six clinicians using the app on two separate occasions (T1 and T2). A single IOTN-calibrated clinician also scored the models using the conventional technique. Clinician scores for both intra- and inter-rater reliability were assessed using Cohen's Kappa. The performance of each clinician in discriminating treatment need using the app against the conventional assessment method at the threshold of treatment acceptance criteria was also assessed using the area under the curve-receiver operating characteristic.Results The intra-rater agreement for the clinician undertaking the conventional assessment of the models was 1.0. Intra-rater agreement scores for clinicians using the Easy IOTN app ranged between 0.37-0.87 (DHC) and 0.22-0.44 (AC). Inter-rater agreement scores at T2 were 0.59 (DHC) and 0.23 (AC). Based on the IOTN DHC, all clinicians displayed an excellent level of accuracy in determining malocclusions qualifying for treatment (range 81.7-90.0%). Based on the IOTN AC, all clinicians showed an acceptable level of accuracy in determining malocclusions qualifying for treatment (range 71.9-79.2%).Conclusions The Easy IOTN app was shown to have moderate inter-rater reliability. Variation in the intra-rater reliability was evident between clinicians of different grades/level of experience. Importantly, the diagnostic accuracy of the app to discriminate between malocclusions that qualify for NHS treatment was rated as excellent (IOTN DHC) and acceptable (IOTN AC) and independent of clinician grade or level of experience.

Perception and attitude of Mongolians on malocclusion

Background/purposeThis study assessed the perception and attitude of Mongolians on malocclusion and compared the perceptions with researchers’ perceptions on malocclusion, and this study aimed to determine factors influencing the self-perception of laypeople.Materials and methodsIn addition, we evaluated the correlation between the subjective and objective perceptions of participants. In a random sample of 133 people (39.8% men and 60.2% women) aged 18–55 years, subjective perceptions were assessed using a questionnaire (oral aesthetic subjective impact scale), and objective perceptions were assessed using 10 intraoral frontal photographs of the aesthetic component of index of orthodontic treatment need (AC-IOTN) and six intraoral lateral photographs of the aesthetic component of lateral occlusion (AC-LO).ResultsA significant association was observed between participants' self-perception and researchers' ratings (P < 0.001). The appearance of teeth was significantly correlated with participants’ self-rating by using AC-IOTN and AC-LO. Most people (74%) believed that well-aligned teeth improve the overall facial appearance. Sex, monthly income, and education level significantly influenced the self-perception of participants on malocclusion.ConclusionPatients’ perception and attitude cannot be overlooked because they are some of the crucial aspects of the entire treatment.

The value of the Aesthetic Component of the Index of Orthodontic Treatment Need (IOTN) in the Assessment of Subjective Orthodontic Treatment Need

Background: Malocclusion is the most common dental problem. Its correction needs assessment before planning Orthodontic treatment. The index of orthodontic treatment need (IOTN) is one of the most valuable method of grading and evaluating malocclusion. A number of studies stated; using the IOTN have reported that there is limited use of aesthetic component in school going children. Objectives: To estimate whether dental concern expressed by the grade of the AC chosen by subjects is reliable and whether it may be predictive for potential co-operation. Materials and Methods: This study was carried out in school going children of 12 year of age in kpk Pakistan. This study was based on a questionnaire and clinical examination. The questionnaire consists of various items. On the basis of these item questionnaire was filled. Clinical examination was done at the schools each time by the same orthodontist. Statistical analysis was done. Results: Out of total children, 29 were female and 58 were male. Most of the children were satisfied with dental aesthetics. Most of them wanted to change colour of the teeth. Most of the enrolled students were classified in “no treatment need’ category” followed by “need borderline treatment” category. Satisfaction with aesthetic were also evaluated (P-value &lt;0.090). Conclusion: It was concluded that large number of people were in need of the orthodontic therapy as per the aesthetic component of IOTN. A comparison of self-esteem between patients undergoing fixed orthodontic treatment, to those not receiving orthodontic treatment

Objective: Comparison of the self-esteem between patients undergoing fixed orthodonticc treatment in the past 6 months or more, to those not receiving orthodontic treatment or the time passed since the commencement of their treatment has been less than six months. Materials and methods: The participants of this study were divided into two groups, group A and group B. The sample size of each group was 75 making the total 150 aged between 16 to 25 years. The malocclusion severity was assessed with the index of orthodontic treatment need aesthetic component (IOTN-AC) which was 6 or more for both groups. Group A involved patients currently receiving no orthodontic treatment or the time elapsed since the start of the treatment was less than 6 months. Group B on the other hand, comprised of patients undergoing orthodontic treatment in the past 6 months or more. Questionnaires were administered among both group A and group B where self-esteem was measured using the Rosenberg Self-esteem (RSE) Scale. Independent Sample T test was applied on both of these variables. Results: The results did not show any statistically significant association between self-esteem and “the time elapsed since the start of the treatment”. Likewise, the additional factors i.e., Age, Education and Gender too had no impact on the Rosenberg score. Conclusion: No association was found between selfesteem of the patients undergoing fixed Orthodontic treatment, to those not receiving it

What are the factors influencing the aesthetic requirement in the choice of orthodontic appliances in adults?

Although adult patients are said to be demanding in terms of discretion of the orthodontic appliance, the factors influencing the choice of the appliance are still poorly understood. The main objective of this study was to determine if there is a correlation, in adults, between the importance of the orthodontic treatment need and the aesthetic preference of the orthodontic appliance (metal braces, ceramic braces in buccal, lingual or aligner). The secondary objectives were to evaluate the impact of the patient's personal factors, his eventual orthodontic past and his perceived orthodontic need. Patients were interviewed by questionnaire before the first consultation. An index, depending of the discretion of the appliance, was used. The need for treatment was calculated via dental health component and aesthetic component of IOTN. Given the size of the sample, only trends can be emitted. The real need for treatment had no influence on the choice of the aesthetic appliance. The most demanding patients would be women, in their thirties, single, employed, executive or practicing a higher profession, with previous orthodontic treatment and having recently consulted an orthodontist, as an adult. These patients felt that their orthodontic treatment need was minimal or significant. Patients estimated the duration of treatment at 17 months. Lingual was considered the most aesthetic appliance far ahead of the aligners. A larger scale study is needed to determine the influence of each factor.

Clinical parameters associated with obesity outcomes in vulnerable adolescents

Objective: Overweight and obesity rates are growing at alarming levels worldwide. Adolescents of higher socioeconomic status are more likely to be overweight, however, information from economically vulnerable populations is lacking. Therefore, the present study aimed to determine the impact of normative and self-perceived clinical aspects on the outcomes of obesity in vulnerable adolescents. Methodology: Observational cross-sectional study with 239 adolescents with an average age of 13 years, enrolled in public schools in Sabará (Minas Gerais). Body Mass Index values were obtained and classified according to AnthroPlus software. Dental Health Component of the Index of Orthodontic Treatment Need (IOTN) evaluated the normative malocclusion and the Aesthetic Component of IOTN the subjective occlusal evaluation. DMFT index evaluated dental caries activity and experience. Crude Odds Ratio values were estimated and adjusted, with a 95% confidence interval and a 5% significance level. Results: Adolescents with self-perceived malocclusion were less likely to be obese (P &lt;0.05). Overall, 20.0% of the adolescents with self-perceived malocclusion were obese as compared to 40.6% of those with no self-perceived malocclusion. Obesity was not found to be associated with the variables of sex and dental caries (P &gt;0.05). Conclusion: Adolescents with self-perceived malocclusion were less likely to be obese, suggesting that obesity is associated with perceived malocclusion, but not with caries experience.

Comparison of orthodontic treatment need among professionals and parents in Dammam, Saudi Arabia.

This study aimed to assess parents' awareness and attitude regarding their children's malocclusion compared with professional assessments and to measure the prevalence of malocclusion among children in Dammam, Saudi Arabia. The study population consisted of 377 randomly selected children (aged 8-12 years) from five schools in Dammam, Saudi Arabia. A questionnaire was used to assess parents' awareness using the Aesthetic Component (AC) of the Index of Orthodontic Treatment Need (IOTN) and knowledge about preventive orthodontics. A clinical examination was performed by two calibrated examiners to measure the prevalence of malocclusion using both the AC and dental health component (DHC) of IOTN. The most common Angle's molar relationship was Class I (78.2%), followed by Class II (17.5%) and Class III (4.2%). The DHC (score 3-5), which indicated the need for orthodontic treatment, was 55.4%. There was a significant difference (P < 0.05) between AC from examiners and parents, whereby the parents tended to underestimate their children's malocclusion by a factor of 1.45. Surprisingly, many parents (67.4%) lacked knowledge about preventive orthodontics. Class I malocclusion was most prevalent among children in the area of Dammam. A moderate percentage (55.4%) of the study subjects required treatment based on the DHC. Based on the lack of knowledge about preventive orthodontics in the majority of parents in our study, it is recommended that oral health-promoting programs be implemented to improve the awareness of malocclusion in the region.

Orthodontic Treatment Needs of Adolescents in North-Western Nigeria Using the Index of Orthodontic Treatment Needs.

Introduction:Orthodontic treatment planning within a public health system needs information on the need for orthodontic treatment within the population. It is imperative to have epidemiological data from which the need for orthodontic care in any region can be estimated. The present study, therefore, aimed to determine the orthodontic treatment need of adolescents in North-Western Nigeria.Materials and Methods:A descriptive cross-sectional study was conducted of 200, 12–15-year-old schoolchildren in Northern Nigeria using the Dental Health Component (DHC) and Aesthetic Component (AC) of the Index of Orthodontic Treatment Need (IOTN). A structured interview and clinical examination were used to assess the subjects. Descriptive statistics and χ2 tests were used for data analysis with statistical significance set at P < 0.05.Results:There were 19.5% with definite need (DHC 4 or 5) for orthodontic treatment and 15.5% borderline need (DHC 3). The AC scores demonstrated that 1% of the studied subjects had a definite need (AC 8–10) for orthodontic treatment and 24% borderline need (AC 5–7). A statistically significant relationship between AC and DHC of IOTN among the study population (P < 0.05) was observed.Conclusion:Using the DHC of the IOTN, the proportion of subjects estimated to have great and very great treatment needs (grades 4 and 5) was 19.5%. However, only 1% of individuals were in need (grades 8–10) of orthodontic treatment according to the AC. Awareness about orthodontic treatment should be a part of public health planning for this region.

Malocclusion, facial and psychological predictors of quality of life in adolescents.

To identify clinical and psychological predictors of OHRQoL. Cross-sectional clinical and questionnaire study with 332 adolescents aged 11-14 years. The facial profile was classified through photographs and dental malocclusion was classified by the Dental Health Component of the Index of Orthodontic Treatment Need. The psychosocial variables were assessed by the Aesthetic Component of IOTN, the Orthodontic Aesthetic Subjective Impact Score and the Global Negative Self-Evaluation. OHRQoL was measured using the Oral Health Impact Profile (OHIP-14). The associations were analyzed by multiple logistic regression models. Higher aesthetic concern and low self-esteem were 3.43 and 3.34 times more likely to affect OHRQoL (p⟨0.05), respectively. The facial and dental aspects of malocclusion were unrelated to OHRQoL. Facial and dental aspects of malocclusion were unrelated to OHRQoL, whereas psychosocial variables such as self-perception of orthodontic treatment need and self-esteem predicted adolescents' oral health related quality of life.

Factors influencing orthodontic treatment outcome in South East Wales and implications for service commissioning.

To determine factors that may influence the outcome of orthodontic treatment undertaken in General Dental Services/Personal Dental Services in South East Wales. A retrospective study of a requested 20 consecutively treated cases (for the year 2014-2015) provided by 26 performers in South East Wales. Performer and patient information was obtained by use of a questionnaire and FP17OW forms, respectively. A calibrated investigator recorded the Index of Orthodontic Treatment Need (IOTN), Peer Assessment Rating (PAR) and the Index of Complexity, Outcome and Need (ICON) on start- and end-study models for each case. Descriptive and regression analyses were undertaken to identify any predictive factors of a good treatment outcome. Two respondents completed < 20 cases, so all of their cases were assessed. A total of 495 cases were assessed. The overall achieved mean end-PAR score of 5 is collectively a good occlusal outcome. Predictive factors of a good quality of occlusal outcome (end-PAR score being ⩽ 5) were: dual arch treatment; use of functional with fixed appliances; treatment undertaken in non-corporate practices; and treatment undertaken by registered specialists (P < 0.001). Predictive factors for occlusal improvement (change in PAR score) were: IOTN aesthetic component (AC); IOTN dental health component (DHC) (1-3 or 4-5); and number of arches treated and malocclusion type (P < 0.001). Dual arch fixed appliances undertaken by orthodontic specialists in non-corporate environments produced the highest quality orthodontic outcomes. Those who have the highest need for treatment according to IOTN DHC and AC benefit most in terms of improvement achieved in PAR score.

Validity and reliability of the Persian version of the psychosocial impact of dental aesthetics questionnaire

BackgroundThe psychosocial impact of dental aesthetics questionnaire (PIDAQ) is an efficient tool for assessment of oral health-related quality of life (OHRQoL). It evaluates the effect of dental esthetics on the psychosocial status of young adults. This questionnaire has been translated to many languages so far. However, it has not yet been translated to Persian. This study aimed to assess the validity and reliability of the Persian version of PIDAQ for use among the young adults.Material and methodThe questionnaire was translated to Persian, back-translated to English and underwent cultural adaptation and pretesting. It was then filled out by 398 young adults (215 females and 183 males) between 18 to 30 years in Shiraz, Iran. The Persian version of PIDAQ along with the index of orthodontic treatment need-aesthetic component (IOTN-AC) and the perception of occlusion (POS) index were administered among participants to assess its discriminant validity.ResultsFactor analysis extracted four domains and the factor loading of domains ranged from 0.479 to 0.837. The Cronbach’s alpha for the Persian version of PIDAQ ranged from 0.809 to 0.886. The mean score for each of the domains and the total score for PIDAQ, classified according to IOTN-AC and POS, showed a significant difference. The mean score acquired by subjects requiring orthodontic treatment was significantly higher than the score acquired by those not requiring orthodontic treatment (P = 0.00).ConclusionThe Persian version of PIDAQ has optimal validity, reliability and responsiveness for assessment of the psychosocial impact of malocclusion on the Iranian young adults.

The evolution of self-esteem before, during and after orthodontic treatment in adolescents with dental malocclusion, a prospective cohort study.

This study aimed (1) to investigate the evolution of self-esteem through orthodontic treatment, and (2) to study how key demographic factors would affect these evolutions and to assess relationships between self-esteem and orthodontic treatment need. This longitudinal prospective cohort study comprised of 326 adolescents (172 girls and 154 boys) aged 11-16 years; data were obtained from 325 adolescents at T0 and 123 at T2. Three hundred twenty-one adolescents filled in questionnaires at T0, whereas 118 at T2. They were selected in the University Hospitals Leuven, Belgium, where they all received orthodontic treatment. Self-esteem was assessed with the Dutch adaptation of the Harter's test and treatment need was defined by the Index of Orthodontic Treatment Need (IOTN). Data were analysed with multivariate linear models and Spearman correlations. There was no evidence of a change in global self-esteem during orthodontic treatment. A significant gender by time interaction for scholastic competence (P < 0.05), a decrease in self-esteem for females, and an increase for males between T0 and T1 was observed. A significant age (at T0) by time interaction for physical appearance and global self-worth (P < 0.05) and a negative correlation between self-esteem and self-assessed IOTN aesthetic component for the subdomain of close friendship (P < 0.05) were found. Global self-esteem acts as a stable construct during orthodontic treatment. The subdomains of self-esteem could be influenced by age and gender. Self-esteem and the subjective need for orthodontic treatment were found to be negatively correlated.

Correlation between oral health-related quality of life and orthodontic treatment need in children and adolescents-aprospective interdisciplinary multicentre cohort study.

Oral health-related quality of life (OHRQoL) is aconstruct for assessing the self-perceived oral health of patients. The aim of this study was to investigate the correlation between OHRQoL and orthodontic treatment need in consideration of demographic and psychological factors. This multicentre study included 250 patients with an indication for orthodontic diagnostics. In cooperation with the Institute of Clinical Psychology at the University of Würzburg, validated and internationally acknowledged questionnaires were selected to assess OHRQoL (COHIP-G19) and health-related quality of life (HRQOL). Self-esteem and behavioural problems were taken into consideration as possible psychological factors. Orthodontic treatment need was assessed using the Index of Orthodontic Treatment Need-Dental Health Component (IOTN-DHC), the Index of Orthodontic Treatment Need-Aesthetic Component (IOTN-AC) and the Dental Aesthetic Index (DAI). Possible significant correlations between the collected parameters and OHRQoL were evaluated by means of linear regression analysis. Objective orthodontic treatment need (IOTN-DHC and DAI) was significantly correlated with OHRQoL. Further factors significantly influencing OHRQoL in children and adolescents were age, HRQOL, self-esteem and behavioural problems. Objective orthodontic treatment need significantly influences OHRQoL in children and adolescents. Further studies are required to investigate if OHRQoL may be improved by correcting misaligned teeth and jaws.

Prevalence of orthodontic treatment need in permanent dentition of Iranian population: A systematic review and meta-analysis of observational studies.

Malocclusion is a common oral health problem and can affect the psychosocial well-being in the long term. Therefore, in the recent decades, demand for orthodontic treatment to correct malocclusion has greatly increased worldwide. This systematic review and meta-analysis was undertaken to assess existing evidence on the prevalence of orthodontic treatment need in Iran. National and international databases were searched for articles on the prevalence of orthodontic treatment need using index of orthodontic treatment need (IOTN) and dental aesthetic index (DAI). The required data were completed by hand-searching. After applying the inclusion and exclusion criteria, the quality of articles was checked by a professional checklist. Data extraction and meta-analysis were performed. A random effects model was employed, and publication bias was checked. From a total of 443 articles that reported orthodontic treatment need in Iran, 24 articles were included in the meta-analysis process. Meta-analysis was performed on components of IOTN and DAI. The pooled prevalence of orthodontic treatment need based on Dental Health Component and Aesthetic Component of IOTN and DAI was 23.8% (19.5%-28.7%), 4.8% (3.3%-7%), and 16.1% (12.3%-20.8%). The results were found to be heterogeneous (P < 0.05). The results of this study revealed that orthodontic treatment need was not high in the Iranian population. Considering the differing prevalence of orthodontic treatment need based on normative index and self-perceived index, it is essential to improve the people's awareness of malocclusion and its side effects on their oral and general health.
