Role of perceived ease of use, usefulness, and financial strength on the adoption of health information systems: the moderating role of hospital size

Adoption of a health information system is always a challenge for hospitals. It is because most of the medical staff do not have enough skills to use the new technology and due to the sensitivity of medical data. These factors pose a challenge for the successful adoption of health information system in hospitals. The aim of this research is to find out the factors which influence the adoption of information systems in hospitals. The study investigated the impact of the Financial status of the Hospital; Perceived Usefulness and Perceived Ease of Use on the adoption of health information systems through a questionnaire survey. Data was collected from 602 healthcare workers from 20 hospitals through close-ended questionnaire in Pakistan, where the adoption of health information systems is very slow. PLS-SEM was used for the analysis. The findings show that the Financial status of the Hospital; Perceived Usefulness and Perceived Ease of Use have positive and significant role in the adoption of Health Information Systems. The finding also shows that hospital size moderates the relationship of Perceived ease of use and the adoption of health information systems and interestingly it does not moderate the relationship among perceived usefulness and financial strength toward the adoption of health information systems. The study concludes that perceived ease of use, perceived usefulness and financial strength are the main factors, necessary for the adoption of health information systems. The findings of the study have useful implications for policy makers, medical professionals to successfully adopt health information systems in hospitals. It also provides new avenues for researchers to explore other factors and test this framework in other countries.

Investigating the role of Cybersecurity's perceived threats in the adoption of health information systems

Information technology is one of the most rapidly growing technologies globally. Over the last decade, its usage in healthcare has been remarkable. Over the last decade, its usage in healthcare has been remarkable. The study examines the impact of various factors as barriers to adopting the information system in healthcare. These factors are categorized into three major types: external attacks, which include phishing attacks and ransomware; employee factors, including lack of skills and the issue of information misuse; and technological factors, including complexity and vulnerability. The findings show that external attacks and technological factors are the main barriers to adopting information systems, while employee factors have no significant impact on the adoption of information systems in the healthcare industry of Pakistan. The study provides implications for healthcare policy makers, professionals and organziations regarding the successful adoption of health information system.

Ten years of research mined for security and privacy concerns of health information system adoption and acceptance: coherent taxonomy, motivations and open challenges

This survey provides a taxonomy of the privacy and security factors influencing the acceptance of health information systems (HIS) and classifies these factors into motivation and limitations of HIS acceptance. We implemented a focused search for each article on (1) HIS (2) acceptance or adoption (3) privacy or security, in six main databases. Results: The final set included 65 articles. Most of the articles (50.76%; 33/65) are empirical studies that refer to the user perspective about privacy and security of HIS and adopt a specific theory of technology acceptance model. The next largest portion of articles (36.92%; 24/65) is reviews and surveys articles, which indicate to explain the existing HIS for a certain system, privacy, and security to provide a general review. The smallest and last section of the work (12.30%; 8/65) refers to proposals for frameworks or models. This review highlights the frequent of privacy and security factors influencing the acceptance and adoption of HIS.

Understanding the impacts of health information systems on patient flow management: A systematic review across several decades of research.

Patient flow describes the progression of patients along a pathway of care such as the journey from hospital inpatient admission to discharge. Poor patient flow has detrimental effects on health outcomes, patient satisfaction and hospital revenue. There has been an increasing adoption of health information systems (HISs) in various healthcare settings to address patient flow issues, yet there remains limited evidence of their overall impacts. To systematically review evidence on the impacts of HISs on patient flow management including what HISs have been used, their application scope, features, and what aspects of patient flow are affected by the HIS adoption. A systematic search for English-language, peer-review literature indexed in MEDLINE and EMBASE, CINAHL, INSPEC, and ACM Digital Library from the earliest date available to February 2022 was conducted. Two authors independently scanned the search results for eligible publications, and reporting followed the PRISMA guidelines. Eligibility criteria included studies that reported impacts of HIS on patient flow outcomes. Information on the study design, type of HIS, key features and impacts was extracted and analysed using an analytical framework which was based on domain-expert opinions and literature review. Overall, 5996 titles were identified, with 44 eligible studies, across 17 types of HIS. 22 studies (50%) focused on patient flow in the department level such as emergency department while 18 studies (41%) focused on hospital-wide level and four studies (9%) investigated network-wide HIS. Process outcomes with time-related measures such as 'length of stay' and 'waiting time' were investigated in most of the studies. In addition, HISs were found to address flow problems by identifying blockages, streamlining care processes and improving care coordination. HIS affected various aspects of patient flow at different levels of care; however, how and why they delivered the impacts require further research.

Towards an Evaluation Framework for Ubiquitous, Self-Evolving Patient Identification Solutions in Health Information Systems

The increasing adoption of Health Information Systems (HIS) does not seem to have resolved the ongoing lack of ubiquitous, dependable and accurate patient information so as to effectively prevent medical errors. Previous work has identified multiple causes, including but not limited to improper or incomplete HIS implementation, incompatibility in healthcare standards, lack of proper data input and validation, and accelerating evolution of technology triggering instability of candidate solutions depending on it. This paper continues the research by describing high-level non-functional requirements that any solution should satisfy relying on current best-practice, and subsequently customizing an established international standard so as to define an evaluation framework that can be used to assess candidate HIS architectures. The ultimate aim of the research is to support selection of stable, sustainable long-term architectural solutions and thus to assist HIS strategic decision making and self-evolution supporting agility.

Technology Acceptance Models to Improve Adoption of Health Information Systems

Technology acceptance model (TAM) used to enhance understanding of technology acceptance in the workplace as healthcare environment. The use of technology in the healthcare environment improves healthcare outcomes, control the healthcare data, improve collaboration and skills among healthcare staff and reduce organizational expenses. Recent literature studies done indicated lack of adoption of technology as the health information systems (HISs), this be affect on collaboration among healthcare staff this in turn affect on healthcare outcomes and medical research findings. This research attempts to address the aforementioned issues by applying the extended TAM based on relevant models. This research identifies success factors influencing the adoption of HISs in healthcare environment. This research employed the exploratory qualitative research method to collections the required data. Results of this study shows that factors such as data confidentiality "security and privacy" becoming more important in addition to current factors such as perceived usefulness, ease of use, quality” system and information". Beside to, privacy factor have significantly affect the intent to use and the adoption of HISs by healthcare staff.

User involvement and satisfaction levels of health information systems during the design and development phase

Objectives: Many factors affect adoption of health information systems, user involvement and satisfaction are some of the main factors of user adoption of health information systems. Despite the benefits due to social, cultural, organizational and technological factors some of the benefits become complex and difficult to achieve in practice. Both users and development team lack time due to busy schedules, sometimes disagreements between the users and the project team to reach concurrence, user at time might lack the necessary skills and knowledge to effectively participate in the design process. This study will investigate the user involvements and satisfaction levels of health information systems during the design and development phase. Methods: Cross sectional survey was utilized. Several existing tools were modified and used for the study namely Patient Satisfaction Questionnaire, Usefulness, Satisfaction, and Ease of use (USE) Questionnaire and Purdue Usability Testing Questionnaire. Structured equation models (SEM) analysis was used in the analysis. Ordinal regression statistics was used. Results: User involvement and satisfaction measure as the dependent variable and the user participation during the design and development of the health information systems as the independent variables. For every one unit increase in independent variable there is a predicted increase (of a certain amount) in the log odds falling at a higher level of the dependent variable. Generally showing that as scores increase on the independent variables, there is an increase probability falling at a higher level on the dependent variable. Conclusions: User involvement and participation has positive impact on the satisfaction levels of users during the design and development of the health information system.

Hospital Information System at Lahore Pakistan

Effective healthcare interoperability is associated with the capability of the service to offer a platform for data distribution between healthcare Professionals, process, procedures, and policies. This includes the way the health data of patient are stored and allows healthcare professionals to effectively access and interpret patient’s conditions. The Survey be targeted healthcare professionals in selected hospitals of Lahore Pakistan to investigate factors affecting health care professional’s adoption of health information systems. The study factors are Behavioural Intention, Usability, Perceived usefulness, Cost Effectiveness, Facilitating Conditions, and Performance Expectancy. Some demographic information also included in it.

A Model for Examining Challenges and Opportunities in Use of Cloud Computing for Health Information Systems

Health Information Systems (HIS) are becoming crucial for health providers, not only for keeping Electronic Health Records (EHR) but also because of the features they provide that can be lifesaving, thanks to the advances in Information Technology (IT). These advancements have led to increasing demands for additional features to these systems to improve their intelligence, reliability, and availability. All these features may be provisioned through the use of cloud computing in HIS. This study arrives at three dimensions pertinent to adoption of cloud computing in HIS through extensive interviews with experts, professional expertise and knowledge of one of the authors working in this area, and review of academic and practitioner literature. These dimensions are financial performance and cost; IT operational excellence and DevOps; and security, governance, and compliance. Challenges and drivers in each of these dimensions are detailed and operationalized to arrive at a model for HIS adoption. This proposed model detailed in this study can be employed by executive management of health organizations, especially senior clinical management positions like Chief Technology Officers (CTOs), Chief Information Officers (CIOs), and IT managers to make an informed decision on adoption of cloud computing for HIS. Use of cloud computing to support operational and financial excellence of healthcare organizations has already made some headway in the industry, and its use in HIS would be a natural next step. However, due to the mission′s critical nature and sensitivity of information stored in HIS, the move may need to be evaluated in a holistic fashion that can be aided by the proposed dimensions and the model. The study also identifies some issues and directions for future research for cloud computing adoption in the context of HIS.

Implementation status of health information systems in hospitals in the eastern province of Saudi Arabia

The implementation and adoption of the Health Information System (HIS) in hospitals has increased worldwide in recent years. HIS have various benefits, as they can streamline operations, increase operational efficiency, and reduce operational costs, especially in relation to management of electronic health records. The objective of this study was to investigate the implementation status of health information systems (HISs) in hospitals in the Eastern province of Saudi Arabia considering the following categories in the structuring of electronic health records: electronic clinical documentation, results viewing, decision support, and bar coding. To collect the information required in this study a questionnaire was developed and sent by email to HIS departments of 18 hospitals located in urban areas in the Eastern region in Saudi Arabia. The outcome variable in the study was the overall implementation score of each hospital. 67% of HIS were purchased, 28% were developed in-house, and 2% were provided by other sources. Most of the functionalities of the electronic clinical documentation and viewing categories were fully implemented in the majority of the hospitals. About a half of the functions in the decision support category were either fully implemented or not implemented. Finally, most of the bar coding functions were fully implemented in the majority of the hospitals, except for the supply chain function, which was not available in 44% of the hospitals. This is the first comprehensive description of the HIS status in hospitals in the Eastern region of Saudi Arabia. The implementation of HIS is promising, especially for basic functions related to patient information such as electronic clinical documentation and viewing results. The outcomes of this research indicated that the hospitals are implementing the health information systems in stages. The information obtained in this research may be useful for the health institutions to develop mechanisms that contribute to the implementation and adoption of health information systems throughout the country.

Theory-Based Model and Prediction Analysis of Information Security Compliance Behavior in the Saudi Healthcare Sector

The adoption of health information systems provides many potential healthcare benefits. The government of the Kingdom of Saudi Arabia has subsidized this field. However, like those of other less developed countries, organizations in the Kingdom of Saudi Arabia struggle to secure their health information systems. This issue may stem from a lack of awareness regarding information security. To date, most related studies have not considered all of the factors affecting information security compliance behavior (ISCB), which include psychological traits, cultural and religious beliefs, and legal concerns. This paper aims to investigate the usefulness of a theory-based model and determine the predictors of ISCB among healthcare workers at government hospitals in the Kingdom of Saudi Arabia. The study investigated 433 health workers in Arar, the capital of the Northern Borders Province in the Kingdom of Saudi Arabia. Two phases involved in this study were the hypothetical model formulation and identification of ISCB predictors. The results suggest that moderating and non-common factors (e.g., religion and morality) impact ISCB, while demographic characteristics (e.g., age, marital status, and work experience) do not. All published instruments and theories were embedded to determine the most acceptable theories for Saudi culture. The theory-based model of ISCB establishes the main domains of theory for this study, which were religion/morality, self-efficacy, legal/punishment, personality traits, cost of compliance/noncompliance, subjective norms, information security policy, general information security, and technology awareness. Predictors of ISCB indicate that general information security, followed by self-efficacy and religion/morality, is the most influential factor on ISCB among healthcare workers in the Kingdom of Saudi Arabia. This study is considered as the first to present the symmetry between theory and actual descriptive results, which were not investigated before.

Adoption of health information systems: Health professionals perspective

ABSTRACT The healthcare industry has embraced Information and Communication Technology in the performance of its duties to provide quality healthcare services. Though this system provides numerous benefits, research has shown that adoption and use of such systems are limited in the developing world. This study examines factors that will motivate healthcare personnel to adopt and use Health Information Systems using the Unified Theory of Acceptance and Use of Technology (UTAUT2) model. One hundred and ten (110) respondents were selected for the study. The data collected was analysed using Partial Least Square (PLS) with a standard error of 0.10. The findings indicate that Performance Expectancy, Habit and Hedonic Motivation are the main factors users considered to influence their behavioural intention to adopt and use health information system. The results show that when users perceive Health Information systems to improve their performance, they are more likely to adopt and use the system. Stakeholders should consider system quality issues, such as response time and interface design, when developing a new system as these serve as motivational factors towards adoption and use of health information system. This study is among the first few attempts to understand adoption and use of health information systems in Sub-Saharan Africa.

Unified Theory of Acceptance and Usage of Technology (UTAUT) Model to Predict Health Information System Adoption

In order to improve the quality and performance of health services, health information systems are one of the most important technologies in health management. This study aims to determine the factors that influence the adoption of nutrition information system by nutrition officers of primary health care by using Unified Theory of Acceptance and Usage of Technology (UTAUT) Model. This research is quantitative research with survey approach, located in 39 primary health care of Banyumas district. Sampling technique using purposive sampling. Respondents are 50 people. The research instrument used questionnaire. Based on regression test results known that factors related to use behaviour is behavioral intention. While the facilitating condition is not related to use behavior. Factors associated with behavioral intention is the performance expectancy, effort expectancy and social influence.

Current Status of the Adoption, Utilization and Helpfulness of Health Information Systems in Korea.

As interest in medical informatization has been increasing steadily worldwide, the adoption of health information systems (HISs) in medical institutions is essential. In this study, we intended to identify users’ adoption, utilization and helpfulness derived from HISs to determine the status of medical informatization by using 2016 Korean Physician Survey data. A total of 8564 members responded to the 2016 survey, we extracted the data of 6949 HISs related part for this study. As a result, about 68% of the self-employed physicians adopted the Order Communication System and Electronic Medical Records, while only one-third adopted the Picture Archiving and Communication System. Overall, the proportions of utilization and helpfulness of HISs were higher for females; for employed physicians or professors; for physicians working in hospitals; and for those specializing in surgical medicine. As applying information technology to the medical environment becomes more important, HIS will become a necessary requirement and the demand for information systems considering user and patient oriented information environments will be increasing. Therefore, it is necessary to discuss the HISs use environment considering not only technical aspects but also users’ or patients’ viewpoints. In that respect, this study will give a meaningful evidence of HIS related policies.

Exploring the Change of Attitude among Healthcare Professionals toward Adopting a National Health Information System: The Case of Jordan

National health information systems (HISs) are spreading globally at a rapid pace. Therefore, it is important to understand the socio-technical factors that influence their adoption by healthcare professionals (HCPs). This study explores the change in attitudes of HCPs toward adopting a national HIS in Jordan. Data were collected from public hospitals and healthcare centres using surveys in two phases over two years and empirically tested using partial least squares (PLS). The first phase results showed that HCPs had positive attitudes toward the national HIS and perceived it to be useful but difficult to use. In the second phase, results showed that users' attitudes positively changed regarding both ease of use and usefulness. This research contributes to both theory and practice by investigating how HCPs' attitudes can be changed regarding adopting a national HIS over time. This provides insights for healthcare providers to better understand and improve HISs adoption among HCPs.

Pre-Implementation evaluation of a nationwide information system for university hospitals: lessons learned from a study in Brazil

ABSTRACTInitiatives to implement nationwide health information systems (HIS) have been developed in different countries. Despite the potential of these initiatives of improving the quality of healthcare, the evidence of failures are numerous. We analysed the perceptions of a nationwide hospital information system by a group of 36 managers and healthcare professionals of a university hospital, prior to its adoption (pre-implementation evaluation), with the purpose of identifying issues which were likely to affect its implementation and use. This research takes a qualitative approach. Interviews, questionnaires and documentary research were used to collect data. Analysis was based on coding and categorisation. The results point out to seven areas of concern of the participants of the research those emerged from the analysis: hardware and software; government management, politics and trade unions; workplace practices; education, training and support; implementation management; staff skills; and communication. We identified that these issues are similar to problems resulting from HIS evaluations performed after their adoption. This discovery reveals that problems that usually arise from the implementation and use of nationwide HIS can be anticipated and dealt with beforehand when pre-implementation qualitative evaluations are performed. These evaluations contribute to improve intra-organisational and inter-stakeholders’ communication in comprehensive nationwide HIS adoption programmes.

Adequacy of UTAUT in clinician adoption of health information systems in developing countries: The case of Cameroon

PurposeDespite the great potential Health Information Systems (HIS) have in improving the quality of healthcare delivery services, very few studies have been carried out on the adoption of such systems in developing countries. This article is concerned with investigating the adequacy of UTAUT11Unified Theory of Acceptance and Use of Technology. in determining factors that influence the adoption of HIS by clinicians in developing countries, based on the case of Cameroon. MethodsA paper-based questionnaire was distributed to clinicians in 4 out of 7 major public hospitals in Cameroon. A modified UTAUT was tested using structural equation modeling (SEM) method to identify the determinants of clinicians’ intention to use HIS. Self-efficacy and cost-effectiveness were determinants used to extend the original UTAUT. Results228 out of 286 questionnaires were validated for this study. The original UTAUT performed poorly, explaining 12% of the variance in clinicians’ intention to use HIS. Age was the only significant moderating factor, improving the model to 46%. Self-efficacy and cost effectiveness has no direct significant effect on HIS adoption in the context of this study. ConclusionsThe original UTAUT is not adequate in identifying factors that influence the adoption of HIS by clinicians in developing countries. Simplifying the model by using age as the only moderating factor significantly increases the model’s ability to predict HIS adoption in this context. Thus, the younger clinicians are more likely and ready to adopt HIS than the older ones. Context-specific should also be used to increase the explanatory power of UTAUT in any given context.
