Effectiveness of the combination of terbutaline and budesonide inhalation in treating acute bronchial asthma during pregnancy

Purpose: To investigate the therapeutic benefits of terbutaline sulfate aerosol inhalation combined with budesonide, and its influence on pulmonary function in pregnant women suffering from acute bronchial asthma attacks.
 Methods: A total of 100 pregnant patients diagnosed with acute bronchial asthma in PLA Strategic Support Force Characteristic Medical Center, Beijing, China were divided into control and study groups (n = 50 each). Control group received aerosol inhalation of normal saline along with standard treatments, viz, oxygen inhalation, sputum aspiration, anti-infection measures, and maintenance of water- electrolyte balance. Study group received combined terbutaline sulfate and budesonide aerosol inhalation in addition to standard treatment. Both groups underwent a 7-day treatment course, with inhalation therapy twice daily.
 Results: Study group showed significantly shorter relief times for cough, wheezing, and chest tightness compared to control group (p < 0.05). After treatment, 92.0 % of patients in study group exhibited improvement or relief from their symptoms, compared to 80.0 % in control group (p < 0.05). Pulmonary function indices, including first vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow (PEF), and FEV1/FVC, improved in both groups after treatment. Study group exhibited significantly lower laboratory indices, including immunoglobulin E (IgE), C-reactive protein (CRP), and eosinophils (EOS) compared to control group (p < 0.05).
 Conclusion: The combination of inhalation therapy of terbutaline sulfate with budesonide in pregnant women experiencing acute bronchial asthma demonstrates significant enhancements in clinical effectiveness, pulmonary function, and laboratory parameters. Clinical trials of this combined therapy should be carried out in other locations to ascertain the effect of population variation on treatment efficacy.

Changes and clinical significance of serum MMP-9, TIMP-1, COX-2, and immune levels in patients with asthma

Objective: To detect serum metalloproteinase-9 (MMP-9), tissue inhibitor of metalloproteinases (TIMP-1), cyclooxygenase-2 (COX-2), and T helper cells 1–T helper cells 2 (Th1–Th2) levels in asthma patients and assess their clinical significance. Methods: A total of 72 patients experiencing acute asthma (acute group), 66 stable asthma patients (stable group), and 60 healthy volunteers (control group) were included in this study. The levels of TIMP-1, COX-2, and Th1–Th2 in patients with acute asthma were measured following treatment with budesonide aerosol inhalation. In addition, the levels of MMP-9, TIMP-1, COX-2 and Th1–Th2 were compared in patients with different severity of acute asthma before and after treatment. Results: The serum levels of MMP-9, TIMP-1, and COX-2 showed an increasing trend in the control, stable, and acute groups, while levels of Th1–Th2 showed a sequential decreasing trend, and the differences were statistically significant. Comparison of lung function indexes among the three groups of patients established a negative correlation between serum MMP-9 and its forced vital capacity% predicted (FEV%pred), TIMP-1, and COX-2, and FEV%pred and forced expiratory volume in 1 s–forced vital capacity (FEV1/FVC) levels, but a positive correlation between Th1–Th2 and FEV1/FVC levels in the acute group. A significant difference was observed on comparing the levels of serum MMP-9, TIMP-1, COX-2, and Th1–Th2 in patients with different conditions in the acute group. Specifically, as the condition worsened, a significant increase in serum MMP-9, TIMP-1, and COX-2 levels but a significant decrease in Th1–Th2 levels was observed. After treatment, we observed a significant decrease in serum MMP-9, TIMP-1, and COX-2 levels but a significant increase in Th1–Th2 levels in the acute group. Conclusion: The serum levels of MMP-9, TIMP-1, COX-2, and Th1–Th2 are valuable indicators reflecting the condition of asthma patients and could be considered promising clinical monitoring indicators.

Effect of budesonide aerosol inhalation on postoperative complications and foreign body sensation in the throat of goiter resection patients

Purpose: To study the effect of budesonide aerosol inhalation on postoperative complications and foreign-body sensation in the throat of patients who underwent goiter resection.Methods: One hundred and twenty patients who underwent goiter resection at The Affiliated Hospital of Putian University (Fujian, China) from January 2019 to January 2020 were included in the study, and then equally and randomly assigned to groups A and B. During the perioperative period, group A patients were given budesonide aerosol inhalation, while group B patients received aerosol inhalation of equivalent volume of normal salineectively. Postoperative complication rate (CR), complication pain scores, scores on mucosal response in the throat, and scores on foreign body sensation in the throat were determined for both groups.Results: Postoperative complications in patients were hoarseness, sore throat and cough. Group A had significantly lower postoperative CR, lower complication pain scores, lower scores on mucosal response in the throat, and lower scores on foreign body sensation in the throat, when compared to group B (p < 0.001).Conclusion: Budesonide aerosol inhalation in patients who underwent goiter resection is effective in relieving throat injury from general anesthesia, minimizing likelihood of postoperative complications, and easing foreign-body sensation in the throat. Thus, this strategy may be suitable for the management of postoperative complications.

A clinical study of ultrasound-guided internal branch of superior laryngeal nerve block to treat postoperative sore throat after extubation under general anesthesia

Objective To investigate the efficacy of ultrasound-guided internal branch of superior laryngeal nerve (ibSLN) block to treat postoperative sore throat (POST) after extubation under general anesthesia. Methods Sixty patients, aged from 18 to 45 years old, American Society of Anesthesiologist (ASA) Ⅰ or Ⅱ who suffered from moderate to severe postoperative sore throat after extubation under general anesthesia were selected. They were divided into two groups according to the random number table method (n= 30): a lidocaine combined with budesonide aerosol inhalation group (group L) and an iBSLN block group (group S). The Visual Analogue Scale (VAS) scores and the significant analgesic efficiency rate of POST were recorded in two groups immediately before treatment (T0), 10 min after treatment (T1), 30 min after treatment (T2), 1 h after treatment (T3), 2 h after treatment (T4), 4 h after treatment (T5), 8 h after treatment (T6), and 24 h after treatment (T7). The mean arterial pressure (MAP), heart rate, and pulse oxygen saturation (SpO2) were also recorded from T0 to T7. Adverse reactions such as chocking on water, regurgitation and aspiration, hoarseness and dyspnea were observed in the two groups, while the score of patient satisfaction towards treatment was evaluated. Results The VAS score of POST in group S was lower than that in group L from T0 to T6 (P<0.05). The significant analgesic efficiency rate of POST in group S was significantly higher than that in group L from T1 to T6 (P<0.05). Compared with group L, heart rate in group S reduced from T1 to T4 (P<0.05), while MAP decreased from T1 to T3 (P<0.05). The satisfaction score of group S was higher than that of group L (P< 0.05). No chocking on water, regurgitation and aspiration, hoarseness, and dyspnea was found in the two groups. Conclusions Ultrasound-guided ibSLN block can effectively treat POST after extubation under general anesthesia, with remarkably improved analgesic effects in comparison with traditional methods where hormone combined with local anesthetic aerosol are inhaled. It provides a good approach to treat POST after extubation under general anesthesia. Effects of budesonide aerosol inhalation on the immunological functions of peripheral dendritic cells in patients with allergic rhinitis

Objective: To investigate the immunologic functional changes of peripheral dendritic cells (DC) in patients with allergic rhinitis (AR) during the treatment by budesonide aerosol inhalation.Method:25 patients with moderate to severe persistent AR were concluded in current study, and they were given budesonide aerosol inhalation for 8 weeks. AR related nasal symptom scores and indices, peripheral DC counts and percentages as well as their activation before and 4 weeks, 8 weeks after treatment were determined.Result:There were significant decrease in nasal symptom scores, eosinophils, AR specific IgE antibodies after 4 and 8 weeks of treatment. The overall clinical efficiency reached 88% at the end of the treatment. Peripheral DC counts and DC/PBMC decreased in comparison with the levels of pre-treatment (P<0.05). Compared with the levels before treatment, DC activation markers CD86 and CD80 reduced to various extent, while glucocorticoid-induced leucine zipper (GILZ) positive cells gradually elevated (P<0.05). In the DC secreted IL-10 decreased against the treatment duration, while IFN-γ from DC conditioned T cells statistically increased after treatment (P<0.05). Conclusion:Budesonide suppressed DC activation and maturation, which might redirect DC-involved Th2 response, in turn alleviating the AR symptoms.

Therapeutic effect of human umbilical cord mesenchymal stem cells on airway remodeling in an asthma model of rat and its possible mechanism

Objective: To observe the effect of human umbilical cord mesenchymal stem cells (hUC-MSCs) on airway remodeling in asthma model of rat and its possible mechanism. Methods: hUC-MSCs were isolated and cultured, and surface markers of hUC-MSCs were identified by flow cytometry. Forty Wistar male rats were divided into 4 groups: Control, Model, MSCs, Budesonide. The rats of Control group were sensitized and challenged by normal saline. The rats of Model, MSCs and Budesonide group were sensitized and challenged by ovalbumin (OVA) for 8 weeks. The MSCs group rats were given a tail vein injection of MSCs 0.2 ml (1×10(6) /ml) on days 35, 45, and 55 half an hour befor each OVA exposure. The Budesonide group rats were given aerosol inhalation of budesonide 2 mg two hours before each OVA exposure. Specimens were collected within 24 hours after the last OVA challenge. The parameters of airway morphological changes and the degree of airway remodeling were analyzed using Image-pro plus computer graphics. The levels of transformation growth factor (TGF) -β(1) in bronchoalveolar lavage fluid (BALF) and serum were detected by enzyme linked immunosorbent assay (ELISA). The expressions of E-cadherin, α-smooth muscle actin (α-SMA) and fibronectin (Fn) were measured by immunohistochemistry. Results: The thickness of airway wall and smooth muscle of Model, MSCs and Budesonide group rats were significantly thicker than Control group. The levels of TGF-β(1) in both BALF and serum of Model, MSCs and Budesonide group rats were significantly higher than Control group. The expression of E-cadherin of Model, MSCs and Budesonide group rats was significantly lower than Control group, while the expression of α-SMA and Fn were significantly higher. The thickness of airway wall and smooth muscle of MSCs and Budesonide group rats were significantly lower than Model group[(38.40±2.50, 45.34±0.33) vs (80.18±1.75) μm and (15.71±0.89, 18.57±0.67) vs (40.97±0.90) μm]. The levels of TGF-β(1) in both BALF and serum of MSCs and Budesonide group were significantly lower than Model group[(3.53±0.43, 3.11±0.05) vs (20.88±0.37) μg/L and (31.07±0.89, 31.12±0.50) vs (70.58±0.39)μg/L](all P<0.01). The expressions of α-SMA, Fn of MSCs and Budesonide group rats were significantly lower than Model group[(0.438±0.057, 0.445±0.027) vs (0.521±0.030) and (0.459±0.041, 0.458±0.029) vs (0.527±0.022)], While the expression of E-cadherin was significantly higher[(0.308±0.023, 0.296±0.010) vs (0.256±0.087)](all P<0.01). Conclusion: MSCs could alleviate asthmatic airway remodeling, the mechanism of which may be associated with the inhibition of TGF-β(1) induced epithelial-mesenchymal transition.

Multimodal analgesic analgesia in patients with obstructive sleep apnea hypopnea syndrome with multiple planar surgery

Objective:To observe the value of multimodal analgesia in patients with OSAHS undergoing multiplanar surgery.Method: A total number of 90 patients with obstructive sleep apnea hypopnea syndrome with tongue hypertrophy or hyperplasia of the root lymphoid tissue were collected. All patients underwent improved uvulatopharyngeal angioplasty (H-UPPP) and tongue root partial resection, or simultaneous tongue ablation at the same time, and they were randomly divided into two groups,45 patients in each group.In multi-modal analgesic group, the parrixibub sodium 40 mg were given intravenously 0.5 h before surgery, and oxygen budesonide aerosol inhalation therapy was given after surgery.Besides,sodium aescinate 10 mg was given intravenously 24, 48, 72 h after surgery,respectively.The control group did not do the above treatment. Both groups received 40 mg paradoxes sodium hydrostatic Bid for 4 days.To perform VAS on two groups of patients, uvula swelling time and first time to eat were recorded，and the symptoms of postoperative nausea and vomiting were observed.Result: The general conditions of the two groups of patients, including age, sex, body mass index, intraoperative blood loss, and operative time, were not statistically significant(all of the P>0.05). The scores of 24, 48, 72, 96 h VAS in multi-mode analgesic group were lower than those in control group after the operation of multi-mode analgesia, and the difference was statistically significant（P<0.05）.The duration of the swelling time of the uvula in the multi-mode analgesic group was significantly shorter than that in the control group, and the difference was statistically significant (5.44±0.88) d compared with (7.68±0.89) d (t=12.01, P<0.01);(30.1±7.3)h compared with (36.5±7.0) h,(t=4.25, P<0.01). Conclusion: Multi-mode analgesia is effective for OSAHS patients after multi-planar surgery. It effectively reduces postoperative pain, shortened postoperative swelling time, and improves the surgical compliance and safety.

The effect of premedication with budesonide aerosol inhalation on the incidence of respiratory adverse events during anesthesia recovery period in pediatric patients

Objective: We aimed to investigate the influence of premedication with budesonide aerosol inhalation on the incidence of respiratory adverse events during anesthesia recovery period in pediatric patients. Method: 120 pediatric patients underwent elective surgery of inguinal hernia repair were enrolled in the study. They were given two different types of anesthesia, group A received general anesthesia with the Laryngeal Mask (LMA), and group B were treated with budesonide aerosol inhalation preoperatively before general anesthesia with LMA. The incidence of adverse events which included laryngospasm, respiratory tract infection, the length of Post-Anesthesia Care Unit (PACU) staying, the volume of throat secretions and hoarseness were monitored closely. Results: Compared with group A, group B had a lower incidence of respiratory adverse events. Conclusion: Preoperative budesonide aerosol inhalation could reduce the incidence of respiratory adverse events during the anesthesia recovery period in children.

Meta-analysis of nursing and clinical efficacy of budesonide on children with bronchial asthma

Objective To explore the nursing and clinical efficacy of budesonide inhalation on children with bronchial asthma. Methods The computer retrieval on China CNKI, Wanfang database and VIP database was conducted for the randomized controlled trials in terms of budesonide inhalation and routine nursing to observe the curative effect of bronchial asthma on children, and the studies meeting the inclusion criteria were evaluated, and Meta analysis was performed by RevMan 5.2 statistical software.The effect of Budesonide aerosol inhalation to alleviate the symptoms of bronchial asthma on children was evaluated from the aspects of curative effect, the change of time for clinical symptoms and signs and vital capacity. Results A total of 11 studies (1 174 cases) were included in this study.Meta analysis showed that there were statistical significances in terms of differences of curative effect, the change of time for clinical symptoms and signs and vital capacity of the two groups. Conclusion Compared with the conventional nursing, budesonide inhalation was shown high effect on the clinical nursing for children with bronchial asthma, which can not only significantly increase the therapeutic effect, reduce the cough, asthma and wheezing disappearance time, but also increase the vital capacity of children with bronchial asthma. The two groups were treated routinely, in addition, the control group was treated with vein administration of methylprednisolone, the observation group received aerosol inhalation of budesonide, observed the clinical effect.&lt;strong&gt; Result: &lt;/strong&gt;The clinical effect of observation group is better and the adverse reaction rate is clearly lower. &lt;strong&gt;Conclusion: &lt;/strong&gt;Comparing with the control group which is treated with vein administration of methylprednisolone on the basis of routine treatment&lt;strong&gt;, &lt;/strong&gt;the effect of observation group which is treated with aerosol inhalation of budesonide is extremely precise and its adverse reaction is also relatively less. Clinical efficacy analysis on budesonide combined with montelukast in treatment of cough after infection in children

Objective To evaluate the efficacy of budesonide combined with montelukast in the treatment of cough after infection in children.Methods 106 children with bronchial asthma were selected and randomly divided into 2 groups with 53 ones in each group.The control group received aerosol inhalation of budesonide,and the treatment group received aerosol inhalation of budesonide combined with oral administration of montelukast after the treatment for 10 weeks.The clinical efficacy was mainly observed.Results The total effective rate of the treatment group was 85.1%,which was higher than 45.2% of the control group,the difference was significant （P〈0.05）.However,between the control group and the research group,there was no significant difference of total incidence rate of adverse reactions （16.9% vs 15.1%） （P〉0.05）.Conclusion Curative effect of budesonide combined with montelukast in treatment of cough after infection in children is good,should be recommended in primary hospitals. Serum interleukin 18 and 33 levels and its clinical significance in asthma patients

To study the levels of serum interleukin (IL)-18 and IL-33 in asthmatic patients pre-and post-treatment with glucocorticoids during the acute exacerbation period and therefore to investigate the role of IL-18 and IL-33 in the pathogenesis of bronchial asthma. Thirty cases of moderate to severe asthma admitted in the Second Affiliated Hospital of Dalian Medical University from June, 2009 to October, 2010 were chosen as the asthma group, and 23 healthy people who underwent routine physical examination were chosen as the control group. The subjects in the 2 groups were all nonsmokers. All the patients were treated with a glucocorticoid (methylprednisolone 40 - 80 mg/d) for 3 to 5 days and combined with aerosol inhalation of budesonide for 1 to 2 weeks as adjuvant therapy. The serum levels of IL-18 and IL-33 were detected for the asthma group before and after the treatment and for the control group. Other data including immunoglobulin E (IgE), the number of eosinophil cells (EOS) and the pulmonary function (FEV(1)%pre) were measured as well. The differences of IL-18 and IL-33 levels between the asthma group before and after treatment and the control group were analyzed, and the correlation between IL-18, IL-33 and IgE, EOS and FEV(1)%pre were analyzed as well by SPSS software. Compared with the healthy control group [(158 ± 45) ng/L and (143 ± 32) ng/L], the levels of IL-18 and IL-33 in the asthma pre-treatment group were significantly increased [(300 ± 181) ng/L and (208 ± 95) ng/L, t = 3.67 and 3.51, all P < 0.05)]; and the IgE level and EOS count showed a similar change (t = 5.76 and 5.24, all P < 0.05). After systemic glucocorticoid treatment and local aerosol inhalation, the levels of IL-18 and IL-33 [(183 ± 83) ng/L and (148 ± 77) ng/L] were decreased significantly in the asthma patients [t = 5.42 and 12.09, all P < 0.05], and the IgE level and EOS count also showed a same trend as well (t = 11.87 and 4.56, all P < 0.05), whereas FEV(1)%pred [(54 ± 14)%] increased significantly [(81 ± 16)%, t = -7.81, P < 0.05]. The Pearson correlative analysis showed that there were positive correlations between the levels of IL-18, IL-33 and IgE and EOS, respectively (all P < 0.05), and there were negative correlations between IL-18, IL-33 levels and FEV(1)%pre, respectively (all P < 0.05). Our results suggest that IL-18 and IL-33 participate in the pathogenesis of asthma, and may play stimulation role in the process of the disease.

Clinical Curative Effect Observation on Treatment of Chronic Obstructive Pulmonary Disease in Exacerbation Stage with Aerosol Inhalation of Budesonide Combined with Chinese Herbal Enema

Objective To observe clinical effect of treating chronic obstructive pulmonary disease(COPD) in exacerbation stage with aerosol inhalation of budesonide combined with Chinese herbal enema. Methods 36 cases with COPD were randomly recurited into a control group and a treatment group. The control group was treated with routine western medicine, and the treatment group was treated with aerosol inhalation of budesonide combined with Chinese herbal enema. The clinical effect was observed in both groups, Results The total effective rate in the treatment group and the control group was 88.9% and 72.2%, respectively, showing significant difference between the two group. (P<0.05) .Conclusion The treatment of aerosol inhalation of budesonide combined with Chinese herbal enema on COPD is better than routine western medicine. Clinical effects of Saibutamol combined Budesonide aerosol inhalation to cure capillary bronchitis

目的 观察毛细支气管炎患儿吸入万托林、万托林+布地奈德后血氧饱和度变化规律及症状、体征消失时间.方法 将确诊为毛细支气管炎的患儿77例随机分A、B 2组.A组45例予2 ml生理盐水加0.25 ml万托林雾化吸入.B组32例予等量生理盐水和万托林加布地奈德0.5 mg雾化吸入.分别记录2组患儿吸入前、吸入后30,60,90,120 min的血氧饱和度值,并比较2组患儿咳嗽、喘息、呼吸困难、喘鸣音消失时间.结果 在吸入后60,90 min,2组患儿的通气及血氧饱和度情况均显著改善;B组症状消失时间显著快于A组.结论 万托林及布地奈德联合雾化吸入治疗毛细支气管炎效果显著。
