Monitoring of urinary nickel and chromium in metal industries workers in Alborz, Iran

Heavy metal contamination from occupational sources is a cause for concern due to its potential accumulation in the environment and the body of living organisms, which leads to long-term toxic effects. There is limited evidence on the concentration of heavy metals in the body fluids of welding workers. This study aimed to measure the concentration of nickel (Ni) and chromium (Cr) as two main toxic heavy metals in the urine samples of welders and determine the potential relationship of various environmental factors on their concentration levels. This study was conducted in Iran in 2022–2023. Eighty-five urine samples were collected from the studied welding workers. Inductively coupled plasma optical emission spectrometry (ICP-OES) was used to measure the concentration of Ni and Cr. Data related to socio-demographic characteristics (education level, income, work experience, age, and body mass index (BMI)), lifestyle profile (consumption of seafood and canned food, smoking, physical activity), type of ventilation and preventive devices at work, and suffering from underlying diseases (cardiovascular and high blood pressure) were collected using valid questionnaires. Ni and Cr were present in 100% of urine samples with mean concentrations of (1.09 ± 0.294) and (11.45 ± 16.751) µg/g creatinine, respectively. A significant correlation was observed between the mean concentration of Ni and Cr with the level of education, income, work experience, age, consumption of seafood and canned food, smoking, type of ventilation and preventive devices, and underlying diseases (p-value < 0.05). Also, there was no significant difference between Ni concentration and physical activity and Cr concentration with BMI. Lifestyle determinants and not using preventive devices are important predictors of urinary heavy metals in welding workers. Therefore, more research is required to determine the adverse effects of long-term exposure to these heavy metals.

The Protective Effect of Curcumin Against Cardiotoxic Effects Induced by Chronic Exposure to Chlorpyrifos.

This study aimed to evaluate the effect of Chlorpyrifos (CPF) in rat heart tissue and the effect of Curcumin (Cur) on cardiac enzymes, oxidative indices, and histopathological changes in the cardiac tissue. CPF, the most used organophosphorus pesticide (OP), has been reported to induce cardiotoxic effects. The cardioprotective effects of Cur against CPF-induced toxicity have not been entirely investigated till now. Forty male Wistar rats were randomized into five groups (n=8). C group (Control animals that received olive oil), CPF group (10 mg/kg/day), CPF + Cur 25, CPF + Cur 50, and CPF + Cur 100 groups (animals received 10 mg/kg/day CPF and 25, 50, and 100 mg/kg Cur, respectively). All treatments were administered via oral gavage for 90 days. Cardiac enzymes (LDH & CPK) and oxidative stress (OS) biomarkers in heart tissue (malondialdehyde, Superoxide Dismutase) were measured. Histopathological changes in the heart tissue were also evaluated. Chronic exposure to CPF significantly increased cardiac enzyme levels and OS biomarkers. Histological changes were found, including disorganization of the cardiac muscle fibers with disorganization and degeneration in myocardial fibers with separation of myofibrils and cytoplasmic vacuolization of cardiac muscle fibers. Administration of Cur (100 mg/kg) reversed serum LDH concentration and OS biomarkers to normal levels in CPF-exposed animals (p < 0.05) and significantly improved cardiac damage. According to the results of this study, Cur can reduce the adverse effects of long-term exposure to CPF in rat heart tissue by modulating OS.

Long-term exposure to fine particulate matter constituents and cognitive impairment among older adults: An 18-year Chinese nationwide cohort study

BackgroundAlthough growing evidence has shown independent links of long-term exposure to fine particulate matter (PM2.5) with cognitive impairment, the effects of its constituents remain unclear. This study aims to explore the associations of long-term exposure to ambient PM2.5 constituents’ mixture with cognitive impairment in Chinese older adults, and to further identify the main contributor. Methods15,274 adults ≥ 65 years old were recruited by the Chinese Longitudinal Healthy Longevity Study (CLHLS) and followed up through 7 waves during 2000–2018. Concentrations of ambient PM2.5 and its constituents (i.e., black carbon [BC], organic matter [OM], ammonium [NH4+], sulfate [SO42-], and nitrate [NO3-]) were estimated by satellite retrievals and machine learning models. Quantile-based g-computation model was employed to assess the joint effects of a mixture of 5 PM2.5 constituents and their relative contributions to cognitive impairment. Analyses stratified by age group, sex, residence (urban vs. rural), and region (north vs. south) were performed to identify vulnerable populations. ResultsDuring the average 3.03 follow-up visits (89,296.9 person-years), 4294 (28.1%) participants had developed cognitive impairment. The adjusted hazard ratio [HR] (95% confidence interval [CI]) for cognitive impairment for every quartile increase in mixture exposure to 5 PM2.5 constituents was 1.08 (1.05–1.11). BC held the largest index weight (0.69) in the positive direction in the qg-computation model, followed by OM (0.31). Subgroup analyses suggested stronger associations in younger old adults and rural residents. ConclusionLong-term exposure to ambient PM2.5, particularly its constituents BC and OM, is associated with an elevated risk of cognitive impairment onset among Chinese older adults.

The Effectiveness of Botulinum Toxin Injections to Treat Post-Stroke Arm Dysfunction

This review was written to determine the effectiveness of Botulinum Toxin (BoNT) as a treatment for upper extremity dysfunction in post-stroke patients. As most patients survive a stroke, many are burdened with disability and impaired limb function. Seventy percent of stroke patients experience motor dysfunction in the arm, with thirty percent of patients suffering from upper extremity spasticity. Spasticity is a speed-dependent condition where certain muscles are abnormally stiff, resulting in involuntary contraction when the spastic muscle is used. BoNT treatment includes injections in localized muscles and is primarily used to treat spasticity by temporarily paralyzing the injected muscle. Seven systematic reviews/meta-analyses and ten randomized-controlled trials were used to determine the effectiveness of BoNT in post-stroke patients. Overall, BoNT is effective in reducing spasticity and may be effective in reducing pain. However, its ability to improve motor function is still unclear. In addition, there is mixed evidence on the most effective way to administer BoNT treatment, such as the proper dosage or combining BoNT with another therapy. The treatment’s effectiveness is well documented in the chronic stage of stroke but less so in the acute and subacute stages. More research is needed to determine the potential adverse effects of long-term exposure to BoNT and the most appropriate dose to administer.

Long-term exposure to outdoor air pollution and asthma in low-and middle-income countries: A systematic reviewprotocol.

Several epidemiological studies have examined the risk of asthma and respiratory diseases in association with long-term exposure to outdoor air pollution. However, little is known regarding the adverse effects of long-term exposure to outdoor air pollution on the development of these outcomes in low- and middle-income countries (LMICs). Our study aims to investigate the association between long-term exposure to outdoor air pollution and asthma and respiratory diseases in LMICs through a systematic review with meta-analysis. This systematic review and meta-analysis will follow the PRISMA (Preferred Reporting for Systematic Reviews and Meta-Analyses) checklist and flowchart guidelines. The inclusion criteria that will be used in our study are 1) Original research articles with full text in English; 2) Studies including adult humans; 3) Studies with long-term air pollution assessment in LMICs, air pollutants including nitrogen oxide (NO2), sulfur oxide (SO2), particulate matter (PM2.5 and PM10), carbon monoxide (CO) and ozone (O3); 4) cohort and cross-sectional studies; 5) Studies reporting associations between air pollution and asthma and respiratory symptoms. A comprehensive search strategy will be used to identify studies published up till August 2022 and indexed in Embase, Medline, and Web of Science. Three reviewers will independently screen records retrieved from the database searches. Where there are enough studies with similar exposure and outcomes, we will calculate, and report pooled effect estimates using meta-analysis. PROSPERO CRD42022311326. Findings from the health effects of long-term exposure to outdoor air pollution may be of importance for policymakers. This review will also identify any gaps in the current literature on this topic in LMICs and provide direction for future research.

GEANT4 dose estimations of solar protons: Al and PMMA-Bi2O3 shielding for space exploration

Adverse effects of long-term exposure to galactic cosmic radiation (GCR) pose a clear obstacle to future space exploration programs. In addition to GCR we have solar particle radiation. We simulated the latter using a scaled fluence profile of solar protons taken from a literature study that comprises about three solar cycles. The model is a three-layer stack that includes shielding material and muscular tissue. Our simulation strategy uses protons as precursor radiation of neutrons. Subsequently, the shield is adjusted for thickness, dictated by an average depth at which neutrons are created through various processes during the simulation. Neutrons are then energy-binned and a corresponding neutron flux is simulated. Particles generated during the second phase of the simulation, i.e. by neutrons, are then counted toward absorbed dose within the muscular tissue layer. Clearly, the dynamics of the process is not captured by the simulation, nevertheless an overview of neutron yield can be estimated and the absorbed dose. The objective is to provide some insight about the effect of the new composite shield, PMMA-Bi2O3, that has an intrinsic capability for gamma dose reduction, compared to a more traditional aluminum shield.

Smartphone-based holographic measurement of polydisperse suspended particulate matter with various mass concentration ratios

Real-time monitoring of suspended particulate matter (PM) has become essential in daily life due to the adverse effects of long-term exposure to PMs on human health and ecosystems. However, conventional techniques for measuring micro-scale particulates commonly require expensive instruments. In this study, a smartphone-based device is developed for real-time monitoring of suspended PMs by integrating a smartphone-based digital holographic microscopy (S-DHM) and deep learning algorithms. The proposed S-DHM-based PM monitoring device is composed of affordable commercial optical components and a smartphone. Overall procedures including digital image processing, deep learning training, and correction process are optimized to minimize the prediction error and computational cost. The proposed device can rapidly measure the mass concentrations of coarse and fine PMs from holographic speckle patterns of suspended polydisperse PMs in water with measurement errors of 22.8 ± 18.1% and 13.5 ± 9.8%, respectively. With further advances in data acquisition and deep learning training, this study would contribute to the development of hand-held devices for monitoring polydisperse non-spherical pollutants suspended in various media.

The adverse effects of long-term exposure to antipsychotics among older people with intellectual disabilities: a scoping review

Background Antipsychotics are among the medications most prescribed to older adults with intellectual disabilities despite limited evidence to support their safety and efficacy in this population. Antipsychotics are associated with a significant burden of long-term adverse effects including movement disorders, metabolic and cardiovascular adverse effects, sedation and anticholinergic effects. This scoping review aims to evaluate the current extent of the literature relating to adverse effects of long-term antipsychotic use in older adults with intellectual disabilities and identify any existing gaps. Methods The review was conducted in line with the framework for scoping reviews proposed by Arksey and O’Malley. A systematic literature search was carried out, including searches of PubMed, Cochrane Library, ScienceDirect, Embase, PsycINFO and grey literature databases. Reference lists of studies were also reviewed as part of the search. Studies were included in the review if they related to adults over 40 years of age with an intellectual disability who had been taking antipsychotic medication for at least 3 months. Results A total of 13 studies were identified for inclusion in the review. These included retrospective reviews,observational studies, case reports, cohort studies and cross-sectional studies. Adverse effects reported include extrapyramidal symptoms, cardiovascular and metabolic effects, and case reports of rhinorrhoea, hypothermia and ischaemic colitis. Increasing age was associated with a greater burden of adverse effects in some studies. Conclusion The available evidence on the adverse effects of long-term antipsychotic use in older adults with intellectual disabilities is weak and conflicting. The studies included were generally of poor quality with numerous limitations including small sample sizes and lack of control groups. More research is needed to inform understanding of adverse effects associated with long-term antipsychotic use in this population.

Spatiotemporally Continuous Reconstruction of Retrieved PM2.5 Data Using an Autogeoi-Stacking Model in the Beijing-Tianjin-Hebei Region, China

Aerosol optical depth (AOD) observations have been widely used to generate wide-coverage PM2.5 retrievals due to the adverse effects of long-term exposure to PM2.5 and the sparsity and unevenness of monitoring sites. However, due to non-random missing and nighttime gaps in AOD products, obtaining spatiotemporally continuous hourly data with high accuracy has been a great challenge. Therefore, this study developed an automatic geo-intelligent stacking (autogeoi-stacking) model, which contained seven sub-models of machine learning and was stacked through a Catboost model. The autogeoi-stacking model used the automated feature engineering (autofeat) method to identify spatiotemporal characteristics of multi-source datasets and generate extra features through automatic non-linear changes of multiple original features. The 10-fold cross-validation (CV) evaluation was employed to evaluate the 24-hour and continuous ground-level PM2.5 estimations in the Beijing-Tianjin-Hebei (BTH) region during 2018. The results showed that the autogeoi-stacking model performed well in the study area with the coefficient of determination (R2) of 0.88, the root mean squared error (RMSE) of 17.38 µg/m3, and the mean absolute error (MAE) of 10.71 µg/m3. The estimated PM2.5 concentrations had an excellent performance during the day (8:00–18:00, local time) and night (19:00–07:00) (the cross-validation coefficient of determination (CV-R2): 0.90, 0.88), and captured hourly PM2.5 variations well, even in the severe ambient air pollution event. On the seasonal scale, the R2 values from high to low were winter, autumn, spring, and summer, respectively. Compared with the original stacking model, the improvement of R2 with the autofeat and hyperparameter optimization approaches was up to 5.33%. In addition, the annual mean values indicated that the southern areas, such as Shijiazhuang, Xingtai, and Handan, suffered higher PM2.5 concentrations. The northern regions (e.g., Zhangjiakou and Chengde) experienced low PM2.5. In summary, the proposed method in this paper performed well and could provide ideas for constructing geoi-features and spatiotemporally continuous inversion products of PM2.5.

The food preservative sodium propionate induces hyperglycaemic state and neurological disorder in zebrafish

Propionate is an effective mould inhibitor widely used as a food preservative. In this study, we used zebrafish to explore the adverse effects of long-term exposure to low concentrations of sodium propionate and the underlying molecular mechanisms (from larvae to adult). When exposed for 3 months, we found that blood glucose, total cholesterol, and triglyceride levels increased, and zebrafish developed a hyperglycaemic state. New tank test results showed depression in zebrafish reduced 5-hydroxytryptamine levels in the brain and damaged the dopamine system. At the same time, the results of the color preference test showed that zebrafish had cognitive impairments. In addition, Hypothalamic-Pituitary-Adrenal axis analysis revealed abnormal gene expression, increased cortisol levels, and reduced glucocorticoid receptor mRNA levels, which were consistent with depressive behavior. We also observed abnormal transcription of inflammatory and apoptotic factors. Overall, we found that chronic exposure to sodium propionate induces depressive symptoms. This may be related to the activation of the HPA axis by the hyperglycaemic state, thereby inducing inflammation and disrupting the dopaminergic system. In summary, this study provides theoretical and technical support for the overlap of the emotional pathogenesis associated with diabetes.

Reproductive toxicity of long-term exposure to environmental relevant concentrations of cyprodinil in female zebrafish (Danio rerio)

In recent years, the widespread use of the pesticide cyprodinil has attracted attention due to its harmful effects on aquatic organisms. The purpose of this study was to evaluate the adverse effects of long-term exposure to cyprodinil on the reproductive system of female zebrafish. After the embryos had been treated with 0.1, 1 and 10 μg/L cyprodinil for 180 days, we observed that female fish treated with 1 and 10 μg/L cyprodinil showed decreased sexual attractiveness, a decreased proportion of primordial follicles in the ovary, an increased proportion of mature follicles, and increased egg production. Moreover, exposed females that mated with normal males produced offspring with increased rates of mortality and deformity (the F1 generation). In addition, the levels of gonadotropin and testosterone (T) were increased in females after cyprodinil exposure, especially in the 10 μg/L treated group. After cyprodinil treatment, some key genes in the hypothalamic-pituitary-gonad axis underwent significant changes. For example, gene expression of brain gonadotropin-releasing hormone receptors (gnrhr1, gnrhr2 and gnrhr4) was significantly downregulated after cyprodinil treatment. The study found that expression of the aromatase (cytochrome P450 family 19 subfamily A polypeptide 1a, cyp19a1a) responsible for converting T into estradiol was significantly downregulated after cyprodinil treatment, consistent with elevated T levels in the ovaries and muscles. In summary, these data provide a more comprehensive understanding of the toxicity of cyprodinil and may inform evaluation of the ecotoxicity of cyprodinil to female reproduction at environmentally relevant concentrations.

Interaction between \u03c9-6 fatty acids intake and blood cadmium on the risk of low cognitive performance in older adults from National Health and Nutrition Examination Survey (NHANES) 2011\u20132014

BackgroundIdentifying preventable diets and environmental exposure is essential to ensuring the health of the aging population. This study evaluated the interaction effect between blood cadmium and ω-6 fatty acids intake on low cognitive performance in Americans.MethodThe data of this cross-sectional study were obtained from the 2011–2012 and 2013–2014 National Health and Nutritional Examination Survey (NHANES). Cognitive performance was measured by the Consortium to Establish a Registry for Alzheimer’s Disease test, Animal Fluency Test, and Digit Symbol Substitution Test. Multivariate logistic regression models were used.ResultsA total of 1,918 individuals were included, with 467 (24.35%) low cognitive performance. Compared with participants with normal-level blood cadmium, those with high-level blood cadmium had a higher risk of low cognitive performance [odds ratio (OR) was 1.558 with 95% confidence interval (CI): 1.144–2.123]. Low-level ω-6 fatty acids intake was positively associated with low cognitive performance [OR = 1.633 (95%CI: 1.094–2.436)] compared with normal-level intake. Moreover, there was a significant interaction between low-level ω-6 fatty acids intake and high-level blood cadmium on the risk of low cognitive performance (relative excess risk due to interaction: 0.570, 95%CI: 0.208-0.932; the attributable proportion of interaction: 0.219, 95%CI: 0.102‐0.336; synergy index: 1.552, 95%CI: 1.189‐2.027).ConclusionsThere was a synergistic interaction between low-level ω-6 fatty acids intake and high-level blood cadmium on low cognitive performance. Low-level ω-6 fatty acids intake may amplify the adverse effects of long-term exposure to cadmium on cognitive performance. This may have a certain significance for the prevention of cognitive decline in the elderly.

Underestimated or overestimated? Dynamic assessment of hourly PM2.5 exposure in the metropolitan area based on heatmap and micro-air monitoring stations.

Spatio-temporal distributions of air pollution and population are two important factors influencing the patterns of mortality and diseases. Past studies have quantified the adverse effects of long-term exposure to air pollution. However, the dynamic changes of air pollution levels and population mobility within a day are rarely taken into consideration, especially in metropolitan areas. In this study, we use the high-resolution PM2.5 data from the micro-air monitoring stations, and hourly population mobility simulated by the heatmap based on Location Based Service (LBS) big data to evaluate the hourly active PM2.5 exposure in a typical Chinese metropolis. The dynamic "active population exposure" is compared spatiotemporally with the static "census population exposure" based on census data. The results show that over 12h on both study periods, 45.83% of suburbs' population-weighted exposure (PWE) is underestimated, while 100% of rural PWE and more than 34.78% of downtown's PWE are overestimated, with the relative difference reaching from -11μg/m3 to 7μg/m3. More notably, the total PWE of the active population at morning peak hours on weekdays is worse than previously realized, about 12.41% of people are exposed to PM2.5 over 60μg/m3, about twice as much as that in census scenario. The commuters who live in the suburbs and work in downtown may suffer more from PM2.5 exposure and uneven environmental resource distribution. This study proposes a new approach of calculating population exposure which can also be extended to quantify other environmental issues and related health burdens.

Integrating random forests and propagation models for high-resolution noise mapping

The adverse effects of long-term exposure to environmental noise on human health are of increasing concern. Noise mapping methods such as spatial interpolation and land use regression cannot capture complex relationships between environmental conditions and noise propagation or attenuation in a three-dimension (3D) built environment. In this study, we developed a hybrid approach by combining a traffic propagation model and random forests (RF) machine learning algorithm to map the total environment noise levels for daily average, daytime, nighttime, and day-evening-nighttime at 30 m × 30 m resolution for the island of Montreal, Canada. The propagation model was used to predict traffic noise surfaces using road traffic flow, 3D building information, and a digital elevation model. The traffic noise estimates were compared with ground-based sound-level measurements at 87 points to extract residuals between total environmental noise and traffic noise. Residuals at these points were fit to RF models with multiple environmental and geographic predictor variables (e.g., vegetation index, population density, brightness of nighttime lights, land use types, and distances to noise contour around the airport, bus stops, and road intersections). Using the sound-level measurements as baseline data, the prediction errors, i.e., mean error, mean absolute error, and root mean squared error of daily average noise levels estimated by our hybrid approach was −0.03 dB(A), 2.67 dB(A), and 3.36 dB(A). Combining deterministic and stochastic models can provide accurate total environmental noise estimates for large geographic areas where sound-level measurements are available.

Simulation of Dose Estimations from Solar Protons: A PMMA-Bi2O3 Shielding Model for Space Exploration

Adverse effects of long-term exposure to galactic cosmic radiation (GCR) pose a non negligible obstacle for future space exploration programs; the high-LET-particle-rich environment has an adverse effect on human health. Concomitant to GCR we have as well solar particle radiation. Long term space exploration will rely on adequate and highly efficient shielding materials that will reduce exposure of both biosystems and electronic equipment to GCR and solar particles. The shield must attenuate efficiently heavy GCR ions, by breaking them up into less-damaging fragments and secondary radiation: biologically damaging energetic neutrons and highly charged and energetic HZE- particles. An approach to this problem is the development of shielding compounds. Shielding materials should address the conditions of different aspects of a given mission, e.g. time duration and travel path. The Monte Carlo method (GEANT4) is here employed to estimate the effects of a shielding material based on the recently developed Bi2O3-based compound (Cao et al., 2020). In the present study GEANT4 code is used to make estimations of attenuation of solar protons. The objective is to provide some insight about the effect of the new composite shield that has an intrinsic capability for dose reduction.

Associations of air pollution and greenness with mortality in Greece: An ecological study

BackgroundEpidemiological studies have documented the adverse effects of long-term exposure to fine particulate matter (PM2.5) on health, while there has been less research on the effects of nitrogen dioxide (NO2), black carbon (BC) and especially ozone (O3). Furthermore, there is limited evidence for the synergistic effects of exposure to pollutants and greenness. We investigated the association of long-term exposure to air pollution and greenness with natural-cause, cardiovascular and respiratory mortality in Greece using an ecological study design. MethodsMortality and socioeconomic data were obtained from 1035 municipal units from the 2011 census data. Annual average PM2.5, NO2, BC and O3 concentrations for 2010 were derived from 100 × 100 m surfaces predicted by hybrid LUR models. The normalized difference vegetation index was used to assess greenness. We applied Poisson regression models on standardized mortality rates adjusted for socioeconomic indicators and lung cancer rates, accounting for spatial autocorrelation. The analysis was conducted initially for the whole country and then separately for urban and rural areas. We assessed interactions between pollutants and greenness and applied two-exposure models. ResultsAn interquartile range (IQR) increase in PM2.5, NO2 and BC was associated with increases in natural-cause mortality (Relative Risk (RR) 1.09, 95% confidence interval (CI): 1.08, 1.11; RR 1.03 (95% CI: 1.03, 1.04) and RR 1.02 (95% CI: 1.02, 1.03), respectively), while PM2.5 and NO2 were also associated with cause-specific mortality. Greenness was associated with lower natural-cause (RR 0.95, 95% CI: 0.94, 0.96 per IQR) and cause-specific mortality. For all outcomes we estimated a protective association with O3 (natural-cause mortality RR 0.79, 95% CI: 0.76, 0.82 per IQR). All associations were stronger in urban areas. We estimated negative statistically significant interactions between air pollution and greenness for respiratory morality and positive ones for cardiovascular mortality. Estimates were mostly robust to co-exposure adjustment. ConclusionsOur findings support associations of air pollution and greenness with mortality, both in urban and rural areas of Greece. Further research is needed to elaborate on the synergies in cause-specific mortality. Our results on the interactions between pollutants and greenness may imply differential biological mechanisms for cause-specific mortality and warrant further investigation.

Are the adverse health effects of air pollution modified among those with a more active lifestyle? A review of the literature

Air pollution concentrations are often high in major cities worldwide. As such, millions of people engage in regular physical activity in highly polluted outdoor environments. It is unclear if the adverse effects of long-term exposure to air pollutants are modified by regular physical activity. Methods: To help understand this complex issue, we reviewed the peer-reviewed literature for studies that examined whether the impacts of air pollution exposure are modified by routine physical activity. Articles were identified through PubMed, Science Direct, ProQuest Agricultural & Environmental Science Database searches on terms encompassing air pollution and exercise/physical activity (n=334). After initial screening, 16 papers were initially included in this review. After full review, three studies did not meet the criteria for review and were excluded. Results: Most studies occurred in Asia (n=5), followed by North America (n=4), and Europe (n=3). Eight studies concluded the risks of exposure to air pollution outweigh the benefits of physical activity especially among children. However, five studies concluded that regular physical activity can negate the negative impacts of long-term air pollution exposure, particularly among healthy adults. Conclusions: Initial results of this review suggest sensitive groups, such as children, who are routinely physically active in areas with high air pollution levels incur more risk than benefit. Yet, regular physical activity among healthy adults may decrease the negative effects of long-term exposure to air pollution. This abstract does not necessarily reflect EPA policy. Polyethylene Glycol Exposure with Antihemophilic Factor (Recombinant), PEGylated (rurioctocog alfa pegol) and Other Therapies Indicated for the Pediatric Population: History and Safety

Polyethylene glycol (PEG) is an inert, water soluble polymer, used for decades in pharmaceuticals. Although PEG is considered safe, concerns persist about the potential adverse effects of long-term exposure to PEG-containing therapies, specifically in children, following the introduction of PEGylated recombinant factor products used for the treatment of hemophilia. Given the absence of long-term surveillance data, and to evaluate the potential risk, we estimated PEG exposure in the pediatric population receiving PEGylated therapies with pediatric indications administered intravenously or intramuscularly. We used a range of pediatric weights and doses based on prescribing information (PI) or treatment guidelines. PIs and reporting websites were searched for information about adverse events (AEs). For a child weighing 50 kg on the highest prophylactic dose of a FVIII product, the range of total PEG exposure was 40–21,840 mg/year; for factor IX (FIX) products, the range was 13–1342 mg/year; and for other products, the range was 383–26,743 mg/year, primarily as a derivative excipient. No AE patterns attributable to PEG were found for any of these products, including potential renal, neurological, or hepatic AEs. Our analyses suggest the pediatric population has had substantial exposure to PEG for several decades, with no evidence of adverse consequences.

Endocrine disruption in Chinese rare minnow (Gobiocypris rarus) after long-term exposure to low environmental concentrations of progestin megestrol acetate

Synthetic progestins are widely used pharmaceutical agents that have become common contaminants in the aquatic environment. The potential adverse effects of long-term exposure on aquatic wildlife, however, are not fully understood. The aim of this study was to investigate the endocrine disruption in Chinese rare minnow (Gobiocypris rarus) in response to megestrol acetate (MTA) exposure. Newly-hatched Chinese rare minnow larvae were exposed to MTA at a nominal concentration of either 1 ng/L (detected concentrations ranged from 0.18 to 0.93 ng/L) or 10 ng/L (detected concentrations ranged from 4.27 to 9.64 ng/L) for 6 months and the effects on growth, sex steroid hormones, gonadal histology, and steroidogenic genes expression were determined. After 6 months of exposure to a nominal concentration of 10 ng/L MTA, the body weight and condition factors were significantly increased in fish of both sexes. Exposure to a nominal concentration of 10 ng/L MTA significantly reduced plasma concentrations of estradiol and 11-ketotestosterone in female fish while also reducing testosterone and 11-ketotestosterone in male fish. Gonad histology revealed significantly reduced proportions of vitellogenic oocytes in female fish exposed to a nominal concentration of 10 ng/L MTA and induction of atretic follicles in female fish exposed to both nominal concentrations of MTA. The expression of cyp19a1a and cyp17a1 in the gonads was up-regulated in the ovaries while down-regulated in the testes. Our results indicate that MTA can induce endocrine disruption in Chinese rare minnow at the low concentrations found in contaminated environments. This indicates a potentially high ecological risk from MTA to fish populations in MTA-contaminated aquatic environments in China and may also in other regions.

Subchronic toxicity study of yttrium nitrate by 90-day repeated oral exposure in rats

Concerns regarding the adverse effects of long-term exposure to low levels of rare earth elements (REEs) from foods on human health have arisen in recent years. Nevertheless, no official acceptable daily intake (ADI) has yet been proposed for either total REEs or individual REE. In accordance with the Organization for Economic Co-operation and Development (OECD) testing guideline, the present study was undertaken to evaluate the subchronic toxicity of yttrium, a representative heavy REE with higher contaminated level in foods in China, to achieve a no observed adverse effect level (NOAEL) which is a critical basis for the establishment of an ADI. Yttrium nitrate was orally administered to rats at doses of 0, 10, 30 and 90 mg/kg/day for 90 days followed by a recovery period of 4 weeks. The following toxicity indices were measured: mortality, clinical signs, daily food consumption and weekly body weight; urinalysis, hematology, blood coagulation, clinical biochemistry and histopathology at the end of administration and recovery periods. No toxicologically significant changes were found in any yttrium-treated group as compared to the concurrent control group. Under the present experimental condition, the NOAEL in rats was thus set at 90 mg/kg for yttrium nitrate, i.e. 29.1 mg/kg for yttrium.
